51. Mandibular Growth Following Implant Restoration: Does Wolff's Law Apply to Residual Ridge Resorption?

Abstract

This study measured bone height under a fixed detachable cantilever restoration supported by five or six endosseous implants in 60 consecutively treated patients. Panoramic films were made at surgery and 1, 2, 3, and 4 years postrestoration. A computer-enhanced method was used to measure mandibular height 5, 10, 15, and 20 mm distal to the last implant, which was used as a length standard to correct for variation in film magnification. Implant restoration resulted in a significant growth of the mandible (baseline: 7.25 ± 0.25 mm, 4 years: 8.18 ± 0.18 mm; P = .05). Abstract

A retrospective quantitative radiographic analysis determined the effect of graft material and smoking status on the maintenance of graft height over 3 years. Analysis of variance models with planned comparison were constructed to compare mean graft change by (1) graft material and (2) smoking status. Maintenance of bone height was significantly greater in intraoral autogenous grafts versus allografts (P < .05). The effect of smoking on implant loss revealed a significant difference in implant survival (P < .05). Autogenous bone generally resulted in a more favorable outcome over a 3-year period. Abstract

This study evaluated the use of a bioresorbable membrane (Guidor USA) for treatment of periodontal angular bone detects in eight periodontitis patients. Presurgical measurements consisted of probing depth, clinical attachment levels, and digital radiographic imaging evaluation. Full-thickness buccal and lingual flaps were elevated allowing for soft tissue debridement, scaling, and root planing. Bioresorbable membranes were placed in at least one of the periodontal detects, and controls were left without barrier placement. Clinical and radiographic results 6 months after the surgical procedure showed clinical and radiographic improvement for control and test lesions. However, sites treated with bioresorbable membranes presented a significant gain in clinical attachment and bone mass when compared to sites that had received flap surgery alone. Abstract

The use of guided bone regeneration to fill large bone voids in the mandible created through en bloc resection and chronic granulation in three primates was examined. In each of three animals, en bloc resections of implants and surrounding bone were done bilaterally (10 x 8 mm and 16 x 8 mm). Tissues were allowed to heal in a chronic state (for 6 months). A reinforced expanded polytetrafluorethylene (e-PTFE) membrane was then surgically placed over each defect site to be followed for 1 year postsurgically. Two of the six membranes became infected during the first month and were removed. The remaining four membranes were removed en bloc after 1 year. Bone regeneration (greater than 90% of the defect) was observed clinically, and subtraction radiology demonstrated bone gain of between 544 and 733 mg. Histomorphometry after fluorescent labeling demonstrated a mean growth rate of 2.77 µm/wk for all sites measured, and a decrease in the number of osteons from the new bone at the crest to the mature bone at the base of the original defect. This study gives evidence that substantial quantities of new bone can be produced in atrophic mandibles and that this regenerated bone exhibits normal maturation dynamics. [ABSTRACT FROM AUTHOR]
