Rationale and Objectives Accurate assessment of air density used to quantitatively characterize amount and distribution of emphysema in chronic obstructive pulmonary disease (COPD) subjects has remained challenging. Hounsfield units (HU) within tracheal air can be considerably less negative than –1000 HU. This study has sought to characterize the effects of improved scatter correction used in dual-source pulmonary computed tomography (CT). Materials and Methods Dual-source dual-energy (DSDE) and single-source (SS) scans taken at multiple energy levels and scan settings were acquired for quantitative comparison using anesthetized ovine (n = 6), swine (n = 13), and a lung phantom. Data were evaluated for the lung, inferior vena cava, and tracheal segments. To minimize the effect of cross-scatter, the phantom scans in the DSDE mode were obtained by reducing the current of one of the tubes to near zero. Results A significant shift in mean HU values in the tracheal regions of animals and the phantom is observed, with values consistently closer to −1000 HU in DSDE mode. HU values associated with SS mode demonstrated a positive shift of up to 32 HU. In vivo tracheal air measurements demonstrated considerable variability with SS scanning, whereas these values were more consistent with DSDE imaging. Scatter effects in the lung parenchyma differed from adjacent tracheal measures. Conclusion Data suggest that the scatter correction introduced into the dual-energy mode of imaging has served to provide more accurate CT lung density measures sought to quantitatively assess the presence and distribution of emphysema in COPD subjects. Data further suggest that CT images, acquired without adequate scatter correction, cannot be corrected by linear algorithms given the variability in tracheal air HU values and the independent scatter effects on lung parenchyma.

Polyethylene glycol-coated liposomal blood pool contrast agents maintain contrast enhancement over several hours. This study aimed to evaluate (long-term) imaging of pulmonary arteries, comparing conventional iodinated contrast with a liposomal blood pool contrast agent. Also, visualization of the (real-time) therapeutic effects of tissue plasminogen activator (t-PA) on pulmonary embolism (PE) was attempted.Six rabbits (weight approximately 4 kg) had autologous blood clots injected through the superior vena cava. Imaging was performed using conventional contrast (iohexol, 350 mg I/ml; GE HealthCare, Princeton, NJ) at a dose of 1400 mg I per animal, and after wash-out, animals were imaged using an iodinated liposomal blood pool agent (88 mg I/mL, dose 900 mg I/animal). Subsequently, five animals were injected with 2 mg of t-PA and imaging continued for up to 4(1/2) hours.Both contrast agents identified PE in the pulmonary trunk and main pulmonary arteries in all rabbits. Liposomal blood pool agent yielded uniform enhancement, which remained relatively constant throughout the experiments. Conventional agents exhibited nonuniform opacification and rapid clearance postinjection. Three of six rabbits had mistimed bolus injections, requiring repeat injections. Following t-PA, pulmonary embolus volume (central to segmental) decreased in four of five treated rabbits (range 10-57%, mean 42%). One animal showed no response to t-PA.Liposomal blood pool agents effectively identified acute PE without need for reinjection. PE resolution following t-PA was quantifiable over several hours. Blood pool agents offer the potential for repeated imaging procedures without need for repeated (nephrotoxic) contrast injections.

The study's aim is to establish lung-imaging methods that provide for the ability to image the lung under dynamic non-breath hold conditions while providing "virtual breath hold" quantifiable volumetric image data sets. Static breath hold images are used as the gold standard for evaluating these virtual breath hold images in both a phantom and sheep.Axial methods for gating image acquisition to multiple points in the respiratory cycle interleaved with incremental table stepping during multidetector-row computed tomographic (MDCT) scanning were developed. Data sets are generated over multiple breaths, providing volume images representative of multiple points within a respiratory cycle. To determine the reproducibility and accuracy of the methods, six anesthetized sheep were studied by means of MDCT in nongated and airway-pressure (P(awy))-gated modes in which P(awy) was 0, 7, and 15 cm H2O.No significant differences were found between coefficients of variation in air volume measured from repeated static scans (1.74% +/- 1.78%), gated scans: inspiratory (1.2% +/- 0.44%) or expiratory gated (1.39% +/- 0.98%), or between static (1.74% +/- 1.78%) and gated (1.39% +/- 0.98%) scanning at similar P(awy) (P.1). Measured air volumes were larger from static versus gated scans by 5.85% +/- 3.77% at 7 cm H2O and 4.45% +/- 3.6% at 15 cm H2O of P(awy) (P.05), consistent with hysteresis. Differences between air volumes at 7 and 15 cm H2O measured from either static or gated scans or that delivered by a super syringe were insignificant (P.05). Visual accuracy of three-dimensional anatomic geometry was achieved, and landmark certainty was within 1 mm across respiratory cycles.A method has been shown that provides for accurate gating to respiratory signals during axial scanning. High-resolution volumetric image data sets are achievable while the scanned subject is breathing. Images are quantitatively similar to breath hold images, with differences likely explained by known pressure-volume hysteresis effects.

Purpose: Assessing pulmonary emphysema using Quantitative CT of the lung depends on accurate measures of CT density. Sinogram-Affirmed-Iterative-Reconstruction (SAFIRE) is a new approach for reconstructing CT data acquired at significantly lower doses. However, quantitative effects of this method remain unexplored. The authors investigated the effects on the median values of materials in the COPDGene2 test-object as a function of the reconstruction method [weighted filtered back projection (WFBP) versus SAFIRE], test-object size, dose, and material composition using a Siemens SOMATOM Definition FLASH CT scanner. Methods: The COPDGene2 test-object contains eight materials; acrylic, water, four foams (20 lb, 12 lb, lung-equivalent, and 4 lb emphysema-equivalent), internal and external-air. The test-object was scanned with three different outer ring sizes, simulating three different body habitus. There is an average size (36 cm) Ring A, large size (40 cm) Ring B, and small size Ring C (30 cm). The CT protocol used 120 kVp, 0.5 s rotation, 1.0 pitch, and a 0.6 slice collimation with progressively decreasing x-ray exposure values, 11.94–0.74 mGy. With a thorax length of 30 cm, the corresponding effective doses would be 5.01–0.31 mSv. The effects of using SAFIRE versus WFBP were assessed using a two tailed t-test for each ring size, material, and dose. Multivariable linear regression was used to evaluate the relative effects of ring size, material composition, dose, and reconstruction method on the measured median value in HU. Results: SAFIRE versus WFBP, at the largest ring size and two lowest doses there was a significant difference in median values of 4 lb-foam, p < 0.01. Using the smallest ring size at the lowest dose level there was a significant difference in the median value of 4 lb-foam, but the effect size was small, 1 HU. There is a significant difference in median values of both internal and external air using both the small and medium size rings at the three lowest dose levels, p < 0.05. There are significant differences noted at both high and low dose levels when using the large ring size in the median values of internal and external air when, p < 0.05. These effects on 4 lb-foam, inside and outside air are shown to be in part due to truncation effects on the median value since the lowest HU value in the CT scale used is −1024 HU. Multivariable linear regression results demonstrated significant effects on the measured material median value and standard deviation due to ring size, material composition, dose level, and reconstruction method, p < 0.05. Conclusions: The authors have shown that there is no significant effect on the median values obtained when using WFBP versus SAFIRE in materials with CT density between 120 and −856 HU using three different test-object sizes and CT doses that vary from 11.94 to 0.74 mGy. The authors have demonstrated there are significant effects on median values obtained when using WFBP versus SAFIRE in materials with CT density values between −937 and −1000 HU depending on the ring size and dose used. As expected, there is considerable reduction in image noise (lower standard deviation) using SAFIRE versus WFBP with all ring sizes, doses, and materials in the COPDGene2 test-object.
