Abstract

Pancreatic ductal adenocarcinoma (PDAC) is projected to become the second cancer-related cause of death by 2030. Identifying novel risk factors, including genetic risk loci, could be instrumental in risk stratification and implementation of prevention strategies. Long noncoding RNAs (lncRNAs) are involved in regulation of key biological processes, and the possible role of their genetic variability has been unexplored so far. Combining genome wide association studies and functional data, we investigated the genetic variability in all lncRNAs. We analyzed 9893 PDAC cases and 9969 controls and identified a genome-wide significant association between the rs7046076 SNP and risk of developing PDAC (P = 9.73 × 10−9). This SNP is located in the NONHSAG053086.2 (lnc-SMC2-1) gene and the risk allele is predicted to disrupt the binding of the lncRNA with the micro-RNA (miRNA) hsa-mir-1256 that regulates several genes involved in cell cycle, such as CDKN2B. The CDKN2B region is pleiotropic and its genetic variants have been associated with several human diseases, possibly though an imperfect interaction between lncRNA and miRNA. We present a novel PDAC risk locus, supported by a genome-wide statistical significance and a plausible biological mechanism. © 2021 UICC

Abstract

In the Active-Controlled Fracture Study in Postmenopausal Women With Osteoporosis at High Risk (ARCH) clinical trial (NCT01631214), 1 year of romosozumab followed by alendronate reduced the risk of vertebral and nonvertebral fractures compared to alendronate alone in women with prevalent fracture. We performed post hoc analyses of data from patients in ARCH (romosozumab, n = 1739; alendronate, n = 1726) who had a baseline BMD measurement and received at least one open-label alendronate dose. We evaluated 1-year mean BMD and corresponding T-score changes; proportions of patients achieving T-scores > -2.5 at the total hip (TH), femoral neck (FN), and lumbar spine (LS); and group differences in fracture rates after 12 months, while all participants were on alendronate. Subsequently, we investigated the relationship between T-scores achieved at the TH, FN, and LS at 12 months and subsequent fracture incidence. At 1 year, mean change from baseline in TH BMD was 6.3% (T-score change 0.31) with romosozumab versus 2.9% (T-score change 0.15) with alendronate (p <.001). The proportion of patients with TH T-score > -2.5 increased from 34% at baseline to 55% after 1 year of romosozumab and from 32% at baseline to 44% after 1 year of alendronate. Compared with patients receiving alendronate in year 1, those receiving romosozumab had a 75% reduction in new or worsening vertebral fracture (p <.001) in year 2, and a 19% reduction in nonvertebral fracture (p =.120) and 40% reduction in hip fracture (p =.041) during the open-label period. TH and FN T-scores achieved at month 12 were associated with subsequent nonvertebral and vertebral fracture rates and the relationships were independent of treatment received. LS T-score at 12 months was associated with vertebral but not nonvertebral fracture risk. We conclude that 1 year of romosozumab leads to larger BMD gains versus alendronate, and that the T-score achieved with either therapy is related to subsequent fracture risk. These data s

Abstract

The area measured at the second to third lumbar level best represents the total visceral adipose tissue (AT) volume in Italians. Computed tomography is used to obtain AT volume from the AT area of five scans and the distances between the scans. The waist circumference along with age efficiently predicts AT volume in men and pre-menopausal women, and in post-menopausal women when height is included. Participants in the study included 72 women and 18 men aged between 18 and 69 years. The results are largely consistent with those of other researchers.

Abstract

Objective: To determine clinical diabetes-related risk factors for fragility fractures in type 1 diabetes (T1D). Research design and methods: History of bone fragility fractures occurring after T1D diagnosis was assessed by questionnaire in this cross-sectional study in 600 T1D subjects. Glycated hemoglobin A1c (HbA1c)over the previous 5 years was used as an index of long-term glycemic control; complications were adjudicated by physician assessment. Multinomial logistic regression models were used to assess the associations between diabetes-related risk factors and fracture history. Results: One-hundred-eleven patients (18.5%)reported at least one fracture; of these 73.8% had only one and 26.2% had more than one fracture. Average age was 41.9 ± 12.8 years, with even gender distribution; disease duration was 19.9 ± 12.0 years; and BMI was 24.4 ± 3.7 kg/m2. The 5-year average HbA1c was 7.6 ± 1.0% (60 mmol/mol). In adjusted models, reduced risk for 1 fracture was found in those with higher creatinine clearance rate (CCr)(RRR 0.22 [95% CI: 0.06–0.83]for 1 unit increase in lnCCr, p = 0.03)and increased risk in those with neuropathy (RRR 2.57 [1.21–5.46], p = 0.01). Increased risk for ≥2 fractures was found in subjects in the highest tertile of HbA1c (≥7.9%)compared with the lowest tertile (≤7.17%)(RRR 3.50 [1.04–11.7], p = 0.04)and of disease duration (≥26 years versus <14 years)(RRR 7.59 [1.60–35.98], p = 0.01). Conclusions: Poor glycemic control and long exposure to the disease are independent diabetes-related risk factors for multiple bone fractures in T1D.

Abstract

Biliary cystadenoma (BCA) is a rare tumor which is difficult to diagnose before surgery. We present a case of 73 years old woman with a previous histological diagnosis of biliary cystadenoma occurred in 2009, surgically removed. In January 2018, abdomen Computerized Tomography scan (CTs) showed a 65 mm diameter cystic mass in the fifth hepatic segment. Even if the patient refused to undergo surgical resection due to her several comorbilities, imaging features and clinical history are suggestive for BCA recurrence.

High levels of ROS may generate oxidative post-translational modification (oxPTMs) that may impair proteins biological functions. We have shown that insulin could also be a target of these modifications that include glycation, oxidation and chlorination. Adipose tissue is a primary target of this hormone and a key player in insulin resistance (IR): evidence from animal studies show that the glycation of insulin or other proteins result in a significant impairment of biological function of adipose tissue, affecting adipokines expression and insulin sensitivity. However, how oxPTMs such as glycation interfere with insulin function and their involvement in IR and type 2 diabetes (T2D) is still unknown. We hypothesize that insulin glycation may be involved in insulin resistance and in the pathogenesis of T2D. Therefore, the aim of this study was to assess the possible effects of glycated vs. native insulin on adipocyte differentiation. Human recombinant insulin was glycated in vitro following our established protocol. Insulin glycation was monitored through PAGE. Human preadipocytes (HPAd), a primary cell line from subcutaneous adipose tissue, and adipose-derived stem cells (ASCs), isolated from tissues of lean and obese subjects (BMI30), were differentiated with standard adipogenic medium in presence of native or glycated insulin. Cellular differentiation was assessed by gene and protein expression analyses of adipogenic markers (adiponectin, peroxisome proliferator-activated receptor gamma -Ppar?-, fatty acid binding protein 4 -Fabp4-, glucose transporter 4 -Glut4-) and of receptors for adiponectin (AdipoR1) and for insulin (InsR), through real time RT-PCR and immunoblot respectively. Our preliminary results showed that preadipocytes (both HPAd and ASCs) exposed to glycated insulin had impaired differentiation capacities: Oil Red O-staining showed that these cells had lower number of lipid droplets compared to those exposed to native insulin, suggesting an impaired adipocyte differentiation. Considering gene expression, we observed an overall downregulation of adipogenic markers, with a significant decrease of adiponectin both in leans- and in obese-derived cells (p=0.007 and p=0.017 respectively), lower levels of Glut4 and a significant lower expression of Fabp4 (p=0.002) in cells from leans differentiated with glycated insulin. Immunoblotting showed a similar pattern to that observed with gene expression analysis but, regarding insulin receptor, protein levels of the receptor presented a significant down regulation in cells treated with glycated insulin, with a p value of 0.0266 in leans and p=0.011 in obese subjects. Conversely, gene expression of insulin receptor did not present any difference between the native and glycated insulin. In conclusion, our data suggest that glycated insulin could affect adipocytes differentiation and impair insulin receptor expression, acting at translational/post-translational level or increasing its degradation. Its role and mechanisms that lead to these effects should be investigated as a possible mediator in the pathogenesis of T2D.

Abstract

The central role of vitamin D in bone health is well recognized. However, controversies regarding its clinical application remain. We therefore aimed to review the definition of hypovitaminosis D, the skeletal and extra-skeletal effects of vitamin D and the available therapeutic modalities.
