Abstract

Controlling carrier concentration is critical in many device applications, and both chemical substitution and modulation doping have been used in industry. For most inorganic materials, very low doping efficiencies are observed as site occupancies depend on both thermodynamic and kinetic factors. We demonstrate that we can make kinetically controlled site-specific substitutions in a series of (BixSn1–xSe)1+δTiSe2 compounds using the modulated elemental reactants method. These compounds were characterized using a combination of X-ray diffraction, resistivity and Hall coefficient measurements, and high angle annular dark field scanning transmission electron microscopy (HAADF STEM). For small x, the doping efficiency is 0.7, close to that observed for B in silicon. For higher x values, a structural distortion is observed in X-ray diffraction data in which the symmetry of the in-plane unit cell decreases. Abstract

OBJECTIVEWe found previously that inappropriate inpatient antimicrobial use was often attributable to erroneous diagnoses. Here, we detail diagnostic errors and their relationship to inappropriate antimicrobial courses.DESIGNRetrospective cohort studySETTINGVeterans Affairs hospitalPATIENTSA cohort of 500 randomly selected inpatients with an antimicrobial courseMETHODSBlinded reviewers judged the accuracy of the initial provider diagnosis for the condition that led to an antimicrobial course and whether the course was appropriate.RESULTSThe diagnoses were correct in 291 cases (58%), incorrect in 156 cases (31%), and of indeterminate accuracy in 22 cases (4%). In the remaining 31 cases (6%), the diagnosis was a sign or symptom rather than a syndrome or disease. The odds ratio of a correct diagnosis was 4.3 (95% confidence interval [CI], 2.2–8.5) if the index condition was related to the reason for admission. When the diagnosis was correct, 181 of 292 courses (62%) were appropriate, compared with only 10 of 208 (5%) when the diagnosis was incorrect or indeterminate or when providers were treating a sign or symptom rather than a syndrome or disease (PCONCLUSIONSDiagnostic accuracy is important for optimal inpatient antimicrobial use. Abstract

Reward mediates the acquisition and long-term retention of procedural skills in humans. Yet, learning under rewarded conditions is highly variable across individuals and the mechanisms that determine interindividual variability in rewarded learning are not known. We postulated that baseline functional connectivity in a large-scale frontostriatal-limbic network could predict subsequent interindividual variability in rewarded learning. Resting-state functional MRI was acquired in two groups of subjects (n = 30) who then trained on a visuomotor procedural learning task with or without reward feedback. We then tested whether baseline functional connectivity within the frontostriatal-limbic network predicted memory strength measured immediately, 24 h and 1 month after training in both groups. We found that connectivity in the frontostriatal-limbic network predicted interindividual variability in the rewarded but not in the unrewarded learning group. Prediction was strongest for long-term memory. Similar links between connectivity and reward-based memory were absent in two control networks, a fronto-parieto-temporal language network and the dorsal attention network. The results indicate that baseline functional connectivity within the frontostriatal-limbic network successfully predicts long-term retention of rewarded learning.

Abstract

Synthesis and characterization of Mg x Zr2−x N2 (0.5 ≤ x ≤ 1.8) thin films deposited by reactive magnetron co-sputtering in nitrogen plasma is reported. Composition measurements show that nitrides with low oxygen content (less than 1%) can be formed up to x = 1.0, at which point an increase in oxygen content is observed. Up to composition of x = 1.6 the Mg x Zr2−x N2 thin films form in a rocksalt-derived crystal structure, as revealed by X-ray diffraction measurements. At x > 1.6 the films rapidly oxidize. The lattice constant of the stoichiometric MgZrN2 composition is a = 4.537 A, and only small changes in lattice parameter are observed with changing composition. Electrical conductivity decreases by several orders of magnitude with increasing Mg-content. The conductivity of Mg-rich (x ≥ 1) films increases with increasing measurement temperature, indicating semiconducting character of Mg-rich Mg x Zr2−x N2. Optical absorption measurements of these Mg-rich samples show a clear absorption onset at 1.8 eV, also indicative of semiconducting behavior.

Abstract

In this paper, we consider classical circle geometries and connect them with places of planar Cayley–Klein geometries. There are, in principle, only three types of \( {{\mathbb{R}}^2} \)-ring structures and, thus, only three types of corresponding circle geometries. Thus, each generalization to non-Euclidean planes turns out to be just another representation of the classical Euclidean cases. We believe that even the Euclidean cases of circle geometries comprise, in principle, already all non-Euclidean cases. Representations of such non-Euclidean circle geometries might also be of interest in themselves. For example, among the planar Cayley–Klein geometries, the quasi-elliptic and quasi-hyperbolic geometry usually are neglected. They can be treated similarly to the isotropic Mobius geometry by suitable projections of the Blaschke cylinder.

Abstract

The most frequent carcinoma in male patients is prostate cancer. D'Amico and colleagues (1998) set up a classification to assess the aggressiveness of prostate cancer growth and the risk of metastatic spread, taking into account the following parameters: PSA, Gleason Score (GS), and tumour spread (T). The combination of these parameters results in a classification of low-, intermediate- and high-risk patients.In the context of a larger heuristic study entitled "Low-Risk Prostate Cancer Patients: Significance And Importance Of Pelvic Lymphadenectomy in Localized Prostate Carcinomas", we observed the correlation of the parameters PSA, GS and T stage with the occurrence of lymph node metastases in low-risk-patients who underwent lymphadenectomy during radical prostatectomy.Due to a lack of evidence - none of the confirmed LR (low-risk=LR) patients was diagnosed with metastases - we set up a subclassification for LR patients for the following corridor transition zone between low-risk and high-risk patients (based on the D'Amico classification):PSA≤10 ng/ml and GS≤7a (c: 7a) and clinical stage T1a-T2c (c: T2b, T2c): The population consists of 288 prostate cancer patients (60 low-risk (LR) patients, 228 corridor (c) patients). We analysed the number and frequency of removed lymph nodes and lymph node metastases. Moreover, we analysed the population with a view to a postoperative GS upgrade. The results are based on heuristic methods.The situation regarding the data in the transition zone, i. e. in the corridor area, and in the verified low-risk range is very complicated:In total, 3 743 lymph nodes were removed in 288 patients (mean: 13 lymph nodes). Of these only 7 lymph nodes in 5 patients from the corridor group were metastatic. The overall number of removed lymph nodes in these 5 patients lies within a small interval [23,32]; 26 lymph nodes were removed on average. A postoperative Gleason Score upgrade was found in 20.14% of the whole patient population.A postoperative Gleason Score upgrade in every fifth patient shows a highly relevant preoperative undergrading in these patients, which may lead to inadequate treatment. An opening of the D'Amico classification to include a corridor category of patients could be an opportunity to minimise preoperative undergrading. In this context, pelvic lymphadenectomy must be considered, at least for corridor patients.

Abstract

Data on muon pair production obtained by the OPAL collaboration at centre of mass energies near the Z peak are analysed. Small angular mismatches between the directions of the two muons are used to assess the effects of initial state photon radiation and initial-final-state radiation interference on the forward-backward asymmetry of muon pairs. The dependence of the asymmetry on the invariant mass of the pair is measured in a model-independent way. Abstract

The lack of a continental shelf in steep volcanic islands leads to significant changes in tropical cyclone inundation potential, with wave setup and runup increasing in importance and wind driven surge decreasing when compared to more gently-sloped mainland regions. This is illustrated through high resolution modeling of waves, surge, and runup on the Hawaiian Islands of Oahu and Kauai. A series of hurricane waves and water levels were computed using the SWAN + ADCIRC models for a suite of 643 synthetic storm scenarios, while local wave runup was evaluated along a series of 1D transects using the phase-resolving model Bouss1D. Waves are found to be an extremely important component of the inundation, both from breaking wave forced increases in storm surge and also from wave runup over the relatively steep topography. This is clear in comparisons with debris lines left by Hurricane Iniki on the Island of Kauai, where runup penetration is much greater than still water inundation in most instances. The difference between steeply-sloping and gently-sloping topographies was demonstrated by recomputing Iniki with the same landfall location as Hurricane Katrina in Louisiana. Surge was greatly increased for the mild-slope Iniki-in-Louisiana case, while pure wind surge for Iniki-in-Kauai was very small. For the entire suite of storms, maxima on Kauai show predicted inundation largely confined to a narrow coastal strip, with few locations showing more than a few hundred meters of flooding from the shoreline. As expected, maximum flooded areas for the 643 storms were somewhat greater than the Iniki inundation. Oahu has significantly more low-lying land compared to Kauai, and consequently hypothetical tropical cyclone landfalls show much more widespread inundation. Under direct impact scenarios, there is the potential for much of Honolulu and most of Waikiki to be inundated, with both still water surge and wave runup contributing. Other regions of Oahu show inundation confined to a more narrow coastal strip, although there is still much infrastructure at risk. Even for very strong storms in Oahu and Kauai, maximum still water surge is relatively small, and does not exceed 3 m in any storm modeled. In contrast, hurricane waves several kilometers from shore regularly exceed 10 m due to the lack of a continental shelf.

Abstract

A search is performed for Higgs bosons decaying into invisible final states, produced in association with a Z(0) boson in e(+)e(-) collisions at energies between 183 and 209 GeV. The search is based on data samples collected by the OPAL detector at LEP corresponding to an integrated luminosity of about 660 pb(-1). The analysis aims to select events containing the hadronic decay products of the Z(0) boson and large missing momentum, as expected from Higgs boson decay into a pair of stable weakly interacting neutral particles, such as the lightest neutralino in the Minimal Supersymmetric Standard Model. The same analysis is applied to a search for nearly invisible Higgs boson cascade decays into stable weakly interacting neutral particles. No excess over the expected background from Standard Model processes is observed. Limits on the production of invisibly decaying Higgs bosons produced in association with a Z(0) boson are derived. Assuming a branching ratio BR(h(0) -> invisible) = 1, a lower limit of 108.2 GeV is placed on the Higgs boson mass at the 95% confidence level. Abstract

This Letter describes a direct search for pair produced magnetic monopoles in e(+)e(-) collisions. The analysis is based on 62.7 pb(-1) of data collected with the OPAL detector at an average centre-of-mass energy of root S = 206.3 GeV. The monopole signal was assumed to be characterized by two back-to-back particles with an anomalously high ionization energy loss dE/dx in the tracking chambers. No evidence for production of monopoles was observed. Upper limits were obtained on the magnetic monopole pair-production cross-section (sigma) in the mass range 45 GeV/c(2) < m(M) < 102 GeV/c(2). The average limit is sigma < 0.05 pb and is essentially independent of the magnetic monopole mass. The cross-section limit is derived at the 95% confidence level and is valid for spin-1/2 magnetic monopoles. (C) 2008 Published by Elsevier B.V.

Abstract

Inclusive jet production (e(+)e(-) -> e(+)e(-) + jet + X) is studied in collisions of quasi-real photons radiated by the LEP beams at e(+)e(-) centre-of-mass energies root s(ee) from 189 to 209 GeV. Jets are reconstructed using the k(perpendicular to) jet algorithm. The inclusive differential cross-section is measured as a function of the jet transverse momentum, p(T)(jet), in the range 5 < p(T)(jet) < 40 GeV for pseudo-rapidities, eta(jet), in the range - 1.5 < eta(jet) < 1.5. The results are compared to predictions of perturbative QCD in next-to-leading order in the strong coupling constant. (c) 2007 Elsevier B.V. All rights reserved.

Abstract

Urachal carcinoma is a rare form of cancer. It often is diagnosed incidentally, like in our case report, because its cardinal symptom also occurs in a number of other urological diseases. We report the case of a 26-year-old man with a mucinous adenocarcinoma of the urachus. The carcinoma was removed via partial cystectomy with umbilical tumour excision.

Abstract

Zysto-/Rektozelen und prolapschirurgische Eingriffe gewinnen innerhalb der urogynakologischen Operationslehre zunehmend an Bedeutung. Das Risiko fur einen prolapschirurgischen Eingriff bei der Frau liegt heute bei ca. 11%, in einem Drittel der Falle handelt es sich um eine Rezidivoperation. Dabei treten 3 mogliche Operationsverfahren in Konkurrenz zueinander: 1. der transvaginale Zugang mit einer Vaginae fixatio sacrospinalis vaginalis oder sacrotuberalis, 2. die transabdominale, offen-chirurgisch durchgefuhrte Sakrokolpopexie und 3. die transabdominale, laparoskopisch durchgefuhrte Sakrokolpopexie.

Abstract

A new scalar resonance, called the radion, with couplings to fermions and bosons similar to those of the Higgs boson, is predicted in the framework of Randall-Sundrum models, proposed solutions to the hierarchy problem with one extra dimension. An important distinction between the radion and the Higgs boson is that the radion would couple directly to gluon pairs, and in particular its decay products would include a significant fraction of gluon jets. The radion had the same quantum numbers as the Standard Model (SM) Higgs boson, and therefore they can mix, with the resulting mass eigenstates having properties different from those of the SM Higgs boson. Existing searches for the Higgs boson are sensitive to the possible production and decay of radions and Higgs bosons in these models. For the first time, searches for the SM Higgs boson and flavour-independent and decay-mode independent searches for a neutral Higgs boson are used in combination to explore the parameter space of the Randall-Sundrum model. In the dataset recorded by the OPAL experiment at LEP, no evidence for radion or Higgs particle production was observed in any of those searches. Abstract

Measurements of R b , the ratio of the b b ¯ cross-section to the q q ¯ cross-section in e + e − collisions, are presented. The data were collected by the OPAL experiment at LEP at centre-of-mass energies between 182 and 209 GeV. Lepton, lifetime and event-shape information is used to tag events containing b quarks with high efficiency. The data are compatible with the Standard Model expectation. The mean ratio of the eight measurements reported here to the Standard Model prediction is 1.055 ± 0.031 ± 0.037 , where the first error is statistical and the second systematic.

Abstract

We present a determination of the LEP beam energy using "radiative return" fermion-pair events recorded at centre-of-mass energies from 183 GeV to 209 GeV. We find no evidence of a disagreement between the OPAL data and the LEP Energy Workings Group's standard calibration. Abstract

Events with a final state consisting of two or more photons and large missing transverse energy have been observed in e + e − collisions at centre-of-mass energies in the range 192–209 GeV using the OPAL detector at LEP. Cross-section measurements are performed within the kinematic acceptance of the selection and compared with the expectations from the Standard Model process e + e − → ν ν ¯ γ γ ( γ ) . No evidence for new physics contributions to this final state is observed. Upper limits on σ ( e + e − → X X ) ⋅ BR 2 ( X → Y γ ) are derived for the case of stable and invisible Y. In the case of massive Y the combined limits obtained from all the data range from 10 to 60 fb, while for the special case of massless Y the range is 20 to 40 fb. The limits apply to pair production of excited neutrinos ( X = ν * , Y = ν ), to neutralino production ( X = χ ˜ 2 0 , Y = χ ˜ 1 0 ) and to supersymmetric models in which X = χ ˜ 1 0 and Y = G ˜ is a light gravitino.

Abstract

A search for the Higgsstrahlung process e + e − → hZ is described, where the neutral Higgs boson h is assumed to decay into hadronic final states. In order to be sensitive to a broad range of models, the search is performed independent of the flavour content of the Higgs boson decay. The analysis is based on e + e − collision data collected by the OPAL detector at energies between 192 and 209 GeV. The search does not reveal any significant excess over the Standard Model background prediction. Results are combined with previous searches at energies around 91 and at 189 GeV. A limit is set on the product of the cross-section and the hadronic branching ratio of the Higgs boson, as a function of the Higgs boson mass. Assuming the hZ coupling predicted by the Standard Model, and a Higgs boson decaying only into hadronic final states, a lower bound of 104 GeV / c 2 is set on the mass at the 95% confidence level.

Abstract

Bose-Einstein correlations between like-sign charged-particle pairs in e(+)e(-) --> W+ W- events recorded with the OPAL detector at LEP at centre-of-mass energies between 183 GeV and 209 GeV are studied. Recently proposed methods which allow direct searches for correlations in the data via distributions of test variables are used to investigate the presence of correlations between hadrons originating from different W bosons in W+W- --> q (q) over barq (q) over bar events. Within the statistics of the data sample no evidence for inter - WW Bose-Einstein correlations is obtained. The data are also compared with predictions of a recent implementation of Bose-Einstein correlation effects in the Monte Carlo model PYTHIA.

Abstract

Hadronic event shape distributions from e+e− annihilation measured by the OPAL experiment at centre-of-mass energies between 91 GeV and 209 GeV are used to determine the strong coupling α S. The results are based on QCD predictions complete to the next-to-next-to-leading order (NNLO), and on NNLO calculations matched to the resummed next-to-leading-log-approximation terms (NNLO + NLLA). The combined NNLO result from all variables and centre-of-mass energies is while the combined NNLO + NLLA result is The completeness of the NNLO and NNLO + NLLA results with respect to missing higher order contributions, studied by varying the renormalization scale, is improved compared to previous results based on NLO or NLO + NLLA predictions only. The observed energy dependence of α S agrees with the QCD prediction of asymptotic freedom and excludes the absence of running.

Abstract

A search for single production of doubly-charged Higgs bosons has been performed using 600.7 pb^-1 of e+e- collision data with sqrt(s)=189--209GeV collected by the OPAL detector at LEP. No evidence for the existence of H++/-- is observed. Upper limits on the Yukawa coupling of the H++/-- to like-signed electron pairs are derived. Additionally, indirect constraints on the Yukawa coupling from Bhabha scattering, where the H++/-- would contribute via t-channel exchange, are derived for M(H++/--) < 2TeV. These are the first results for both a single production search and constraints from Bhabha scattering reported from LEP., 20 pages, 9 figures, Submitted to Phys. Lett. B

Abstract

A search for stable and long-lived massive particles of electric charge |Q/e|=1 or fractional charges of 2/3, 4/3, and 5/3 is reported using data collected by the OPAL detector at LEP, at centre-of-mass energies from 130 to 209 GeV. These particles are assumed to be pair-produced in e+e- collisions and not to interact strongly. No evidence for the production of these particles was observed. Model-independent upper limits on the production cross-section between 0.005 and 0.028 pb have been derived for scalar and spin-1/2 particles with charge +-1. Within the framework of the Constrained Minimal Supersymmetric Standard Model (CMSSM), this implies a lower limit of 98.0 (98.5) GeV on the mass of long-lived right (left)- handed scalar muons and scalar taus. Long-lived charged heavy leptons and charginos are excluded for masses below 102.0 GeV. For particles with fractional charge +-2/3, +-4/3, and +-5/3, the upper limit on the production cross-section varies between 0.005 and 0.020 pb. All mass and cross-section limits are derived at the 95% confidence level and are valid for particles with lifetimes longer than 10-6s. Abstract

A search was performed for charginos with masses close to the mass of the lightest neutralino in e+e- collisions at centre-of-mass energies of 189-209 GeV recorded by the OPAL detector at LEP. Events were selected if they had an observed high-energy photon from initial state radiation, reducing the dominant background from two-photon scattering to a negligible level. No significant excess over Standard Model expectations has been observed in the analysed data set corresponding to an integrated luminosity of 570pb-1. Upper limits were derived on the chargino pair-productin cross-section, and lower limits on the chargino mass were derived in the context of the Minimal Supersymmetric Extension of the Standard Model for the gravity and anomaly mediated Supersymmetry breaking scenarios., 26 pages, 9 figures

Abstract

The tau -> mu nu nu branching ratio has been measured using data collected from 1990 to 1995 by the OPAL detector at the LEP collider. The resulting value of B(tau -> mu nu nu) = 0.1734 +- 0.0009(stat) +- 0.0006(syst) has been used in conjunction with other OPAL measurements to test lepton universality, yielding the coupling constant ratios gmu/ge=1.0005+-0.0044 and gt/ge=1.0031+-0.0048, in good agreement with the Standard Model prediction of unity. A value for the Michel parameter eta = 0.004 +- 0.037 has also been determined and used to find a limit for the mass of the charged Higgs boson mH+- > 1.25 tan beta, in the Minimal Supersymmetric Standard Model.

Abstract

Searches for first generation scalar and vector leptoquarks, and for squarks in R-parity violating SUSY models with the direct decay of the squark into Standard Model particles, have been performed using e+e- collisions collected with the OPAL detector at LEP at e+e- centre-of-mass energies between 189 and 209 GeV. No excess of events is found over the expectation from Standard Model background processes. Limits are computed on the leptoquark couplings for different values of the branching ratio to electron-quark final states.

Abstract

Data collected around sqrt{s}=91 GeV by the OPAL experiment at the LEP e+e- collider are used to study the mechanism of baryon formation. As the signature, the fraction of Sigma-hyperons whose baryon number is compensated by the production of a Sigma-, Lambda or Xi- antihyperon is determined. The method relies entirely on quantum number correlations of the baryons, and not rapidity correlations, making it more model independent than previous studies. The diquark fragmentation model without the popcorn mechanism is strongly disfavored with a significance of 3.8 standard deviations including systematic uncertainties. It is shown that previous studies of the popcorn mechanism are not conclusive if parameter uncertainties are considered., Comment: No of pages 31 - No of figures 7

Abstract

Interindividual differences in the effects of reward on performance are prevalent and poorly understood, with some individuals being more dependent than others on the rewarding outcomes of their actions. The origin of this variability in reward dependence is unknown. Here, we tested the relationship between reward dependence and brain structure in healthy humans. Subjects trained on a visuomotor skill-acquisition task and received performance feedback in the presence or absence of reward. Reward dependence was defined as the statistical trial-by-trial relation between reward and subsequent performance. We report a significant relationship between reward dependence and the lateral prefrontal cortex, where regional gray-matter volume predicted reward dependence but not feedback alone. Multivoxel pattern analysis confirmed the anatomical specificity of this relationship. These results identified a likely anatomical marker for the prospective influence of reward on performance, which may be of relevance in neurorehabilitative settings.

Abstract

As the Internet is used increasingly as a platform for business transactions, security becomes a primary issue for Internet applications. Some applications are too sensitive for software-only security mechanisms. Higher levels of protection can be achieved with smart-card-based authentication schemes and transaction protocols. In this paper, we provide examples of typical banking applications implemented with smart cards using symmetrical (DES) and asymmetrical (RSA) cryptography. We present a pure JavaTM architecture for such applications, which is intended for use on standard Web application servers and client devices enabled for Web browsing and the Java language. It employs applets on the client side to access smart cards via the OpenCard Framework. The applets communicate with authentication servlets or application servlets on the server side and act as a mediator between the smart card and the application logic on the server.

Abstract

Individuals who experience interpersonal sensitivity (IPS) may be at an increased risk for developing eating disorder symptomatology. The purpose of the present study was to assess the predictive capacity of IPS and related constructs in the development of bulimic symptomatology both cross-sectionally and longitudinally while controlling for depressive symptoms. Participants were 119 female undergraduate psychology students attending a private mid-size Midwestern university. Data were collected at both the beginning and end of the academic semester. Participants completed the Center for Epidemiological Study - Depression Scale, Brief Fear of Negative Evaluation Scale, Interpersonal Sensitivity Measure, and the Bulimia Test - Revised. The Fragile Inner Self subscale of the IPSM was found to significantly account for additional variability in BULIT-R scores after controlling for depression both cross-sectionally and longitudinally (8% and 2%, respectively). IPS is a suitable candidate for inclusion in the dual pathway model of bulimic symptomatology.

Abstract

* BACKGROUND AND OBJECTIVE: To assess the safety and efficacy of erbium:YAG laser-assisted cataract removal. * PATIENTS AND METHODS: A total of 15 patients underwent cataractous lens removal. All the patients had a visual acuity of 20/50 or worse secondary to senile cataract. The endothelial cell count was calculated preoperatively and at 6 weeks postoperatively. A 2. 94-µm-wavelength erbium:YAG laser with a zirconium-fluoride fiber optic and silica tip was used to fracture and emulsify the nucleus. The erbium: YAG laser was chosen due to its high absorption in water, a primary component of a cataractous lens. * RESULTS: The postoperative visual acuity was 20/30 or better in all the eyes that were treated with surgery. The endothelial cell loss at 3 months was 0% to 10%. No laser-related complications were noted. A conversion to an ultrasound surgical technique was utilized in six cases. Vitreous loss occurred in one case due to the posterior extension of an anterior capsulotomy tear. * CONCLUSION: This study demonstrated the ability of an erbium:YAG laser system to safely and effectively emulsify the lens nucleus. We conducted a meta-analysis of Parkinson's disease genome-wide association studies using a common set of 7,893,274 variants across 13,708 cases and 95,282 controls. Twenty-six loci were identified as having genome-wide significant association; these and 6 additional previously reported loci were then tested in an independent set of 5,353 cases and 5,551 controls. Of the 32 tested SNPs, 24 replicated, including 6 newly identified loci. Conditional analyses within loci showed that four loci, including GBA, GAK-DGKQ, SNCA and the HLA region, contain a secondary independent risk variant. In total, we identified and replicated 28 independent risk variants for Parkinson's disease across 24 loci. Although the effect of each individual locus was small, risk profile analysis showed substantial cumulative risk in a comparison of the highest and lowest quintiles of genetic risk (odds ratio (OR) = 3.31, 95% confidence interval (CI) = 2.55-4.30; P = 2 × 10-16). Abstract

The exclusive production of proton-antiproton pairs in the collisions of two quasi-real photons has been studied using data taken at rootS(ee) = 183 GeV and 189 GeV with the OPAL detector at LEP. Results are presented for p (p) over bar invariant masses, W, in the range 2.15 < W < 3.95 GeV. The cross-section measurements are compared with previous data and with recent analytic calculations based on the quark-diquark model.

Abstract

Objective.To determine whether antimicrobial (AM) courses ordered with an antimicrobial computer decision support system (CDSS) were more likely to be appropriate than courses ordered without the CDSS.Design.Retrospective cohort study. Blinded expert reviewers judged whether AM courses were appropriate, considering drug selection, route, dose, and duration.Setting.A 279-bed university-affiliated Department of Veterans Affairs (VA) hospital.Patients.A 500-patient random sample of inpatients who received a therapeutic AM course between October 2007 and September 2008.Intervention.An optional CDSS, available at the point of order entry in the VA computerized patient record system.Results.CDSS courses were significantly more likely to be appropriate (111/254, 44%) compared with non-CDSS courses (81/246, 33%, P = .013). Courses were more likely to be appropriate when the initial provider diagnosis of the condition being treated was correct (168/273, 62%) than when it was incorrect, uncertain, or a sign or symptom rather than a disease (24/227, 11%, P< .001). In multivariable analysis, CDSS-ordered courses were more likely to be appropriate than non-CDSS-ordered courses (odds ratio [OR], 1.83; 95% confidence interval [CI], 1.13–2.98). Courses were also more likely to be judged appropriate when the initial provider diagnosis of the condition being treated was correct than when it was incorrect, uncertain, or a sign or symptom rather than a disease (OR, 3.56; 95% CI, 1.4-9.0).Conclusions.Use of the CDSS was associated with more appropriate AM use. To achieve greater improvements, strategies are needed to improve provider diagnoses of syndromes that are infectious or possibly infectious.
