57. A Structured Approach to Detecting and Treating Depression in Primary Care: VitalSign6 Project

Abstract

PURPOSE This report describes outcomes of an ongoing quality-improvement project (VitalSign6) in a large US metropolitan area to improve recognition, treatment, and outcomes of depressed patients in 16 primary care clinics (6 charity clinics, 6 federally qualified health care centers, 2 private clinics serving lowincome populations, and 2 private clinics serving patients with either Medicare or private insurance). METHODS Inclusion in this retrospective analysis was restricted to the first 25,000 patients (aged >12 years) screened with the 2-item Patient Health Questionnaire (PHQ-2) in the aforementioned quality-improvement project. Further evaluations with self-reports and clinician assessments were recorded for those with positive screen (PHQ-2 [greater than or equal to]2). Data collected from August 2014 though November 2016 were available at 3 levels: (1) initial PHQ-2 (n = 25,000), (2) positive screen (n = 4,325), and (3) clinician-diagnosed depressive disorder with 18 or more weeks of enrollment (n = 2,160). RESULTS Overall, 17.3% (4,325/25,000) of patients screened positive for depression. Of positive screens, 56.1% (2,426/4,325) had clinician-diagnosed depressive disorder. Of those enrolled for 18 or more weeks, 64.8% were started on measurement-based pharmacotherapy and 8.9% referred externally. Of the 1,400 patients started on pharmacotherapy, 45.5%, 30.2%, 12.6%, and 11.6% had 0, 1, 2, and 3 or more follow-up visits, respectively. Remission rates were 20.3% (86/423), 31.6% (56/177), and 41.7% (68/163) for those with 1, 2, and 3 or more follow-up visits, respectively. Baseline characteristics associated with higher attrition were: non-white, positive drug-abuse screen, lower depression/anxiety symptom severity, and younger age. Abstract

Further research is needed to help improve both the standard of care and the outcome for patients with treatment-resistant depression. A particularly critical evidence gap exists with respect to whether pharmacological or non-pharmacological augmentation is superior to antidepressant switch, or vice-versa. The objective of this study was to compare the effectiveness of augmentation with aripiprazole or repetitive transcranial magnetic stimulation versus switching to the antidepressant venlafaxine XR (or duloxetine for those not eligible to receive venlafaxine) for treatment-resistant depression. In this multi-site, 8-week, randomized, open-label study, 278 subjects (196 females and 82 males, mean age 45.6 years (SD 15.3)) with treatment-resistant depression were assigned in a 1:1:1 fashion to treatment with either of these three interventions; 235 subjects completed the study. 260 randomized subjects with at least one post-baseline Montgomery-Asberg Depression Rating (MADRS) assessment were included in the analysis. Repetitive transcranial magnetic stimulation (score change (standard error (se)) = −17.39 (1.3) (p= 0.015) but not aripiprazole augmentation (score change (se) = −14.9 (1.1) (p= 0.069) was superior to switch (score change (se) = −13.22 (1.1)) on the MADRS. Aripiprazole (mean change (se) = −37.79 (2.9) (p= 0.003) but not repetitive transcranial magnetic stimulation augmentation (mean change (se) = −42.96 (3.6) (p= 0.031) was superior to switch (mean change (se) = −34.45 (3.0)) on the symptoms of depression questionnaire. Repetitive transcranial magnetic stimulation augmentation was shown to be more effective than switching antidepressants in treatment-resistant depression on the study primary measure. In light of these findings, clinicians should consider repetitive transcranial magnetic stimulation augmentation early-on for treatment-resistant depression.

86. Pandemic precarity and the complicated case of Maharashtra: Interrogating state capacity and its fault lines

Abstract

The state of Maharashtra and the city of Mumbai have been referred to as the epidemic epicentre of India since the time of the plague of 1896 and influenza epidemic of 1918. During the COVID-19 pandemic too, the state experienced the highest cases with maximum fatalities. Against the structural backdrop of India's underfinanced public health infrastructure, this paper engages with the subnational state's capacity, legitimacy, and authority in managing the pandemic. The paper highlights the socio-economic and spatial-demographic context to explain the reasons behind the severity of the pandemic in Maharashtra. Through an analysis of regulatory information at different levels during crucial phases of the pandemic, and statistics about tests, caseloads, medical resources (such as hospital beds, essential drugs, and oxygen), migrants' welfare, excess deaths, vaccines, etc., the paper explores how knowledge about the unfolding crisis and the capacity to respond kept changing as the pandemic progressed. The emphasis is on demonstrating how the state improved its response, from a situation of confusion to an augmentation of infrastructure and a coordinated response. Despite fiscal constraints and centre-state tensions, the reliance on rational bureaucracy, scientific knowledge, and the decentralization of the response helped to develop several models and good practices that were widely acknowledged. The paper emphasizes that the case of Maharashtra underscores the importance of decentralized planning and recognizes the value of collaborative federalism.

Abstract

The construction of migrant population across India–Bangladesh borders is premised on cultural affiliation, religious sentiments and the contingent political economy. With a history of partition in different phases, the subjective conceptions of identity of migrants are layered and complex. The article unravels how identities of migrants are shaped in everyday life through the frame of legality–illegality, religious–political and economic–social aspects. Drawing on previous research and empirical engagement, the article engages with the questions on citizenship, residency, identity, belonging, exclusion and inclusion. The field work in the borderland district of North 24 Parganas provides rich description about the life and circumstances of migrants at the threshold of security and insecurity, belonging and unbelonging around layers of caste and communal tangle. The article presents a grounded understanding on the politics of documenting and phenomenon of maintaining undocumentedness. To explain the social construction of identity, the article explores the role of Hindu nationalist ideas that influences the negotiation of migrant populations around religious lines; either accepted, ignored, patronized or kept insecure, susceptible to fear and exclusion.
