OBJECTIVE: The transoral approach is the most direct and commonly used method to access the ventral craniocervical junction. Recently, an endonasal, endoscopic approach to the craniovertebral junction was proposed. We reasoned that the coupling of the endoscope with the direct transoral approach and image guidance could result in a minimally invasive, simple approach to the ventral craniovertebral junction. We inves- tigated the potential usefulness of such an approach in a cadaver model. METHODS: A direct transoral approach to the craniovertebral junction was performed using computed tomography-based image guidance in 9 fresh adult head specimens. Endoscopic odontoidectomy was performed in 5 specimens. In the remaining 4 specimens, the sur- gical working area and surgical freedom associated with an endoscopic and a microscopic approach to the ventral craniovertebral junction were evaluated and compared. In these 4 specimens, we also measured and compared the exposure of the clivus provided by the endoscope and by the operating microscope without splitting the soft palate. RESULTS: With variously angled endoscopic assistance and image guidance, it was possible to tailor the excision of the anterior arch of the atlas and to precisely identify the odontoid process and its related ligaments intraoperatively, resulting in a complete and controlled odontoidectomy. The surgical area exposed over the posterior pharyngeal wall was significantly improved using the endoscope (606.5 ± 127.4 mm 3 ) compared with the operating microscope (425.7 ± 100.8 mm 3 ), without any compromise of surgical freedom (P < 0.05). The extent of the clivus exposed with the endoscope (9.5 ± 0.7 mm) without splitting the soft palate was significantly improved compared with that associated with microscopic approach (2.0 ± 0.4 mm) (P < 0.05). CONCLUSION: With the aid of the endoscope and image guidance, it is possible to approach the ventral craniovertebral junction transorally with minimal tissue dissection, no palatal splitting, and no compromise of surgical freedom. In addition, the use of an angled-lens endoscope can significantly improve the exposure of the clivus without splitting the soft palate. An endoscope-assisted transoral approach is a direct and powerful tool for the treatment of surgical pathology at the craniovertebral junction.

The authors review all cases in which ventriculosubgaleal (VSG) shunts were placed at Columbus Children's Hospital for the treatment of posthemorrhagic hydrocephalus in order to assess the surgical procedure, effectiveness of surgery, and complications of cerebrospinal fluid diversion to the subgaleal space. The purpose of the review is to make a comparison between cases in which shunts were placed in the operating room (OR) and those in which they were placed in the neonatal intensive care unit (NICU). Considerations and complications specific to patient transport to the OR or surgical implantation in the NICU are discussed.Seventeen infants with posthemorrhagic hydrocephalus were treated with VSG shunt placement over a period of 4 years. A retrospective analysis of these cases was performed to evaluate multiple aspects of the procedure. Specifically, the surgical procedure, duration of shunt function prior to shunt conversion, neuroimaging changes, operative complications, and risk of infection are discussed. The authors also performed a comparative analysis of shunt placement in the NICU and the OR.The length of the procedure was similar in the two locations. No differences in perioperative or intraoperative risks and no increased risk of infection were seen in either location in this pilot study. Interestingly, the mean lifespan of primary implants placed in the NICU (73 days) was longer than that of those placed in the OR (43 days).Ventriculosubgaleal shunt placement offers a safe and effective temporary means of treating post-hemorrhagic hydrocephalus and can be reliably and safely performed at the bedside.

✓Spinal hemangiomas can be categorized into three different groups based on location. Vertebral body (VB) heman-giomas are frequent incidental findings on magnetic resonance (MR) imaging. There is a subdivision of these with spinal epidural extension that have been reported in the literature. Spinal hemangiomas can also be epidural without VB involvement; these are extremely rare with few reported cases in the thoracic epidural spinal column. The diagnosis and imaging characteristics as well as the surgical tools used in gross-total resection of spinal epidural hemangioma are not well understood. The authors present a detailed characterization of a spinal epidural hemangioma in a 30-year-old woman who presented with complaints of gradual onset of low-back pain that worsened over 1 year. The MR imaging findings indicated a large L2–S1 epidural spinal mass causing thecal sac compression. The patient underwent an L2–S1 laminectomy, and a vascular extradural mass was noted on the posterior aspect of the dura mater. Preoperative spinal angiography as well as intraoperative angiography was performed. Total resection of the tumor was achieved using intraoperative embolization with sodium tetradecyl sulfate and microscopic dissection. The postoperative MR imaging findings and clinical outcome were excellent. The findings and use of sodium tetradecyl sulfate in gross-total resection are discussed. The authors also review treatment modalities and demonstrate the utility and effectiveness of intraoperative sodium tetradecyl sulfate in gross-total resection of large difficult spinal epidural hemangiomas.

✓The seat of consciousness has not always been thought to reside in the brain. Its “source” is as varied as the cultures of those who have sought it. At present, although most may agree that the central nervous system is held to be the root of individualism in much of Western philosophy, this has not always been the case, and this viewpoint is certainly not unanimously accepted across all cultures today. In this paper the authors undertook a literary review of ancient texts of both Eastern and Western societies as well as modern writings on the organic counterpart to the soul. The authors have studied both ancient Greek and Roman material as well as Islamic and Eastern philosophy. Several specific aspects of the human body have often been proposed as the seat of consciousness, not only in medical texts, but also within historical documents, poetry, legal proceedings, and religious literature. Among the most prominently proposed have been the heart and breath, favoring a cardiopulmonary seat of individualism. This understanding was by no means stagnant, but evolved over time, as did the role of the brain in the definition of what it means to be human. Even in the 21st century, no clear consensus exists between or within communities, scientific or otherwise, on the brain's capacity for making us who we are. Perhaps, by its nature, our consciousness—and our awareness of our surroundings and ourselves—is a function of what surrounds us, and must therefore change as the world changes and as we change.

OBJECT: The authors performed an analysis of retrospectively obtained data to compare the outcomes of pediatric patients admitted to their institution for traumatic injuries resulting from car, motorcycle, and all-terrain vehicle (ATV) accidents. METHODS: The authors conducted a retrospective review of Columbus Children's Hospital's Trauma Registry data collected between January 1993 and December 2003. Data obtained in patients admitted with motor vehicle-related injuries were compiled for a total of 1608 patient records. Data regarding sex, age, hospital length of stay (LOS), Glasgow Coma Scale (GCS) score, revised trauma score, injury severity score (ISS), and use of a protective device were analyzed. CONCLUSIONS: Of 1608 patients, 1257 (78%) were injured in automobile accidents, 123 (7.6%) in motorcycle accidents, and 228 (14.2%) in ATV accidents. Injuries sustained in all vehicle types peaked during the summer months. Patients involved in automobile crashes presented with significantly lower GCS scores than those injured in motorcycle and ATV accidents; however, there was no statistically significant difference in LOS among all three injury modalities. Protective devices were underutilized in all three motor vehicle categories but, when used, were associated with significantly higher GCS scores, ISSs, and shorter LOSs among patients admitted after automobile accidents. This paper demonstrates the potentiality of foraminifera in detecting mining pollution. Goa’s Mandovi–Zuari estuarine complex is most vulnerable to deterioration caused by the mining industry. The objective of the present study is to estimate the health of the Zuari through foraminiferal distribution in its surface sediments. The foraminiferal data generated was compared with the three‐decade‐old foraminiferal data collected in 1972 and total suspended matter (TSM) data over the years. There has been a substantial increase in the maximum total foraminiferal number (TFN), from 1143 specimens in 1972 to 3057 specimens per gram sand in 2003. Even the Total Species Number (TSN) has increased from 24 in 1972 to 50 in 2003. This is in compliance with the TSM data collected over the years, which has considerably decreased. Field experiments were conducted in 1994 and 1995 at Vegreville, Legal, and Lacombe, AB, to determine the effects of a preharvest application of glyphosate on seedling emergence and growth of field pea. Glyphosate was applied at 0.9 kg ai/ha at each of the three crop development stages, as determined by seed moisture content (SMC), to determinate ('Ascona' and 'Radley') and indeterminate ('Miko' and 'Trapper') cultivars. Applying glyphosate when the SMC was less than 30% had little to no effect on seedling emergence but reduced seedling shoot fresh weight in two of six experiments. Applying glyphosate at SMC above 40% reduced seedling emergence and shoot fresh weight in two and three of the six experiments, respectively. Reductions in seedling emergence and shoot fresh weight were greater from seeds collected from the top than from seeds collected from the bottom one-third of sprayed plants. Differences in response between determinate and in- determinate cultivars occurred, but there was no consistent trend. Given the variable maturity in most fields and on individual pea plants, applications of preharvest glyphosate to peas destined for seed production may decrease seed germination and biomass accumulation. Bone cement implantation syndrome (BCIS) is a well-described and potentially fatal complication of orthopaedic surgery involving pressurised bone cement. Although also described for certain spinal procedures, it is most commonly associated with cemented hip and knee arthroplasty and with cemented hemiarthroplasty following neck of femur fracture in particular. Donaldson et alproposed the definition of BCIS as a syndrome “characterized by hypoxia, hypotension or both and/or unexpected loss of consciousness occurring around the time of cementation, prosthesis insertion, reduction of the joint or, occasionally, limb tourniquet deflation in a patient undergoing cemented bone surgery”. Other features include increased vascular resistance, cardiac arrhythmias and cardiac arrest post cement use. We describe a case of a patient who suffered a catastrophic reaction to cement during surgery for a comminuted proximal femoral fracture.

The luminescent properties of europium (Eu)- and dysprosium (Dy)-co-doped K3 Ca2 (SO4 )3 Cl halosulfate phosphors were analyzed. This paper reports the photoluminescence (PL) properties of K3 Ca2 (SO4 )3 Cl microphosphor doped with Eu and Dy and synthesized using a cost-effective wet chemical method. The phosphors were characterized by X-ray diffraction and scanning electron microscopy. The CIE coordinates were calculated to display the color of the phosphor. PL emission of the prepared samples show peaks at 484 nm (blue), 575 nm (yellow), 594 nm (orange) and 617 nm (red). The emission color of the Eu,Dy-co-doped K3 Ca2 (SO4 )3 Cl halophosphor depends on the doping concentration and excitation wavelength. The addition of Eu in K3 Ca2 (SO4 )3 Cl:Dy greatly enhances the intensity of the blue and yellow peaks, which corresponds to the (4) F9/2 → (6) H15/2 and (4) F9/2 → (6) H13/2 transitions of Dy(3+) ions (under 351 nm excitation). The Eu(3+) /Dy(3+) co-doping also produces white light emission for 1 mol% of Eu(3+) , 1 mol% of Dy(3+) in the K3 Ca2 (SO4 )3 Cl lattice under 396 nm excitation, for which the calculated chromaticity coordinates are (0.35, 0.31). Thus, K3 Ca2 (SO4 )3 Cl co-doped with Eu/Dy is a suitable candidate for NUV based white light-emitting phosphors technology.

The effectiveness of preharvest applications of glyphosate on yellow toadflax was evaluated at five sites in Alberta from 1992 to 1994. At each site, glyphosate at 0.9 to 1.8 kg ae/ha was applied with or without nonionic surfactant and/or ammonium sulfate. Glyphosate at 2.7 kg/ha and glufosinate at 0.6 kg ai/ha were applied without additional adjuvants. The treatments were applied 7 to 10 d before crop harvest, when the majority of the yellow toadflax was in a flowering stage. Eleven to 12 mo after glyphosate application, yellow toadflax density was reduced by more than 80%. In most instances, there was no advantage in increasing the glyphosate rate above 0.9 kg/ha. The addition of nonionic surfactant and/or ammonium sulfate did not enhance glyphosate activity. Glufosinate did not control yellow toadflax in the year following treatment. Barley, canola, and flax yields in the year following applications were significantly higher in all preharvest glyphosate-treated plots than in untreated plots.

In Pakistan, dengue viral infection hasbecome hyper-endemic. Renal transplantation is also expanding. Weaimed to study dengue in renal transplant recipients (RTR).Methods. We conducted a study of RTR reported to be anti-dengueimmunoglobulin-M antibody positive from January 2009 toDecember 2010 at our institution in Karachi and follow their clinicalcourse and outcome.Results. Median age was 28 years; 75 (73.7%) were males. Clinicalpresentation included fever in 82 (80.4%), gastrointestinal symptomsin 35 (34.3%), hemorrhagic complications in 9 (8.8%), andthrombocytopenia in 97 (95%), which was of >15 days duration in24%. Fever was seen less frequently in patients on high-dose(>7.5 mg) steroids as compared with low-dose (≤7.5 mg) steroids.Forty-four patients (43%) had primary and 58 (56.8%) had secondarydengue infection. Dengue fever (DF) occurred in 90 (88%), anddengue hemorrhagic fever/dengue shock syndrome (DHF/DSS)occurred in 12 (11.7%). DHF/DSS was seen in 3 (6.8%) of those withprimary and in 9 (15.5%) of those with secondary infection(P < 0.22). In secondary infection, patients on cyclosporine-containing regimen had less severe disease, with DHF/DSS in 22%as opposed to DF in 59% (P < 0.04). Of 102 RTR, 68 (66.7%) had graftdysfunction, 5 of whom died. Of the remaining 63, in 54 patients(85.7%) creatinine returned to baseline by an average of 12.6 days.Of 102 patients, 95 (93%) recovered and 7 (6.9%) died, 6 of whom hadbacteremia with sepsis and 1 had respiratory failure. None died dueto dengue infection alone.Conclusion. In conclusion, in RTR without life-threatening co-morbidities, the clinical course of dengue infection is mild, withgood recovery and preserved renal function.A. Nasim

Objective: Anxiety towards dental procedures are common difficulties that may be experienced by dental patients all over the world. This study focused on evaluating the dental anxiety frequency and its relationship with age, gender, educational level, and past dental visits among patients attending the outpatient clinics of College of Dentistry, Al Jouf University, Saudi Arabia. Material and Methods: A total of 221 patients, aged 21–50 years were selected for the study. A questionnaire comprising the Modified Dental Anxiety Scale (MDAS) was used to measure the level of dental anxiety. Data was analyzed using SPSS version 20. Results: The mean anxiety score of the 221 patients was 11.39 (SD ± 2.7). Independent t-test showed a significant variation between the age groups with regards to their mean overall anxiety score (P < 0.05), which reduced with increasing age. A significant difference was found by independent t-test in the mean total score between male and female groups and regarding previous dental visit (P < 0.05). Regarding education level, there was no significant difference between the groups (P > 0.05). Conclusion: Younger patients, female, and patients with previous unpleasant dental experience were associated with increased MDAS score. Clinical Significance: The present study was done for better patient management and proper treatment plan development for dentally anxious patients.

Single-access laparoscopic surgery is a recent vogue in the field of minimally invasive colorectal surgery. While selected series have indicated feasibility, we prospectively examined its usefulness for resectional surgery in routine practice.All patients undergoing laparoscopic colorectal resection over a 12-month period were considered for a single-access approach by a single surgical team in a university hospital. This utilized a 'glove' port via a 3-5 cm periumbilical or stomal site incision, with standard rigid laparoscopic instruments then being used.Of 76 planned laparoscopic colorectal resections, 35 (47%) were performed by this single-incision laparoscopic modality without disruption of theatre list efficiency or surgical training obligations. The mean (range) age and body mass index of these 25 consecutive right-sided resections, eight total colectomies (seven urgent operations) and two anterior resections was 58 (22-82) years and 23.9 (18.6-36.2) kg/m(2) , respectively. The modal postoperative day of discharge was 4. For right-sided resections, the mean (range) postoperative stay in those undergoing surgery for benign disease was 4.0 days, while for those undergoing operation for neoplasia (n=18, mean age 71 years) it was 5.8 days and the average lymph node harvest was 13. Use of the glove port reduced trocar cost by 58% (€60/£53) by allowing the use of trocar sleeves alone without obturators.Single-incision laparoscopic surgery is an effective option for abdominal surgery and seems especially suited for laparoscopic-assisted right-sided colonic resections. The glove port technique facilitates procedural frequency and familiarity and proves economically favourable.

The effectiveness of single and repeated annual preharvest glyphosate applications (1 to 2 wk before crop harvest) to reduce Canada thistle populations was evaluated in field experiments near Bezanson, AB, Scott, SK, and St. Albert, AB. Single applications of glyphosate at 0.45 to 1.8 kg/ha at Bezanson and St. Albert reduced Canada thistle shoot density by more than 75% but in the three experiments at Scott 1.8 kg/ha was required to consistently obtain a similar reduction. Four consecutive, annual applications of glyphosate at 0.45 kg/ha or more did not eliminate Canada thistle but reduced shoot densities by more than 98% at Bezanson. At Scott, similar reductions occurred in two of three sites following two consecutive, annual preharvest applications of glyphosate at the same rates. In the third Scott site the 1.8 kg/ha rate of glyphosate provided a similar reduction but lower rates did not. At Bezanson, barley yields following the first, second, and third applications of glyphosate at 0.45 kg/ha or more were 14% greater than check plots where MCPA was applied in the spring. Yields of wheat seeded 1 yr after a single application of glyphosate at St. Albert were 26% greater than from unsprayed checks. Barley yields seeded 1 yr after each glyphosate application at Scott were not affected because Canada thistle populations were sparse.

In response to farmer complaints of poor triallate performance, wild oat seed was collected from 34 fields in Alberta in the fall of 1990. Screening trials in the greenhouse indicated that 15 of the populations were highly resistant to triallate applied at the equivalent of the recommended field rate (1.7 kg ha-1), whereas the other 19 populations were adequately controlled. All triallate-resistant populations were also highly resistant to difenzoquat applied at 1.7 kg ha-1(equivalent to twice the recommended field rate). The effect of increasing rates of both herbicides on dry weight of five of the resistant and two of the susceptible populations was determined in greenhouse experiments. Triallate applied up to 3.4 kg ha-1had little or no effect on the resistant populations, whereas the susceptible populations were controlled at 1.7 kg ha-1. At rates of 6.8 kg ha-1or higher, there were differences among the resistant populations and among individuals within the populations in the response to triallate. Response of the resistant populations to increasing difenzoquat rates was variable between experiments, but in all cases the effect of difenzoquat on wild oat dry weight was considerably less in triallate-resistant than triallate-susceptible populations. Effects of increasing rates of triallate and difenzoquat on resistant and susceptible wild oat populations growing with barley in field experiments were generally similar to the responses in the greenhouse.

The aim of this study was to search for Bacillus thuringiensis (Bt) harbouring cry1A gene which could effectively control cotton pest, American bollworm, Helicoverpa armigera. cry gene profiling of 50 Bt isolates showed the presence of cry1, cry2, cry3, cry4, cry7, cry8 and cry9 genes. None of the isolates harboured cry1 gene alone. It was always found in combination with cry3. There was no isolate positive for cry10 gene. Considering isolates with single cry genes, the frequency of cry4 was predominant (22%) followed cry2 (6%), cry3 (4%) and cry8 (2%). Isolates having two cry genes in combination had 14% incidence for cry2 + cry4, 12% for cry3 + cry4 and 10% for cry1 + cry3. The most dominant three gene linkage was cry1 + cry3 + cry4. Further profiling of cry1 gene showed that cry1K gene was abundantly present in all combinations such as cry1A, cry1D, cry1F and cry1I. However, cry1C existed independent of other subtypes. Finally, the Bt isolates with cry1A were analyzed for 16S rRNA gene, which showed two distinct groups of isolates on the basis of sequence homology. Bioassays of spore-crystal mixtures of SBS-Bt4, 8, 17, 21 and 26 harbouring cry1 against neonate larvae of H. armigera showed LC(50) 1288, 1202, 467·7, 524·8 and 108·5 μg ml(-1) . The SBS-Bt26 showed fourfold higher toxicity than the cry 1Ac harbouring positive control, HD-73. None of the isolates harboured single cry 1 gene. They were always in combination of two or three genes. A Bt isolate (Bt26) had fourfold higher toxicity against H. armigera larvae compared with the positive control HD 73 and hence can be commercially exploited to control insect pest. The inter relationship between the cry genes content and the toxicity may allow better understanding of Bt ecology.

In experimental plots, the proportion of resistant (R) to susceptible (S) wild oat (Avena fatua L.) increased between 1992 and 1996 and was highest in continuous wheat (2.4:1) and lowest in continuous barley (1.2:1). Conversely, the relative frequency of R plants decreased in most farmers' fields between 1990 and 1997 and varied from 12 to 100%. Frequency of resistance was positively correlated with the number of triallate or difenzoquat applications since 1988. Sphenopalatine neuralgia, or Sluder’s neuralgia, refers to a consistent clustering of clinical symptoms: intermittent episodes of vasomotor hyperactivity causing conjuctival injection, lacrimation, serous nasal discharge and unilateral nasal mucosal inflammation, sensory disturbances of the palate and oropharynx with distorted gustatory sensations, and lancing, unilateral pain most often located in the area of the inferomedial orbit and nasal base or at the region of the mastoid process. This particular clinical entity has also proven difficult to manage effectively, especially when not clearly secondary to other medical conditions such as paranasal sinus infection or bony nasal deformities. This condition has been treated with success using Gamma Knife radiosurgery in at least 1 other case reported in the literature. We present a second patient whose sphenopalatine neuralgia was treated successfully with stereotactic radiosurgery and discuss the possibilities of this modality as an option for patients with a refractory condition.

Polarized IR spectra of partially oriented benzimidazole crystals grown from the melt pressed between salt plates were recorded. The depolarization ratios of the Raman bands in the melt and the polarized Raman spectra of a single crystal in all possible orientations were measured. ✓In recent studies spinal surgery has replaced cardiac surgery as a leading cause of postoperative vision loss (POVL). Estimates of the incidence of POVL after spinal surgery range from 0.028 to 0.2%, but with advances in complex spinal instrumentation and the rise in annual spinal operations, POVL may see an ominous increase in its incidence. Postoperative vision loss is an uncommon but devastating complication, with unknown origin and pathogenesis. The authors undertook a literature review and summarize the current understanding of its pathophysiology, highlight the limitations of existing knowledge, and recommend practical guidelines for avoiding this devastating outcome.

We will discuss a potential role of percutaneous vertebroplasty (PVP) in the management of patients with severe fibrous dysplasia of the spine with multiple cervical lesions and C2–C3 pathologic fractures that may not be a good surgical candidate. Polyostotic fibrous dysplasia involvement of the cervical spine is rare. Review of literature indicates only few reported cases of surgical management with one case of mortality indicating increased risks associated with surgical intervention. While PVP is commonly used for the treatment of osteoporotic thoracolumbar vertebral compression fractures, its role in vertebral stabilization for fibrous dysplasia has not been reported. A 35-year-old man with McCune–Albright syndrome and severe polyostotic fibrous dysplasia of C2 and C3 vertebrae presented with severe neck pain, radiculopathy, quadriparesis and myelopathy. The lesion had pathologic fractures, and there was an os odontoideum with cervical cord atrophy at the C1 level. After discussing need for aggressive surgical management and potential complications, we offered PVP due to surgical risks involved. PVP was performed with a posterolateral transpedicular approach without complication. The patient had remarkable improvement in clinical relief of neck pain and improvement of myelopathic symptoms at 1-year follow-up. We present a case that illustrates a potential use of PVP in the management of a patient with symptomatic spinal fibrous dysplasia with associated pathologic fractures who was poor surgical candidate.

Introduction: Laparoscopic appendicectomy (LA) is the most commonly performed surgical emergency procedure. The aim of this study was to highlight a series of iatrogenic bladder injuries during LA and suggest a simple method of prevention.Methods: A retrospective review was carried out of all LA performed in a university teaching hospital over a two year period 2012-2013. Iatrogenic visceral injuries were identified and operative notes examined.Results: During the study period 1124 appendicectomies were performed. Four iatrogenic bladder injuries occurred related to secondary trocar insertion. No patient was catheterised preoperatively. One of the injuries was identified intra-operatively, another in the early postoperative period where as two re-presented acutely unwell post-discharge from hospital. Three were repaired by laparotomy and one laparoscopically.Conclusion: Iatrogenic secondary trocar induced bladder injuries are a rare but preventable and potentially serious complication of LA. Urethral catheterisation during LA is a safe and simple method which can prevent this complication.

Thermal barrier coatings (TBCs) are employed to protect hot section components in industrial and aerospace gas turbine engines. Conventional TBCs frequently fail due to high residual stresses and difference between coefficient of thermal expansion (CTE) of the substrate & coatings. Functionally graded thermal barrier coatings (FG-TBCs) with gradual variation in composition have been proposed to minimize the problem. In this work, a five layered functionally graded thermal barrier coating system was deposited by atmospheric plasma spray (APS) technique on Nimonic 90 substrates using Ni-5Al as bond coat (BC) and magnesia stabilized zirconia as top coat (TC). The coatings were characterized by SEM, EDS, XRD & optical profilometer. Microhardness and coefficient of thermal expansion of the five layers deposited as individual coatings were also measured. The deposited coating system was oxidized at 800°C. SEM analysis showed that five layers were successfully deposited by APS to produce a FG-TBC. The results also showed that roughness (Ra) of the individual layers decreased with an increase in TC content in the coatings. It was found that microhardness and CTE values gradually changed from bond coat to cermet layers to top coat. The oxidized coated sample revealed parabolic behavior and changes in the surface morphology and composition of coating.

Recently there has been a great interest in the role of Helicobacter pylori in gastroesophageal reflux disease. Many studies do not show any significant difference in the overall prevalence of H. pylori in patients with endoscopic oesophagitis and controls. In this prospective study we assessed the influence of H. pylori density and activity of inflammation in different parts of stomach on histological oesophagitis.One Hundred and forty consecutive patients undergoing endoscopy for dyspepsia and heartburn were evaluated. Three biopsies were taken from antrum and two each from corpus, cardia and lower oesophagus. Urease test (CLO test) was performed. Density and activity of infection was assessed in a semi-quantitative way.One Hundred and Fourteen (81%) patients from the 140 endoscoped, were positive for H. pylori and had H. pylori positive antral gastritis. Of these 114 cases, H. pylori were detectable in 104 (91%) of biopsies taken from corpus and 96 (84%) of biopsies from cardia. There was a strong correlation of density of H. pylori (0-3) in antrum with body and of body with cardia by Spearman correlation tests (p = 0.000). But H. pylori were more dense in antrum as compared to corpus and in corpus as compared to cardia (p = 0.0000 and 0.0003 respectively by Wilcoxon's rank test). Neutrophil activity and degree of mononuclear infiltrate were also greater in antrum as compared to corpus (p = 0.000 and 0.059). The activity and degree of inflammation was not significantly different in corpus-cardia pair. Out of 114 H. pylori positive patients, 75 had histological oesophogitis (p = 0.855). After excluding cases of hiatal hernia (H.H) and gapping lower oesophageal sphincter (LOS), number of H. pylori positive patients decreased to 73, out of these 50 had histological oesophagitis (p = 0.103). In all H. pylori positive patients with histological oesophagitis, H. pylori density (1-3) in antrum correlated with severity of oesophagitis (P = 0.011). Neutrophil activity in antrum and corpus also correlated with the severity of histological oesophagitis (P = 0.024 and 0.035 respectively). Correlation further improved after excluding cases of HH and gapping LOS (P = 0.002 for H. pylori density and 0.026 and 0.004 for activity in antrum and corpus). No correlation could be found of density and activity of infection in cardia with histological oesophagitis.Our H. pylori positive patients had more dense and severe infection in antrum. Those who had histological oesophagitis in addition showed a positive correlation of the density of H. pylori in antrum and neutrophil activity in antrum and corpus with the severity of histological oesophagitis.

In this study, retrospective data analysis was performed to analyze the utility of head computed tomography (CT) scanning in the diagnosis of Chiari malformation Type I (CM-I) in the pediatric population.The authors conducted a retrospective review of radiology charts describing head CT results obtained at Columbus Children's Hospital between January 2004 and January 2005. The records were searched for the key words "Chiari," "cerebellar ectopy," or "tonsillar ectopy." The exclusion criteria included patients with previously known Chiari malformation Type I or Type II or those who had undergone follow-up magnetic resonance (MR) imaging at other institutions. Head CT and MR images for the remaining patients were reviewed to verify accuracy.Of the 72 patients with suspicious findings of tonsillar ectopy on CT, only 37 (51.4%) had MR imaging findings consistent with CM-I. The tonsillar ectopy in these patients ranged from 3 mm to 17 mm below the foramen magnum.The authors' findings indicate that incidental standard CT scans of the head have limited value in identifying CM-I.

We report the synthesis, characterization, and device incorporation of copolymers based on a common green-emitting polyfluorene but containing a small proportion of a low energy gap donor-acceptor-donor unit for red emission in photo- and electro-luminescence. At just 1%–3% random incorporation, the low-gap unit is not present on all chains, yet we demonstrate that efficient charge and energy transfer can yield electroluminescent devices with 1% quantum efficiency and a color that can be tuned by adjusting the density of low-gap units to achieve primary red (National Television System Committee). The high current density tail off in the efficiency is reduced by replacing the hole-injection layer with a photochemically cross-linked electron‑blocking layer.

Postoperative visual loss (POVL) after spine surgery performed with the patient prone is a rare but devastating postoperative complication. The incidence and the mechanisms of visual loss after surgery are difficult to determine. The 4 recognized causes of POVL are ischemic optic neuropathy (approximately 89%), central retinal artery occlusion (approximately 11%), cortical infarction, and external ocular injury. There are very limited guidelines or protocols on the perioperative practice for “prone-position” surgeries. However, new devices have been designed to prevent mechanical ocular compression during prone-position spine surgeries. The authors used PubMed to perform a literature search for devices used in prone-position spine surgeries. A total of 7 devices was found; the authors explored these devices' features, advantages, and disadvantages. The cause of POVL seems to be a multifactorial problem with unclear pathophysiological mechanisms. Therefore, ocular compression is a critical factor, and eliminating any obvious compression to the eye with these devices could possibly prevent this devastating perioperative complication.

High-altitude coal waste areas are usually difficult to revegetate. However, naturally occurring plant species may be adapted to the extremely harsh substrate of these waste areas. Ten coal waste areas (spoils) of varying ages in the Rocky Mountains in Alberta were analyzed for physical and chemical properties and vegetation composition. Older spoils have a reduced fraction of coarse material, a higher clay content, a greater moisture retention capacity at -1.5 MPa, and higher levels of available N, P, K, and S, as compared to younger spoils, indicating physical and chemical weathering of the spoil rock minerals at the older sites. Natural revegetation of the coal spoils is sparse and patchy with low plant cover and this fact may be attributed to poor moisture and nutrient availability. Colonization by certain species is effective because of their low growth-form, efficient vegetative reproduction mechanism, low nutrient requirements, and/or N-fixing ability. A total of 59 frequently occurring native plant species on coal waste areas were evaluated for their potential in revegetation. A majority of these species seem to have excellent potential for revegetation of high-altitude coal mine disturbed areas in the Rocky Mountains of Alberta.

The control of quackgrass (Agropyron repens (L.) Beauv.) using glyphosate applied with either a conventional sprayer or a rope wick applicator, each followed by several levels of tillage, was studied in 1983, 1984 and 1985. With the conventional sprayer, glyphosate was either applied alone or with ammonium sulfate (3.4 kg ha−1) and the surfactant, nonylphenoxy polyethoxy ethanol (0.5% vol/vol of spray volume). Applications made with the sprayer were more effective in reducing quackgrass culm density than those made with the rope wick applicator. The density of quackgrass culms declined as the number of tillage operations increased from one, performed at 115 or more days after herbicide application (DAT), to three performed at 5–8, 39–60 and 115 or more DAT. Doubling the rate of sprayer-applied glyphosate, from 0.8 to 1.7 kg acid equivalent ha−1, did not result in a reduction in the density of quackgrass culms. However, in 1 of the 2 years in which comparisons were made, both of these treatments reduced the density of quackgrass culms more than a similar application of glyphosate alone at 0.5 kg ha−1. The addition of ammonium sulfate and surfactant to glyphosate enhanced quackgrass control only at the 0.5 kg ha−1 rate and only in 1 of 2 yr at that rate. Glyphosate, applied at 0.5 kg ha−1 with the sprayer, and followed by three tillage operations consistently reduced the density of quackgrass culms by 95% or more. Both the herbicide and the tillage treatments resulted in increases in the yield of barley seeded the following year. In 2 of the 3 years, barley yields, averaged across tillage treatments, were greater in the plots where the sprayer had been used to apply the glyphosate treatments than in the plots where the rope wick applicator had been used. Barley yields decreased as the quackgrass culm density increased, although other factors seemed to have influenced the yields in one of the years.Key words: Glyphosate, tillage, barley, quackgrass, fallow
