Tetanus, although rare in vaccinated regions, remains common in developing countries. Cephalic tetanus, caused by Clostridium tetani, is a rare, potentially fatal neurological condition. The presenting symptoms include spasms, rigidity, and paralysis in the head and neck. This case report describes a 5-year-old girl with unilateral facial paralysis initially diagnosed as Bell's palsy. Subsequent development of trismus and dysphagia led to a revised diagnosis of cephalic tetanus. Peripheral facial palsy may be an early sign of cephalic tetanus and should be considered in patients with relevant exposure history. Our patient’s recent nasal foreign body removal raised suspicion of cephalic tetanus, confirmed through clinical findings. Treatment with tetanus immunoglobulin and antibiotics led to gradual improvement. This case emphasizes the importance of considering cephalic tetanus in the differential diagnosis of facial nerve palsy, especially if there is a history of foreign body exposure. Early recognition and treatment are essential for favorable outcomes.

Summary: Background: People with diabetes are at increased risk of hospitalisation, morbidity, and mortality following SARS-CoV-2 infection. Long-term outcomes for people with diabetes previously hospitalised with COVID-19 are, however, unknown. This study aimed to determine the longer-term physical and mental health effects of COVID-19 in people with and without diabetes. Methods: The PHOSP-COVID study is a multicentre, long-term follow-up study of adults discharged from hospital between 1 February 2020 and 31 March 2021 in the UK following COVID-19, involving detailed assessment at 5 and 12 months after discharge. The association between diabetes status and outcomes were explored using multivariable linear and logistic regressions. Findings: People with diabetes who survived hospital admission with COVID-19 display worse physical outcomes compared to those without diabetes at 5- and 12-month follow-up. People with diabetes displayed higher fatigue (only at 5 months), frailty, lower physical performance, and health-related quality of life and poorer cognitive function. Differences in outcomes between diabetes status groups were largely consistent from 5 to 12-months. In regression models, differences at 5 and 12 months were attenuated after adjustment for BMI and presence of other long-term conditions. Interpretation: People with diabetes reported worse physical outcomes up to 12 months after hospital discharge with COVID-19 compared to those without diabetes. These data support the need to reduce inequalities in long-term physical and mental health effects of SARS-CoV-2 infection in people with diabetes. There is increasing concern for public health over inadvertent human exposure to MPs due to potential adverse health effects linked to MPs polymeric composition, toxic chemical additives, and/or harmful microorganisms adsorbing onto their surfaces. While numerous studies have reported MPs occurrence and risk in the freshwater aquatic environment and drinking water sources (e.g., rivers, lakes, and reservoirs), the current state-of-knowledge on MPs pollution in drinking water (i.e., tap water and bottled water) remains limited at a global level. This paper provides the first comprehensive study of the occurrence, concentrations, size distribution, shape, and polymer type of MPs in 177 tap water samples from 13 cities in the United Kingdom, as well as 85 samples of bottled water from 17 popular brands, with various packaging materials, on the UK market.MPs were detected in all tap water samples (range 6–100 MP/L) and bottled water samples (range 12–62 MP/L). IntroductionThe present study was conducted to investigate the effects of PMIX, a polyphenol-rich extract mixture from chestnut wood and olive, on growth performance, hematological parameters, immunity in serum and skin mucus, hepatic antioxidant enzymes, and intestinal cytokine expression in rainbow trout (Oncorhynchus mykiss).MethodsFour experimental diets containing 0 g PMIX kg−1 diet (control, C), 0.5 g PMIX kg−1 diet (P0.5), 1 g PMIX kg−1 diet (P1), and 2 g PMIX kg−1 diet (P2) were fed to rainbow trout in an eight-week feeding trial. Triplicate groups of fish received each treatment. Growth performance, feed conversion ratio, protein efficiency, hepatosomatic and viscerosomatic indices, hematological parameters, immunity markers, hepatic antioxidant enzyme activities, and intestinal cytokine expression were determined.ResultsPMIX supplementation significantly improved feed conversion ratios, while groups P0.5 and P1 evidenced an increase in growth performance along with protein efficiency ratios. It also showed decreased hepatosomatic and viscerosomatic indices in the P1 group. Except for higher percentages of monocytes in P0.5 and P1, most hematological parameters of the fish did not differ from that of the control. Serum lysozyme and respiratory burst activities were heightened in all PMIX-treated groups, and the skin mucus lysozyme activity was enhanced in P1. The blood phagocytic activity and phagocytic index, serum immunoglobulin, total protein, and bactericidal activity against A. hydrophila did not change. Hepatic superoxide dismutase activity significantly increased in P1 and P2, even though catalase activity did not change. Intestinal interleukin-6 expression was upregulated in all PMIX-treated groups, while tumor necrosis factor-alpha and interleukin-1 beta were upregulated in P1, P2, and P0.5 respectively.DiscussionThe present results evidence that dietary polyphenols from chestnut wood and olive extracts enhance growth performance, antioxidant capacity, and several immunological parameters in the blood, skin mucus, and intestine of rainbow trout. A suitable supplementation level was 1 g PMIX kg−1 diet to separate these improvements.

The pre-mRNA life cycle requires intron processing; yet, how intron-processing defects influence splicing and gene expression is unclear. Here, we find that TTDN1/MPLKIP, which is encoded by a gene implicated in non-photosensitive trichothiodystrophy (NP-TTD), functionally links intron lariat processing to spliceosomal function. The conserved TTDN1 C-terminal region directly binds lariat debranching enzyme DBR1, whereas its N-terminal intrinsically disordered region (IDR) binds the intron-binding complex (IBC). TTDN1 loss, or a mutated IDR, causes significant intron lariat accumulation, as well as splicing and gene expression defects, mirroring phenotypes observed in NP-TTD patient cells. A Ttdn1-deficient mouse model recapitulates intron-processing defects and certain neurodevelopmental phenotypes seen in NP-TTD. Fusing DBR1 to the TTDN1 IDR is sufficient to recruit DBR1 to the IBC and circumvents the functional requirement for TTDN1. Collectively, our findings link RNA lariat processing with splicing outcomes by revealing the molecular function of TTDN1.

Background and Aims: Assessment of the severity of substance use disorders (SUDs) in a culture-sensitive manner can help gauge the current condition of the substance user and assess change with time. The present study aimed to develop a scale for the assessment of the severity of SUDs in the Indian clinical context. Methods: Based upon the review of literature on previously available instruments and a consultative meeting of experts, a clinician-rated scale was developed that finally comprised 41 items. A briefer 5-item scale with current and lifetime versions was also developed. The scales were applied to patients with SUDs at six different clinical sites. Results: The instrument was applied to 720 patients (98.2% males, mean age: 34.6 years). The Cronbach’s alpha of the full scale was 0.852. The inter-rater reliability Pearson correlation coefficient of the full-scale was r = 0.821 (P < 0.001), and the intra-class correlation coefficient single measure was 0.800 (95% confidence interval: 0.724–0.956). A four-factor solution was suggested to be the most tenable. The mean application duration of the full scale was 13.4 minutes, and that of the briefer version was 2 minutes. Conclusion: This validated scale could be a potentially useful assessment measure for the severity of SUDs in the Indian context. The utility lies in the simplicity of administration and scoring and the balance between brevity and thorough assessment.

Abstract This study assessed the association between atherosclerosis indices, serum uric acid to high‐density lipoprotein cholesterol ratio (UHR) and triglyceride‐glucose (TyG) index and the prevalence of hypertension among MASHAD cohort participants. In this cross‐sectional study, the participants were divided into hypertensive and non‐hypertensive subjects. The atherosclerosis indices, UHR and TyG index of the two groups were compared. Logistic regression analyses were used to determine the associations of these indices with hypertension in both sex. In recent years, attention has focused on improving the health and satisfaction of employees by enhancing visual comfort in workplaces. This involves providing adequate natural daylight, glare control, and outdoor views. Adaptive facades, leveraging technological advancements, have gained popularity for improving indoor conditions but still face issues like window-area glare and insufficient daylight in rear spaces. Previous research indicates that inclined walls can help control glare and allow larger windows with better light distribution. This study examines the integration of an adaptive facade with an inclined wall to improve efficiency. It analyzes visual comfort in three cities with different latitudes and solar radiation angles: Mogadishu, Tehran, and Moscow. Using Grasshopper in Rhino software, the study evaluates visual comfort indices (UDI, ASE, sDA, DGP) for six employees on December 21 and June 21 at 9:00, 12:00, and 15:00. The results show that the integrated system effectively distributes daylight and controls glare, reducing it to 34% in Mogadishu and 29% in Tehran. However, in Moscow, the system is less effective, requiring facade rotation towards the sky to reduce glare to 25%. The research highlights the need to consider more variables such as window-to-wall ratio, material, and employees’ position or other objectives like energy consumption and thermal comfort for further research to improve the system.

The Covid-19 virus first discovered in February 2020 has defied borders by domesticating itself in all countries around the globe. Like in all other countries, the Covid-19 pandemic even though at first a health crisis has impacted all spheres of social life in the UAE from the economy to education as well as professions and their practices. The spread of infectious diseases is also known to live behind anxiety, fear, and general psychological distress among people. Government and health agencies in the UAE have embarked on public education and engagement to curb the spread of the virus as well as mitigate some of its adverse psychological consequences including anxiety and fear. The development of the Internet and new media has provided health authorities with channels for reaching out to the public on desirable health behavior. The discourse on digital divide for instance, has, among others, questioned technologically deterministic views about the novelty of the Internet by pointing at the divides or barriers it has created in society. While the UAE is reported to be the most digitally connected country in the MENA region in terms of the spread and access to Internet and broadband facilities thus overcoming the first level of digital divide, this study uses both focus group discussions and in-depth interviews to explore Internet and social media use patterns, and extent of reproduction of the second and third levels of digital divide among communities in the country during Covid-19 pandemic.

Our research demonstrates the practical implementation of using multilayer (three layers) composites for electromagnetic interference (EMI) shielding. Three layers are: (1) The composites of multi-walled carbon nanotubes (MWCNT)/ carboxymethylcellulose (CMC) as the bottom layer; (2) The composites of MWCNT/CMC/iron oxide (Fe3O4) particles as the middle layer; (3) The composites of MXene/MWCNT/CMC/Fe3O4 as the top layer. Composition of each layer was optimized in order to maximize the electromagnetic properties of each layer. Each composite layer has outstanding electrical conductivity and excellent EMI shielding performance. The multilayer exhibits an EMI SET of 48 dB, SEA of more than 33 dB, and low SER of 15 dB in the X-band frequencies range, significantly enhanced compared to the single-layered materials for EMI applications.

Due to the necessity of performing thermal operations, heat exchangers are widely employed in many different areas. The heat transfer and fluid flow within a spiral double-pipe heat exchanger fitted with a novel turbulator were numerically assessed in this work. The presented novel turbulator is a curved tube with holes incorporated into its thickness and spiral ribs on its inner wall. The turbulator wall's curved rib design produces secondary flows at the turbulator output when fluid flows through the tube and the perforations. A commercial CFD tool, based on the finite volume technique, was used to conduct the numerical simulations. The fluid flow regime is turbulence (Re = 8,000 – 14,000). Two sections make up this work. The first portion looked at how the hydrothermal behavior of the fluid flow inside the proposed turbulator was affected by the angle at which the curved ribs rotated. For this angle, three values were considered: θ = 30, 90, and 150°, and the outcomes were contrasted with those of a plain spiral double-tube heat exchanger (turbulator not included). Then, the number of embedded holes in the turbulator's thickness changes in the second part, and three values of N = 12, 16, and 20 were considered. According to the first part's findings, the model exhibiting θ = 90° had a greater thermal performance factor at Re = 10,000. This model has a more noteworthy thermal performance factor than the models with θ = 150 and θ = 30° by approximately 15.62 % and 22.65 %, respectively (at Re = 10,000). Furthermore, the second section's numerical findings showed that the model with N = 20 had more extraordinary thermal performance at Re = 10,000. Model N = 20 has a thermal performance factor of about 16.93 % and 17.55 % greater than models N = 16 and N = 12. Within the proposed heat exchanger, the recommended turbulator produced a sizable rotating flow, and including embedded holes significantly reduced the pressure drop this kind of turbulator causes.

IntroductionThis study addresses the rising cardiovascular disease (CVD) rates in the United Arab Emirates (UAE) by investigating the occurrence and impact of genetic variants in CVD-related genes.MethodsWe collected all genes linked to heritable CVD from public and diagnostic databases and mapped them to their corresponding biological processes and molecular pathways. We then evaluated the types and burden of genetic variants within these genes in 343 individuals from the Emirati Mendelian Study Cohort and 3,007 national electronic health records.ResultsWe identified a total of 735 genes associated with heritable CVD, covering a range of cardiovascular conditions. Enrichment analysis revealed key biological processes and pathways, including Apelin, FoxO, and Ras signaling, that are implicated across all forms of heritable CVD. Analysis of a UAE cohort of 3,350 individuals showed a predominance of rare and unique CVD variants specific to the population. The study found a significant burden of pathogenic variants in families with CVD within the Emirati Mendelian cohort and re-assessed the pathogenicity of 693 variants from national health records, leading to the discovery of new CVD-causing variants.DiscussionThis study underscores the importance of continuously updating our understanding of genes and pathways related to CVD. It also highlights the significant underrepresentation of the UAE population in public databases and clinical literature on CVD genetics, offering valuable insights that can inform future research and intervention strategies.

Extra-thyroidal effects of TSH have been reported in various tissues expressing the TSH receptor (TSHR) including several areas of the brain. However, the influence of TSH on neuronal phenotypes has not been examined. Using a well-characterized human neuroblastoma cell line (SH-SY5Y), we have examined TSH signaling effects on the phenotype of these cells after their neuronal differentiation. Following an 18-day differentiation protocol, we successfully redifferentiated the SH-SY5Y cells into ~100% neuronal cells as indicated by the development of extensive neurofilaments with SMI-31 expression. Furthermore, using absolute digital PCR, we quantified TSHR mRNA, and also TSHR protein expression, in the redifferentiated cells and found that the neuronal cells expressed high quantities of both TSHR message and protein at baseline. Exposure to TSH induced primary, secondary, and tertiary neurite outgrowths, which are essential for cell–cell communication. Quantitative analysis of neurites using ImageJ showed a dose-dependent increase in neurites. The addition of TSH up to 1 mU/ml resulted in a ~2.5-fold increase in primary, and ~1.5-fold in secondary and tertiary neurites. The lengths of the neurites remained unaffected with the dosage of TSH treatment. Furthermore, TSHR signaling in the differentiated cells resulted in enhanced generation of cAMP, pPI3K, pAKT, and pNFkB pathways and suppression of pMAPK suggesting an influence of these signals in driving neurite outgrowth. These data showed that the TSH/TSHR axis in neurons may contribute to enhanced neurite outgrowth. The potential pathophysiological effects of TSH on the induction of neurite outgrowth and its relationship to neurodegenerative diseases remain to be explored.

BackgroundFrailty is a critical concern for older adults, impacting their susceptibility to adverse events and overall quality of life. This study aimed to determine the frailty status of patients 60 years or older in Abu Dhabi Ambulatory Healthcare Services (AHS) and assess its relation to the stress exerted by Ramadan fasting and the occurrence of any adverse outcomes.MethodsIn this prospective observational study, participants were included if the attending physicians used the IDF-DAR risk stratification assessment tool. A tele-interview was conducted to complete the FRAIL score within 6 weeks before Ramadan 1,444 (CE 2022). The outcome was assessed through another tele-interview and an electronic medical record review after Ramadan.ResultsAccording to the FRAIL assessment tool, among the 204 patients aged 60 years or older included in the study, 109 (53.4%) were classified as either frail or pre-frail. In total, 20 (9.8%) patients were frail, that is, 1 out of 10, and 89 (43.6%) were pre-frail. The remaining 95 (46.6%) patients were robust. Using logistic regression to assess the occurrence of adverse outcomes after Ramadan fasting, a higher frailty score was identified as the third independent risk factor [B = 0.4, OR = 1.5 (1–2.02–1.86), and p = 0.039] for experiencing an adverse event. The identified factors associated with frailty were age, increased albumin-to-creatinine ratio (ACR), chronic kidney diseases (CKDs), and ischemic heart diseases (beta = 0.27, p = 0.003; beta = 0.24, p = 0.004; beta = 0.2, p = 0.039; and beta = 0.18, p = 0.041, respectively). One-third of the frail patients had an event, while the incidence in pre-frail patients was 11.2%, and among the robust patients, the incidence was 6.3%. Physicians’ global assessment of frailty did not align well with the structured FRAIL scoring. Only five (25%) out of the 20 patients identified as frail by the FRAIL assessment tool were also judged as frail or having cognitive function impairment by the physicians’ global assessment tool.ConclusionFrailty is prevalent among elderly patients with diabetes. Disparity exists between subjective and objective frailty assessments, emphasizing the need for standardized evaluation methods. Using the FRAIL tool is recommended for patients aged 60 or older with diabetes in Abu Dhabi.

Introduction: Knee osteoarthritis is a degenerative disease of joints and is the major cause of chronic impairment in older ages. This is the consequence of several years of use accompanied by the effect of wear and tear of bone, joint, and cartilage. Aims & Objectives: To evaluate the quality of life among patients with osteoarthritis actively and non-actively performing religious prayers in district Swat, Pakistan. Place and Duration of Study: The study was conducted at District Swat, from November 2019 to September 2020. Material & Method: A comparative cross-sectional study was conducted among osteoarthritic patients. A pre-tested and validated KOOS questionnaire was administered to osteoarthritic patients (group 1), who had modified their prayer method by using a chair to perform prayers, and compared to another group of osteoarthritic patients (group 2) who were practicing normal praying position. Pain, symptoms, everyday activities, sports and recreation, and knee-related quality of life (QOL) were assessed. SPSS version 22 was used for statistical analysis. P-value ?0.05 was considered significant. Results: The total number of patients in both groups was 800. Among those, 54.8% had a family history of knee pain, 60.4% had bilateral knee problems and 93.4% had no prior knee injuries. Group 1 was found to have more complaints related to the knee as compared to group 2 in all subscales of KOOS. Statistically significant relationship was observed between duration of knee pain, family history, knee problem, and previous knee injury between group 1 and group 2. (P

Objective: Radiodermatitis (RD) is a frequent adverse event of radiotherapy (RT). Currently, there is no consensus and approved protocol for the treatment of RD. Curcumin (CUR) is a natural polyphenol obtained from turmeric and it has low intrinsic toxicity in humans. The aim of this systematic review was to explore the efficacy of CUR for prevention and treatment of RD.Materials and Methods: A systematic literature review was performed in the following online databases: Cochrane library, PubMed, Scopus, Web of Science, MEDLINE, and EMBASE. Among the 5 selected records, 3 had a randomized clinical trial (RCT)-design and the other had a pilot and controlled study designed. The included studies were performed on breast cancer (N=3), head and neck cancers (N=1) and different types of cancer (N=1).Results: Four of the studies reported that the application of curcumin in cancer patients undergoing radiotherapy is associated with decreased intensity of radiodermatitis. However, one study did not report any significant effect of CUR on radiodermatitis. This review provides substantial evidence which confirm the clinical value of CUR in cancer supportive care.Conclusion: Further prospective clinical trials in larger scales are warranted in order to determine the " supplemental form and dose of CUR" for RD prevention and treatment in patients receiving radiotherapy.

This study aims to synthesize akermanite (Ca2MgSi2O7) using an eco-friendly and fast microwave-assisted method and understand the effect of using citric acid (CA) as a chelating agent to optimize the calcination temperature for synthesizing pure akermanite nanoparticles (along with a trace amount of merwinite) at low calcination temperatures. This study developed a new predictive model by machine learning-assisted modeling to advance the understanding of relationship between synthesis parameters and the resulting purity and particle size of akermanite. To evaluate the reliability of the developed model in predicting the purity and characteristics of the resultant Ak powder, the crystallization behavior and phase formation sequence of citric acid-containing samples and those without citric acid were experimentally investigated by differential thermal analysis (DTA) and X-ray diffraction (XRD) techniques. Following validation and training, the developed model indicates promising effects of citric acid on reducing crystallization temperature, enhancing purity, and decreasing particle size. The synthesized powders were further characterized by field emission scanning electron microscopy (FESEM), energy-dispersive X-ray spectroscopy (EDS), and transmission electron microscopy (TEM) analyses. For the first time, our results addressed the microwave-assisted synthesis as an effective method via experimental evidence and machine learning-assisted modeling that allows for the production of akermanite containing a trace amount of merwinite with the aid of citric acid at a relatively low calcination temperature of 950 °C. In vitro bioactivity evaluation supported the potential suitability of Ak powders for bone tissue engineering applications.

Climate change, including drought, is an important threat to sustainable development. Entrepreneurial businesses are undeniable due to their acceptance and adaptation to environmental changes to respond to critical conditions. This research has been done with the aim of "analyzing the responses of entrepreneurs to the water crisis". In addition to identifying the answers, the effects of the water crisis on entrepreneurs and the consequences of the entrepreneurs' actions on the business performance were identified. In terms of results, this research is practical and of a qualitative type of case study. The research community is entrepreneurial businesses in the three provinces of Fars, Hormozgan, and Isfahan and the field of activity: agriculture, food products, tourism, mineral and industrial materials, and animal husbandry that were affected by the water crisis. Purposeful sampling method was used and data collection was done through in-depth and semi-structured interviews between February and March of 2022, which reached theoretical saturation after conducting 13 interviews. Data analysis was done by qualitative content analysis. The findings showed that the water crisis had an impact on two stages of the business value chain, i.e. "supply chain" and "production chain". Supply chain impacts include impacts on water resources and raw materials, and production impacts include impacts on products, process, assets, and production factors. The results indicate that entrepreneurs have reacted to these effects with three strategies, diversification, cooperation contracts, and processes modification. It was also found that the response of entrepreneurs to the water crisis has positive and negative effects on aspects of the company's performance (operational performance, financial performance, social performance, environmental performance).

Aim: The present study aimed to evaluate the knowledge, attitude, and practice (KAP) of endodontists in India for working length measurements using Apex locators. Methods: This study is an online questionnaire-based survey consisting of 24 questions in total, divided into five parts. The first part consisted of questions related to demographic details, the second part inquires about the tools and technology used for working length determination, and the remaining questions evaluate the KAP regarding the use of apex locators to determine working length (five questions each). Participants in the study were either pursuing or completed postgraduation in endodontics. A total of 257 responses were recorded; however, six participants were excluded due to incomplete submission. Thus, 251 responses were analyzed as descriptive analyses. Correlation analysis and Chi-square tests were performed to find any association between demographic variables and years of experience in practice with KAP of using apex locators by endodontists. Results: In this study, it was observed that the knowledge of the majority of endodontic practitioners was fair, with a positive attitude and practice of using apex locators. Knowledge of the study participants was significantly (P ≤ 0.05) associated with the good attitude and practice of the study participants. Furthermore, years of experience in practice have a positive association with the KAP of endodontists. Conclusion: It can be concluded that endodontists have better clinical acceptance, as they show positive attitudes and practices for using apex locators for working length determination.

Objectives: The visual system plays a crucial role in controlling the posture of individuals by updating information on the positions and movements that different parts of the body perform. This study aimed to investigate and compare the role of vision in balance recovery strategies between healthy, prone to injury, Coper, and chronic ankle instability (CAI). Methods: The current research was of an applied and comparative type, which was conducted on male athletes in the age group of 18 to 30 years. Subjects were divided purposefully into four groups including CAI (n=15), Coper (n=15), prone to ankle injury (n=15), and healthy group (n=15). In this study, treadmills were used to create disturbance in the subjects’ postures in both anterior and posterior directions, and Kinovea software to record kinematic information about subjects during sudden disturbances. Paired t-test was used to compare the scores of the subjects in open and closed eyes. Results: The results of the paired t-test showed that in the variables of ankle oscillations, hip oscillations, and the ratio of the hip to ankle oscillations in both anterior and posterior disturbances in the groups of CAI, Coper, prone to injury, and healthy, there was a significant difference between open eyes (P=0.001) and closed eyes (P=0.001). The order of most fluctuations in all variables was from CAI group to Coper, prone to injury, and healthy, indicating the greater effectiveness of balance recovery strategies by eliminating visual feedback in the CAI group. Discussion: To summarize the research findings, compared with uninjured and Coper participants, those with CAI had less balance and more functional limitations. Therefore, in training programs for ankle sprain rehabilitation, special attention should be paid to balance recovery strategies and the role of visual feedback to control posture.
