52. Enteral feeding of preterm neonates: a survey of Australian neonatologists.

- Abstract
Postnatal growth restriction has been recently identified as a serious problem in preterm, especially extremely low birth weight neonates who are also at the highest risk for necrotizing enterocolitis (NEC). The fear of NEC and the difficulties in interpreting the signs of feed intolerance continue to be responsible for the variations in enteral feeding practices for these neonates. Such variations in clinical practice (e.g. fluid management, feeding regimens) have also been proposed to represent the 'iatrogenic' component of NEC. The findings of a survey (a questionnaire for self-administration and anonymous response) to document the enteral feeding practices of Australian neonatologists for neonates < 32 weeks' gestation are reported. A few controversial feeding practices are discussed in the light of the current evidence. The importance of minimizing variations in nutritional practices is emphasized. [ABSTRACT FROM AUTHOR]

This article reflects on multivariate analysis of hyperglycemia in extremely low birth weight infants (ELBW). The ELBW infants comprise a large subset of the neonatal intensive care unit (ICU) population. These infants have impaired glucose tolerance attributed to insufficient processing of pro-insulin by the immature pancreas, and decreased sensitivity of the liver to insulin, leading to continuous glucose production despite hyperglycemia.

Human milk is frequently heat-treated in hospitals, particularly milk that is banked, to destroy contaminating bacteria and viruses, but this treatment simultaneously reduces the content of some vitamins, enzymes, and immunological and nutritional factors. This study was performed to find the optimal conditions for heat treatment. The effects of 2 pasteurization temperatures on levels of protein sulfur amino acids (methionine, cystine) and some free amino acids (taurine, glutamine, glutamic acid) in light of the oxidative instability that occurs especially during thermal treatment were examined. These substances in raw human milk and in milk treated at 56.5 °C and 62.5 °C for 30 min were compared. Samples of mature human milk from all feeds over 24 h were obtained from 13 healthy well-nourished mothers of term infants. Each sample was divided into 3 parts: raw, treated at 56.5 °C for 30 min, treated at 62.5 °C for 30 min. The results showed that the availability of sulfur amino acids and free taurine is the same after heat treatment, whereas milk processing increased positively the levels of free glutamic acid and glutamine, but there is significance only for glutamine. The mean quantities of considered amino acids were similar in milk treated at the recommended pasteurization temperature (30 min at 62.5 °C) and at 56.5 ° for 30 min. [ABSTRACT FROM AUTHOR]

- Abstract
A consummate classic with a fresh approach to pediatric dermatology Children's skin is different. Maturation affects the epidermal barrier, the cutaneous microbiome, adnexal structures, vasculature, and transcutaneous absorption of drugs. The immature skin is more susceptible to pathogens and environmental disruption. Many genetic disorders are either present at birth or manifest early in childhood. Skin diseases thus present differently in children than in adults. Pediatric dermatology has seen significant advances over the last decade, particularly in the field of molecular genetics research, which has furthered our understanding of the pathogenesis of many skin diseases and the development of new approaches to treatment. This fourth edition of the Harper classic provides state-of-the-art information on all aspects of skin disease in children. It covers the diagnosis and treatment of all conditions - both common and rare - with a consistently evidence-based approach. Existing content has been refreshed and fully updated to reflect emerging thinking and to incorporate the latest in research and clinical data - especially at the genetic level. This new fourth edition includes: Greater focus on the genetics behind skin disease, including new genes/genodermatoses, progress in genetic analysis, and stem cell transplants Increased coverage of lasers and other technologies used to treat skin disease More summary tables, learning points, tables of differential diagnosis, and clinical algorithms for diagnosis and management Additional online features, including patient information links and multiple choice questions Harper's Textbook of Pediatric Dermatology delivers crucial clinical insights and up-to-date research information that spans the breadth of the field. As the most comprehensive reference book on this subject available, this revised fourth edition will support and guide the daily practice of both dermatologists and pediatricians across the world.

- Abstract
Alfred C. Young III's Complete Roster and Service Records of Lee's Army of Northern Virginia during the Overland Campaign is the first compilation of the entire roster and service records for all the various units that composed Robert E. Lee's Army of Northern Virginia during the 1864 Overland Campaign. It is the ultimate reference guide to the more than 120,000 soldiers who served with Lee in Virginia as he led his army into a series of battles against Union General Ulysses S. Grant. While there are specific guides to several of Lee's units, Young's work is the first comprehensive companion that features data on all of the men who served under the general during this campaign. Using an array of primary source material, from official Confederate records to southern newspapers, Young provides the enlistment and unit data for each soldier as well as a concise history of their service, including records on their rank, time served, promotion, hospitalization, wounds, capture, desertion, absence without leave, furloughs, and death. An essential archive for both genealogists and Civil War scholars, the Complete Roster and Service Records of Lee's Army of Northern Virginia during the Overland Campaign is the most wide-ranging catalog in existence of each soldier's record during the campaign. This vast array of research is available only as an e-book, enabling ease of search and annotation.

- Abstract
Glutamine: Biochemistry, Physiology, and Clinical Applications describes the different functions of glutamine (Gln) in animals and humans. Gln is both a nutrient and a signaling molecule, and its functions go beyond those of a simple metabolic fuel or protein precursor. This book has gathered together, in an unbiased and critical manner, all the available evidence and research on Gln including pathology (neurological diseases, intestinal diseases, critical illness, and cancer), physiology (successful aging), catabolic states, immunity, and exercise. Special attention is given to the potential benefit of Gln in states of insulin resistance and the role of Gln as a'conditionally essential'amino acid. The contributors are either pioneers or experts in the area of Gln from all around the globe, including Australia, Brazil, Canada, Europe, China, and the United States. This book is a valuable source of information for nutrition scientists, medical doctors, sports scientists, food scientists, dietitians, and anyone interested in nutrition. It is also a valuable resource for students in these fields and will be an important addition to university libraries.

- Abstract
Survival of extremely premature neonates has improved significantly following the advances in neonatal intensive care. Extrauterine growth restriction is a serious issue in this population. Nutritional exposures during critical period of life influence the individual's risk of disease throughout life. Nutritional deficit and poor growth are associated with long term neurodevelopmental impairment, short stature and metabolic disorders in extremely preterm neonates. Optimising nutrition in the early postnatal life of the preterm neonate is therefore a priority. However this is easier said than done considering the frequency of feed intolerance, fear of necrotising enterocolitis, and the hesitancy in adopting an aggressive approach to parenteral nutrition in this population. Some of the finest researchers in the field have come together to provide the clinical perspective on the A to Z of nutrition in the preterm neonate in simple and clear fashion in this book.

- Abstract
Neonatal nutrition has a pivotal role in normal child development and is of even greater importance in the sick or premature neonate. This 2006 edition includes a comprehensive account of the basic science, metabolism and nutritional requirements of the neonate, and a greatly expanded number of chapters dealing in depth with clinical issues ranging from IUGR, intravenous feeding, nutritional therapies for inborn errors of metabolism, and care of the neonatal surgical patient. Evolving from these scientific and clinical aspects, the volume highlights the important long-term effects of fetal and neonatal growth on health in later life. In addition, there are very practical chapters on methods and techniques for assessing nutritional status, body composition, and evaluating metabolic function.

- Abstract
During the last two decades the increase in allergic diseases in children, such as atopic dermatitis and asthma, has been drastic. However, this is not true for the entire world: the incidence of allergies in children has risen only in developed countries. The observation of this socio-geographic discrepancy has led to careful study of the environmental differences that exist between the diverse settings in which children are born and has resulted in the so-called ‘hygiene hypothesis': the ‘sterility'of modern hospitals and birth places in the developed world might lead to a lack of microbial stimulation required for the development of a balanced mucosal immune response, including expansion of T-helper (Th) cell subsets that can mediate immune responses. Therefore, this workshop was held to consider in depth the environmental factors that influence the changing pattern of worldwide childhood allergy. This publication is a valuable source of knowledge and update for nutritionists, pediatricians, immunologists, microbiologists, as well as professionals concerned with preventive medicine.

THE MAJORITY OF premature infants, especially very low birth weight (birth weight < 1500 g) infants, receive ≥ 1 red blood cell transfusion for anemia of prematurity during their first [...], OBJECTIVES. Erythropoietin is frequently administered to premature infants to stimulate erythropoiesis. The primary goal of erythropoietin therapy is to reduce transfusions, but the efficacy of erythropoietin has not been convincingly demonstrated in this regard. The aim of this trial was to investigate whether combined administration of vitamin [B.sub.12], folic acid, iron, and erythropoietin could decrease transfusion requirements in extremely low birth weight infants. PATIENTS AND METHODS. In a randomized, controlled trial, extremely low birth weight infants with a birth weight ≤ 800g and a gestational age ≤ 32 weeks were randomly assigned to a group receiving combination treatment or a control arm. RESULTS. The treatment increased levels of folate in red blood cells, vitamin [B.sub.12], ferritin, transferrin receptor levels in plasma, and reticulocyte counts. The proportion of infants requiring no transfusions was lower in the treatment group (38%) as compared with controls (5%). The treatment group and the need for mechanical ventilation were independent predictors of the number of transfusions in multiple regression analysis. Cox regression analysis indicated that combined therapy resulted in a 79% risk reduction for any transfusion. CONCLUSION. Combined treatment with erythropoietin, intravenous iron, folate, and vitamin [B.sub.12] during the first weeks reduces the need for transfusion in extremely low birth weight infants. HYPERGLYCEMIA IS ASSOCIATED with increased mortality and morbidity rates among nondiabetic adults admitted to ICUs for treatment of myocardial infarction, (1) stroke, (2) or trauma (3,4) and among adults admitted [...], OBJECTIVES. The objectives of this study were to determine the prevalence of hyperglycemia in extremely low birth-weight infants and to determine whether hyperglycemia increases the risk of early adverse outcomes (death or intraventricular hemorrhage of grade 3 or 4) and/or affects the length of hospital stay among survivors without intraventricular hemorrhage. METHODS. The charts of all extremely low birth-weight infants (n = 93) admitted to Texas Children's Hospital (Houston, TX) during 2001 were reviewed. The highest daily blood glucose concentrations, highest dopamine infusion rates, highest daily percentage of inspired oxygen, and mean blood sodium concentrations were averaged over the first week of life or before death or occurrence of grade 3 or 4 intraventricular hemorrhage. Among survivors without severe intraventricular hemorrhage, the time ratio for blood glucose concentrations of > 150 mg/dL was calculated. RESULTS. More than 50% of the infants had persistent blood glucose concentrations of >150 mg/dL during their first week of life. Early adverse outcomes were associated with the average highest daily blood glucose concentration through interaction with the Clinical Risk Index for Babies score and with the average highest daily percentage of inspired oxygen. The length of hospital stay was associated with the time ratio for blood glucose concentrations of >150 mg/dL through interaction with birth weight and the average highest daily percentage of inspired oxygen. CONCLUSION. These data confirm the high prevalence of hyperglycemia among parenterally fed, extremely low birth-weight infants and show that high blood glucose concentrations increase the risk of early death and grade 3 or 4 intraventricular hemorrhage and the length of hospital stay among survivors without intraventricular hemorrhage, which suggests that prevention and treatment of hyperglycemia may improve the outcomes of extremely low birth-weight infants. www.pediatrics.org/cgi/doi/10.1542/ peds.2006-0628 doi:10.1542/peds.2006-0628 Key Words hyperglycemia, outcomes, extremely low birth weight Abbreviations IVH--intraventricular hemorrhage ELBW--extremely low birth weight CRIB--Clinical Risk Index for Babies LOS--length of stay PMAD--postmenstrual age at discharge

IMPROVED OXYGENATION AFTER birth results in systemic oxygen delivery that far exceeds oxygen consumption among newborns. Without hypoxic stimuli for erythropoietin production, (1) serum erythropoietin concentrations decrease and erythropoiesis declines [...], BACKGROUND. Premature infants, especially those with birth weights of METHODS. In a randomized, controlled trial, 64 premature infants (birth weight: 801-1300 g) receiving erythropoietin and iron supplementation were assigned randomly to receive either vitamin [B.sub.12] (3 µg/kg per day) and folate (100 µg/kg per day) (treatment group) or a lower dose of folate (60 µg/kg per day) (control group). RESULTS. During the 4-week observation period, vitamin [B.sub.12] and folate enhanced erythropoietin-induced erythropoiesis significantly, as indicated by a 10% increase in red blood cell counts, compared with folate alone. Hemoglobin and hematocrit levels remained stable in the treatment group, whereas they decreased in the control group. Vitamin [B.sub.12] levels in the treatment group increased over baseline and control values, whereas red blood cell folate levels were comparable between the groups. Subsequent analysis showed slight nonsignificant differences in baseline red blood cell count, hemoglobin level, hematocrit level, and mean corpuscular volume values, which must be addressed as a limitation. CONCLUSIONS. With the limitation of a slight imbalance in baseline data between the study groups, combined therapy with vitamin [B.sub.12], folate, erythropoietin, and orally and intravenously administered iron seemed more effective in stimulating erythropoiesis among premature infants, compared with erythropoietin, iron, and low-dose folate alone. Additional trials are necessary to confirm these data. www.pediatrics.org/cgi/doi/10.1542/ peds.2005-2475 doi:10.1542/peds.2005-2475 Key Words anemia of prematurity, erythropoietin, folic acid, vitamin [B.sub.12] Abbreviations RBC--red blood cell MCV--mean corpuscular volume TFR--transferrin receptor

Although glutamine is the most abundant amino acid in both plasma and human milk, (1) it is not included in standard intravenous amino acid solutions because of its limited stability [...], Background. Glutamine is one of the most abundant amino acids in both plasma and human milk, yet it is not included in standard intravenous amino acid solutions. Previous studies have suggested that parenteral nutrition (PN) supplemented with glutamine may reduce sepsis and mortality in critically ill adults. Whether glutamine supplementation would provide a similar benefit to extremely low birth weight (ELBW) infants is not known. Methods. We performed a multicenter, randomized, double-masked, clinical trial to assess the safety and efficacy of early PN supplemented with glutamine in decreasing the risk of death or late-onset sepsis in ELBW infants. Infants 401 to 1000 g were randomized within 72 hours of birth to receive either TrophAmine (control) or an isonitrogenous study amino acid solution with 20% glutamine whenever they received PN up to 120 days of age, death, or discharge from the hospital. The primary outcome was death or late-onset sepsis. Results. Of the 721 infants who were assigned to glutamine supplementation, 370 (51%) died or developed late-onset sepsis, as compared with 343 of the 712 infants (48%) assigned to control (relative risk: 1.07; 95% confidence interval: 0.97-1.17). Glutamine had no effect on tolerance of enteral feeds, necrotizing enterocolitis, or growth. No significant adverse events were observed with glutamine supplementation. Conclusions. Parenteral glutamine supplementation as studied did not decrease mortality or the incidence of late-onset sepsis in ELBW infants. Consequently, although no harm was demonstrated, routine use of parenteral glutamine supplementation cannot be recommended in this population. ABBREVIATIONS. PN, parenteral nutrition; ELBW, extremely low birth weight; RIK relative risk; CI, confidence interval.
