Fifty-two patients with systemic lupus erythematosus (SLE) were examined by fluorescein angiography, and retinopathy was detected in 15. Three patterns of retinopathy were discerned: 4 patients had disc vasculitis, 6 had multiple cotton-wool spots, and 5 had a normal fundal appearance but fluorescein leakage on angiography. One patient had arterial occlusive disease with retinal neovascularisation and another had extensive venous disease. With 3 exceptions retinopathy was found only in patients with active SLE. No association was discovered between retinopathy and cerebral disease; in particular, fluorescein angiography did no assist the diagnosis of mild cerebral lupus. [ABSTRACT FROM PUBLISHER]

To determine the relationship between vitreous fluorophotometry (VF) and severity of diabetic retinopathy (DR) 13 patients with mild to moderate background DR starting continuous subcutaneous insulin infusion were followed up serially for 12 months. They were studied by colour stereo retinal photographs, which were assessed by the Wisconsin Grading System, and by VF, which was assessed by the permeability index of the retina for fluorescein. By four months the severity of DR on colour photographs had deteriorated by at least one level in at least one eye in eight patients. In four patients the DR did not deteriorate. Assessment of anatomical abnormalities by severity of DR on colour photographs correlated well with the functional abnormalities of the blood-retinal barrier(s) assessed by VF, especially for the macular field. Comparison of permeability index data in the patients developing preproliferative or proliferative features of DR (group A) with the same data in patients who did not develop such changes (group B) indicated that group A patients had more severe DR than did group B patients at entry. Grading of colour photographs showed a similar trend but with greater overlap. Considerable overlap in fluorescein permeability remained between those subjects with no visible DR and those with microaneurysms with or without haemorrhages and small hard exudates. The blue light entopic phenomenon was used to measure retinal blood flow in 87 diabetics and 10 normal controls in a longitudinal study. The time interval between the initial and final studies was 21-39 months (mean 31.4). The retinal blood flow velocity did not change in the 10 subjects in the control group nor in those nine patients with no retinopathy, all but one of whom remained free of retinal lesions. In the 24 with background retinopathy the flow velocity fell significantly from 0.71 (SD 0.35) mm/s to 0.48 (0.13) mm/s (p less than 0.01). The velocity also decreased in the preproliferative group of 16 patients from 0.62 (0.39) mm/s to 0.41 (0.14) mm/s (p less than 0.05). The retinopathy status changed in 11 of this group. The value of the Canon CR3-45NM non-mydriatic fundus camera in screening for diabetic retinopathy has been assessed in two ethnic groups, namely, 45 Indian and 40 European diabetic patients. There was 72% agreement between ophthalmoscopy by an ophthalmologist and the photographic assessment, and 100% agreement on clinically important lesions requiring treatment. However, peripheral retinal lesions, seen on ophthalmoscopy but lying outside the photographic field, occurred in 4% of eyes. 6% of photographs were totally unassessable and an additional 12% were only partially assessable. The prevalence and severity of retinopathy was similar in Indian and European patients. The case history of a 33-year-old diabetic patient who has had diabetes for 24 years is presented. When first seen in 1975 he had bilateral proliferative retinopathy with new vessels in the retinal periphery. He had large areas of capillary non-perfusion lateral to the macula in the right eye associated with the new vessels. Nine years later, after extensive repeated photocoagulation, revascularisation of large areas previously not perfused were seen. Retinal blood flow velocity was measured in 24 normal volunteers and compared with the results obtained in 101 diabetics at different stages of retinopathy. The blue-light entoptic phenomenon was used to measure retinal flow velocity. Results showed that there was a significantly higher capillary flow velocity in those patients with background retinopathy (0.74 +/- 0.32 mms/s) over the group of normals (0.54 +/- 0.19 mm/s) and those diabetics with no retinopathy (0.51 +/- 0.24 mm/s). Retinal flow velocity was reduced at the preproliferative stage of retinopathy to 0.37 +/- 0.2 mm/s. Proliferative retinopathy showed a slightly greater flow velocity (0.56 +/- 0.27 mm/s), which was reduced following photocoagulation (0.42 +/- 0.14 mm/s). [ABSTRACT FROM PUBLISHER]

A study of 62 untreated eyes of 54 diabetic patients with peripheral neovascularisation is reported. The study shows that neovascularisation rarely remains confined to one sector of the retina; 18% of eyes had developed disc new vessels within one year, 27% had a vitreous haemorrhage within five years, and 10% developed either macular traction or a retinal detachment. A study of 90 eyes in 72 diabetic patients with peripheral retinal neovascularisation treated by photocoagulation indicated that the subsequent development of further peripheral or disc new vessels is infrequent if panretinal photocoagulation is the initial treatment procedure. Focal or sectorial treatment may be considered only if a small area of neovascularisation is present initially and provided regular follow-up can be guaranteed. [ABSTRACT FROM PUBLISHER]

This paper reports on 9 patients aged 13-18 years (mean 15.4) with severe diabetic retinopathy. Two patients were prepubertal and 4 were going through puberty. Hypertension was present in 2 patients, while 4 had proteinuria. Severe preproliferative disease was present in 3 patients initially and proliferative retinopathy in the remainder. In 5 this retinopathy was considered to be florid. Two patients seen prior to 1975 had pituitary ablation, while those seen after 1975 were treated by extensive argon and xenon arc photocoagulation. Proliferative lesions regressed in both groups. At the latest follow-up 7 of the 9 patients achieved a final visual acuity of 6/9 or better in at least one eye. One patient became blind. We report the case history of a 20-year-old diabetic girl with long-standing poorly controlled disease who presented with severe retinopathic features and who was treated with continuous subcutaneous insulin infusion (CSII). When first seen she had macular oedema and a visual acuity of 6/18 in her only eye, together with multiple blot haemorrhages and intraretinal microvascular abnormalities. Marked reversal of the retinal lesions occurred within 65 days of initiating SCII and after only 36 days of constant normoglycaemia. Subsequent good blood glucose control on conventional subcutaneous insulin injections has been achieved, and the retinopathy remains quiescent with a visual acuity of 6/5. Pathological changes in the optic disc and anterior part of the optic nerve of monkeys were studied 5 hours and several days after occlusion of the temporal short posterior ciliary arteries. Ischaemic vacuolation of neural tissue was observed in some of the animals studied and was restricted to the lamina cribrosa and the immediately retrolaminar portion of the optic nerve. Axonal swelling and organelle aggregation were demonstrated in the prelaminar region, and autoradiography after intravitreal injection of tritiated leucine revealed an associated obstruction of rapid and slow orthograde axoplasmic transport. The experimental findings correlate with those seen clinically in acute ischaemic optic neuropathy. The pale swelling of the optic disc in this condition represents an accumulation of axoplasmic debris in retinal ganglion-cell axons owing to obstruction of axoplasmic transport at the lamina cribrosa; it is thus equivalent to a 'cotton-wool spot' of the disc. Four cases of diabetic maculopathy with excessive permeability are described. Fluorescence angiography is distinctive, showing profuse early leakage from the entire capillary bed of the posterior pole. Careful studies have failed to reveal any cause for this excessive permeability response at the macula or any constantly associated medical abnormality. The prognosis for visual acuity is poor, and photocoagulation has only rarely been successful in maintaining or improving vision in these patients. [ABSTRACT FROM PUBLISHER]

Experimental branch vein occlusion by laser photocoagulation in the rhesus monkey leads to early, marked, and lasting reduction of blood flow to the affected retinal region. The radioactive microsphere method demonstrates this reduction of blood flow in both gamma counting and autoradiography. The reduction of flow becomes evident as early as 2 hours after occlusion and persists up to 1 week. The mechanism of the altered flow is discussed. [ABSTRACT FROM PUBLISHER]

Branch and hemisphere vein occlusion were produced in rhesus monkeys by argon laser photocoagulation. The following observations were made: (1) Immediately after occlusion there was venous dilatation, delayed filling of the artery, delayed drainage by the occluded vein, and capillary leakage. (2) Two patterns of evolution were identified within the first week. In some animals the fundus changes resolved and the retina returned to normal, while in others there was progressive retinal capillary closure. (3) Those animals destined to have capillary closure had diffuse or cluster retinal haemorrhages at 24 hours. (4) Capillary closure took place over 1 week and was usually complete over large areas of retina. (5) Retinal atrophy and major vessel changes occurred over several weeks. (6) Retinal revascularisation occurred in those areas of closure, though this was often limited. It was concluded that the early changes mimicked those seen in human retinal vein occlusion, though persistent retinal oedema and preretinal neovascularisation were not identified.
