Abstract

This study investigated effects of healthy ageing and of non-verbal attentional control on speech production. Young and older speakers participated in a picture-word interference (PWI) task with compound targets. To increase the processing load, the two pictures of the compounds' constituents were presented side-by-side for spoken naming (e.g., a picture of a sun + a picture of a flower to be named with sunflower). Written distractors either corresponded to the first or second constituent (sun or flower → sunflower), or were semantically related either to the first constituent of the target (moon → sunflower) or to the second constituent/whole word (tulip → sunflower). Morpho-phonological facilitation was obtained for both constituents, whereas semantic interference was restricted to first-constituent-related semantic distractors. Furthermore, a trend towards facilitation was obtained for distractors that were semantically related to the whole word. Older speakers were slower and produced more errors than young speakers. While morphological effects of first-constituent distractors were stronger for the elderly, the semantic effects were not affected by age. Non-verbal attentional control processes, measured in the Simon task, significantly contributed to morpho-phonological priming in the elderly, but they did not affect semantic interference or semantic facilitation. With a picture naming task that increases the semantic and lexical processing load, we corroborate earlier evidence that word-finding difficulties in the elderly result from deficient phonological encoding, whereas lexical-semantic and morpho-phonological representations remain stable with age. 57. Semantically Transparent and Opaque Compounds in German Noun-Phrase Production: Evidence for Morphemes in Speaking

Abstract

This study examines the lexical representation and processing of noun-noun compounds and their grammatical gender during speech production in German, a language that codes for grammatical gender (masculine, feminine, and neuter). Using a picture-word interference paradigm, participants produced determiner-compound noun phrases in response to pictures, while ignoring written distractor words. Compound targets were either semantically transparent (e.g., birdhouse) or opaque (e.g., hotdog), and their constituent nouns either had the same or a different gender (internal gender match). Effects of gender-congruent but otherwise unrelated distractor nouns, and of two morphologically related distractors corresponding to the first or second constituent were assessed relative to a completely unrelated, gender-incongruent distractor baseline. Both constituent distractors strongly facilitated compound naming, and these effects were independent of the targets' semantic transparency. This supports retrieval of constituent morphemes for semantically transparent and opaque compounds during speech production. Furthermore, gender congruency between compounds and distractors did not speed up naming in general, but interacted with gender match of the compounds' constituent nouns, and their semantic transparency. A significant gender-congruency effect was obtained with semantically transparent compounds, consisting of two constituent nouns of the same gender, only. In principle, this pattern is compatible with a multiple lemma representation account for semantically transparent, but not for opaque compounds. Abstract

Background: In picture naming, both unimpaired and impaired speakers are usually better in naming singular than plural forms of the same noun, such as cat/cats. This singular-advantage is especially present in the case of singular-dominant nouns (e.g., table has higher surface frequency compared to its corresponding plural tables). However, for plural-dominant items (e.g., eyes has a higher surface frequency compared to eye) such singular advantage disappears. Thus, the lexical representation/processing of singular-dominant and plural-dominant nouns seem to differ but the exact underlying source for this dominance effect is still a matter of debate. While most of the available data stem from English experiments, less data are available from German. Aims: This study examines the effect of plural dominance for the German - n plural, a plural form, which can be predictable or non-predictable on the basis of the ending of the singular word form and its grammatical gender. Hence, this study examines the role of dominance and predictability of plural production in aphasia. Our data will enrich the development of materials for the assessment of morpho-lexical impairments in aphasia. Methods & Procedures: In a case-series design, five people with aphasia with severe word-finding difficulties participated in two picture naming tasks with single- and multiple-depictions of objects. Materials included nouns of the German -n plural type. Exp. 1 tested for effects of number and plural dominance in naming fully predictable -n plurals and their corresponding singulars. Exp. 2 tested for effects of number and predictability, using subsets of fully predictable and non-predictable -n plurals, and their corresponding singulars. Outcomes & Results: Exp. 1 revealed a significant plural dominance effect in spoken picture naming across five German speakers with aphasia: a singular advantage was observed for singular-dominant nouns, but plural-dominant singular and plural nouns did not differ. Further, in Exp. 2, no effect of predictability for the German plural affix -n was found, but an overall singular advantage across both groups. Conclusions: We interpret the dominance pattern of Exp.1 as manifestation in the links between concept and lemma level for singular- and plural-dominant nouns. Exp. 2, confirmed the singular advantage for singular-dominant nouns for both -n plural groups, indicating that both -n plural groups follow one plural production mechanism, however, we cannot be confident about the type of mechanism. Auszug: In der psycho- und neurolinguistischen Morphologieforschung wird die Frage behandelt, wie polymorphematische Wörter, d. h. Wörter, die aus mehr als einem Morphem bestehen (z. B. Apfel-baum; Be-mal-ung, mal-e), im mentalen Lexikon repräsentiert sind und wie sie verarbeitet werden. Spielt die interne morphologische Wortstruktur dabei überhaupt eine Rolle oder sind solche Wörter ganzheitlich repräsentiert? Die beiden großen konkurrierenden Theorien zur Verarbeitung polymorphematischer Wörter sind die Dekompositionshypothese und die Auflistungshypothese. Nach der Dekompositionshypothese werden morphologisch komplexe Wörter bei der rezeptiven Worterkennung in ihre Einzelteile aufgespalten (dekomponiert), beim expressiven Wortabruf müssen die zugrunde liegenden Morpheme einzeln vom Lexikon abgerufen und zu einer Vollform zusammengesetzt (komponiert) werden (z. B. Taft & Forster 1976). Im Unterschied dazu besagt die Auflistungshypothese, dass komplexe Wörter als Vollformen im Lexikon repräsentiert sind und abgerufen werden (Butterworth 1983). Wortbildungsregeln kommen nach der Dekompositionshypothese also grundsätzlich zum Einsatz, während nach der Auflistungshypothese morphologische Prozesse nur bei der Verarbeitung von unbekannten Vollformen oder bei der Bildung neuer Vollformen ablaufen. 63. Die Behandlung von Wortabrufstörungen bei Aphasie : eine methodenvergleichende Studie zum Bildbenennen

Abstract

In einer multiplen Einzelfallstudie mit zehn aphasischen Patienten wurde die Wirksamkeit eines semantischen und eines phonologischen Therapieansatzes zur Behandlung von Wortabrufstörungen verglichen. Detaillierte Einzelfalluntersuchungen ermöglichten die Diagnose der zugrundeliegenden funktionalen Störungen bei jedem Patienten. Auf diese Weise konnten die erzielten Therapieeffekte auf die individuellen kognitiv-neurolinguistischen Störungsmuster bezogen werden. Im Vordergrund der Therapie stand in beiden Ansätzen das mündliche Benennen von Objektabbildungen mit unterschiedlichen Arten von Hilfen. Während in der semantischen Therapie Teilaspekte des semantischen Zielkonzepts als Benennhilfen eingesetzt wurden, handelte es sich bei den phonologischen Hilfen um Teilinformationen der Zielwortform. Bei der Erhebung von spezifischen Therapieeffekten wurde zwischen itemspezifischen und itemübergreifenden Verbesserungen sowie kurz- und langfristigen Effekten auf die mündlichen Benennleistungen unterschieden. Dabei wurden neben den quantitativen Verbesserungen (% korrekt) auch die qualitativen Effekte (Fehlertypen) der beiden Ansätze berücksichtigt, und es wurden Transfereffekte in die Spontansprache der Patienten untersucht. Zusätzlich wurden auch die Soforteffekte der verschiedenen Benennhilfen in den Therapiesitzungen erhoben. Im Methodenvergleich zeigte sich, dass die phonologische Therapiephase kurzfristig bei der Mehrzahl der Patienten signifikante Verbesserungen beim Bildbenennen bewirkte, diese Effekte haben sich jedoch überwiegend als nicht stabil erwiesen. Im Gegensatz dazu erwies sich die semantische Therapiephase auch als langfristig effektiv. Im Unterschied dazu erwiesen sich die phonologischen Benennhilfen bei fast allen Patienten als unmittelbar effektiver als die semantischen Benennhilfen. Somit waren die Soforteffekte der Hilfetypen in den Therapiesitzungen kein sicherer Indikator für die Dauer der Gesamteffekte einer Therapiephase. Außerdem zeigte sich nicht bei allen Patienten ein direkter Zusammenhang zwischen der Art ihrer zugrundeliegenden funktionalen Störung und den erzielten Therapieeffekten. Einerseits profitierten Patienten mit erhaltenen semantischen Verarbeitungsleistungen von der semantischen Therapie, andererseits zeigten sich signifikante phonologische Therapieeffekte bei Patienten mit zentral-semantischen Störungen. Die Wirkmechanismen der beiden Therapieansätze werden unter Berücksichtigung unterschiedlicher kognitiv-neurolinguistischer Theorien zum mündlichen Wortabrufprozess beim Bildbenennen interpretiert. This study compared semantic and phonological treatments of word retrieval in ten aphasic patients using a multiple single case paradigm. The patients' single word processing was investigated before therapy, so that specific treatment effects could be interpreted with regard to underlying functional deficit(s) in each patient. In both treatments, the main task was spoken picture naming with different types of cues. In the semantic treatment, different aspects of the semantic target concept were used as a cue in picture naming, in the phonological treatment, different types of word-form specific information were provided as a cue. Treatment effects were assessed in terms of both short- and long-lasting effects on spoken picture naming accuracy in each patient. Both quantitative (proportion of correct responses) and qualitative (error types) improvements were evaluated. Furthermore, generalisation to untreated pictures (control items) and related tasks and transfer effects into spontaneous speech were examined. In addition, the immediate effects of the different types of prompts in the phonological and semantic treatment sessions were analysed. Regarding the stability of effects, different outcomes were found for semantic versus phonological treatments. The phonological treatment phase produced only short-lasting effects in most of the patients. In contrast, the semantic treatment phase produced more stable effects in some participants. With regard to the immediate effects of the different types of cues in the treatment sessions, it turned out that the phonological cues produced stronger effects than the semantic cues in most of the participants. Nevertheless, the effects of the semantic treatment phase were more stable than the phonological treatment effects. Specific effects of the phonological and the semantic treatment were not directly linked to the underlying functional disorder in each patient. Patients with preserved and with impaired semantic processing of concrete nouns could profit from both methods. The underlying mechanisms of effectiveness of the two treatment methods are interpreted in the framework of different cognitive-neuropsychological theories of spoken picture naming.

71. Die Farn- und Blütenpflanzen des ehemaligen Truppenübungsplatzes Oranienbaumer Heide

Abstract

Die Ergebnisse umfangreicher floristischer Kartierungen von Farn- und Blütenpflanzen auf dem ehemaligen Truppenübungsplatz Oranienbaumer Heide in den Jahren 2008 bis 2010 werden vorgestellt. Dabei wird vor allem auf naturschutzfachlich wertgebende Pflanzenarten fokussiert. Die Oranienbaumer Heide zählt heute aufgrund ihrer Nutzungsgeschichte, der Überschneidung kontinentaler und subatlantischer Verbreitungsareale sowie vielfältiger Standortbedingungen zu den artenreichsten Gebieten in Sachsen-Anhalt. Durch umfangreiche Literaturrecherchen sowie über die Auswertung historischen Kartenmaterials konnten die Nutzungsgeschichte des Gebietes sowie die historische Flora seit Beginn des 18. Jahrhunderts, zumindest in Teilen, rekonstruiert werden. Die Recherchen ergaben unter anderem, dass über einen langen Zeitraum ein Kontinuum an mehr oder weniger großen Offenflächen (Heiden, Magerrasen) im Gebiet vorhanden war. Eine Auswertung historischer Daten zu Pflanzenfunden für die Oranienbaumer Heide hat ergeben, dass ein großer Teil der schon früher für das Gebiet bekannt gewesenen Flora auch heute noch vorzufinden ist. Nur 18 Taxa konnten seit 1990 nicht wieder nachgewiesen werden. Insgesamt wurden für die Oranienbaumer Heide über 800 Pflanzenarten erfasst, darunter 74 Arten der Roten Liste des Landes Sachsen-Anhalt (FRANK et al. 2004), 38 Arten der Roten Liste Deutschlands (KORNECK et al. 1996) und 22 Arten nach Bundesartenschutzverordnung (BArtSchV 2005). Viele der im Untersuchungsgebiet naturschutzfachlich wertgebenden Pflanzenarten kommen darüber hinaus in außergewöhnlich großen Populationen vor., This study presents results of extensive floristic mapping of ferns and flowering plants from 2008 to 2010 on the former military training area „Oranienbaumer Heide“. It is focused on plant species with a high value for nature conservation. The investigation area Oranienbaumer Heide belongs to the most species-rich territories in Saxony-Anhalt because of the history of landuse, the overlap of continental and subatlantic distribution areas as well as varied site conditions. The history of land use as well as the flora of the area since the beginning of the 18th century could partly be reconstructed due to extensive literature research as well as the interpretation of old maps. A continuum of more or less large grasslands and heathlands over a long lasting period can be concluded. Most plants known to occur in the area Oranienbaumer Heide in the past can be found even today. Only 18 taxa known up to 1990 could not be reconfirmed. There is a total number of more than 800 plant species, among them 74 species of the Red List of Saxony-Anhalt (FRANK et al. 2004) and 38 species of the Red List of Germany (KORNECK et al. 1996) as well as 22 species which are protected by law (BArtSchV 2005). Many species with high value for nature conservation occur in extraordinary large populations in the investigated area.

Abstract

This paper examines the effects of phonological and orthographic cues on spoken naming in aphasia. The aim of the study was to determine the relationship between the effects of phonological and orthographic cues and the underlying functional language impairment and to identify the mechanisms by which the cues have their effects within a neurolinguistic word-processing model (Patterson & Shewell, 1987). We present the results from six anomic participants with chronic aphasia using a case-series design. Both effects of a single application and of multiple applications of cues were assessed, as were both immediate and delayed effects.

Abstract

This multiple single case intervention study focusses on the production of non-canonical sentences with seven agrammatic participants. Cross-over training involved the production of object relative clauses, i.e. syntactically complex sentences and relying on higher nodes in the syntactic tree and object derived who-questions, i.e. less complex sentences and relying on lower nodes. The investigation evaluates individual structure specific effects, generalized effects for sentence structures of the same movement type and different movement type. Individual results, unambiguously attributable to the intervention are reported and discussed within two accounts of treatment of agrammatic sentence production, the Tree-Pruning-Hypotheses (TPH) and the Complexity-Account-Treatment-Efficacy (CATE).

83. Compound naming in aphasia: effects of complexity, part of speech, and semantic transparency.

Abstract

This study examines spoken naming of German nominal compounds in four individuals with chronic aphasia in order to test current theories about the production of morphologically complex nouns. We investigated the impact of morphological complexity, grammatical class of the modifier, and of the constituents' semantic transparency on spoken naming accuracies, error types, and position of preserved constituents in erroneous responses. The results show that most participants were specifically impaired with compounds compared to long matched simple nouns. Furthermore, the grammatical class of the modifier affected the qualitative error pattern for three participants with agrammatic aphasia who were particularly impaired in retrieving the verbal modifier constituent as compared to the nominal head. In contrast, for noun-noun compounds no head superiority effect was obtained. Finally, semantic transparency did not affect overall naming accuracies, but the occurrence of constituent errors, i.e. erroneous responses overlapping with the target in one constituent. More constituent errors were observed for transparent than opaque targets. The results are compatible with decompositional and dual route accounts. [ABSTRACT FROM AUTHOR]

Abstract

Background: Collation of aphasia research data across settings, countries and study designs using big data principles will support analyses across different language modalities, levels of impairment, and therapy interventions in this heterogeneous population. Big data approaches in aphasia research may support vital analyses, which are unachievable within individual trial datasets. However, we lack insight into the requirements for a systematically created database, the feasibility and challenges and potential utility of the type of data collated. Aim: To report the development, preparation and establishment of an internationally agreed aphasia after stroke research database of individual participant data (IPD) to facilitate planned aphasia research analyses. Methods: Data were collated by systematically identifying existing, eligible studies in any language (>= 10 IPD, data on time since stroke, and language performance) and included sourcing from relevant aphasia research networks. We invited electronic contributions and also extracted IPD from the public domain. Data were assessed for completeness, validity of value-ranges within variables, and described according to pre-defined categories of demographic data, therapy descriptions, and language domain measurements. We cleaned, clarified, imputed and standardised relevant data in collaboration with the original study investigators. We presented participant, language, stroke, and therapy data characteristics of the final database using summary statistics. Results: From 5256 screened records, 698 datasets were potentially eligible for inclusion; 174 datasets (5928 IPD) from 28 countries were included, 47/174 RCT datasets (1778 IPD) and 91/174 (2834 IPD) included a speech and language therapy (SLT) intervention. Participants' median age was 63 years (interquartile range [53, 72]), 3407 (61.4%) were male and median recruitment time was 321 days (IQR 30, 1156) after stroke. The lexical representation of compound words in speech production is still under debate. While most studies with healthy adult speakers suggest that a single lemma representation is active during compound production, data from neuropsychological studies point toward multiple representations, with activation of the compound's constituent lemmas in addition to the compound's lemma. This study exploits the cumulative semantic interference effect to investigate the lexical representation of compounds in speech production. In a continuous picture naming experiment, category membership was established through the compounds' first constituents (category animals: zebra crossing, pony tail, cat litter ...), while the compounds themselves were not semantically related. Moreover, pictures depicting the compounds' first constituents (zebra, pony, cat ...) were presented as a control condition. As expected, naming latencies within categories increased linearly with each additionally named category member when producing monomorphemic words, which is interpreted as increasing interference during lexical selection. Importantly, this cumulative semantic interference effect was also observed for compounds. This indicates that the lemmas of the compounds' first constituents were activated during compound production, causing interference due to their semantic relationship and thereby hampering the production of the whole compound. The results are thus in line with the multiple-lemma representation account (Marelli et al., 2012). We argue that the apparent contradiction between results of previous studies with healthy adult speakers and our current study can be explained by the different experimental paradigms used. Diese Dissertation untersucht einige ungeklärten Aspekte der Sprachproduktion. Das erste Ziel war es zu klären, wie Komposita (z.B. Goldfisch) auf der lexikalisch-syntaktischen Ebene unseres Sprachproduktionssystems repräsentiert sind. Gibt es dort einen einzelnen lexikalischen Eintrag für das gesamte Kompositum (GOLDFISCH) oder mehrere Einträge für jedes seiner Konstituenten (GOLD und FISCH), welche beim Sprechen zusammengesetzt werden? Zur Beantwortung dieser Frage wurde die sogenannte kumulative semantische Interferenz (KSI) verwendet. Dieser semantische Kontexteffekt beschreibt die Beobachtung, dass die Benennlatenzen von Sprechern systematisch länger werden, wenn diese eine Reihe von semantisch verwandten Bildern benennen. Obwohl KSI bereits viel als Instrument in der Sprachproduktionsforschung genutzt wird, sind einige Fragen rund um den Effekt selbst noch offen. Das zweite Ziel dieser Dissertation war es daher einige dieser Fragen mit Hilfe von behavioralen und elektrophysiologischen Maßen zu beantworten, um so unser Verständnis von KSI zu erweitern. Die Ergebnisse deuten darauf hin, dass KSI ihren Ursprung auf der konzeptuellen Ebene des Sprachproduktionssystems hat und dass sie nicht von der morphologischen Komplexität der verwendeten Begriffe moduliert wird, aber davon, wie häufig diese benannt werden. Diese Erkenntnisse ermöglichen es in der Zukunft zielgenauere Vorhersagen zu machen, wenn KSI als Forschungsinstrument verwendet wird. Die Ergebnisse zeigen zudem, dass die Konstituenten von Komposita während deren Produktion aktiviert werden. Somit zeigen diese Ergebnisse, dass die Morphologie bereits die Repräsentationen auf der lexikalisch-syntaktischen Ebene beeinflusst und erweitern somit unser Wissen über den Aufbau unseres Sprachproduktionssystems., This dissertation addresses unresolved issues concerning speech production processes and the cognitive architecture of our speech production system. The first aim was to answer the question how compounds (e.g., goldfish) are represented on the lexical-syntactic level of our speech production system. Is there a single entry for the whole compound (GOLDFISH) or multiple ones for each of its constituents (GOLD and FISH), which are assembled for each use? To investigate this question, we used the cumulative semantic interference (CSI) effect. This semantic context effect describes the observation that speakers’ naming latencies systematically increase when naming a sequence of semantically related pictures. Although CSI has been extensively used as a tool in language production research, several aspects of it are not fully understood. Thus, the second aim of this dissertation was to close some of these knowledge gaps and gain a more comprehensive understanding of CSI. In three studies, we first investigated the CSI effect, before using it as a tool to study the lexical representation of compounds. Behavioural and electrophysiological data from the first two studies point to a purely conceptual origin of CSI. Furthermore, they revealed that CSI is not influenced by the items’ morphological complexity but affected by item repetition. These findings advance our understanding of CSI and thus allow us to make more informed predictions when using CSI as a research tool. The last study showed that the compounds’ constituents are activated during compound production, which provides evidence for a complex lexical-syntactic representation of compounds, consisting of one entry for the holistic compound and additional entries for each of its constituents. This dissertation thus reveals that the morphological complexity of compounds affects the lexical-syntactic level during speech production and thus advances our understanding of the architecture of our speech production system.

Abstract

Background: Stroke rehabilitation interventions are routinely personalized to address individuals' needs, goals, and challenges based on evidence from aggregated randomized controlled trials (RCT) data and meta-syntheses. Individual participant data (IPD) meta-analyses may better inform the development of precision rehabilitation approaches, quantifying treatment responses while adjusting for confounders and reducing ecological bias., Aim: We explored associations between speech and language therapy (SLT) interventions frequency (days/week), intensity (h/week), and dosage (total SLT-hours) and language outcomes for different age, sex, aphasia severity, and chronicity subgroups by undertaking prespecified subgroup network meta-analyses of the RELEASE database., Methods: MEDLINE, EMBASE, and trial registrations were systematically searched (inception-Sept2015) for RCTs, including ⩾ 10 IPD on stroke-related aphasia. We extracted demographic, stroke, aphasia, SLT, and risk of bias data. Overall-language ability, auditory comprehension, and functional communication outcomes were standardized. A one-stage, random effects, network meta-analysis approach filtered IPD into a single optimal model, examining SLT regimen and language recovery from baseline to first post-intervention follow-up, adjusting for covariates identified a-priori . Data were dichotomized by age (⩽/> 65 years), aphasia severity (mild-moderate/ moderate-severe based on language outcomes' median value), chronicity (⩽/> 3 months), and sex subgroups. We reported estimates of means and 95% confidence intervals. Where relative variance was high (> 50%), results were reported for completeness., Results: 959 IPD (25 RCTs) were analyzed. For working-age participants, greatest language gains from baseline occurred alongside moderate to high-intensity SLT (functional communication 3-to-4 h/week; overall-language and comprehension > 9 h/week); older participants' greatest gains occurred alongside low-intensity SLT (⩽ 2 h/week) except for auditory comprehension (> 9 h/week). For both age-groups, SLT-frequency and dosage associated with best language gains were similar. Participants ⩽ 3 months post-onset demonstrated greatest overall-language gains for SLT at low intensity/moderate dosage (⩽ 2 SLT-h/week; 20-to-50 h); for those > 3 months, post-stroke greatest gains were associated with moderate-intensity/high-dosage SLT (3-4 SLT-h/week; ⩾ 50 hours). For moderate-severe participants, 4 SLT-days/week conferred the greatest language gains across outcomes, with auditory comprehension gains only observed for ⩾ 4 SLT-days/week; mild-moderate participants' greatest functional communication gains were associated with similar frequency (⩾ 4 SLT-days/week) and greatest overall-language gains with higher frequency SLT (⩾ 6 days/weekly). Males' greatest gains were associated with SLT of moderate (functional communication; 3-to-4 h/weekly) or high intensity (overall-language and auditory comprehension; (> 9 h/weekly) compared to females for whom the greatest gains were associated with lower-intensity SLT (< 2 SLT-h/weekly). Consistencies across subgroups were also evident; greatest overall-language gains were associated with 20-to-50 SLT-h in total; auditory comprehension gains were generally observed when SLT > 9 h over ⩾ 4 days/week., Conclusions: We observed a treatment response in most subgroups' overall-language, auditory comprehension, and functional communication language gains. For some, the maximum treatment response varied in association with different SLT-frequency, intensity, and dosage. Where differences were observed, working-aged, chronic, mild-moderate, and male subgroups experienced their greatest language gains alongside high-frequency/intensity SLT. In contrast, older, moderate-severely impaired, and female subgroups within 3 months of aphasia onset made their greatest gains for lower-intensity SLT. The acceptability, clinical, and cost effectiveness of precision aphasia rehabilitation approaches based on age, sex, aphasia severity, and chronicity should be evaluated in future clinical RCTs.

Abstract

Background: People with language problems following stroke (aphasia) benefit from speech and language therapy. Optimising speech and language therapy for aphasia recovery is a research priority., Objectives: The objectives were to explore patterns and predictors of language and communication recovery, optimum speech and language therapy intervention provision, and whether or not effectiveness varies by participant subgroup or language domain., Design: This research comprised a systematic review, a meta-analysis and a network meta-analysis of individual participant data., Setting: Participant data were collected in research and clinical settings., Interventions: The intervention under investigation was speech and language therapy for aphasia after stroke., Main Outcome Measures: The main outcome measures were absolute changes in language scores from baseline on overall language ability, auditory comprehension, spoken language, reading comprehension, writing and functional communication., Data Sources and Participants: Electronic databases were systematically searched, including MEDLINE, EMBASE, Cumulative Index to Nursing and Allied Health Literature, Linguistic and Language Behavior Abstracts and SpeechBITE (searched from inception to 2015). The results were screened for eligibility, and published and unpublished data sets (randomised controlled trials, non-randomised controlled trials, cohort studies, case series, registries) with at least 10 individual participant data reporting aphasia duration and severity were identified. Existing collaborators and primary researchers named in identified records were invited to contribute electronic data sets. Individual participant data in the public domain were extracted., Review Methods: Data on demographics, speech and language therapy interventions, outcomes and quality criteria were independently extracted by two reviewers, or available as individual participant data data sets. Meta-analysis and network meta-analysis were used to generate hypotheses., Results: We retrieved 5928 individual participant data from 174 data sets across 28 countries, comprising 75 electronic (3940 individual participant data), 47 randomised controlled trial (1778 individual participant data) and 91 speech and language therapy intervention (2746 individual participant data) data sets. The median participant age was 63 years (interquartile range 53–72 years). We identified 53 unavailable, but potentially eligible, randomised controlled trials (46 of these appeared to include speech and language therapy). Relevant individual participant data were filtered into each analysis. Statistically significant predictors of recovery included age (functional communication, individual participant data: 532, n  = 14 randomised controlled trials) and sex (overall language ability, individual participant data: 482, n  = 11 randomised controlled trials; functional communication, individual participant data: 532, n  = 14 randomised controlled trials). Older age and being a longer time since aphasia onset predicted poorer recovery. A negative relationship between baseline severity score and change from baseline ( p  < 0.0001) may reflect the reduced improvement possible from high baseline scores. The frequency, duration, intensity and dosage of speech and language therapy were variously associated with auditory comprehension, naming and functional communication recovery. There were insufficient data to examine spontaneous recovery. The greatest overall gains in language ability [14.95 points (95% confidence interval 8.7 to 21.2 points) on the Western Aphasia Battery-Aphasia Quotient] and functional communication [0.78 points (95% confidence interval 0.48 to 1.1 points) on the Aachen Aphasia Test-Spontaneous Communication] were associated with receiving speech and language therapy 4 to 5 days weekly; for auditory comprehension [5.86 points (95% confidence interval 1.6 to 10.0 points) on the Aachen Aphasia Test-Token Test], the greatest gains were associated with receiving speech and language therapy 3 to 4 days weekly. The greatest overall gains in language ability [15.9 points (95% confidence interval 8.0 to 23.6 points) on the Western Aphasia Battery-Aphasia Quotient] and functional communication [0.77 points (95% confidence interval 0.36 to 1.2 points) on the Aachen Aphasia Test-Spontaneous Communication] were associated with speech and language therapy participation from 2 to 4 (and more than 9) hours weekly, whereas the highest auditory comprehension gains [7.3 points (95% confidence interval 4.1 to 10.5 points) on the Aachen Aphasia Test-Token Test] were associated with speech and language therapy participation in excess of 9 hours weekly (with similar gains notes for 4 hours weekly). While clinically similar gains were made alongside different speech and language therapy intensities, the greatest overall gains in language ability [18.37 points (95% confidence interval 10.58 to 26.16 points) on the Western Aphasia Battery-Aphasia Quotient] and auditory comprehension [5.23 points (95% confidence interval 1.51 to 8.95 points) on the Aachen Aphasia Test-Token Test] were associated with 20–50 hours of speech and language therapy. Network meta-analyses on naming and the duration of speech and language therapy interventions across language outcomes were unstable. Relative variance was acceptable (< 30%). Abstract

Background: Speech and language therapy (SLT) benefits people with aphasia following stroke. Group level summary statistics from randomised controlled trials hinder exploration of highly complex SLT interventions and a clinically relevant heterogeneous population. Creating a database of individual participant data (IPD) for people with aphasia aims to allow exploration of individual and therapy-related predictors of recovery and prognosis., Aim: To explore the contribution that individual participant characteristics (including stroke and aphasia profiles) and SLT intervention components make to language recovery following stroke., Methods and Procedures: We will identify eligible IPD datasets (including randomised controlled trials, non-randomised comparison studies, observational studies and registries) and invite their contribution to the database. Where possible, we will use meta- and network meta-analysis to explore language performance after stroke and predictors of recovery as it relates to participants who had no SLT, historical SLT or SLT in the primary research study. We will also examine the components of effective SLT interventions., Outcomes and Results: Outcomes include changes in measures of functional communication, overall severity of language impairment, auditory comprehension, spoken language (including naming), reading and writing from baseline. Data captured on assessment tools will be collated and transformed to a standardised measure for each of the outcome domains., Conclusion: Our planned systematic-review-based IPD meta- and network meta-analysis is a large scale, international, multidisciplinary and methodologically complex endeavour.
