International audience; The XXL Survey is the largest homogeneous survey carried out with XMM-Newton. Covering an area of 50 deg2, the survey contains several hundred galaxy clusters out to a redshift of ≍2, above an X-ray flux limit of ~6 × 10-15 er g cm-2 s-1. The GAMA spectroscopic survey of ~300 000 galaxies covers ≍286 deg2, down to an r-band magnitude of r < 19.8 mag. The region of overlap of these two surveys (covering 14.6 deg2) represents an ideal opportunity to study clusters selected via two independent selection criteria. Generating two independently selected samples of clusters, one drawn from XXL (spanning a redshift range 0.05 ≤ z ≤ 0.3) and another from GAMA (0.05 ≤ z ≤ 0.2), both spanning 0.2 ≲ M500 ≲ 5 × 1014 M⊙, we investigate the relationship between X-ray luminosity and velocity dispersion (LX - σv relation). Comparing the LX - σv relation between the X-ray selected and optically selected samples, when not accounting for the X-ray selection, we find that the scatter of the X-ray selected sample is 2.7 times higher than the optically selected sample (at the 3.7σ level). Accounting for the X-ray selection to model the LX - σv relation, we find that the difference in the scatter increases (with the X-ray selected sample having a scatter 3.4 times larger than the optically selected sample). Although the scatter of the optically selected sample is lower, we find 13 optically selected GAMA groups undetected in X-rays. Inspection of the difference in magnitude between the first and second brightest galaxies in the cluster, and a stacked X-ray image of these 13 groups, suggests that these are young systems still in the process of forming.

Introduction Transthyretin cardiac amyloidosis (ATTR-CA) is an emerging cause of heart failure (HF) and is associated with cardiac mass hypertrophy and voltage impairment, but little is known on how theses markers interplay with survival. Objective To evaluate if cardiac voltage/mass ratio (VMR) is associated with prognosis in HF patients involving ATTR-CA. Methods Patients with confirmed mutated (m) or wild-type (wt) ATTR-CA addressed in a single cardiology center underwent retrospective ECG and echocardiographic study to define voltage with Sokolow indice (SV1+RV5, in mV) and left ventricular mass (LVM, g/m²) according to ASE definition. Two groups of patients were defined according to the median cardiac voltage/mass ratio (VMR in µV/g/m²) : Low (VMR < 6.1) or normal (VMR ≥ 6.1). Tc99m-HDP myocardial uptake (MU) on bone scintigraphy (BS) was described according to Perugini (P) classification : PI (MU < chest bones), PII (MU = chest bones) and PIII (MU > bones). Main end-point was overall mortality. Results Among the 74 patients included, mean age was 82±7 years, 80% were males (Graph 1) and 25 (34%) were classified as ATTRwt-CA, 3 (4%) as ATTRm-CA, and 46 (62%) had ongoing genetic exploration. Mean follow-up was 543±332 days. Overall mortality was 21%. On BS, 6 patients (8%) were ranked as PI, 30 (42%) PII and 36 (50%) PIII, with no difference on mortality rates. After ANOVA, mean VMRi significantly decreased with intensity of Tc99m-HDP myocardial uptake on BS (p=0.033, Graph 2A). After log-rank test, survival was impaired in the low VMR group (p=0.034, Graph 2B). After Cox regression, low voltage/masse ratio remained associated with increased risk of mortality (HR 4.6, 95% CI [1.1–18.6], p=0.03), after adjusting on age, sex, left ventricle ejection fraction and log(NT-proBNP). Conclusion VMR is a simple index to collect and significantly associated with impaired survival in HF patients with ATTR-CA. Further studies are needed to confirm these results.

The coronavirus disease 2019 (COVID-19) pandemic highlighted the lack of agreement regarding the definition of aerosol-generating procedures and potential risk to healthcare personnel. We convened a group of Massachusetts healthcare epidemiologists to develop consensus through expert opinion in an area where broader guidance was lacking at the time.

Resume But Le but de ce travail etait de rendre compte, apres dix annees de pratique, de notre experience de la genitoplastie feminisante chez l’enfant pre-pubere et l’adolescente presentant une anomalie du developpement genital (ADG) dans le contexte d’un pays en developpement. Materiels et methode Nous avons mene une etude observationnelle, descriptive et retrospective, allant du 1er novembre 2009 au 1er avril 2019, soit une periode de 9 ans. Ont ete inclus tous les enfants pre-puberes (8–12 ans) et les adolescentes elevees filles, porteuses d’une anomalie du developpement genital, desireuses de chirurgie ainsi que leurs familles et ayant ete l’objet d’une genitoplastie feminisante. Les parametres etudies ont ete : âge au moment de la presentation et de la chirurgie, diagnostic precis, gestes complementaires, suites operatoires, resultat cosmetique et recul. Pour chaque patient, la chirurgie n’a ete de mise qu’apres un diagnostic precis au sein d’une equipe multidisciplinaire experimentee. Les resultats cosmetiques prenaient en compte une reduction de la taille du clitoris et une ouverture separee de l’uretre et du vagin au perinee. Resultats Neufs enfants pre-puberes et adolescentes eleves filles d’âge median 8 ans (EI :10,75) ont ete colliges. L’âge median au moment de la chirurgie etait de 11 ans (EI : 9,5). Les diagnostics precis retenus ont ete : 46, XX ovotestis DSD elevee en fille (n = 2) ; 46, XY DSD insensibilite partielle aux androgenes (n = 2), 46, XY DSD deficit en 5 alpha reductase elevee fille (n = 1), 46, XY DSD deficit en 17-ceto-reductase elevee en fille (n = 1), 46, XY DSD dysgenesie gonadique (mutation du gene NR5A1) elevee en fille (n = 2) ; 45,X/46, XY dysgenesie gonadique mixte elevee en fille (n = 1). La gonadectomie a domine les gestes associes, bilaterale dans 7 cas et unilaterale gauche dans 2 cas. Avec un recul median de 26 mois (EI: 18,25), aucune complication hemorragique n’a ete notee, une retention aigue d’urine transitoire a pu etre observee et les resultats cosmetiques juges satisfaisants. Conclusion La genitoplastie feminisante au Cameroun reste un defi majeur pour le chirurgien pediatrique et elle ne doit se concevoir qu’apres un diagnostic precis. Le traditionnel retard porte a la consultation dans ce contexte apparait comme un avantage, puisqu’il permet aux patientes de participer aux decisions therapeutiques qui engagent leur avenir en termes de developpement psychosocial mais aussi de fertilite. We perform the first known census of X-ray clusters that are undetected due to significant central AGN contamination. In recent work (Bhargava et al. 2022, submitted to A&A), we attempt to characterise all known AGN contaminated clusters within the XXL survey footprint and provide a systematic assessment of the cosmological impact of such systems. We find that 5% of genuine clusters within the XXL survey area are undetected due to AGN contamination - the majority of these clusters are above z > 0.6, where X-ray cluster detection efficiency drops significantly and cosmological AGN activity increases. With the increased sensitivity of Athena, we can expect to recover clusters at the current X-ray photon limit up to approximately z ~ 1.9. Hence, the fraction of clusters missed due to AGN is likely to be higher than 5%. Understanding the distribution of AGN and clusters in X-ray samples is therefore crucial to maximise the cosmological potential of clusters, particularly at high redshift.
