Clusters of low fitness and high obesity in childhood are associated with poorer health outcomes in later life, however their relationship with cognition is unknown. Identifying such profiles may inform strategies to reduce risk of cognitive decline. This study examined whether specific profiles of childhood fitness and obesity were associated with midlife cognition.Prospective study.In 1985, participants aged 7-15 years from the Australian Childhood Determinants of Adult Health study were assessed for fitness (cardiorespiratory, muscular power, muscular endurance) and anthropometry (waist-to-hip ratio). Participants were followed up between 2017 and 2019 (aged 39-50). Composites of psychomotor speed-attention, learning-working memory and global cognition were assessed using CogState computerised battery. Latent profile analysis was used to derive mutually exclusive profiles based on fitness and anthropometry. Linear regression analyses examined associations between childhood profile membership and midlife cognition adjusting for age, sex and education level.1244 participants were included [age: 44.4 ± 2.6 (mean ± SD) years, 53% female]. Compared to those with the highest levels of fitness and lowest waist-to-hip ratio, three different profiles characterised by combinations of poorer cardiorespiratory fitness, muscular endurance and power were associated with lower midlife psychomotor-attention [up to -1.09 (-1.92, -0.26) SD], and lower global cognition [up to -0.71 (-1.41, -0.01) SD]. No associations were detected with learning-working memory.Strategies that improve low fitness and decrease obesity levels in childhood could contribute to improvements in cognitive performance in midlife.

We evaluated the safety and potency of the Kyasanur Forest disease (KFD) vaccine inactivated with different formalin concentrations in mice, since the side effects due to higher formalin concentrations have been a major reason for vaccine refusal. Furthermore, with an objective to reduce the use of mice in vaccine testing, we performed quantification of the KFD virus by real-time PCR and compared it with in vivo titration in mice. The KFD vaccine prepared in chicken embryo fibroblast cells was inactivated with 0.04%, 0.06%, and 0.08% concentrations of formalin. The vaccine inactivated with 0.04% and 0.06% formalin failed the safety test, whereas the KFD vaccine inactivated with 0.08% formalin was safe and potent with a log protective index of 5678 in mice. This reduced formalin content may induce no/lesser side-effects of pain/swelling which may increase the vaccine acceptance. This study evaluated the impact of contrasting tillage and nitrogen management options on the growth, yield attributes, and yield of maize (Zea mays L.) in a conservation agriculture (CA)-based maize-wheat (Triticum aestivum L.) system. The field experiment was conducted during the rainy (kharif) seasons of 2020 and 2021 at the research farm of ICAR-Indian Agricultural Research Institute (IARI), New Delhi. The experiment was conducted in a split plot design with three tillage practices [conventional tillage with residue (CT), zero tillage with residue (ZT) and permanent beds with residue (PB)] as main plot treatments and in sub-plots five nitrogen management options [Control (without N fertilization), recommended dose of N @150 kg N/ha, Green Seeker-GS based application of split applied N, N applied as basal through urea super granules-USG + GS based application and 100% basal application of slow release fertilizer (SRF) @150 kg N/ha] with three replications. Results showed that both tillage and nitrogen management options had a significant impact on maize growth, yield attributes, and yield in both seasons. However, time to anthesis and physiological maturity were not significantly affected. Yield attributes were highest in the permanent beds and zero tillage plots, with similar numbers of grains per cob (486.1 and 468.6). The highest leaf area index (LAI) at 60 DAP was observed in PB (5.79), followed by ZT(5.68) and the lowest was recorded in CT (5.25) plots. The highest grain yield (2-year mean basis) was recorded with permanent beds plots (5516 kg/ha), while the lowestwas observed with conventional tillage (4931 kg/ha). Therefore, the study highlights the importance of CA practices for improving maize growth and yield, and suggests that farmers can achieve better results through the adoption of CA-based permanent beds and use of USG as nitrogen management option.

Introduction The prevalence of diabetes is increasing globally . 3 to 5 % of all pregnant women show glucose intolerance. Approximately 90% of these women have GDM.Infants born to diabetic mothers are prone to complications like congenital malformations and metabolic abnormalities.Most of these complications depend on maternal glycemic control which can be prevented by good periconceptional and prenatal care. Aims and objectives This study is taken up to evaluate complications occurring in infants of diabetic mothers and also to compare the outcome of infants of preGDM mothers and GDM mothers Methodology This is hospital based observational study done on infants of diabetic mothers admitted in NICU, KGH, Visakhapatnam Neonates of diabetic mothers who have other complications like heart disease, PIH, Pre eclampsia, twin gestation, etc. which may effect the baby adversely are excluded from study Results A total of 50 neonates are studied out of whom 37 neonates are born to mothers with gestational diabetes and 13 to mothers with overt diabetes . Mean birth weight of infants of GDM is 3.7kg. Hypoglycemia is more common in infants of GDM mothers than in overt DM. Congenital anamolies are seen in 14% of babies. Birth injures including clavicle fracture , erb's palsy are seen in 5 babies, all of then weighed >3.5kg and delivered by assisted vaginal delivery. Conclusion Macrosomia,birth injuries and metabolic complications are common in infants of GDM mothers than in infants of pre GDM mothers.This shows importance of timely screening of all pregnant women for GDM and counsel them regarding importance of glycemic control to prevent neonatal complications.

A study was conducted to find the impact of lockdown due to the pandemic of coronavirus disease 2019 (COVID-19) on agriculture, in general, and on oil palm cultivation, in specific. Results revealed that the majority of the farmers contacted had 26-30 years of farming experience, were between 31-60 years of age, were educated up to high school level, with an average of four family members and possessed medium landholdings. The crops grown by these farmers include oil palm, maize, coconut, paddy, cocoa and vegetables. The majority of them had irrigated dry lands. All the farmers were aware about the reasons for the lockdown due to the COVID-19 pandemic. Areas where lockdown impact was visible on agriculture, either directly or indirectly, include labour availability, high cost of labour, harvesting, transport, and market prices. Resources affected due to lockdown were labour, fertilizers, harvesting, transport etc. All cultural practices were adopted in oil palm. The majority of the respondents perceived that oil palm was not affected compared to other crops during the lockdown; they opined to have obtained a good market price for oil palm during the lockdown period. Most of them informed that they received oil palm cultivation related messages sent by ICAR-IIOPR, Pedavegi. They perceived to be aware of the technologies during the lockdown period. Messages (SMS) on oil palm irrigation, cultivation practices, and fertigation were adopted. The need for Government assistance during the lockdown period was felt in the following aspects for different crops, viz., marketing, cultivation assistance, and transport. Farmers perceived that lockdown might have long-term implications due to the non-availability of labour for harvesting crops. It is evident from the study that crops affected severely during the lockdown period require long term strategies to mitigate the problems faced. Farmers must be supported with minimum support price (MSP), transport facilities and ad hoc strategies to sell their products through the government purchase mechanism. A need for monitoring the increase in the prices of agricultural inputs was also felt.

The recent trends in the healthcare sector have catalyzed the emergence of several healthcare ecosystems. The inevitable and innate health data exchange amongst ecosystem components for provisioning of healthcare services as well as secondary use has raised several trust issues. The consent is a predominant mechanism to ensure privacy preserving, secure and ethical exchange, as well as sharing of health records of a patient by establishments. The proposed consent management system supports the life cycle of consent given by the patient or their authorized entity as well as validation and delegation of consent by a provider for health records requested by a user in an ecosystem. It reinforces different types as well as flavors of consent to strike a balance between privacy protection, ease of use, and patient safety. It supports the simple, flexible, and efficient management of consent at different granularity of health resource, user, and purpose. This is enabled by embedding consent structure in tree representation of EHR and a tree traversal algorithm for conflict resolution.

The safety of the piping systems used in any nuclear or thermal power plant is essential to the environment and also to the people involved. Pipe bends are one of the critical components of the piping system. Hence, fatigue life estimation becomes primary focus when the designing pipe bends for their safety limits. The current study uses finite element analysis and the revised manson universal slope method to estimate the life of elbows under in-plane opening and closing cyclic bending moment. The estimated fatigue life was compared with low-cycle fatigue tests in the literature. The displacement of 20 mm was given as input to estimate the number of cycles. The principal stresses were obtained on the highly strained node of the finite element model to locate the crack and its direction accurately. The elbows with thinning have a reduction in the fatigue life of about 19%.

Falls are a major public health concern for the elderly around the world. When it comes to old age, we must keep an eye on our loved ones to ensure their health and safety. It is therefore critical to determine if an elderly person has fallen so that help can be provided promptly. Proposing a person fall detection system based on a wearable device for detecting the falls of people in every situation, which takes advantage of lowpower wireless sensor networks, smart devices, and analyses human body motions. The system detects movement using an accelerometer and a gyro sensor. The sensor is wired to a microprocessor, which transmits the acceleration data continuously. Fall detection and sudden movement changes in the person would be monitored by the system. The sensors are getting values from a quick movement shift with shock in the system. When a person falls and becomes unconscious, the system determines whether the person has indeed fallen. If the person has truly fallen, the system will send an alert to the caregivers and sound an alarm to alert anyone nearby. When the system detects that a person has fallen, it immediately sends an alert to the individual's care takers. It is an IoT-based fall detection system that assists people by telling their caregivers about their fall so that quick attention may be drawn to the situation and essential actions can be taken to save the person who has fallen. To review the good documentation practices in the pharmaceutical industry as per the guidelines provided in the European Commission and WHO (World Health Organisation). GDocP is a systematic method of planning, evaluating, approving, issuing, documenting, preserving, and archiving records. GDocP is a must when operating in an atmosphere of existing GMP. To provide an overview of good documentation practices for those employed in the pharmaceutical and healthcare sectors. The GDocP stands for good documentation practice which can be described as an integral part of quality assurance (QA) and thus related to all aspects of GMP. There are very few articles related to the comparison of GDocP in the pharmaceutical manufacturing unit as per European and WHO guidelines hence we are trying to develop a study on the comparison of GDocP guidelines. Public and private organizations, institutions, and regulatory authorities working and cooperating and with the pharmaceutical industry are involved at the international and national level to reach a consensus on the guidelines and laws for the production of medicinal products for human. This article explains how these participants work and cooperate and set out current regulations along the lines of the European community and WHO referencing, where appropriate, the practiced guidelines, outside space regulatory action referred to above. In this way, the goal is to achieve exceptional standards of quality, protection, and efficacy in the manufacture of health products.

Background To examine the effect of frailty on cognitive decline independent of cerebral small vessel disease (cSVD) and brain atrophy, and whether associations between neuropathology and cognition differed depending on frailty status. Methods The Tasmanian Study of Cognition and Gait was a population-based longitudinal cohort study with data collected at 3 phases from 2005 to 2012. Participants aged 60–85 were randomly selected from the electoral roll. Various data were used to operationalize a 36-item frailty index (FI) at baseline. Brain MRI was undertaken to obtain baseline measures of neuropathology. A neuropsychological battery was used to assess cognition at each time point. Generalized linear mixed models were used to examine the effect of frailty and MRI measures on cognition over time. The associations between MRI measures and cognition were explored after stratifying the sample by baseline frailty status. All analyses were adjusted for age, sex, and education. Results A total of 385 participants were included at baseline. The mean age was 72.5 years (standard deviation [SD] 7.0), 44% were female (n = 171). In fully adjusted linear mixed models, frailty (FI × time β −0.001, 95% confidence interval [CI] −0.003, −0.001, p = .03) was associated with decline in global cognition, independent of brain atrophy, and cSVD. The association between cSVD and global cognition was significant only in those with low levels of frailty (p = .03). Conclusion These findings suggest that frailty is an important factor in early cognitive dysfunction, and measuring frailty may prove useful to help identify future risk of cognitive decline.

Objectives:It is unknown whether there are sex-related profiles of cardiometabolic health that contribute differently to age-related changes in brain health during midlife. We studied how latent classes of middle-aged individuals clustering by age, sex, menopause and cardiometabolic health were associated with brain structure and cognitive performance.Methods:Health, brain and abdominal magnetic resonance imaging (MRI) data from the UK Biobank cohort (men and women >40 years in the United Kingdom) were used. We applied latent class analysis to identify groups of individuals based on age, sex, menopausal status and cardiometabolic health. We examined associations of class membership with brain volumes (total brain [TBV], grey matter [GMV], white matter [WMV], hippocampal [HV], white matter hyperintensities [WMHV]) and cognitive performance.Results:Data were available for 36,420 individuals (mean age 64.9 years, 48.5% women). Eight latent classes differing in age, sex and cardiometabolic risk were identified. Class 1 (reference class) included individuals with the lowest probability of older age and cardiometabolic risk, and they had the most positive associations with cognitive function and brain volumes. In those aged >60, but not in those aged 50-60 years, the negative associations of age with TBV, GMV and WMV were greater in the class comprising healthier older women than classes comprising older men of varying cardiometabolic and vascular health. There were no age-class interactions for cognitive test performance.Conclusions:Latent class analysis detected groups of middle-aged individuals clustering by cardiometabolic health. The relationship of age with brain volumes varies by sex, menopausal status and cardiometabolic health profile.

BackgroundThere has been a decline in the stroke incidence across high income countries but such knowledge exists at Country or State rather than areal unit level such local government area (LGA). In this disease mapping study, we evaluate if there are local hot spots or temporal trends in TIA rate. Such knowledge will be of help in planning healthcare service delivery across regions.MethodsLinked hospital discharge data (Victorian Admitted Episodes Dataset or VAED) was used to collect TIA (defined by ICD-10-AM codes G450-G459) cases from 2001 to 2011. The State of Victoria is the second most populous state in Australia, with a population of 6.7 million and can be divided into 79 administrative units or LGA. The data is anonymized and contains residence of the patient in terms of LGA but not exact location. The date of the TIA event when the patient is admitted to hospital is provided in the dataset. The number of TIAs per year was aggregated for each LGA. Standardized TIA ratios were calculated by dividing actual over expected cases for each LGA per year. We used Integrated Nested Laplace Approximation (INLA) to perform spatial and spatiotemporal regression, adjusting for hypertension, sex and population, age (≥60), and socio-economic status (SES) decile within the LGA. The final model was chosen based on the lowest the Deviance Information Criterion (DIC) and Watanabe-Akaike information criteria (WAIC).ResultsChoropleth maps showed a higher standardized TIA ratios in North-West rural region. Compared to the baseline model (DIC 13,159, WAIC 13,261), adding in a spatial random effect significantly improved the model (DIC 6,463, WAIC 6,667). However, adding a temporal component did not lead to a significant improvement (DIC 6,483, WAIC 6,707).ConclusionOur finding suggests a statically significant spatial component to TIA rate over regional areas but no temporal changes or yearly trends. We propose that such exploratory method should be followed by evaluation of reasons for regional variations and which in turn can identify opportunities in primary prevention of stroke, and stroke care.

Background The media play a critical role in informing the public about the COVID-19 pandemic. Throughout the pandemic, international travel has been a highly contested subject at both the international and national levels. We examined Canadian media reporting on international travel restrictions during the pandemic, how these restrictions aligned with the International Health Regulations (IHR 2005), and how the narrative around international travel evolved over time. Methods We analysed articles from Canada’s top three national newspapers by circulation – The Globe and Mail, The National Post and The Toronto Star - published between Jan 1, 2020 - May 31, 2020. Our search yielded a total of 378 articles across the three newspapers. After removing duplicates and screening the remaining articles, we included a total of 62 articles for the analysis. We conducted a qualitative media content analysis by using an inductive coding approach. Results Three major themes were identified within the articles. These included: 1) The role of scientific and expert evidence in implementing travel restrictions; 2) Federal legislation, regulation and enforcement of international travel measures; and 3) Compliance with World Health Organization (WHO) guidelines in travel restriction policy- and decision-making. The federal government relied primarily on scientific evidence for implementing international travel restrictions and fully exercised its powers under the Quarantine Act to enforce travel regulations and comply with the IHR 2005. The government embraced a rules-based international order by following WHO recommendations on international travel, contributing to a delay in border closure and travel restrictions until mid-March. Conclusion The media focussed significantly on international travel-related issues during the early phase of the pandemic. The dominant media narrative surrounded the need for earlier travel restrictions against international travel.

Energy in the world is on high emergency now days. Energy resources are abundant but exhausted as per present conditions. Search for alternate energy sources has been trending worldwide. Solar and wind energies are of present interest to minimize cost of investment. But utilization of Municipal and Solid waste as an alternate source of energy is vital in research. Producing fuel briquettes with naturally available waste materials which are biodegradable is an asset. The experimentation is carried out for moisture content, ash content, volatile matter, fixed carbon, sulfur, calorific value and water resistance. The waste materials are mixed with all purpose flour in different ratio’s. Calorific value is evaluated based on ultimate analysis. The effects of various parameters as fuel briquettes for power production were measured of data consisting of characterization (FTIR and XRD), Analysis of materials (Proximate and Ultimate) and Estimation of properties for fuel briquettes (Calorific value, porosity, waster absorption, shatter index, compressive strength, optimization of materials quantity using Response Surface Methodology, Tumbling test, Degree of Densification, Water Boiler Test–Thermal Efficiency, Power Estimation and Power Calculated). The experimental data are obtained by conducting batch experimentations. In the present investigation, the prospective of four materials (leaves) namely red sandal, Wrightiatinctoria, teak and mixed leaves were evaluated to estimate their performance as fuel briquettes for the power production. The result of four biomasses wastes are in fact potential sources of energy which has been validated by the quite substantial amounts of briquettes obtained using these as raw materials and fuel briquettes have environmental benefits that include reduced tree degradation, better management of waste and reduced emissions.

Composites compressed by Camellia Sinensis/Anana Comosus/GFRP are universally recognized as a stronger substitute for acoustic damping. The environmental consequences due to carcinogenic materials, including polyhydride, polyurethane (PU) foams, polyethylene (PE), etc., act as a starting point for the development of high strength/weight, biodegradable and corrosion-resistant natural products (natural fiber-based composites). In this investigation, a single-dimensional pineapple fiber is combined with GRPF and waste tea flakes with help of an epoxy binder. Five different specimens of varying weight percentages were analyzed. The optimization method (TOPSIS & WASPAS) optimizes input parameter of weight fraction percentage with six corresponding different output responses. The best-integrated weight fraction percentage to enhance acoustic absorption is selected as the optimal concoction for the manufacture of acoustic damping materials.

Dual decline in gait speed and cognition has been found to be associated with increased dementia risk in previous studies. However, it is unclear if risks are conferred by a decline in domain-specific cognition and gait.To examine associations between dual decline in gait speed and cognition (ie, global, memory, processing speed, and verbal fluency) with risk of dementia.This cohort study used data from older adults in Australia and the US who participated in a randomized clinical trial testing low-dose aspirin between 2010 and 2017. Eligible participants in the original trial were aged 70 years or older, or 65 years or older for US participants identifying as African American or Hispanic. Data analysis was performed between October 2020 and November 2021.Gait speed, measured at 0, 2, 4, and 6 years and trial close-out in 2017. Cognitive measures included Modified Mini-Mental State examination (3MS) for global cognition, Hopkins Verbal Learning Test-Revised (HVLT-R) for memory, Symbol Digit Modalities (SDMT) for processing speed, and Controlled Oral Word Association Test (COWAT-F) for verbal fluency, assessed at years 0, 1, 3, 5, and close-out. Participants were classified into 4 groups: dual decline in gait and cognition, gait decline only, cognitive decline only, and nondecliners. Cognitive decline was defined as membership of the lowest tertile of annual change. Gait decline was defined as a decline in gait speed of 0.05 m/s or greater per year across the study.Dementia (using Diagnostic and Statistical Manual of Mental Disorders [Fourth Edition] criteria) was adjudicated by an expert panel using cognitive tests, functional status, and clinical records. Cox proportional hazard models were used to estimate risk of dementia adjusting for covariates, with death as competing risk.Of 19 114 randomized participants, 16 855 (88.2%) had longitudinal gait and cognitive data for inclusion in this study (mean [SD] age, 75.0 [4.4] years; 9435 women [56.0%], 7558 participants [44.8%] with 12 or more years of education). Compared with nondecliners, risk of dementia was highest in the gait plus HVLT-R decliners (hazard ratio [HR], 24.7; 95% CI, 16.3-37.3), followed by the gait plus 3MS (HR, 22.2; 95% CI, 15.0-32.9), gait plus COWAT-F (HR, 4.7; 95% CI, 3.5-6.3), and gait plus SDMT (HR, 4.3; 95% CI, 3.2-5.8) groups. Dual decliners had a higher risk of dementia than those with either gait or cognitive decline alone for 3MS and HVLT-R.Of domains examined, the combination of decline in gait speed with memory had the strongest association with dementia risk. These findings support the inclusion of gait speed in dementia risk screening assessments.

ObjectivesWe compared the performance of laboratory-based cardiovascular risk prediction tools in a low-income and middle-income country setting, and estimated the use of antihypertensive and lipid-lowering medications in those deemed at high risk of a cardiovascular event.DesignA cross-sectional study.SettingThe study population comprised adult residents (aged ≥18 years) of the Rishi Valley region located in Chittoor District, south-western Andhra Pradesh, India.Participants7935 participants were surveyed between 2012 and 2015. We computed the 10-year cardiovascular risk and undertook pair-to-pair analyses between various risk tools used to predict a fatal or non-fatal cardiovascular event (Framingham Risk Score (FRS), World Health Organization Risk Score (WHO-RS) and Australian Risk Score (ARS)), or a fatal cardiovascular event (Systematic COronary Risk Evaluation (SCORE-high and SCORE-low)). Concordance was assessed by ordinary least-products (OLP) regression (for risk score) and quadratic weighted kappa (κw, for risk category).ResultsOf participants aged 35–74 years, 3.5% had prior cardiovascular disease. The relationships between risk scores were quasi-linear with good agreement between the FRS and ARS (OLP slope=0.96, κw=0.89). However, the WHO-RS underestimated cardiovascular risk compared with all other tools. Twenty per cent of participants had ≥20% risk of an event using the ARS; 5% greater than the FRS and nearly threefold greater than the WHO-RS. Similarly, 16% of participants had a risk score ≥5% using SCORE-high which was 6% greater than for SCORE-low. Overall, absolute cardiovascular risk increased with age and was greater in men than women. Only 9%–12% of those deemed ‘high risk’ were taking lipid-lowering or antihypertensive medication.ConclusionsCardiovascular risk prediction tools perform disparately in this setting of disadvantage. Few deemed at high risk were receiving the recommended treatment.

Identifying the key Quality of Service attributes of such platforms is vital for their continued success and growth. In this paper, a set of quality attributes for platforms is first extracted from different extant quality models. Then actual user feedback data from three platform providers are analysed and mapped against the set of quality attributes to determine the key attributes that are relevant. These findings are corroborated with qualitative data from interviews of different stakeholders. The results show that service quality characteristics are important to the success of platforms. Functional characteristics of platforms assume importance where the digital contributions of the platform is higher. Apart from these, ‘fitness for use’ as a major determinant of quality is also important in digital platforms.
