Abstract

Importance: Return to work after stroke is an important rehabilitation outcome. Vocational rehabilitation may support people with stroke to return to work. Objective: To report the return-to-work rate of people with stroke who completed a local community-based vocational rehabilitation program and to describe the program’s impact on their community reintegration. Design: Retrospective database study. Setting: Community-based voluntary welfare organization. Participants: Fifty-eight men and women ages 21 to 55 yr, formally diagnosed with stroke, with no significant cognitive impairment, and fit to undergo rehabilitation. Intervention: A pilot community-based interdisciplinary vocational rehabilitation program implemented between 2014 and 2017. The program included physical rehabilitation, psychosocial support, employment support, and caregiver support services. Outcomes and Measures: Return-to-work rate and community reintegration as measured by the Community Integration Questionnaire (CIQ). Results: Fifty participants completed the program. Their median age was 44 yr, and most were male (n = 37; 74%). Forty-four participants (88%) returned to work after completing the program. Statistically significant differences (p < .001) were found between their CIQ scores at program enrollment and at completion. Conclusions and Relevance: The program appeared to benefit people with stroke in their return-to-work process. Future studies are recommended to determine the effectiveness of such programs and identify features contributing to their success. Potential implications for occupational therapy practice are that vocational rehabilitation interventions can be initiated earlier, comprehensive return-to-work programs can better address the unique needs of people with stroke, and interdisciplinary evaluations are needed to assess suitability for return to work. What This Article Adds: One of occupational therapy’s unique goals is to facilitate performance and participation in valued activities such as work. Occupational therapists can play a pivotal role across the continuum of care to support their clients to explore and achieve their vocational goals after stroke.

Abstract

Objectives: To investigate the effect of virtual reality (VR) rehabilitation on upper extremity motor performance of patients with early stroke. Design: Pilot randomized controlled trial. Setting: Rehabilitation wards. Participants: Twenty three adults with stroke (mean age (SD) = 58.35 (13.45) years and mean time since stroke (SD) = 16.30 (7.44) days). Interventions: Participants were randomly assigned to VR group ( n=11) or control group ( n=12). VR group received nine 30 minutes upper extremity VR therapy in standing (five weekdays in two weeks) plus conventional therapy, which included physical and occupational therapy. Control group received only conventional therapy, which was comparable to total training time received by VR group (mean training hours (SD):VR = 17.07 (2.86); control = 15.50 (2.79)). Main outcome measures: The main outcome measure was the Fugl-Meyer Assessment (FMA). Secondary outcomes included Action Research Arm Test, Motor Activity Log and Functional Independence Measure. Results were taken at baseline, post intervention and 1-month post intervention. Participants’ feedback and adverse effects were recorded. Results: All participants improved in FMA scores (mean change (SD) = 11.65 (8.56), PConclusion: Although additional VR training was not superior to conventional therapy alone, this study demonstrates the feasibility of VR training in early stroke.

Abstract

Introduction: This study aimed to evaluate the safety and efficacy of a combination of levodopa and virtual reality (VR)-based therapy for the enhancement of upper limb recovery following acute stroke.Methods: This was a pilot single-blinded case series of acute stroke patients with upper extremity hemiparesis. Patients were randomised to standard care with concomitant administration of either levodopa alone (control group) or combination therapy consisting of VR-based motivational visuomotor feedback training with levodopa neuromodulation (VR group). Main clinical outcome measures were the Fugl-Meyer Upper Extremity (FM-UE) assessment and Action Research Arm Test (ARAT). Kinematic measurements of affected upper limb movement were evaluated as a secondary measure of improvement.Results: Of 42 patients screened, four patients were enrolled in each of the two groups. Two patients dropped out from the control group during the trial. Patients receiving combination therapy had clinically significant improvements in FM-UE assessment scores of 16.5 points compared to a 3.0-point improvement among control patients. Similarly, ARAT scores of VR group patients improved by 15.3 points compared to a 10.0-point improvement in the control group. Corresponding improvements were noted in kinematic measures, including hand-path ratio, demonstrating that the quality of upper limb movement improved in the VR group.Conclusion: Our results suggest that VR-based therapy and pharmacotherapy may be combined for acute stroke rehabilitation. Bedside acquisition of kinematic measurements allows accurate assessment of the quality of limb movement, offering a sensitive clinical tool for quantifying motor recovery during the rehabilitation process after acute stroke. Abstract

Rehabilitation medicine is the medical specialty that integrates rehabilitation as its core therapeutic modality in disability management. More than a billion people worldwide are disabled, and the World Health Organization has developed the International Classification of Functioning, Disability and Health as a framework through which disability is addressed. Herein, we explore paradigm shifts in neurorehabilitation, with a focus on restoration, and provide overviews on developments in neuropharmacology, rehabilitation robotics, virtual reality, constraint-induced therapy and brain stimulation. We also discuss important issues in rehabilitation systems of care, including integrated care pathways, very early rehabilitation, early supported discharge and telerehabilitation. Finally, we highlight major new fields of rehabilitation such as spasticity management, frailty and geriatric rehabilitation, intensive care and cancer rehabilitation. [ABSTRACT FROM AUTHOR]

Abstract

Longitudinally extensive transverse myelitis (LETM) is usually associated with neuromyelitis optica and other autoimmune and inflammatory disorders but this is the first report linking it with dengue fever. Dengue infection can cause a variety of neurological complications which may result in poor recovery and long-term disability. The authors report here a patient who developed LETM in the para-infectious stage of dengue fever. The patient had a complicated clinical course resulting in severe paraparesis and urinary retention. Treatment with immunoglobulins and antiviral agents supported by a spell of early intensive rehabilitation programme produced excellent results in terms of recovery. [ABSTRACT FROM AUTHOR]

100. Gait speed assessment in confined spaces: Development of a novel automated 4-m static-start test to measure dynamic-start gait speed.

Abstract

Objectives: To promote standardization and feasible measurements of gait speed across the field, we developed a device that used light detection and ranging (LiDAR) technology to measure gait speed from a standing-start testing procedure conducted on a 4-m total walkway. We compared this automated (LiDAR-based) standing-start 4-m gait speed test (AS-4MG) with automated and manual (stopwatch-based) dynamic-start 4-m gait speed tests (AD-4MG and MD-4MG, respectively) on between-method agreement, measurement repeatability, and predictive validity with functional outcomes., Methods: A sample of 48 community-dwelling adults (mean [SD], 69 [9] years) participated in this method comparison study. Participants completed a survey and a physical function assessment, from which self-reported stair difficulty, handgrip strength, sit-to-stand performance, and gait speed (AS-, AD-, and MD-4MG) were measured., Results: Mean AS-4MG, AD-4MG, and MD-4MG were 1.12, 1.13, and 1.14 m/s, respectively, with a strong correlation (r = 0.94) and no systematic bias observed between AS-4MG and the referent AD-4MG. In the analyses of repeated gait measurements, coefficients of repeatability were <0.20 m/s for the automated tests (0.16 and 0.18 m/s for AS-4MG and AD-4MG, respectively) but not for MD-4MG (0.22 m/s). Correlations between gait speed and functional measures ranged between 0.44 and 0.68 (Ps < 0.01). Correlations for the automated tests were comparable, while AS-4MG correlations tended to exceed MD-4MG correlations., Conclusions: In community-dwelling adults, the AS-4MG test was unbiased when compared with the AD-4MG, encompassing a 9-m walkway, with both tests showing equivalence of measurement repeatability and predictive validity. The AS-4MG potentially allows gait measurements in confined spaces (e.g., doctor's office) where the gait speed test was previously unfeasible.
