53. Breast cancer treatment-related lymphedema self-care: education, practices, symptoms, and quality of life.

Abstract

Purpose: The primary purpose of this study was to cross-sectionally examine breast cancer treatment-related lymphedema self-care education, self-care practices, and perceived self-care barriers, burdens, and benefits. We also explored the associations among self-care education, practices, symptoms, and quality of life (QOL) in breast cancer survivors with known lymphedema.Methods: A cross-sectional design was used to obtain data about lymphedema self-care education, self-care practices, perceived self-care barriers, burdens, and benefits, symptoms, and QOL. Frequency distributions and Spearman rank-order correlations were used to analyze quantitative data. Thematic qualitative analysis of perceived barriers, benefits, and burdens of self-care was also undertaken.Results: Surveys were completed and returned by 51 of 58 eligible individuals. Of the 51 responding, 48 had received some self-care education, but three did not think it was adequate. Wearing a compression garment was the most frequently cited self-care activity, and 18 of the 51 required help with self-care. Thirty-three percent, 17/51, spent 15 minutes or less per day on self-care. Multiple barriers to self-care were identified. Those with more symptoms spent more time on self-care activities and had a poorer QOL.Conclusions: Opportunities exist to improve lymphedema self-care education. Breast cancer survivors with lymphedema experience symptoms beyond that of simple swelling of the affected limb. Self-care is burdensome. Barriers to providing lymphedema self-care exist and may vary across patients. A multidisciplinary approach to lymphedema management, including self-care education and monitoring, is likely needed to improve QOL in this population. [ABSTRACT FROM AUTHOR]

60. Assisted living residents' perspectives of their occupational performance concerns.

Abstract

Assisted living residents' concerns regarding their occupational performance are reported. Researchers interviewed twenty-two residents from four different assisted living facilities using the Canadian Occupational Performance Measure, a tool assessing concerns with valued life activities. Most common concerns were lack of desired active recreation, mobility, and passive recreation. Residents attributed identified concerns to physical health conditions and vision, along with facility features. Differing styles of life acceptance, degree to which residents felt like the facility was home, and degree to which activity options were meaningful vs. just 'keeping busy' were themes that emerged. Abstract

The life-stage mortalities of the bark-beetle pest P. scarabaeoides were studied in a Spanish olive orchard by placing cut logs in the field for adult females to breed. Mortality was found to be greatest at the larval stage with about 25% of the original egg number surviving to adulthood. The density of eggs laid per maternal gallery does not appear to affect the degree of subsequent mortality at the egg, larval or pupal stages. However, the density of maternal galleries on a log has a positive (density-dependent) influence on the mortality of larvae, but not of eggs or pupae. The density-dependent larval mortality could be attributed to both insect parasitoids and intraspecific competition, but the relative contribution of each was unclear. The proportion of adult females excavating maternal galleries was also shown experimentally to be density dependent. A simple model, based on the information presented, showed that the population is regulated primarily by density-dependence in the level of female attack, stabilizing the population at three attacking females/dm2. Mortality of overwintering adults in the trees is unlikely to affect this equilibrium significantly. [ABSTRACT FROM AUTHOR]

Abstract

Female Dicondylus indianus were provided with host densities of 2, 4, 8, 10, 20, 40 or 60 for the duration of their lives. Rates of oviposition (and consequently of parasitism) and host-feeding varied both with host availability and with parasitoid age. The period of intensive egg-laying shifted from a later to an earlier age as host availability increased. Whilst the wasps both oviposited and fed less at lower host densities, the ratio of feeding: oviposition attacks (F: O ratio), measured on both a lifetime total and age-specific basis, increased with decreasing host availability, in accordance with the predictions of the behavioural models of J ervis and K idd (1986). There is a strong positive relationship between a wasp's lifetime fecundity and the number of hosts fed upon over its lifetime. In wasps supplied with hosts, longevity was shortest at the lowest host densities, but it exceeded the longevity of wasps deprived of hosts. The total number of eggs laid by a female over her lifetime was a strongly positive function of lifespan. Zusammenfassung Zum Einfluß der Verfügbarkeit der Wirte auf Eiablage, Fraßverhalten und Lebensdauer von Dicondylus indianus Olmi (Hym., Dryinidae), ein Parasitoid von Nilaparvata lugens Stål (Hern., Delphacidae) Dicondylus indianus-Weibchen wurden während der gesamten Lebensdauer bei Wirtsdichten von 2, 4, 8, 10, 20, 40 oder 60 Tieren gehalten. Sowohl die Eiablagerate (und damit verbunden die Parasitierungsrate) als auch die Fraßhäufigkeit an den Wirten änderte sich mit der Verfügbarkeit der Wirte und auch mit dem Alter der Parasitoide. Mit zunehmender Wirtsverfügbarkeit waren die Weibchen zur Haupteiablagephase zunehmend jünger. Während D. indianus bei niedriger Wirtsdichte weniger Eier ablegte und weniger fraß, nahm das Verhältnis Fraßangriffe: Eiablage mit abnehmender Wirtsdichte zu. Diese Beobachtung stimmt mit den Voraussagen des Verhaltensmodells von J ervis und K idd (1986) überein. Abstract: Following antennal contact with the honeydew of the black scale (Saissetia oleae Olivier), female adult green lacewings (Chrysoperla carnea Stephens) showed a reduced mean walking speed (inverse orthokinesis), an increased mean turning frequency (positive klinokinesis), and an increased mean turning angle. Similar, but generally less pronounced, behavioural changes occurred after contact with solutions of acid hydrolysed L-tryptophan and water. Reduced walking speed, lower mean turning frequency and increased mean turning angle were also observed in insects exposed to honeydew during the period prior to antennal contact with the honeydew. The observed alterations in behaviour in response to honeydew could increase the likelihood of both honeydew and prey being located in the field.

Abstract

Chrysoperla carnea (Stephens) is an important egg predator of the olive moth, Prays oleae (Bernard). Its eggs are laid on pedicels, which are likely to provide some protection against predators and parasitoids. Ants figure prominently in the insect fauna of olive orchards. Six of the most common species in Granada were tested both in the laboratory and the field to establish their effect on chrysopid egg numbers. Crematogaster scutellaris Ol. and Tapinoma nigerrimum Nyl. were the most effective predators of chrysopid eggs with probably the latter being the most damaging due to both its large nest size and its activity. Plagiolepis pygmaea (Latr.), Camponotus micans Nyl. and Camponotus foreli Em. proved to be the least efficient predators. Formica subrufa Roger was intermediate in terms of its ability to eat lacewing eggs but a small sample prevents any firm conclusions from being drawn. [ABSTRACT FROM AUTHOR]

Abstract

Planned experiments in the Joint European Torus [Plasma Physics and Controlled Fusion Research, Proceedings, 13th International Conference, Washington, D.C., 1990 (International Atomic Energy Agency, Vienna, 1991), Vol. 1, p. 27] (JET) with deuterium-tritium (D-T) plasmas require high fusion performance for alpha-particle heating studies and for investigation of isotope dependence in conditions relevant to the International Thermonuclear Experimental Reactor [Plasma Phys. Controlled Fusion 37, A19 (1995)]. In deuterium plasmas, the highest neutron rates have been obtained in the hot-ion high-confinement mode (H mode) which is ultimately limited by magnetohydrodynamic (MHD) phenomena when the pressure gradient approaches ideal ballooning and kink stability limits in the vicinity of the edge transport barrier. Results are reported confirming the MkII divertor's increased closure and pumping in this regime, progress in understanding the MHD-related termination is discussed, and the use of ion cyclotron resonance heating (ICRH) in combination with high-power neutral beams to increase the neutron yield is described. In separate experiments internal transport barriers have been established through careful programming of the current ramp and heating waveforms, and neutron emission comparable with the best hot-ion II-modes achieved. Steady-state II-mode discharges exhibiting edge localized modes (ELMs) in reactor-like configurations and conditions have been demonstrated, including cases in which relevant dimensionless parameter values are preserved, ready also for testing in D-T. Abstract

The JET experimental campaign has focused on studies in support of the ITER physics basis. An overview of the results obtained is given for the reference ELMy H mode and advanced scenarios, which in JET are based on internal transport barriers. JET studies for ELMy H mode have been instrumental in the definition of ITER FEAT. Positive elongation and current scaling in the ITER scaling law have been confirmed, but the observed density scaling fits a two term (core and edge) model better. Significant progress in neoclassical tearing mode limits has been made showing that ITER operation with q(95) around 3.3 seems to be optimized. Effective helium pumping and divertor enrichment is found to be well within ITER requirements. Target asymmetries and hydrogen isotope retention are well simulated by modelling codes taking into account drift flows in the scrape-off plasmas. Striking improvements in fuelling effectiveness have been made with the new high field pellet launch facility. Good progress has been made on scenarios for achieving good confinement at high densities, both with radiation improved modes and with high field side pellets. Significant development of advanced scenarios, in view of their application to ITER, has been achieved. Progress towards integrated advanced scenarios is well developed with edge pressure control (impurity radiation). An access domain has been explored showing, in particular, that the power threshold increases with magnetic field but can be significantly reduced when lower hybrid current drive is used to produce target plasmas with negative shear. The role of ion pressure peaking on MHD has been well documented. Lack of sufficient additional heating power and interaction with the septum at high beta prevents assessment of the beta limits (steady plasmas achieved with beta (N) up to 2.6). Plasmas with a non-inductive current (I-NI/I-p = 60%), well aligned with the plasma current, high beta and good confinement have also been obtained.

This presentation by Natasha Kidd (Course Leader of the BA Fine Art course at Bath School of Art) and Jo Addison (Course Leader of the BA Fine Art course at Kingston University) offered an over view of 'No Working Title', a collaborative project between BA students and staff from 4 UK Art Schools that culminates in an event at Tate Modern. \ud \ud Since the 1960’s artists have been inventing or adopting rules, systems and strategies as a way of generating artworks. Often removing the execution of the artwork from the hand of the artist himself artists such as Sol Lewitt, Alghiero Boetti, Vito Acconci, Douglas Huebler, John Baldessari and Fluxus have not only challenged the status of the art object but questioned the role of the artist as author. In 2009, following this framework 'No Working Title' (NWT) was devised. This project sets out to provoke a dialogue between students and staff across 4 HE institutions, establish a collaborative relationship between the students and staff from these institutions and an international gallery. Through this relationship an authorial approach to practice is exchanged for an anti-authorial counterpart. Labour and skills are both relinquished and acquired and there is an opportunity to investigate what the points of interchange offer to all involved\ud \ud In order to promote cross-institutional exchange, each student or member of staff is paired with another from a different college. Each person devises a set of instructions, with their own practice at the centre of the process. This enables their partner to generate an artwork. The instructions take many forms and are communicated by various means, including email, text, post and social networking. At this stage they do not meet face-to-face. Communication is limited to the issuing of instructions only. \ud \ud On receiving their instructions, each person carefully follows the specifics of their partner’s request to make the work. Upon completion all works are packaged for transport to Tate Modern where partners and works meet for the first time. A critique / debate surrounding this ‘pop-up’ exhibition is chaired by a curator. \ud \ud For students and staff alike, it is a challenging process. It questions important issues of provenance, control, labour, duty and ego. It encourages dialogue about art in which ideas are primary and are communicated and interpreted through verbal and non-verbal language. It questions the artist’s role as author and suggests that rather than de-skilling the artist/student through the negation of the act of production, the process re-skills the artist/student with more astute conceptual rigor and an evolved visual and verbal language. \ud \ud Misunderstanding, misinterpretation and unfulfilled promise are central to the project. Failure and disappointment are integral components of the process. Ambiguity is questioned. Egos are checked. The language of the instructions given is scrutinized. \ud \ud This paper contextualised the project; explored the prevalent themes and presented images of the work produced. It looked at ways in which the questions raised through the project and themes explored have more broad pedagogic application.

Abstract

JET was extensively modified in the 1992/93 shutdown. The new pumped divertor and many new systems were brought into operation early in 1994. Operations have progressed to 4MA plasma current and, with substantial additional heating, H-mode confinement results confirm the expected scaling. The high power handling capability of the pumped divertor with sweeping is estimated at 20MW for 20s. H-mode plasmas have large Type I ELMs. With lower hybrid heating alone, 2MA full current drive has been achieved with good efficiency, with ICRF power, effective heating and direct electron heating have been demonstrated.

Abstract

All major JET systems have been fully commissioned for D-T and the DTE1 series of experiments has started with the D-T fuel mixture and operating conditions foreseen for ITER. In the area of ITER physics, significant results have been produced in both D-D and D-T. In D-D, the LH threshold power database has been extended, the bounds on edge-electron temperature and density in ELMy H-modes have been defined and the advantages of Types I and III ELMy discharges have been compared. In D-T plasmas, the isotope effect on H-mode threshold power and transport has been determined so that a more accurate assessment can be made of the ignition margin and heating requirements for ITER. Trace tritium experiments have provided first particle transport measurements and an assessment of the ITER reference ion-cyclotron resonance-frequency heating scenarios has been started, In the area of fusion performance, record D-D neutron yields have been obtained by controlling the plasma and current profiles in hot ion ELM-free H-modes and optimized shear modes. In D-T, internal transport barriers have been readily established in optimized shear discharges and Alfven eigenmodes have been observed.

Abstract

Abstract Background Chemokine and chemokine receptors play an essential role in tumorigenesis. Although chemokine-associated single nucleotide polymorphisms (SNPs) are associated with various cancers, their impact on prostate cancer (PCA) among men of African descent is unknown. Consequently, this study evaluated 43 chemokine-associated SNPs in relation to PCA risk. We hypothesized inheritance of variant chemokine-associated alleles may lead to alterations in PCA susceptibility, presumably due to variations in antitumor immune responses. Methods Sequence variants were evaluated in germ-line DNA samples from 814 African-American and Jamaican men (279 PCA cases and 535 controls) using Illumina’s Goldengate genotyping system. Results Inheritance of CCL5 rs2107538 (AA, GA+AA) and rs3817655 (AA, AG, AG+AA) genotypes were linked with a 34-48% reduction in PCA risk. Additionally, the recessive and dominant models for CCR5 rs1799988 and CCR7 rs3136685 were associated with a 1.52-1.73 fold increase in PCA risk. Upon stratification, only CCL5 rs3817655 and CCR7 rs3136685 remained significant for the Jamaican and U.S. subgroups, respectively. Conclusions In summary, CCL5 (rs2107538, rs3817655) and CCR5 (rs1799988) sequence variants significantly modified PCA susceptibility among men of African descent, even after adjusting for age and multiple comparisons. Our findings are only suggestive and require further evaluation and validation in relation to prostate cancer risk and ultimately disease progression, biochemical/disease recurrence and mortality in larger high-risk subgroups. Such efforts will help to identify genetic markers capable of explaining disproportionately high prostate cancer incidence, mortality, and morbidity rates among men of African descent.
