Nutritional status, nutrient density, and dietary diversity of children under five during the COVID-19 pandemic in Bangli Regency, Bali Background: Stunting in children caused the lack of nutritionally balanced food intake in the first two years of life. During the COVID-19 pandemic, the fulfillment of balanced nutrition for children must still be considered to maintain body resistance to avoid COVID-19 infection. Objective: This study was conducted to find out the nutritional status of children and also the knowledge and attitudes of mothers regarding the practice of balanced nutrition based on local food in the COVID-19 pandemic situation. Methods: Cross-sectional study with 202 children aged 23-59 months located in Susut District, Bangli Regency, Bali Province. Data collection includes anthropometric measurements and food recall. Characteristics, knowledge, and attitudes of mothers were conducted by interview using a questionnaire. Results: The nutritional status of subjects showed that the average weight for height z-score (WHZ) was 0.29 ± 1.37, height for age z-score (HAZ) was 0.56 ± 2.38, and weight for age z-score (WAZ) which is -1.18 ± 2.03. The average density of energy intake in the adequate (1.66±0.456), as well as protein density (4.12±0.884 g/100 calories), vitamin B12 (0.225±0.457 mg/100 kcal), and vitamin A (71.7 ±104.6 mg/100 kcal). The majority (61.9%) of subjects consumed less than five food groups in the last 24 hours. The average knowledge of respondents is 69.5±15.3, which is 35.1% in well nutrition knowledge, while most respondent’s attitude has in the good category. Conclusions: Overall children are in normal nutritional status, but it is necessary to attention to the density of vitamins and minerals which are still low, and the lack of food diversity. Suggestion to increase education on balanced nutrition practices based on local food to mothers and families so that they can help grow and also maintain children's health during the COVID-19 pandemic.

Pendahuluan: Periodontitis apikalis adalah peradangan dan kerusakan jaringan pada daerah apikal jaringan periodonsium. Terjadi akibat pulpa nekrosis mengalami perluasan infeksi dari bakteri saluran akar menuju apeks gigi. Penelitian terdahulu membuktikan bahwa periodontitis apikalis dapat dikaitkan dengan peningkatan kadar interleukin-6. Interleukin-6 adalah salah satu sitokin pro-inflamatori yang mempunyai peran penting dalam respon inflamasi. Tujuan penelitian menganalisis perbedaan kadar interleukin-6 dalam darah vena antara pasien dengan periodontitis apikalis dan pasien tanpa periodontitis apikalis. Metode: Jenis penelitian observasional analitik dengan dua puluh sampel stok darah vena yang terdiri atas 10 sampel darah vena pasien dengan periodontitis apikalis dan 10 sampel darah vena pasien tanpa periodontitis apikalis. Serum darah yang telah dipisahkan dengan metode sentrifugasi dipakai sebagai sampel uji enzyme-linked immunosorbent assay (ELISA) sandwich untuk mengukur kadar IL-6 dengan membaca nilai absorbansi dan kurva standar. Data yang diperoleh dianalisis menggunakan uji Shapiro-Wilk, dilanjutkan dengan uji T tidak berpasangan. Hasil: Kadar IL-6 teridentifikasi pada semua sampel. Sampel darah vena pasien tanpa periodontitis apikalis memiliki kadar IL-6 berkisar antara 4,7-18,74 mg/L, sedangkan kadar IL-6 pada pasien dengan periodontitis apikalis 4,0-90,75 mg/L. Uji T tidak berpasangan menunjukan bahwa terdapat perbedaan yang bermakna (p=0.02). Simpulan: Kadar IL-6 pada darah vena dengan periodontitis apikalis lebih tinggi dibandingkan kadar IL-6 pada tanpa periodontitis apikalis. Occurs due to pulp necrosis experiencing an expansion of infection from root canal bacteria to the apex of the tooth. Previous studies have shown that apical periodontitis can be associated with elevated levels of interleukin-6. Interleukin-6 is a pro-inflammatory cytokine that has a vital role in the inflammatory response. The study aimed to analyze differences in interleukin-6 levels in venous blood between patients with apical periodontitis and patients without apical periodontitis. Methods: This research was an analytic observational study with twenty venous blood samples consisting of 10 venous blood samples from patients with apical periodontitis and ten venous blood samples from patients without apical periodontitis. Blood serum that has been separated by centrifugation method was used as a sandwich enzyme-linked immunosorbent assay (ELISA) test sample to measure IL-6 levels by reading absorbance values and standard curves. The data obtained were analyzed using the Shapiro-Wilk test, followed by an unpaired T-test. Results: IL-6 levels were identified in all samples. Venous blood samples from patients without apical periodontitis had IL-6 levels ranging from 4.7-18.74 mg/L, while IL-6 levels in patients with apical periodontitis were 4.0-90.75 mg/L. The unpaired t-test showed that there was a significant difference (p=0.02). Conclusion: IL-6 levels in venous blood with apical periodontitis were higher than IL-6 levels in those without apical periodontitis. BackgroundOne challenge to achieving Lymphatic filariasis (LF) elimination is the persistent coverage-compliance gap during annual mass drug administration (MDA) and the risk of ongoing transmission among never treated individuals. Our analysis examined factors associated with individuals who were never treated during MDA.MethodsData were derived from two cross-sectional surveys conducted in Waihaong and Air Salobar Health Center in 2018 and 2019. We analyzed information from 1915 respondents aged 18 years or above. The study outcome was individuals who self-reported never treatment during any round of MDA. All potential predictors were grouped into socio-demographic, health system, therapy and individual factors. Logistic regression analyses were used to examine factors associated with never treatment in any year of MDA.ResultsNearly half (42%) of respondents self-reported they were never treated during any round of MDA. Factors associated with increased odds of never treatment were respondents working in formal sectors (aOR = 1.75, p = 0.040), living in the catchment area of Waihaong Health Center (aOR = 2.33, p = 0.029), and those perceiving the possibility of adverse events after swallowing LF drugs (aOR = 2.86, pConclusionsEfforts to reassure community members about adverse events, specific instructions on how to take LF drugs, and improving awareness that MDA participation is part of one's contribution to promoting community health are essential drivers for uptake with LF drugs during MDA.

Adolescents are a population group that is vulnerable to nutritional problems other than toddlers, especially young women. Special attention to the dietary issues of adolescent girls needs to be obtained along with the increase in the adolescent population in Indonesia because this affects the growth and development of the body and will impact adult nutrition problems. The purpose of the study was to analyze the relationship between diet assessment and oral health status of adolescent girls, the relationship between oral hygiene behavior and oral health status of adolescent girls, and the simultaneous relationship between dietary assessment and oral hygiene behavior with the oral health status of adolescent girls. Analytical research using the survey method was conducted on 96 young women in two junior high schools in Bandung. Assessment of diet seen from eating behavior and anthropometric examination. Eating behavior using the Adolescent Food Habit Checklist Index questionnaire and anthropometric investigations were carried out by looking at body height, body weight, and Mid Upper Arm Circumference using the standards from the Indonesian Minister of Health Regulation 2020. Oral hygiene behavior using the Oral Hygiene Behavior Index questionnaire. Oral health status using the Dental Health Status Assessment. The results were statistically analyzed with Spearman’s Rank Correlation, and Multiple Linear Regression tests showed no significant relationship between dietary assessment and oral health status (eating behavior with a p-value = 0.429 and anthropometric examination with a p-value = 0.262). A significant association between oral hygiene behavior and oral health status, with a p-value of 0.003, while there is no simultaneous relationship between diet assessment and oral hygiene behavior with oral health status, with multiple r2 = 13.2%.

Nyeri pascabedah harus diatasi dengan baik menggunakan analgetik yang memiliki efek analgesia adekuat dengan efek samping minimal. Penelitian ini bertujuan mengetahui perbedaan nyeri pascabedah laparotomi kolesistekomi yang dinilai dengan numeric rating score (NRS) antara pasien yang menggunakan continuous intravenous patient controlled analgesia (IV-PCA) fentanil dan oksikodon. Penelitian dilakukan pada periode Agustus–November 2020 di RSUP dr. Hasan Sadikin Bandung. Penelitian randomized control trial (RCT) dilakukan terhadap 32 subjek status fisik ASA I–II yang terbagi menjadi dua kelompok, yaitu kelompok yang mendapat continuous IV-PCA fentanil 0,5 µg/kgBB/jam dan kelompok dengan continuous IV-PCA oksikodon 30 µg/kgBB/jam. Nyeri pascabedah dinilai dengan NRS pada jam pertama, ke-6, ke-12, dan ke-24. Analisis statistik data numerik dengan Uji Mann-Whitney, data kategorik dengan uji chi-square dan alternatif Uji Fisher’s Exact. Nilai NRS pada kelompok oksikodon lebih rendah dibanding dengan fentanil pada jam pertama hingga jam ke-24 (p=0,001). Kebutuhan rescue analgetik pada jam pertama dan ke-6 juga lebih rendah pada kelompok oksikodon (p=0,012; p=0,022, berurutan). Penelitian dengan rasio fentanil:oksikodon 1:60 ini tidak menunjukkan perbedaan efek samping kejadian mual, muntah, pusing, sakit kepala, dan pruritus antara kedua kelompok. Pemberian analgesia oksikodon intravena pada pasien pascabedah laparotomi kolesistektomi lebih baik dibanding dengan fentanil intravena.

Abstract In contrast to humans and dogs, the skin microbiota of wolves is yet to be described. Here, we investigated the skin microbiota of dogs and wolves kept in outdoor packs at the Wolf Science Center (WSC) via 16S rRNA gene amplicon sequencing. Skin swab samples were also collected from human care takers and their pet dogs. When comparing the three canine groups, representing different degrees of human contact to the care takers and each other, the pet dogs showed the highest level of diversity. Additionally, while human skin was dominated by a few abundant phylotypes, the skin microbiota of the care takers who had particularly close contact with the WSC animals was more similar to the microbiota of dogs and wolves compared to the humans who had less contact with these animals. Our results suggest that domestication may have an impact on the diversity of the skin microbiota, and that the canine skin microbiota can be shared with humans, depending on the level of interaction.

Penyakit yang mengakibatkan kompresi medulla spinalis dapat mengakibatkan iritasi pada sistem saraf otonom. Hiperinervasi saraf simpatis berisiko tinggi pada aritmia ditandai adanya perubahan pada elektrokardiografi, yakni perubahan durasi gelombang P, durasi QRS, depresi segmen ST, interval puncak gelombang T dan ventrikel ekstrasistol. Laki- laki 52 tahun dengan tumor intra-ektramedullar pada area cervikalis 2-3 dengan tetraparesis dan ventrikel ektrasistol dilakukan wide eksisi tumor dan stabilisasi posterior. Pemeriksaan fisik nadi 90 x/menit teraba tidak teratur. Elektrokardiogarfi (EKG) didapatkan hasil irama irreguler 82 x/menit, ventrikel ektrasistol 10 x/menit. Echocardiography menunjukkan disfungsi diastolik grade 3 preserved LV function. Sebelum operasi pasien diberikan terapi ventrikel ektrasistol dengan menggunakan analgetik dan amiodaron 150 mg (10 mL) pada 10 menit pertama, dilanjutkan dengan 360 mg (200 mg) untuk 6 jam selanjutnya, 540 mg untuk 18 jam berikutnya dan analgetik. Induksi anestesi dilakukan dengan midazolam 3 mg, fentanyl 200 mcg, lidokain 60 mg, propofol 100 mg, dan atricurium 30 mg serta intubasi manual in-line. Dilakukan pemasangan arteri line dan kateter vena sentral setelahnya pasien diposisikan prone. Pembedahan berlangsung 6 jam. Pasien dirawat di ICU 2 hari sebelum pindah ruang rawat biasa. Pemberian amiodarone sendiri dapat dipertimbangkan pada ventrikel ekstrasistol maligna yang memerlukan tatalaksana segera dengan pertimbangan hemodinamik pasien dalam keadaan stabil. Anesthesia Management for Cervical 2-3 Spinal Cord Tumor with Ventricles Extrasystole Abstract Compression of the spinal cord can cause irritation to the autonomic nervous system. Hyperinervation of sympathetic nerves at high risk for arrhythmias characterized by electrocardiographic results in changes in P-wave duration, QRS duration, ST-segment depression, T-wave peak interval, and ventricular extrasystole. A 52-year-old male with an intra-extramedullar tumor in cervical 2-3, tetraparesis, dysrhythmias, and ventricular extrasystole bigemini. Wide excision of tumor and posterior stabilization would be performed. The pulse was 90x/minute palpable irregularly. Electrocardiography examination revealed irregular rhythm 82 x/minute and ventricular extrasystole 10 x/minute. Echocardiography showed grade 3 diastolic dysfunction with preserved LV function. Before the procedure, the patient was given management for the dysrhythmia and ventricular extrasystole with analgetics and amiodaron 150mg (10ml) in the first 10 minutes followed by 360mg (200mg) for the next 6 hours, 540mg for the next 18 hours and analgetics. General anesthesia carried out with midazolam 3mg, fentanyl 200mcg, lidocaine 60mg, propofol 100mg, and atricurium 30mg, with manual intubation in-line. After arterial line and central venous catheter insertion, the patient was placed in the prone position. Surgery lasted for approximately 6 hours. The patient was treated in the ICU for 2 days before moving to the usual ward. Amiodarone can be considered in ventricular extrasystole requiring immediate treatment with stable hemodynamic.

Students are required to have the ability to implement mathematics in solving everyday life problems. A good solving process will produce an excellent solving ability. The existing problem-solving stages cannot be used in solving problems with the Higher Order Thinking Skills (HOTS) category. The aims of this research are 1) to know students' problem-solving process in solving HOTS category questions; 2) to design the stages of problem-solving that can be used to solve HOTS category questions. Twenty-four students who took the differential calculus course in mathematics education at one of Indonesia's private universities made up the study group. The solving process is divided into high, medium, and low. The descriptive and qualitative research method describes the Polya stages' problem-solving approach. Researchers used four sets of problem-solving by making nine indicators. Based on the analysis results, it can be concluded that several indicators of the problem-solving stages have not been appropriately implemented, including I4, I8, and I9. In addition, the steps at the Polya problem-solving stage of devising a plan and looking back must be improved. Based on the findings above, research recommendations design the stages of problem-solving consisting of 6 steps. Future research will develop steps of problem-solving with the characteristics of the HOTS category questions.

Objectives Vaccine hesitancy remains a major barrier to immunisation coverage worldwide. We explored influence of hesitancy on coverage and factors contributing to vaccine uptake during a national measles–rubella (MR) campaign in Indonesia.Design Secondary analyses of qualitative and quantitative data sets from existing cross-sectional studies conducted during and around the campaign.Methods Quantitative data used in this assessment included daily coverage reports generated by health workers, district risk profiles that indicate precampaign immunisation programme performance, and reports of campaign cessation due to vaccine hesitancy. We used t-test and χ2 tests for associations. The qualitative assessment employed three parallel national and regional studies. Deductive thematic analysis examined factors for acceptance among caregivers, health providers and programme managers.Results Coverage data were reported from 6462 health facilities across 395 districts from 1 August to 31 December 2018. The average district coverage was 73%, with wide variation between districts (2%–100%). One-third of districts fell below 70% coverage thresholds. Sixty-two of 395 (16%) districts paused the campaign due to hesitancy. Coverage among districts that never paused campaign activities due to hesitancy was significantly higher than rates for districts ever-pausing the campaign (81% vs 42%; p

Abstract Suboptimal infant young child feeding practices are frequently reported globally, including in Indonesia. This analysis examined the impact of a package of behaviour change interventions on breastfeeding practices in Malang and Sidoarjo Districts, East Java Province, Indonesia. The BADUTA study (which in the Indonesian Language is an acronym for BAwah DUa TAhun, or children aged less than 2 years) was an impact evaluation using a cluster‐randomized controlled trial with two parallel treatment arms. We conducted household surveys in 12 subdistricts from Malang and Sidoarjo. We collected information from 5175 mothers of children aged 0–23 months: 2435 mothers at baseline (February 2015) and 2740 mothers at endline (January to February 2017). This analysis used two indicators for fever and diarrhoea and seven breastfeeding indicators (early initiation of breastfeeding, prelacteal feeding, exclusive breastfeeding under 6 months, predominant breastfeeding, continued breastfeeding, age‐appropriate breastfeeding and bottle‐feeding). We used multilevel logistic regression analysis to assess the effect of the intervention. After 2 years of implementation of interventions, we observed an increased odds of exclusive breastfeeding under 6 months (adjusted odds ratio [aOR] = 1.85; 95% confidence interval [CI]: 1.35–2.53) and age‐appropriate breastfeeding (aOR = 1.39; 95% CI: 1.07–1.79) in the intervention group than in the comparison group, at the endline survey. We found significantly lower odds for prelacteal feeding (aOR = 0.52; 95% CI: 0.41–0.65) in the intervention than in the comparison group. Our findings confirmed the benefits of integrated, multilayer behaviour change interventions to promote breastfeeding practices. Further research is required to develop effective interventions to reduce bottle use and improve other breastfeeding indicators that did not change with the BADUTA intervention.

A renewed experimental procedure for the unknown carbonyl experiment has been developed using hydrazone synthesis, thin layer chromatography (TLC) analysis, and melting point determination to differentiate between structurally similar ketones and aldehydes in undergraduate organic chemistry practical courses. Benzaldehyde, p-tolualdehyde, acetophenone, and p-methylacetophenone can be differentiated from one another by first forming the respective carbomethoxyhydrazone, followed by confirming the presence of a ketone or an aldehyde moiety in the initial unknown by TLC analysis, and finally, identifying the initial unknown by determining the melting point of the hydrazone and comparing it to literature values. Purification by simple recrystallization yields the pure carbomethoxyhydrazone in high yield, and the reaction and analysis of the product can easily be completed within a single 8-h laboratory class (or within two 4-h sessions). Students not only learn basic synthesis techniques such as refluxing and recrystallization and analysis techniques such as TLC and melting point determination but also are exposed to elements of critical thinking in the need for and use of nuclear magnetic resonance (NMR) in everyday analysis, especially in light of conducting analysis when NMR or other sophisticated instrumentation is not available.

The integration of children's literature, specifically mathematical story picture books, in mathematics education has demonstrated significant benefits. Nevertheless, its actual implementation largely hinges on teachers' beliefs. This exploratory mixed-methods study examines the beliefs of 78 teachers regarding the integration of children's literature into mathematics teaching and learning, with a focus on identifying its barriers and enablers. Data were collected through an open-ended survey and semi-structured interviews and analyzed using thematic analysis framed by the concept of belief indication. The study identifies 15 barriers (across five themes) and 16 enablers (across six themes) that, teachers believe, affect their decisions to integrate children's literature into mathematics teaching and learning. This paper contextualizes the findings within the Theory of Planned Behavior (TPB), a framework from social psychology, to provide actionable recommendations and compare findings from studies conducted in Asian and Western countries. Ultimately, this research offers a broader understanding of teachers' behaviors and their receptiveness to educational reforms, such as the integration of children's literature, across diverse cultural and international settings.

Abstract Background In inflammatory bowel disease (IBD) in humans, phosphorylated signal transducer and activator of transcription 3 (pSTAT3) is upregulated in mucosal epithelial cells and correlates with clinical severity. Hypothesis/Objective To investigate the expression pattern of pSTAT3 in the mucosa of dogs with chronic inflammatory enteropathy (CIE) and explore correlations between its expression and clinical and histopathological severity scoring. Animals Twenty‐eight canine CIE patients grouped into food‐responsive enteropathy (FRE; 9), steroid‐responsive enteropathy (SRE; 10), and protein‐losing enteropathy (PLE; 9). Abstract Background Tetanus is a rare disease caused by Clostridium tetani, which produces tetanolysin and tetanospasmin. In 2018, there were only approximately ten tetanus cases reported in Indonesia. Despite widespread vaccination, especially in low–middle-income countries, tetanus still occurs (mostly in adults) due to the lack of immunization related to religious tenets, cultural belief, or inaccessibility to medical care. In addition, tetanus in the pediatric population shows features which are quite distinct from the adult group. Case presentation We report a case of a 7-year-old girl presented to our institution with a history of falling 10 days prior to admission, with only skin laceration on her forehead. For 1 day prior to admission, the patient looked drowsy and difficult to be awakened, accompanied with stiffness of her jaw; we diagnosed her as an unimmunized child with an open depressed skull fracture of her frontal bone and wound infection complicated with “lockjaw.” Perioperative management of this rare case is reported and discussed. Conclusion The pediatric intensive care of such patients requires halting further toxin production, neutralization of circulating toxin, and control of the clinical manifestation induced by the toxin that has already gained access to the central nervous system. The basic tenets of anesthetic care in such case must be well-managed and planned prior to surgery.

Waktu reaksi dibutuhkan oleh dokter anestesi karena harus berpikir dan bertindak cepat serta tepat dalam situasi kritis mengancam jiwa pasien. Jaga malam selama 24 jam dapat meyebabkan kelelahan dan gangguan tidur sehingga waktu reaksi menjadi lebih lambat. Tujuan penelitian ini mengetahui perubahan waktu reaksi terhadap stimulus visual pada PPDS Anestesiologi dan Terapi Intensif FK Unpad setelah bertugas selama 24 jam di RSUP Dr. Hasan Sadikin Bandung. Penelitian ini berlangsung pada bulan Oktober–November 2019 merupakan pretest and posttest control group design dilakukan pada 58 peserta dengan menilai waktu reaksi sebelum bertugas dan setelah bertugas selama 24 jam diperiksa menggunakan metode Ruler Drop. Analisis data menggunakan Uji Wilcoxon karena data distribusi tidak normal berdasar atas hasil Uji Kolgomorov Smirnov. Pada penelitian ini diperoleh median waktu reaksi setelah bertugas selama 24 jam menjadi lebih lama dibanding dengan sebelum bertugas. Nilai median waktu reaksi sebelum bertugas adalah 0,20 detik (range 0,10–0,25 detik), sedangkan waktu reaksi setelah bertugas selama 24 jam adalah 0,23 detik (range 0,17–0,32 detik), dengan rerata lama jam tidur selama bertugas 24 jam adalah 2,32±1,552 jam. Simpulan, waktu reaksi lebih lama pada peserta PPDS Anestesiologi dan Terapi Intensif FK UNPAD setelah bertugas selama 24 jam.

Esophageal atresia/tracheoesophageal fistula (TEF) is a congenital defect that often presents with respiratory distress in neonates. The anatomical defect in the form of connection between the esophagus and the trachea often affects respiratory function. Gastric distention often occurs due to large air leak into the stomach and may lead to respiratory distress. Surgical correction of the anatomical defect will improve the outcome in these patients. Preoperative preparation and intraoperative management is an essential point. Suboptimal preparation may lead to a life-threatening situation intraoperative. Focusing on airway and ventilatory management may help in reducing morbidity and mortality. We report a case of anesthesia management of TEF repair in a premature neonate with a complication during airway and ventilatory management.

ABSTRACT Objective: the study aims to analyze the performance and outcome of resuscitation thoracotomy (TR) performed in patients victims of penetrating and blunt trauma in a trauma center in southern Brazil during a 7 years period. Methods: retrospective study based on the analysis of medical records of patients undergoing TR, from 2014 to 2020, in the emergency service of the Hospital do Trabalhador, Curitiba - Paraná, Brazil. Results: a total of 46 TR were performed during the study period, of which 89.1% were male. The mean age of patients undergoing TR was 34.1±12.94 years (range 16 and 69 years). Penetrating trauma corresponded to the majority of indications with 80.4%, of these 86.5% victims of gunshot wounds and 13.5% victims of knife wounds. On the other hand, only 19.6% undergoing TR were victims of blunt trauma. Regarding the outcome variables, 84.78% of the patients had declared deaths during the procedure, considered non-responders. 15.22% of patients survived after the procedure. 4.35% of patients undergoing TR were discharged from the hospital, 50% of which were victims of blunt trauma. Conclusion: the data obtained in our study are in accordance with the world literature, reinforcing the need for a continuous effort to perform TR, respecting its indications and limitations in patients victims of severe penetrating or blunt trauma.

The penetration of Indonesian internet users in first quarter of 2020 has increased by 17 percent compared to 2019. Based on Google Consumer Barometer in 2018, many 79% of Internet users in Indonesia use the internet on a daily basis. During the Covid-19 Pandemic, universities had to do Study From Home and Work From Home. This resulted, use of information technology and computers also increasing. This increase will have an impact on the level of cybercrime vulnerability. The scale of cyber vulnerability is needed to measure level of cybersecurity in universities, especially in data managers. There are five scales, Very Safe, Safe, Vulnerable, Very Vulnerable, and Dangerous. Where the scale is used in negative and positive statements. The measurement results show an average value of 3.3 or a vulnerable scale. Total average value of negative statements is 2.53 or scale close to vulnerability. So it is necessary to socialize the importance of cybersecurity to minimize occurrence of cybercrime.

Introduction: Burns is a condition that is experienced by the community that can cause skin damage to the epidermis. This is often experienced by housewives due to their daily life in the kitchen. So that this does not cause serious problems, then housewives need sufficient knowledge. The use of demonstration and audiovisual (Demavi) health education is packaged to increase one's knowledge. The study aims to recognize the impact of the demonstration health education package and audiovisual on the first aid knowledge of the burns on housewives in Bangetayu Wetan Methods: This type of research is quantitative research. The design used a quasi-experiment with the control group pretest-posttest. The instrument used in this study was a questionnaire with 27 closed-ended questions with multiple choice answer. The number of respondents is 107 which are 54 in treatment group and 53 in control group using the simple random sampling. Data obtained are statistically processed using chi-square test Results: The result of the statistical test shows significant effects of demonstrations and audiovisual and health education packages on knowledge about first aid burns among housewives with p-value of 0.094. Conclusion: The demonstration and audiovisual health education packages has an effect on housewives’ knowledge of first aid in burns. Other package methods can be developed to make it easier for health workers to do health promotion, and other methods of health promotion package development should be adapted to the theme of the material and the existing audience characteristic.

Cedera otak traumatik (COT) adalah suatu proses patologis pada otak yang berasal dari luar tubuh, yang dapat menyebabkan kerusakan permanen atau sementara dari fungsi otak. Salah satu perdarahan otak yang sering menyertai terjadinya COT berat adalah subdural hematoma (SDH). Pasien laki-laki 41 tahun, datang ke rumah sakit dengan penurunan kesadaran GCS E2M2V2 akibat kecelakaan. Pasien dilakukan tatalaksana dan diintubasi di IGD, hasil CT-Scan memperlihatkan SDH, dan edema serebri. Pasien dilakukan evakuasi hematom dengan anestesi umum, diinduksi dengan fentanil 100 μgr, propofol 20 mg dan atracurium 20 mg intravena. Pemeliharaan dengan sevofluran, propofol kontinyu, fentanil dan atracurium intermiten. Pascaoperasi pasien mengalami ventilator associated pneumonia (VAP), diberikan antibiotik sprektum luas untuk pemulihan paru, pada hari ke 10 pasien dilakukan trakeostomi. Pasien dapat disapih dari ventilator hari ke 21 dengan GCS E4M4Vtrach tanpa kontak, dengan skor GOS (Glasgow Outcome Scale) 3, direncanakan untuk homecare. COT berat membutuhkan tatalaksana pascaoperasi yang lebih kompleks, antisipasi penggunaan ventilasi mekanik yang memanjang, risiko VAP serta pertimbangan pemasangan trakeostomi secara cepat hingga pemberian antibiotik yang adekuat untuk mendapatkan hasil luaran terbaik. Prolonged Mechanical Ventilation in Severe Traumatic Brain Injury Patients with Subdural Hematomas Abstract Traumatic brain injury (TBI) is a pathological process in the brain that originates from outside the body, which can lead to permanent or temporary damage to brain function. One of the brain hemorrhages that often accompanies severe TBI is subdural hematoma (SDH). Male patient 41 years old, was admitted to our hospital with decreased of consciusness with GCS E2M2V2 due to a motorcycle accident. The patient was intubated in the emergency room and then performed head CT scan examination which showed SDH and cerebral edema as the result. Patients then underwent hematoma evacuation under general anesthesia, induced with fentanyl 100 g, propofol 20 mg and atracurium 20 mg intravenously. Maintenance of anesthesia with sevoflurane, continuous propofol, fentanyl and intermittent atracurium. Postoperatively the patient experienced ventilator associated pneumonia (VAP) and given broad-spectrum antibiotics for lung recovery, then on the 10th day tracheostomy was performed. Patients can be weaned from the ventilator on day 21st with GCS E4M4Vtrach without contact, with a GOS (Glasgow Outcome Scale) score of 3, and was planned for homecare. Severe TBI requires more complex postoperative management, anticipation of prolonged use of mechanical ventilation, risk of VAP and consideration of rapid tracheostomy installation to adequate antibiotic administration to obtain the best outcome.

Tatalaksana pasien dengan cedera otak traumatik (COT) berat mengalami perubahan berkesinambungan selama 30 tahun terakhir. Tatalaksana yang diarahkan di unit perawatan intensif (intensive care unit/ICU) mengacu pada tatalaksana klinis sebagai titik akhir terapi utama, bertujuan untuk mempertahankan variabel fisiologis tertentu secara ketat dalam rentang target yang telah ditentukan. Satu alternatif terhadap terapi konvensional ini adalah konsep Lund yang mengutamakan penurunan tekanan mikrovaskular. Konsep Lund termasuk suatu strategi target volume untuk mengendalikan tekanan intrakranial, berasal dari Universitas Lund Swedia, lebih dari 27 tahun yang lalu dan tetap masih kontroversi sampai saat ini. Sejak tahun 1996, American Brain Trauma Foundation dan European Brain Injury Consortium, yang mengacu pada konsep Rosner, telah menerbitkan dan memperbarui panduan untuk tatalaksana cedera otak traumatik. Para ahli sangat menyadari adanya patologi intrakranial multifaktorial yang terlihat pada pasien COT berat dan kompleksitas mekanisme cedera otak sekunder setelah trauma primer, akan menemukan bahwa revisi ini sulit untuk dipahami. Review of Lund Concept and Rosner Concept for Therapy of Severe Traumatic Brain Injury Abstract The management of patients with severe traumatic brain injury (TBI) has undergone continuous changes over the past 30 years. Management directed at the intensive care unit (ICU) refers to clinical management as the main end point of therapy, aiming to maintain certain physiological variables strictly within a predetermined target range. One alternative to this conventional therapy is the Lund concept which prioritizes the reduction of microvascular pressure. The concept of Lund includes a volume target strategy for controlling intracranial pressure, from Lund University in Sweden, more than 27 years ago and remains controversial to date. Since 1996, the American Brain Trauma Foundation and the European Brain Injury Consortium, which refers to the Rosner concept, have published and updated guidelines for the management of traumatic brain injury. Experts are well aware of the multifactorial intracranial pathology seen in severe TBI patients and the complexity of the mechanism of secondary brain injury after primary trauma will find that this revision is difficult to understand. The relationship between increased intracranial pressure (ICP) and worse clinical outcome has been proven. Simplifying the physiology of the brain after severe TBI to the patient's management strategy based on the threshold is closely related to the relationship between complex interactions: increased ICP, cerebral blood flow (CBF), and brain metabolism.

Usia lanjut merupakan faktor risiko terjadinya postoperative cognitive dysfunction (POCD). Pemilihan teknik anestesi yang tepat pada usia lanjut akan menurunkan risiko kejadian POCD. Penelitian ini bertujuan untuk membandingkan angka kejadian POCD pada pasien usia lanjut yang menjalani operasi joint arthroplasty antara anestesi umum dan anestesi epidural. Penelitian berlangsung pada bulan Juli–Oktober 2019 di RSUP Dr. Hasan Sadikin Bandung. Penelitian dilakukan menggunakan metode randomized control trial dengan melibatkan 48 pasien yang menjalani tindakan joint arthroplasty. Subjek penelitian dibagi menjadi kelompok anestesi umum (GA) dan anestesi epidural (EA) menggunakan bilangan acak. Penilaian status mental pasien dilakukan menggunakan Mini mental state examination (MMSE) pada saat preoperasi dan tujuh hari pascaoperasi. Perbandingan proporsi kajadian POCD pada kedua kelompok dianalisis dengan menggunakan uji chi-square. Trauma multipel bertanggung jawab atas 5 juta kematian per tahun di seluruh dunia dan merupakan penyebab kematian utama bagi orang-orang muda di bawah 40 tahun, mewakili peristiwa akut dan tak terduga. Obese populations increase, both in developed and developing countries, so anesthesiologists more often face challenges in managing obese patients. Multiple traumas is responsible for 5 million deaths per year worldwide and is the leading cause of death for young people under 40, representing acute and unexpected events. We will report a 36-year-old man with morbid obesity, a BMI of 48.97 kg/m2, who experienced multiple traumas due to a traffic accident, who will undergo an evacuation operation for subdural hemorrhage and craniectomy decompression. The selection of drugs and safe doses is very difficult in patients with multiple traumas, because their volume status may not be accurately known. Anesthetic plan must consider resuscitation status and history of other comorbidities. Another important role of anesthesiologist is the prevention of secondary injury caused by recurrent shock or improper resuscitation.

Abstract Background T cells play a key role in the pathogenesis of chronic inflammatory enteropathy (CIE) in dogs. Cluster of differentiation 3 (CD3) antigen serves as a marker for T cells. In human medicine, Ki‐67 is an indicator for cell growth but there are only a few studies in dogs with CIE. Objective To investigate Ki‐67 in relation to T cells as a marker for CIE in dogs. Animals Eleven dogs with CIE and 6 healthy beagle controls (CO). Methods Retrospective case‐control study. Dogs were clinically assessed by the Canine Chronic Enteropathy Clinical Activity Index (CCECAI). ABSTRACT Objective: to describe, analyze, and trace the epidemiological profile for cardiac trauma victims on a referral trauma hospital of a major urban center. Methods: a case series study to review, describe, compile and analyze medical records of all patients sustaining traumatic cardiac injuries, from January 2015 to January 2020 admitted to the referral trauma hospital of Curitiba, Brazil. Patients sustaining traumatic heart injuries were identified using the hospitals database. Patients who died prior to reaching hospital care were excluded. Results: all 22 cases were urban victims, mostly penetrating injuries (12 stab wounds, 9 gunshot wounds); 82% were male; mean age, 37.1 years. 17 cases (77%) occurred during night hours, 15 between Friday and Sunday, and 15 were admitted hemodynamically stable. Only 27% were diagnosed with FAST, the remainder requiring other imaging exams. About incisions, 14 had thoracotomies, 6 median sternotomies and in 2 cases both. Of injuries, 8 affected the right ventricle, 3 right atrium, 9 left ventricle, 1 right coronary sulcus and 1 anterior wall. All had cardiorrhaphy repair. 3 patients died, 17 were discharged and 2 were transferred. 17 received postoperative echocardiograms, revealing ejection fractions ranging 55.1% to 75%. Patients spent a mean of 9.6 days on ICU and a mean of 15.2 days of total hospital stay. The mortality rate was 14%. Conclusions: cardiac traumas predominantly occurred in adult males, due to violent causes, during night hours on weekends. The overall mortality rate found (14%), as well as total hospital stay, accords with the literature.

Across Southeast Asia and China, more than 17000 Asian bears are kept under suboptimal conditions and farmed for their bile to meet the consumer demand for traditional medicine products. Years of unsterile and repetitive bile extraction contribute to the development of chronic sterile or bacterial cholecystitis, a pathology commonly diagnosed in formerly bile-farmed bears. In both human and veterinary medicine, the diagnostic value of the macroscopic bile examination for assessing gallbladder disease is unclear. The objective of this study is to identify the role of gallbladder bile color, viscosity, and turbidity, while comparing them with established markers of cholecystitis. Moreover, it aims to define the optimal duration of oral antibiotic treatment for chronic bacterial cholecystitis in bears associated with bile farming. Thirty-nine adult, formerly bile-farmed Asiatic black bears (Ursus thibetanus) were examined under anesthesia and underwent percutaneous ultrasound guided cholecystocentesis. A total of 59 bile samples were collected with 20 animals sampled twice to evaluate the therapeutic success. All bile aspirates were assessed macroscopically and microscopically followed by submission for bacterial culture and antimicrobial sensitivity. In the majority of bears, samples with cytological evidence of bactibilia lacked inflammatory cells and did not always correlate with positive bacterial cultures. The most common bacterial isolates were Enterococcus spp, Streptococcus spp and Escherichia coli. Based on our findings, the optimal duration of antibiotic treatment for chronic bacterial cholecystitis is 30 days. Moreover, unlike Gamma-glutamyl Transferase (GGT) and gallbladder wall thickness, the organoleptic properties of bile were found to be reliable markers of chronic gallbladder inflammation with color and turbidity indicating cholestasis. The current study highlights the importance of cholecystocentesis for the management of gallbladder disease and provides initial results on the possible diagnostic value of macroscopic bile examination.

Given the high prevalence of intestinal disease in humans and animals, there is a strong need for clinically relevant models recapitulating gastrointestinal systems, ideally replacing in vivo models in accordance with the principles of the 3R. We established a canine organoid system and analysed the neutralising effects of recombinant versus natural antibodies on Clostridioides difficile toxins A and B in this in vitro system. Sulforhodamine B cytotoxicity assays in 2D and FITC-dextran barrier integrity assays on basal-out and apical-out organoids revealed that recombinant, but not natural antibodies, effectively neutralised C. difficile toxins. Our findings emphasise that canine intestinal organoids can be used to test different components and suggest that they can be further refined to also mirror complex interactions between the intestinal epithelium and other cells.

Penelitian ini bertujuan mengetahui pengaruh MgSO4 i.v. dengan dosis bolus 50 mg/kgBB 20 menit prainduksi terhadap kebutuhan analgetik pasca-simple mastectomy. Penelitian ini merupakan penelitian analitik komparatif dengan data tidak berpasangan secara prospektif dengan uji klinis acak terkontrol buta ganda (RCT double blind) yang dilakukan terhadap 26 subjek penelitian yang menjalani simple mastectomy di RSUP Dr. Hasan Sadikin Bandung pada bulan Agustus sampai Desember 2018. The neonatal mortality rate in Indonesia is still relatively high (15 per 1,000 live births). The number of children born to a woman (parity) is also still high. The Total Fertility Rate (TFR) is 2.4 per woman. This study aims to determine the effect of parity on neonatal deaths in Indonesia. We used 2017 IDHS data with a cross-sectional design includes 14,827 live births in the 2012-2017 period. Data were analyzed using the multiple logistic regressions method. The results showed that primipara had a statistically insignificant relationship (p-value > 0.05) compared with multipara. While parity≥4 had a risk of 1.90 times experienced on neonatal mortality compared with multipara (95% CI:1,00-3,63) after being controlled by the maternal age, birth attendant, and place of delivery and statistically significant (p-value≤0,05). Thus, parity ≥4 was significantly associated with neonatal mortality. The more children born to mothers, the higher the risk of neonatal death. While parity 1 did not have a significant relationship with neonatal mortality. It is recommended that family planning programs be increased to reduce parity to avoid the risk of neonatal death. The BKKBN program should be improved in reducing TFR.

Menurut sebuah studi terbaru metode pengukuran lebar lidah lebik baik daripada metode berat badan. Tujuan penelitian ini adalah mengetahui ketepatan ukuran CLMA antara metode berat badan dan lebar lidah yang dinilai dengan skor fiberoptik dan oropharyngeal leak pressure di RSUP Dr. Hasan Sadikin Bandung. Penelitian ini dilakukan pada bulan Februari 2018 menggunakan uji eksperimental analitik crossection setiap subjek mendapat dua perlakuan yang berbeda antara kedua metode. Uji statistik menggunakan uji chi-square untuk skor fiberoptik dan uji t berpasangan untuk oropharyngeal leak pressure. Hasil penelitian skor fiberoptik lebih optimal daripada metode lebar lidah. One Health describes the importance of considering humans, animals, and the environment in health research. One Health and the 3R concept, i.e., the replacement, reduction, and refinement of animal experimentation, shape today’s research more and more. The development of organoids from many different organs and animals led to the development of highly sophisticated model systems trying to replace animal experiments. Organoids may be used for disease modelling in various ways elucidating the manifold host–pathogen interactions. This review provides an overview of disease modelling approaches using organoids of different kinds with a special focus on animal organoids and gastrointestinal diseases. We also provide an outlook on how the research field of organoids might develop in the coming years and what opportunities organoids hold for in-depth disease modelling and therapeutic interventions.

BackgroundSoil-transmitted helminth (STH) infections are still prevalent in Indonesia, with roughly one-third of infected population being preschool-age children (PSC), which are generally at higher risk of morbidity such as malnutrition and anemia. This study aimed to investigate the association of STH infections with nutritional status and anemia among PSC in Nangapanda subdistrict, Ende, East Nusa Tenggara.MethodsA cross-sectional survey involving PSC ranging from 12 to 59 months old from Nangapanda subdistrict, Ende district, East Nusa Tenggara was performed. Socio-demographic, breastfeeding, and complementary feeding information was obtained from structured questionnaires, while nutritional and anemia status was determined from anthropometry and hemoglobin measurements, respectively. Anthropometric z-scores were calculated based on the World Health Organization 2006 standards and stool samples were examined using Kato-Katz method.ResultsA total of 393 PSC randomly selected from 22 villages were examined. The prevalence of underweight, stunting, wasting, and anemia were 33.1%, 40.2%, 17.1%, and 60.3%, respectively. STH infection, predominated by Ascaris lumbricoides, was found in 160 (58.8%) PSC. Single STH infection, but not multiple infection, was independently associated with a lower risk of anemia (odds ratio [OR] 0.320, 95% confidence interval [CI]: 0.126-0.809, p = 0.016). Similar association with anemia was also found on mild STH infection (OR 0.318 [95% CI: 0.114-0.887], p = 0.029). On the other hand, younger children were found to have a higher risk of anemia and stunting. None of the examined variables were independently associated with underweight and wasting.ConclusionSTH infection as well as anemia and malnutrition were prevalent in this region. However in this study, current STH infections seemed to have minimal negative impact on children's nutritional status.

This study examines the conceptual understanding of numbers and operations among 372 fifth-grade students, based on their responses to an online three-tier test designed to assess their number sense, declarative knowledge, explanatory knowledge, and confidence. The results showed that most students had low performance and low number sense, with only approximately 18% demonstrating profound conceptual understanding of numbers and operations based on their high performance in five number sense components and high confidence in their solutions. In addition, the majority of students struggled with judging the reasonableness of computational results and solving context-based problems. Furthermore, approximately 78% of participants had misconceptions, with many expressing high confidence in their answers and reasoning. The findings of this study can serve as critical references when designing teaching content and materials for elementary students by carefully considering their performance, number sense, misconceptions, and confidence. The report concludes with a discussion of the implications of the results and suggestions for future research to address critical concerns in mathematics teaching and learning.

Malnutrisi kronis adalah suatu keadaan dimana tubuh mengalami gangguan dalam penggunaan zat gizi untuk pertumbuhan, perkembangan dan aktivitas. Gangguan metabolik yaitu kondisi adanya kelainan proses metabolisme pada tubuh akibat defisiensi enzim atau hormon. Posisi prone pada neuroanestesi pediatrik tulang belakang dilakukan untuk mencapai tekanan perfusi otak dan tulang belakang yang normal. Tujuan kasus ini membahas pengelolaan neuroanestesi pediatrik pada posisi prone dengan adanya kelainan malnutrisi kronis merupakan hal yang rumit sehingga diperlukan manajemen agar menghasilkan outcome yang baik. Anak laki laki, 8 tahun, 15 kg, 102 cm, benjolan di punggung belakang yang keluar merembes cairan dengan diameter 7 cm sejak lahir, kelemahan anggota gerak, buang air besar dan buang air kecil tertahan. Dilakukan VP-shunt 2 minggu lalu. Premedikasi dengan midazolam dan fentanyl. Induksi menggunakan propofol dan ada penambahan saat laringoskopi dan intubasi. Fasilitas intubasi dengan atracurium. Pemeliharaan anestesi dengan O2 + udara + sevofluran 2% dengan fraksi oksigen 50% + propofol dan rocuronium bolus intravena. Monitoring tanda vital (tekanan darah, denyut jantung, frekuensi nafas, saturasi oksigen, jumlah dan warna urin) serta end tidal CO2. Hasil setelah pengangkatan tumor selama 2 jam hemodinamik relatif stabil, tekanan darah 100–120/60–80 mmHg. Pasien ditempatkan di Pediatric Intensive Care Unit. Simpulan: diperlukan manajemen neuroanestesi pediatrik untuk menangani kasus malnutrisi kronik dan gangguan metabolik pada pengangkatan tumor torakal posterior agar menghasilkan outcome yang baik. Prone Position Anesthesia for Posterior Thoracal Tumor Removal Pediatric Patient with Chronic Malnutrition and Metabolic Disorder Abstract Chronic malnutrition is a condition where the body experiences a disruption in the use of nutrients for growth, development and activity. Metabolic disorders are conditions in which there are abnormal metabolic processes in the body due to enzyme or hormone deficiency. The prone position in pediatric spinal neuroanesthesia is performed to achieve normal cerebral and spinal perfusion pressure. The purpose of this case is to discuss the management of pediatric neuroanesthesia in the complicated prone position in order to produce good outcomes. Boy, 8 years old, 15 kg, 102 cm, a lump in the back that came out seeping fluid with a diameter of 7 cm from birth, weakness in limbs, bowel movements and urinary retention. Performed a VP shunt 2 weeks ago. The premedication method uses midazolam and fentanyl. Induction uses propofol and there are additions during laryngoscopy and intubation. Intubation facilities with atracurium. Maintenance of anesthesia with O2 + air + sevoflurane 2% with 50% oxygen fraction + propofol and intravenous rocuronium bolus. Monitoring vital signs (blood pressure, heart rate, respiratory rate, oxygen saturation, amount and color of urine) and end tidal CO2. Tumor removal results for 2 hours hemodynamically are relatively stable, blood pressure 60–80/40–50 mmHg. Patients are admit to the Pediatric Intensive Care Unit. Conclusion: pediatric neuroanesthesia management is needed to handle chronic malnutrition and metabolic disorders in the removal of posterior thoracic tumors in order to produce good outcomes.

Cedera otak traumatik (COT) merupakan salah satu penyebab utama kematian dan disabilitas di seluruh dunia. Kelompok yang paling berisiko mengalami COT adalah pasien geriatrik, dimana mayoritas disebabkan oleh jatuh. Kejadian jatuh salah satunya dikaitkan dengan hemiparesis akibat riwayat stroke. Seorang laki-laki, 68 tahun dengan Glasgow Coma Scale (GCS) M3V1E4. Pasien mengalami cedera kepala akibat jatuh ketika naik tangga. Pasien memiliki riwayat gangguan keseimbangan akibat stroke iskemik dan mendapatkan terapi clopidogrel. Hasil pemeriksaan CT scan memperlihatkan adanya perdarahan akut sub dural dengan penyimpangan garis tengah 0,9 cm. Operasi pertama yang dilakukan adalah kraniektomi dekompresi dan pemasangan kasa untuk kontrol perdarahan. Perdarahan baru ketika operasi, timbul di midparietal kanan dan diduga berasal dari vena penghubung yang dekat dengan sinus. Perdarahan sulit dihentikan dengan total perdarahan 2500 cc. Tanda vital selama operasi stabil dengan topangan norepinefrin 0,2 mcg/kg/menit. Anestesi dengan menggunakan fentanyl 200 mcg, propofol 100 mg dan vecuronium berkelanjutan. Pemeliharaan dengan oksigen, compressed air dan sevofluran 2%. Pasien ditransfusi pack red cel (PRC) 700 ml. Setelah delapan hari perawatan ICU, kadar fibrinogen dan hemoglobin menjadi normal,dilakukan pengangkatan kassa dari dalam kepala pasien. Saat ini pasien telah dipulangkan dari rumah sakit dengan Glasgow Outcome Scale 3 dan hemiparese sinistra. Pengelolaan anestesi pada operasi pasien memperhatikan kondisi otak akibat COT, stroke iskemik dan efek dari terapi Clopidogrel pada operasi cito. Brain Trauma Foundation memberikan panduan untuk pengelolaan COT yang bertujuan untuk memberikan luaran yang lebih baik. Anesthetic Management for Traumatic Brain Injury in Patient with Ischemic Stroke Abstract Traumatic brain injury (TBI) is one of the leading cause of death and disability around the world. Geriatric being the most vulnerable group, mainly because of falling, of which associated with hemiparesis following stroke. A 68 years old man with Glaslow Coma Scale (GCS) M3V1E4 had brain injury after falling from stairs. The patient is having balance disorder following stroke and receiving clopidogrel afterwards. CT-scan showed acute subdural hemorrhage (SDH) with 0.9cm midline shift. Decompression craniectomy and gauze insertion to stop the bleeding was done on the first surgery. New onset of bleeding occurred in right midparietal. Bleeding was uncontrollable with total volume of 2500cc. Vital signs remained stable with norepinephrine 0.2mcg/kg/min. Anesthesia under fentanyl 200 mcg, propofol 100mg, and continuous vecuronium. Patient was transfused with pack red cel (PRC) total of 700 cc. After eight days in ICU, fibrinogen and hemoglobin returned to normal level, therefore the patient undergone gauze removal. The patient discharged with GOS 3 and left hemiparesis. Anesthesia management in this patient’ surgery focused on brain condition following TBI, ischemic stroke and the effect of clopidogrel therapy in emergency operation. Brain Trauma Foundation issued a management guideline of TBI for better outcome.

In recent years, minimization of logistics and transportation costs has become essential for manufacturing companies to increase profits. One thing is done to reduce logistics and transportation costs by optimizing the route of taking or transporting components from each supplier. Route optimization to minimize total transportation costs is a problem that often finds in Vehicle Routing Problems (VRP). Problem Capacitated Vehicle Routing with Time Windows (CVRPTW) is one variant of VRP that considers the vehicle capacity and the service period of each vehicle. CVRPTW is a Non-Polynomial Hard (NP-Hard) problem that requires an efficient and effective algorithm in solving problems that occur in this automotive company. This study uses the Ant Colony Optimization (ACO) algorithm by testing using several parameters to solve the CVRPTW problem. The test results using the ACO algorithm obtained a faster route compared to the method applied by the company.

Objectives The perceptions of family-planning (FP) acceptors regarding contraception influence the reasons for which they choose to switch their method of contraception. The objective of this study was to analyze the perceptions of contraception and rationales for switching contraceptive methods among female FP acceptors in West Nusa Tenggara, Indonesia. Methods This study involved the analysis of secondary data from the Improve Contraceptive Method Mix study, which was conducted in 2013 by the Center for Health Research, University of Indonesia. The design of the study was cross-sectional. We performed 3 stages of sampling using the cluster technique and selected 4819 women who were FP acceptors in West Nusa Tenggara Province, Indonesia as the subjects of this study. The data were analyzed using multiple logistic regression. Results The predominant pattern of switching contraceptive methods was switching from one non-long-term method of contraception to another. Only 31.0% of the acceptors reported a rational pattern of switching contraceptive methods given their age, number of children, and FP motivations. Perceptions of the side effects of contraceptive methods, the ease of contraceptive use, and the cost of the contraceptives were significantly associated (at the level of α=0.05) with rational patterns of switching contraceptive methods. Conclusions Perceptions among FP-accepting women were found to play an important role in their patterns of switching contraceptive methods. Hence, fostering a better understanding of contraception through high-quality counseling is needed to improve perceptions and thereby to encourage rational, effective, and efficient contraceptive use.

Background: Patients with type-2 Diabetes Mellitus (DM) need support from their families to perform self-care to prevent complications; however, not all families have effective support. Family empowerment is a family-based strategic intervention which can improve family support to those patients. Purpose: This study aimed to determine the effects of family empowerment on increasing family support in patients with type 2 DM. Methods: This study used a quasi-experiment with non-equivalent control group design. Forty-six respondents were recruited using a consecutive sampling technique and assigned to the control and intervention groups. Respondents in the intervention group were given family empowerment intervention by visiting their homes four times, for 120 minutes each. In contrast, the control group was given the intervention of standard booklets. The Hensarling Diabetes Family Support Scale (HDFSS) was used to observe the family support on both groups. Data were analyzed by independent t-test. Results: Results showed that there was a significant difference between the intervention and the control group with t=7.86 and p-value of 0.00. There were mean differences of 2.29 and 0.28 between the intervention and the control group, respectively. Conclusion: Family empowerment affected family support in patients with type-2 DM. Based on this study, it is recommended that the health workers advocate and encourage the family in the planning management of patients with diabetes mellitus.

BACKGROUND: Degenerative cervical myelopathy (DCM) is a progressive chronic spinal cord injury estimated to affect 1 in 50 adults. Without standardised guidance, clinical research studies have selected outcomes at their discretion, often underrepresenting the disease and limiting comparability between studies. Utilising a standard minimum data set formed via multi-stakeholder consensus can address these issues. This combines processes to define a core outcome set (COS)-a list of key outcomes-and core data elements (CDEs), a list of key sampling characteristics required to interpret the outcomes. Further how these outcomes should be measured and/or reported is then defined in a core measurement set (CMS). This can include a recommendation of a standardised time point at which outcome data should be reported. This study defines a COS, CDE, and CMS for DCM research. METHODS AND FINDINGS: A minimum data set was developed using a series of modified Delphi processes. Phase 1 involved the setup of an international DCM stakeholder group. Phase 2 involved the development of a longlist of outcomes, data elements, and formation into domains. Phase 3 prioritised the outcomes and CDEs using a two-stage Delphi process. Phase 4 determined the final DCM minimal data set using a consensus meeting. Using the COS, Phase 5 finalised definitions of the measurement construct for each outcome. In Phase 6, a systematic review of the literature was performed, to scope and define the psychometric properties of measurement tools. Phase 7 used a modified Delphi process to inform the short-listing of candidate measurement tools. The final measurement set was then formed through a consensus meeting (Phase 8). To support implementation, the data set was then integrated into template clinical research forms (CRFs) for use in future clinical trials (Phase 9). In total, 28 outcomes and 6 domains (Pain, Neurological Function, Life Impact, Radiology, Economic Impact, and Adverse Events) were entered into the final COS. Thirty two outcomes and 4 domains (Individual, Disease, Investigation, and Intervention) were entered into the final CDE. Finally, 4 outcome instruments (mJOA, NDI, SF-36v2, and SAVES2) were identified for the CMS, with a recommendation for trials evaluating outcomes after surgery, to include baseline measurement and at 6 months from surgery. CONCLUSIONS: The AO Spine RECODE-DCM has produced a minimum data set for use in DCM clinical trials today. These are available at https://myelopathy.org/minimum-dataset/. While it is anticipated the CDE and COS have strong and durable relevance, it is acknowledged that new measurement tools, alongside an increasing transition to study patients not undergoing surgery, may necessitate updates and adaptation, particularly with respect to the CMS.

BackgroundChronic kidney disease (CKD) is accompanied by increases in circulating fibroblast growth factor 23 (FGF23) and aldosterone levels. Here, we tested the hypothesis that aldosterone may be one of the driving forces behind increased FGF23 secretion in CKD.MethodsUsing data from a prospective study in humans, a retrospective study in dogs and cats, and an experimental study in 5/6-nephrectomized mice, we analyzed the relationship between circulating FGF23 and serum aldosterone levels in CKD across four species. To assess the effects of acute inhibition of aldosterone signaling on circulating FGF23, we acutely treated mice with established CKD with the mineralocorticoid receptor blocker canrenone (50 mg/kg iv/sc), and measured intact FGF23 before and 24 h as well as 72 h after start of administration of the drug.ResultsWe found a tight positive association between circulating intact FGF23 and serum aldosterone in human, canine, and feline CKD patients, as well as in experimental murine CKD (humans: rS = 0.57, p = 0.0368; dogs: rS = 0.66, p = 0.0019; cats: rS = 0.75, p = 0.0003; mice: rS = 0.49, p = 0.0004). Injection of canrenone in mice with established CKD did not lead to changes in FGF23 levels within 24 h, but reduced FGF23 in all mice at 72 h.ConclusionAldosterone may drive enhanced FGF23 secretion in CKD, possibly explaining the tight positive association between circulating intact FGF23 and aldosterone in human, canine, and feline CKD patients as well as in experimental CKD models.

Mual dan muntah pascaoperasi (postoperative nausea and vomiting, PONV) menjadi masalah yang umum terjadi setelah operasi mastektomi dengan angka kejadian berkisar 21–92%. Pemberian minuman karbohidrat (CHO) praoperasi ditawarkan sebagai sebuah solusi menurunkan PONV dan juga merupakan salah satu komponen penting dan disarankan untuk mencapai enhanced recovery after surgery (ERAS). CHO berhubungan dengan penurunan katabolisme dan resistensi insulin pascaoperasi. Penelitian ini bertujuan mengetahui pengaruh pemberian minuman berkarbohidrat (CHO) oral sebelum operasi terhadap tingkat kejadian PONV pascaoperasi mastektomi. Penelitian menggunakan eksperimental uji acak buta tunggal di Rumah Sakit Dr. Hasan Sadikin (RSHS) Bandung pada November 2019–Februari 2020. Sebanyak 46 pasien dibagi menjadi dua kelompok, yakni kelompok K (kelompok kontrol, air mineral, n=23 subjek) dan kelompok C (kelompok CHO, n=23 subjek). Analisis data hasil penelitian dilakukan dengan Uji Mann-hitney. Objectives: Contraceptive methods vary in effectiveness, actions required of users, and side effects. This article aims to analyze the perceptions about contraceptive methods among family planning clients of the East Java and West Nusa Tenggara (NTB) provinces in Indonesia. Methods: The data were obtained from the operational research of the Improving Contraceptive Method Mix Project in Indonesia, conducted in 2015–2016 in six districts in East Java and West Nusa Tenggara. The total sample size was 12,190 women aged 15–49 years. The perceptual mapping method uses cross-sectional surveys that require the respondent to rate the level to which they associate specific elements with each other, based on similarities and differences of perceived association. The correlation matrices for six contraceptive methods and five attributes were subjected to a multidimensional scaling analysis. Results: The results showed injectable to be the most preferred method, which was positioned closest to the attributes ‘easy to use’ and ‘easy to get’, followed by pills. Implants ranked higher than other long-acting and permanent method. Conclusion: Injectables and pills were the most preferred because people believe that they were ‘easy to use’ and ‘easy to get’. The least preferred methods were tubectomy and vasectomy because the respondents thought those were not close to any attributes at all. To increase the demand for the long-acting and permanent method choice in the provinces of East Java and West Nusa Tenggara, the attributes ‘easy to use’ and ‘easy to get’ should be emphasized in the long-acting and permanent method Information Education and Communication messages.

Introduction: Multiple studies suggest that SARS-CoV-2 infection is associated with a pro-thrombotic state and thrombotic events have been recorded in several organs and systems. We report a patient with no respiratory symptoms, presented with abdominal pain and an extensive splenic infarction after COVID-19. Case report: A 67 year-old man was admitted to the emergency department with a moderate, dull, left-sided abdominal pain. The patient denied respiratory symptoms but referred contact with family members positive for COVID-19. He tested positive for COVID-19 and had increased D-dimer levels. Imaging studies revealed splenic infarcts and ground-glass opacities in bilateral pulmonary bases. He was treated with full-dose anticoagulation and was discharged home on oral Rivaroxaban. Discussion: Although rare in the literature, cases of acute abdomen in COVID-19 patients associated with vascular infarctions have increased. Coagulopathy may be present even without clinical respiratory manifestations of the disease. Patients meeting disseminated intravascular coagulation criteria or with markedly elevated D-dimer may benefit from anticoagulant therapy. Conclusion: Clinicians should suspect of abdominal visceral infarctions in COVID-19 patients presented with acute abdominal pain, despite the absence of respiratory symptoms.

BackgroundUnderstanding the actual prevalence of COVID-19 transmission in the community is vital for strategic responses to the pandemic. This study aims to estimate the actual infection of COVID-19 through a seroprevalence survey and to predict infection fatality rate (IFR) in Tanjung Priok, the hardest-hit sub-district by the COVID-19 in Jakarta, Indonesia.MethodsWe conducted a venous blood sampling (phlebotomy) to 3,196 individuals in Tanjung Priok between Nov 23, 2020, and Feb 19, 2021 to detect their antibodies against SARS-CoV-2. Using an enumerator-administered questionnaire, we collected data on the respondents' demographic characteristics, COVID-19 test history, COVID-19 symptoms in the last 14 days, comorbidities, and protective behaviours during the last month. We employed descriptive analysis to estimate the seroprevalence and IFR.FindingsThe prevalence of Antibody against SARS-CoV-2 was 28.52% (95% CI 25.44-31.81%), with the result being higher in females than males (OR 1.20; 95% CI 1.02-1.42). By the end of the data collection (February 9, 2021), the cumulative cases of COVID-19 in Tanjung Priok were reported to be experienced by 9,861 people (2.4%). Those aged 45-65 were more likely to be seropositive than 15-19 years old (OR 1.42; 95% CI 1.05-1.92). Nearly one third (31%) of the subjects who developed at least one COVID-19 symptom in the last 14 days of the data collection were seropositive. The estimated IFR was 0.08% (95% CI 0.07-0.09), with a higher figure recorded in males (0.09; 95% CI 0.08-0.10) than females (0.07; 95% CI 0.06-0.08), and oldest age group (45-65) (0.21; 95% CI 0.18-0.23) than other younger groups.ConclusionAn under-reporting issue was found between the estimated COVID-19 seroprevalence and the reported cumulative cases in Tanjung Priok. More efforts are required to amplify epidemiological surveillance by the provincial and local governments.
