Abstract Background Mounting evidence suggests that the blood-brain barrier (BBB) plays an important role in the regulation of brain iron homeostasis in normal brain development, but these imaging profiles remain to be elucidated. We aimed to establish a relationship between brain iron dynamics and BBB function during childhood using a combined quantitative magnetic resonance imaging (MRI) to depict both physiological systems along developmental trajectories. Methods In this single-center prospective study, consecutive outpatients, 2–180 months of age, who underwent brain MRI (3.0-T scanner; Ingenia; Philips) between January 2020 and January 2021, were included. Children with histories of preterm birth or birth defects, abnormalities on MRI, and diagnoses that included neurological diseases during follow-up examinations through December 2022 were excluded. In addition to clinical MRI, quantitative susceptibility mapping (QSM; iron deposition measure) and diffusion-prepared pseudo-continuous arterial spin labeling (DP-pCASL; BBB function measure) were acquired. Atlas-based analyses for QSM and DP-pCASL were performed to investigate developmental trajectories of regional brain iron deposition and BBB function and their relationships. Results A total of 78 children (mean age, 73.8 months ± 61.5 [SD]; 43 boys) were evaluated. Rapid magnetic susceptibility progression in the brain (Δsusceptibility value) was observed during the first two years (globus pallidus, 1.26 ± 0.18 [× 10− 3 ppm/month]; substantia nigra, 0.68 ± 0.16; thalamus, 0.15 ± 0.04). The scattergram between the Δsusceptibility value and the water exchange rate across the BBB (k w ) divided by the cerebral blood flow was well fitted to the sigmoidal curve model, whose inflection point differed among each deep gray-matter nucleus (globus pallidus, 2.96–3.03 [mL/100 g]−1; substantia nigra, 3.12–3.15; thalamus, 3.64–3.67) in accordance with the regional heterogeneity of brain iron accumulation. Conclusions The combined quantitative MRI study of QSM and DP-pCASL for pediatric brains demonstrated the relationship between brain iron dynamics and BBB function during childhood. In recent years, environmental concerns have led to a demand for sustainable materials, and natural rubber (NR) reinforced with cellulose nanofibers (CNFs) has been attracting attention as one such material. When CNFs are added to NR, the elastic modulus is greatly improved. This is considered to be caused by deformation constraint of NR by reinforcing CNFs. On the other hand, it is known that there exists a region around reinforcing fibers where the rubber is tightly connected to the surface of fibers, which is called bound rubber. The volume fraction of the bound rubber is usually measured by swelling tests using organic solvent. In this paper, we focused on the relationship between the tensile properties of NR reinforced with CNFs and the volume fraction of bound rubber. Firstly, cyclic tensile tests were performed using plate specimens of CNF-reinforced NR and the effect of reinforcing CNFs on the tensile properties of CNF-reinforced NR and their change with loading cycles were investigated. Then, the swelling tests were conducted for similar specimens and the volume fraction of the bound rubber was evaluated. It was found from a series of experimental results that tensile deformation resistance of NR increased largely by adding 5 phr (4.8 mass%, 3.2 vol.%) of CNFs but that this increase decreased from the second loading cycle. Moreover, a clear relationship was found between the tensile deformation resistance of CNF-reinforced NR and the volume fraction of bound rubber. These results suggest that the tensile deformation resistance of CNF-reinforced NR can be evaluated by swelling tests.

Objective To investigate the relationship between oral frailty (OF), nutrient intake and calf circumference (CC) in middle-aged and older adults.Design Cross-sectional study.Setting Residents of four model districts of Shika town, Ishikawa Prefecture, Japan, using data from November 2017 to February 2018.Participants One hundred and ninety-four residents aged ≥50 years in four model districts of Shika town. The OF total score ≥3 was defined as OF. Participants were divided into OF and non-OF groups and divided into the low-CC/kg and the high-CC/kg groups.Outcome measures The primary outcome is to use a two-way analysis of covariance to analyse the interaction between the two CC/kg groups and the two OF groups on nutrition intake. The secondary outcome is to use multiple regression analysis to investigate the nutrients significantly related to CC/kg when stratified by OF, with age, sex, body mass index, drinking status, smoking status and regular exercise as input covariates.Results A two-way analysis of covariance revealed a significant interaction between the two CC/kg groups and the two OF groups on animal protein intake (p=0.039). Multiple comparisons using the Bonferroni analysis revealed a significantly lower animal protein intake in the OF group than in the non-OF group with a low CC/kg (p=0.033) but not in the group with a high CC/kg. The multiple regression analysis stratified by OF revealed a positive correlation between animal protein intake and CC/kg (p=0.002).Conclusions The present results revealed a significantly lower animal protein intake in the OF group than in the non-OF group in the low-CC/kg group, but no such difference was observed in the high-CC/kg group. Further longitudinal studies are needed to elucidate this relationship.

Although the relationship between dyslipidaemia (DL) and coronary artery disease (CAD) or between trace minerals intake and CAD is well known separately, the exact nature of this relationship remains unknown. We hypothesize that the relationship between trace mineral intake and CAD may differ depending on whether or not the individual has DL. The present study analysed the relationships among trace mineral intake, DL, and CAD in middle-aged and older adults living in Shika town, Ishikawa prefecture, Japan. This study included 895 residents following the exclusion of those with genetic risk carriers for familial hypercholesterolemia. Trace mineral intake was evaluated using the brief-type self-administered diet history questionnaire. Interactions were observed between DL and CAD with zinc (p = 0.004), copper (p = 0.010), and manganese intake (p < 0.001) in a two-way analysis of covariance adjusted for covariates such as sex, age, body mass index, and current smokers and drinkers. Multiple logistic regression analysis showed that zinc (odds ratio (OR): 0.752; 95% confidence interval (CI): 0.606, 0.934; p = 0.010), copper (OR: 0.175; 95% CI: 0.042, 0.726; p = 0.016), and manganese (OR: 0.494; 95% CI: 0.291, 0.839; p = 0.009) were significant independent variables for CAD in the dyslipidaemic group. The present results suggest that DL with a low trace mineral intake is associated with CAD. Further longitudinal studies are required to confirm this relationship.

The nucleic acids found in food play a crucial role in maintaining various bodily functions. This study investigated the potential anticancer effects of dietary nucleic acids, an area that is still not fully understood. By utilizing an in vivo mouse model and an in vitro cell model, we discovered an anti-proliferative impact of RNA in both systems. DNA exhibited anti-proliferative effects in the mouse model, while this phenomenon wasn't observed in the in vitro cell model using Ehrlich ascites tumor (EAT) cells. Conversely, DNA hydrolysate demonstrated distinct anti-proliferative effects in EAT cells, suggesting that nucleotides or nucleosides generated during nucleic acid digestion act as active constituents. Furthermore, we examined various nucleosides and two sodium-independent equilibrative nucleoside transporter inhibitors (ENTs), identifying guanosine and 2'-deoxyguanosine as pivotal in the anti-proliferative effect. We also found that the anti-proliferation activity with both nucleosides was suppressed by the treatment of dipyridamole, a non-selective inhibitor for ENT1 and ENT2, but not nitrobenzylthioinosine, a low inhibitor for ENT2. The uptake of these compounds into cells is likely facilitated by ENT2. These nucleotides impeded the progression of cancer cells from the G1 phase to the S phase in the cell cycle. Another significant finding is the increased expression of CCAAT/enhancer-binding protein (C/EBPβ) induced by guanosine and 2'-deoxyguanosine. Furthermore, immunostaining revealed that C/EBPβ diffuses into the nucleus, indicating its presence. This suggests that guanosine or 2-deoxyguanosine induces G1 arrest in cancer cells via the activation of C/EBPβ. Encouraged by these promising results, guanosine and 2'-deoxyguanosine show potential applications in cancer prevention.

Airway-centered fibroelastosis is characterized by peribronchovascular fibroelastosis, predominantly in the upper lobes, with little-to-no pleural involvement. In this study, we describe two cases of airway-centered fibroelastosis diagnosed based on radiological and pathological findings. The first case comprised a 44-year-old man whose forced vital capacity improved over three months following treatment with nintedanib. The second case involved a 50-year-old woman who was treated with oral corticosteroids but yielded an unfavorable outcome. An effective treatment for airway-centered fibroelastosis has not yet been identified; therefore, this study may help contribute to a more thorough discussion regarding treatment strategies for this disease.

Key Clinical Message Cognitive impairment associated dementia is treatable non‐pharmacologically. Monitoring tools are important to provide proper treatment. The present study showed that the resting‐state brain activity measured using magnetoencephalography reflects their outcomes and captures clinical impressions better than neuropsychological assessments, which have inherent limitations such as the practice effect. Abstract Mild cognitive impairment (MCI) is a prodromal phase of dementia caused by brain diseases. Non‐pharmacological treatments are sometimes effective in improving patient's cognition and quality of life. To provide better treatments, monitoring the treatment outcomes, which is done using neuropsychological assessments, is important. However, these assessments have inherent limitations, such as practice effects. Therefore, complementary assessments are anticipated. Magnetoencephalography (MEG) is a neuroimaging technique that is sensitive to changes in brain activity associated with cognitive impairment. It represents the state of brain activity in terms of MEG spectral parameters associated with neuropsychological assessment scores. MEG spectral parameters could reasonably be used to monitor treatment outcomes without the aforementioned limitations. However, few published longitudinal reports have assessed MEG spectral parameters during the non‐pharmacological treatment period for cognitive impairment associated with dementia. In this study, we retrospectively examined the clinical records of two patients with MCI. Changes in neuropsychological assessment scores and MEG spectral parameters were qualitatively evaluated along with the patients' conditions, as described in the medical records during non‐pharmacological treatments provided for more than 2 years. The changes in neuropsychological assessment scores and MEG spectral parameters showed comparable trends, with some discrepancies. Changes in MEG spectral parameters were more consistent with the subjective reports from caregivers and medical staff in the medical records. Our results suggest that MEG is a promising tool for monitoring patient conditions during treatment.

Abstract Background Patient motivation is an important determinant of rehabilitation outcomes. Differences in patients’ and clinicians’ perceptions of motivational factors can potentially hinder patient-centered care. Therefore, we aimed to compare patients’ and clinicians’ perceptions of the most important factors in motivating patients for rehabilitation. Methods This multicenter explanatory survey research was conducted from January to March 2022. In 13 hospitals with an intensive inpatient rehabilitation ward, 479 patients with neurological or orthopedic disorders undergoing inpatient rehabilitation and 401 clinicians, including physicians, physical therapists, occupational therapists, and speech-language-hearing therapists, were purposively selected using inclusion criteria. The participants were asked to choose the most important factor motivating patients for rehabilitation from a list of potential motivational factors. Results Here we show that realization of recovery, goal setting, and practice related to the patient’s experience and lifestyle are the three factors most frequently selected as most important by patients and clinicians. Abstract Quinacridone, a π-conjugated planar molecule, and common red pigment in industrial and painting applications, easily aggregates to form large clusters of pigment particles, resulting in a reduction in color strength. Cotton-derived cellulose nanofiber (NF), which almost consists of cellulose without hemicellulose and lignin, has been found to adsorb quinacridone on the surface, which inhibits pigment aggregation. The aggregation inhibition property of cellulose NF was induced by the strong intermolecular interactions between cellulose and quinacridone. In this study, the properties of lignocellulosic fibers for suppressing the aggregation of quinacridone pigments were investigated to reveal the influence of hemicellulose and lignin on the intermolecular interactions between quinacridone and fibers. Two lignocellulosic fibers with different degrees of fibrillation were used as dispersants of the pigment. In the scanning electron microscopy (SEM) images of the quinacridone–lignocellulose mixture, quinacridone particles were observed along the lignocellulose fiber, indicating that the quinacridone particles were well-adsorbed on the fiber surface. Consequently, the color of the aqueous suspension of quinacridone–lignocellulose mixture became increasingly vivid as the weight ratio of the lignocellulose fibers increased and as the fiber was fibrillated. The nuclear Overhauser effect spectroscopy (NOESY)–nuclear magnetic resonance (NMR) spectrum for quinacridone–lignocellulose suspension in d-dimethyl sulfoxide showed several NOE cross-peaks between quinacridone and cellulose/hemicellulose, whereas no cross-peaks between quinacridone and lignin were observed. It can be concluded that cellulose and hemicellulose promote the adsorption of quinacridone on the fiber surface, whereas lignin does not interact with quinacridone, even though both are aromatic molecules. This suggests that the intermolecular interactions based on hydrogen bonding and CH–π attraction are more dominant than the π–π attraction between quinacridone and lignocellulosic fibers.

Abstract Background In previously reported cases of lesser omental hernia, a rare clinical presentation, the herniated intestinal tract was passing through both peritoneal layers of the lesser omentum to herniate into the peritoneal cavity or bursa omentalis. Here we present a very rare case of lesser omentum hernia, where the transverse colon entered through only the posterior layer of the lesser omentum to form a hernia between the anterior and posterior layers. Case presentation A 43-year-old man was admitted to the emergency department with acute abdominal pain. Plain abdominal computed tomography (CT) revealed a change in the caliber of the transverse colon between the stomach and pancreas, forming a closed loop on the cephaloventral side of the stomach. On contrast-enhanced CT images, vessels were observed in the contrast-enhanced lesser omentum surrounding the herniated intestine. The patient was diagnosed with a lesser omental hernia and underwent laparoscopic surgery. Intraoperatively, the transverse colon was covered by the anterior layer of the lesser omentum, and a defect was found in the posterior layer of the lesser omentum on the dorsal side of the stomach. A 2-cm incision was made in the posterior layer of the lesser omentum to widen the small defect. The herniated intestinal section was removed from the hernia sac, and the transverse colon was retained unresected. The postoperative course was uneventful. Conclusions As illustrated in this first case of a lesser omental hernia forming between the anterior and posterior layers, characteristic CT findings may play an active role in the diagnosis of this rare presentation.

Abstract Although suppression of sympathetic activity is suggested as one of the underlying mechanisms for the cardioprotective effects afforded by sodium–glucose cotransporter 2 (SGLT2) inhibitors, whether the modulation of glucose handling acutely affects sympathetic regulation of arterial pressure remains to be elucidated. In Goto–Kakizaki diabetic rats, we estimated the open-loop static characteristics of the carotid sinus baroreflex together with urine glucose excretion using repeated 11-min step input sequences. After the completion of the 2nd sequence, an SGLT2 inhibitor empagliflozin (10 mg kg−1) or vehicle solution was administered intravenously (n = 7 rats each). Empagliflozin did not significantly affect the baroreflex neural or peripheral arc, despite significantly increasing urine glucose excretion (from 0.365 ± 0.216 to 8.514 ± 0.864 mg·min−1·kg−1, P

The mouse Harderian gland (HG) is a secretory gland that covers the posterior portion of the eyeball, opening at the base of the nictitating membrane. The HG serves to protect the eye surface from infection with its secretions. Mice open their eyelids at about 2 weeks of age, and the development of the HG primordium mechanically opens the eye by pushing the eyeball from its rear. Therefore, when HG formation is disturbed, the eye exhibits enophthalmos (the slit-eye phenotype), and a line of Fgf10+/− heterozygous loss-of-function mice exhibits slit-eye due to the HG atrophy. However, it has not been clarified how and when HGs degenerate and atrophy in Fgf10+/− mice. In this study, we observed the HGs in embryonic (E13.5 to E19), postnatal (P0.5 to P18) and 74-week-old Fgf10+/− mice. We found that more than half of the Fgf10+/− mice had markedly degenerated HGs, often unilaterally. The degenerated HG tissue had a melanized appearance and was replaced by connective tissue, which was observed by P10. The development of HGs was delayed or disrupted in the similar proportion of Fgf10+/− embryos, as revealed via histology and the loss of HG-marker expression. In situ hybridization showed Fgf10 expression was observed in the Harderian mesenchyme in wild-type as well as in the HG-lacking heterozygote at E19. These results show that the Fgf10 haploinsufficiency causes delayed or defective HG development, often unilaterally from the unexpectedly early neonatal period.

Background: Glycoprotein non-metastatic melanoma protein B (GPNMB) is expressed in macrophages during recovery from acute liver injury (ALI) in carbon tetrachloride (CCl4)-induced liver injury model mice. In this retrospective study, we assessed whether GPNMB levels in the serum and injured liver correlate with liver injury severity and prognosis in patients with ALI or acute liver failure (ALF).Methods: The study involved 56 patients with ALI or ALF who visited the Kagoshima University Hospital. Serum GPNMB level was measured over time, and the localization, proportion, origin, and phenotype of GPNMB-expressing cells in the injured liver were assessed. Finally, the phenotypes of human monocyte-derived macrophages and peripheral blood mononuclear cells (PBMCs) of patients with ALI and ALF were analyzed.Results: Peak GPNMB levels were significantly higher in patients with ALF and hepatic encephalopathy (HE), as well as in those who underwent liver transplantation or died, than in others. The peak GPNMB level correlated with prothrombin activity, prothrombin time-international normalized ratio, Model for End-stage Liver Disease score, and serum hepatocyte growth factor level. GPNMB was expressed in CD68-positive macrophages, and its level increased with the severity of liver injury. The macrophages showed the same polarization as M2c macrophages induced with interleukin-10 from human monocytes. Moreover, PBMCs from patients with ALF exhibited an immunosuppressive phenotype.Conclusion: We found that GPNMB levels in the serum and injured liver, which increased in patients with ALF, especially in those with HE, correlated with the severity of liver injury and prognosis of ALI and ALF. Moreover, GPNMB-positive macrophages exhibited the M2c phenotype. Our results indicate that persistently high GPNMB levels may be a prognostic marker in patients with ALI and ALF.

Introduction Redo pyeloplasty can be difficult due to scar tissue or fibrosis. Ureteral reconstruction with a buccal mucosal graft is performed safely and successfully, but most reports of ureteral reconstruction using a buccal mucosal graft are of robot‐assisted surgery, with few reports of laparoscopic‐assisted surgery. A case of laparoscopic‐assisted redo pyeloplasty using a buccal mucosal graft is presented. Case presentation A 53‐year‐old woman was diagnosed with ureteropelvic junction obstruction, and a double‐J stent was placed to relieve backache. She visited our hospital 6 months after double‐J stent placement. Three months later, laparoscopic pyeloplasty was performed. At 2 months postoperatively, anatomic stenosis occurred. Holmium laser endoureterotomy and balloon dilation were performed; however, the anatomic stenosis recurred, and laparoscopic redo pyeloplasty with a buccal mucosal graft was performed. After redo pyeloplasty, obstruction was improved, and her symptoms disappeared. Conclusion This is the first case of using a buccal mucosal graft for laparoscopic pyeloplasty in Japan.

The purpose of this study was to clarify the characteristics of lower-extremity kinematics during the running of soccer players with chronic ankle instability (CAI) in comparison to those without CAI. Twenty-two male college soccer players participated in this study. Twelve players were assigned to the CAI group and ten players to the non-CAI group, and players were diagnosed according to the Cumberland Ankle Instability Tool. Kinematic data of the hip, knee, ankle, foot, and ground reaction force components during the stance phase of running were obtained using a three-dimensional motion analysis system. The results revealed that soccer players with CAI who landed with ankle inversion and other characteristic kinematics in their lower extremity during the stance phase of running were similar to those without CAI. These results show that running kinematics in soccer players are not affected by the presence or absence of CAI. Future studies based on the results of this study may contribute to the analysis of the risk of developing CAI during soccer and may also help prevent lateral ankle sprains.

Abstract Hepatoblastoma (HB) is the most common malignant liver tumor in children. Although the development of treatment strategies with advances in chemotherapy has greatly improved the prognosis of HB, surgical resection and liver transplantation still play a vital role in the treatment of HB. In recent years, technological innovations have led to the development of new surgical approaches for HB. In this review, we describe the latest research on the surgical management of HB, including new imaging technologies, minimally invasive approaches, and the application of associating liver partition portal vein ligation for staged hepatectomy. We also discuss the current role of liver transplantation, use of ante‐situm or ex‐situ liver resection with auto‐transplantation, and management of metastatic HB.

Abstract Introduction This study aimed to evaluate the correlation and agreement between end‐tidal CO2 (EtCO2) measured with the novel portable capnometer (CapnoEye®) and partial pressure of arterial carbon dioxide (PaCO2) levels in patients with respiratory diseases and to compare the efficacy of EtCO2 and PvCO2 in predicting PaCO2 levels. Methods We analyzed the correlation and the agreement between EtCO2 and PaCO2 and between PvCO2 and PaCO2 using Pearson's moment correlation coefficient in patients with type 1 and type 2 respiratory failure and both groups overall. Results A total of 100 samples were included that comprised 67 men (67%). The mean age of the subjects was 77 ± 13 years. Chronic obstructive pulmonary disease (COPD) (43%) was the most common disease. There was a high correlation between EtCO2 and PaCO2 (r = 0.88; p < 0.0001). Sixty‐six PvCO2 samples were obtained, and there was a high correlation between PvCO2 and PaCO2 (r = 0.81; p < 0.0001). Regarding type 2 respiratory failure, there was a high correlation between EtCO2 and PaCO2 (r = 0.81). The Bland–Altman analysis between PaCO2 and EtCO2 revealed a bias of 5.7 mmHg, with limits of agreement ranging from −5.1 mmHg to 16.5 mmHg. In contrast, the analysis between PaCO2 and PvCO2 revealed a bias of −6.8 mmHg, and the limits of agreement ranged from −22.13 mmHg to 8.53 mmHg. Conclusion EtCO2 measured by CapnoEye® was significantly correlated to PaCO2 levels in patients with respiratory diseases. Moreover, CapnoEye® may be more useful for predicting hypercapnia conditions in which respiratory diseases are compared with measure PvCO2.

Objectives: To compare the eye axial length (AL), equatorial horizontal diameter (HD), and equatorial vertical diameter (VD) of normal eyes using a novel wide-angle, arc-scanning, ultrasound diagnostic device for wide-angle B-mode echography. Methods: In this cross-sectional study, wide-angle B-mode echography and magnetic resonance imaging (MRI) were conducted on 22 normal eyes; the AL, HD, and VD were measured. AIM: To determine the frequency of the surgery wound infection, its favoring factors, causative germs and its antibiotic treatment of choice in the surgery "A" department of the University Hospital Point G. Method: In a retrospective descriptive study over a 6-month period in the surgery "A" department of the University Hospital Point G, we enrolled all patients who underwent surgery necessitating at least 48 hours of hospital admission during our study period and in conformity with the Atlanta CDC criteria. Surgery patients with less than 48 hours postoperative hospital stay and those admitted to the hospital without surgery were not included. After the surgery, the nature and location of the infection have been clarified. In this work, ethical considerations have been respected and there is no conflict of interest. Results: Two hundred and sixty-five (n=265) patient files were collected including 24 cases of surgery wound infection (a hospital frequency of 9%. The average age was 41.41 years (y.o) old with extremes of 7 y.o and 102 y.o. The sex ratio was 0.9 in favor of women. The commonly studied pathologies were digestive surgery in 52.8%, gynecological surgery in 24.5%, etc... Patients were seen in regular outpatient surgery visits in 75.8% and surgical emergencies in 24.2%. Of the patients urgently admitted, 26.5% presented an ISO; for those received in ordinary consultation it was 3.48%. Fifteen (15) cases were classified ASA III and two (2) ASA II. Based on the Altemeier classification, surgery was clean in 66.8% of our patients, contaminated clean in 12.4%, contaminated in 12.1%, and dirty in 8.7% (including half resulting in surgery wound infection). In total, 79% of patients whose surgeries lasted more than two (2) hours presented a surgery wound infection. Postoperative infection of patients was superficial in 58.3% and deep in 33.3%. In 8.3% of cases, it was a space infection. At the cytobacteriological examination of the pus from the operative site, Escherichia coli (E. coli) was the most common germ with 58.3%. E. coli was sensitive to amoxicillin-clavulanic acid in 57.14%. Surgically, a re-intervention was performed in 20.8% of cases, a secondary suture in 12.5%, and a single dressing in 66.7%. The average postoperative stay was 6.5 days with extremes of 2 days and 69 days. In 3 months postoperatively we recorded 4 cases of death. Conclusion: The surgery wound infection constitutes a major complication in a surgical environment starting with surgical act itself. Particular emphasis should be placed on prevention, which will reduce the risks of ISO occurrence. [ABSTRACT FROM AUTHOR]

Abstract Background Brain imaging studies have reported that the effect of repetitive transcranial magnetic stimulation (rTMS) is associated with the activities of the dorsolateral prefrontal cortex (DLPFC) and ventral medial prefrontal cortex (VMPFC). However, few studies have been conducted in Japanese patients. Aim We aimed to identify brain regions associated with depressive symptom changes by measuring regional cerebral blood flow (rCBF) in the DLPFC and VMPFC before and after the high‐frequency rTMS to the left DLPFC in Japanese patients with treatment‐resistant depression. Method Fourteen patients participated in the rTMS study and were assessed with the 17‐item Hamilton depression rating scale (HAM‐D17). Among them, 13 participants underwent magnetic resonance imaging scan of the brain using the arterial spin labeling method. The rCBF was calculated using the fine stereotactic region of interest template (FineSRT) program for automated analysis. We focused on eight regions reported in previous studies. Results Depression severity significantly decreased after 2 week (HAM‐D17:11.4 ± 2.8, P = 0.00027) and 4 week (HAM‐D17: 11.0 ± 3.7, P = 0.0023) of rTMS treatment. There was no significant change in rCBF at each region in the pre‐post design. However, there was a significantly negative correlation between baseline rCBF in the right DLPFC and the improvement in HAM‐D17 score (r = −0.559, P = 0.047). Conclusion We obtained supportive evidence for the effectiveness of rTMS to the prefrontal cortex in treatment‐resistant depression, which may be associated with reduced rCBF of the right DLPFC before initiation of rTMS.

Study Design This cross-sectional study was conducted in a single hospital. Purpose To clarify the relationship between lower limb pain intensity and dynamic lumbopelvic-hip alignment in patients with lumbar spinal canal stenosis (LSS), using a three-dimensional (3D) motion analysis system. Overview of Literature Although it is well known that leg symptoms have a close relationship with posture in patients with LSS, the relationship under dynamic conditions, such as gait, remain unclear. Methods Thirty patients with LSS scheduled for spine surgery participated in this study. Lower limb pain was assessed using the Visual Analog Scale (VAS), and the patients were divided into two groups based on the mean scores (patients with scores above and below the mean were classified as the high-VAS and low-VAS groups, respectively). The kinematics of the spine, pelvis, and hip joints during gait were then measured using a 3D motion analysis system. Student paired t-tests were used to compare the angles of the spine, pelvis, and hip during gait between the two groups. Results Compared to those in the low-VAS group, the spine was significantly extended and bent toward the more painful lower limb side, and the pelvis was significantly anteriorly tilted among individuals in the high-VAS group. Conclusions Patients with LSS experiencing severe pain in their lower limb tend to keep the spine in a more extended position, bend laterally toward the painful side, and have an anteriorly tilted pelvic posture. The dynamic spinal and pelvic alignment was closely related to the intensity of the lower limb pain.

Abstract In this study, the rotation of liquid crystal droplets induced by elliptically polarized laser light was investigated using optical tweezers. The rotation mechanism was analyzed based on the arrangement of liquid crystal molecules within the droplets. The change in the rotation behavior of nematic liquid crystal (NLC) droplets was evaluated by varying the droplet size. The experimental results were analyzed based on the waveplate effect and light-scattering process. The rotation behavior of cholesteric liquid crystal droplets was examined by varying the droplet size and helical pitch, which was controlled by the chiral dopant concentration. The results are discussed in terms of the selective reflection of the incident beam by the helical structure. The dependence of the rotation frequency on the ellipticity of the incident beam was also studied. The main contribution to the rotation gradually changes from light transmission to reflection with increasing chirality of the droplet. An NLC rotator system was constructed using holographic optical tweezers. Such an optically controllable rotator is a typical micro-optomechanical device. Complex flow fields, including multiple vortex and localized shear fields, were realized at the micron scale.

Study Design Single-hospital cross-sectional study. Purpose The aim of the present study was to investigate the physical functions influencing dynamic postural change in patients with adult spinal deformity (ASD). Overview of Literature Dynamic postural change leading to increased forward lean during gait is a problem in patients with ASD; however, the relationship between this change and trunk and hip extensor strength is unclear. Methods Thirty patients with ASD aged ≥50 years who were admitted to our hospital between July 2016 and September 2019 were included in this study. X-ray parameters (i.e., sagittal vertical axis, pelvic tilt, and pelvic incidence minus lumbar lordosis) were evaluated from the full-length standing radiographs of the subjects. Trunk and hip extensor strength was evaluated using a hand-held dynamometer. Dynamic postural changes (i.e., sagittal trunk shift during standing, sagittal trunk shift during gait, and delta sagittal trunk shift) were assessed using a three-dimensional motion analysis system. The relationships between dynamic postural change and various X-ray parameters, as well as trunk and hip extensor strength, were examined using multivariable analysis. Results Multivariable analysis showed that hip extensor strength is the factor most strongly associated with dynamic postural change among the X-ray parameters and physical functions assessed in this study (β=−0.41, R2=0.12). Conclusions We demonstrated the association between dynamic postural change and hip extensor strength in patients with ASD. Our results may be useful to healthcare providers treating patients with ASD. Interventions for dynamic postural change in patients with ASD should focus on hip extensor strength.

Abstract Resistance training (RT) progress is determined by an individual’s muscle strength, measured by one-repetition maximum (1RM). However, this evaluation is time-consuming and has some safety concerns. Bioelectrical impedance analysis (BIA) is a valid and easy-to-use method to assess skeletal muscle mass (SMM). Although BIA measurements are often correlated with muscle strength, few studies of 1RM for RT and BIA measurements are available. This observational study examined the relationship between 1RM and BIA measurements and developed BIA-based prediction models for 1RM. Thirty-five healthy young Japanese adults were included. SMM and the skeletal muscle mass index (SMI) were measured using the BIA device. In addition, dominant-leg 1RM was measured using a unilateral leg-press (LP) machine. The correlations between BIA measurements and 1RM were calculated, and simple regression analyses were performed to predict 1RM from the BIA variables. The results showed significant correlations between 1RM and dominant-leg SMM (R = 0.845, P = 0.0001) and SMI (R = 0.910, P = 0.0001). The prediction models for 1RM for LP derived from SMM of the dominant leg and SMI were Y = 8.21x + 8.77 (P = 0.0001), R2 = 0.73, and Y = 15.53x − 36.33 (P = 0.0001), R2 = 0.83, respectively. Our results indicated that BIA-based SMI might be used to predict 1RM for LP accurately.

Abstract Background: Dengue is an arboviral disease that has a large effect on public health in subtropical and tropical countries. Rapid and accurate detection of dengue infection is necessary for diagnosis and disease management. We previously developed highly sensitive immunochromatographic devices, the TKK 1st and TKK 2nd kits, based on dengue virus (DENV) nonstructural protein 1 detection. However, these TKK kits were evaluated mainly using DENV type 2 clinical specimens collected in Bangladesh, and further validation using clinical specimens of other serotypes was needed. Methods: In the present study, one of the TKK kits, TKK 2nd, was evaluated using 10 DENV-1, 10 DENV-2, 4 DENV-3, 16 DENV-4, and 10 zika virus-infected clinical specimens collected in Bangkok, Thailand. Results: The TKK 2nd kit successfully detected all four DENV serotypes in patient serum specimens and did not show any cross-reactivities against zika virus serum specimens. The IgM and/or IgG anti-DENV antibodies were detected in seven serum specimens, but did not seem to affect the results of antigen detection in the TKK 2nd kit. Conclusion: The results showed that the TKK 2nd kit successfully detected all four DENV serotypes in clinical specimens and confirmed the potential of the kit for dengue diagnosis in endemic countries.

BackgroundR2* relaxometry and quantitative susceptibility mapping can be combined to distinguish between microstructural changes and iron deposition in white matter. Here, we aimed to explore microstructural changes in the white matter associated with clinical presentations such as cognitive impairment in patients with idiopathic normal-pressure hydrocephalus (iNPH) using R2* relaxometry analysis in combination with quantitative susceptibility mapping.MethodsWe evaluated 16 patients clinically diagnosed with possible or probable iNPH and 18 matched healthy controls (HC) who were chosen based on similarity in age and sex. R2* and quantitative susceptibility mapping were compared using voxel-wise and atlas-based one-way analysis of covariance (ANCOVA). Finally, partial correlation analyses were performed to assess the relationship between R2* and clinical presentations.ResultsR2* was lower in some white matter regions, including the bilateral superior longitudinal fascicle and sagittal stratum, in the iNPH group compared to the HC group. The voxel-based quantitative susceptibility mapping results did not differ between the groups. The atlas-based group comparisons yielded negative mean susceptibility values in almost all brain regions, indicating no clear paramagnetic iron deposition in the white matter of any subject. R2* and cognitive performance scores between the left superior longitudinal fasciculus (SLF) and right sagittal stratum (SS) were positively correlated. In addition to that, R2* and gait disturbance scores between left SS were negatively correlated.ConclusionOur analysis highlights the microstructural changes without iron deposition in the SLF and SS, and their association with cognitive impairment and gait disturbance in patients with iNPH.

An optically pumped magnetometer (OPM) is a new generation of magnetoencephalography (MEG) devices that is small, light, and works at room temperature. Due to these characteristics, OPMs enable flexible and wearable MEG systems. On the other hand, if we have a limited number of OPM sensors, we need to carefully design their sensor arrays depending on our purposes and regions of interests (ROIs). In this study, we propose a method that designs OPM sensor arrays for accurately estimating the cortical currents at the ROIs. Based on the resolution matrix of minimum norm estimate (MNE), our method sequentially determines the position of each sensor to optimize its inverse filter pointing to the ROIs and suppressing the signal leakage from the other areas. We call this method the Sensor array Optimization based on Resolution Matrix (SORM). We conducted simple and realistic simulation tests to evaluate its characteristics and efficacy for real OPM-MEG data. SORM designed the sensor arrays so that their leadfield matrices had high effective ranks as well as high sensitivities to ROIs. Although SORM is based on MNE, the sensor arrays designed by SORM were effective not only when we estimated the cortical currents by MNE but also when we did so by other methods. With real OPM-MEG data we confirmed its validity for real data. These analyses suggest that SORM is especially useful when we want to accurately estimate ROIs’ activities with a limited number of OPM sensors, such as brain-machine interfaces and diagnosing brain diseases.

An 82-year-old woman fell down the stairs and was brought to our hospital. When she came to our hospital, she had a left acute epidural hematoma, brain contusion, and splenic injury. During plain computed tomography (CT) imaging, hypotension and deteriorated level of consciousness was observed, and simultaneous head and abdominal surgery was performed to control intracranial hematoma growth and hemorrhagic shock. The head was positioned in right rotation and the trunk in supine position, and craniotomy and splenectomy were performed simultaneously. Simultaneous head and abdominal surgery is a very effective treatment strategy for multiple trauma because it does not require repositioning of the patient.

Background: Concerns regarding the impact of screen-based sedentary behavior on health have been increasing. Therefore, the present study investigated the longitudinal relationship between multiple screen time and nutrient intake in children and adolescents. Methods: The present study was conducted utilizing 3 years longitudinal data. Study subjects were 740 Japanese children aged between 6 and 12 years at baseline and between 9 and 15 years in the follow-up. Screen-based sedentary behavior was assessed using screen time, including television (TV) viewing, personal computer (PC) use, and mobile phone (MP) use. The main outcomes were the intakes of nutrients. Mixed effect multivariate linear regression analyses were used to examine the longitudinal relationship between screen-based sedentary time and nutrient intake. Covariates included in the multivariable analysis consisted of sex, age, solitary eating, skipping breakfast, staying up late, and body weight status, as confounders, and physical inactivity, as mediator. Results: In boys, a longer total screen time longitudinally correlated with higher intake of energy and lower intakes of protein, dietary fiber, minerals, and vitamins. In girls, longer total screen time longitudinally associated with higher intake of sucrose and lower intakes of protein, minerals, and vitamins. In boys, a longer TV viewing time was associated with higher intake of sucrose and lower intakes of protein, minerals, and vitamins. In girls, a longer TV viewing time was associated with higher intake of carbohydrates and lower intakes of protein, fat, minerals, and vitamins. In boys, relationships were observed between a longer PC use time and higher intakes of energy as well as lower intakes of protein, minerals, and vitamins. Relationship was observed between longer PC use time and lower intakes of minerals in girls. An increased MP use time was associated with higher intakes of energy, and lower intakes of protein, sucrose, dietary fiber, minerals, and vitamins in boys. A longer MP use time was associated with higher intakes of fat, and salt as well as lower intakes of carbohydrates, protein, minerals, and vitamins in girls. Conclusions: The present results revealed that longer screen-based sedentary behaviors were longitudinally associated with nutrient intake in children and adolescents. Future study is needed to elucidate these relationships.

We developed a cryocooler system to reveal the temperature dependence of the quality ( Q ) factor of high-temperature superconducting (HTS) bulk coil and the power transfer efficiency (PTE) and power handling capability (PHC) of a wireless power transfer (WPT) system using the HTS bulk coil. The measured Q factors of the HTS bulk coil were 9051 at 77 K and 14 050 at 25 K. The measured PTEs and PHCs of the WPT system were 44.2% and 4.48 W at 77 K, and 50.8% and 25.2 W at 25 K.

Herein, we report the orthodontic management of a patient with excessive bone and permanent tooth loss after surgical cyst removal. The patient was a 13-year-old Japanese boy who was referred to our department by an oral surgeon. He had an edentulous space with alveolar bone loss and loss of 2 permanent molars in the left mandibular region, following surgical removal of a large dentigerous cyst. We decided to close this space orthodontically. First, we moved the left mandibular second premolar into the edentulous region and autotransplanted the left maxillary lateral incisor in the adjacent distal space. We then performed comprehensive orthodontic treatment to establish stable occlusion. Following treatment, functional and stable occlusion of all permanent teeth was achieved without any spaces. The findings from this case suggest that orthodontic treatment is effective in growing patients with edentulous spaces and alveolar bone loss.

Distal transradial approach (dTRA) for neuroendovascular procedures has received much attention in recent years as a newer and less invasive alternative to the conventional transfemoral or transradial approaches. We present the case of an 89-year-old woman with a basilar artery aneurysm requiring simultaneous catheterization of the bilateral vertebral arteries who was successfully embolized using bilateral dTRA. The aneurysm was accessed from the right vertebral artery using the right dTRA. Control angiograms during the procedure were performed from the left vertebral artery via the left dTRA. The operator's posture was ergonomically comfortable, and the catheters were easy to handle during the procedure. To the best of our knowledge, this is the first case of a bilateral dTRA used for neuroendovascular procedures. Bilateral dTRA is a safe and minimally invasive method for patients and ergonomically comfortable for operators.

Abstract Precise control of molecule‐electrode interface is essential for molecular devices. Herein, new ruthenium acetylide molecular wires with long‐legged phosphine ligands to form a sterically controlled molecule‐electrode interface are designed. The sharpened Raman signals ascribed to acetylene stretching are observed for the self‐assembled monolayers (SAMs) of the molecular wires with the biphenyl‐ (2Au) and tert‐butylbiphenyl‐substituted long‐legged dppe‐type ligands (3Au), suggesting that steric hindrance causes formation of uniform SAMs. Scanning tunneling microscope break‐junction (STM‐BJ) study of 3Au reveals narrow conductance features compared with those of 1Au bearing the parent dppe ligands, indicating formation of a uniform molecular junction. Furthermore, the effective electronic interactions between the molecule and electrodes are unique to the long‐legged derivatives, as revealed by the surface‐enhanced Raman scattering study. Thus, the bulky long‐legged strategy turns out to provide a design concept for a well‐defined molecule‐electrode interface.

Abstract Objectives In July 2017, supplementary guidelines on anticoagulants, including direct oral anticoagulants, were published in Japan. We investigated the changes in endoscopic submucosal dissection (ESD) of gastric mucosal lesions after the publication of the supplement, examined the risk factors, and developed a predictive model for post‐ESD bleeding. Methods We included 2272 gastric ESD cases from our hospital between May 2003 and June 2021 and classified them into two groups: 1789 cases before and 483 after the publication of the supplementary guidelines. A predictive model for post‐ESD bleeding was developed using the pre‐publication cohort data. Results The proportion of patients receiving warfarin decreased (5.0% vs. 1.4%) and those receiving direct oral anticoagulants increased (1.2% vs. 6.8%) after the publication of the supplementary guidelines. Post‐ESD bleeding occurred in 61 patients, but there was no significant difference in the bleeding rate between the groups (50 [2.8%] vs. 11 [2.3%] patients, respectively). Five risk factors (number of antithrombotic agents, dialysis, heparin replacement, resection specimen size, and procedure time) were identified for model development. The C‐statistic for the model and post‐publication cohorts were 0.83 and 0.72, respectively. In the model, each risk factor for postoperative bleeding was scored, and the risk was classified into three levels according to the total score. Bleeding rates at low, intermediate, and high risks were 1.6%, 10.3%, and 38.9%, respectively. Conclusion Despite changes in patient characteristics and clinical practice regarding ESD before and after the publication of the supplementary guidelines, we could still develop a simple and useful predictive model.

Visible light-driven benzoyl azides formation catalyzed by a rhodium ion modified TiO2 (Rh3+/TiO2) is reported. The Rh3+/TiO2 was prepared as a visible light responsive photocatalyst by a simple procedure from TiO2 and RhCl3･3H2O. The Rh3+/TiO2 exhibited a broaden visible light absorption from 400 nm to 600 nm. Benzoyl azide formation from a benzotrichloride and a trimethylsilyl azide (TMS-N3) was performed catalyzed by the Rh3+/TiO2 under visible light irradiation (λ ≥ 420 nm) in air at room temperature. In this reaction, the benzotrichloride was effectively reduced by the single electron transfer (SET) from the Rh3+/TiO2, and the benzoyl azide was produced in 71% yield via the reaction between the benzoyl chloride and TMS-N3. In addition, several benzotrichloride derivatives were applied to this reaction and the corresponding benzoyl azide derivatives were formed in up to 71% yield. A kinetic analysis was also performed on these reactions, and it was suggested that the SET is the rate determining step in this reaction.

Abstract Some asymptomatic patients with diabetes mellitus (DM) have critical coronary artery disease (CAD), although the guidelines do not recommend aggressive screening for CAD in asymptomatic patients. Chronic kidney disease (CKD) is among the serious co-morbidities of severe systemic atherosclerosis. Thus, CKD may be associated with potential myocardial ischaemia. Therefore, the present study aimed to determine the impact of CKD on the incidence of silent myocardial ischaemia (SMI) and the long-term outcomes in asymptomatic patients with DM. This study investigated 461 consecutive patients with DM. All patients who were asymptomatic and self-sufficient in daily life underwent the ergometer exercise (ERG) test. Coronary angiography was performed if the stress test was positive, or if the patient did not achieve 90% of the target heart rate. The primary end point included major adverse cardiac and cerebrovascular events (MACCE) including death, non-fatal myocardial infarction and stroke. The median follow-up duration after study enrolment was 35 months for the entire cohort of 461 patients. Eighty-one patients were diagnosed with SMI. The estimated glomerular filtration rate was significantly lower in the SMI group (70.5 ± 23.8 vs. 81.8 ± 30.0 mL/min/1.73 m2, P stage 3a in the SMI subgroup. Urinary albumin (mg/gCr) was associated with MACCE in the SMI subgroup [HR 3.37, 95%CI (1.170–9.521), P = 0.025] after adjusting for age, sex, and conventional risk factors. SMI was more prevalent in patients with CKD and the incidence was proportional to the CKD stage in asymptomatic patients with DM. Those Patients with CKD and SMI exhibited poor clinical outcomes. CKD may be a key factor for the identification and management of SMI in asymptomatic patients with DM in routine clinical practice. Trial Registration: UMIN000038340.

This study melds Japanese principles of 3K and Dirty Work into a new model of Distasteful Work. Elements of work stigma and the associated taint affects workers’ both intrinsically and extrinsically. While 3K is traditionally associated with bluecollar occupations, the results of interviews with employees in Japanese companies suggest a willingness to transpose these associated stigma to white-collar work activities. Adverse stereotyping caused by distasteful work activities has the potential to cause a decrease in mental health leading to lower work efficiency. Particularly in countries that are suffering from labor shortage, maximizing the potential of existing experienced workforces and top talent is paramount, consequently adapted models are necessary. This research examines occupations within commercial banking and identify types of work activities which have higher potential to cause occupational health issues through associated work stigma.

Abstract A cytokine storm induces acute respiratory distress syndrome, the main cause of death in coronavirus disease 2019 (COVID-19) patients. However, the detailed mechanisms of cytokine induction due to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) remain unclear. To examine the cytokine production in COVID-19, we mimicked the disease in SARS-CoV-2-infected alveoli by adding the lysate of SARS-CoV-2-infected cells to cultured macrophages or induced pluripotent stem cell-derived myeloid cells. The cells secreted interleukin (IL)-6 after the addition of SARS-CoV-2-infected cell lysate. Screening of 25 SARS-CoV-2 protein-expressing plasmids revealed that the N protein-coding plasmid alone induced IL-6 production. The addition of anti-N antibody further enhanced IL-6 production, but the F(ab’)2 fragment did not. Sera from COVID-19 patients also enhanced IL-6 production, and sera from patients with severer disease induced higher levels of IL-6. These results suggest that anti-N antibody promotes IL-6 production in SARS-CoV-2-infected alveoli, leading to the cytokine storm of COVID-19.

Abstract Objectives The β3‐adrenergic receptor (ADRB3) gene polymorphism has been implicated in obesity. Therefore, the contribution of ADRB3 Trp64Arg polymorphism to obesity‐related indicators was investigated, taking into account the lifestyle‐related factors in a Japanese rural population. Methods A total of 600 Japanese adults aged ≥40 years in a population‐based cohort study were analyzed. The ADRB3 polymorphism was determined using peripheral blood samples. Associations between genotype and body mass index (BMI), waist circumference (WC), and body fat (BF) percentage were examined, adjusting for lifestyle‐related factors, including daily nutrient intake. Results The frequency of Arg64 allele carriers was 36%. There was no significant difference in BMI, WC, or BF between the groups with or without the Trp64Arg polymorphism. Multivariable logistic regression analysis showed that the Trp64Arg polymorphism was not associated with these three indicators, but lifestyle factors including physical inactivity, higher energy and sodium consumption, and less animal protein intake were significantly related to increased WC and BF percentages. Conclusions The Trp64Arg polymorphism of ADRB3 gene did not contribute to increased BMI, WC, or BF. However, lifestyle‐related factors impacted these indicators in middle‐aged and older Japanese individuals living in rural areas.

Soccer players with chronic ankle instability (CAI) may stabilize their supporting leg by the proximal joint to compensate for the ankle instability during kicking motion. This study aimed to investigate the characteristics of leg and joint stiffness of the supporting leg during side-foot kicking in soccer players with CAI. Twenty-four male collegiate-level soccer players with and without CAI participated in this study. The kinematic and kinetic data were obtained using a three-dimensional motion analysis system. Leg stiffness and joint (hip, knee, and ankle) stiffness in the sagittal and frontal planes were calculated and analyzed. The results clarified that soccer players with CAI (0.106 ± 0.053 Nm/°) had greater knee stiffness in knee adduction during the kicking cycle compared to those without CAI (0.066 ± 0.030 Nm/°; p = 0.046), whereas no characteristic differences were observed in knee stiffness in knee flexion and hip and ankle stiffness (p > 0.05). Knee stiffness is believed to occur to compensate for ankle joint instability in the supporting leg. Therefore, adjusting knee stiffness to accommodate ankle joint instability is crucial for maintaining kicking performance. Based on results of this study, it may be important to consider training and exercises focused on joint coordination to improve knee stiffness in soccer players with CAI.

Abstract Background Few epidemiological studies have been performed to clarify the association between glucose metabolism disorders in early adults (20 years old) and physiological and environmental factors, including body mass index (BMI) in junior high school days. Therefore, we examined the association between hemoglobin A1c (HbA1c) level and body size (BMI) in early adulthood and lifestyles, including sleep habits and BMI in junior high school days in Shika town, a small town in Japan, by conducting a retrospective cohort study. Methods We examined the HbA1c levels and body size (BMI) of 99 early adults who turned 20 years old between 2016 and 2020 and were residing in Shika town, Ishikawa Prefecture. We obtained the information on lifestyles and living environment factors, including BMI, from a questionnaire survey conducted among the subjects during their junior high school days (13–15 years old) from 2009 to 2013. Results No correlations were observed between the HbA1c levels and the BMI values of the early adults. A two-way analysis of covariance (with the HbA1c levels and BMI values of the early adults as main factors) of the body size and lifestyle habits of the junior high school students revealed that “sleep quality in junior high school” was significantly poorer in the high HbA1c group than in the low HbA1c group in the early adults with high BMI values only. This result was also supported by the logistic regression analysis result. Conclusions The present results indicate that poor sleep quality in junior high school was associated with the high HbA1c levels of the early adults with higher BMI values, which suggests that good sleep quality in junior high school prevents the development of hyperglycemia. However, the present study did not find any relationship between early-adult BMI and HbA1c level.
