Background: Non-alcoholic fatty liver disease (NAFLD), non-alcoholic steatohepatitis (NASH), and hepatic fibrosis have emerged as one of the leading causes of chronic liver disease. The prevalence of the NAFLD spectrum has increased, which can be attributed to the rise in obesity. As NAFLD can ultimately lead to liver cirrhosis, it is imperative to identify modifiable risk factors associated with its onset and progression to provide timely intervention to prevent potentially disastrous consequences. Considering the pivotal role of the endocrine axis in several metabolic pathways such as obesity and insulin resistance, thyroid hormones are crucial in the pathophysiology of NAFLD. The study is focused on the identification of an association between thyroid function and radiographic and histological parameters of NAFLD in patients with severe obesity. Methods: Ninety patients were recruited for this study and underwent initial assessments, including demographic profiles, anthropometric measurements, hepatic biopsy, and basic laboratory tests. Liver stiffness was evaluated using two-dimensional shear wave elastography (2D-SWE) at least 2 weeks before liver biopsy. Results: Among the 90 participants, 80% were women. The mean age was 38.5±11.1 years, and the mean body mass index (BMI) was 45.46±6.26 kg/m2 . The mean levels of serum T3 and free T4 in patients with positive histology were not statistically significant compared with patients with negative histology. Furthermore, there was no statistical significance in the mean T3 and free T4 levels between patients diagnosed with hepatic steatosis or fibrosis (on ultrasonography and elastography) and those with negative hepatic imaging. Serum levels of thyroid-stimulating hormone (TSH) were negatively correlated with ultrasonography (P=0.007). Binary logistic regression analysis revealed that none of the thyroid hormones was a predictive factor for liver histology in both adjusted and crude models. Conclusion: The results from our analysis did not suggest an association between thyroid hormones and NAFLD, which is in line with several previously published studies. However, the authors note that there are published data that do propose a link between the two entities. Therefore, well-designed large-scale clinical studies are required to clarify this discrepancy.

Background: Oral candidiasis is one of the most common infections in diabetic patients that may occur due to a decrease in salivary flow rate, alterations in the salivary composition, or both. This study aimed to investigate the effect of diabetic and non-diabetic saliva on Candida albicans adherence to the acrylic resin disc specimens, regardless of saliva volume. Methods: In this case-control study, the population consisted of 26 subjects in 2 groups (13 diabetic patients and 13 non-diabetic patients). In both groups, unstimulated whole saliva was collected. It was investigated for pH, salivary flow rate, and adhesion of C. albicans to polymethylmethacrylate disc. After preparing the polymethylmethacrylate discs, the samples were stored in diabetic saliva, non-diabetic saliva, and distilled water (negative control group) for 60 minutes at 37ºC. Then they were immersed in the yeast suspensions containing C. albicans and stained with gram stain. Yeast cells were counted using a light microscope. Results: The pH of unstimulated whole saliva in diabetic patients was significantly lower than in the control group. There was no significant difference between the flow rates of saliva in the two groups. Findings indicated that the adhesion of C. albicans to resin in diabetic saliva was higher than in non-diabetic saliva. Conclusion: This study showed the same amount of salivary flow rate in diabetic and non-diabetic individuals, lower pH, and higher Candida adherence to heat polymerization acrylic resin in the diabetic group.

Purpose: This study aimed to ascertain the diagnostic accuracy of non-invasive liver function tests in liver fibrosis and assess their changes after metabolic surgery. Materials and Methods: 1005 individuals with severe obesity who were referred for metabolic surgery were analysed. All participants had blood samples taken for liver enzymes and lipid profile. In addition, hepatic indexes, including AAR, APRI, NFS and Fibrosis-4 (FIB4), were checked. Furthermore, all participants underwent two-dimensional shear wave elastography (2D-SWE). All investigations were repeated 6–8 months after metabolic surgery. The receiver operating characteristic (ROC) curve and the area under the ROC curve was utilised to determine the optimal cut-off values for baseline study parameters. Logistic regression was applied to predict the relationship between study parameters—as predictors—and change in 2D-SWE. Results: AST/ALT (AAR) was the most sensitive (79%) pre-operative non-invasive serological marker for detecting liver fibrosis, whereas NAFLD Fibrosis Score (NFS) was the most specific (84%). AST/upper limit of the normal AST range × 100/platelets (× 109/L) (APRI) showed a positive correlation with 2D-SWE post-metabolic surgery (p-value = 0.021). Regression analysis from both adjusted and unadjusted models showed that baseline AAR was a predictor of postoperative liver status in terms of hepatic fibrosis. Conclusion: AAR has a high sensitivity, whereas NFS exhibits a high specificity in diagnosing liver fibrosis. The authors recommend using both investigations in conjunction with 2D-SWE to increase the likelihood of detecting liver fibrosis. Background: Aluminum phosphide (AlP), also known as “rice tablet,” is widely used as an effective pesticide. However, AlP poisoning is a common cause of mortality in many countries, such as Iran. Unfortunately, there is no specific antidote for AlP toxicity to date. AlP releases phosphine gas when it is exposed to moisture and acid. Phosphine is a robust mitochondrial toxin that could significantly inhibit cellular energy metabolism. AlP poisoning is an emergency condition that needs instant and effective intervention. Dihydroxyacetone (DHA) is a simple saccharide used for several pharmacological as well as cosmetic purposes. Previously, we found that DHA could significantly prevent mitochondrial impairment induced by toxic agents such as cyanide and phosphine in various in vitro and in vivo experimental models.Methods: Hospitalized patients (n = 111) were evaluated for eligibility criteria. Among these patients, n = 35 cases were excluded due to incomplete data (n = 11) and suspicion of poisoning rather than AlP (n = 24). Meanwhile, n = 76 cases with confirmed AlP poisoning were included in the study. AlP-poisoned patients who did not receive DHA (n =18) were used as the control group.Patients (n = 58) received at least one dose of DHA (500 ml of 5% DHA solution w/v, i.v.) in addition to routine treatment of AlP poisoning. Arterial blood gas (ABG), blood pH, bicarbonate levels, and other vital signs and biochemical measurements were monitored. Moreover, the mortality rate and time of hospitalization were evaluated in both DHA-treated (n = 58) and AlP-poisoned patients without DHA administration (n = 18). Several biomarkers were assessed before (upon hospitalization) and after DHA treatment. The routine tests for AlP-poisoned patients in this study were the measurement of electrolytes (K+ and Na+), WBC, RBC, hemoglobin, INR, carbonate (HCO3), blood pH, PaCO2, and PaO2 and SGPT, SGOT, BUN, Cr. Results: Upon patients' admission, significant decreases in blood pH (acidosis), blood PaO2, and HCO3 levels were the hallmarks of AlP poisoning. It was found that DHA significantly alleviated biomarkers of AlP poisoning and tremendously enhanced patients’ survival rate compared to patients treated based on hospital routine AlP poisoning protocols (no DHA). No significant adverse effects were evident in DHA-treated patients in the current study.Interpretation: These data suggest that parenteral DHA is a novel and effective antidote against AlP poisoning.Trial registration: IR.SUMS.REC.1394.102

Introduction: Sulfate is one of the important groundwater pollutant sources in many parts of the world and it can enter into groundwater from various sources, such as lithology (dissolution of evaporative and pyrite oxidation), atmosphere (sea water spray), industrial (combustion of fossil fuels, sulfide- minerals production, and agricultural fertilizers), and etc. Identifying sources of soluble sulfate in water sources is important. The sulfate in groundwater can be investigated using isotope and geochemistry techniques. Using isotope (34S and 18O) and hydro geochemical techniques, it can be possible to find out: sulfate origins and the effective processes/reactions on sulfate concentrations and hydrogen sulfide gas (H2S) production. In this paper, for the first time, the sulfate source in groundwater of Sarpol Zahab and the parameters affecting sulfate concentration and its isotopic compositions (d34S and d18O) in groundwater were studied. Sarpol-e Zahab is located in the catchment area of Alvand river in the west of Kermanshah province, west of Iran. The formations of the region, based on age from old to new, include the Ilam, Gurpi, Amiran, Telezang, Pabdeh, Asmari, Gachsaran, Aghajari, Bakhtiari and Quaternary alluvium formations. Asmari formation and Quaternary alluvial sediments form the largest area of the region. Gachsaran formation contains evaporative materials which is reducing the quality of groundwater in the region. Ilam formation is effective in providing the organic matter required for the bacterial sulfate reduction process. Materials and Methods: 13 water samples were taken from the water resources (wells, springs and river) of Sarpol-e Zahab region in two steps (December 2014 and September 2015). Measuring field parameters (T, pH, Eh, Ec, and TDS) and sampling of water resources were performed according to the instructions of Groundwater and Geothermal Research Center (GRC), Ferdowsi University of Mashhad. Field parameters were measured by VWR Handheld Multi parameter Research meter at the location of each water source. The concentration of cations and the anions were determined by the devices inductively coupled plasma elemental analyzer (ICP-EA) and ion chromatography (IC), respectively. Chemical and isotopic analyses of all water samples were performed in Ottawa university geochemistry laboratory and Waterloo university isotope laboratory, respectively. The measurement reference for isotopic sulfate and oxygen were VCDT and VSMOW, respectively, and the value isotopic are expressed as permil ((‰. Results and Discussion: The sulfate concentrations in different water resources of the region varied from 5 to 950 mg L, however, it is very high in Gandab spring’ water, due to the association with hydrocarbons, and in Patagh Tunnel water, due to discharging of water from Gachsaran Formation (339.6 mg L and 950.1mg L, respectively). Chenarpiran spring has the lowest sulfate concentration because it is located in highlands and is discharged from Asmari formation with good water quality. The amount of d18O varied between 5.8 to 13.1 VCDT ‰ and the amount of d34S ranges from 9.5 to 31.8 VSMOW ‰. In Gandab spring’ water, due to sulfate reduction by microorganisms, in addition to the unpleasant smell of sulfide hydrogen (H2S), the d34S and d18O values were enriched than the isotopic composition of other water sources ( about 31.8 ‰ and 10.3 ‰, respectively). In other hand, the Gel va Darreh spring's water, due to the effect of upstream bath waste water (soap and detergent with a value of about 1 ‰ for d34S), indicated depleted isotope values. Conclusion: The concentration and isotopic composition of sulfate in the water resources of the Sarpol-e Zahab region are affected by the geological, atmospheric, human and hydrocarbon materials. The impact of human and hydrocarbon factors on the concentration and isotopes of sulfate is local, however, the lithology effects are on all water resources of the region. Since, Gachsaran formation forms the alluvial aquifer bedrock of Ghaleshahin plain, the dissolution of gypsum has an effective role in the hydro chemical evolution of the alluvial aquifer of this plain. The lithology of the area is often limestone and evaporation. Due to the high solubility of these materials in water, they have been able to have a significant effect on sulfate isotopic in water sources. The amount of measured isotopic values indicated that the origin of evaporators and atmospheric sulfate is in water. The dissolution of the evaporation formation has reduced the water quality. The association of bedrock in the Gandab spring has led to have a reduction environment and the occurrence of sulfate reduction and production of hydrogen sulfide gas (H2S). The human activities (by making bath upstream) caused oxidizing conditions, isotopic depletion and contamination of the Glodarreh spring.

Decreased food intake is an effective mechanism for gastric bypass surgery (GBS) for successful weight loss. This cross-sectional study aimed to assess dietary intake, micro-and macro-nutrients in the patients undergoing GBS and determine the possible associations with weight changes. We assessed anthropometric indices and food intake at 24 month-post gastric bypass surgery. Dietary data was evaluated using three-day food records. After the 24 months of surgery, among 35 patients (mean age: 43.5 ± 11.2 years; 82.85% females), with the mean body mass index (BMI) of 30.5 ± 4.5 kg/m2, 17 cases were < 50% of their excess weight. The average daily calorie intake was 1,733 ± 630 kcal, with 14.88% of calories from protein. Consumption amounts of protein (0.82 ± 0.27 g/kg of the current weight), as well as fiber, and some micro-nutrients (vitamin B9, E, K, B5, and D3) were lower than recommended amounts. Patients were classified into three groups based on their success in weight loss after surgery. Calorie intake was not significantly different between groups, but successful groups consumed considerably more protein and less carbohydrate than the unsuccessful group (p < 0.05). Based on our findings, the patients undergoing GBS had inadequate macro- and micro-nutrient intake after 24 months. However, protein intake can affect patients' success in achieving better weight loss. Long-term cohort and clinical studies need to be conducted to comprehend this process further.

The aim of this study was to evaluate the performance of various radiobiological models in predicting the occurrence of acute esophagitis (AE) during radiation therapy (RT) of head, neck, and thoracic tumors with concurrent and sequential chemotherapy. According to recent studies, the probability of AE following RT by normal tissue complication probability models is predictable.A total of 100 patients with nasopharynx, larynx, Hodgkin's lymphoma, spinal metastases, and oral cavity and lung tumors were included in the study. Half of these patients were treated by concurrent chemo-radiotherapy (Con. CRT) and the other half were treated by radiotherapy alone or sequential chemo-radiotherapy (RT + seq. CRT). Radiobiological models of several types were used as follows,: Lyman-generalized equivalent uniform dose (gEUD), Lyman-MED, log-logistic, logit, and logistic. Parameters were estimated using maximum likelihood estimation, and models were compared using Akaike information criteria.Based on follow-up data, the behavior of dose-response curves differed markedly between the Con. CRT and RT + seq. CRT groups. The best fit with clinical results was offered by the Lyman-MED model for the Con. CRT group and the Lyman-gEUD model for the RT + seq. CRT group. Depending on the model used, the parameter of DThe incidence of AE significantly differed between the two treatment groups in all the models. New parameter estimates could be used for predicting the probability of acute esophagitis after chemo-RT.

BACKGROUND: In recent years, severe electromagnetic interference among electronic devices has been caused by the unprecedented growth of communication systems. Therefore, microwave absorbing materials are required to relieve these problems by absorbing the unwanted microwave. In the design of microwave absorbers, magnetic nanomaterials have to be used as fine particles dispersed in an insulating matrix. Besides the intrinsic properties of these materials, the structure and morphology are also crucial to the microwave absorption performance of the composite. In this study, Ni-Co-MWCNT composites were synthesized, and the changes in electric permittivity, magnetic permeability, and reflectance loss of the samples were evaluated at frequencies of 2 to 18 GHz. METHODS: Nickel-Cobalt-Multi Wall Carbon Nanotubes (MWCNT) composites were successfully synthesized by the co-precipitation chemical method. The structural, morphological, and magnetic properties of the samples were characterized and investigated by X-ray diffractometer (XRD), Scanning Electron Microscopy (SEM), Transmission Electron Microscopy (TEM), Vibrating Sample Magnetometer (VSM), and Vector Network Analyzer (VNA). RESULTS: The results revealed that the Ni-Co-MWCNT composite has the highest electromagnetic wave absorption rate with a reflectance loss of -70.22 dB at a frequency of 10.12 GHz with a thickness of 1.8 mm. The adequate absorption bandwidth (RL CONCLUSION: Based on this study, it can be argued that the Ni-Co-MWCNT composite can be a good candidate for making light absorbers of radar waves at frequencies 2- 18 GHz.

Au-Kline syndrome (AKS) is a neurodevelopmental disorder associated with multiple malformations and a characteristic facial gestalt. The first individuals ascertained carried de novo loss-of-function (LoF) variants in HNRNPK. Here, we report 32 individuals with AKS (26 previously unpublished), including 13 with de novo missense variants. We propose new clinical diagnostic criteria for AKS that differentiate it from the clinically overlapping Kabuki syndrome and describe a significant phenotypic expansion to include individuals with missense variants who present with subtle facial features and few or no malformations. Many gene-specific DNA methylation (DNAm) signatures have been identified for neurodevelopmental syndromes. Because HNRNPK has roles in chromatin and epigenetic regulation, we hypothesized that pathogenic variants in HNRNPK may be associated with a specific DNAm signature. Here, we report a unique DNAm signature for AKS due to LoF HNRNPK variants, distinct from controls and Kabuki syndrome. This DNAm signature is also identified in some individuals with de novo HNRNPK missense variants, confirming their pathogenicity and the phenotypic expansion of AKS to include more subtle phenotypes. Furthermore, we report that some individuals with missense variants have an "intermediate" DNAm signature that parallels their milder clinical presentation, suggesting the presence of an epi-genotype phenotype correlation. In summary, the AKS DNAm signature may help elucidate the underlying pathophysiology of AKS. The hydroxyapatite layer was deposited on the commercial magnesium alloy of AZ91 by electrophoretic deposition route, and the corrosion behavior of applied layers was studied by polarization and electrochemical impedance spectroscopy at the Simulated Body Fluid (SBF) solution. The best corrosion resistance improvement was obtained for the sample synthesized at 40 V within 4 minutes. Also, the morphology of coated samples was studied by atomic force microscopy (AFM) and the surface parameters were measured. It could be concluded that the calculated values for surface parameters including surface roughness, maximum peak height, maximum pit depth, and maximum peak have a meaningful relationship with corrosion resistance.

Introduction: The pituitary gland is frequently irradiated during radiation therapy of head and neck tumors which can influence the quality of life of the patients after radiation therapy. This study aimed to estimate the normal tissue complication probability (NTCP) for the pituitary gland in head and neck cancers using two radiobiological models. Material and Methods: 53 patients including 20 cases with nasopharyngeal cancer and 33 cases with brain tumor were studied. The dosimetric properties of each plan including minimum, mean and maximum doses were extracted from the dose-volume histogram curve. For estimation of pituitary gland response for each patient, the BIOPLAN software was used to calculate NTCP by LKB model and Matlab software was applied to calculate NTCP by equivalent uniform dose (EUD) model. Models’ parameters including TD50, , and‘a’ were extracted from a previous study of radiobiological modeling of pituitary gland response to radiation therapy. For statistical analysis, the T-test was used to compare two models. Results: The average mean doses of 30.42 and 51.29 (Gy) of the pituitary gland were obtained for nasopharyngeal and brain tumor patients, respectively. The average NTCPs of the pituitary gland for nasopharyngeal patients estimated by LKB and Log-logistic models were 3.84 and 3.91%, respectively. In brain tumors, the average NTCP was 16.33% for LKB and 16.41% for Log-logistic models. The results showed that the log-logistic and LKB models provided comparable results and no statistically significant difference (P-value< 0.05) was found between two models. Conclusion: The NTCP results indicated that the average NTCP of the pituitary gland for nasopharyngeal patients was approximately four times lower than that of brain tumors. Finally, implementation of follow up studies and modeling investigations are recommended for accurate estimation of pituitary gland complications following radiation therapy.

Multiple sclerosis (MS) is an autoimmune demyelinating disease that leads to axon degeneration as the major cause of everlasting neurological disability. The cis-phosphorylated tau (cis-p-tau) is an isoform of tau phosphorylated on threonine 231 and causes tau fails to bind micro-tubules and promotes assembly. It gains toxic function and forms tangles in the cell which finally leads to cell death. An antibody raised against cis- p-tau (cis mAb) detects this isoform and induces its clearance. Here, we investigated the formation of cis-p-tau in a lysophosphatidylcholine (LPC)-induced prolonged demyelination model as well as the beneficial effects of its clearance using cis mAb. Cis -p-tau was increased in the lesion site, especially in axons and microglia. Behavioral and functional studies were performed using visual cliff test, visual placing test, and visual evoked potential recording. Cis-p-tau clearance resulted in decreased gliosis, protected myelin and reduced axon degeneration. Analysis of behavioral and electrophysiological data showed that clearance of cis-p-tau by cis mAb treatment improved the visual acuity along with the integrity of the optic pathway. Our results highlight the opportunity of using cis mAb as a new therapy for protecting myelin and axons in patients suffering from MS.
