Abstract

This review summarizes the last decade of work by the ENIGMA (Enhancing NeuroImaging Genetics through Meta Analysis) Consortium, a global alliance of over 1400 scientists across 43 countries, studying the human brain in health and disease. Building on large-scale genetic studies that discovered the first robustly replicated genetic loci associated with brain metrics, ENIGMA has diversified into over 50 working groups (WGs), pooling worldwide data and expertise to answer fundamental questions in neuroscience, psychiatry, neurology, and genetics. Most ENIGMA WGs focus on specific psychiatric and neurological conditions, other WGs study normal variation due to sex and gender differences, or development and aging; still other WGs develop methodological pipelines and tools to facilitate harmonized analyses of "big data" (i.e., genetic and epigenetic data, multimodal MRI, and electroencephalography data). These international efforts have yielded the largest neuroimaging studies to date in schizophrenia, bipolar disorder, major depressive disorder, post-traumatic stress disorder, substance use disorders, obsessive-compulsive disorder, attention-deficit/hyperactivity disorder, autism spectrum disorders, epilepsy, and 22q11.2 deletion syndrome. More recent ENIGMA WGs have formed to study anxiety disorders, suicidal thoughts and behavior, sleep and insomnia, eating disorders, irritability, brain injury, antisocial personality and conduct disorder, and dissociative identity disorder. Here, we summarize the first decade of ENIGMA's activities and ongoing projects, and describe the successes and challenges encountered along the way. We highlight the advantages of collaborative large-scale coordinated data analyses for testing reproducibility and robustness of findings, offering the opportunity to identify brain systems involved in clinical syndromes across diverse samples and associated genetic, environmental, demographic, cognitive, and psychosocial factors.

54. Characterization of a highly conserved island in the otherwise divergent Bordetella holmesii and Bordetella pertussis genomes

Abstract

The recently discovered pathogen Bordetella holmesii has been isolated from the airways and blood of diseased humans. Genetic events contributing to the emergence of B. holmesii are not understood, and its phylogenetic position among the bordetellae remains unclear. To address these questions, B. holmesii strains were analyzed by comparative genomic hybridization (CGH) to a Bordetella pertussis microarray and by multilocus sequence typing. Both methods indicated substantial sequence divergence between B. pertussis and B. holmesii. However, CGH identified a putative pathogenicity island of 66 kb that is highly conserved between these species and contains several IS481 elements that may have been laterally transferred from B. pertussis to B. holmesii. This island contains, among other genes, a functional, iron-regulated locus encoding the biosynthesis, export, and uptake of the siderophore alcaligin. The acquisition of this genomic island by B. holmesii may have significantly contributed to its emergence as a human pathogen. Horizontal gene transfer between B. pertussis and B. holmesii may also explain the unusually high sequence identity of their 16S rRNA genes.

Abstract

Distress models are required during design to ensure the pavement have a longer service life that is conducted for different axle weight. This research addresses by modeling and analyzing StormPav pavement under various wheel loads to ensure pavement structures can withstand these loads. StormPav pavement is made up of precast concrete with Grade 50, which is also known as the innovative IBS Green Pavement, which has a combination of structural, environmental and economic advantages. The analysis of mechanistic behavior for different structures of pavements and mechanistic behavior of precast StormPav pavements using different wheel loads. WinJULEA and KENPAVE are software used for this research for modeling the real scenario to obtain the deflection, stress, strain and stress of each layer of pavement. It is found that total ESAL increases when there is a heavier truck acting on top of the pavement, and precast StormPav pavement receives the lowest total ESAL compared to flexible and rigid pavement. Among various types of pavements, precast StormPav has lesser maximum deflection, which had 65.11% lesser compared to the flexible pavement due to its largest elastic modulus. Besides, when there is loading from a 24-tonne truck acting on top of the precast StormPav pavement, total deflection is 0.761mm and increases 0.064mm (8.345%) when there is loading change to 26-tonne truck, as precast StormPav pavement is made up of concrete grade 50, which can resist to heavier loading. Precast StormPav pavements have the highest maximum allowable load repetitions to prevent fatigue cracking due to their increased flexural strength, which enables them to withstand greater stresses above the structure. As a result, it can be concluded that precast StormPav pavement is a viable alternative to the pavement because it requires less time for construction and sustains less damage when subjected to heavier loads [ABSTRACT FROM AUTHOR]

75. Using Patient-Reported Outcomes to Predict Revision Arthroplasty Following Femoral Neck Fracture: Enhancing the Value of Clinical Registries through Data Linkage

Abstract

The aim of this study was to determine the association between patient-reported outcome measures (PROMs) six months following femoral neck fracture after a low fall and future arthroplasty, and the factors associated with this. Six-month post-fracture PROMs were collected from the Victorian Orthopaedic Trauma Outcomes Registry (VOTOR) for patients aged >55 years who were admitted for a femoral neck fracture after a low fall between March 2007 and June 2015. These cases were linked with those registered by Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) up to October 2016. Multivariable analysis was performed using a Cox proportional hazards model to determine factors associated with future arthroplasty, including six-month PROMs. Of the 7077 hip fracture patients registered by VOTOR during the study period, 2325 met the inclusion criteria. Internal fixation being used for the initial hip fracture surgery, being younger and having no pre-injury disability were all independently associated with future revision or conversion to arthroplasty. Out of all PROMs, reporting pain and discomfort six months post-fracture was associated with a 9.5-fold increase in the risk of future arthroplasty (95% CI: 3.81, 23.67). The value of clinical registries can be enhanced via data linkage, in this case by using PROMs to predict arthroplasty following femoral neck fracture.

Abstract

Dual‐phase dielectric ceramic samples of Mg2SiO4/Ba(AlSiO4)2 were fabricated by ceramic injection molding (CIM), and they were systematically compared with the ones fabricated by conventional powder pressing (CP). It was found that by tuning the processing parameters, CIM samples with density, microstructure, grain size, and phase composition comparable with the CP counterparts can be produced. With optimized processing parameters, CIM samples with a relative density of 94.2% were accomplished, along with homogeneous phase distribution and uniform grain size. Meanwhile, satisfactory dielectric properties were also achieved (a dielectric constant of 6.67, a temperature coefficient of −0.94 ppm/°C, and a quality factor of 352,000 GHz). Moreover, a demonstrator with various blind holes and slots was fabricated by the CIM method, which exhibited good geometrical integrity, uniform microstructure, and consistent mechanical properties. 86. SMOOTH: Self-Management Open Online Trials in Health An Analysis of Existing Online Trials

Abstract

BACKGROUND: The use of online clinical trials is growing, but there remains little practical guidance on their conduct and it is sometimes challenging for researchers to adapt the conventions used in face-to-face trials and maintain the validity of the work. Online trials of self-management may indicate how an intervention will be used in daily practice as the online environment can mirror the self-management of care increasingly expected. The Online Randomized Controlled Trials of Health Information Database (ORCHID) contains health trials undertaken using the internet which were systematically sought and cataloged. This ORCHID analysis provides insight into the current state of online clinical trials. \ud \ud AIM: To systematically explore existing self-recruited online randomized trials of self-management interventions and analyze the trials to assess their strengths and weaknesses, the quality of reporting and the involvement of participants in the research process.\ud \ud METHODS: ORCHID was used as a sampling frame to identify a subset of self-recruited looking at self-management interventions. These were appraised to explore the qualities of self-recruited online randomized trials and to evaluate the usefulness of online trials for obtaining trustworthy answers to questions about health self-management and citizen research involvement.\ud \ud RESULTS: The sample included (n=41) online trials published from 2002-2015. Trial quality was critically appraised as High (n=9), Medium-high (n=15), Medium (n=17), and low as (n=1). Descriptive settings in (N=23/41) trials provided insufficient information to be replicable and did not report piloting or testing platforms before the trial launch. Reporting of patient and public involvement was more common than in face-to-face trials, however reporting, replicability, and methods used in online randomized trials of self-recruited self-management interventions were sub-optimal and dissemination strategies were sparse and reported in only (n=1) trial. \ud \ud CONCLUSIONS: The information gained in this study catalogs the state of online trials of self-management in the early 21st century and provides insights for online trials development as early as the protocol planning stage.

Abstract

The objective of this analysis is to evaluate the necessity of large clinical trials using FLOW trial data. The FLOW pilot study and definitive trial were factorial trials evaluating the effect of different irrigation solutions and pressures on re-operation. To explore treatment effects over time, we analyzed data from the pilot and definitive trial in increments of 250 patients until the final sample size of 2447 patients was reached. At each increment we calculated the relative risk (RR) and associated 95% confidence interval (CI) for the treatment effect, and compared the results that would have been reported at the smaller enrolments with those seen in the final, adequately powered study. The pilot study analysis of 89 patients and initial incremental enrolments in the FLOW definitive trial favored low pressure compared to high pressure (RR: 1.50, 95% CI: 0.75–3.04; RR: 1.39, 95% CI: 0.60–3.23, respectively), which is in contradiction to the final enrolment, which found no difference between high and low pressure (RR: 1.04, 95% CI: 0.81–1.33). In the soap versus saline comparison, the FLOW pilot study suggested that re-operation rate was similar in both the soap and saline groups (RR: 0.98, 95% CI: 0.50–1.92), whereas the FLOW definitive trial found that the re-operation rate was higher in the soap treatment arm (RR: 1.28, 95% CI: 1.04–1.57). Our findings suggest that studies with smaller sample sizes would have led to erroneous conclusions in the management of open fracture wounds. Abstract

Background: Femoral neck fractures are associated with high rates of revision surgery after management with internal fixation. Using data from the Fixation using Alternative Implants for the Treatment of Hip fractures (FAITH) trial evaluating methods of internal fixation in patients with femoral neck fractures, we investigated associations between baseline and surgical factors and the need for revision surgery to promote healing, relieve pain, treat infection or improve function over 24 months postsurgery. Additionally, we investigated factors associated with (1) hardware removal and (2) implant exchange from cancellous screws (CS) or sliding hip screw (SHS) to total hip arthroplasty, hemiarthroplasty, or another internal fixation device. Methods: We identified 15 potential factors a priori that may be associated with revision surgery, 7 with hardware removal, and 14 with implant exchange. We used multivariable Cox proportional hazards analyses in our investigation. Abstract

Aims The primary aim of this prognostic study was to identify baseline factors associated with physical health-related quality of life (HRQL) in patients after a femoral neck fracture. The secondary aims were to identify baseline factors associated with mental HRQL, hip function, and health utility. Patients and Methods Patients who were enrolled in the Fixation using Alternative Implants for the Treatment of Hip Fractures (FAITH) trial completed the 12-item Short Form Health Survey (SF-12), Western Ontario and McMaster Universities Arthritis Index, and EuroQol 5-Dimension at regular intervals for 24 months. We conducted multilevel mixed models to identify factors potentially associated with HRQL. Results The following were associated with lower physical HRQL: Older age (-1.42 for every ten-year increase, 95% confidence interval (CI) -2.17 to -0.67, p < 0.001); female gender (-1.52, 95% CI - 3.00 to -0.05, p = 0.04); higher body mass index (-0.69 for every five-point increase, 95% CI - 1.36 to -0.02, p = 0.04); American Society of Anesthesiologists class III (versus class I) (-3.19, 95% CI -5.73 to -0.66, p = 0.01); and sustaining a displaced fracture (-2.18, 95% CI -3.88 to - 0.49, p = 0.01). Additional factors were associated with mental HRQL, hip function, and health utility. Conclusion We identified several baseline factors associated with lower HRQL, hip function, and utility after a femoral neck fracture. These findings may be used by clinicians to inform treatment and outcomes.

RESULTS: Factors associated with increased risk of revision surgery included: female sex, [hazard ratio (HR) 1.79, 95% confidence interval (CI) 1.25-2.50; P = 0.001], higher body mass index (for every 5-point increase) (HR 1.19, 95% CI 1.02-1.39; P = 0.027), displaced fracture (HR 2.16, 95% CI 1.44-3.23; P < 0.001), unacceptable quality of implant placement (HR 2.70, 95% CI 1.59-4.55; P < 0.001), and smokers treated with cancellous screws versus smokers treated with a sliding hip screw (HR 2.94, 95% CI 1.35-6.25; P = 0.006). Additionally, for every 10-year decrease in age, participants experienced an average increased risk of 39% for hardware removal. CONCLUSIONS: Results of this study may inform future research by identifying high-risk patients who may be better treated with arthroplasty and may benefit from adjuncts to care (HR 1.39, 95% CI 1.05-1.85; P = 0.020).
