Cancer is a heterogenous disease displaying marked inter- and intra-tumoral diversity. The existence of cancer stem cells (CSCs) has been experimentally demonstrated in a number of cancer types as a subpopulation of tumor cells that drives the tumorigenic and metastatic properties of the entire cancer. Thus, eradication of the CSC population is critical for the complete ablation of a tumor. This is, however, confounded by the inherent resistance of CSCs to standard anticancer therapies, eventually leading to the outgrowth of resistant tumor cells and relapse in patients. The cellular mechanisms of therapy resistance in CSCs are ascribed to several factors including a state of quiescence, an enhanced DNA damage response and active repair mechanisms, up-regulated expression of drug efflux transporters, as well as the activation of pro-survival signaling pathways and inactivation of apoptotic signaling. Understanding the mechanisms underlying the acquisition of resistance to therapy may hold the key to targeting the CSC population. Le cancer est une maladie heterogene dans laquelle la diversite s'exprime tant d'une tumeur a l'autre qu'au sein d'une meme tumeur. Des experiences ont montre que les cellules souches cancereuses (CSC) d'un certain nombre de types de cancers constituent une sous-population de cellules tumorales responsables des proprietes tumorigenes et metastatiques de l'ensemble du cancer. Par consequent, l'eradication de la population de CSC est essentielle a l'ablation complete de la tumeur. Toutefois, l'application de cette option est compliquee par la resistance aux traitements anticancereux standards qui est inherente aux CSC et qui mene eventuellement au developpement de cellules tumorales resistantes et a des recidives chez les patients. Les mecanismes cellulaires de la resistance des CSC au traitement sont tributaires de plusieurs phenomenes, y compris les suivants: etat de quiescence, amplification de la reaction aux dommages subis par l'ADN et de la presence des mecanismes de reparation active, regulation a la hausse de l'expression des transporteurs d'efflux de medicaments, activation de voies de signalisation favorisant la survie et inactivation de la signalisation apoptotique. La comprehension des mecanismes sous-jacents de l'acquisition de la resistance au traitement pourrait etre essentielle en vue de cibler les populations de CSC. Aims: This international study aimed to assess: 1) the prevalence of preoperative and postoperative COVID-19 among patients with hip fracture, 2) the effect on 30-day mortality, and 3) clinical factors associated with the infection and with mortality in COVID-19-positive patients. Methods: A multicentre collaboration among 112 centres in 14 countries collected data on all patients presenting with a hip fracture between 1st March-31st May 2020. Demographics, residence, place of injury, presentation blood tests, Nottingham Hip Fracture Score, time to surgery, management, ASA grade, length of stay, COVID-19 and 30-day mortality status were recorded. Results: A total of 7090 patients were included, with a mean age of 82.2 (range 50–104) years and 4959 (69.9%) being female. Of 651 (9.2%) patients diagnosed with COVID-19, 225 (34.6%) were positive at presentation and 426 (65.4%) were positive postoperatively. Positive COVID-19 status was independently associated with male sex (odds ratio (OR) 1.38, p = 0.001), residential care (OR 2.15, p < 0.001), inpatient fall (OR 2.23, p = 0.003), cancer (OR 0.63, p = 0.009), ASA grades 4 (OR 1.59, p = 0.008) or 5 (OR 8.28, p < 0.001), and longer admission (OR 1.06 for each increasing day, p < 0.001). Patients with COVID-19 at any time had a significantly lower chance of 30-day survival versus those without COVID-19 (72.7% versus 92.6%, p < 0.001). COVID-19 was independently associated with an increased 30-day mortality risk (hazard ratio (HR) 2.83, p < 0.001). Increasing age (HR 1.03, p = 0.028), male sex (HR 2.35, p < 0.001), renal disease (HR 1.53, p = 0.017), and pulmonary disease (HR 1.45, p = 0.039) were independently associated with a higher 30-day mortality risk in patients with COVID-19 when adjusting for confounders. Conclusion: The prevalence of COVID-19 in hip fracture patients during the first wave of the pandemic was 9%, and was independently associated with a three-fold increased 30-day mortality risk. Among COVID-19-positive patients, those who were older, male, with renal or pulmonary disease had a significantly higher 30-day mortality risk.

71. Thrombolysis in Stroke Mimics: Comprehensive Stroke Centers vs Telestroke Sites

- Abstract
ABSTRACT:Background:Hyperacute treatment of acute stroke may lead to thrombolysis in stroke mimics (SM). Our aim was to determine the frequency of thrombolysis in SM in primary stroke centers (PSC) dependent on telestroke versus comprehensive stroke centers (CSC).Method:Retrospective review of prospectively collected data from the Quality improvement and Clinical Research (QuICR) registry, the Discharge Abstract Database (DAD), and The National Ambulatory Care Reporting System (NACRS) of consecutive patients treated with intravenous thrombolysis for acute ischemic stroke in Alberta (Canada) from April 2016 to March 2021.Result:A total of 2471 patients who received thrombolysis were included. Linking the QuICR registry to DAD 169 (6.83%) patients were identified as SM; however, on our review of the records, only 112 (4.53%) were actual SM. SMs were younger with a mean age of 61.66 (±16.15) vs 71.08 (±14.55) in stroke. National Institute of Health Stroke Scale was higher in stroke with a median (IQR) of 10 (5–17) vs 7 (5–10) in SM. Only one patient (0.89 %) in SM groups had a small parenchymal hemorrhage versus 155 (6.57%) stroke patients had a parenchymal hemorrhage. There was no death among patients of thrombolysed SM during hospitalization versus 276 (11.69%) in stroke. There was no significant difference in the rate of SM among thrombolysed patients between PSC 27 (5.36%) versus CSC 85 (4.3%) (P= 0.312). The most responsible diagnosis of SM was migraine/migraine equivalent, functional disorder, seizure, and delirium.Conclusion:The diagnosis of SM may not always be correct when the information is extracted from databases. The rate of thrombolysis in SM via telestroke is similar to treatment in person at CSC.

- Abstract
Aims: This international study aimed to assess: 1) the prevalence of preoperative and postoperative COVID-19 among patients with hip fracture, 2) the effect on 30-day mortality, and 3) clinical factors associated with the infection and with mortality in COVID-19-positive patients. Methods: A multicentre collaboration among 112 centres in 14 countries collected data on all patients presenting with a hip fracture between 1st March-31st May 2020. Demographics, residence, place of injury, presentation blood tests, Nottingham Hip Fracture Score, time to surgery, management, ASA grade, length of stay, COVID-19 and 30-day mortality status were recorded. Results: A total of 7090 patients were included, with a mean age of 82.2 (range 50–104) years and 4959 (69.9%) being female. Of 651 (9.2%) patients diagnosed with COVID-19, 225 (34.6%) were positive at presentation and 426 (65.4%) were positive postoperatively. Positive COVID-19 status was independently associated with male sex (odds ratio (OR) 1.38, p = 0.001), residential care (OR 2.15, p < 0.001), inpatient fall (OR 2.23, p = 0.003), cancer (OR 0.63, p = 0.009), ASA grades 4 (OR 1.59, p = 0.008) or 5 (OR 8.28, p < 0.001), and longer admission (OR 1.06 for each increasing day, p < 0.001). Patients with COVID-19 at any time had a significantly lower chance of 30-day survival versus those without COVID-19 (72.7% versus 92.6%, p < 0.001). COVID-19 was independently associated with an increased 30-day mortality risk (hazard ratio (HR) 2.83, p < 0.001). Increasing age (HR 1.03, p = 0.028), male sex (HR 2.35, p < 0.001), renal disease (HR 1.53, p = 0.017), and pulmonary disease (HR 1.45, p = 0.039) were independently associated with a higher 30-day mortality risk in patients with COVID-19 when adjusting for confounders. Conclusion: The prevalence of COVID-19 in hip fracture patients during the first wave of the pandemic was 9%, and was independently associated with a three-fold increased 30-day mortality risk.

The prospect of engineering the bandgap in semiconductor nanostructures all the way from ultraviolet to visible is highly significant in various applications such as photocatalysis, sensing, optoelectronics and biomedical applications. Since many semiconductors have their bandgaps in the UV region, various techniques are used to tune their bandgaps to the visible region. Doping and co-doping with metals and non-metals have been found to be highly effective in bandgap narrowing as doping creates a continuum of mid-bandgap states which effectively reduces the bandgap. Other than these techniques, the modulation of intrinsic vacancies is an effective way to control the bandgap. Among all semiconductors, titanium dioxide (TiO2) is a well-studied material for UV photocatalytic applications. TiO2 has oxygen and titanium vacancies as intrinsic defects which influence the bandgap based on its phase of existence. The oxygen vacancies generate unpaired electrons associated with Ti3+ species, resulting in the creation of donor levels within the bandgap. Trivacancies give a p-type nature to TiO2 due to excess holes and generate acceptor levels in the bandgap. The existence of a continuum of such intrinsic defect states within the bandgap appears to narrow the bandgap and enhances the visible light absorption in TiO2, although the effect is an apparent narrowing. Doping and co-doping of TiO2 with metals such as Au, Ag, Fe, Co, Ni, Pt and Pd and non-metals such as B, C, N, Br and Cl, doping with Ti3+ ions and hydrogenation have all been found to narrow the bandgap of TiO2. In this review, we focus on such intrinsic vacancy-modulated bandgap narrowing in TiO2. This review covers significant recent advancements in bandgap engineering of TiO2. [ABSTRACT FROM AUTHOR]

Intratumoral heterogeneity is a major ongoing challenge in the effective therapeutic targeting of cancer. Accumulating evidence suggests that a fraction of cells within a tumor termed Cancer Stem Cells (CSCs) are primarily responsible for this diversity resulting in therapeutic resistance and metastasis. Adding to this complexity, recent studies have shown that there can be different subpopulations of CSCs with varying biochemical and biophysical traits resulting in varied dissemination and drug-resistance potential. Moreover, cancer cells can exhibit a high level of plasticity or the ability to dynamically switch between CSC and non-CSC states or among different subsets of CSCs. The molecular mechanisms underlying such plasticity has been under extensive investigation and the trans-differentiation process of Epithelial to Mesenchymal transition (EMT) has been identified as a major contributing factor. Besides genetic and epigenetic factors, CSC plasticity is also shaped by non-cell-autonomous effects such as the tumor microenvironment. In this review, we discuss the recent developments in understanding CSC plasticity in tumor progression at biochemical and biophysical levels, and the latest in silico approaches being taken for characterizing cancer cell plasticity with implications in improving existing therapeutic approaches.

Transition metal oxide-based magnetic nanocomposites attract great attention due to their unique properties and applications in the field of energy storage. Herein, we present a facile microwave procedure for the synthesis of CuFe
2 O4 (CF) and CuFe2 O4 incorporated with reduced graphene oxide CuFe2 O4 /rGO (CG) as potential electrode materials for hybrid supercapacitor. The structure and morphology of CF and CG nanoparticles are examined. The electrochemical performance is studied in 6 M aqueous KOH electrolyte using cyclic voltammetry (CV), galvanostatic charge-discharge (GCD), and electrochemical impedance spectroscopy (EIS) techniques. The attractive CG nanocomposite exhibits high specific capacity of 800 C/g at a current density of 2 A/g and better cycling stability when compared to pure CF, due to the formation of nanostructure composed of ferrite nanoparticles homogeneously incorporated onto rGO sheets. Furthermore, the practicability of CG electrode is investigated by the fabrication of CG and activated carbon. The hybrid supercapacitor device shows excellent electrochemical performance with specific energy of 18.3 Wh/kg and a specific power of 455 W/kg. It is noteworthy that the cyclic stability is excellent with a capacity retention of ~98% after 3000 cycles manifesting the superiority of CG electrode. The proposed device demonstrates the potential to fabricate other metal oxides with activated carbon via a facile synthesis method for promoting application in energy storage materials and promoting new opportunities of binary nanocomposite.
