Abstract

To improve health among the population and reduce the societal burden of care and health-related costs in a rapidly aging environment, the Japanese government launched the “National Health Promotion Program in the 21st Century” (HJ21), which contains goals concerning areas such as lifestyle behavior and the use of preventive medicine. While health care personnel are responsible for guiding others’ health choices, they may not maintain healthy lifestyles themselves. Whether these individuals are meeting the HJ21 goals has not yet been examined. This study aims to determine whether certified specialists in health management are meeting such goals, and to compare their performance with the national average. This is a cross-sectional survey study. Study participants, sourced from all prefectures in Japan, were specialists certified in health management. We measured data concerning demographic information, lifestyle (physical activity, nutrition, diet, sleep, rest, smoking, and alcohol use), and the use of preventive medicine. The study participants exhibited many characteristics of high health literacy. They had an overall healthy lifestyle and met most of the HJ21 goals of healthy lifestyle, and a high proportion underwent health maintenance examinations and cancer screenings. These practices and behaviors maybe associated with high health literacy and social engagement activities.

Abstract

Experiments were performed to characterize renal hemodynamics in Thy-1 nephritic rats. A monoclonal antibody against Thy-1 was intravenously injected to induce mesangiolysis in rats, and 2 days later renal hemodynamic responses to variations in blood pressure were determined. In the first series of experiments, autoregulation of renal plasma flow (RPF) or glomerular filtration rate (GFR) was impaired in nephritic rats. In response to a reduction in blood pressure (98 [+ or -] 2 to 80 [+ or -] 1 mmHg), both RPF (4.17 [+ or -] 0.63 to 3.20 [+ or -] 0.45 ml x [min.sup.-1] x g kidney [wt.sup.-1], P < 0.05, n = 6) and GFR (0.88 [+ or -] 0.05 to 0.75 [+ or -] 0.06 ml x [min.sup.-1] x g kidney [wt.sup.-1], P < 0.05) were decreased in nephritic rats. Intravenous administration of furosemide and 30% albumin, both of which inhibit tubuloglomerular feedback, diminished renal autoregulation in control but not nephritic rats. In the second studies, the infusion of 5'-nucleotidase, an enzyme expressed on mesangial cells, into a renal artery ameliorated the magnitude of autoregulatory decrements in GFR in nephritic rats (-16 [+ or -] 5 to -6 [+ or -] 2%, P < 0.05, n = 6), but this enzyme failed to alter renal autoregulation in control rats. In the third studies, the effects of indomethacin were examined in nephritic rats. Inhibition of prostaglandin synthesis reduced RPF (4.07 [+ or -] 0.30 to 1.54 [+ or -] 0.22 ml x [min.sup.-1] x g kidney [wt.sup.-1], P < 0.05, n = 5) and GFR (1.03 [+ or -] 0.18 to 0.69 [+ or -] 0.13 ml x [min.sup.-1] x g kidney [wt.sup.-1], P < 0.05) in nephritic rats. However, cyclooxygenase inhibition failed to restore renal autoregulation in nephritic rats. Our results indicate that renal autoregulation is impaired in Thy-1 nephritis. Furthermore, the present data provide evidence that prostanoids contribute to maintain renal circulation in nephritic rats. In particular, the health economics of the “PD first” concept, which can be advantageous for clinical practice and healthcare systems, must be evaluated.Methods: This multicenter study investigated the cost-effectiveness of PD. The major effectiveness indicator was quality-adjusted life year (QALY), with a preference-based utility value based on renal function, and the cost indicator was the amount billed for a medical service at each medical institution for qualifying illnesses. In comparison with hemodialysis (HD), a baseline analysis of PD therapy was conducted using a cost-utility analysis (CUA). Abstract

The effects of angiotensin-converting enzyme (ACE) inhibitors such as captopril and imidaprilat during low-renin hypertension were analyzed in conscious deoxycorticosterone acetate (DOCA) salt hypertensive dogs. Administration of captopril and imidaprilat on conscious DOCA salt hypertensive dogs increased urine flow rate and reduced mean arterial pressure. The significant hypotensive and renal effects of the ACE inhibitors were due to the action of bradykinin and prostaglandin formation which was mediated by angiotensin II synthesis in the kidney.

Abstract

Introduction In Japan, dialyzers are classified based on β2-microglobulin clearance. Type I dialyzers are classified as low-flux dialyzers (<10 mL/min clearance), type II and III as high-flux dialyzers (≥10 to <30 mL/min and ≥30 to <50 mL/min clearance, respectively), and type IV and V as super high-flux dialyzers (≥50 to <70 mL/min and ≥70 mL/min clearance, respectively). Super high-flux dialyzers are commonly used, but their superiority over low-flux dialyzers is controversial. Methods In this nationwide prospective cohort study, we analyzed Japanese Society for Dialysis Therapy Renal Data Registry data collected at the end of 2008 and 2011. We enrolled 242,467 patients on maintenance hemodialysis and divided them into five groups by dialyzer type. We assessed the associations of each dialyzer type with 3-year all-cause mortality using Cox proportional hazards models and performed propensity score matching analysis, adjusting for potential confounders. Results By the end of 2011, 53,172 (21.9%) prevalent dialysis patients had died. Mortality significantly decreased according to dialyzer type. Hazard ratios (HRs) were significantly higher for type I, II and III compared with type IV (reference) after adjustment for basic factors and further adjustment for dialysis-related factors. HR was significantly higher for type I, but significantly lower for type V, after further adjustment for nutrition- and inflammation-related factors. These significant findings were also evident after propensity score matching. Conclusions Hemodialysis using super high-flux dialyzers might reduce mortality. Abstract

Oral administration of selective arginine vasopressin AVP V1 receptor antagonist OPC-21268 causes an increase in cardiac output and renal function while that of AVP V2 receptor antagonist OPC-31260 results in an increase in plasma renin activity, plasma levels and serum sodium concentration of AVP in dogs with congestive heart failure. AVP V1 receptor antagonist does not affect serum electrolytes and hormones while AVP V2 receptor antagonist induces water diuresis. Combined administration of both the receptor antagonists triggers additive metabolic and renal responses and supra-additive hemodynamic responses.
