Helicobacter pylori, a helix-shaped gram-negative microaerophilic bacteria has been classified as a human carcinogen (Group I carcinogen) by the International Agency for Research on Cancer (IARC) on the basis of numerous animal and epidemiological studies. Chronic Helicobacter pylori infection can cause lead to high levels of intracellular reactive oxygen species (ROS) and genomic instability both directly by genetic and/or epigenetic pathways. It is known that high levels of intracellular ROS can trigger apoptosis and induce DSBs. In this study, we aimed to investigate intracellular ROS levels and apoptotic effects of Helicobacter pylori infection on AGS cells in the presence of a DSB repair inhibitor [8-(4-dibenzothienyl)-2-(4-morpholinyl)-4H-1- benzopyran-4-one, Nu7441]. After incubation of AGS cells with different multiplicities of infection (MOIs; 25, 50, 75, 100, 200 and 400), we observed that Helicobacter pylori causes MOI-dependent cytotoxicity and intracellular ROS levels significantly increased in all study groups vs. control. Both caspase 3 and caspase 8 levels were higher all of the study group when compared to control group. In HP+Nu7441 group, caspase 8 levels 3.5 times higher vs. control; indicating unrepaired DSBs in Helicobacter pylori infection can highly be induce apoptosis in AGS cells. Other cell death mechanisms, including autophagy, should be studies in order to fully understand the cytotoxicity mechanisms of the bacterium. Moreover, we can state that in organisms with unrepaired DSBs or with high DSB levels, Helicobacter pylori can cause more oppressive adverse effects, the importance of which should be elucidated with mechanistic studies.

Benign prostat hiperplazisi (BPH), orta ve ileri yaşlardaki erkeklerde, sıklıkla görülen önemli sağlık sorunlarından birisidir. Prostat bezinin büyümesi sonucu üretra çıkısında oluşan darlık, alt üriner sistem semptomları (LUTS) ve buna bağlı olarak gelişen erektil disfonksiyona (ED) neden olmaktadır. Bu ilişkinin en kuvvetli teorilerinden biri artan obstrüksiyon kaynaklı oksidatif stres ve inflamasyondur. Bu çalışmadaki amaç, sıçanlarda oluşturulan parsiyel mesane obstrüksiyonunun (PBOO), erektil fonksiyonu nasıl etkilediği ve bu değişiklikler üzerinde bir hidrojen sülfür donörü sodyum hidrojen sülfit ve fosfodiesteraz 5 inhibitörü (PDE-5i) olan tadalafilin tedavi edici etkisinin olup olmadığını araştırmaktır. Yetişkin 300-350g ağırlığındaki SpragueDawley erkek sıçanlar (n=75) beş eşit gruba ayrılmıştır kontrol (sham-operasyon yapılanlar); 6 hafta PBOO yapılan sıçanlar; 6 hafta PBOO ve NaHS (5.6mg/kg) tedavisi yapılan sıçanlar; 6 hafta PBOO ve tadalafil (2mg/kg) tedavisi yapılan sıçanlar; 6 hafta PBOO ve NaHS ve tadalafil tedavisi yapılan sıçanlar. İn vivo erektil yanıtlar, intrakavernozal basınç (ICP)/ortalama kan basıncı (MAP) oranları ve total ICP değerleri ölçülerek hesaplanmıştır. İn vitro deneylerde, kavernosal doku izole edilerek, gevşeme ve kasılma deneyleri ilgili ajanlarla organ banyosunda çalışmalar yapılmıştır. Tüm gruplardan alınan penis dokularında nitrik oksit sentaz (NOS) ve hidrojen sülfür sentaz türevlerinin, hipoksi, oksidatif stres ve fibrozis belirteçlerinin proteinlerinin ekspresyon düzeyleri immünohistokimyasal ve Western blot analizi ile ve bunlara ek olarak, TUNEL metodu ile de apoptozis ve Masson trikrom boyama ile düz kas kollajen oranı tayin edilmiştir. Ayrıca penil dokularda siklik guanozin monofosfat (cGMP), H2S ve malondialdehid düzeyleri ölçülmüştür. 6 hafta sonunda, PBOO'lu grupta artmış mesane ağırlığı, azalan in vivo erektil yanıt ve yine azalan endotel ve nitrerjik gevşeme yanıtları bulunmuştu. Tekli tedavi gruplarında bu değişimler kısmen düzelirken, kombine tedavide tamemen düzelmiştir. PBOO'ya bağlı azalan kasılma yanıtları tüm tedavili gruplarda düzelmiştir. Obstrüksiyon sonucunda penil dokuda azalan nöronal NOS ve 3-merkaptopiruvat transferaz enzim ekspresyonları da kombine tedavi ile tamamen düzelmiştir. Tekli tedavi grupları artan hipoksi, oksidatif stres ve fibrozis belirteçlerini, azalan düz kas oranını, cGMP ve H2S seviyelerini kısmen iyileştirirken, kombine tedavi bu değişmleri tamamen düzeltmiştir.Çalışmamız sonuçları ilk olarak, obstrüksiyonun neden olduğu olduğu ED, peniste iskemiye bağlı gelişen fonksiyonel ve morfolojik değişiklikler üzerine H2S donörü ve PDE-5i kombinasyonunun başaralı olduğunu göstermiştir. Erektil fonksiyonun moleküler düzenlenmesinde ve penil dokuda ortaya çıkan etkilerin H2S ve NO'nun sinerjik etkisine bağlı olabilir ve ED ve BPH/LUTS hastalarında klinik yarar sağlayabilir. Benign prostatic hyperplasia (BPH) is one of the major health problems typically seen in middle-aged and elderly men and causes partial bladder outlet obstruction (PBOO). Clinically, BPH with lower urinary tract symptoms (LUTS) linked to benign prostatic obstruction (BPO) causes erectile dysfunction (ED). Previous studies have shown that the severity of LUTS is related to ED and there are many similar common mechanisms such as increased oxidative stress and inflammation which lead to ED. The aim of this study was to investigate if there is any beneficial effect on ED after treatment, phosphodiesterase type 5 inhibitor (PDE-5i) tadalafil and sodium hydrosulfide (NaHS), a hydrogen sulfide donor, and combinations can be used as a pharmacological agent for the treatment of ED in patients with BPH/LUTS.Adult Sprague-Dawley rats (n = 75) were equally divided into five groups: 1) sham-operated control; 2) PBOO (6-week); 3) NaHS (6-week, 5.6 mg/kg/day, i.p.)-PBOO; 4) tadalafil (6-week, 2 mg/kg/day, oral)-PBOO; and 5) combination treatment with NaHS and tadalafil in PBOO. PBOO was induced by ligation of the urethra. Erectile response was evaluated by cavernosal nerve stimulation. The relaxant and contractile responses of CC were obtained from organ bath studies. Western blotting and immunohistochemistry were used to determine protein expression and localization of nitric oxide synthase (NOS) and hydrogen sulfide synthase enzymes, oxidative stress, hypoxia and fibrosis markers. The relative area of smooth muscle to collagen using Masson trichrome staining was evaluated. Apoptosis was analyzed using TUNEL assay. In addition, cyclic guanosine monophosphate (cGMP), H2S and malondialdehyde levels were determined in penile tissues. At the end of 6 weeks, increased bladder weight, decreased in vivo erectile function and reduced endothelial and nitrergic relaxation responses in the PBOO group, which were partially improved with monotherapy while combined therapy recovered completely. Decreased contraction responses induced by PBOO were ameliorated by all treatments. Decreased neuronal NOS and 3-mercaptopiruvate transferase enzyme expressions in penile tissue as a result of obstruction also completely increased with combined treatment. Monotherapy partially improved the levels of increased hypoxia, oxidative stress, and fibrosis markers, decreased smooth muscle mass, cGMP and H2S levels, while combined therapy completely enhanced.We firstly pointed out that beneficial combination with H2S donor and PDE-5i treatment contributed to the restoration of erectile function, the progression of ischemia-associated functional as well as morphological penile alterations in obstruction. H2S and NO most likely might have a synergistic effect on the structure of penile tissue and molecular modulation of erectile function, and be effective for ameliorating clinical outcomes in men with ED and BPH/LUTS. 105

ÖZET Parotis bezi tükürük salgılayan üç büyük bezden biridir. Günlük tükürük miktarının büyük bölümünü karşılayan bu bezde tükürüğün belli ortamlarda farklı salgılandığı gözlenmiştir. Konuşan insanda konuşmayana göre gündüz gece karalığına göre tükürük salınımı artış göstermektedir. Farklı ortamlardan değişik şekilde etkilenen bu bez üzerinde sürekli ışık ve karanlık ortamın etkilerini araştırmak amacıyla planlanan bu çalışmada deneklerin bir grubu 50 gün süreyle sürekli ışığa, bir grubu aynı sürede sürekli karanlığa bırakıldı. Kontrol grubu ise aynı sürede günlük aydınlık karanlık periyodunda tutuldu. Bu deneklerden alman parotis bezi doku örnekleri elektron mikroskobik takibe alındı. Elde edilen bloklardan alınan yan ince kesitler toluidin mavisi ile boyanarak ışık mikroskobunda değerlendirilerek resimlendirildi. Işık ve karanlık uygulanan her iki gruptada salgılamanın tam olarak durdurulamadığını ve salgılama döngüsünün sürdüğü yargısına varıldı. 14 SUMMARY Parotis is one of the biggest three glands which secrete saliva. In this gland, secreting the highest amount of daily saliva, it is observed that in certain conditions the saliva is secreted in different amounts. There is an increase in secretion of saliva in talking people more than people who are not talking and in days more than in nights. The effects of continuous bright and darkness environments on this gland, which is effected in different ways from different conditions, are planned to search in this research. One of the groups of subjects is exposed to continuous light for 50 days, the other group is exposed to continuous darkness and the control group is exposed to daily darkness and light in the same period. The tissue examples which are taken from these subjects' Parotis glands persecuted with electron-microscope. Half-slim crosscuts which are taken from obtained blocks are fist, painted with toluidin blue next, evaluated in light-microscope and finally photographed. Secretion of saliva couldn't stopped exactly and it is decided that the rotation of saliva secretion continues in both light applied and darkness applied groups. 51

Endojen ve ekzojen birçok etkene bağlı olarak DNA’da farklı hasarlar oluşabilir. DNA hasarlarının en tehlikelisi ise DNA çift sarmal kırıkları double strand breaks, DSB ’dır. DSB onarımı normalde iki farklı yolla gerçekleştirilir. Ancak, bu onarım yolaklarında meydana gelen herhangi bir bozukluk kanser, yaşlanma ve birçok genetik hastalığa yol açabilir. Fiziksel, kimyasal ve biyolojik ajanların indüklediği reaktif oksijen türleri ROS DSB’na neden olan başlıca etkenlerdir. İyonize radyasyon da dolaylı yolla canlı dokuda yoğun olarak bulunan su molekülüyle etkileşerek oksidatif strese, ROS ve ardından DSB oluşumuna neden olmaktadır. Çevresel veya tedavi amaçlı maruz kalınan pek çok kimyasal ajan ya da ilacın antikanser ajanlar, ağır metaller, asbest, ftalatlar, benzen, okratoksin A gibi da DSB’ye neden olduğu yapılan pekçok çalışma ile gösterilmiştir. Diğer taraftan, insanda gastrit ve ülsere neden olan bir bakteri olan Helicobacter pylori’nin de DSB oluşumu üzerine etkileriyle ilgili çalışmalar devam etmektedir. Ayrıca pek çok bakteri toksinin de DSB’ye neden olabileceği bildirilmiştir. Bu derleme kapsamında DSB’e neden olan fiziksel, kimyasal ve biyolojik ajanlar ve etki mekanizmaları tartışılacaktır., Endogenous and exogenous factors can cause different types of DNA damages. The most dangerous DNA damage is double strand break DSB . Under normal conditions, DSB repair is substantiated by two different mechanisms. However, any disruption in these repair pathways can cause cancer, aging and different types of genetic disorders. Reactive oxygen species ROS induced by physical, chemical and biological agents are the main causes that lead to the formation of DSBs. In living tissues, ionized radiation can interact with water molecules and can cause oxidative stress and formation of ROS and DSB, respectively. Several chemical agents or drugs that humans are exposed to, by environment or therapy anticancer drugs, heavy metals, asbestos, phthalates, benzene, ochratoxin A , were shown to cause DSB by several studies. Research on DSB causing effects of Helicobacter pylori, a bacterium that causes gastritis and ulcer in humans, is still in progress. In addition, several bacterial toxins were stated to cause DSB. This review will mainly focus on agents that cause DSB and their mechanisms of action.
