Background: HIV-1 can be inhibited by RNA interference in vitro through the expression of short hairpin RNAs (shRNAs) that target conserved genome sequences. In silico shRNA design for HIV has lacked a detailed study of virus variability constituting a possible breaking point in a clinical setting. We designed shRNAs against HIV-1 considering the variability observed in naïve and drug-resistant isolates available at public databases., Methods: A Bioperl-based algorithm was developed to automatically scan multiple sequence alignments of HIV, while evaluating the possibility of identifying dominant and subdominant viral variants that could be used as efficient silencing molecules. Student t-test and Bonferroni Dunn correction test were used to assess statistical significance of our findings., Results: Our in silico approach identified the most common viral variants within highly conserved genome regions, with a calculated free energy of ≥ -6.6 kcal/mol. This is crucial for strand loading to RISC complex and for a predicted silencing efficiency score, which could be used in combination for achieving over 90% silencing. Resistant and naïve isolate variability revealed that the most frequent shRNA per region targets a maximum of 85% of viral sequences. Adding more divergent sequences maintained this percentage. Specific sequence features that have been found to be related with higher silencing efficiency were hardly accomplished in conserved regions, even when lower entropy values correlated with better scores. We identified a conserved region among most HIV-1 genomes, which meets as many sequence features for efficient silencing., Conclusions: HIV-1 variability is an obstacle to achieving absolute silencing using shRNAs designed against a consensus sequence, mainly because there are many functional viral variants. Our shRNA cocktail could be truly effective at silencing dominant and subdominant naïve viral variants. Additionally, resistant isolates might be targeted under specific antiretroviral selective pressure, but in both cases these should be tested exhaustively prior to clinical use.

Objectives: Examination of cancer rates in a single Hispanic subgroup-Puerto Ricans- and comparison of incidence rates among mainland Puerto Ricans living in the United States, island Puerto Ricans in Puerto Rico, and U.S. non-Hispanic whites to reveal ethnic-specific cancer patterns and disparities in Puerto Ricans., Methods: Incidence data were obtained from the cancer registries of Puerto Rico and three U.S. northeastern states (New York, New Jersey, and Connecticut) with a high density of mainland Puerto Ricans. Age-adjusted rates were compared by standardized rate ratios (SRRs)., Results: Total cancer incidence was the lowest in island Puerto Ricans, intermediate for mainland Puerto Ricans, and highest in U.S. non-Hispanic whites. Compared to mainland Puerto Ricans, islanders had significantly lower rates (p<0.05) for major cancers-lung (SRRs=0.36 in males and 0.29 in females), prostate (SRR=0.71), female breast (SRR=0.73), and colon-rectum (SRRs=0.74 in males and 0.65 in females)-as well as several less common cancers (urinary bladder; non-Hodgkin lymphoma; liver; kidney and renal pelvis; pancreas; thyroid; leukemia; and skin melanoma). Overall cancer rates in mainland Puerto Ricans were modestly lower than those in U.S. non-Hispanic whites, but mainland Puerto Ricans had the highest rates for stomach, liver, and cervical cancers among the three populations., Conclusion: Despite socioeconomic disadvantages, island Puerto Ricans have relatively low cancer incidence. Identifying contributing factors would be informative for cancer research, and understanding the reasons for increased cancer risk in their mainland counterparts would facilitate the development of ethnic-specific intervention programs.

To determine the effects of suckling on postpartum (pp) reproductive efficiency in Pelibuey ewes, two experiments were performed. In Experiment 1, 112 ewes were randomly assigned to one of two groups at parturition: Without restriction of suckling (WRS) 24 h day(-1) for 60 days (n=56), and Weaned Ewes (WE), weaned at 40 days pp (n=56). On Day 30 pp, all ewes were given Prostaglandin (PGF2alpha) and one of four treatments (n=14): T1, intravaginal progestagen (FGA; 40 mg) for 12 days from day 30 pp+equine chorionic gonadotropin (eCG; 300 UI) until 2 days before removing FGA; T2, FGA was applied for 12 days; T3, a second application of PGF2alpha was given on day 40 pp+eCG on the same day; T4, a second injection of PGF2alpha was applied on day 40 pp only. In all the analyzed characteristics, the best results were obtained in WE. Within the WE group, the best treatment (P<0.05) was T1 with 85.7% of the ewes in oestrus, 71.4% pregnant and a prolificacy of 1.9. Within the WRS group the best results were observed in T1. In both groups, the lowest results (P<0.05) were obtained in T4. In Experiment 2, 75 ewes were randomly assigned to one of three groups (n=25) immediately after parturition: Group 1, Without restriction of suckling (WRS, as in Experiment 1); Group 2, with restriction of suckling (RS; suckling for 30 min day(-1)); Group 3, Early Weaning (EW: at 7 days pp). All ewes were given PGF2alpha at 30 days pp and the same hormonal treatment, FGA for 12 days+PGF2alpha and eCG 2 days before removing FGA. No differences were observed (P>0.05) between RS and EW for the presentation of oestrus (96% vs. 92%), pregnancy (72% vs. 76%) or prolificacy (1.9 vs. 1.9), although group WRS did not perform (P<0.05) as well as groups RS and EW for any measure of performance. In conclusion, the combination of hormonal treatment (FGA plus eCG) with weaning at 7 or 40 days pp, or restricted suckling, improves postpartum reproductive efficiency in Pelibuey ewes, demonstrating the inhibitory role of suckling on postpartum reproduction in this breed.

We analyzed the repercussions on renal function between suprarenal endograft fixation and open surgery in the treatment of infrarenal abdominal aortic aneurysms (IAAAs) and determined the influential factors. Between 1999 and 2005, 59 IAAAs were treated with elective OS and 56 with SEF. The serum creatinine (Cr) level and its clearance were determined before the procedure, in the intensive care unit (ICU), on discharge, and after 1, 6, 12, and 24 months. A deterioration in renal function was considered to be a >30% increase in Cr or a Cr >2 mg/dL. A univariate statistical analysis and a logistical regression analysis were carried out to determine the predictive factors for repercussions on renal function. There were no statistically significant differences in the rate of renal exacerbation between the groups either on discharge (p = 0.52) or after 1 month (p = 0.483), 6 months (p = 0.451), 12 months (p = 0.457), and 24 months (p = 0.682). The only significant difference was that detected in the ICU (p = 0.033). Diabetes mellitus, time spent in the ICU, postoperative intubation time, intraoperative transfusion, and transfusion in the ICU were factors that influenced the deterioration of renal function in the univariate analysis. The only significant factor in the multivariate analysis was the need for transfusion in the ICU. Exacerbation of renal function occurred in both groups independently of treatment type. In the immediate postoperative period, hemodynamic deterioration is more frequent in the open surgery group. Renal exacerbation tended to disappear in both groups during follow-up.

A double channel laparoscope aids to locate and exteriorize the appendix., Objective: We studied this practice to determine if this technique makes it possible to make use of the advantages of laparoscopic surgery and of open surgery, decreasing costs without increasing the rate of complications., Materials and Methods: Clinical prospective paper. The study group was made up of transumbilical appendectomies (TA) for acute, uncomplicated appendicitis carried out in 2004 and 2005. The control group consisted of a group of open appendectomies (OA) performed for acute, uncomplicated appendicitis in our department in 2002 and 2003 (from a previous study)., Results: Both study groups were comparable. We performed a total of 162 TA due to uncomplicated appendicitis. The mean hospital stay was 2.84 days (versus 4.83 days with OA) (significant difference). Infectious complications presented in 4.29% (versus 1.75% in the OA group) (this difference was not significant). Postoperative pain and need for analgesia were less in the TA group than in the OA (difference did not reach significance)., Conclusions: Described technique is easily performed. The savings resulting from the transumbilical technique represent some 90,561.97 euros / year compared with the cost that laparoscopic appendectomy would incur, and some 42,232.37 euros/ year versus traditional, open surgery. Postoperative complications didn't increase significantly if compared with open appendectomy.

As the demographic makeup of the United States undergoes rapid transformation, the importance of studying ethnic and cultural variations between and within population subgroups becomes increasingly clear. Researchers must challenge themselves to move beyond simply matching their methodology to the research question at hand and seek out methods that are compatible with the culture, language, traditions, and particular life circumstances of the cultural group under study. Community-based participatory research is collaborative and includes community members as participants in the research process, which ensures the cultural and community specificity of the research. This article demonstrates how experiential learning of community members as research participants provided culturally grounded insights used in grant writing. The experiences and involvement of the community researchers shaped the research questions and provided the major conceptual basis in response to a National Institutes of Health Request for Applications.

Differential pulse polarography (DPP) and square wave voltammetry (SWV) were investigated, in order to know the stability of solid propellants which contain diphenylamine. The simultaneous determination of N-nitrousdiphenylamine (NnDPA), 2-nitrodiphenylamine (2nDPA), 4-nitrodiphenylamine (4nDPA) and 2,4-dinitrodiphenylamine (2,4dnDPA) by DPP and SWV was proposed due to these nitro-derivatives appear during the stabilisation process from degradation of diphenylamine (DPA) used as stabiliser in propellant compositions. The proposed methods were successfully applied to the simple base solid propellant (with nitrocellulose as the only active component), with its stabiliser DPA. Malarial ookinetes express an immunodominant surface protein (P28) that is a priority candidate for the development of transmission-blocking vaccines. The full length P28 gene from Plasmodium berghei [Pbs21(1-213)] and a deletion construct [Pbs21(1-188)] encoding a protein that lacks the 25 C-terminal amino acids, including the glycosylphosphatidylinositol (GPI) anchor signal, were expressed in insect cells using baculovirus vectors. Pbs21(1-213) protein is strongly hydrophobic, found in the cytoplasm and on the surface of Spodoptera Sf21 cells, and in the culture medium. Pbs21(1-188) protein was largely found in the aqueous phase of the medium and in the cytoplasm of Sf21 cells, but was not detected on the cell surface. The presence of 25 C-terminal amino acids is therefore critical to the attachment of recombinant Pbs21 to the parasite plasma membrane. Mice were immunized subcutaneously or intramuscularly with affinity purified recombinant Pbs21(1-213), Pbs21(1-188) or native Pbs21 proteins. Following two immunizations, native Pbs21 induces higher titres when administered by either route, than the recombinant protein bearing an insect GPI anchor, which in turn is markedly more immunogenic than the recombinant polypeptide lacking a GPI anchor. When specific anti Pbs21 antibody titres exceeded 1 mg/ml all three antigens were capable of inducing transmission blockade > or = 90%, below 1 mg/ml blockade did not correlate with antibody concentration.
