Summary Background and objective The aim of this study was to investigate the association between serum pepsinogens, serum gastrin, serum vascular endothelial growth factor, serum interleukin-1 Beta, serum toll-like receptor-4 levels and Helicobacter pylori Cag A status in patients with various gastric precancerous lesions. Methods One hundred and sixty two consecutive patients with various gastric lesions [38 (23.5%) H. pylori positive chronic non-atrophic gastritis, 45 (27.8%) autoimmune gastritis, 42 intestinal metaplasia and 37 dysplasia] were enrolled into the study. Serum pepsinogen I and II, gastrin 17, vascular endothelial growth factor, interleukin-1 Beta, toll-like receptor-4 levels, H. pylori Cag A status were evaluated. Results H. pylori was positive in 98 (60.5%) patients and 38 of these patients were Cag A positive. Serum pepsinogen level was significantly lower in patients with autoimmune atrophic gastritis compared to the patients with non-atrophic chronic gastritis ( p P P = 0.002). Mean serum gastrin was 1209.6 ± 268.48 pg/mL in patients with autoimmune atrophic gastritis and 234.95 ± 184.018 pg/mL in patients with chronic non-atrophic gastritis. Mean toll-like receptor-4 level was 0.56 ± 0.098 ng/mL in patient with dysplasia, and this value was higher compared to patients with chronic non-atrophic gastritis ( P = 0.007), autoimmune atrophic gastritis ( P = 0.003) and intestinal metaplasia ( P = 0.006). Interleukin-1 Beta level was significantly lower in patients with dysplasia compared to patients with chronic non-atrophic gastritis ( P = 0.034). Conclusions Serum pepsinogens, serum gastrin and H. pylori Cag A status are important tests in detecting gastric precancerous lesions. However, toll-like receptor-4 may be a sensitive test to differentiate the patients with dysplasia from the other precancerous gastric lesions. Non-invasive tests are sensitive in the diagnosis of gastric precancerous lesions.

WOS: 000386954300001, PubMed: 27809934, Background: The aim of this study was to assess health-related quality of life (HRQOL) among chronic hepatitis B (CHB) patients in Turkey and to study related factors. Methods: This multicenter study was carried out between January 01 and April 15, 2015 in Turkey in 57 centers. Adults were enrolled and studied in three groups. Group 1: Inactive HBsAg carriers, Group 2: CHB patients receiving antiviral therapy, Group 3: CHB patients who were neither receiving antiviral therapy nor were inactive HBsAg carriers. Study data was collected by face-to-face interviews using a standardized questionnaire, Short Form-36 (SF-36) and Hepatitis B Quality of Life (HBQOL). Values equivalent to p < 0.05 in analyses were accepted as statistically significant. Results: Four thousand two hundred fifty-seven patients with CHB were included in the study. Two thousand five hundred fifty-nine (60.1 %) of the patients were males. Groups 1, 2 and 3, consisted of 1529 (35.9 %), 1721 (40.4 %) and 1007 (23.7 %) patients, respectively. The highest value of HRQOL was found in inactive HBsAg carriers. We found that total HBQOL score increased when antiviral treatment was used. However, HRQOL of CHB patients varied according to their socio-demographic properties. Regarding total HBQOL score, a higher significant level of HRQOL was determined in inactive HBV patients when matched controls with the associated factors were provided. Conclusions: The HRQOL score of CHB patients was higher than expected and it can be worsen when the disease becomes active. Use of an antiviral therapy can contribute to increasing HRQOL of patients.

SUMMARY Barrett's esophagus (BE) is a complication of chronic gastroesophageal reflux disease and can be diagnosed when there is an endoscopically irregular Z-line and intestinal metaplasia (IM) in a biopsy obtained lower esophagus. It is still not clear whether IM in the gastric cardia or columnar mucosa without IM in the lower esophagus have any significance as BE, which is considered as preneoplastic. The aim of the study was to determine the immunohistochemical features of BE and columnar mucosa in the distal esophagus and also to evaluate the value of chromoendoscopy in the diagnosis of BE in a prospective manner. A total of 12 chromoendoscopic biopsies (six from normal-looking unstained esophagus and six from esophageal mucosa stained with methyl blue suspicious of BE) were taken from 111 cases who underwent endoscopy because of a variety of upper gastrointestinal symptoms. Immunohistochemical analysis was performed using CK7, CK20, p53, Ki67, and cyclooxygenase 2 (COX2). Of the 111 cases, 19 cases with carcinoma (nine adeno, six squamous, four undifferentiated carcinomas) and 17 cases with normal squamous epithelium were excluded, while 75 cases showing columnar epithelium, including 46 (61.3%) with IM and 29 (38,7%) without IM, were further evaluated immunohistochemically. CK7 was observed in surface, crypt, and glandular epithelium, whereas CK20 was expressed in surface and superficial crypt epithelium. No significant difference was observed between the Barrett and non-Barrett type of CK7/20 staining pattern (P > 0,05). Expression of p53 did not show any difference between BE and columnar mucosa without IM, whereas COX2 expression was significantly increased in BE (P < 0.05) in comparison with columnar mucosa without IM. Ki67 expression was significiantly higher both in upper and lower crypts in BE (P < 0.05). The present study showed that a Barrett pattern does not seem to exist; however, the analysis of COX2 expression and the Ki67 proliferation fraction by immunohistochemistry can be used to separate BE from non-Barrett's metaplasia of the distal esophagus. In our point of view, the immunohistochemical detection of p53 expression in Barrett's metaplasia stage is useless as a marker for early detection of high-risk patients.

Incidence of the esophagus adenocarcinoma has been dramatically increasing in Western countries since the last decade. Gastroesophageal reflux disease and Barrett's esophagus are risk factors for adenocarcinoma. Methylenetetrahydrofolate reductase (MTHFR) genes play a key role not only in folate metabolism but also in esophagus, stomach, pancreatic carcinoma, and acute leukemias. Studies have suggested that genetic polymorphisms of MTHFR (C677T) may clarify the causes and events involved in esophageal carcinogenesis. In this study, we evaluated MTHFR C677T and A1298C polymorphisms, and vitamin B12, folate, and plasma homocystein levels in patients with esophageal adenocarcinoma (EAC), Barrett's esophagus (BE), chronic esophagitis, and healthy controls (n = 26, n = 14, n = 30, and n = 30, respectively). The mean age of patients in the EAC and BE groups was significantly higher compared with the control group (P < 0.001, P = 0.003, respectively). In all patient groups, serum folate levels were significantly lower than that of the control group (P < 0.01, P < 0.05, and P < 0.01, respectively). There was no statistically significant association between folate levels and MTHFR gene polymorphisms. No differences were found in terms of MTHFR gene polymorphisms, homocystein, and B12 levels among the groups. MTHFR gene polymorphisms and folate deficiency are not predictors of early esophageal carcinoma. However, further studies using larger series of patients are needed to evaluate the effect of genetic polymorphisms in the folate metabolic pathway and to clarify the role of folate deficiency and folate metabolism in the development of esophagus adenocarcinoma.

Behcet disease is multisystemic disorder which is characterized by a vasculitis. Gastrointestinal involvement is seen in 0-5% of the patients with Behcet’s disease. The most common places for the involvement are terminal ileum and cecum. However, it may be seen anywhere of the gastrointestinal system from mouth to anus. The lesions are seen as a deep or localized ulcer. Behcet Disease may radiologically imitate inflammatory bowel disease or neoplastic disorders. In those circumstances, Behçet’s disease should be thought for the differential diagnosis.

Aim/Materials and Methods Between January 2000 and June 2007, 3,548 endoscopic retrograde cholangiopancreatography (ERCP) were performed for extrahepatic cholestasis, cholangitis, and choledocholithiasis. The results of ERCPs were evaluated retrospectively and examined carefully to investigate the management and endoscopic therapy of biliary parasites. Results Of the 3,548 patients who underwent ERCP, 24 (0.66%) were found to have biliary parasitosis. The mean age of the biliary parasitosis patients (16 women) was 48.6 (15–77) years. Of these 24 cases, 16 patients had hydatid cystic disease (eight with partial obstruction of the biliary tract, and eight with ruptured cysts), four patients had Fasciola hepatica, and four patients had Ascaris lumbricoides infestation. Endoscopic sphincterotomy was performed, after which the choledochus was examined carefully by balloon catheter and basket procedure. Conclusion The ERCP procedure is very useful in the therapy of biliary parasitic infestations.

Goal: Recent studies suggest an increasing prevalence of atypical initial symptoms in patients with celiac disease (CD). The aim of this study was to compare the presenting symptoms of CD at 2 comparable referral institutions in South Florida, and in Ankara, Turkey. We retrospectively reviewed the records of patients with an initial diagnosis of CD by small bowel biopsy during the same (1991 to 2001) 10-year period at Cleveland Clinic Florida and University of Ankara, Turkey. A numerical score was assigned to presenting symptom and signs. Results: There were 28 patients in the South Florida group (51% men) and 40 patients in the Turkish group (35% men) (P = NS). Barrett's esophagus is a condition that is premalignant for adenocarcinoma of the esophagus and the esophagogastric junction. Early detection of Barrett's metaplasia and dysplasia is very important to decrease the mortality and morbidity from esophageal adenocarcinoma cancer. This study aimed to evaluate the effectiveness of methylene blue-targeted biopsies in the differential diagnosis of intestinal metaplasia, dysplasia, and superficial esophageal carcinoma.A total of 109 patients (43 women and 66 men; average age, 62.32 +/- 10.61 years; range, 33-82 years) were enrolled for the study. Four groups were designed before endoscopic examinations. The patients for these groups were selected at the conventional endoscopy, and then chromoendoscopy was performed. The esophagus was stained with methylene blue, after which six biopsies were taken from stained and unstained areas.Conventional and chromoendoscopic assessments were compared with histopathologic examination. The sensitivity of chromoendoscopy for Barrett's epithelium was superior to that of conventional endoscopy (p0.05). However, there was no statistical difference between the two methods in the diagnosis of esophagitis or esophageal carcinoma (p0.05). Stained biopsies were superior to unstained biopsies in terms of sensitivity for Barrett's epithelium and esophageal carcinoma (p0.001).Chromoendoscopy is useful for delineating Barrett's epithelium and for indicating the correct location for securing biopsies where dysplasia or early esophageal cancer is suspected.

Introduction: Gastrointestinal (GI) involvement in Behçet’s disease (BD) mainly appears in mucosa and affects 5–40% of patients, however the effects of the disease on lower esophageal sphincter (LES) pressure and esophageal contractions are not well known. The aims of this study were to evaluate esophageal motor function and to identify whether there was any specific motility pattern for patients with BD who had upper GI symptoms without endoscopic abnormality. Materials and Methods: 25 patients with BD, with a mean age of 43.1 (range 20–66) years, were admitted to our clinic whose main complaints were dyspeptic such as reflux, epigastric pain, vomiting and bloating. 25 healthy and age-matched individuals were also included in the study as controls. After one night fasting, LES pressure and esophageal contractions were measured. Results: Esophageal motor abnormalities were detected in 16% (4/25) of these patients with manometric studies (non-specific esophageal motor disorder in 1, esophageal hypomotility in 2, and LES hypotension in 1 patient); 16% (4/25) of these patients had endoscopic findings and overall 32% (8/25) of the cases showed esophageal pathology. All cases with esophageal motor abnormalities were suffering from reflux and endoscopy showed grade B esophagitis in 2 of these cases. Median LES pressure and LES relaxation were significantly lower in patients with BD compared to the control group (16.8 ± 10.5 vs. 20.4 ± 6.1, p = 0.02, and 92.1 ± 10.1 vs. 96.4 ± 4.5, p = 0.03 respectively). Conclusion: Esophageal involvement in BD is significantly high. We propose manometric studies are necessary to evaluate esophageal manifestations in BD patients with esophageal symptoms even without endoscopic findings.

Background and Aims:Intestinal Behcet’s disease may cause serious complications, including massive hemorrhage, fistulisation and intestinal perforation, which are encountered in approximately 50% of patients. Currently, there is little data on iatrogenic ileocolonic perforation during colonoscopy in patients with intestinal Behcet’s disease; therefore, our aim is to perform a retrospective review of records of intestinal Behcet’s disease patients who suffered perforation during or after colonoscopy. Materials and Methods: A total of 2615 colonoscopic examinations were performed between May 2002 and December 2007. The main indication for colonoscopy was intestinal Behcet’s disease in 135 of the 2615 patients. Results:135 patients with Behcet’s disease were evaluated by colonoscopy due to presumed ileocolonic involvement. Eight out of 135 (5.9%) patients had ileal and colonic ulcers. 3 patients (2.22%) had iatrogenic perforation; 2 of whom had profound ulcers in proximal colon and ileum. The third case had ulcers in the sigmoid, descending and transverse colon segments. All 3 patients had undergone surgical intervention that included ileal resection and right hemicolectomy. Conclusion:Colonoscopic examination is commonly used in Behcet’s disease not only for diagnostic purposes but also for surveillance of intestinal involvement. Volcano-shaped ulcers are especially prone to perforate. Both clinicians and endoscopists should be alert against barotrauma during colonoscopy since it may cause perforation. Patients should also be followed, and in case of abdominal pain after colonoscopic examination, colonic perforation should be considered., Giriş ve Amaç:İntestinal Behçet Hastalığı ciddi komplikasyonlara neden olabilir. Masif kanama, fistülizasyon ve intestinal perforasyon, intestinal Behçet Hastalığı olanların yaklaşık %50’sinde rastlanan komplikasyonlardır. İntestinal Behçet tanısı alanlarda, kolonoskopi sırasındaki iatrojenik ileokolonik perforasyonu inceleyen yeterli çalışma ve data yoktur. Bu nedenle biz intestinal Behçet Hastalığı olanlarda kolonoskopi sırasında ve kolonoskopi sonrasında gelişen perforasyon sorununu incelemeyi amaçladık. Gereç ve Yöntem:Mayıs 2002 ile Aralık 2007 tarihleri arasında üniversitemizde yapılan 2615 kolonoskopi olgusu değerlendirildi. Bu 2615 olgunun 135’inin kolonoskopi için ana endikasyonu intestinal Behçet Hastalığı idi. Bulgular: Toplam 135 intestinal Behçet hastasında ileokolonik tutulum olup olmadı-ğını anlamak için kolonoskopi yapıldı. 135 hastanın 8’inde (%5,9) ileal ve kolonik ülserler saptandı. İatrojenik perforasyon üç olguda (%2,22) görülürken, bunların 2’sinde proksimal kolon ve ileum’da ülserler bulundu. Üçüncü olgunun ülserleri sigmoid kolon, inen ve transvers kolon segmentlerindeydi. Bu 3 olgunun hepsi de ileal rezekziyon ve sağ hemikolektomi için cerrahiye gönderildiler. Sonuç:Behçet Hastalığında yalnız tanı için değil, aynı zamanda Behçet Hastalığının intestinal tutulumunun sürveyansı için kolonoskopi muayenesi çok yaygın olarak kullanılır. Volkan biçimli ülserler perforasyona özellikle eğilimlidirler. Konoskopi sırasında aşırı hava verilmesi perforasyona sebep olabileceğinden klinisyenler ve endoskopistler bu konuda uyanık olmalıdır. Ayrıca hastalar; kolonoskopik inceleme sonrasında karın ağrısı durumunda mutlaka takip edilmeli ve kolonik perforasyon her zaman akılda tutulmalıdır.

BACKGROUND: Receptor-binding cancer antigen expressed on SiSo cells (RCAS1) is a novel tumour marker that has been described in various kinds of cancer. The majority of observations include immunohistochemical studies; however, there are not enough data about the utility of this antigen as a serum tumour marker and its tumour specificity.AIM: To measure the serum levels of RCAS1 in patients with gastrointestinal (GI) tract cancers and compare them with other GI tract tumour markers.PATIENTS AND METHODS: Sera collected from patients with GI cancers (14 esophagus, 32 gastric and 36 colon) and from healthy volunteers (30 individuals) were analyzed for RCAS1 and compared with carcinoembryonic antigen (CEA) and cancer antigen 19-9. The relationship between serum RCAS1, tumour stage and tumour grade was also evaluated.RESULTS: Mean serum RCAS1 level was higher in patients with GI tract cancers compared with the control group (P=0.001). Among GI tract cancers, RCAS1 had lowest and highest sensitivity for esophagus and colon cancer diagnosis, respectively. Serum RCAS1 had a higher sensitivity for malignancy, except in the colon, and lower specificity in all groups compared with CEA. In comparison with cancer antigen 19-9, serum RCAS1 was more sensitive but less specific for all GI cancer groups. Mean serum RCAS1 levels were not statistically significant among histopathological tumour types (P>0.05). Although serum RCAS1 levels were significantly higher in cases with lymph node involvement compared with lymph node-negative cases (P=0.009), there was no difference between cases with and without serosal involvement, vascular invasion and distant metastasis; no correlation was found between tumour size and RCAS1 levels.CONCLUSIONS: RCAS1 may be used and combined with CEA as a tumour marker in GI tract cancers.

Chronic hepatitis B virus (HBV) infection is a common infectious disease in the world. Two percent of the patients with chronic HBV infection will develop cirrhosis each year, and will die prematurely from cirrhosis or hepatocellular carcinoma. So far interferon alfa and lamivudine are the only effective drugs. Interferon alfa can be used at the dosage of 9-10 million units thrice a week for 4-6 months either intramuscularly or subcutaneously for standard treatment. Interferon provides sustained response in 1/3 of the patients when HBe Ag is (+). Response rate to interferon therapy in HBe Ag(-) patients was similar to that reported in HBe Ag(+) ones. However the responses were not sustained in many patients, serum HBV-DNA reappear in most after stopping the treatment. Extension of therapy to 1 year may be needed in patients who are unresponsive to therapy and having HBV-DNA levels10 pg/ml and ALT100 IU. Establishment of unresponsiveness in early phases of the therapy is important and careful follow up of serum HBe Ag may be helpful. Interferon alfa treatment of chronic HDV infection may result in temporary normalization of ALT values. Disappearance of HBV-DNA and HBs Ag from the serum upon completion of therapy dictates sustained response in chronic HDV infection. The long term effect of interferon with respect to virological and histopathological responses is poor in HDV infection and longer periods of therapy in higher doses may be beneficial.

Early gastric cancer (EGC) is defined as gastric cancer confined to mucosa or submucosa, regardless of the presence or absence of lymph node metastasis. Generally, patients with multifocal EGC have two or three lesions but rarely have more than three lesions. In this article, we present a patient who was admitted to the hospital with complaints of epigastric pain and fatique. His endoscopic examination revealed four EGC lesions. We performed endoscopic mucosectomy for all lesions successfully. Endoscopic mucosal resection is an organ conservative approach and can be performed to patients with early multifocal gastric cancer. During this procedure life-threatening severe complications can occur. In our experience, severe bleeding occured during mucosectomy and our patient underwent subtotal gastrectomy. Objectives: Gastrointestinal stromal tumors (GIST) are originated from mesenchymal / stromal cells and are rarely seen in gastrointestinal tract. In this study, we aimed to determine the clinical and histopathological presentations, and tumor localizations of the patients with GIST.Materials and methods: Totally, 18 patients were included in the study. Ten patients were male (56%), and the mean age was 54.2 (30-75) years.Results: The most common sites of organs involved are stomach, peritonea, colon, small intestine, omentum and esophagus, respectively. The major symptom of the patients with GIST is abdominal pain. Distant organ metastasis was detected in 22.2% of the patients.Conclusion: We found that the most common presentation of patients with GIST is abdominal pain, and the most common involved organ is the stomach.

Nonalcoholic fatty liver disease (NAFLD) is an umbrella term that covers both a relatively benign condition, which is simple steatosis, and nonalcoholic steatohepatitis (NASH). NASH is characterized by a chronic and progressive liver pathology that may progress to cirrhosis, end-stage liver disease, hepatocellular carcinoma, and liver transplantation. Despite the growing body of evidence, one of the important and unresolved problems is the pathogenesis of NASH. It might be a metabolic disturbance as a primary abnormality in NAFLD. Insulin resistance is at the center of these metabolic abnormalities. Then, hepatocyte injury might be induced by oxidative stress. This ongoing process progresses to NASH, even to cirrhosis in some patients. In addition to oxidative stress, possibilities for the next hit are lipid peroxidation, reactive metabolites, adipose tissue products, transforming growth factor-β₁, Fas ligand, mitochondrial dysfunction, respiratory chain deficiency, and intestinal microbiota. Currently, there is no well-established and approved therapy. Recommendations are to improve existing co-morbidities, such as obesity, hyperlipidemia, or type 2 diabetes, and lifestyle modification with weight loss and exercise.

We retrospectively evaluated 6 cases to explore the role of curative pancreaticoduodenectomy and endoscopic resection of ampulla of Vater tumors. Six patients diagnosed with stage 1 ampullary tumors at Ankara University Hospital between January 1994 and September 2009 were analyzed retrospectively. Icterus was the cardinal symptom in all patients. The median time for the symptoms caused by the lesion of the papilla was 7 weeks in patients with carcinoma. The diameters of the visualized tumors were less than 20 mm. Diabetes mellitus was observed in 2 patients. When the patients were classified according to TNM staging, all patients were in stage 1. Endoscopic resection was performed in 3 patients, and the other 3 patients underwent curative pancreaticoduodenectomy. The mean ages in the curative pancreaticoduodenectomy group and endoscopic resection group were 54.3 and 80.3 years, respectively. Tumor recurrence was not observed in any patient. Among the endoscopic resection group, there was no sign of tumor recurrence for 72 months in 1 patient, 6 months in another patient, and 46 months in the last patient. One of the patients died 46 weeks after endoscopic ampullectomy. This patient also had diabetes mellitus and cardiac problems. Radical pancreaticoduodenectomy is presently the most common method for resecting malignant ampullary tumors. Endoscopic resection of carcinoma of the ampulla of Vater appears to be an alternative to surgical therapy, particularly in patients for whom surgical approach carries a high risk.

Gilead Sciences Chronic infection with hepatitis C virus (HCV) is a leading indicator for liver disease. New treatment options are becoming available, and there is a need to characterize the epidemiology and disease burden of HCV. Data for prevalence, viremia, genotype, diagnosis and treatment were obtained through literature searches and expert consensus for 16 countries. for some countries, data from centralized registries were used to estimate diagnosis and treatment rates. Data for the number of liver transplants and the proportion attributable to HCV were obtained from centralized databases. Viremic prevalence estimates varied widely between countries, ranging from 0.3% in Austria, England and Germany to 8.5% in Egypt. the largest viremic populations were in Egypt, with 6358000 cases in 2008 and Brazil with 2106000 cases in 2007. the age distribution of cases differed between countries. in most countries, prevalence rates were higher among males, reflecting higher rates of injection drug use. Diagnosis, treatment and transplant levels also differed considerably between countries. Reliable estimates characterizing HCV-infected populations are critical for addressing HCV-related morbidity and mortality. There is a need to quantify the burden of chronic HCV infection at the national level. Gilead Sciences The number of hepatitis C virus (HCV) infections is projected to decline while those with advanced liver disease will increase. A modeling approach was used to forecast two treatment scenarios: (i) the impact of increased treatment efficacy while keeping the number of treated patients constant and (ii) increasing efficacy and treatment rate. This analysis suggests that successful diagnosis and treatment of a small proportion of patients can contribute significantly to the reduction of disease burden in the countries studied. the largest reduction in HCV-related morbidity and mortality occurs when increased treatment is combined with higher efficacy therapies, generally in combination with increased diagnosis. With a treatment rate of approximately 10%, this analysis suggests it is possible to achieve elimination of HCV (defined as a >90% decline in total infections by 2030). However, for most countries presented, this will require a 3-5 fold increase in diagnosis and/or treatment. Thus, building the public health and clinical provider capacity for improved diagnosis and treatment will be critical. Introduction/Purpose: Intestinal Behcet’s disease may cause serious complications. Massive hemorrhage, fistulisation and intestinal perforation are encountered complications in approximately 50% of patients suffering from intestinal Behcet’s disease. Currently, there is not enough data inquiring iatrogenic ileocolonic perforation during colonoscopy in patients with intestinal Behcet’s disease; therefore we aimed to study intestinal Behcet’s disease patients who suffered perforation during or after colonoscopy. Methods: A total of 2615 colonoscopic examinations were performed between May 2002 and December 2007. Of these 2615 patients, main indication for colonoscopy was intestinal Behcet’s disease in 135 patients. Results: In total 135 patients with Behcet’s patients were assessed with colonoscopy due to presumed ileocolonic involvement. 8 out of 135 (5.9%) patients had ileal and colonic ulcers. 3 patients (2.22%) had iatrogenic perforation, 2 of whom had profound ulcers in proximal colon and ileum. The third case had ulcers in sigmoid colon, descending and transverse colon segments. All these 3 patients had undergone surgical intervention including ileal resection and right hemicolectomy. Discussion: Colonoscopic examination is commonly used in Behcet’s disease not only for diagnostic purposes but also for surveillance of intestinal involvement in Behcet’s disease. Volcano-shaped ulcers are specially inclined to perforate. Both clinicians and endoscopists should be alert against barotrauma applied during colonoscopy may cause perforation. Also patients should be followed-up necessarily and in case of abdominal pain after colonoscopic examination, colonic perforation should always be kept in mind.f

To evaluate the value of nuclear medicine procedures in the diagnosis of gastrointestinal involvement of Behcet's disease in asymptomatic patients, Tc-99m human immunoglobulin (HIG) and Tc-99m leucocyte (LC) whole body scintigraphies were performed on 30 patients with major symptoms of the disease. Comparison of the results with other diagnostic techniques showed that Tc-99m HIG whole body scanning can be a useful diagnostic aid before the disease becomes clinically active in the gastrointestinal system.

Cyanoacrylate injection has been found in large series to be a remarkably safe and efficient treatment of gastric variceal bleeding. Dysphagia is a common local side-effect of endoscopic intravariceal histoacryl injection. A 49-year-old woman with diagnosis of cirrhosis and portal hypertension related to hepatitis B virus infection was admitted to the Ibni Sina hospital due to dysphagia that started after injection of N-butyl-2-cyanoacrylate (histoacryl) for gastric varices. She had undergone endoscopic gastric intravariceal histoacryl injection 2 years previously and again 1 month before the admission. There were no complications related to injection of the histoacryl or the endoscopy procedure itself. Dysphagia started immediately after the second endoscopy and was refractory to prokinetic and analgesic medication. She did not have any other complaint of medical significance. A diagnostic endoscopy revealed a heterogeneous foreign body stuck in the distal esophagus. Dysphagia resolved after endoscopic removal of the foreign body by endoscopic biopsy forceps. The heterogeneous material, which was mixed with food particles and tissue debris, was analyzed via infrared spectrophotometer and was found to be cyanoacrylate glue. In the present case, extrusion of the glue occurred earlier then expected and retching or vomiting might have dislodged the extruded glue from the stomach to the esophagus. If prolonged and intractable dysphagia starts after the endoscopy procedure, impaction of glue to the esophagus should be considered.

Cytomegaloviral enterocolitis is an uncommon infection that can complicate inflammatory bowel disease. A case of a patient with a three-year history of Crohn's disease is reported. He had been in a stable condition on mesalamine 4 g/day and methylprednisolone 10 mg/day for three years until four weeks before admission. The patient was admitted with complaints of fever, abdominal pain and watery diarrhea. A diagnosis of an exacerbation of Crohn's disease was established. The radiological examination revealed narrowing of the terminal ileum. Multiple fistulas and abscess-like images were observed. The patient then underwent ileocolic resection and ileostomy. The histopathological examination revealed Crohn's ileocolitis with superimposed cytomegalovirus infection. In patients with rapidly deteriorating inflammatory bowel disease, cytomegalovirus infection should be kept in mind as one of the differential diagnoses.

Herein, we report a case presented with intermittent rectal bleeding and finally diagnosed with ganglioneuromatous polyposis and multiple adenomatous polyps in the colon. We also discuss the patients reported in the pertinent literature.

Adenosine deaminase (ADA), 5'-Nucleotidase (5NT), Xanthine oxidase (XO), Cu-Zn Superoxide dismutase (SOD) and Catalase (CAT) activities were determined in gastric juices from patients with gastric cancer, ulcer, gastritis and from healthy subjects. Enzyme activities were given as units per mi gastric juice and units per mg protein in gastric juice. ADA, 5NT and XO activities were found lower and protein concentrations were found higher in the cancer group than controls. There was however no significant difference between Cu-Zn SOD activities of the cancer and control groups. In all groups including control one, we could not find catalase activities in most of the samples. On the other hand, ADA, 5NT activities and protein concentrations in the gastric juice were lower in the gastritis group than control group. In the ulcer group, we found higher Cu-Zn SOD and XO activities and lower 5NT activity and protein concentrations compared with control values. In an attempt to establish statistical correlations between mean enzyme activities, pH and protein concentrations in the gastric juices of the groups, we found noticeable intra and inter-correlations, which indicated possible relations between DNA and free radical metabolizing enzymes.

Background: Therapeutic plasma exchange (TPE) seems to be an effective approach for clearing toxins, immune-mediated antigens, and other particles from the circulation. The aim of this study was to analyze the positive effects of TPE on clinical and biochemical parameters of liver failure. Patients and Methods: Between January 2001 and March 31, 2005 individuals (men/women, 17/14; median age, 42.7 ± 15.8 y) with acute and chronic liver failure who underwent a total of 113 TPEs (median session 3.7) were retrospectively reviewed. TPE was performed using the Fresenius AS-TEC 204 cell separator (Fresenius AG, Germany). The indication for TPE was severe coagulopathy (prothrombin time > 20 s), severe hepatic encephalopathy, hyperbilirubinemia, and candidacy for liver transplantation. All patients were examined before and immediately after the last TPE session. Results: When compared with baseline, there was significant improvement in hepatic encephalopathy stage (from median score 3.0 to 1.0, P = 0.001), serum prothrombin time (from median 26.0 to 20.0s, P = 0.003), aminotransferases (P < 0.001), and total bilirubin levels (from median 35.0 to 23.3 mg/dL, P < 0.001) after TPE. Thirteen of the thirty-one individuals (41.9%) died in the hospital. The mean follow-up period of 18 survival patients was 35.9 ± 5.6 months and 10 of those survived (55.6%, 10/18). No serious adverse effect of TPE was observed in any of the patients during or after completion of TPE. Only 6 patients experienced minor transfusion reactions. Conclusions: TPE seems to be effective in improving hepatic encephalopathy stage and liver tests in individuals with acute and chronic liver failure. The data suggest that TPE is safe and tolerable in such individuals, however, overall survival remains poor despite TPE.

Aim: In developed countries, there has been a recent increase in the prevalence of adenocarcinoma of the esophagus and cardia, along with a decrease in distal gastric cancers. Little is known regarding the prevalence of these diseases in developing countries. The aim of the present study was to evaluate changes in the prevalence of gastric adenocarcinomas in Turkey as a function of anatomic location. Methods: Data were retrospectively collected from 16 centers from January 1990 to December 2000. Owing to the exclusion criteria, a total of 4065 cases of tumors of the stomach and distal esophagus were included. Tumors localized to the body, the antrum and pyloric channel were considered distal cancers. Helicobacter pylori (H. pylori) was also detected. Results: Patients' mean age was 60.7 ± 9 years, with a male : female ratio of 68:32. - Abstract
Background and Aims: Gastroesophageal reflux is the most frequent disease affecting the esophagus. The purpose of this study was to determine if there has been any change in esophagitis detection rates in recent years. Materials and Methods: All 63,854 upper gastrointestinal endoscopies performed at Ankara University, Gastroenterology Department, Ibn-i Sina and Cebeci Endoscopy Centers, between 1990 – 2008 were analyzed retrospectively. Grading for esophagitis was done using Savary – Miller Classification until 1999, after which the Los Angeles Classification was used. Age, sex, and other endoscopic findings like hiatal hernia, duodenal ulcer, bulbar deformity, gastric ulcer, pyloric stenosis, apical stenosis, gastric operations, gastritis, and alkaline reflux were recorded. Results: Of these 63,854 cases, 52.9% were women. The mean age was 46.75 (15-98). Esophagitis was detected in 10,275 (16.1%) cases. Severity was grade I in 61.9%, II in 26.5%, III in 8.1%, and IV in 3.4%. Esophagitis frequency and severity increased with age ( Giris Mide iceriginin ozofagusa gecmesi fizyolojik bir olaydir. Ozofagusa gecen mide iceriginin rahatsizlik yaratmasi veya ozofagus hasarina neden olmasina Gastroozofageal Reflu Hastaligi (GORH) denir. Gelisimindeki en onemli unsur reflu onleyici mekanizmalarin bozulmus olmasidir. Gastroozofageal Reflu Hastaligi toplumda sik gorulen, hayat kalitesini olumsuz etkileyen ve saglik sistemine giderek daha fazla yuk olan pahali bir hastaliktir (1, 2). Hastaligin seyrinde, Barret ozofagusu, hemoraji, striktur, perforasyon ve kanser gelisimi gibi ciddi komplikasyonlar gelistigi bilinmektedir. Yetiskinlerin %15-44'unde ayda bir, %20'sinde haftada bir, %7'sinde gunde bir kez reflu semptomlari goruldugu bildirilmistir. Bati toplumunda ust sindirim sistemi endoskopisi yapilan hastalarin %15-25'inde ozofajit bulundugu raporlanmistir. Asya'dan yapilan calismalarda ise bu sikligin %0.8-16.3 gibi cok daha az siklikta goruldugu bildirilmekte ve batililasmayla beraber sikligin giderek arttigi iddia edilmektedir (3). Son yillarda hem hekim ve hastalarin farkindaliginin artmasi ile daha fazla tani konmasi, hem de prevalansinda gercek bir artis oldugu bildirilmektedir. Gastroenterolojistlerin %90'i ve aile hekimlerinin %67'sinin gorusunun prevalansin arttigi yonunde oldugu bildirilmistir (4). Bu calismanin amaci ulkemizde yillar icinde ozofajit sikliginda degisiklik olup olmadigini degerlendirmektir. Gerec ve Yontem Ankara Universitesi Gastroenteroloji Bilim Dali Ibni Sina Hastanesi ve Cebeci Klinigi Endoskopi merkezlerinde 1990 – 2008 yillarinda yapilan 63854 ust gastrointestinal sistem endoskopisi sonucu retrospektif olarak degerlendirildi. Objective: The aims of this study were to evaluate the typing accuracy of conventional smear (CS), cell block (CB) preparations and combined use of both procedures (CS + CB) for the diagnosis of hepatic malignancies and to determine whether immediate on-site cytopathological evaluation improves the diagnostic yield of liver fine-needle aspiration cytology (FNAC). Methods: Ultrasound-guided FNABs were performed on 323 consecutive cases with liver masses between December 2002 and December 2004. Histologically and/or clinically correlated 167 cases were included in the study. Preliminary FNAB results, results of CS, CB, and combined use of CS and CB were compared regarding diagnostic sensitivity, specificity, and accuracy for the diagnosis of malignancy. Subtyping accuracies of different methods were also compared. Results: The sensitivity of on-site cytopathological examination and CS were both 92.8%. The sensitivity of CS + CB was slightly better than that of CB (93.5% versus 84.8%). Specificity of all procedures was achieved 100%. Diagnostic accuracy of on-site cytopathological evaluation, CS, CB, and CS + CB were 93.9%, 93.9%, 87.2%, and 94.5%, respectively. A specific subtype diagnosis of malignant tumours could be rendered accurately on the basis of preliminary diagnosis in 71%, CS in 75.4%, CB in 78.3% and combined approach in 92% of cases. In terms of typing accuracy, 87.5% of HCCs, 93.2% of adenocarcinomas, 92.3% of neuroendocrine carcinomas, 100% of lymphomas and 100% of other malignant tumours were correctly subclassified in the final cytopathological diagnosis. The agreement between preliminary diagnosis and final cytopathological diagnosis was 77.2%. Conclusion: With use of on-site cytopathological evaluation and combined use of CS and CB, the diagnostic accuracy of liver tumours approaches 100% and also significantly improve diagnostic and subtyping accuracy of liver malignancies.

Hydatid disease is an important health problem in areas where it is endemic. There are several therapeutic modalities, the most important being surgery, antibiotherapy, and percutaneous treatment. In recent years percutaneous treatment has become popular, and for this method or surgery it is sometimes lifesaving to know the relation between the biliary ducts and the cyst cavity. The aim of this study was to examine the usefulness of endoscopic retrograde cholangiopancreatography and (99m)Tc-labeled albumin macroaggregates in diagnosing hydatid disease fistulae before percutaneous or surgical treatment. A total of 72 patients diagnosed with hepatic hydatid disease via ultrasound and serologic tests were enrolled in the study. Endoscopic retrograde cholangiopancreatography was successfully performed in all patients. (99m)Tc-labeled albumin macroaggregates also were injected into cysts at a dose of 1.5-2 mCi just before the treatment. All but three patients were treated percutaneously. Scintigraphy of abdominal and thoracic areas was performed with a GE Starcam 3200 XC/T gamma camera at 30 and 120 min after Tc-labeled albumin macroaggregate injections. Endoscopic retrograde cholangiopancreatography revealed communications between biliary ducts and cyst cavities in nine patients (12.5%). However, (99m)Tc-labeled albumin macroaggregates showed not only leakage into the systemic circulation in nine patients but also into the biliary ducts in two (15.4%). In one patient, mild acute pancreatitis occurred as a complication of endoscopic retrograde cholangiopancreatography. No complications of (99m)Tc-labeled albumin macroaggregates injection were seen. Three patients were surgically treated because of clinically manifested cystobiliary fistulae. We conclude that endoscopic retrograde cholangiopancreatography is a gold standard technique for the diagnosis of communication between the biliary duct and the cyst cavity, and (99m)Tc-labeled albumin macroaggregate injection is useful for revealing leakage into the systemic circulation. The diagnosis of biliary fistulae before percutaneous treatment of hydatid disease may enable planning of the optimal therapy.

A new classification algorithm, called benefit maximizing classifier on feature projections (BCFP), is developed and applied to the problem of diagnosis of gastric carcinoma. The domain contains records of patients with known diagnosis through gastroscopy results. Given a training set of such records, the BCFP classifier learns how to differentiate a new case in the domain. BCFP represents a concept in the form of feature projections on each feature dimension separately. Classification in the BCFP algorithm is based on a voting among the individual predictions made on each feature. In the gastric carcinoma domain, a lesion can be an indicator of one of nine different Levels of gastric carcinoma, from early to late stages. The benefit of correct classification of early levels is much more than that of late cases. Also, the costs of wrong classifications are not symmetric. In the training phase, the BCFP algorithm learns classification rules that maximize the benefit of classification. In the querying phase, using these rules, the BCFP algorithm tries to make a prediction maximizing the benefit. A genetic algorithm is applied to select the relevant features. The performance of the BCFP algorithm is evaluated in terms of accuracy and running time. The rules induced are verified by experts of the domain. (C) 2004 Elsevier B.V. All rights reserved.

OBJECTIVES: The aims of this study were to investigate the efficacy of a new percutaneous treatment modality of hydatid disease of the liver and to present the results of long term follow-up. METHODS: Eighty-seven patients (55 female, mean age 43.5 yr) with 98 hydatid cysts (73 type I, 15 type II, and 10 type III) in the liver underwent percutaneous treatment. All patients were examined by ultrasonography and some of them were examined by CT. They were all positive by indirect hemagglutination test. Sonographic guidance was used in all patients. The procedure included the puncture and free drainage of the cyst fluid. After free drainage was stopped, absolute alcohol and polidocanol 1% were used as sclerosing agents. The patients were followed-up with periodic ultrasonographic examinations. RESULTS: The mean follow-up time was 33 months. The mean diameter of the cysts decreased from 77.0 ± 2.7 mm to 63.0 ± 2.5 mm (p < 0.001). The entire cyst cavity filled with a solid echo pattern in 32 cysts, two-thirds of the cyst cavity showed a pseudotumor echo pattern in 34 cysts, and one-third of the cyst cavity showed a pseudotumor pattern in 23 cysts, whereas no pseudotumor appearence was observed in eight cysts. Apart from an anaphylactoid reaction observed in one patient, no major complication occurred during the follow-up period. CONCLUSIONS: Long term results indicate that this new percutaneous treatment modality of the hydatid disease of the liver is an effective and safe method without causing major complications. Percutaneous treatment of hydatid cysts of the liver offers good results and should be the first choice, especially for patients who are contraindicated to surgery.

Hepatic epithelioid hemangioendothelioma (HEH) is a rare vascular tumor of the liver of unknown etiology. Although HEH is usually characterized by a low grade malignancy and a good long-term prognosis, its growth can be progressive and lead to hepatic failure, extrahepatic metastasis and death. Several different antineoplastic agents have been proposed for cases of nonresectable HEH. We report the case of a 45-year-old man with HEH successfully treated with intravenous infusion of Adriamycin.

We have studied the prevalence of HBV, HCV and HDV in patients with hepatocellular carcinoma (HCC), referred to our unit, from 1983 to 1992. This study was carried out on 169 patients (149 men, 20 women, mean age 50.1, range 18–80) diagnosed by imaging procedures, confirmed by histopathology. Between 1983 and 1989, 110 of these patients were examined for hepatitis B virus’ marker only. While between 1989 and 1992, 59 patients were tested for both HBV and HCV. Sera were tested for HBV markers by RIA (Abbott) and for anti HCV by ELISA (Abbott, second generation). In 110 patients studied from 1983 to 1989, seropositivity for HBsAg was detected in 66/110 (60.0%); while in 59 patients who were studied from 1989 to 1992 the findings are 37/59 (62.7%) seropositivity for HBsAg and 8/59 (13.6%) seropositivity for anti HCV. Of these, 8 patients, 3 (5.1%) were positive for both HBsAg and anti HCV. 14 cases (23.7%) of the study group were negative for both HBsAg and anti HCV. In summary, no significant difference was found between patients with chronic liver disease and the study group for HBV and HCV status. Whereas the prevalence of anti-HDV (14.4%) of those patients with HCC was less than that of patients with HDV in chronic liver disease.

The Stomach cancer has been encountered as the most common tumor in gastrointestinal system in Turkey.We have periodically performed the upper gastrointestinal endoscopy for the patients with precancerous lesions such as intestinal metaplasia, atrophic gastritis and polyps. We have used dyes such as metilen blue, indigocarmin during the examination in selective cases.We have preoperatively diagnosed forty one cases ,31 male and 10 female, in early stage of the cancer since 1987 up to now.In order to diagnose the early stomach cancer, it is necessary to perform periodically endoscopic examinations and dye endoscopy in high risk groups for gastric cancer.

Ninetynine patients, 67 male and 32 female, who applied to endoscopy laboratory of gastroenterology department of the Ankara University School of Medicine were diagnosed to esophageal carcinoma between the years 1986 and 1993. Lugol was routinely used in selective cases. Four out of 99 cases were in early stages. Two cases had submucosal invasion without lymph node metastasis. One case showed flat undyed area with lugol and she was proved to have squamous cell carcinoma but she refused the operation. One case showed severe dysplasia and she had liver metastasis after three years of operation.
