Skin prick testing (SPT) in combination with the clinical history of the patient is one important step in the diagnosis of IgE-mediated occupational allergies. However, skin test performance is related to the quality of allergen extracts. The present consensus document was prepared by an EAACI Task Force consisting of an expert panel of allergologists and occupational physicians from Germany, Italy, Spain, France, Austria, and Poland. All members of the panel were also involved in the data collection within the European multicentre study STADOCA ( Standard diagnosis for occupational allergy). The aim of this Task Force was the assessment of the quality of commercially available SPT solutions for selected occupational allergens under standardized procedure conditions in different European centres and institutes of Occupational Medicine. The data evaluation shows a wide variability among SPT solutions and also indicates that the sensitivity of several SPT solutions is low. Therefore, improvement and standardization of SPT solutions for occupational allergens is highly recommended. Clinical practitioners should also not presume that their SPT solutions are fully reliable. The main objective of the document is to issue consensus suggestions for the use of SPT with occupational allergens based on the European multicentre study STADOCA, on existing scientific evidence and the expertise of a panel of allergologists. Background Skin prick testing ( SPT) is an important step in the diagnosis of Ig E-mediated occupational allergic diseases. The outcome of SPT is related to the quality of allergen extracts. Thus, the aim of the study was to assess different commercially available SPT solutions for selected occupational allergens. Methods SPT was performed in 116 bakers, 47 farmers and 33 subjects exposed to natural rubber latex ( NRL), all with work-related allergic symptoms. The SPT solutions from different manufacturers ( n = 3-5) for wheat flour, rye flour, soy, cow hair/dander, storage mites ( Tyrophagus putrescentiae, Lepidoglyphus destructor, Acarus siro) and NRL were analysed with respect to their protein and antigen contents. SPT was carried out in 16 allergy centres in six European countries using standardized procedures. Specific Ig E values were used as the gold standard to calculate the sensitivity and specificity of SPT solutions. The optimal cut-point for each SPT solution was determined by Youden Index. Results Protein and antigen contents and patterns of the SPT solutions varied remarkably depending on the manufacturer. While SPT solutions for wheat flour and soy reached overall low sensitivities, sensitivities of other tested SPT solutions depended on the manufacturer. As a rule, solutions with higher protein and antigen content showed higher sensitivities and test efficiencies. Conclusions There is a wide variability of SPT solutions for occupational allergens, and the sensitivity of several solutions is low. The aim of the study was to examine the relationship between the distance from the Vistula river and the distribution and mobility of Zn, Cu and Mn within profiles of Fluvisols from the Vistula River Valley (Unislaw Basin, Poland). The alluvial soils in this area were recurrently flooded by the river once or twice a year what changes their physico-chemical and biological properties. The range of the floods was varied and of local character, only sometimes exceeded 600-900 m from the river. Over the last 10 years such a flood was noted just once. The total contents of Zn ranged from 30.1 to 228.5 mg·kg-1, Cu from 4.6 to 26.1 mg·kg-1 and Mn from 261.0 to 1560.8 mg·kg-1. These contents of analyzed heavy metals did not exceed environmental quality standards for the soils used currently in Poland. However elevated concentrations of Zn and Mn were observed in profiles near the bank of the Vistula (50 m profiles M1, M2 and 200 m profiles M3, M4). Such a clear difference in the concentrations of heavy metals at the zone of 600-900 m from the river suggests a predominating effect of flooding waters. In samples with high pH values, mobile forms of Zn and Cu were low. The highest content of DTPA exctractable metal forms were in the surface horizons, and gradually decreased with depth along with a decrease in organic matter. Enzymatic activity differentiated significantly in the investigated soil profiles. Soil samples taken from the surface horizon (0-10 cm) had the highest enzymes activity as compared to their activity found in soil samples from deeper layers. The article presents analysis on the results of 171 second-look laparotomy (SLL) and equate survival of patients having advanced ovarian cancer. The study reveals that the survival rate in patients having recurrence after negative SLL was importantly lower equated to patients with microscopic disease. It shows that probable explanation for encouraging prognosis in the group having microscopic disease was early medication of chemotherapy after SLL.

Hepatopulmonary syndrome (HPS) is associated with increased waitlist mortality in liver transplantation (LT) candidates. Children with HPS are granted Model for End-Stage Liver Disease (MELD)/Pediatric End-Stage Liver Disease (PELD) exception points for waitlist prioritization in the United States based on criterion developed for adults. In this study, the impact of this MELD/PELD exception policy on post-LT survival in children was examined. A retrospective cohort of patients aged younger than 18 years with a MELD/PELD exception request who underwent LT between 2007 and 2018 were identified in the Scientific Registry of Transplant Recipients. Patients were stratified by waitlist partial pressure of arterial oxygen (PaO 2 ) to assess risk factors for waitlist mortality and post-LT survival. Among 3082 pediatric LT recipients included in the study, 124 patients (4%) received MELD/PELD exception points for HPS. Patients with HPS were a median age of 9 years (interquartile range: 6, 12 years), 54.8% were girls, and 54% were White. Most patients (87.9%) were listed with laboratory MELD/PELD scores <15. Waitlist mortality for patients with HPS exception points was rare and not different from patients without HPS. When stratified by pre-LT PaO 2 , hypoxemia severity was not associated with differences in 1-, 3-, or 5-year survival rates after LT ( p = 0.13). However, patients with HPS showed a slightly lower survival rate at 5 years compared with patients without HPS (88.7% vs. 93.4%; p = 0.04). MELD/PELD exceptions for children with HPS mitigated waitlist mortality, and recipients with HPS experienced excellent 5-year survival after LT, although slightly lower than in patients without HPS. Unlike adults with HPS, the severity of pre-LT hypoxemia in children does not impact post-LT survival. These data suggest that adult criteria for granting MELD/PELD exception points may not appropriately capture HPS severity in pediatric patients. Formalized classification of the class Isoëto-Nanojuncetea has not been performed in Poland. We used 69,562 relevés stored in Polish Vegetation Database. Based on the literature and expert knowledge we selected 63 diagnostic species for the Isoëto-Nanojuncetea class. Unequivocal classification was applied in this work according to Cocktail method. A set of formal definitions was established using a combination of logical operators of total cover of species in case of high-rank syntaxa while sociological species groups and cover of particular species were used for logical formulas describing class, alliances and associations. An Expert System was prepared and applied to classify the whole data set of PVD and 1,340 relevés were organized at the class level. We stratifies the data and finally we used data set of 903 relevés to prepare synoptic tables, distribution maps and descriptions of the syntaxa. Twelve associations and two plant communities were identified. Vegetation of the Isoëto-Nanojuncetea class occur in Poland's central and southern part, with scattered stands in northern region. We described two new plant communities within Eleocharition and Radiolion alliance. Background: Radiation-related screening guidelines for survivors of childhood cancer currently use irradiated regions (IR) to determine risk for late effects. However, contemporary radiotherapy techniques utilize volumetric dosimetry (VD) to determine organ-specific exposures, which could inform need for late effect surveillance., Methods: This cross-sectional cohort study involved patients treated for cancer using computerized tomography-planned irradiation at Children's Hospital Los Angeles from 2000-2016. Organs at risk were identified using both VD and IR. Under each method, Children's Oncology Group Long-Term Follow-Up Guidelines were applied to determine radiation-related potential late effects and their correlative recommended screening practices. Patients served as their own controls. Mean number of potential late effects per patient and recommended screening practices per patient per decade of follow-up were compared using paired t-tests; comparisons were adjusted for diagnosis and gender using random effects, repeated measure linear regression., Results: In this cohort (n = 132), median age at end of treatment was 10.6 years (range, 1.4-20.4). Brain tumor was the most common diagnosis (45%) and head/brain the most common irradiated region (61%). Under IR and VD, the mean number of potential late effects flagged was 24.4 and 21.7, respectively (-11.3%, p < 0.001); concordance between the two methods was 6.1%. Under VD, the difference in mean number of recommended screening practices per patient was -7.4% in aggregate but as large as -37.0% for diagnostic imaging and procedures (p < 0.001 for both)., Conclusion: Use of VD rather than IR is feasible and enhances precision of guideline-based screening for radiation-related late effects in long-term childhood cancer survivors., (© 2020 The Authors.
