55. A novel score system for predicting conversion to no evidence of Disease (C-NED) in initially unresectable colorectal cancer liver metastases

Abstract

An estimated 70-80% of cases of colorectal cancer liver metastasis (CRLM) are defined as initially unresectable. “Converting” to no evidence of disease (NED) status may prolong survival. The current study aimed to develop a novel scoring system that predicts the conversion outcome for initially unresectable CRLM. A total of 215 consecutive CRLM patients who received first-line systemic therapy from December 2012 to January 2020 at Sun Yat-sen University Cancer Center were enrolled in the internal cohort. Forty CRLM patients from the database of the Chinese Colorectal Cancer Multidisciplinary Team Alliance were enrolled in the external cohort. A logistic regression model was applied to identify risk factors associated with the conversion outcome. The tumor-to-liver volume ratio (TLVR) was calculated as the total tumor volume divided by the total liver volume, and its cutoff value was 0.23. Three predictors of conversion failure were identified in the internal cohort and incorporated into the C-NED score: poor tumor differentiation (1 point), number of liver metastases > 8 (1 point) and TLVR ≥ 0.23 (1 point). The conversion rate was significantly negatively associated with the C-NED score (P < 0.001). The C-indexes of the C-NED score for predicting successful conversion outcome in the internal cohort and external cohort were 0.734 (95% confidence interval (CI), 0.668-0.800) and 0.736 (95% CIs, 0.566-0.907), respectively. Median progression-free survival (PFS) time (P = 0.001) and overall survival (OS) time (P = 0.003) were statistically significant different among different C-NED score groups. Our study demonstrated that the C-NED score is an effective scoring system that indicates the actual conversion probability for initially unresectable CRLM patients before treatment, which can serve as a tool that guides optimal first-line management strategies.

57. Rituximab for connective tissue disease‐associated interstitial lung disease: A systematic review and meta‐analysis.

Abstract

Objective: To assess the efficacy of rituximab (RTX) on lung function and the prevalence of adverse events (AEs) in connective tissue disease‐associated interstitial lung disease (CTD‐ILD) by meta‐analysis. Methods: EMBASE, Web of Science, PubMed and ClinicalKey were searched up to July 16, 2021. The lung function (forced vital capacity, FVC% predicted, and diffusing capacity of the lung for carbon monoxide, DLCO% predicted) and prevalence of AEs of RTX in CTD‐ILD were analyzed by meta‐analysis, and 95% confidence interval (CI) was calculated. Subgroup analyses and meta‐regression were used to explore the heterogeneity. Results: We identified 29 studies, including 827 CTD‐ILD patients with a median age of 53.05 years. In observational studies, FVC% (mean difference − 1.24, 95% CI [−2.35, −0.12]; P =.030) and DLCO% (−7.71, [−11.79, −3.63]; P =.014) of CTD‐ILD decreased significantly after RTX treatment. In randomized controlled trials, FVC% of CTD‐ILD decreased after RTX treatment (−5.24, [−9.94, − 0.54]; P =.029), but the difference of DLCO% was not significant (1.15, [−4.33, 6.63]; P =.681). The prevalence of AEs, all‐cause mortality and infections was 29.7% (95% CI [0.17, 0.42]), 11.6% (95% CI [0.08, 0.16]) and 20.9% (95% CI [0.15, 0.27]), respectively. Conclusions: RTX was associated with AEs such as decreased pulmonary function, all‐cause mortality, and infections in CTD‐ILD. Adverse reactions during and after RTX treatment should be carefully monitored. Further prospective studies are needed to compare RTX with other immunosuppressants, antifibrotic drugs or placebos, which can provide therapeutic approaches for CTD‐ILD. [ABSTRACT FROM AUTHOR]

Featured Application The NISDL method proposed in this paper can be used for real time contactless measuring of human skin temperature, which reflects human body thermal comfort status and can be used for control HVAC devices. Abstract In human-centered intelligent building, real-time measurements of human thermal comfort play critical roles and supply feedback control signals for building heating, ventilation, and air conditioning (HVAC) systems. Due to the challenges of intra- and inter-individual differences and skin subtleness variations, there has not been any satisfactory solution for thermal comfort measurements until now. In this paper, a contactless measuring method based on a skin sensitivity index and deep learning (NISDL) was proposed to measure real-time skin temperature. A new evaluating index, named the skin sensitivity index (SSI), was defined to overcome individual differences and skin subtleness variations. To illustrate the effectiveness of SSI proposed, a two multi-layers deep learning framework (NISDL method I and II) was designed and the DenseNet201 was used for extracting features from skin images. The partly personal saturation temperature (NIPST) algorithm was use for algorithm comparisons. Another deep learning algorithm without SSI (DL) was also generated for algorithm comparisons. Finally, a total of 1.44 million image data was used for algorithm validation. The results show that 55.62% and 52.25% error values (NISDL method I, II) are scattered at (0 degrees C, 0.25 degrees C), and the same error intervals distribution of NIPST is 35.39%., QC 20190624
