Abstract

We report the case of a patient with cryptogenic cirrhosis, new onset ascites, and hyperinfection with Strongyloides stercoralis who had significant eosinophilia of the peritoneal fluid. The eosinophilia resolved with treatment of the S. stercoralis infection, and did not recur during two subsequent episodes of ascites and spontaneous bacterial peritonitis. Eosinophilic ascites is rare in parasitic infection, but it has been described in a variety of disorders which are discussed.

Abstract

Objectives : To describe the technique of pneumatic dilation for achalasia without fluoroscopic guidance and to assess its safety and efficacy. Methods : Twenty-seven consecutive patients who underwent pneumatic dilation with the Rigiflex achalasia balloon under direct endoscopic visualization were reviewed. The balloon was passed into the stomach over a guidewire, withdrawn across the gastroesophageal junction, and dilated with the endoscope positioned proximally. Patients were graded pre- and posttreatment on the frequency of dysphagia, daytime regurgitation, nighttime symptoms, chest pain, and heartburn. Response was assessed by the improvement in dysphagia frequency. Results : Twenty-seven patients (16 females, 11 males ; mean age 54.0 yr) underwent 30 pneumatic dilations. The 30-mm balloon was used in most cases (67%). The mean postdilation follow-up was 21.1 months (1.5-57.4 months). The range of inflation pressures was 8-18 psi (median 15 psi), and the duration of inflation was 30-120 s (median 90 s). Eighteen of 27 patients (67%) had excellent or good results, six (22%) had fair results, and three (11%) had poor results. The outcome of pneumatic dilation was successful in 78% of patients after a single dilation and in 89% of patients overall. There were no perforations related to balloon inflation. Conclusions : The Rigiflex balloon can be successfully positioned across the gastroesophageal junction and dilated under direct endoscopic observation. Pneumatic dilation for achalasia can therefore be performed simply, safely, and effectively without the use of fluoroscopy.

Recommendations to improve service quality and efficiency in this area include presence of a high-profile coordinator for the 12 agencies which are involved in funding projects in Russia, adopting measures that promote macroeconomic stabilization and direct private investment.

Abstract

The article focuses on Jewish self-defense during the Russian Programs of 1903-1906. The Bund, the radical Jewish workers organization, reacted by urging the Jewish community to abandon its passive, non-violent, and accommodating policies. The Bund set about organizing self-defense groups: it supplied arms, men, and most important, organization in combating the waves of pogroms. it argued that effective resistance required an armed group that would pose as great a threat to would-be pogromists as the latter posed to Jews. Nevertheless, since anti-Jewish propaganda had proved effective, to support the Bund on the issue of defense would only add fuel to the fire by giving the notion that all Jews were revolutionaries even more credibility. Alternative solutions had been advocated. The Zionist groups were in favor of increased emigration, but this offered no immediate assistance to the problem at hand. Others, especially the religiously orthodox, urged greater co-operation with the authorities. They also argued that pogroms were a manifestation of the will of God, and therefore one had to endure them.

Abstract

Objectives: To determine the spectrum of diseases that can involve the rectus abdominis muscle and sheath (RMS) and to describe the clinical features of these conditions. Method: A retrospective medical record review of RMS disorders seen at The New York Hospital--Cornell Medical Center from 1971 to 1992. Results: A total of 40 patients with diseases of the RMS were identified. Thirty patients had primary diseases of the RMS, most commonly desmoid tumor and hematoma. Secondary disorders of the RMS included abscesses from diverticulitis, a perforated sigmoid carcinoma, gallblader empyema, and disseminated actinomycosis. Eleven of 18 patients with desmoid tumors had familial adenomatous polyposis (FAP). The desmoid tumors in patients with FAP resulted in greater morbidity and mortality than those in patients without FAP. All patients with hematomas were on anticoagulation or had a history of trauma, vigorous coughing, or physical exertion. Conclusions: 1) RMS disease should be suspected in patients with a palpable abdominal mass and a history of familial adenomatous polyposis, trauma, anticoagulation., or vigorous coughing or exercise. 2) The most common non-neoplastic condition of the RMS is a hematoma. 3) The desmoid tumor is the most common neoplasm of the RMS. 4) Abdominal ultrasonography and CT readily distinguish RMS disease from intraabdominal pathology. Abstract

Reaction and revolution, so much a part of the reign of Nicholas II, became even more prevalent in the opening years of the twentieth century. The foundation of the Russian Empire had begun its erosion process which would topple the autocracy by 1917. Nicholas and his ministers had to act decisively and aggressively if they hoped to stem the process. To fight the wave of revolution and popular discontent in April of 1902, Nicholas appointed Viacheslav Pleve as Minister of the Interior. Pleve was a careerist who was devoutly loyal to the autocracy. In contemporary terms he might be described as an ‘aparatchick,‘ a party man who towed, and at times shaped, the party line. V. I. Gurko, a colleague, characterized him as “a legal clerk … a very superior clerk it is true, but a clerk nevertheless.” Pleve had also earned the dubious honor of being the autocracy's “most famous policeman,” a reflection of fourteen years of service as Director of the Police Department and Assistant Minister of the Interior.

Abstract

Russia has never been a good place to be a Jew, particularly in the last century. The history of Russian Jewry is one of discrimination punctuated by episodes of violence leading to mass emigration. It would be a mistake to understand these past 100 years as a culmination of a long tradition of Russian antipathy toward Jews. Prior to the Polish partitions in the late 18th century there lived only a handful of Jews in Russia. Violent attacks, or pogroms, were virtually nonexistent before 1881. When the violence did begin, damage to Russian Jewry was massive. Three times, between the years 1881 and 1921, Jews were beaten, killed and burned out of their homes. Each attack was more brutal than the preceding. Mass destruction, thousands killed, hundreds of thousands wounded, orphaned and rendered homelessthis was the legacy of pogroms. The first wave of pogroms took place in 1881, shortly after the assassination of Alexander II by the radical group the People's Will (Narodnaya Volya). The second wave swept the Pale of Settlement in 1903-06, during the years of the Russo-Japanese War and the Revolution of 1905. The third series unfolded against the backdrop of the Russian Civil War. The focus of this essay is pogroms occurring directly before and during the years of the 1905 Revolution. The first of these took place in the Bessarabian city of Kishinev, in April, 1903. After Kishinev, there were over 650 pogroms during the next three year period. By 1906, when the government of Nicholas II had regained its pre-Revolutionary control, the violence stopped, not to reappear again until the Civil War. The traditional interpretation places the responsibility for these events on the government of Nicholas II. According to this view, the tsar and his ministers willingly and consciously sponsored these attacks as a means of dealing with the burgeoning revolutionary movement. The traditional school explains pogroms as the autocracy's tool in coping with a disgruntled peasantry and a desperately poor working class.
