Heavy menstrual bleeding causes significant health and social problems for up to 30% of women at some point of their lives. Medical treatment is not always sufficient or tolerated by women. Hysterectomy is a definitive solution, but it is a major operation associated with long disability and potential severe complications. Endometrial ablation techniques have been developed to avoid the risks related to hysterectomy. Further evolution of these techniques also offers a possibility of a quick and simple outpatient procedure. This study evaluated the long-term effects of endometrial ablation on heavy menstrual bleeding and later endometrial diagnostics. Another aim was to compare the costs when the procedure was performed in different settings. Finally, the effect of hyaluronic acid gel on intrauterine adhesion formation was assessed. Endometrial ablation had a good long-term effect on heavy menstrual bleeding in a retrospective study of 172 women, and up to 84% avoided hysterectomy during the follow-up time mean of 5 years. Seventy-six per cent of the patients were satisfied with the procedure. Due to the formation of intrauterine adhesions, prior endometrial ablation compromised later diagnostics of endometrium, and outpatient endometrial sampling failed in 23% of 57 women who had undergone endometrial ablation a mean of 6 years earlier, but that did not seem to have clinical importance. In a prospective, randomized and double-blind pilot study of 36 patients, hyaluronic acid gel did not prevent the formation of intrauterine adhesions. In a cost-minimisation analysis based on real resource use, performing endometrial ablation as an outpatient procedure under local anaesthetic instead of a day case procedure performed in the operating theatre under general anaesthetic reduced the costs significantly, from 1,865 to 1,065 euros. Sex hormone-binding globulin (SHBG) is a glycoprotein responsible for the transport and biologic availability of sex steroid hormones, primarily testosterone and estradiol. SHBG has been associated with chronic diseases including type 2 diabetes (T2D) and with hormone-sensitive cancers such as breast and prostate cancer. We performed a genome-wide association study (GWAS) meta-analysis of 21,791 individuals from 10 epidemiologic studies and validated these findings in 7,046 individuals in an additional six studies. We identified twelve genomic regions (SNPs) associated with circulating SHBG concentrations. Loci near the identified SNPs included SHBG, PRMT6, GCKR, ZBTB10, JMJD1C, SLCO1B1, NR2F2, ZNF652, TDGF3, LHCGR, BAIAP2L1, and UGT2B15. These genes encompass multiple biologic pathways, including hepatic function, lipid metabolism, carbohydrate metabolism and T2D, androgen and estrogen receptor function, epigenetic effects, and the biology of sex steroid hormone-responsive cancers including breast and prostate cancer. We found evidence of sex-differentiated genetic influences on SHBG. In a sex-specific GWAS, the loci 4q13.2-UGT2B15 was significant in men only. Additionally, three loci showed strong sex-differentiated effects: 17p13.1-SHBG and Xq22.3-TDGF3 were stronger in men, whereas 8q21.12-ZBTB10 was stronger in women. Conditional analyses identified additional signals at the SHBG gene that together almost double the proportion of variance explained at the locus. Using an independent study of 1,129 individuals, all SNPs identified in the overall or sex-differentiated or conditional analyses explained ~15.6% and ~8.4% of the genetic variation of SHBG concentrations in men and women, respectively. The evidence for sex-differentiated effects and allelic heterogeneity highlight the importance of considering these features when estimating complex trait variance.

The first aim of this randomized, placebo-controlled and double-blind study was to investigate whether low-dose aspirin therapy, started prior to controlled ovarian hyperstimulation, improves ovarian stimulation response, uterine haemodynamics and clinical pregnancy rates in unselected patients who underwent in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI). The second aim was to examine if the maternal serum placental proteome is different in IVF/ICSI pregnancies compared with spontaneous pregnancies, and whether low-dose aspirin modifies maternal serum placental protein expression and uteroplacental haemodynamics during the first half of pregnancy. Finally, the effect of low-dose aspirin therapy on the incidence of hypertensive pregnancy complications among women who became pregnant after IVF/ICSI was investigated. Low-dose aspirin therapy did not increase the number of oocytes retrieved, the total number of embryos or number of top-quality embryos, endometrial thickness or uterine haemodynamics on the day of embryo transfer (ET) or clinical pregnancy rates compared with placebo-treated IVF/ICSI women. On the day of ET, low-dose aspirin did not affect UtA vascular impedance, but the incidence of non-optimal uterine artery haemodynamics (UtA PI≥3.0) was statistically significantly lower (p<0.05) in the aspirin group compared with the placebo group. In the placebo-treated IVF/ICSI patients, maternal serum proteome analysis showed altered protein expression compared with women with spontaneous pregnancies. Between aspirin- and placebo-treated IVF/ICSI patients, proteome analysis showed a unique and distinct pattern of differentially expressed proteins including extra-cellular matrix, complement and transport proteins.
