Introduction: Augmented reality can improve planning and execution of surgical procedures. Head-mounted devices such as the HoloLens® (Microsoft, Redmond, WA, USA) are particularly suitable to achieve these aims because they are controlled by hand gestures and enable contactless handling in a sterile environment. Objectives: So far, these systems have not yet found their way into the operating room for surgery of the parotid gland. This study explored the feasibility and accuracy of augmented reality-assisted parotid surgery. Methods: 2D MRI holographic images were created, and 3D holograms were reconstructed from MRI DICOM files and made visible via the HoloLens. 2D MRI slices were scrolled through, 3D images were rotated, and 3D structures were shown and hidden only using hand gestures. The 3D model and the patient were aligned manually. Results: The use of augmented reality with the HoloLens in parotic surgery was feasible. Gestures were recognized correctly. Mean accuracy of superimposition of the holographic model and patient’s anatomy was 1.3 cm. Highly significant differences were seen in position error of registration between central and peripheral structures (p = 0.0059), with a least deviation of 10.9 mm (centrally) and highest deviation for the peripheral parts (19.6-mm deviation). Conclusion: This pilot study offers a first proof of concept of the clinical feasibility of the HoloLens for parotid tumor surgery. Workflow is not affected, but additional information is provided. The surgical performance could become safer through the navigation-like application of reality-fused 3D holograms, and it improves ergonomics without compromising sterility. Superimposition of the 3D holograms with the surgical field was possible, but further invention is necessary to improve the accuracy.

ZusammenfassungIn den letzten Jahren verdichten sich Hinweise, dass eine Tumorstammzellpopulation in Plattenepithelkarzinomen der Kopf-Hals-Region (HNSCC) existiert. Man vermutet, dass diese Tumorstammzellen (CSC) über Selbsterneuerungskapazität verfügen und für Wachstum, Metastasierung, Rekurrenz und Therapieresistenz des Tumors essenziell sind. Stammzellen werden von einer spezialisierten zellulären und nichtzellulären Mikroumgebung unterstützt, die „Stammzellnische“ genannt wird. Eine mögliche Strategie, um CSC zu attackieren, könnte eine Beeinflussung ihrer Nische sein.Stromal cell-derived factor-1 (SDF-1) gilt als Schlüsselregulator des Zellverkehrs zwischen Nische und der peripheren Blutzirkulation. SDF-1 ist ein multifunktionelles Zytokin, das u. a. von Endothel-/Stromazellen sezerniert wird. Als sein Rezeptor wurde der 7-Transmembranrezeptor CXCR4 identifiziert. Die SDF-1-CXCR4-Achse ist ein wesentlicher Faktor für Migration und Mobilisation von CSC in und aus ihrer Nische. Ihre Existenz und Funktionalität in der Tumorstammzellnische von HNSCC konnte bereits nachgewiesen werden. Strategien, die in diese Interaktion eingreifen, könnten einen entscheidenden Beitrag in der Therapie von HNSCC bieten.Um Mechanismen in der Tumorstammzellnische von HNSCC in vitro zu untersuchen, werden Modelle benötigt. Wir erwarten, dass die 3D-Zellkultur und das 3D-Bioprinting bei der Entwicklung neuer, individualisierter Therapiestrategien bahnbrechenden Charakter aufweisen werden.Hier geben wir eine Übersicht über den Wissensstand hinsichtlich der Interaktionen in der Tumorstammzellnische von HNSCC am Beispiel der SDF-1-CXCR4-Achse.

Fur die Rekonstruktion von Defekten im Gesichtsbereich bieten sich hinsichtlich Asthetik und Funktion lokale und regionale Lappenplastiken am besten an. Sie erfordern eine sorgfaltige praoperative Planung. Kenntnisse in der Vielzahl der operativen Moglichkeiten, ebenso deren praktische Umsetzung sind von wesentlicher Bedeutung. Die Kriterien fur die Wahl des Operationsverfahrens setzen sich u. a. aus den Wunschen des jeweiligen Patienten sowie den Gegebenheiten des Operationssitus zusammen. In der vorliegenden Ubersichtsarbeit werden haufig angewandte und empfehlenswerte Methoden zur Rekonstruktion von Defekten der Nase dargestellt.

Background Head and neck squamous cell cancer (HNSCC) affects the oral cavity and the pharynx. The aim of the study was to investigate the effects of selective tyrosine kinase inhibitors (TKIs) erlotinib, gefitinib, nilotinib and dasatinib and the mammalian target of rapamycin (mTOR) inhibitor everolimus on the expression of apoptosis-related proteins caspase-3, FAS cluster of differentiation (CD)-95 and FAS ligand in human papilloma virus (HPV)-dependent squamous cancer. Materials and methods Two HPV-negative cell lines (UMSCC-11A/-14C) and one HPV-positive cell line (CERV196) were incubated with TKIs or everolimus and protein concentrations of target proteins were analyzed with enzyme-linked immunosorbent assay (ELISA). Results Caspase-3 was affected by the tested TKIs in HPV-positive SCC, whereas FAS CD95 and FAS ligand were influenced in HPV-negative SCC. Discussion This is the first study to analyze the influence of TKIs and everolimus on key proteins of apoptosis. Our results provide novel information contributing to a better understanding of the cell biology of HPV-dependent HNSCC and might contribute to the discovery of novel pharmaceutical treatment strategies for HNSCC.

Since December 2019, the SARS-CoV-2 virus has been rapidly spreading worldwide. In Germany, an exponential increase in the number of infections was registered at the beginning of March 2020 and led to a call of the Ministry of Health to create more capacity for intensive medical treatment in hospitals. The aim of the present study was to determine the effects of the SARS-CoV-2 pandemic on Oto-Rhino-Laryngology (ORL) university hospitals regarding patient care.An online survey was sent out to all chairmen of the 39 ORL university hospitals in Germany. The answers to the questions referred to the defined period from March 15All chairmen of the 39 university hospitals in Germany participated in the survey. The collected data prove the considerable impact on organizational, structural and medical aspects of patient care. For example, the surveyed clinics reported a decrease in outpatient cases by 73.8 % to 26.2 ± 14.2 % and in surgical treatments by 65.9 % to 34.1 ± 13.9 %. In contrast, emergency treatment remained unchanged or even increased in 80 % of the facilities and surgical treatment of emergency patients remained unchanged or even increased in more than 90 %. Emergency outpatient and surgical treatment of patients was provided throughout the pandemic in all facilities. In total, about 35 000 outpatients and about 12 000 surgical cases were postponed. As a result of the acute structural changes, the potential danger of falling below current treatment standards was seen in individual areas of patient care.The assessment of the impact of the SARS-CoV-2 pandemic is heterogeneous. The majority of the chairmen are critically aware of the risk of falling below current medical treatment standards or guidelines. In the phase of an exponential increase in the number of infections, significant changes in treatment processes had to be accepted for understandable reasons. However, with the currently significantly reduced number of infections, falling below treatment standards and guidelines should not be allowed to remain constant and tolerated.This study shows a differentiated picture with regard to the effects of the SARS-CoV-2 pandemic on outpatient, inpatient and operative patient care at the ORL university hospitals in Germany and illustrates the importance of these institutions for ensuring patient care during this critical phase.Seit März 2020 wurden infolge des exponentiellen Anstiegs der Infektionen mit SARS-CoV-2 in Deutschland/Europa die Krankenhäuser durch das Gesundheitsministerium aufgefordert, verstärkt Kapazitäten zur intensivmedizinischen Behandlung zu schaffen. An vielen Kliniken wurden deshalb kurzfristig massive Umstrukturierungen und Ressourcenverschiebungen vorgenommen. Ziel der vorliegenden Arbeit war die Erfassung der Auswirkungen der SARS-CoV-2-Pandemie auf die universitäre Hals-Nasen-Ohren(HNO)-Heilkunde in Bezug auf die Krankenversorgung.Eine Online-Befragung wurde an alle Direktorinnen und Direktoren der 39 Universitäts-HNO-Kliniken in Deutschland versendet. Die Fragen bezogen sich auf den definierten Zeitraum vom 15.03. bis 15.04.2020 und die Beantwortung wurde mithilfe des Online-Befragungstools „SurveyMonkey“ durchgeführt. 87 Fragen bezogen sich auf allgemeine Angaben, die Krankenversorgung und die strukturellen Auswirkungen in der jeweiligen Einrichtung.An der Befragung beteiligten sich alle Direktorinnen und Direktoren der 39 Universitäts-HNO-Kliniken in Deutschland. Die erhobenen Daten belegen die erheblichen Auswirkungen auf organisatorische, strukturelle und medizinische Aspekte der Krankenversorgung. So gaben die befragten Kliniken einen Rückgang der ambulant behandelten Fälle um 73,8 % auf 26,2 ± 14,2 % und der operativen Behandlungen um 65,9 % auf 34,1 ± 13,9 % an. Im Gegensatz hierzu erfolgte in 80 % der Kliniken eine unveränderte oder sogar vermehrte Notfallbehandlung und in mehr als 90 % eine unveränderte oder sogar vermehrte operative Behandlung von Notfällen. In allen Kliniken war die ambulante und operative Notfallbehandlung während der Pandemie durchgehend gewährleistet. Insgesamt wurden an allen HNO-Universitätskliniken circa 35 000 ambulante und circa 12 000 operative Fälle verschoben. Infolge der akuten strukturellen Veränderungen wurde in einzelnen Bereichen der Krankenversorgung die potenzielle Gefahr einer Unterschreitung geltender Behandlungsstandards gesehen.Die Bewertung der Auswirkungen der SARS-CoV-2-Pandemie fällt heterogen aus. Kritisch wird mehrheitlich die Gefahr der Unterschreitung geltender Behandlungsstandards oder Leitlinien bewertet. In der Phase der exponentiellen Zunahme der Infektionszahlen mussten teilweise erhebliche Veränderungen von Behandlungsabläufen aus nachvollziehbaren Gründen akzeptiert werden. Mit den aktuell deutlich reduzierten Infektionszahlen darf aber eine Unterschreitung von Behandlungsstandards und Leitlinien nicht verstetigt und toleriert werden.Die hier dargestellte Untersuchung zeigt ein differenziertes Bild hinsichtlich der Auswirkungen der SARS-CoV-2-Pandemie auf die ambulante, stationäre und operative Krankenversorgung an den Kliniken der universitären HNO-Heilkunde in Deutschland und verdeutlicht die Bedeutung der Universitätskliniken für die Krankenversorgung während dieser kritischen Phase.

Die nekrotisierende Fasziitis (NF) ist eine lebensbedrohliche, meist bakterielle Weichteilinfektion, die nur selten die Kopf-Hals-Region betrifft. Charakteristisch sind der rasche Progress und die systemische Mitbeteiligung, die die Diagnosestellung erschweren und eine sofortige chirurgische und antibiotische Therapie erfordern. Es werden 3 Falle von zervikofazialer NF vorgestellt. Alle Patienten stellten sich aufgrund einer rasch progredienten, druckschmerzhaften zervikofazialen Schwellung und Rotung vor. Nach Diagnosestellung der NF mittels Klinik, Labor und Computertomographie (CT) erfolgte die Einleitung einer systemischen intravenosen Antibiose sowie ausgedehnte, mehrfache Nekrosektomien. Aufgrund der starken systemischen Mitbeteiligung verstarben 2 der 3 Patienten innerhalb von 48 h an einem septischen Multiorganversagen, ein Patient konnte nach intensivmedizinischer Behandlung und plastischer Deckung der Wunddefekte nach Nekrosektomie in die hausliche Umgebung entlassen werden. Die NF ist eine rapide verlaufende Weichteilinfektion mit hoher Mortalitat und Morbiditat, die oft spat erkannt wird. Die Diagnose erfolgt klinisch anhand der progredienten, diffusen Schwellung, Schmerzen, Erythem und einer fruhen systemischen Mitbeteiligung. Hilfreich sind auserdem Laborparameter (Laboratory Risk Indicator for Necrotizing Fasciitis Score [LRINEC-Score]) und Bildgebung mittels CT oder Magnetresonanztomographie (MRT), in der sich eine Weichteilschwellung mit pathognomonischer Gasbildung zeigen kann. Eine unmittelbare chirurgische Nekrosektomie und der Beginn einer systemischen Antibiose sind entscheidend fur das Uberleben des Patienten. Die Gabe von intravenosen Immunglobulinen und die hyperbare Sauerstofftherapie stellen ausschlieslich additive Therapiemoglichkeiten dar.

Background/aim The effects of tyrosine kinase inhibitors (TKI) in head and neck squamous cell cancer (HNSCC) are not fully understood. We investigated the effects of selective TKIs erlotinib, gefitinib, nilotinib, and dasatinib and the mTOR-inhibitor everolimus on the expression of insulin-like growth factor 1 receptor (IGF1R) in HPV-positive and HPV-negative squamous cancer cell lines. Materials and methods HPV-negative UMSCC-11A and UMSCC-14C cells and HPV-positive CERV196 cells were treated with TKIs or everolimus. Protein concentration of IGF1R was measured using ELISA. Results IGF1R expression was significantly reduced by all tested TKIs and everolimus in both HPV-negative cancer cell lines. In HPV-positive squamous cancer cells we observed significant protein inhibition. Conclusion The crosstalk between epidermal growth factor receptors and IGF1R could be of central interest for the development of novel medical approaches for individualized therapy.

Exosomen, virusgrose Nanovesikel, werden als Botensysteme unseres Korpers zur Kommunikation mit anderen Korperzellen und Regulation von Immunfunktionen genutzt. Die von fast allen Korperzellen sezernierten Exosomen konnen unterwegs in der Blutzirkulation und in der Korperperipherie mit Immunzellen interagieren. Unterschiedliche Marker auf der Exosomenoberflache sind fur die Immunzelladhasion und -interaktion notig. Daruber hinaus sind mehrere Arten der Exosomen-Immunzell-Interaktion bereits bekannt, wie beispielsweise die direkte Oberflachenrezeptorinteraktion und Phagozytose. Als Transportmedium konnen Exosomen unterschiedliche Immunzelltypen in Kopf-Hals-Tumoren beeinflussen: Bisher sind T‑Zellen, naturliche Killerzellen, Makrophagen und dendritische Zellen in diesem Kontext beschrieben. Fur diagnostische Zwecke konnte eine kombinierte Analyse unterschiedlicher Parameter, bestehend aus Proteinmenge, Nukleinsaure‑/Proteinexpression und dem immunsupprimierenden Effekt der Exosomen, ein wertvolles Hilfsmittel fur die Beurteilung der Tumorentwicklung und -progression darstellen.

The rise of targeted therapies in the treatment of head and neck squamous cell carcinoma (HNSCC) has considerably widened the treatment range. Matrix metalloproteinases (MMPs) are key regulators of the tumor development of many cancer entities, which makes them a promising target for new treatment options. We examined the expression patterns of MMP2 and MMP14 in human papillomavirus (HPV)-positive and -negative SCC lines after treatment with small molecule tyrosine kinase inhibitors (TKIs) and a mechanistic target of rapamycin (mTOR) inhibitor in vitro.Cells of two human HPV-negative cell lines (UMSCC-11A/-14C) and one HPV-positive cell line (CERV196) were incubated with 20 μmol/l of erlotinib, gefitinib, nilotinib, dasatinib, or everolimus for 24-96 h. Cell proliferation was assessed by proliferation assay and the protein concentrations of MMP2 and MMP14 by sandwich enzyme-linked immunosorbent assay. For statistical analysis, the results were compared with those of untreated SCC cells.MMP2 and MMP14 were expressed in all three tested cell lines; expression levels were highest in the UMSCC-14C cell line. The tested TKIs significantly (p0.05) reduced MMP2 expression in the UMSCC-14C cell line. In the HPV-positive cell line, the drugs led to an increase in MMP2 and MMP14 expression.Dysregulations in MMP signaling are involved in tumorigenesis and metastasis of HNSCCs; MMP2 has been noted as a potential biomarker. The expression of MMP2 and MMP14 is influenced effectively by small molecule TKIs and everolimus. Based on our data, future research should concentrate on a better understanding of the interplay between tumor microenvironment and tumor cells in vitro and in vivo.

Epidermal growth factor receptor (EGFR) upregulation is a typical characteristic of head and neck squamous cell carcinoma (HNSCC). However, tyrosine kinase inhibitors have not yet been able to achieve enough therapeutic benefit in clinical trials to justify their use in standard therapy regimens. At present, little is known about the reasons for this treatment failure. In the present study, the HNSCC cell lines UM-SCC-11B and UM-SCC-22B were tested for their response to tyrosine kinase inhibitors (TKI) under 2D and 3D cell culture conditions. Absorption and luciferase-based viability assays were used for this, as well as optical evaluation via fluorescence microscopy. In addition, EGFR and HER3 expression as well as the downstream signalling pathways PI3K/AKT/mTOR and RAS/RAF/MEK/ERK were investigated using western blotting. Cell line UM-SCC-11B revealed a strong resistance to lapatinib under 3D cell culture conditions, while a good response to TKI therapy was observed under 2D cell culture conditions. An associated overexpression of phosphorylated HER3 under 3D cell culture conditions offered a plausible explanation for the altered treatment response. The results of the present study represent an idea of how signalling mechanisms of cancer cells can be changed using different cell culture methods. Overall, 3D cell culture could be an important component in the analysis of resistance mechanisms in cancer therapy.

Immunotherapy has evolved into a powerful tool in the fight against a number of types of cancer, including head and neck squamous cell carcinomas (HNSCC). Although checkpoint inhibition (CPI) has definitely enriched the treatment options for advanced stage HNSCC during the past decade, the percentage of patients responding to treatment is widely varying between 14‑32% in second‑line setting in recurrent or metastatic HNSCC with a sporadic durability. Clinical response and, consecutively, treatment success remain unpredictable in most of the cases. One potential factor is the expression of target molecules of the tumor allowing cancer cells to acquire therapy resistance mechanisms. Accordingly, analyzing and modeling the complexity of the tumor microenvironment (TME) is key to i) stratify subgroups of patients most likely to respond to CPI and ii) to define new combinatorial treatment regimens. Particularly in a heterogeneous disease such as HNSCC, thoroughly studying the interactions and crosstalking between tumor and TME cells is one of the biggest challenges. Sophisticated 3D models are therefore urgently needed to be able to validate such basic science hypotheses and to test novel immuno‑oncologic treatment regimens in consideration of the individual biology of each tumor. The present review will first summarize recent findings on immunotherapy, predictive biomarkers, the role of the TME and signaling cascades eliciting during CPI. Second, it will highlight the significance of current promising approaches to establish HNSCC 3D models for new immunotherapies. The results are encouraging and indicate that data obtained from patient‑specific tumors in a dish might be finally translated into personalized immuno‑oncology.
