Abstract

Prejudice and discriminatory behaviour remain significant barriers to positive intergroup relations. In a time in which global migration rates are at their highest (and continue to rise), recent emphasis has been placed on the negative perceptions of certain immigrant and refugee group members in Western media and political discourse. A prominent feature of these debates involves the perceived “humanness” of migrant individuals. The tendency to view culturally dissimilar others as less human compared to one’s own cultural group is not a novel social phenomenon however. In this chapter, we draw from the fields ofphilosophy, social and developmental psychology to examine the nature of dehumanisation in the context of migration. Through understanding the psychological processes by which dehumanising perceptions are acquired and expressed, this research ultimately hopes to inform strategies to foster the inclusion of newcomers within their host countries.

72. Early preantral follicles of the domestic cat express gonadotropin and sex steroid signaling potential

Abstract

Key biomolecular processes, which regulate primordial ovarian follicle dormancy and early folliculogenesis in mammalian ovaries, are not fully understood. The domestic cat is a useful model to study ovarian folliculogenesis and is the most relevant for developing in vitro growth methods to be implemented in wild felid conservation breeding programs. Previously, RNA-sequencing of primordial (PrF), primary (PF), and secondary follicle (SF) samples from domestic cat implicated ovarian steroidogenesis and steroid reception during follicle development. Here, we aimed to identify which sex steroid biosynthesis and metabolism enzymes, gonadotropin receptors, and sex steroid receptors are present and may be potential regulators. Differential gene expression, functional annotation, and enrichment analyses were employed and protein localization was studied too. Gene transcripts for PGR, PGRMC1, AR(steroid receptors), CYP11A1, CYP17A1, HSD17B1and HSD17B17(steroidogenic enzymes), and STS(steroid metabolizing enzyme) were significantly differentially expressed (Qvalues of ≤0.05). Differential gene expression increased in all transcripts during follicle transitions apart from ARwhich decreased by the secondary stage. Immunohistochemistry localized FSHR and LHCGR to oocytes at each stage. PGRMC1 immunostaining was strongest in granulosa cells, whereas AR was strongest in oocytes throughout each stage. Protein signals for steroidogenic enzymes were only detectable in SFs. Products of these significantly differentially expressed genes may regulate domestic cat preantral folliculogenesis. In vitro growth could be optimized as all early follicles express gonadotropin and steroid receptors meaning hormone interaction and response may be possible. Protein expression analyses of early SFs supported its potential for producing sex steroids.Primordial, primary, and secondary ovarian follicles of the domestic cat express gonadotropin and sex steroid receptors with secondary follicles presenting a steroidogenic potential too.

Abstract

Background Ovarian cancer is a leading cause of female gynecological cancer-related death, and there are no effective screening procedures or early diagnostic approaches. Aims To examine risk factors and risk-reducing strategies for both sporadic and familial tumors. Materials and methods Literature review and consensus of expert opinion. Results and conclusions In women with a genetic predisposition to ovarian cancer, salpingo-oophorectomy reduces the risk of ovarian malignancy, and to a lesser degree of breast cancer. Opportunistic bilateral salpingo-oophorectomy and bilateral salpingectomy may also prevent epithelial ovarian cancer. In premenopausal women, bilateral salpingectomy should be preferred to tubal ligation, and be performed when hysterectomy is carried out for benign uterine disease. Hysterectomy and the use of combined oral contraceptives and non-steroid anti-inflammatory drugs are also recognized to reduce the risk of ovarian cancer, as do the prevention of obesity and smoking cessation. © 2017 Elsevier B.V.

INTRODUCTION: Ovarian cancer (OC) is associated with non-specific symptoms such as bloating, making accurate diagnosis challenging: only 1 in 3 women with OC presents through primary care referral. National Institute for Health and Care Excellence guidelines recommends sequential testing with CA125 and routine ultrasound in primary care. However, these diagnostic tests have limited sensitivity or specificity. Improving accurate triage in women with vague symptoms is likely to improve mortality by streamlining referral and care pathways. The Refining Ovarian Cancer Test Accuracy Scores (ROCkeTS; HTA 13/13/01) project will derive and validate new tests/risk prediction models that estimate the probability of having OC in women with symptoms. This protocol refers to the prospective study only (phase III). METHODS AND ANALYSIS: ROCkeTS comprises four parallel phases. The full ROCkeTS protocol can be found at http://www.birmingham.ac.uk/ROCKETS. Phase III is a prospective test accuracy study. The study will recruit 2450 patients from 15 UK sites. Recruited patients complete symptom and anxiety questionnaires, donate a serum sample and undergo ultrasound scored as per International Ovarian Tumour Analysis (IOTA) criteria. Recruitment is at rapid access clinics, emergency departments and elective clinics. Models to be evaluated include those based on ultrasound derived by the IOTA group and novel models derived from analysis of existing data sets. Estimates of sensitivity, specificity, c-statistic (area under receiver operating curve), positive predictive value and negative predictive value of diagnostic tests are evaluated and a calibration plot for models will be presented. ROCkeTS has received ethical approval from the NHS West Midlands REC (14/WM/1241) and is registered on the controlled trials website (ISRCTN17160843) and the National Institute of Health Research Cancer and Reproductive Health portfolios.

Abstract

Trinidad, a predominantly Catholic and Francophone island, became a British colony in 1802. The lynchpin of the Catholic Church's authority and influence in the region, Trinidad's religious culture was defined by powerful racial tensions. Francis de Ridder, a Roman Catholic priest, was born in 1800 to a slave mother and a Belgian-born planter father. As a priest and free person of colour in Trinidad between 1825 and the early 1830s, his experience reveals much about the complex, yet collaborative relationship that existed between the Catholic Church and Britain's colonial office. This relationship, which expanded significantly between 1820 and the early 1840s, was informed by demographic change and the reorganization of ecclesiastical structures. The de Ridder case is used here as a springboard for exploring some of the ways that slaves and free people of colour were excluded from religious practice and Church authority structures. In working closely with colonial officials on the ground in Trinidad, these Catholic leaders, who were often bishops or their coadjutors, embedded the Church within the governing structures of elite white society. What emerges is a picture of a Church whose missionary leadership actively pushed non-whites to the periphery of the story of its global spread. [ABSTRACT FROM AUTHOR]

Abstract

Objective: To determine which descriptors of cytoreductive surgical extent in advanced ovarian cancer (AOC) best predict postoperative morbidity.Design: Retrospective notes review.Setting: A gynaecological cancer centre in the UK.Population: Six hundred and eight women operated on for AOC over a period of 114 months at a tertiary cancer centre, between 16 August 2007 and 16 February 2017.Methods: Outcome data were analysed by six approaches to classify the extent of surgery: standard/ultra-radical surgery; standard/radical/supra-radical surgery; presence/absence of gastrointestinal resections; low/intermediate/high surgical complexity score (SCS); presence of bowel anastomoses and/or diaphragmatic surgery; and the presence/absence of multiple bowel resections.Main Outcome Measures: Major (grades 3-5) postoperative morbidity and mortality.Results: Forty-three (7.1%) patients experienced major complications. Grade-5 complications occurred in six patients (1.0%). Patients who underwent multiple bowel resections had a relative risk (RR) of 7.73 (95% confidence interval, 95% CI 3.92-15.26), patients with a high SCS had an RR of 6.12 (95% CI 3.25-11.52), patients with diaphragmatic surgery and gastrointestinal anastomosis had an RR of 5.57 (95% CI 2.65-11.72), patients with 'any gastrointestinal resection' had an RR of 4.69 (95% CI 2.66-8.24), patients with ultra-radical surgery had an RR of 4.65 (95% CI 2.26-8.79), and patients with supra-radical surgery had an RR of 4.20 (95% CI 2.35-7.51) of grades 3-5 morbidity, compared with patients undergoing standard surgery as defined by the National Institute for Health and Care Excellence (NICE) in the UK. No significant difference was seen in the rate of major morbidity between standard (6/59, 10.2%) and ultra-radical (9/81, 11.1%) surgery within the cohort who had intermediate complex surgery (P > 0.05).Conclusions: The numbers of procedures performed significantly correlate with major morbidity. The number of procedures performed better predicted major postoperative morbidity than the performance of certain 'high risk' procedures. We recommend using SCS to define a higher risk operation. Abstract

Purpose of investigation: To determine the pattern of disease recurrence in patients treated for advanced ovarian cancer (AOC). Materials and Methods:A retrospective review of the locations of recurrence in all patients treated for AOC between August 16, 2007 and February 3, 2014 who underwent cytoreductive surgery and platinum based chemotherapy achieving complete (R0) or optimal (< 1cm) (R1) cytoreduction. Results: The study identified 254 patients. Peritoneal site relapses affected 65.4-91.1% of patients and were more common in R1 compared to R0 patients (p = 0.01891). Ascites at relapse was less prevalent in R0 compared to R1 patients (p = 0.00185) and occurred less with more complex surgery (p = 0.00497) and 19.2% of patients who required high complexity surgery to achieve R0 only experienced retroperitoneal recurrences. Conclusion: Irrespective of the surgery undertaken, the peritoneum is the commonest site of recurrence. The surgical complexity required to achieve R0 seemingly alters the relapse pattern by reducing ascites and peritoneal relapse compared to those with R1 disease, and increasing retroperitoneal relapse.
