While NBMA-associated anaphylaxis is commonly peri-intubation and sugammadex reactions are typically peri-extubation, patients can also be allergic to the sugammadex-rocuronium complex, further complicating attempts to distinguish between the two for future avoidance. B Introduction: b Anaphylaxis in the perioperative and operative period is a potentially life-threatening reaction to medications or substances, most commonly neuromuscular blocking agents (NBMAs), antibiotics, disinfectants, and latex. The most frequently used NBMAs are succinylcholine and rocuronium, but sugammadex - typically used for rocuronium reversal and controversially touted as a possible treatment for rocuronium-induced anaphylaxis - has been increasingly reported in the literature as an anaphylaxis trigger itself. Abstract

Objective: Therapeutic alliance has been associated with better treatment engagement, better adherence, and less dropout across various treatments and disorders. In treatment of posttraumatic stress disorder (PTSD), it may be particularly important to establish a strong early alliance to facilitate treatment adherence. However, factors such as childhood sexual abuse (CSA) history and poor social support may impede the development of early alliance in those receiving PTSD treatment. We sought to examine treatment adherence, CSA history, and social support as factors associated with early alliance in individuals with chronic PTSD who were receiving either prolonged exposure therapy (PE) or sertraline. Method: At pretreatment, participants (76.6% female; 64.9% Caucasian; mean age 37.1 years, SD = 11.3) completed measures of trauma history, general support (Inventory of Socially Supportive Behaviors), and trauma-related social support (Social Reactions Questionnaire). Over the course of 10 weeks of FE or sertraline, they completed early therapeutic alliance (Working Alliance Inventory) and treatment adherence measures. Results: Early alliance was associated with PE adherence (r = .32, p < .05) and overall treatment completion (r = .19, p < .05). Only trauma-related social support predicted the strength of early alliance beyond the effects of treatment condition (~3 = .23, p < .05); CSA history was not predictive of a lower early alliance. Conclusions: Given the associations with adherence, clinicians may find it useful to routinely assess alliance early in treatment. Positive trauma support, not CSA history, may be particularly important in the development of a strong early therapeutic alliance. [ABSTRACT FROM AUTHOR]

66. HOMEWORK ADHERENCE IN PROLONGED EXPOSURE FOR CHRONICPOSTTRAUMATIC STRESS DISORDER

Abstract

Overview: Across psychological disorders, homework adherence is related to improvedcognitive behavioral therapy outcome (e.g., Beutler et al., 2004; Kazantzis et al., 2010).Patients with PTSD may have difficulty engaging in between-session activities, giventhat avoidance of trauma-reminders is a core symptom of the disorder (Scott & Stradling,1997). Despite homework assignments being a primary component of ProlongedExposure (PE), little is known regarding the role of adherence in treatment outcome.Method: 84 individuals (76.6% female; 53.6% Caucasian) with a primary diagnosis ofPTSD received up to 10 weekly sessions of PE. Self-reported PTSD symptoms (PSS-SR)and ratings of imaginal and in vivo homework completion (Usefulness of TechniquesInventory) were assessed at each session. Patient beliefs were assessed at pre-treatmentvia self-report (Dysfunctional Attitudes Scale) and in-session via a PE-adapted observerrated coding system (CHANGE; Hayes et al., 2006). Results: Time-lagged mixedregression models indicated that higher levels of in vivo homework adherence and higherperceptions of helpfulness of in vivo and imaginal homework assignments precededreductions in PTSD symptoms. Higher levels of imaginal homework adherence did notprecede reductions in PTSD symptoms. Hierarchical modeling suggested that patientswith co-occurring major depression (MDD) reported lower levels of in vivo homeworkadherence during PE and rated their in vivo homework as less helpful than those without8MDD. No differences were found in levels or patterns of imaginal homework helpfulnessbetween those with and without MDD. Finally, in-session positive beliefs predictedhigher levels of in vivo homework adherence during PE. Conclusion: Results supporthomework activities as a critical component of the PE protocol. 67. Depression Symptom Discontinuities over the Course of Treatment for Chronic Posttraumatic Stress Disorder

Abstract

Changes in symptoms during depression treatment occur discontinuously and such discontinuities predict better outcome (Hayes et al., 2007a). While depression commonly co-occurs with PTSD, we know little about how change unfolds in depression symptoms during PTSD treatment. This study examined transitions in depression symptoms during PTSD treatment, namely sudden gains and depression spikes. Social support was examined as a predictor of these symptom fluctuations. At pre-treatment, 200 participants with a primary diagnosis of PTSD completed measures of PTSD and depression severity, and social support. During 10 weeks of prolonged exposure or sertraline, depression was assessed. Overall, 18% of participants experienced a depression sudden gain and 22.5% experienced a transient depression spike. Higher negative trauma-related support was associated with experiencing a depression spike. Similar rates of depression discontinuities occurred between treatments. Sudden gains predicted better treatment outcome. Clinicians should be encouraged that transient depression worsenings were not associated with poorer outcome.
