In impoverished communities in Mexico, most adolescent mothers do not attend school; but typically, they become pregnant once they dropped out. Understanding the experiences of adolescents who have had a pregnancy and continue in school is complicated since few manage to do it. The goal of this study is to describe experiences within the family and school context and plans for the future of a sample of Mexican students who have had a pregnancy. We analyze information from the questionnaires of 68 women and 44 men and interviews to 6 women and 5 men. First intercourse was at age 15 and first pregnancy at 16; 39% of men and 57% of women already have a child; 54% of men and 19% of women work and study simultaneously. Dropping out because of a pregnancy/marriage was reported by 41% of women and 14% of men. The support of family and teachers is crucial to stay in school; but the institution remains indifferent to their plight. The immediate needs easily defeat their aspirations. It is fundamental to acknowledge and notice the experiences of these vulnerable young people in order to design focused strategies promoting their success in adult life.

To describe condom use among Mexican women living with HIV and analyze factors that facilitate or impede its utilization.Qualitative analysis of 55 interviews with women of reproductive age living with HIV.Inconsistent condom use and non-use at last sexual intercourse was common, and not clearly related to the male partners' HIV-status. Factors that influenced condom use included perceptions of health benefits, symbolic meaning assigned to the condom within the relationship, and the transformation or persistence of inequitable gender norms.Gender norms and male partners' attitudes strongly influence condom use among women living with HIV. To increase consistent condom use the health system must implement counseling and service delivery with a gender perspective and innovative actions to involve male partners.

Background: Most studies on parent-adolescent sexual health communication come from developed countries and are based on either parents’ or children’s reports. In developing countries, there is little evidence about the agreement among reports of all parties involved in parent-adolescent sexual health communication. Objective: The objective of this study is to explore the congruence (agreement) between adolescents and their parents about how frequently they discuss on selected sexual health topics. Subjects: A total of 1606 parent-adolescent dyads of adolescents attending the first year in public high schools and their parents, in Morelos, Mexico were sampled in this study. Methods: The participants completed a self-administered questionnaire that included the frequency of parent-adolescent communication about eight sexual health topics. An ordinal logistic threshold model was used to estimate intra-class correlation coefficients within parent-adolescent dyads (as a measure of congruence) and to test if thresholds were equal between parents and adolescents. Results: Congruence in reported frequency of parent-adolescent sexual health communication ranged from 0.205 (menstruation) to 0.307 (condoms) for mother-adolescent dyads, and from 0.103 (ejaculation) to 0.380 (condoms) for father-adolescent dyads. The thresholds (i.e., the cutoff points that define the categories in the observed ordinal variable) differed between parents and adolescents for each of the sexual health topics explored (p Conclusion: Our findings suggest a low congruence between parents’ and adolescents’ reports on parent-adolescent sexual health communication. This might be due to interpretation of frequency and intensity of sexual health communication which differs between parents and adolescents.

To evaluate the effects of psychosocial support during labour, delivery and the immediate postpartum period provided by a female companion (doula).The effects of the intervention were assessed by means of a randomised clinical trial. Social support by a doula was provided to women in the intervention group, while women in the control arm received routine care.A large social security hospital in Mexico City.Seven hundred and twenty-four women with a single fetus, no previous vaginal delivery,6 cm of cervical dilatation, and no indications for an elective caesarean section were randomly assigned to be accompanied by a doula, or to receive routine care.Breastfeeding practices, duration of labour, medical interventions, mother's emotional conditions, and newborn's health.Blinded interviewers obtained data from the clinical records, during encounters with women in the immediate postpartum period, and at their homes 40 days after birth. Relative risks and confidence intervals were estimated for all relevant outcomes.The frequency of exclusive breastfeeding one month after birth was significantly higher in the intervention group (RR 1.64; I-C: 1.01-2.64), as were the behaviours that promote breastfeeding. However, the programme did not achieve a significant effect on full breastfeeding. More women in the intervention group perceived a high degree of control over the delivery experience, and the duration of labour was shorter than in the control group (4.56 hours vs 5.58 hours; RR 1.07 CI (95%) = 1.52 to -0.51). There were no effects either on medical interventions, mothers' anxiety, self-esteem, perception of pain and satisfaction, or in newborns' conditions.Psychosocial support by doulas had a positive effect on breastfeeding and duration of labour. It had a more limited impact on medical interventions, perhaps because of the strict routine in hospital procedures, the cultural background of the women, the short duration of the intervention, and the profile of the doulas. It is important to include psychosocial support as a component of breastfeeding promotion strategies.Studies in numerous countries have documented the positive contributions of doulas--women experienced in childbirth who provide continuous physical, emotional, and informational support to women before, during, and just after childbirth. The present study, conducted in a Mexican Institute of Social Security public hospital, explored the hypothesis that psychosocial support from a doula increases exclusive and full breast feeding by improving the mother's emotional status, shortening the duration of labor, and decreasing medical intervention. 724 women with no previous vaginal delivery and no indications for cesarean section delivery were randomly assigned to be accompanied by a doula (n = 361) or to receive routine care (n = 363). Blinded interviewers obtained outcome data from the clinical records, encounters with mothers in the immediate postpartum period, and home visits 40 days after delivery. The frequency of exclusive breast feeding 1 month after birth was significantly higher in the intervention group than the control group (12% vs. 7%; relative risk (RR), 1.64; 95% confidence interval (CI), 1.01-2.64). However, the program did not achieve a significant effect on full breast feeding (37% and 36%, respectively). The duration of labor was shorter in the intervention group than the control group (4.56 vs. 5.58 hours; RR, 1.07; 95% CI, -1.52-0.51). A significantly larger proportion of women in the intervention group than the control group perceived a high level of control over labor (79.8% vs. 77.1%; RR, 1.14; 95% CI, 1.03-1.27). There were no effects on medical interventions, maternal anxiety, self-esteem, perception of pain, maternal satisfaction, or newborn Apgar scores. Although the prevalence of exclusive breast feeding was low in both groups, these findings suggest that psychosocial support during labor and the immediate postpartum period should be part of a comprehensive strategy to promote breast feeding.

This article presents some of the most relevant qualitative results of a trial to evaluate the effects of the provision of psychosocial support to first-time mothers during labor, childbirth and in the immediate postpartum period in a social security hospital in Mexico City. The article focuses on the experiences of mothers who have received psychosocial support from a doula (the term doula is used to identify a woman who provides continuous support to a woman during labor. delivery and the immediate postpartum period) and compares them with the experiences of those women who gave birth following normal hospital routine. Sixteen in-depth interviews were held with women in the immediate post partum period (eight of whom had been accompanied by a doula and eight who had not) before they were discharged from hospital, and the results were analyzed using qualitative techniques. The interviews showed that the women accompanied by a doula had a more positive childbirth experience. The differences between both groups related to their perceptions of the childbirth experience; the treatment they received from hospital staff; the information they were given and how well they understood it; their perception of hospital routines; their feelings about cesarean sections and, spatial and temporal perceptions. The most important difference between the two groups was the way they expressed their feelings about their own labor, their sense of control and their self-perception.

Objetivos. Ahondar sobre las causas y circunstancias particulares en que ocurren las experiencias sexuales de los migrantes centroamericanos en su tránsito por México e identificar cómo afecta esto al ejercicio de sus derechos sexuales. Material y métodos. Estudio cualitativo (2009- 2010). Se realizaron 22 entrevistas semiestructuradas a migrantes centroamericanos en tránsito por México y 10 a diversos actores clave. La sistematización, procesamiento y análisis de los datos se realizó a partir de algunos de los elementos de la teoría fundamentada. Resultados. Las experiencias sexuales de los migrantes en tránsito por México están vinculadas con aspectos de coerción, abuso, inequidad y violencia sexual. Los migrantes identifican riesgos a los que se enfrentan en el tránsito y tienen escaso acceso a servicios que les permitan hacer efectivos sus derechos, incluidos los sexuales. Su vulnerabilidad los hace asumir la violencia y las agresiones sexuales como parte ineludible del trayecto y de su destino como personas. Conclusiones. El tránsito de migrantes está relacionado con la violencia, incluida la sexual. Las diferencias entre migrantes y otros grupos, así como entre hombres y mujeres, son determinadas por las relaciones inequitativas de género y poder.

To describe the trend of cesarean section practice in Mexico, and its association with women's sociodemographic and reproductive characteristics.Based on the 2000, 2006 and 2012 National Health Surveys, information on c-section was analyzed. A multivariate logistic regression model was used in 2012.A 50.3% increase in the use of c-section was observed nationally from 2000 to 2012. Women more likely to undergo a c-section include those whose delivery care takes place in the private sector (OR=2.84, 95%CI:2.15-3.74). When associating women's age and parity, the greatest risks are observed among primiparous women between 12 and 19 years of age, and those aged 35 years and more (OR=6.02, 95%CI:1.24-29.26 and OR=5.20, 95%CI:2.41-11.21, respectively).Some recommendations to revert the increase of this clinical practice, especially when there is no full justification for its realization, are proposed.

Background In Mexico 87% of births are attended by physicians. However, the decline in the national maternal mortality rate has been slower than expected. The Mexican Ministry of Health’s 2009 strategy to reduce maternal mortality gives a role to two non-physician models that meet criteria for skilled attendants: obstetric nurses and professional midwives. This study compares and contrasts these two provider types with the medical model, analyzing perspectives on their respective training, scope of practice, and also their perception and/or experiences with integration into the public system as skilled birth attendants. Methodology This paper synthesizes qualitative research that was obtained as a component of the quantitative and qualitative study that evaluated three models of obstetric care: professional midwives (PM), obstetric nurses (ON) and general physicians (GP). A total of 27 individual interviews using a semi-structured guide were carried out with PMs, ONs, GPs and specialists. Interviews were transcribed following the principles of grounded theory, codes and categories were created as they emerged from the data. We analyzed data in ATLAS.ti. Results All provider types interviewed expressed confidence in their professional training and acknowledge that both professional midwives and obstetric nurses have the necessary skills and knowledge to care for women during normal pregnancy and childbirth. The three types of providers recognize limits to their practice, namely in the area of managing complications. We found differences in how each type of practitioner perceived the concept and process of birth and their role in this process. The barriers to incorporation as a model to attend birth faced by PMs and ONs are at the individual, hospital and system level. GPs question their ability and training to handle deliveries, in particular those that become complicated, and the professional midwifery model particularly as it relates to a clinical setting, is also questioned. Conclusions Hospitals in the Mexican public health sector have a heavy obstetric workload; physicians carry the additional burden of non-obstetric cases. The incorporation of a non- physician model at the primary health center level to attend low-risk, normal deliveries would contribute to the reduction of non-necessary referrals. There is also a role for these providers at the hospital level.

To explore the causes and circumstances that determine the way in which migrants experience their sexuality and how this impacts their sexual rights.Qualitative study conducted between April 2009 and July 2010 in Chiapas, Oaxaca, San Luis Potosí, and Tamaulipas. We conducted 22 in-depth interviews to migrants in transit and to ten different key actors. For the analysis we used elements of grounded theory.Migrants know and identify the risks they may encounter in their transit but have scarce access to services to effectively exercise their sexual and reproductive rights. Their vulnerability makes them internalize and accept the violence enacted on them as part of their destiny and as what they must suffer in order to reach the USA.Violence, including sexual violence, determines much of the experiences of their transit through Mexico. Differences between groups and between male and female migrants are determined by gender inequalities and power.

To examine the effects of educational interventions that incorporate parent participation to modify adolescent sexual behavior.We conducted a systematic search of 5 electronic databases for articles published between 2002 and 2009 that evaluated educational interventions involving parents.Nineteen articles evaluated 15 interventions that met all the criteria for inclusion. They found a significant increase in adolescents' intentions to postpone sexual intercourse and use contraceptives, a reduction in self-reported sexual relations and an increase in condom use. Positive results were also found for individual protective factors such as knowledge and attitude, and family factors such as parent-child communication, perception of rules, monitoring/supervision by parents and family support.Educational interventions that include parents support healthy sexual behavior among adolescents. Parental participation can be included in any intervention aimed at adolescents.

In this article reporting on our qualitative study, we describe changes in parent—adolescent sexual health communication following an intervention for parents of 10th graders in Mexico. The intervention was aimed to sensitize and develop skills for appropriate parent—child communication about the prevention of sexually transmitted infections (STIs), unplanned pregnancy, and birth control, and focused on encouraging condom use with emergency contraception backup. We conducted 66 in-depth interviews with adolescents and parents after the intervention. Following the principals of grounded theory, we carried out inductive analysis to create codes and organize central themes. Our findings identify previously undefined critical steps or movements important for parent—child communication about sex. When parents are sensitized to the risks their adolescent children face, it is easier to initiate communication about prevention. These initial movements are essential to achieve substantive conversation, and must be considered in future strategies that aim to promote parent—adolescent communication about sexuality and the prevention of STIs and unplanned pregnancy.

Introducción: Aun cuando las cifras de infección en México son mayores entre hombres que en mujeres (71.5 y 28.5% respectivamente), el análisis de la epidemia desde un enfoque de género arroja luz sobre posibles similitudes y diferencias en la experiencia de vivir con VIH/SIDA para ambos. El interés del presente estudio se centró en identificar diferencias de género en el soporte emocional y social proveniente de las redes de apoyo de estas personas. En este artículo nos referiremos a dos situaciones particulares: primero, la percepción de la manera en que se adquirió el virus, lo cual sirve como ejemplo de la construcción social del género que impacta en las emociones y vivencias de la enfermedad según se sea mujer u hombre, y segundo, el proceso de transformación por el que las personas con VIH reorganizan sus necesidades y circunstancias después del diagnóstico.

Objective To present, through an evaluation in Latin America and the Caribbean, the current regulatory and curricular landscape of sexuality and HIV prevention in the school setting, and contrast it with sexual behaviors as reported by demographic and health surveys. Methods In May 2008, with the support of the United Nations Population Fund (UNFPA), a survey was conducted involving key contacts in 34 countries of the Region. The self-administered questionnaire sought in-depth information on school-based HIV/AIDS prevention programs from the staff of various stakeholders, including ministries of education and of health. Results Twenty-seven countries responded to the survey, representing 95.5% of the target population (6-18 years of age). Most countries reported having at least one textbook or one specific chapter devoted to teaching sex education and HIV prevention. Many, but not all of the important sex education topics, were covered in secondary school; for example, the issue of discrimination based on sexual orientation/ preference is not included in school curricula. Conclusions Sex education materials should be reviewed and updated periodically to reflect progress being made on the issues and the handling of the subject matter. In each country, the curriculum should address the issue of respecting differences in sexual orientation, preference, and identity, and especially, an appropriate approach toward sexually-transmitted infection (STI) prevention education among males who have sex with males. Efforts to assess the programs' effectiveness should consider outcomes, such as biomarkers (STI incidence and prevalence and pregnancy), not just indicators of self-reported sexual knowledge and behavior.

Objective To evaluate the effectiveness of an educational intervention for parents of first year high school students in the State of Morelos, Mexico, whose aim was to impact adolescents’ sexual behavior, knowledge and access to contraception. Material and methods Quasi-experimental prospective study with eleven control and eleven intervention schools using self-administered questionnaires for parents and adolescents pre- and post-intervention. Parent–child dyads in the control and intervention schools were matched according to parents’ propensity score; the average treatment effect (ATE) was estimated for adolescent’s outcome variables. Results At follow-up, we found significant differences for adolescents in the intervention schools: 6.8% delayed initiation of sexual intercourse, 14.4% had correct knowledge about emergency contraception (EC), and 164% reported having received condoms from their parents, when comparing with students in control schools. Conclusions Our results suggest that parent-focused interventions could be an innovative and effective strategy to promote adolescents sexual health.

To analyze experiences of heterosexual Mexican people living with HIV (PLWHA) related to the exercise of their sexual and reproductive rights.Qualitative study based on 40 in-depth interviews with PLWHA in four Mexican States.Lack of support and counseling limits the exercise of sexual and reproductive rights by PLWHA, especially women. Principal limitations include feelings of frustration and confusion, fear of re-infection, scanty information, lack of power to negotiate condom use, social stigma and discrimination, and limited access to services and adequate technologies.To increase awareness and exercise of rights by PLWHA it is necessary to: improve sexual education; promote processes of cultural change to combat gender inequality and stigma and discrimination; and provide timely and scientific information about HIV prevention.

The prevalence of obesity in childhood and adolescence has increased in Mexico. This increase can be explained partly by a decrease in physical activity. The objective of this study was to assess the association between the prevalence of obesity and overweight with physical activity and inactivity among Mexican adolescents. We analyzed data from a longitudinal study with baseline and follow-up measures in 446 adolescents 12 to 17 years old, attending public schools in Cuernavaca, Morelos, Mexico. We collected information on weight, physical activity and demographic characteristics. Obesity and overweight were determined using as indicador the body mass index, and the cutoff points proposed by the International Obesity Task Force. The baseline prevalence of obesity or overweight was 37.22% (42.99% for males and 30.95% for females), and in the final measure 34.30% (38.46% for males and 32.28% for females). The median time of TV viewing was 3.90 h/d, with 0.73 h/d dedicated to vigorous physical activity and 0.58 h/d to moderate physical activity. Using logistic regression, after adjustment for obesity in the baseline measure, sex and clustering by school, we found no significant associations between the prevalence of obesity or overweight with time dedicated to TV viewing, vigorous or moderate physical activity. However, when conducting the analysis with an interaction term by sex, we found significantly lower risk of obesity or overweight associated with an increase in vigorous physical activity among males only (beta = 0.22, 95% CI (0.05, 0.89). Results suggest that vigorous physical activity may reduce the risk of obesity or overweight among male adolescents.

OBJETIVO: Identificar la influencia de las caracteristicas reproductivas y el empoderamiento femenino en el uso de servicios de planificacion familiar (PF). MATERIAL Y METODOS: Diseno de casos y no casos pareado por controles vecindarios (147 usuarias y 146 no usuarias de servicios de PF de la Secretaria de Salud durante 2003), en dos municipios del estado de Guanajuato, Mexico. Analisis logistico multivariado para identificar diferencias entre usuarias y no usuarias. RESULTADOS: El uso de servicios de PF se asocio positiva y significativamente con poder de decision de la mujer (alto: RM=3.2; IC95% 1.4-7.4); comunicacion con la pareja para el uso de metodos de PF (RM =3.5; IC95% 1.4-9.3), y numero de embarazos en su vida (> 6 hijos: RM =4.4; IC95% 1.4-13.8). CONCLUSIONES: El desarrollo de estrategias que involucren a los hombres y que, por otra parte, fomenten y fortalezcan el empoderamiento femenino, puede contribuir a una mayor utilizacion de servicios de PF.

To identify the influence of reproductive characteristics and women empowerment with the use of family planning services.Cases and non-cases design with neighborhood controls (147 users and 146 non-users of family planning services during 2003), in Guanajuato State, Mexico. Various indexes were constructed to evaluate women's empowerment and its relationship with family planning use.The use of family planning services was positively and significantly associated with the woman's power to make decisions (High: OR 3.2, CI95% 1.4-7.4), a high level of communication with her partner on contraceptive use (OR 3.5, CI95% 1.4-9.3); and a greater number of pregnancies (6 children: OR 4.4, CI95% 1.4-13.8).Factors such as a high level of female decision-making and more partner support for contraceptive use are related to the use of family planning services. Therefore, developing strategies that involve men and support female empowerment could contribute to increasing the use of family planning services.

Unprotected sex between men is the major risk factor for HIV infection in Mexico and many other Latin American countries. There is a substantial body of literature demonstrating that the relationship between sexual identity and sexual practice is not binary or causal -- men who have sex with other men do not necessarily perceive themselves as gay -- and there is increasing interest in HIV prevention with men who have sex with both men and women. In Mexico, HIV prevention with men who have sex with women and men and who are not socially affiliated or identified with gay men is lacking. This paper explores the sexual histories and HIV-risk perception of HIV-positive Mexican men who indicated that they have sex with women in a screening interview and then in the context of an in-depth interview also reported having had sex with men. We consider the sexual practices and sexual and social identities of these men, examining their explanations for having sex with other men, the strategies used to affirm their masculinity, the management of their sexual identity in their social networks, HIV-risk perception before diagnosis and sexual practices after diagnosis. Recommendations are made to improve HIV prevention for men who have sex with men as well as women and who do not assume a gay or bisexual identity.

Objetivo. Describir prácticas de uso del condón en mujeres mexicanas con VIH y analizar factores que lo facilitan u obstaculizan. Material y métodos. Análisis cualitativo de 55 entrevistas con mujeres que viven con VIH en edad reproductiva. Resultados. El uso inconsistente y el no uso del condón en la última relación sexual fueron comunes y no mostraron una relación clara con el estatus serológico de la pareja. Factores favorables al uso del condón fueron las percepciones de su beneficio para la salud, significados simbólicos del condón en relaciones de pareja y transformación o persistencia de normas de género. Conclusiones. Las normas de género y las actitudes de la pareja masculina intervienen en el uso del condón entre mujeres con VIH. Para incrementar su uso consistente, el sistema de salud debe ofrecer consejería y servicios con perspectiva de género e implementar acciones innovadoras que involucren a las parejas masculinas.

To assess the influence of the relationship between people living with HIV/AIDS and health care providers on treatment adherence and quality of life.Qualitative study conducted in Mexico, in 2003, with informants recruited from HIV/AIDS support groups, non-governmental organizations, and infectious disease clinics in public hospitals. A total of 40 people living with HIV/AIDS and five support group leaders were interviewed. Data were collected using sociodemographic questionnaires and in-depth interviews exploring the experience of living with HIV/AIDS, including treatment, disease perception, quality of life, physical and emotional health, and relationship with physicians. Responses were recorded, transcribed and organized thematically based on codes using an inductive analysis.Most respondents aged between 26 and 45 years. The main obstacles to care were related to lack of risk awareness and information among people living with HIV/AIDS and physicians. Physicians proved not to be well trained on HIV/AIDS care. During the follow-up care, most problems were related to inadequate treatment adherence mainly caused by poor communication with physicians and discrimination in public services.Overall the problems identified were related to information deficiencies, physicians' lack of training and paternalism, and social stigmatization of people living with HIV/AIDS associated with the epidemic.

To document the under-registration of violent deaths related to pregnancy and the importance of considering these violent deaths within the definition of maternal mortality.The study was carried out in the state of Morelos, based on the review of all death certificates (394) of reproductive aged women (12-49 years) who died during 2001. Based on a list of diagnostic criteria we eliminated 167 certificates that were neither violent deaths nor maternal deaths. The remaining 227 certificates were further evaluated through verbal autopsy and/or review of medical charts.Fifty-one violent deaths were found. Eighteen maternal deaths were officially reported in 2001, however, our study identified 23 direct maternal deaths and four violent deaths during pregnancy and the post-partum period. We found that this reproductive event was the direct trigger for the homicide or suicide of these four women, and only one of these cases was documented officially.Violent deaths related to pregnancy should be included in official maternal mortality statistics as indirect causes of maternal deaths. This would allow for a greater and more accurate understanding of violent maternal deaths and guide appropriate prevention and care policies, programs and services. Verbal autopsy is a useful technique for identifying cases of violent maternal deaths.

To describe the trends in caesarian sections (CS) associated with non-clinical factors in women who attended a birthing educational facility, also known as 'Centro de Educación,' (CEPAPAR) in Mexico City.Data from 992 births of 847 women that occurred from 1988-2000 were analyzed using linear regression to identify the association between non-clinical factors and time-trends in CS.The overall incidence of CS was 32.8%, with an 8% average annual increase. Until 1994 the percentage of CS remained relatively steady at 30%, increasing to 40% in subsequent years. In analyzing trends in CS and their relationship with non-clinical factors from 1988-2000, the increased incidence of CS during this period was higher among births where no birth attendant was present, for first-time mothers, and in larger hospitals (50 beds).Results show that the increased incidence of CS is associated with non-clinical factors such as size of the hospital and the presence of a birth attendant. A model of support for women which includes information, counseling, and the presence of a birth attendant during labor can contribute to reducing the risk of unnecessary CS in populations such as that studied.

Documentar el subregistro de muertes violentas relacionadas con el embarazo y la importancia de considerar, dentro de la definición de mortalidad materna, aquellas muertes producidas también por causas relacionadas con violencia. Material y métodos. El estudio se realizó en el estado de Morelos, a partir de 394 certificados de defunción de mujeres entre 12 y 49 años de edad que murieron a lo largo del año 2001. Con base en una lista de diagnósticos de los certificados de defunción, se excluyeron 167 casos que, por las causas de muerte determinadas en el certificado de defunción, no se consideraron que podrían haber sido muertes maternas ni muertes violentas. Posteriormente se realizó el análisis de los 227 certificados restantes a través de la revisión de expedientes clínicos y/o autopsias verbales. Resultados. Se encontraron 51 muertes violentas. Las estadísticas oficiales señalan que en 2001 hubo 18 muertes maternas, mientras que este estudio identificó 23 muertas maternas directas más cuatro muertas violentas durante el embarazo y el posparto. Es decir, se encontró que el evento reproductivo fue el factor que desencadenó el homicidio o suicidio de cuatro mujeres. Excepto en un caso, este hecho no está señalado ni relacionado en los registros oficiales. Conclusiones. Se recomienda la inclusión de la violencia relacionada con la reproducción en los registros oficiales de mortalidad materna como una causa indirecta. Esto permitiría profundizar la comprensión de las causas de la mortalidad materna y orientaría la elaboración de políticas, programas y servicios de prevención y atención. La autopsia verbal (AV) es una técnica que ayuda a la identificación de casos de embarazo y muertes maternas violentas.

This study measured the contribution of abortion-related deaths to overall maternal mortality and calculated the underestimation of maternal mortality using verbal autopsy and clinical record review where available. We reviewed 807 death certificates of women aged 12–50 who died in 2001 in two sites of about 1.5 million inhabitants each in the state of Morelos (primarily rural) and the municipality of Nezahualcoyotl (primarily urban) in the state of Mexico. Deaths were classified as definite, possible or non-maternal deaths. Finally, we identified abortion-related deaths and calculated the underestimation of maternal mortality. Among 326 possible maternal deaths, we encountered five misclassified cases: one spontaneous abortion and four non-abortion maternal deaths. Among 32 registered maternal deaths, we found four misclassified cases that were actually second trimester, abortion-related deaths. There were no officially registered abortion-related deaths in either Morelos or Nezahualcoyotl, making the overall underestimation of abortion mortality 100%. Abortion contributed 13.5% of all maternal deaths. The overall underestimation of maternal mortality was 13.5%, higher in Morelos (21.7%). There were no unregistered maternal deaths in Nezahualcoyotl. Unsafe abortion continues to be an important cause of maternal mortality, though first trimester deaths appear to be decreasing. We identified domestic violence as an important cause of death among pregnant and post-partum women, and two abortion-related suicides, and believe these should be reconsidered as indirect maternal deaths. The misclassification of second trimester abortion deaths as maternal deaths from other causes is an obstacle to preventing them.

This paper presents some thoughts on the problems endured by women facing HIV/AIDS. World and nationwide statistics show an upward trend of the epidemic among women, calling for the need to address this problem. A case is made for the way in which gender structure and relations facilitate the vulnerability of women to this disease, increasing their risk of infection related with specific factors like migration, employment unsafe conditions, and domestic violence, among others. Emphasis is made on the main obstacles faced by women to flag their concerns and needs in society, as well as on their lack of empowerment to cope effectively with their problems. Some initiatives advanced by the organized civil society and governmental strategies are presented, including accomplishments, deficiencies, needs, and challenges. Finally, conclusions and recommendations are given, highlighting the need to establish and evaluate preventative programs and policies from a gender and human rights perspective, to promote a greater awareness and improved responses to the problems imposed on women by the HIV/AIDS epidemic.

This report presents the main qualitative results of a verbal autopsy study carried out in three states of Mexico, which aimed at identifying the factors associated with maternal mortality that could be subject to modifications through concrete interventions. By reviewing death certificates issued in 1995, it was possible to identify 164 households where a maternal death had occurred. One hundred forty-five of these households were visited, and a precoded questionnaire was completed to explore socioeconomic and living conditions, as well as causes of death. An open-ended question to prompt the relatives to narrate all the facts that led to the maternal deaths was included in the questionnaire. This study presents an analysis of that question, focusing on the delays in the care-seeking process and organized according to the model of the three delays: in deciding to seek care, in reaching a care facility, and in actually receiving care after arrival. Additionally, problems related to quality of care are examined. For analysis of the accounts, structural, interactional/community, and subjective variables were identified that allowed refining of our understanding of the problem of maternal deaths. Finally, based on the findings of the study, this article presents a series of recommendations, highlighting that interventions should address the early stages of a complication and focus on decreasing the various forms of inequality (gender and socioeconomic) associated with the occurrence of maternal deaths.

Objetivo. Se llevó a cabo un estudio exploratorio para profundizar en aspectos relativos a la aceptabilidad y el apego al consumo de suplementos nutricios que forman parte de un programa gubernamental de apoyo a la nutrición, dirigido a grupos vulnerables. Material y métodos. Se entrevistó a mujeres embarazadas o en lactancia, así como a madres de niños lactantes, niños destetados y niños menores de dos años con desnutrición. Los datos se recolectaron mediante visitas al hogar, aplicando una guía de entrevista con formato libre. Las entrevistas se grabaron previo consentimiento informado de las mujeres. El análisis incluyó temas y subtemas que abordaron las entrevistadas. Resultados. En general hubo una buena aceptabilidad al suplemento cuando se ofreció en forma de bebida. Al considerar sus beneficios, las mujeres embarazadas antepusieron la salud del bebé a la propia y asociaron el valor del suplemento con su contenido de “vitaminas”. La aceptación a los sabores que se les ofrecieron se relacionó con la costumbre de consumirlos. Los niños menores de un año de edad prefirieron la consistencia líquida, y los mayores, la papilla. La opinión general fue que la cantidad ofrecida era suficiente, y se comentó que conforme transcurría el tiempo de intervención, aumentaba el apetito. No hubo efecto de sustitución de la dieta por el suplemento. Conclusiones. La aceptabilidad al suplemento puede fortalecerse mediante mensajes orientados al bienestar y la salud del niño, introduciendo la noción de que el suplemento también es benéfico para la mujer gestante. Se recomienda involucrar a los proveedores de salud de las comunidades a fin de que ayuden a lograr un mejor consumo y aceptabilidad del producto. La identificación del contenido de “vitaminas” por parte de las madres puede utilizarse para reforzar el concepto de los beneficios del consumo de micronutrientes. Es importante llevar a cabo evaluaciones cualitativas como parte de la evaluación de proceso del programa

La categoría de "apoyo social" ha permitido desarrollar toda una corriente de investigación que busca explicar algunas de las diferencias existentes en la distribución de ciertas enfermedades tanto físicas como mentales. El concepto de apoyo social presenta varios problemas que es preciso resolver. Este artículo discute algunos de estos problemas, y mostra la posibilidad de enriquecer el tema desde una perspectiva sociológica. Se discute el origen conceptual del término y se analizan diversas definiciones del mismo. Se analizar las formas en que la investigación reciente ha tratado de vincular la disponibilidad de apoyo social con la existencia de ciertos niveles de salud y enfermedad. Se discuten las diferencias entre el modelo del efecto directo y el modelo del efecto amortiguador. Se muestra la necesidad de estudiar al apoyo social no sólo como posible determinante de ciertas formas de enfermedad, sino también como una variable dependiente. Se subraya la importancia de buscar las determinantes del apoyo social en todos los niveles de la realidad social, dando especial importancia a las variables de clase social y género. Se presenta una discusión sobre los avances logrados en el estudio de los factores que se asocian a la disponibilidad de apoyo social por parte de los individuos. Se muestra que el apoyo social es a su vez producto de ciertos ordenamientos sociales que pueden ser identificados con claridad a través de un análisis sociológico del fenómeno. Para ilustrar este argumento, se discute el caso de la atención, entendida como una forma de apoyo social, y se muestra que la distribución de la misma está determinada socialmente. Se finaliza con una reflexión sobre la necesidad de integrar diversos enfoques teórico-metodológicos como condición necesaria para lograr nuevos avances en el estudio del apoyo social On the borderline between sociology and psychology, the concept of social support has made it possible to develop a whole research tradition aimed at explaining some of the differences existing in the distribution of mental and physical disease. The concept of social support itself, however, still presents some ambiguities which must be clarified. These problems are discussed and it is showed and shows that the topic can be enriched with the contribution of the social sciences. Different definitions of the concept itself, are briefly analysed and the complexity and multidimensional character of social support are highlighted. The association between social support and health is discussed. The differences and implications of the direct effect model and the buffering effect model are addressed, and the relevance of the construct of social support is enhanced. The need to study social support not only as a likely determinant of some forms of disease, but also as a dependent variable, is brought out. Some recent developments in the study of factors associated with the availability of social support are discussed. It is shown that social support is a product of certain social factors which can be clearly identified by sociological analysis. In order to illustrate this point, the structural determinants of a specific dimension of social support -attention- are discussed. The importance of structural variables like social class and gender in attempting to explain the differential distribution of certain forms of social support is emphasized. It is proposed, on the theoretical level, that the intersection between two related autonomous constructs -social class and social support- may better explain those aspects of these concepts that are of importance to observed heterogeneity in the distribution of mental and physical health. Different theoretical and methodological approaches to the understanding of the determinants of social support and its effect on health are suggested.

Objetivo. Describir la tendencia de la cesárea en México y su asociación con características sociodemográficas y reproductivas. Material y métodos. Con base en Encuestas Nacionales de Salud 2000, 2006 y 2012, se analizó información de cesáreas en mujeres. Se utilizó un modelo de regresión logística multivariado en 2012. Resultados. Se identificó un incremento de 50.3% de la operación cesárea a nivel nacional en el periodo de 2000 a 2012. Las mujeres con mayor posibilidad de tener una cesárea fueron las que se atendieron en el sector privado (RM=2.84, 95%IC:2.15- 3.74). Al asociar la edad y la paridad se observan los riesgos más altos en primíparas de 12 a 19 y de 35 o más años (RM=6.02, 95%IC:1.24-29.26 y RM=5.20, 95%IC:2.41-11.21, respectivamente). Conclusiones. Se proponen algunas recomendaciones encaminadas a revertir el incremento de esta práctica clínica, sobre todo en aquellos casos donde no hay una indicación precisa para su realización.

Objetivo. Analizar la percepción de mujeres y proveedores de salud sobre cuándo y cómo realizar acciones para la detección temprana del cáncer de mama y cervicouterino en localidades de Morelos con presencia de población indígena. Material y métodos. Se entrevistó a 10 proveedores de salud y 58 usuarias en unidades médicas del primer nivel de atención de cinco localidades; luego se analizó la información con base en el paradigma de la teoría fundamentada. Resultados. El personal de salud está deficientemente familiarizado con los lineamientos oficiales para la detección de cáncer cervicouterino y de mama. Pocos practican sus labores bajo una perspectiva de sensibilización intercultural. Las usuarias tienen nociones imprecisas o equivocadas de las acciones de detección. Conclusiones. La necesidad de capacitación con apego a las normas es evidente. Urge asumir un abordaje con pertinencia cultural que permita la comunicación eficiente y alfabetización en salud para la detección oportuna de estos dos cánceres.

Revisar los efectos de intervenciones educativas que incorporan a padres de familia, para modificar el comportamiento sexual de sus hijos adolescentes. Material y métodos. Búsqueda sistemática en cinco bases de datos electrónicas de estudios de evaluación de intervenciones educativas con padres (2002-2009). Resultados. Diecinueve publicaciones evalúan a 15 programas que cumplen con los criterios de selección. Éstas encuentran un aumento en las intenciones de retrasar las relaciones sexuales y de usar anticonceptivos, reducción en el auto-reporte de relaciones sexuales e incremento en el uso de condón. También se encuentran resultados positivos en factores individuales protectores como conocimientos y actitudes; y en factores familiares como comunicación padres-hijos, percepción de reglas, monitoreo/supervisión de los padres y apoyo familiar. Conclusiones. Las intervenciones educativas que incluyen a padres de familia favorecen comportamientos sexuales saludables en los adolescentes. La participación de padres es un elemento que puede ser incluido en cualquier intervención para adolescentes.

Objetivo. Explorar las opiniones sobre el embarazo adolescente y su posible relación con comportamientos sexuales en varones adolescentes de escuelas públicas del estado de Morelos y de la Ciudad de México. Material y métodos. Estudio transversal con cuestionarios autoadministrados a una muestra no probabilística de varones (15-19 años) de ocho secundarias y preparatorias. Se obtuvieron estadísticas descriptivas y modelos multivariados para identificar la asociación entre las opiniones y el comportamiento sexual. Resultados. El 68% identifica al embarazo adolescente como un evento malo. Si hoy embarazaran a alguien, 56% continuaría en la escuela y 18% abandonaría sus estudios. Aquellos que consideran al embarazo adolescente como un evento muy malo tienen mayor posibilidad de usar condón (RM=1.8; p menor que 0.05). Conclusión. Las opiniones de varones muestran constantes contradicciones e incluso algunas de sus opiniones aumentan o disminuyen el riesgo de un embarazo. El diseño y validación de instrumentos exclusivos para varones es imprescindible.
