El Consenso Latinoamericano de Estreñimiento Crónico serealizó con el objeto de proveer guías para mejorar la iden-tificación, el diagnóstico y el tratamiento de este trastornoen la región. Dos coordinadores, y uno honorario, establecie-ron las líneas de consenso, basado en una revisión sistemáti-ca de la literatura médica de los últimos 10 años a partir de1995. Participaron 17 miembros con el aval de sus respecti-vas sociedades locales de gastroenterología. Éstos revisarony presentaron los temas con sus niveles de evidencia y gra-dos de recomendación para discutirlos en una reunión ple-naria. Tras un informe final de los miembros, los coordinadores prepararon las declaraciones finales para someterlas a votación en octubre de 2006. El consenso concluyó que elestreñimiento crónico tiene una prevalencia estimada del 5-21% en la región, con una relación mujer:varón de 3:1. El75% de los sujetos que lo presenta utiliza algún tipo de me-dicamentos y más del 50% usa medicamentos caseros. Se re-comendó un diagnóstico basado en los Criterios de Roma ylos estudios diagnósticos sólo en pacientes mayores de 50años o con algún signo de alarma. El uso de enema de colonse recomendó como investigación inicial en países con eleva-da frecuencia de megacolon idiopático y/o enfermedad deChagas. En cuanto al tratamiento, se recomendó incremen-tar la fibra en la dieta a 25-30 g/día (grado C) y no se encon-traron evidencias para ciertas medidas, como el ejercicio, elaumento de la ingesta de agua o las visitas programadas alexcusado. El Psylliumrecibió recomendación grado B y tra-tamientos farmacológicos, como tegaserod y polietilenglicolgrado A. Abstract: Androgen-independent prostate cancer cells DU-145 express a number of G protein-coupled receptors, including histamine H1 receptors. There is evidence for the presence of β-adrenoceptors in the human prostate, and in this work we set out to characterise the expression of β-adrenoceptors by DU-145 cells, their linking to cyclic AMP (cAMP) formation and the possible modulation by histamine H1 receptors of β-adrenoceptor function. Saturation [3H]-dihydroalprenolol binding indicated that DU-145 cells express moderate levels of β-adrenoceptors (22.7±2.5fmol/mg protein), which belong to the β2-subtype as assessed by inhibition by the antagonists ICI-118,551 and CGP-20712A. Inhibition of [3H]-dihydroalprenolol binding by agonists (noradrenaline, adrenaline and isoproterenol) showed the presence of both high-(53–59%) and low-affinity binding sites. β-Adrenoceptor stimulation with isoproterenol resulted in robust [3H]-cAMP accumulation (10–30-fold of basal, EC50 142nM; pEC50 6.85±0.05). While not having effect of its own on basal [3H]-cAMP accumulation, histamine significantly augmented the β2-adrenoceptor-induced response (overall effect 152±6% of isoproterenol alone) with EC50 1.35μM (pEC50 5.87±0.06). This effect was independent of extracellular Ca2+, insensitive to antagonists/agonists at H1, H2 or H3/H4 receptors and mimicked by drugs containing an imidazole ring in their chemical structure and by imidazole itself. Taken together, our results show that in DU-145 cells histamine augments β2-adrenoceptor-induced cAMP independently of the activation of known histamine receptors. The effect may involve other mechanisms such as allosteric modulation of β2-adrenoceptors by the imidazole moiety of histamine. [Copyright &y& Elsevier]

Aim is to determine whether lactate clearance has value as mortality prognostic factor in shock patients, or not. A scrutiny, descriptive, prolective, homodemic, longitudinal study in patients older than 19 years, excluding chronic renal illness, hepatic insufficiency, neoplasias and diabetics on biguanides treatment, was performed. Arterial gasometry was used to obtain lactate rates at admission, 6, 12, and 24 hours as well as lactate's clearance. Determinations between survivors and not survivors were compared, as well as correlation with APACHE II was performed. 41 patients were studied, 53.7% women and 46.3% men, mean age was 63.95 years. The diagnostics were septic shock in 29.2%, hypovolemic 63.4%, and cardiogenic 7.3%. The mortality in cardiogenic shock was 100%, with null lactate clearance. The survival rate was higher in hypovolemic shock as its lactate clearance rates (at 12 and 24 hours). Lactate clearance rates of less than 0 at 24 hours had a positive predictive value and a sensitivity of 80%. Lactate clearance at 6, 12, 24 hours among survivors and not survivors demonstrated statistical significance in the Univariate Analysis (p<0.002, p<0.000, p<0.000) respectively. The lactate clearance rate at 24 hours is a prognostic factor for mortality in patients in shock state. [ABSTRACT FROM AUTHOR]

Descriptive statistics were performed with the calculation of frequencies and percentages, and chi-squared tests were calculated., Results: Mean age was 61.3 years, 49 (52.1%) were women; 53 (56.4%) were left-sided and 41 (43.6%) right-sided. The main symptom was hematochezia in 32 (60.4%), in patients with left cancer; and diarrhea in 20 (48.8%), in patients with right-sided colon cancer. The presentation of stage I tumors and polyps, p = 0.044 and p = 0.043, respectively, was more frequent on the right side compared to the left side; in the left, hematochezia (p = 0.001), narrow stools(p = 0.05), and a history of type 2 diabetes mellitus (T2DM) (p= 0.036) were more frequent compared to the opposite site., Conclusions: Stage I and the presence of polyps were more frequent in right-sided cancer compared to left-sided cancer; T2DM, as well as hematochezia and narrow stools were more associated with the left side compared to the right side., (Licencia CC 4.0 (BY-NC-ND) © 2023 Revista Médica del Instituto Mexicano del Seguro Social.)

- Abstract
Objective: To determine the prevalence of pulmonary thromboembolism (PET) and identify it's risk factors in patients operated on a scheduled basis, in the general surgery service from the High Specialty Medical Unit Hospital de Especialidades de Puebla, of the Mexican Social Security Institute., Method: Descriptive, retrospective and cross sectional study of 52 patients' records with PET and pulmonary angiotomography whose indication was respiratory distress; from 2612 operated patients from November 2015 to March 2018, the sampling was not probabilistic. The statistics was descriptive, with measures of central tendency and dispersion. Fisher test, and Rho Spearman were used., Results: There were association between the development of PET and trans-surgical time (p = 0.018), with the D Dimer (p = 0.014), there was correlation between PET and Wells scale (p=0.001) and blood transfusion (p=0.044). Age, gender, body mass index, preoperative diagnosis, thromboprophylaxis, blood transfusion and surgical approach were not risk factors., Conclusion: The prevalence of postoperative PET in patients with respiratory distress after surgical procedure is 5.7% and the mortality is 33.3%., (Copyright: © 2020 Permanyer.)

- Abstract
Background: Colorectal cancer (CRC) is one of the main five causes of morbidity and mortality by oncologic diseases in our country and worldwide. Recently, fecal immunochemical test (FIT) has proven to be a noninvasive screening test that allows to select patients most likely to have a pre-malign lesion in order to perform a colonoscopy., Objective: To report the findings of a CRC screening program using FIT in our country population., Method: A multicentric study was performed, by inviting open population older than 50 years to participate in a CRC screening. Quantitative FIT specific for human hemoglobin was used, with a cut point of 100 ng/ml or higher to consider as positive. Those patients with positive results were asked to undergo a colonoscopy. In the cases where polypoid lesions were found, biopsies were performed., Results: In total, 751 FIT were processed, and 51 (6.8) of those were positive, with a rate of 15.9 premalign lesions for 1,000 individuals, and 1.3 patients with CRC for every 1,000., Conclusions: The present study matches worldwide reports, supporting the initiative of establishing a formal and standardized CRC screening program in the public health sector., (Copyright: © 2020 Permanyer.)

- Abstract
Background: Gastroesophageal reflux disease develops when the stomach contents causes troublesome symptoms and complications. Mild forms are non-erosive and erosive esophagitis, and severe forms are Barrett's esophagus and Esophageal adenocarcinoma. Matrix metalloproteinases are endopeptidases that can degrade components of the extracellular matrix, they play an important role in tumor invasion as well as in metastasis., Objective: To correlate the expression of metalloproteinase 9 (MMP-9) in esophageal biopsies from patients with mild and severe forms of Gastroesophageal reflux disease., Method: Cross-sectional study. The expression of MMP-9 was determined in biopsies of esophageal tissue of patients with mild and severe GRD. The included variables were age, sex, diagnosis, smoking and alcoholic habits, body mass index (BMI) and expression of MMP-9. Descriptive statistics was performed, Kappa for concordance in diagnosis as well as X2., Results: There were 50 patients, 32 (64%) men and 18 (36%) women, mean age 52.13 ± 14.75 years of age. 12 (24%) with smoking and 7 (14%) with alcoholism. Average BMI was 26.71 ± 4.07 kg/m
2 (15 to 33); 40 (80%) with obesity. The inter observer concordance for histopathological diagnosis was 1.0 and 0.84 for esophagitis degrees. 27 (54%) patients had esophagitis, 16 (32%) Barrett's esophagus and 7 (14%) esophageal cancer. There was expression of MMP-9 in four patients with esophagitis, five with Barrett's esophagus and five with esophageal cancer. Objective: To correlate ultrasonographic and histopathological diagnosis in patients categorized as breast imaging reporting and data system (BI-RADS) 4., Method: Cross-sectional study, performed on patients with BI-RADS 4 (A, B or C) diagnosed by ultrasonography in the high specialty medical unit, hospital of specialties, Centro Médico Nacional General de División Manuel Ávila Camacho, of the Instituto Mexicano del Seguro Social. Age, ultrasonographic and histopathological diagnosis, menarquia, menopause, gestations, lactation, use of contraceptives and family history of breast cancer were evaluated., Results: 101 patients categorized BI-RADS 4 were included; average age was 53.0 ± 8.3 years old, 6.9% of the patients were nulliparous, 92.2% with positive breastfeeding and 25.7% had a family history of breast cancer. By ultrasonography, 55.4% of the cases were BI-RADS 4A, 22.8% 4B and 21.8% 4C. 75.2% of the biopsies performed on the patients were benign and 24.8% were malignant. The overall ultrasonographic-histopathological concordance index was 0.25; 0.11 for BI-RADS 4A, 0.22 for BI-RADS 4B and 0.63 for BI-RADS 4C., Conclusion: The ultrasound-histopathological correlation is weak and inferior to that reported in the literature., (Copyright: © 2019 Permanyer.)

- Abstract
Introduction: The assessment of lymphoma response to treatment is based on imaging studies., Objective: To correlate the assessment of lymphoma treatment response by computed tomography (CT) and by positron emission tomography/computed tomography (PET/CT)., Method: Cross-sectional, observational study, where records of patients with lymphoma under surveillance by CT and PET/CT were reviewed., Results: The study population consisted of 43 patients with a mean age of 32.7 ± 22.4 years; 26 (60.5 %) had a diagnosis of Hodgkin's lymphoma and 17 (9.5 %) had non-Hodgkin lymphoma. By CT, 34 (79.1 %) were diagnosed with disease and nine (20.9 %) without disease. The criteria used to assess the response was radiologist experience in 39 (90.7 %) and RECIST 1.1 criteria in four (9.3 %). The diagnosis by 18-FDG PET/CT was no response to treatment or partial response-recurrence in 32 (74.4 %) and response to treatment in 11 (25.6 %); with PERCIST criteria in 13 (30.2 %) and Deuaville criteria in 30 (69.8 %). When the diagnosis by CT versus 18-FDG PET/CT was compared, out of 11 patients with complete response on PET/CT, three had a similar CT diagnosis. Of the 34 patients with data consistent disease diagnosed by CT, 26 had similar results by 18-FDG PET/CT (p = 0.54)., Conclusion: The value of lymphoma treatment response on CT does not agree with that obtained by 18-FDG PET/CT., (Copyright: © 2019 Permanyer.)

- Abstract
Background: The test for evaluating phonological simplification processes (TEPROSIF, according to its initials in Spanish) is a tool which is used to identify dyslalias. Our objective was to establish, by using TEPROSIF, the association between dyslalias and malocclusion in children from 4 to 6 years of age., Methods: After we standardized the tool with a kappa of 0.9, we applied it to 116 children from 4 to 6 years of age. Patients were conducted to a central occlusion through deglutition. We observed type of bite, overbite, occlusion and terminal plane in order to relate them to the dyslalia presented. For statistical analysis, we used chi-squared test (p < 0.05)., Results: They were 55 (47.4 %) boys and 61 (52.6 %) girls, with a median age of 5 years ± 0.71. Phoneme substitution was the most common alteration. We found significant correlation between omission of the phonemes and terminal plane (p = 0.01), Baume type I arch (p = 0.00) and absence of teeth (p = 0.00), as well as between phoneme substitution and terminal plane (p = 0.03), bite type (p = 0.01) and absence of teeth (p = 0.00)., Conclusions: Phoneme substitution is the most common alteration, followed by omission and distortion of phonemes in children between 4 and 6 years of age.

- Abstract
Background: Idiopathic thrombocytopenic purpura is characterized by extravasation of blood into the subcutaneous tissue, mucous membranes or skin; it generates bleeding manifestations as ecchymotic lesions, petechiae of sudden onset, epistaxis, bleeding from the gums, and serious complications such as intracranial hemorrhage. This condition is due to a platelet destruction mediated by antibodies directed against the surface of the platelets. The objective is to present the clinical characteristics of patients with idiopathic thrombocytopenic purpura in a third level medical facility in Puebla, México., Methods: A descriptive study, which included 200 patients from the Hematology Service, was carried out. All patients included had the diagnosis of idiopathic thrombocytopenic purpura. We studied their clinical manifestations, type of diagnosis, as well as medical and/or surgical treatment. The statiscal analysis was made using descriptive measures., Results: The entire sample (200 patients) had idiopathic thrombocytopenic purpura as a diagnosis established, 57 % had ecchymoses, 42 % petechiae, 23 % bleeding from the gums, 31.5 % epistaxis, 6.5 % abnormal uterine bleeding, 11 % malaise, 7.0 % hemorrhage, 0.5 % pruritus, 0.5 % seizures, 8 % two or more signs and symptoms and 73 % was asymptomatic when the diagnosis was established., Conclusions: Symptoms of idiopathic thrombocytopenic purpura are varied, a large percentage of patients can be processed unnoticed. The results described were similar to those reported in the literature.

82. [Use of collagen-polyvinylpirrolidone in the treatment of Parry-Romberg syndrome.]

- Abstract
Background: Parry-Romberg syndrome is characterized by the presence of progressive hemifacial atrophy that affects the growth and development of structures on one side of the face. Our objective was to introduce collagen-polyvinylpirrolidone as a new therapeutic option for Parry-Romberg syndrome in two clinical cases. Clinical cases: two women, aging 56 and 28 years old, with facial hypocrhomic lesions, and right and left fronto-malar sunken area respectively, treated with topic steroids + penicilamina, fat grafts and silicone implants without improvement. We used collagen-polyvinylpirrolidone and they showed improvement: after six months there were not clinical or esthetical complications. Conclusions: collagen-polivinilpirrolidone could be a new therapeutic option for Parry-Romberg syndrome with good clinical and esthetical results. It is easy to apply and it has few side effects and without complications.

- Abstract
Background: one of the most used scales for the evaluation of children's pain is the CHEOPS. This is complex, reliable but not commonly used in spanish. We decided to create a new Pain Facial Scale to be considered to be used in spanish., Objective: to compare the validity and consistency of two different scales for the evaluation of post-operative pain in children., Methods: process, comparative, longitudinal, homodemic, and prolective study. It was elaborated a simple and easy scale, to evaluate the post-operative pain in children. 5 experts evaluated their appearance and content. The original version of CHEOPS was translated to the Spanish by an expert; later this version was corrected and re-translated by a native Anglo-Saxon speaker, the result submitted for evaluation by 5 experts. The validity and consistency of both scales were evaluated by two investigators in a blind way. We used Cronbach's α for the internal consistency of CHEOPS, coefficient of intraclass correlation for the external consistency (inter observer's variability), effect size for sensitivity to the change of category, change of status for internal validity and Spearman's correlation for the convergent analysis., Results: there was great external consistency, and a good and high internal validity, for the Spanish version of the CHEOPS scale, and an excellent internal validity for the Facial Pain Scale, as well as an excellent internal validity for both scales., Conclusions: two scales can be used to evaluate the post-operative pain in children between 4 and 8 years old.

Background: There are multiple therapeutic options for the management of constipation, from lifestyle modifications to the use of laxatives and in extreme cases surgery., Objectives and Methods: To establish the clinical guidelines for diagnosis and treatment of chronic constipation in Mexico we conducted a review of the literature regarding medical and surgical treatments for chronic constipation and have made recommendations based on evidence., Results: Low water consumption, physical inactivity and low intake of fiber are conditions associated with chronic constipation, but the evidence to prove these associations is scarce. Bolus forming agents are useful in the management of constipation with normal colonic transit and defecation without dissynergia. Evidence supports the use of lactulose (IB) and polyethylene glycol (IA) as the most safe and effective agents in the long term in adults. The use of stimulant laxatives (docusate, picosulfate, senna) is recommended only for short periods. Tegaserod is an agonist of 5-HT4 receptors and there are many clinical trials supporting its effectiveness in the management of functional constipation (IA). However "their cardiovascular safety has been questioned recently. Biofeedback therapy is the gold standard in the management of constipation associated with pelvic floor dyssynergia. Surgical treatment is reserved for extreme cases of colonic inertia., Conclusions: The treatment of constipation should be based on the underlying pathophysiological mechanisms and the selection of drugs must be made according to the scientific evidence.

- Abstract
Background: Approximately 30% of women who undergo mastectomy without reconstructive treatment due to breast cancer present sequelae. These include paresthesias, keloid healing, hypoesthesia, lymphedema and limitation of the function of the ipsilateral upper extremity. We undertook this study to present the results using collagen-polyvinylpyrrolidone (Clg- Pvp) as treatment for posmastectomy sequelae in women with breast cancer., Methods: We conducted a unicentric, longitudinal and prospective clinical trial between August 1, 2007 and July 31, 2008. Included variables were age, lymphedema, limitation of the function of the ipsilateral upper extremity, collapse of the wound, keloid healing, paresthesias, and appearance of the surgical area. The appearance of the surgical area (aesthetic aspect) was evaluated before and 6 months after treatment was initiated. Clg-Pvp was administered weekly for a 6-month period., Results: Seven women were included with a median age of 49 years (range: 40-72 years). One patient (14.28%) presented lymphedema, two patients (28.57%) presented collapse of the wound, two patients (28.57%) had keloid healing, three patients (42.85%) experienced paresthesias, five patients (71.4%) reported pain, and five patients (71.4%) reported limitation of the function of the ipsilateral upper extremity. At the completion of the treatment, aesthetic improvement was statistically significant (p = 0.0020, Mann-Whitney U test)., Conclusions: Clinical and aesthetic results are good after application of Clg-Pvp for treating sequelae in women with breast cancer who underwent mastectomy without reconstructive surgery.

- Abstract
The Latin-American Consensus on Chronic Constipation aimed to establish guidelines to improve the identification, diagnosis and treatment of this disorder in the region. Two coordinators and an honorary coordinator established the process and the topics to be discussed, based on a systematic review of the literature published in the previous 10 years, since 1995. Seventeen members participated with the support of their local gastroenterology societies. The members reviewed the different subjects based on the levels of evidence and grades of recommendation; the topics were then discussed in a plenary session. A written report was drafted and the coordinators prepared the final declarations to be submitted to a vote by all the members in October 2006. The consensus concluded that chronic constipation has an estimated prevalence of 5-21% in the region, with a female-to-male ratio of 3:1. Among individuals with constipation, 75% use some type of medication, with more than 50% using home remedies. A diagnosis based on Rome Criteria was recommended and diagnostic testing only in persons older than 50 years or with alarm symptoms. The use of barium enema as an initial investigation was recommended only in countries with a high prevalence of idiopathic megacolon or Chagas' disease. Recommendations on treatment included an increase in dietary fiber of up to 25-30 g/day (grade C). No evidence was found to recommend measures such as exercise, increased water intake, or frequent visits to the toilet. Fiber supplements such as Psyllium received a grade B and pharmacological treatments such as tegaserod and polyethylene glycol, both grade A. There was insufficient evidence to recommend lactulose, but the consensus did not disadvise its use when necessary. Complementary investigations such as colonic transit followed by anorectal manometry and defecography were only recommended to rule out colonic inertia and/or obstructive defecation in patients not responding to treatment. Biofeedback was recommended (grade B) for those with pelvic dyssynergia.

Background: Gynecomastia describes a benign increase of the mammary gland in men. When medical treatment fails, symptoms and psychological alterations persist. Subdermal mastectomy is the definitive treatment and can be achieved by different incisions, each with potential complications. We undertook this study to present clinical characteristics of 11 patients with gynecomastia and the results obtained with subdermal mastectomy by means of external periareolar incision., Methods: A descriptive cohort study in male patients with gynecomastia was carried out in a third-level medical care hospital. Patients were treated with subdermal mastectomy by means of external periareolar incision., Results: There were 11 male patients with an average age of 19 years (range: 11-60 years), 3 patients (27.2%) with bilateral gynecomastia and 8 patients (72.7%) with unilateral gynecomastia. Average time of evolution was 22 months (range: 16-48 months), 9 patients (81.8%) reported pain, 11 patients (100%) reported psychological alterations with cutaneous alteration, 11 patients (100%) had normal secondary sexual characteristics, 1 patient (9%) had supernumerary nipple development, and 11 patients (100%) had well-defined lesions. According to Simon's classification: seven patients (63.6%) were classified as grade 1, three patients (27.2%) as grade 2 and one patient (9.09%) as grade 3. Each patient had a subdermal mastectomy with external periareolar incision, 11 patients (100%) had a histopathological report of gynecomastia; 1 patient (9.09%) displayed keloid healing and none displayed complications inherent to the surgical procedure., Conclusions: Mastectomy by means of external periareolar incision is useful in the treatment of gynecomastia.

Objectives: To determine the clinical features, as well as to present an analysis of the cases of maternal death during a period of ten years., Patients and Methods: A descriptive, transverse, retrospective and cases series research was carried out. There was a revision of the expedients of maternal death occurred at the Regional General Hospital number 36 in Puebla, from January 1st, 1996 to December 31st, 2005. They were selected the patients who passed away because of obstetric death. The data collection included the following variables: age, body mass index, education, number of gestations, type of labour, prenatal control, basic cause of dead, and the month and the year in which it occurred. The descriptive statistics was used with central tendency and dispersion measurements., Results: 75 maternal deaths were registered; one of them was excluded because it was not an obstetric cause. The maternal death rate was 63.46 per 100,000 born alive. The 17.5% corresponded to indirect obstetric causes and 82.5% to direct obstetric causes; the 73% were preventable, 27% were no preventable. Forty-eight cases corresponded to ages between 21 and 34 years. Forty-six cases corresponded to mothers with 2 to 4 pregnancies. In 43 cases they had a level of education which ranks between elementary and junior high school. In 31 cases they had a body mass index of 21 to 26. The eclampsia was the cause of maternal death in 20 cases and the obstetric haemorrhage in 16 cases. There were appropriate prenatal controls in 38 cases., Conclusions: The maternal death cases corresponded to an average age women, multigravidas, no obese, and apparently appropriate prenatal control. During this period there was a tendency to the decrease of obstetric haemorrhage and an increase of the preeclampsia-eclampsia as causes of maternal death.

- Abstract
Objective: To examine the efficacy and safety of oxitocin administered through umbilical vein after delivery to diminish the incidence of retained placenta, blood loss and third stage of labour., Methodology: A double blind, clinical trial was performed in which 64 women in labour, any age, with normal pregnancies, with gestational time between 37 to 42 weeks, were randomized to one of two groups. Group 1 received 10 UI of oxitocin (2 mL) diluted in 18 mL of saline solution through umbilical vein, immediately after delivery. Group 2 received only 20 mL of saline solution. Blood loss, time of third stage of labour, retained placenta, nausea, vomit and arterial hypotension were measured., Results: Blood loss was 263.7 +/- 220.9 mL in group 1 (n=32) and 286.7 + 230.4 in group 2 (n=32) (p=0.68, Student T test). Time of third stage of labour was 265.3 +/- 383.9 seconds in group 1 and 197.1 +/- 314.3 in group 2 (p=0.44, Student T test). There was one retained placenta in group 1 and two in group 2 (RR 0.5 IC 0.04 a 5.24). There were not side effects in any group., Conclusions: Oxitocin trough umbilical vein immediately after delivery was not useful to diminish blood loss, retained placenta or time of third stage of labour.
