- Abstract
Purpose: To assess the impact of the duration and intensity of episodes of increased intracranial pressure on 6-month neurological outcome in adult and paediatric traumatic brain injury. Methods: Analysis of prospectively collected minute-by-minute intracranial pressure and mean arterial blood pressure data of 261 adult and 99 paediatric traumatic brain injury patients from multiple European centres. The relationship of episodes of elevated intracranial pressure (defined as a pressure above a certain threshold during a certain time) with 6-month Glasgow Outcome Scale was visualized in a colour-coded plot. Results: The colour-coded plot illustrates the intuitive concept that episodes of higher intracranial pressure can only be tolerated for shorter durations: the curve that delineates the duration and intensity of those intracranial pressure episodes associated with worse outcome is an approximately exponential decay curve. In children, the curve resembles that of adults, but the delineation between episodes associated with worse outcome occurs at lower intracranial pressure thresholds. Intracranial pressures above 20 mmHg lasting longer than 37 min in adults, and longer than 8 min in children, are associated with worse outcomes. In a multivariate model, together with known baseline risk factors for outcome in severe traumatic brain injury, the cumulative intracranial pressure–time burden is independently associated with mortality. When cerebrovascular autoregulation, assessed with the low-frequency autoregulation index, is impaired, the ability to tolerate elevated intracranial pressures is reduced. When the cerebral perfusion pressure is below 50 mmHg, all intracranial pressure insults, regardless of duration, are associated with worse outcome. Conclusions: The intracranial pressure–time burden associated with worse outcome is visualised in a colour-coded plot. Introduction: \ud We have aimed to describe haemodynamic changes when haemodialysis is instituted in the critically ill. 3 \ud hypotheses are tested: 1)The initial session is associated with cardiovascular instability, 2)The initial session is \ud associated with more cardiovascular instability compared to subsequent sessions, and 3)Looking at unstable \ud sessions alone, there will be a greater proportion of potentially harmful changes in the initial sessions compared \ud to subsequent ones. \ud Methods: \ud Data was collected for 209 patients, identifying 1605 dialysis sessions. Analysis was performed on hourly \ud records, classifying sessions as stable/unstable by a cutoff of >+/-20% change in baseline physiology \ud (HR/MAP). Data from 3 hours prior, and 4 hours after dialysis was included, and average and minimum values \ud derived. 3 time comparisons were made (pre-HD:during, during HD:post, pre-HD:post). Initial sessions were \ud analysed separately from subsequent sessions to derive 2 groups. If a session was identified as being unstable, \ud then the nature of instability was examined by recording whether changes crossed defined physiological ranges. \ud The changes seen in unstable sessions could be described as to their effects: being harmful/potentially harmful, \ud or beneficial/potentially beneficial. \ud Results: \ud Discarding incomplete data, 181 initial and 1382 subsequent sessions were analysed. A session was deemed to \ud be stable if there was no significant change (>+/-20%) in the time-averaged or minimum MAP/HR across time \ud comparisons. By this definition 85/181 initial sessions were unstable (47%, 95% CI SEM 39.8-54.2). Therefore \ud Hypothesis 1 is accepted. This compares to 44% of subsequent sessions (95% CI 41.1-46.3). Comparing these \ud proportions and their respective CI gives a 95% CI for the standard error of the difference of -4% to 10%. \ud Therefore Hypothesis 2 is rejected. In initial sessions there were 92/1020 harmful changes. This gives a \ud proportion of 9.0% (95% CI SEM 7.4-10.9). In the subsequent sessions there were 712/7248 harmful changes. \ud This gives a proportion of 9.8% (95% CI SEM 9.1-10.5). Comparing the two unpaired proportions gives a \ud difference of -0.08% with a 95% CI of the SE of the difference of -2.5 to +1.2. Hypothesis 3 is rejected. Fisher’s \ud exact test gives a result of p=0.68, reinforcing the lack of significant variance. \ud Conclusions: \ud Our results reject the claims that using haemodialysis is an inherently unstable choice of therapy. Although \ud proportionally more of the initial sessions are classed as unstable, the majority of MAP and HR changes are \ud beneficial in nature.

Five single nucleotide polymorphisms (SNPs) associated with thyroid cancer (TC) risk have been reported: rs2910164 (5q24); rs6983267 (8q24); rs965513 and rs1867277 (9q22); and rs944289 (14q13). Most of these associations have not been replicated in independent populations and the combined effects of the SNPs on risk have not been examined. This study genotyped the five TC SNPs in 781 patients recruited through the TCUKIN study. Genotype data from 6122 controls were obtained from the CORGI and Wellcome Trust Case-Control Consortium studies. Significant associations were detected between TC and rs965513A (p=6.35×10(-34)), rs1867277A (p=5.90×10(-24)), rs944289T (p=6.95×10(-7)), and rs6983267G (p=0.016). rs6983267 was most strongly associated under a recessive model (P(GG vs GT + TT)=0.004), in contrast to the association of this SNP with other cancer types. However, no evidence was found of an association between rs2910164 and disease under any risk model (p>0.7). The rs1867277 association remained significant (p=0.008) after accounting for genotypes at the nearby rs965513 (p=2.3×10(-13)) and these SNPs did not tag a single high risk haplotype. The four validated TC SNPs accounted for a relatively large proportion (∼11%) of the sibling relative risk of TC, principally owing to the large effect size of rs965513 (OR 1.74).

Two of the authors (van Benthem and Pacuit) recently introduced evidence logic as a way to model epistemic agents faced with possibly contradictory evidence from different sources. For this the authors used neighborhood semantics, where a neighborhood N indicates that the agent has reason to believe that the true state of the world lies in N. A normal belief modality is defined in terms of the neighborhood structure. In this paper we consider four variants of evidence logic which hold for different classes of evidence models. For each of these logics we give a representation theorem using extended evidence models, where the belief operator is replaced by a standard relational modality. With this, we axiomatize all four logics, and determine whether each has the finite model property.

As McKinsey and Tarski [20] showed, the Stone representation theorem for Boolean algebras extends to algebras with operators to give topological semantics for (classical) propositional modal logic, in which the "necessity" operation is modeled by taking the interior of an arbitrary subset of a topological space. This topological interpretation was recently extended in a natural way to arbitrary theories of full first-order logic by Awodey and Kishida [3], using topological sheaves to interpret domains of quantification. This paper proves the system of full first-order S4 modal logic to be deductively complete with respect to such extended topological semantics. The techniques employed are related to recent work in topos theory, but are new to systems of modal logic. They are general enough to also apply to other modal systems.

This paper discusses the range of knowledge acquisition, including machine learning, approaches used to develop knowledge bases for Intelligent Systems. Specifically, this paper focuses on developing techniques which enable an expert to detect inconsistencies in 2 (or more) perspectives that the expert might have on the same (classification) task. Further, the INSIGHT system has been developed to provide a tool which supports domain experts exploring, and removing, the inconsistencies in their conceptualization of a task. We report here a study of Intensive Care physicians reconciling 2 perspectives on their patients. The high level task which the physicians had set themselves was to classify, on a 5 point scale (A-E), the hourly reports produced by the Unit’s patient management system. The 2 perspectives provided to INSIGHT were an annotated set of patient records where the expert had selected the appropriate category to describe that snapshot of the patient, and a set of rules which are able to classify the various time points on the same 5-point scale.\ud Inconsistencies between these 2 perspectives are displayed as a confusion matrix; moreover INSIGHT then allows the expert to revise both the annotated datasets (correcting data errors, and/or changing the assigned categories) and the actual rule-set. Each expert achieved a very high degree of consensus between his refined knowledge sources (i.e., annotated hourly patient records and the rule-set).\ud Further, the consensus between the 2 experts was ~95%. The paper concludes by outlining some of the follow-up studies planned with both INSIGHT and this general approach.

Refinements generated for a knowledge base often involve the learning of new knowledge to be added to or replace existing parts of a knowledge base. However, the justifiability of the refinement in the context of the domain (domain acceptability) is often overlooked. The work reported in this paper describes an approach to the generation of domain acceptable refinements for incomplete and incorrect ontology individuals through reasoning by analogy using existing domain knowledge. To illustrate this approach, individuals for refinement are identified during the application of a knowledge-based system, EIRA; when EIRA fails in its task, areas of its domain ontology are identified as requiring refinement. Refinements are subsequently generated by identifying and reasoning with similar individuals from the domain ontology. To evaluate this approach EIRA has been applied to the Intensive Care Unit (ICU) domain. An evaluation (by a domain expert) of the refinements generated by EIRA has indicated that this approach successfully produces domain acceptable refinements.

Background Laboratory studies suggest that nitrous oxide augments brain injury after ischemia or hypoxia. The authors examined the relation between nitrous oxide use and outcomes using data from the Intraoperative Hypothermia for Aneurysm Surgery Trial. Methods The Intraoperative Hypothermia for Aneurysm Surgery Trial was a prospective randomized study of the impact of intraoperative hypothermia (temperature = 33 degrees C) versus normothermia (temperature = 36.5 degrees C) in patients with aneurysmal subarachnoid hemorrhage undergoing surgical clipping. Anesthesia was dictated by a limited-options protocol with the use of nitrous oxide determined by individual anesthesiologists. All patients were assessed daily for 14 days after surgery or until hospital discharge. Neurologic and neuropsychological testing were conducted at 3 months after surgery. Outcome data were analyzed via both univariate tests and multivariate logistic regression analysis correcting for factors thought to influence outcome. An odds ratio (OR) greater than 1.0 denotes a worse outcome in patients receiving nitrous oxide. Results Outcome data were available for 1,000 patients, of which 373 received nitrous oxide. There was no difference between groups in the development of delayed ischemic neurologic deficit. At 3 months after surgery, there were no significant differences between groups in any outcome variable: Glasgow Outcome Score (OR, 0.84; 95% confidence interval [CI], 0.63-1.14; P = 0.268), National Institutes of Health Stroke Scale (OR, 1.29; 95% CI, 0.96-1.73; P = 0.087), Rankin Disability Score (OR, 0.84; 95% CI, 0.61-1.15; P = 0.284), Barthel Activities of Daily Living Index (OR, 1.01; 95% CI, 0.68-1.51; P = 0.961), or neuropsychological testing (OR, 1.26; 95% CI, 0.85-1.87; P = 0.252). Conclusions In a population of patients at risk for ischemic brain injury, nitrous oxide use had no overall beneficial or detrimental impact on neurologic or neuropsychological outcomes.

BACKGROUND: Surgery for intracranial aneurysm often results in postoperative neurologic deficits. We conducted a randomized trial at 30 centers to determine whether intraoperative cooling during open craniotomy would improve the outcome among patients with acute aneurysmal subarachnoid hemorrhage.METHODS: A total of 1001 patients with a preoperative World Federation of Neurological Surgeons score of I, II, or III ("good-grade patients"), who had had a subarachnoid hemorrhage no more than 14 days before planned surgical aneurysm clipping, were randomly assigned to intraoperative hypothermia (target temperature, 33 degrees C, with the use of surface cooling techniques) or normothermia (target temperature, 36.5 degrees C). Patients were followed closely postoperatively and examined approximately 90 days after surgery, at which time a Glasgow Outcome Score was assigned.RESULTS: There were no significant differences between the group assigned to intraoperative hypothermia and the group assigned to normothermia in the duration of stay in the intensive care unit, the total length of hospitalization, the rates of death at follow-up (6 percent in both groups), or the destination at discharge (home or another hospital, among surviving patients). At the final follow-up, 329 of 499 patients in the hypothermia group had a Glasgow Outcome Score of 1 (good outcome), as compared with 314 of 501 patients in the normothermia group (66 percent vs. 63 percent; odds ratio, 1.14; 95 percent confidence interval, 0.88 to 1.48; P=0.32). Postoperative bacteremia was more common in the hypothermia group than in the normothermia group (5 percent vs. 3 percent, P=0.05).CONCLUSIONS: Intraoperative hypothermia did not improve the neurologic outcome after craniotomy among good-grade patients with aneurysmal subarachnoid hemorrhage.

Chronically culturing HIT-T15 cells in media containing high glucose concentrations leads to decreased insulin mRNA levels, insulin content, and insulin secretion. These changes can be prevented by culturing the cells in media containing lower glucose levels (Robertson, R. P., H.-J. Zhang, K. L. Pyzdrowski, and T. F. Walseth. 1992. J. Clin. Invest. 90:320-325). The mechanism of this seemingly paradoxical phenomenon was examined by transiently transfecting HIT cells with a chloramphenicol acetyl transferase (CAT) reporter gene controlled by the 5'-regulatory domain of the human insulin gene (INSCAT). Early passages of HIT cells readily expressed INSCAT, whereas late passages of cells chronically cultured in 11.1 mM glucose expressed only 28.7 +/- 2.3% (mean +/- SEM) of the CAT activity expressed in early passages. In contrast, late passages of HIT cells chronically cultured in 0.8 mM glucose retained the ability to express the INSCAT reporter gene to 69.6 +/- 10.0% of the CAT activity observed in early passages. The decrease in INSCAT expression in late passages of cells serially cultured in 11.1 mM glucose was associated with the inability to form a specific nuclear protein-DNA complex with the CT motifs of the human insulin promoter. Formation of this specific protein-DNA complex was preserved in late passages of HIT cells when serially cultured in 0.8 mM glucose. Mutations of the CT motifs caused markedly diminished CAT activity in all passages examined. These data indicate that chronic exposure of the beta cell to high glucose concentrations can paradoxically decrease insulin gene transcription, in part, by altering the ability of a regulatory protein (GSTF) to interact with the insulin gene promoter. This provides a potential mechanism for glucotoxic effects on the beta cell at the level of the insulin gene.

The Space Shuttle Main Engine (SSME) High Pressure Fuel Turbopump (HPFTP) blades are currently manufactured using a directionally solidified (DS) material, MAR-M-246+Hf. However, a necessity to reduce the occurrence of fatigue cracking within the DS blades has lead to an interest in the use of a single crystal (SC) material, PWA-1480. A study was initiated to determine the dynamic characteristics of the HPFTP blades made of SC material and find possible critical engine order excitations. This study examined both the first and second stage drive turbine blades of the HPFTP. The dynamic characterization was done analytically as well as experimentally. The analytical study examined the SC first stage HPFTP blade dynamic characteristics under typical operating conditions. The blades were analyzed using MSC/NASTRAN and a finite element model. Two operating conditions, 27500 RPM and 35000 RPM, were investigated.
