Abstract

Aims: A variety of surgical techniques are available for vaginal prolapse repair, indicating a lack of consensus. A debate regarding the utility of hydrodissection for splitting the surgical plane of the vaginal wall exists. The aim of this study is to evaluate the impact of hydrodissection in anterior colporrhaphy (AC). Materials: Patients undergoing primary AC were randomly assigned to an approach with (study group) versus without (control group) hydrodissection. Five surgeons performed both techniques, and the trimmed vaginal tissue was retrieved for histological analysis. Two pathologists, blinded to the surgical approach, evaluated the presence of a loose connective tissue at the surgical dissection plane (controversially deemed 'fascia', as explained in this article). In addition, we compared the operative time, pain score and haemoglobin levels. After statistical analysis, data were presented using percentile, and statistical significance was tested using the χ2 and Fisher's exact tests. Results: Forty‐six patients underwent primary elective AC, with 23 patients in each, the study and control groups. The groups were comparable regarding age (study group 60.33 ± 11.95 years and control group 59.86 ± 12.04, P = 0.90), menopausal status (study group 17 (73.9%) and control group 15 (68.2%), P = 0.67) and other characteristics. We found no difference in sample characteristics between the two groups. Connective tissue was found in only 13.6% (n = 3) of patients after hydrodissection and in 27.3% (n = 6) of patients without hydrodissection (P = 0.46). The hydrodissection group had significantly less bleeding than the control group (ΔHB 0.66 ± 0.66 vs 1.21 ± 0.84, P = 0.05). Abstract

Objectives. Understanding more about the psychological state of persons notified of human immunodeficiency virus (HIV) infection is critical for designing notification and counseling programs that will have the most positive effect. Methods. The subjects were blood donors who had been notified of HIV infection by the New York Blood Center. A nurse elicited a medical history, performed a limited medical examination, and asked the subjects to complete a questionnaire that included questions about drug use, sexual behavior, and psychological characteristics. The subjects completed another questionnaire approximately 2 weeks later. Results. The average depressive symptom scores for both men and women were substantially higher than scores typically found in representative population samples. More than a quarter of the men and more than a third of the women reported seeking psychological or psychiatric services in the first few weeks following notification. Conclusions. Anticipating and meeting individuals' psychological needs may be necessary if HIV screening programs are to address effectively the needs of persons infected with HIV. Am J Public Heath. 1993;83:534-539), HIV screening programs should anticipate the psychological needs of people who test positive. Depressive symptoms were evaluated in 271 men and women who were notified that they were HIV positive after donating blood. Overall, men and women were most likely to cope with the news by talking with a health professional and least likely to cope by using alcohol or drugs or by resignation. Women were more likely than men to feel they had less control over their illness and to have problems with sexual functioning. Women scored higher on a test of depressive symptoms as did people with low self-esteem, poor social support and functioning and little optimism about the possibility of AIDS treatment discoveries. Gay and bisexual men were less likely than women and heterosexual men to seek professional psychological help.

Abstract

Blood centers screen donations for a variety of problems, including the presence of HIV antibodies, to ensure the safety of the blood supply. They notify donors with abnormal test results so they can get treatment and, hopefully, to reduce the spread of infection. However, it is not known whether screening and notification result in positive behavior changes. Short-term behavior changes were assessed in HIV-positive blood donors. The subjects did not volunteer for HIV antibody testing, but were tested as a part of the standard protocol when they donated blood. Of 708 HIV-positive donors identified during the study period, 196 completed an initial interview and baseline questionnaire, and returned for a two-week follow-up visit. Questionnaires elicited information about drug use, sexual behavior, and psychological characteristics prior to notification and after. Women and men both reported practicing safer sex behavior during the two weeks between interviews than prior to the first interview. However, 40 percent reported that they still were practicing unsafe sex. The results of notification of positive HIV status was not significantly associated with behavior changes among these subjects. Abstract

Background Methylotrophy describes the ability of organisms to grow on reduced organic compounds without carbon-carbon bonds. The genomes of two pink-pigmented facultative methylotrophic bacteria of the Alpha-proteobacterial genus Methylobacterium, the reference species Methylobacterium extorquens strain AM1 and the dichloromethane-degrading strain DM4, were compared. Methodology/Principal Findings The 6.88 Mb genome of strain AM1 comprises a 5.51 Mb chromosome, a 1.26 Mb megaplasmid and three plasmids, while the 6.12 Mb genome of strain DM4 features a 5.94 Mb chromosome and two plasmids. The chromosomes are highly syntenic and share a large majority of genes, while plasmids are mostly strain-specific, with the exception of a 130 kb region of the strain AM1 megaplasmid which is syntenic to a chromosomal region of strain DM4. Both genomes contain large sets of insertion elements, many of them strain-specific, suggesting an important potential for genomic plasticity. Most of the genomic determinants associated with methylotrophy are nearly identical, with two exceptions that illustrate the metabolic and genomic versatility of Methylobacterium. A 126 kb dichloromethane utilization (dcm) gene cluster is essential for the ability of strain DM4 to use DCM as the sole carbon and energy source for growth and is unique to strain DM4. The methylamine utilization (mau) gene cluster is only found in strain AM1, indicating that strain DM4 employs an alternative system for growth with methylamine. The dcm and mau clusters represent two of the chromosomal genomic islands (AM1: 28; DM4: 17) that were defined. The mau cluster is flanked by mobile elements, but the dcm cluster disrupts a gene annotated as chelatase and for which we propose the name “island integration determinant” (iid). Abstract

Large Insert Genome Analysis (LIGAN) is a broadly applicable, high-throughput technology designed to characterize genome-scale structural variation. Fosmid paired-end sequences and DNA fingerprints from a query genome are compared to a reference sequence using the Genomic Variation Analysis (GenVal) suite of software tools in order to pinpoint locations of insertions, deletions and rearrangements. Fosmids spanning regions that contain new structural variants can then be sequenced. Clonal pairs of Pseudomonas aeruginosa isolates from four cystic fibrosis patients were used to validate LIGAN technology. Approximately 1.5 Mb of inserted sequences were identified, including 742,869 bp containing 651 ORFs that are absent from published P. aeruginosa genomes. Six rearrangement break points and 220 kb of deleted sequences were also identified. Our study expands the “genome universe” of P. aeruginosa and validates a technology that complements emerging, short-read sequencing methods that are better suited to characterizing single-nucleotide polymorphism than structural variation.

The definitive version was published in The ISME Journal 6 (2012): 670–679, doi:10.1038/ismej.2011.128., Akinetes are dormancy cells commonly found among filamentous cyanobacteria, many of which are toxic and/or nuisance, bloom-forming species. Development of akinetes from vegetative cells is a process that involves morphological and biochemical modifications. Here we applied a single cell approach to quantify genome and ribosome content of akinetes and vegetative cells in Aphanizomenon ovalisporum (Cyanobacteria). Vegetative cells of A. ovalisporum were naturally polyploid and contained on average 8 genome copies per cell. However, the chromosomal content of akinetes increased up to 450 copies, with an average value of 119 genome copies per akinete, 15 fold higher that in vegetative cells. Based on fluorescence in situ hybridization with a probe targeting 16S rRNA and detection with confocal laser scanning microscopy we conclude that ribosomes accumulated in akinetes to a higher level than that found in vegetative cells. We further present evidence that this massive accumulation of nucleic acids in akinetes is likely supported by phosphate supplied from inorganic polyphosphate bodies that were abundantly present in vegetative cells, but notably absent from akinetes. These results are interpreted in the context of cellular investments for proliferation following long term dormancy, as the high nucleic acid content would provide the basis for extended survival, rapid resumption of metabolic activity and cell division upon germination., Supported by the Gruss Lipper Foundation research award (AS). Here we report the complete, accurate 1.89-Mb genome sequence of Francisella tularensis subsp. holarctica strain FSC200, isolated in 1998 in the Swedish municipality Ljusdal, which is in an area where tularemia is highly endemic. This genome is important because strain FSC200 has been extensively used for functional and genetic studies of Francisella and is well-characterized.

Assim, o estudo da arquitetura desta exposição, desenvolvido ao longo da dissertação, pretendeu contribuir para o debate em torno da produção arquitetônica do final do século XIX e início do XX, especialmente no Rio de Janeiro, cidade que passou no início do século por uma reforma urbana radical. Um panorama das Exposições Universais, com a análise de seu projeto conceitual e de sua produção arquitetônica, foi traçado de forma a enriquecer o estudo a respeito da exposição carioca e, através de análises comparativas, são estabelecidas correlações relevantes. Os prédios da Exposição Nacional são analisados dentro da questão do estilo, das tipologias e, quando as parcas informações permitem, da técnica. Marcada pela efemeridade característica deste tipo de evento, a exposição durou apenas três meses e suas construções foram, quase em sua totalidade, demolidas em seguida. Assim, a análise de seus exemplares baseia-se nas descrições de época e no material iconográfico. On the occasion of the National Exposition that took place in Rio de Janeiro in 1908, some large structures – Palaces and Pavillions – were built, as well as a number of smaller ones, making up a grand ensemble characterized by predominantly eclectic esthetics. This dissertation studies the architecture of this Exposition, as a contribution to the study of turn-of-the-century architectural output in Brazil and particularly in Rio, where radical urban remodeling took place at the beginning of the 20th century. We include an overview of the World’s Fairs and an analysis of their conceptual framework and their architectural output, in order to place the Rio National Exposition in context and make relevant correlation and comparisons. We analyze the buildings of the National Exposition from the point of view of style, typology, and, where possible in spite of the paucity of relevant information, technique. The Exposition was short-lived (three months only) and almost all the buildings were subsequently torn down, which was not unusual for events of this type. Therefore our study is based on comtemporaneous documents, including descriptions, photographs, and other illustrations.

Abstract

BACKGROUND: Francisella tularensis subspecies tularensis and holarctica are pathogenic to humans, whereas the two other subspecies, novicida and mediasiatica, rarely cause disease. To uncover the factors that allow subspecies tularensis and holarctica to be pathogenic to humans, we compared their genome sequences with the genome sequence of Francisella tularensis subspecies novicida U112, which is nonpathogenic to humans. RESULTS: Comparison of the genomes of human pathogenic Francisella strains with the genome of U112 identifies genes specific to the human pathogenic strains and reveals pseudogenes that previously were unidentified. In addition, this analysis provides a coarse chronology of the evolutionary events that took place during the emergence of the human pathogenic strains. Genomic rearrangements at the level of insertion sequences (IS elements), point mutations, and small indels took place in the human pathogenic strains during and after differentiation from the nonpathogenic strain, resulting in gene inactivation. CONCLUSION: The chronology of events suggests a substantial role for genetic drift in the formation of pseudogenes in Francisella genomes. Mutations that occurred early in the evolution, however, might have been fixed in the population either because of evolutionary bottlenecks or because they were pathoadaptive (beneficial in the context of infection). Because the structure of Francisella genomes is similar to that of the genomes of other emerging or highly pathogenic bacteria, this evolutionary scenario may be shared by pathogens from other species.

Abstract

Francisella tularensis is a bacterial pathogen that causes the zoonotic disease tularemia and is important to biodefense. Currently, the only vaccine known to confer protection against tularemia is a specific live vaccine strain (designated LVS) derived from a virulent isolate of Francisella tularensis subsp. holarctica. The origin and source of attenuation of this strain are not known. To assist with the design of a defined live vaccine strain, we sought to determine the genetic basis of the attenuation of LVS. This analysis relied primarily on the comparison between the genome of LVS and Francisella tularensis holarctica strain FSC200, which differ by only 0.08% of their nucleotide sequences. Under the assumption that the attenuation was due to a loss of function(s), only coding regions were examined in this comparison. To complement this analysis, the coding regions of two slightly more distantly related Francisella tularensis strains were also compared against the LVS coding regions. Thirty-five genes show unique sequence variations predicted to alter the protein sequence in LVS compared to the other Francisella tularensis strains. Due to these polymorphisms, the functions of 15 of these genes are very likely lost or impaired. Seven of these genes were demonstrated to be under stronger selective constraints, suggesting that they are the most probable to be the source of LVS attenuation and useful for a newly defined vaccine.
