Abstract

A inserção geográfica do Algarve, no extremo sul do território nacional, moldou a mentalidade das suas gentes e corroeu as barreiras sociais, suavizadas por um cosmopolitismo emergente do trato mercantil. Contudo, não permitiu que se emancipasse da sua inexorável condição periférica de reino integrado. O Algarve sempre foi encarado pelo poder central como uma região extremada, mas estratégica na conservação da unidade nacional. Perdê-lo seria abrir as portas à temível anexação ibérica. Na sua identidade patriótica, os algarvios sempre se esforçaram por permanecerem no seio da nação. Nos momentos cruciais da nossa história, nunca deixaram que se pusesse em causa o seu patriotismo, nem invocaram ou alimentaram qualquer pretensão separatista. E foi esse orgulho, associado ao seu apreço pela liberdade, que lhe deu alento e protagonismo no contexto político-militar da primeira metade do século XIX. O relacionamento mercantil com os empresários britânicos, as tentaculares influências maçónicas e as várias praças de guerra sediadas no Algarve, colocaram este pequeno e esquecido reino em sintonia com os tumultos sociais e pronunciamentos militares que despontaram na cidade do Porto. Tornou-se sui generis essa participação do Algarve ao lado da cidade Invicta em todo o século XIX. Abstract

Introdução: Em 1950 Hanhart descreveu três casos de aglossia e deformidades dos membros. A associação de malformações oromandibulares e dos membros é rara, verificando-se grande variabilidade fenotípica entre os casos descritos. Caso Clínico: Gravidez com diagnóstico pré-natal ecográfico (24 semanas) de agenesia do pé direito, ausência do primeiro dedo do pé esquerdo e presença apenas da primeira falange do primeiro e quinto dedos da mão esquerda. Às 38 semanas de gestação nasce um recém-nascido, sexo masculino, com hipotonia generalizada e dificuldade respiratória com necessidade de manobras de reanimação. Ao exame físico destacava-se a presença de microretrognatia, microstomia e várias malformações ósseas a nível das mãos e pés. Desde as primeiras horas de vida que apresentou quadro de apneias com dessaturações recorrentes vindo a falecer na decorrência de um desses episódios. Discussão: A associação de microstomia, microretrognatia, hipoglossia, malformações dos membros e atingimento dos pares cranianos, permite -nos estabelecer o diagnóstico clínico de Síndrome de Hanhart. O exame histopatológico permite o diagnóstico definitivo, com classificação definitiva e exata, numa percentagem elevada de casos, contudo o exame citológico por ser menos oneroso e rápido é muito adotado na prática clínica diária nos Centros de Atendimento Médico Veterinários, estando descrito como mais indicado em neoplasias sólidas, de origem epitelial, com estroma escasso, linfadenopatias e menos nas neoplasias mesenquimatosas, com estroma abundante, menos propícias à aspiração. Os autores pretendem apresentar o exame microscópico de três preparações citológicas, coradas com Diff-Quik, com o objetivo de fomentar a discussão e determinar o seu diagnóstico definitivo. 75. Norovirus gastroenteritis outbreaks in military units: a systematic review

Abstract

IntroductionNorovirus gastroenteritis is one of the most frequent causes of personnel unavailability in military units, being associated with significant morbidity and degradation of their operational effectiveness. The disease is usually mild but can be severe and life-threatening in young and healthy soldiers, who are prone to dehydration due to intensive daily activity. Despite its impact, the full extent of the norovirus gastroenteritis burden in military forces remains unclear. This systematic review aims to evaluate the impact and ascertain clinical and epidemiological features of norovirus outbreaks that have occurred in the military forces.MethodsThe systematic review followed the Preferred Reporting Items for Systemic Reviews and Meta-Analysis (PRISMA) guidelines and used three databases: PubMed, Scopus, and LILACs. Papers published up to 1 September 2019 were included without restrictions if they reported one or more outbreaks in the military forces on active duty, either on national territories or deployed overseas.ResultsA total of 343 papers were retrieved from the literature search. After inclusion/exclusion criteria a total of 39 eligible papers were considered. From 1988 (first reported outbreak in the military) to 2018 more than 101 norovirus outbreaks have been reported in the military, accounting for at least 24 332 cases. Secondary transmission was emphasised as the main route of norovirus transmission in the military forces, with eating outside the military setting an important route for the primary cases.ConclusionsThe present review highlights that norovirus gastroenteritis has been a burden to military troops both in combat and on peacekeeping operations. Norovirus disease has been shown to exact a substantial toll on mission readiness and operational effectiveness. It is noteworthy that the impact of norovirus outbreaks among military units is underestimated because the literature review retrieved information from the armed forces from only nine countries.

76. Simultaneous norovirus outbreak in three Portuguese army bases in the Lisbon region, December 2017

Abstract

IntroductionNorovirus outbreaks frequently occur in communities and institutional settings acquiring a particular significance in armed forces where prompt reporting is critical. Here we describe the epidemiological, clinical and laboratorial investigation of a multicentre gastroenteritis outbreak that was detected simultaneously in three Portuguese army units with a common food supplier, Lisbon region, between 5 and 6 December 2017.MethodsQuestionnaires were distributed to all soldiers stationed in the three affected army units, and stool specimens were collected from soldiers with acute gastrointestinal illness. Stool specimens were tested for common enteropathogenic bacteria by standard methods and screened for a panel of enteric viruses using a multiplex real-time PCR assay. Food samples were also collected for microbiological analysis. Positive stool specimens for norovirus were further genotyped.ResultsThe three simultaneous acute gastroenteritis outbreaks affected a 31 (3.5%) soldiers from a total of 874 stationed at the three units and lasted for 2 days. No secondary cases were reported. Stool specimens (N=11) were negative for all studied enteropathogenic agents but tested positive for norovirus. The recombinant norovirus GII.P16-GII.4 Sydney was identified in all positive samples with 100% identity.ConclusionsThe results are suggestive of a common source of infection plausibly related to the food supplying chain. Although centralisation of food supplying in the army has economic advantages, it may contribute to the multifocal occurrence of outbreaks. A rapid intervention is key in the mitigation of outbreak consequences and in reducing secondary transmission.

Abstract

Objectives: To identify possible apoptotic events in renal tissue, using an animal model in which was simulated acute controlled haemorrhage, a frequent condition after a clinical situation resulting from a serious traumatic event, followed by volume replacement. Methods: 18 Large White pigs underwent total intravenous anaesthesia and randomly divided in three groups (n = 6). Group 1 and 2 were submitted to passive 25ml/kg arterial blood bleeding followed by volume replacement with LR in group1, and HES in group2, while group3 (control) was submitted to the same conditions, except bleeding and volume replacement. Renal tissue samples were processed for immunohistochemistry, using in situ TUNEL and Cytochrome c methods. The immunoreactivity levels were compared between groups using the Kruskal-Wallis test with Dunn’s post test. Background: Recent studies stated that canine norovirus (caNoV) infection is an endemic and highly contagious disease in the dog population in Portugal, with peak shedding in winter months. Previous studies reported caNoV as a potentially important zoonotic virus with increased importance since dogs are an integral part of household, potentially, affecting both owners and small animal veterinarians. However, despite the impact of norovirus-induced disease, the pathogenic features of infection are not well understood. The aim of this study was to evaluate the histopathological lesions in gastroenteritis, associated to CaNoV, in order to contribute to the knowledge of its pathogenic features. Material and methods: Samples of small intestine from 2 diarrheic dogs housed in a kennel, previously diagnosed with caNoV, were collected and fixed (10% neutral buffered formalin) for a maximum of 48 hours, embedded in paraffin wax, and 3µm sections were stained for routine histopathological evaluation. Results: The microscopic findings revealed a more notorious mononuclear inflammatory infiltrate within lamina propria of small intestine proximal segments, with increasing numbers of lymphocytes and plasma cells, changing from severe to moderate, from the duodenum to jejunum, respectively, like IBD pattern. Discussion and Conclusions: The histopathological features led to the hypothesis that caNoV could be significantly involved in the complex aetiopathogenesis of IBD, as reported with murine norovirus and human norovirus, the later having an important role exacerbating or complicating other diseases, like human Crohn disease - an example of human IBD, which highlights the relevance of norovirus, in its role of indirect infectious agent. Abstract

A duração da diabetes mellitus é um factor de risco no aparecimento de complicações oculares, A prevalência da retinopatia diabética é praticamente nula antes dos 10 anos de idade, independentemente da duração da doença, atingindo 95% após 20-30 anos de evolução. Na idade pediátrica outras manifestações oculares associadas são, também, raramemente detectadas. Na Consulta de Oftalmologia Pediátrica do Hospital de D. Estefânia avaliámos retrospectivamente 62 crianças com diagnóstico de diabetes mellitus tipo 1, no intervalo de tempo compreendido entre 1 de Janeiro de 1999 e 31 de Junho de 2000, As idades oscilavam entre os 3 e 17 anos (média 1J,8 anos), tendo sido 29 casos do sexo feminino e 33 do sexo masculino, O período de evolução da doença variava entre 6 meses e 16 anos (média de 6,2 anos). Na população estudada constatámos uma incidência de 6A% de lesões oculares. Verificámos 1 caso (1,6%) de rctinopatia de fundo, numa adolescente de 17 anos de idade e 11 anos de evolução da doença, e 3 casos (4,8%) de catarata bilateral. Na diabetes juvenil a probabilidade de ocorrência de lesões oculares precoces é baixa. A presente pesquisa tem em vista analisar a filosofia pol?tica kantiana a partir de seus escritos sobre o direito na obra Doutrina do Direito. Desse modo, come?a por um minucioso estudo sobre o estado de natureza, entendido, em termos kantianos, por direito privado, para, posteriormente, introduzir nossa aten??o no conceito de sociedade civil. O desenvolvimento da pesquisa gira em torno da propriedade e do contrato, no??es usadas para intermediar a rela??o entre direito natural e direito positivo, este ?ltimo encontrado somente no direito p?blico quando regido por leis jur?dicas. O conceito de posse jur?dica sob o estatuto do direito tamb?m ser? analisado, pois esse conceito apresenta, de forma mais evidente, as diferen?as jur?dicas que perpassam desde o estado de natureza privado de leis at? o Estado pol?tico formado pela sociedade civil. This research aims to analyze the Kantian political philosophy from his writings about the law in the work Doctrine of Right. Thus, it begins by a thorough study about the state of nature, understood, in Kantian terms, as general law, to then introduce our attention on the concept of civil society. The development of the research revolves around property and contract, notions used to intermediate the relationship between natural law and positive law, the latter found only in public when governed by juridical laws. The concept of juridical ownership under the statute of law will also be examined, as this concept presents, in a more evident way, the juridical differences that permeate from the state of private nature of laws to the private political State formed by civil society. Kant identifies the man as a rational being capable of self-legislation and gifted with an innate freedom. However, despite all having freedom even in the private law, Kant draws attention to the need of this freedom to turn into an external freedom in the political sphere, in order to prevent the abuse of the freedom of one and another. The civil society not only has the law that interferes with the freedom of everyone in order to preserve the rules of law in Society, however, encompasses citizens that have to be conducted to a better collective organization.

Abstract

Animal bodies represent one of the more complex and perfect systems of nature but despite its complexity and functionality, the control of its basic functions is performed by only two systems: the nervous system and the endocrine system. Reproduction, gestation, muscle contraction, nutrient digestion and absorption, hydroelectrolytic balance, haematopoiesis and so many other bodily functions, are directly or indirectly controlled by these systems. In a simplistic way, we can say that the nervous system is responsible for rapid organic activities, while the endocrine system is responsible for a slower, more durable activity, the metabolic functions of the organism. The endocrine system is essential for growth and development, sexual function, reproductive processes and metabolism. In the present work the authors describe the main endocrine glands in the dog, namely anterior and posterior pituitary gland, thyroid, parathyroid, pancreas and adrenal glands, referring their location, macroscopic and histologic features, using original figures. Main endocrine hormones and hormonal functions will also be addressed in a simple, straightforward and original manner, using cutting-edge, innovative schemes that will contribute for an easy and direct understanding of the complex endocrine mechanisms. The endocrine system is often subjected to pathological changes. Improving knowledge in the field of endocrinology will allow for a better clinical approach to animal patients, enhancing, without doubt, the provision of better medical veterinary care and its advancement towards excellence.
