Primary cardiac tumors are infrequent, with an incidence between 0.001% and 0.2%, mostly comprising benign histological characteristics in 75% of these cases. Myxomas account for approximately 50% of these neoplasms. As regards location, 75-80% of myxomas are in the left atrium, 18% in the right atrium, and more rarely in the ventricles. We report a case of a patient in functional class (FC) IV New York Heart Association (NYHA) and postoperative histological diagnosis of multilobular myxoma originating in the posterior left atrial wall. Clinical evaluation 3 months after surgery suggested NYHA functional class I and echocardiographic absence of intracardiac masses.

To determine, even during postoperative period, the confiability of the cardiac index correlate with the data data given by a central atrial venous blood gasometry in patients who underwent cardiac surgery.From the sample of arterial and venous blood of right atrium gathered in postoperative of cardiac surgery, it was determinated the hemoglobin concentration and the gasometric study through what was observed of the venous oxygen saturation (SvO2) and the partial pressure of oxygen from venous blood gathered in right atrium (PvO2), add to the calculation of artery-venous difference of the oxygen content--radial artery / right atrium (C( a-v )O2). Afterwards, these parameters were compared with the cardiac index determined by thermodilution.There was good correlation between SvO2, C(av)O2 of the venous right atrial blood and cardiac index measured by thermodilution method, with sensibility and specificity good and high positive predict value and negative predict value. The PvO2 demonstrated poor sensibility in the estimative of low output.In cardiac surgery postoperative, the SvO2 and the C(a-v)O2 were safe parameters correlated with low cardiac output. The PvO2 demonstrated poor sensibility in the estimative of low output in postoperative cardiac surgery.

O tratamento da Persistência do Canal Arterial (PCA), em adultos, ainda é controverso. A utilização de próteses auto-expansíveis tem-se mostrado como uma alternativa eficaz ao tratamento cirúrgico. Apresentamos um caso de uma paciente de 45 anos submetida ao tratamento endovascular com o uso de stent auto-expansível.

The treatment for closure of persistent ductus arteriosus (PDA) in adults still controversial. The endovascular approach has been shown as an effective alternative to surgical treatment. We report a case of 45 years old patient submitted to endovascular approach for PDA closure.

OBJETIVO: Avaliar o efeito do pré e pós-condicionamento isquêmico sobre a lesão tecidual na mucosa intestinal de ratos submetidos ao processo de isquemia e reperfusão mesentérica. MÉTODOS: Foram estudados 30 ratos Wistar, distribuídos em três grupos: grupo A, em que se realizou isquemia (30 minutos) e reperfusão (60 minutos) mesentérica; grupo B, isquemia e reperfusão mesentérica precedidos pelo pré-condicionamento isquêmico por três ciclos de isquemia e reperfusão com duração de dois minutos cada; grupo C, isquemia e reperfusão mesentérica e, precedendo o início da reperfusão, foi realizado o pós-condicionamento isquêmico por três ciclos de reperfusão e isquemia com duração de dois minutos cada. Ao final, ressecou-se um segmento do intestino delgado para análise histológica. Avaliaram-se os resultados pela classificação de Chiu et al. e procedeu-se ao tratamento estatístico. RESULTADOS: As médias dos graus de lesão tecidual segundo a classificação de Chiu et al. foram: no grupo A, 3,5; grupo B, 1,2; grupo C, 1. A diferença entre o resultado do grupo A com os resultados dos grupos B e C foi considerada estatisticamente significativa (P < 0,05). CONCLUSÃO: O pré e pós-condicionamento isquêmico foram capazes de minimizar, com intensidade semelhante, a lesão tecidual na mucosa intestinal de ratos submetidos ao processo de isquemia e reperfusão mesentérica. To evaluate the results of the surgical treatment of atrial fibrillation for ablation of the posterior left atrial wall using electrocautery in mitral valve surgery.From May 2004 to December 2006, 23 patients underwent surgical correction of mitral valve disease and treatment of atrial fibrillation using the conventional electrocautery for the accomplishment of lines of endocardial ablation in the left atrium. The mean age of the patients was 59 years, and 60.8% were female. The left atrium mean diameter was 50.3 +/- 5.09 mm and the left ventricular ejection fraction was 53.6 +/- 11.03%.The mean time of extracorporeal circulation was 52.5 +/- 13.3 min; aortic clamping, 35.6 +/- 12.9 min; atrial ablation, 3.05 +/- 0.16 min. All the patients were free of atrial fibrillation after the procedure; on hospital discharge, 69.5%; at 6 months, 91.3%; at 12 months, 76.4%; at 18 months, and at 24 months, 68.4%. At 12 months, left atrium mean diameter was 42.1 +/- 3.5 mm; left ventricular ejection fraction was 59.2 +/- 3.48%; In addition, left atrial contraction was present in 68.8% of the patients.The surgical treatment of the atrial fibrillation with electrocautery in mitral valve surgery was capable to determine the reversion of this arrhythmia in a significant number of patients during short- and middle-term clinical follow-up without mortality and fewer complications.

To analyze the short-term and mid-term follow-ups of patients with heart failure and moderate to severe mitral valve insufficiency and who have undergone mitral valve replacement with crossed papillopexy and annular constriction.Thirteen patients in NYHA functional class III or IV, with a mean age of 54.1 years and with idiopathic etiology, underwent mitral valve replacement with ring constriction and crossed papillopexy. Echocardiograph parameters, functional class and survival actuarial curve were analyzed.There were no deaths during surgery or in the postoperative period. The mean left ventricular diastolic diameter was reduced from 71 +/- 8.6 mm to 65.3 +/- 8.6 mm (p=0.049) and the mean left ventricular systolic diameter was reduced from .1 +/- 8.5 mm to 50.4 +/- 11.1 mm (p=0.002). The atrial diameters varied from 49.4 +/- 6.4 mm to 44 +/- 5.9 mm (p=0.017); the percentage of the left ventricular shortening was 17 +/- 4 % to 24 +/- 8.3% (p=0.014); the ejection fraction varied statistically and significantly from 34 +/- 9% to 45 +/- 14% (p=0.008). Eleven (84.6%) patients were in FC I and II. At 1, 6 and 12 months after follow-up surgery, the survival rate was 100%, 82.6%, 71.6%, respectively. This rate was maintained at 7.6% for more than 36 months.The results obtained from patients with heart failure and moderate to severe mitral valve insufficiency and who underwent mitral valve replacement with crossed papillopexy and annular constriction presented evidence of improved heart remodeling and significant improvement in left ventricular performance.

PURPOSE: To evaluate the effect of the ischemic preconditioning and the ischemic postconditioning over the tissue injury in the intestinal mucosa of rats undergoing the procedure of mesenteric ischemia and reperfusion. METHODS: Thirty Wistar rats were studied, divided in three groups: group A, undergoing mesenteric ischemia (30 minutes) and reperfusion (60 minutes); group B, mesenteric ischemia and reperfusion preceded by ischemic preconditioning; group C, mesenteric ischemia and reperfusion and, before the beginning of reperfusion, the ischemic postconditioning was performed. At the end, a segment of the small intestine was dissected for histological analysis. The results were evaluated using the CHIU et al.6 classification followed by the statistic treatment. RESULTS: The mean values of the tissue injury levels were: group A, 3.5; group B, 1.2; and group C, 1. The difference between the result of group A with the results of groups B and C was considered statistically significant (p < 0,05). CONCLUSION: The ischemic preconditioning and postconditioning are able to minimize the tissue injury in the intestines of rats that underwent the procedure of mesenteric ischemia and reperfusion. OBJETIVO: Analisar os resultados em curto e médio prazo de pacientes portadores de insuficiência cardíaca e insuficiência mitral moderada/grave submetidos a substituição valvar mitral com a técnica da papilopexia cruzada e constrição anular. MÉTODOS: Treze pacientes em classe funcional III ou IV (NYHA), idade média de 54,1 anos, etiologia idiopática, foram submetidos a substituição valvar mitral com constrição do anel e papilopexia cruzada. Foram analisados os parâmetros ecocardiográficos, classe funcional e curva atuarial de sobrevivência. RESULTADOS: Não houve mortes no período trans e pósoperatório imediato. A média dos diâmetros diastólicos e sistólicos ventricular esquerdo reduziu de 71 ± 8,6 mm para 65,3 ± 8,6 mm (p=0,049) e de 59,1 ± 8,5 mm para 50,4 ± 11,1 mm (p=0,002), respectivamente. Os diâmetros atriais variaram de 49,4 ± 6,4 mm para 44 ± 5,9 mm (p=0,017); o percentual de encurtamento sistólico do ventrículo esquerdo foi de 17 ± 4% para 24 ± 8,3% (p=0,014), a fração de ejeção variou de 34 ± 9% para 45 ± 14% (p=0,008), todos estatisticamente significativos. Onze (84,6%) pacientes se encontram em classe funcional I e II. A sobrevivência foi de 100%, 82,6%, 71,6%, respectivamente para 1, 6 e 12 meses após a cirurgia de seguimento, mantendo-se 71,6% em período superior a 36 meses. CONCLUSÃO: Os resultados obtidos, em pacientes com insuficiência cardíaca e insuficiência mitral moderada/grave submetidos a substituição valvar mitral com papilopexia cruzada e constrição anular, apresentaram evidências de remodelamento cardíaco favorável e significativa recuperação funcional ventricular esquerda.

OBJETIVOS: Avaliar a incidência, a mortalidade e os fatores de risco no desenvolvimento de insuficiência renal aguda (IRA) após cirurgia de revascularização miocárdica com o emprego de circulação extracorpórea (CEC), no serviço de Cirurgia Cardiovascular do Hospital Universitário da Universidade Federal de Mato Grosso do Sul, de janeiro de 2002 a novembro de 2004. MÉTODOS: Estudo retrospectivo de 74 pacientes submetidos à revascularização miocárdica com circulação extracorpórea, distribuídos em dois grupos segundo o desenvolvimento ou não de insuficiência renal aguda, diagnosticada por critérios laboratoriais. Foram realizadas análises estatísticas bivariada e multivariada, com significância de 5%. RESULTADOS: A incidência de IRA foi de 24,32%, sendo que 5,56% necessitaram de diálise, correspondendo a 1,35% do total de pacientes avaliados. A mortalidade por IRA foi de 5,56%. O uso de drogas inotrópicas ou vasoconstritoras no pós-operatório (p=0,048) e o IMC maior que 25 kg/m² (p=0,004) foram fatores determinantes para o desenvolvimento de IRA. O tempo de circulação extracorpórea foi baixo, não influenciando um aumento significativo de IRA pós-operatória neste estudo. CONCLUSÃO: O tempo de CEC não esteve associado a um aumento estatisticamente significativo da incidência de IRA pós-operatória na cirurgia de revascularização do miocárdio. Até pouco tempo atrás se acreditava que na isquemia mesentérica todas as alterações orgânicas desta afecção eram devidas à obstrução total ou parcial do fluxo arterial intestinal. Recentes descobertas quanto à fisiopatologia do processo de isquemia e reperfusão mesentérica demonstraram que os radicais livres, principalmente, atuam durante a reperfusão, levando à lesão tecidual muito mais importante do que as lesões que ocorrem na fase de isquemia isoladamente. Assim, surge uma nova possibilidade terapêutica além do tratamento cirúrgico, em que uma determinada substância poderia atuar de modo a inibir ou minimizar a cascata de alterações no nível celular que culminam na lesão e morte celular. Realizamos uma ampla revisão da literatura médica atual pelos bancos de dados LILACS e MEDLINE, visando verificar quais os fármacos estudados para este fim e os resultados obtidos nas pesquisas. Constatamos que inúmeras substâncias têm sido avaliadas em estudos experimentais, em sua maioria utilizando ratos e a maioria não apresentou resultados satisfatórios a ponto de permitirem seu emprego na prática clínica; algumas, entretanto, apresentaram resultados promissores, necessitando ainda de novos estudos a fim de se descobrir uma substância que possa ser empregada em seres humanos em situações de isquemia e reperfusão mesentérica, a fim de se evitar tratamentos intervencionistas com altos índices de morbi-mortalidade.A short time ago, it was believed that all of the organic alterations in the mesenteric ischemia were due to the total or partial obstruction of the intestinal arterial flow. As for the physiopathology of the ischemia process and mesenteric reperfusion, recent discoveries demonstrated that the free radicals, mainly, act during the reperfusion taking to the lesion much more important tecidual than the lesions that happen in the ischemia phase separately. So a new therapeutic possibility appears besides the surgical treatment when a certain substance could act in the way to inhibit or to minimize the cascade of alterations at the cellular level that culminate to the cellular lesion and death. We accomplished a wide revision of the current medical literature for the databases LILACS and MEDLINE looking to verify the studied drugs for this gol and the results obtained in the researches. We verified that countless substances have been appraised in experimental studies and most of them didn't show satisfactory results for their acceptance in practice clinic. Although some presented promising results, new studies are still needed to discover a substance that can be used in ischemia situations and mesenteric reperfusion in human beings to avoid interventionist treatments with high mortality rate.

OBJETIVO: Apresentar a tecnica de papilopexia cruzada e seus resultados iniciais na preservacao dos musculos papilares em operacoes de substituicao valvar mitral e remodelamento ventricular na insuficiencia cardiaca (ICC). METODO: Foram estudados dez pacientes submetidos a cirurgia de troca valvar mitral, sendo sete (70%) do sexo masculino, com idade variavel entre 15 e 75 anos (media de 44,4 ± 18,7 anos) portadores de disfuncao valvar mitral reumatica (50%), prolapso valvar mitral (10%) ou miocardiopatia dilatada e ICC (40%). Apos atriotomia e adequada exposicao da valva mitral, a cuspide anterior foi desinserida do anel e centralmente dividida, sendo cada metade, com seu complexo de cordas tendineas, fixada a comissura oposta por sua extremidade medial. Em seguida, foi implantada a protese valvar mecânica (sete casos) ou bioprotese porcina (tres casos) fixada ao anel valvar por meio de pontos separados, com reducao do anel valvar mitral, para melhor remodelamento ventricular nos casos com ICC. OBJETIVO: Avaliar o efeito, na contratilidade miocárdica e no fluxo coronariano, de drogas comumente utilizadas na prática clínica (diazepam, midazolam, propofol e etomidato). MÉTODO: Foram estudados 50 corações isolados de ratos Wistar divididos em cinco grupos de dez, em preparação de Langendorff com líquido de perfusão de Krebs-Henseleit (K-H), mantendo-se constantes a pressão de perfusão (90 centímetros de água) e a temperatura (37° + 0,5 graus Celsius). Com exceção do Grupo I (Controle), foram feitas infusões únicas, em um minuto, de diazepam (50 microgramas) - Grupo II; midazolam (25 microgramas) - Grupo III; propofol (25 e 50 microgramas) - Grupo IV e etomidato (25 microgramas) - Grupo V. Cada dose foi diluída e administrada em 0,1 mililitro de K-H, mantendo-se o fluxo e a pressão de perfusão coronarianos do sistema, durante sua infusão. Aferiu-se a freqüência cardíaca em batimentos por minuto (BPM), a tensão miocárdica em gramas (g), e o fluxo coronariano em mililitros por minuto (ml/min) em 1, 3, 5, 10, 15, 20, 25 e 30 minutos; a contratilidade miocárdica foi avaliada pelo cálculo da primeira derivada tensão/tempo (dT/dtmax), naquelas marcas. RESULTADOS: A freqüência cardíaca apresentou variações nos Grupos I, III e IV. Em relação à tensão miocárdica, apenas o Grupo I não sofreu declínio; o fluxo coronariano, exceto no Grupo IV, apresentou variações para menos, ao longo do estudo; a contratilidade miocárdica decresceu em todos os grupos estudados, exceto no Grupo I. CONCLUSÃO: As drogas ensaiadas diminuíram a contratilidade miocárdica (p0,05). OBJECTIVE: The effects on myocardial contractility (dT/dtmax) and coronary blood flow (CBF) of common drugs used in clinical practice (diazepam, midazolam, propofol and etomidate) were studied. METHOD: Fifty Wistar rat's hearts were divided into five groups of ten, and perfused by Langendorff method with Krebs-Henseleit solution (K-H), with the perfusion pressure stable at 90 cm H2O and temperature at 37.0+0.5 °C. With the exception of the Group I (control), they were submitted to single one-minute infusions of diazepam (50 micrograms) - Group II; midazolam (25 micrograms) - Group III; propofol (25 and 50 micrograms) - Group IV and etomidate (25 micrograms) - Group V. The drugs were diluted in 0.1 mL of K-H solution and the CBF rate and the perfusion pressure controlled during their infusion.The heart rate (beats per minute), myocardial tension (grams) and coronary blood flow (millimeters per minute) were measure at 1, 3, 5, 10, 15, 20, 25 and 30 minutes. The myocardial contractility was obtained by calculating the first derived tension/time (dT/dt max), at each time interval. RESULTS: The heart rate showed variations in Groups I, III and IV. A variation in the myocardial tension was seen in all groups except Group I and alterations in the CBF were seen in all groups, except in Group IV during the experiment.
