[Purpose] To investigate responses of Korean physical therapy students, receiving medical terminology education in physical therapy both in Korean and English, after practice with a virtual anatomical system. [Subjects and Methods] The participants were 25 physical therapy students from Konyang University in South Korea visiting the International University of Health and Welfare for training purposes. The virtual anatomy practice was conducted in English using 3 dimensional virtual anatomy software constructed using real cadaver photographs. A questionnaire about this practice and anatomy was completed after the practice. [Results] The results of the questionnaire showed a trend toward high scores for virtual anatomy practice. [Conclusion] The present virtual anatomy system was created using multi-directional photographs from a real cadaver; therefore, it can be used as an auxiliary means of education using cadavers.

Objective: The object of this study was to determine culture conditions that create stable scaffold-free cartilage-like cell-sheets from human bone marrow-derived mesenchymal stem cells (hBMSCs) and to assess their effects after transplantation into osteochondral defects in nude rats., Design: (Experiment 1) The hBMSCs were harvested from 3 males, the proliferative and chondrogenic capacities were assessed at passage 1, and the cells were expanded in 3 different culture conditions: (1) 5% fetal bovine serum (FBS), (2) 10% FBS, and (3) 5% FBS with fibroblast growth factor 2 (FGF-2). The cells were harvested and made chondrogenic pellet culture. The cell proliferation rate, glycosaminoglycan/DNA ratio, and safranin-O staining intensity of pellets cultured condition 3 were higher than those of conditions 1 and 2. (Experiment 2) The hBMSCs were expanded and passaged 3 times under culture condition 3, and fabricate the cell-sheets in chondrogenic medium either with or without FBS. The cell-sheets fabricated with FBS maintained their size with flat edges. (Experiment 3) The cell-sheets were transplanted into osteochondral defects in nude rats. Histological analysis was performed at 2, 4, and 12 weeks after surgery., Results: The osteochondral repair was better after sheet transplantation than in the control group and significantly improved Wakitani score. Immunostaining with human-specific vimentin antibody showed that the transplanted cells became fewer and disappeared at 12 weeks., Conclusions: These results indicate that culture with FGF-2 may help to quickly generate sufficient numbers of cells to create stable and reliable scaffold-free cartilage-like cell-sheets, which contribute to the regeneration of osteochondral defects., Competing Interests: The author(s) declared no potential conflicts of interest with respect to the research, and/or publication of this article.

Purpose: Posterior reference guides are provided by many manufacturers. However, the true posterior femoral condyle bone resection thicknesses using posterior reference guides are not stated by the manufacturers. The purpose of this study was to analyse the influence of the posterior reference guide designs on the posterior femoral condyle bone resection thickness., Methods: Thickness of posterior femoral condyle bone resection and thickness of prostheses were investigated in 8 types of total knee prostheses using the production drawings provided by the manufacturers., Results: Posterior femoral condyle bone resection thickness differed between prostheses. Change in size of the medial posterior condyle ranged from -0.5 to 1.4 mm at 0° external rotation, from -2.9 to 1.4 mm at 3° external rotation, and from -5.3 to 1.4 mm at 6° external rotation. Change in size of lateral posterior condyle ranged from -0.5 to 1.4 mm at 0° external rotation, from -0.4 to 3.4 mm at 3° external rotation, and from -0.4 to 5.3 mm at 6° external rotation., Conclusions: This study showed that posterior femoral condyle bone resection thickness was different for each posterior reference guide. The rotation centre of posterior reference guides influenced the bone resection thickness in the posterior femoral condyle. The size of the posterior femoral condyles increased in some guides but decreased in other guides. The maximum differences in size changes of the posterior femoral condyles between the guides were 1.9 mm at 0° external rotation, 4.1 mm at 3° external rotation, and 6.3 mm at 6° external rotation. To control the size of posterior femoral condyles, the posterior reference guide design should be checked before use.

Background: Soft tissue balancing is crucial to the success of total knee arthroplasty (TKA). To create a rectangular flexion joint gap, the rotation of the femoral component is important. The purpose of this study is to determine whether or not anatomical landmarks of the distal femoral condyles are parallel to the tibial bone cut surface in flexion., Methods: Forty-eight patients (three male and 45 female) with a mean age of 74years were examined. During the operation, we estimated the flexion joint gap with the following three techniques. 1) a three degree external cut to the posterior condylar line (MR1), 2) a parallel cut to the surgical transepicondylar axis (MR2), and 3) a parallel cut to the anatomical transepicondylar axis (MR3)., Results: The flexion joint gap was 1.1±3.0° (mean±standard deviation (SD)) in internal rotation in the case of MR1, 0.9±3.4° in internal rotation in the case of MR2, and 2.1±3.4° in external rotation in the case of MR3. An outlier (flexion joint gap >3.0°) was observed in 12 cases (25%) in MR1, 13 cases (27%) in MR2, and 15 cases (31%) in MR3., Conclusions: The anatomical landmarks of the distal femoral condyles are not always parallel to the tibial bone cut surface in flexion. To create a rectangular flexion joint gap, the rotation of the femoral component rotation is based not only on the anatomical landmarks but also on the ligament balance., (Copyright © 2016 Elsevier B.V. Purpose: Range of motion is a crucial measure of the outcome of total knee arthroplasty. Gap balancing technique and mobile-bearing prosthesis can improve postoperative range of motion. The purpose of this study was to determine the factors that are predictive of the postoperative range of motion., Methods: A total of 94 knees with varus osteoarthritis were prospectively randomized to receive either a posterior-stabilized mobile-bearing or a posterior-stabilized fixed-bearing prosthesis. All preoperative and postoperative protocols and operative techniques were identical in the two groups. Extension and flexion joint gaps were measured using a tensor device during the operation. Multiple regression analysis was conducted to determine the best predictors of the knee flexion angle 2 years after the operation. The independent variables were type of prosthesis (mobile-bearing or fixed-bearing), difference between flexion and extension joint gaps (mm), age, gender, body mass index (BMI), preoperative and intraoperative knee flexion angles, change in posterior condylar offset, and posterior tilt of the tibial plateau., Results: The mean difference between flexion and extension joint gaps was 0.8 ± 1.3 (mean ± SD) mm for mobile-bearing and 0.8 ± 1.9 mm for fixed-bearing prosthesis. The mean flexion angle for mobile-bearing and fixed-bearing groups was 120 ± 16° and 116 ± 20° preoperatively (n.s.), 142 ± 9° and 141 ± 12° intraoperatively (n.s.), and 129 ± 10° and 128 ± 13° at 2 years postoperatively (p = 0.773), respectively. Predictors were identified in the following three categories: (1) preoperative flexion angle, (2) intraoperative radiographic flexion angle, and (3) BMI (R = 0.603, p < 0.001)., Conclusions: Mobile-bearing prosthesis and optimal gap balancing did not result in superior postoperative flexion angle. Better preoperative and intraoperative flexion angles and lower BMI were the significant predictors for better postoperative flexion angle., Level of Evidence: Therapeutic study, Level I.

Clinical trials of various procedures, including bone marrow stimulation, mosaicplasty, and autologous chondrocyte implantation, have been explored to treat articular cartilage defects. However, all of them have some demerits. We focused on autologous culture-expanded bone marrow mesenchymal stem cells (BMSC), which can proliferate without losing their capacity for differentiation. First, we transplanted BMSC into the defective articular cartilage of rabbit and succeeded in regenerating osteochondral tissue. We then applied this transplantation in humans. Our previous reports showed that treatment with BMSC relieves the clinical symptoms of chondral defects in the knee and elbow joint. We investigated the efficacy of BMSC for osteoarthritic knee treated with high tibial osteotomy, by comparing 12 BMSC-transplanted patients with 12 cell-free patients. At 16-month follow-up, although the difference in clinical improvement between both groups was not significant, the arthroscopic and histological grading score was better in the cell-transplanted group. At the over 10-year follow-up, Hospital for Special Surgery knee scores improved to 76 and 73 in the BMSC-transplanted and cell-free groups, respectively, which were better than preoperative scores. Additionally, neither tumors nor infections were observed in all patients, and in the clinical study, we have never observed hypertrophy of repaired tissue, thereby guaranteeing the clinical safety of this therapy. Although we have never observed calcification above the tidemark in rabbit model and human histologically, the repair cartilage was not completely hyaline cartilage. To elucidate the optimum conditions for cell therapy, other stem cells, culture conditions, growth factors, and gene transfection methods should be explored.

Objective: The objective of this study was to assess the effect of granulocyte colony-stimulating factor (G-CSF) on osteochondral defect repair in the rat knee., Design: Twenty-six 12-week-old male Lewis rats were randomly divided into 2 groups. From day 0 to day 4, the G-CSF group received glycosylated G-CSF, and the control group received phosphate-buffered saline. A 1.5-mm diameter and 1.0-mm deep osteochondral defect was introduced in the patellar groove of the bilateral femur in all rats on day 4. The peripheral blood nucleated cells were counted for 14 days from the first day of injection, the appearance of the cartilage repair was observed histologically and macroscopically for 2, 4, 8, 12, and 24 weeks after surgery., Results: The number of peripheral blood leukocytes increased 3 days and returned to normal levels 7 days after the first injection. Compared with the control group, the G-CSF group had more fibrous and/or bony tissue at earlier points in time. The tissue repair rate, which is defined as the percentage of repaired osteochondral defects, was significantly higher in the G-CSF group 4 weeks after surgery. However, there were no significant differences in the cartilage repair rate and the modified Wakitani score between the 2 groups at each time point., Conclusions: The defect filling was significantly better in the G-CSF group in the early phases. Our findings suggest that G-CSF may promote the repair of osteochondral defects by mediating an increase in the number of peripheral blood nucleated cells.

Since the days of ancient Greece, it has been known that articular cartilage has poor potential to repair. In April, 2013, autologous chondrocyte implantation (ACI) was approved by the Japanese ministry of Health, Labour and Welfare, as the first orthopedic cell therapy in Japan. Half a century has passed since Chesterman's report on rabbit allogeneic chondrocyte implantation, and approximately 20 years since the first human ACI report by Brittberg. In Japan, 24 years has passed since Wakitani's allogeneic chondrocyte implantation, and over 10 years since the human ACI report by Ochi. A long-term follow-up clinical trial with the high statistical power is needed to verify the safety and efficacy of new cartilage joint therapy. Moreover, we need the infrastructure, including drug and/or device regulation and timely approved by the government, to apply new therapies in similar time frames to other developed nations.

A collarless, proximally porous-coated type of Spongiosa Metal II hip prosthesis was used for cementless total hip arthroplasty in 52 hips, and 48 hips were followed and assessed during a mean follow-up period of 6.3 years (minimum 5 years). There were 8 hips (16.7%) that had stress shielding, and 2 hips (4.2%) that had thigh pain. In terms of stability, the stem was bony stable in 39 hips, fibrous stable in 6, and unstable in 3. The stability of the stem was significantly correlated with the stem canal filling ratio. It was suggested that the much more proximal transfer of loading stress by a collarless, proximally porous-coated stem is not suitable for the Spongiosa Metal II series because it may unacceptably reduce the stability of the stem., ((c) 2010 Elsevier Inc. In this study, influences of infliximab to health-related quality of life (HRQOL) and active status of RA were assessed. Between 2003 and 2006, 22 patients with rheumatoid arthritis (RA) began receiving infusion of infliximab. Of all the patients, 17 patients who were followed for at least 30 weeks (102 weeks at maximum) after the start of infliximab were included in this study. The mean age was 54.6+/-10 years. HRQOL was evaluated with use of the SF-8trade mark, which is a simple version of the Medical Outcome Study Short Form 36. As an index of active status of RA, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and rheumatoid factor (RF) were collected. CRP and ESR significantly improved at the final follow-up, but RF did not. All subscales of the SF-8 were significantly improved after the start of infliximab. However, there were three patients whose laboratory data were improved, but HRQOL was not. We should not be prepossessed only with laboratory data in treating patients with RA. We recommend that the SF-8 to evaluate HRQOL of patients with RA in busy outpatient clinics because it is both simple and convenient.

We report a 73-year-old female patient who underwent total hip arthroplasty for osteoarthritis of the right hip. An anteroposterior radiograph obtained 1 week after surgery showed an intra-articular striated foreign body. This was subsequently removed and found to be a radio-opaque marker of a surgical sponge that had been used during the operation. The patient made an uneventful recovery. In recent years, cases of retained surgical sponge after surgery have been reported occasionally. Counting surgical sponges and using sponges with radio-opaque markers are methods for preventing them from being accidentally left in situ. However, to our knowledge, there has been no report of retention of a surgical sponge radio-opaque marker at the operation site, appearing as a foreign body on an X-ray film.

Background: Limb-length inequality is not uncommon after total hip arthroplasty (THA) and may cause subjective problems for patients. During THA a stable reference point must be established to determine changes in leg length. Several methods have been useful in minimizing the incidence and magnitude of this problem. The equalization of leg-length discrepancy during THA can be achieved through the use of a simple measuring device, the calipers dual pin retractor (CDPR)., Materials and Methods: The CDPR was developed to establish a fixed point on the pelvis that would remain constant throughout the procedure and from which the distance to the greater trochanter could be measured prior to dislocation of the hip. Limb lengths were measured in 56 patients undergoing primary THA, between 2004 and 2006. The CDPR was used in all cases., Results: The preoperative diagnoses were osteoarthritis in 44 patients, osteonecrosis in five, and rheumatoid arthritis in seven. Average postoperative limb-length inequality was 4.2 mm. None of the patients had to use shoe lifts for equalization of limb lengths or complained of limb-length inequality., Conclusion: This method of measurement is beneficial with most THA approaches, and the technique is helpful for minimizing limb-length inequality during THA.

Methicillin-resistant Staphylococcus aureus (MRSA) septic arthritis is frequently associated with a poor clinical outcome. Joint reconstruction may be worth attempting in patients with a history of persistent MRSA infection. Here we report an unusual case of MRSA septic arthritis of the knee joint that was treated successfully by joint replacement after initial placement of antibiotic-loaded hydroxyapatite (HA) blocks.

A 58-year-old woman suffered spontaneous recurrent hemarthrosis of the knee. In the clinical course, pigmented villonodular synovitis was mostly suspected, but in arthroscopic surgery the lateral meniscus appeared to be upturned and stuck into the lateral pouch with the meniscal ganglion cyst. It was suggested that meniscal tear with meniscal ganglion cyst was related with recurrent hemarthrosis. Generally, both the meniscal ganglion cysts and spontaneous recurrent hemarthrosis are highly rare conditions. In this case, we speculated that a negligible power could induce the meniscal tear with recurrent hemarthrosis in the particular situation in which the meniscal ganglion cyst existed. In other words, the meniscal ganglion cyst might basically and physically relate with hemorrhagic condition. Arthroscopically, the meniscal ganglion cyst was removed together with the anterior segment of the lateral meniscus. Recurrent hemarthrosis was treated successfully by resection of the meniscus.

The purpose of the present study was to retrospectively evaluate the therapeutic outcome of minimally invasive synovectomy assisted with arthroscopy (MISAA). From 1995 to 2003, MISAA was performed on 30 knees of 18 rheumatoid arthritis patients. The mean follow-up period was 74.4 months. Radiographic assessment according to Larsen's radiological classification, the conversion ratio to total knee arthroplasty (TKA), and postoperative change of the range of flexion were evaluated retrospectively. There were 10 knees in grade I, 14 knees in grade II, and 6 knees in grade III, preoperatively. All the knees in grade I and 8 of 14 knees in grade II maintained the same grade until the latest follow-up. The other 6 knees in grade II and all the knees in grade III deteriorated to grade IV and were converted to TKA after MISAA. The mean range of flexion significantly increased postoperatively except the knees that were converted to TKA. MISAA is effective for rheumatoid knees in grade I or II, and improves range of flexion.

A nondissecting aneurysm of the distal descending thoracic aorta, 55 mm in diameter, in a 35-year-old woman who was undergoing 24-year steroid therapy of systemic lupus erythematosus was successfully resected and replaced with a tube graft. The suture line of the aortic wall was reinforced with a polytetrafluoroethylene felt strip. Histologically, the media and adventitia contained numerous lymphoplasmacytic infiltrates that consisted of B and activated T cells mixed with plasmacytes. These findings would have been in accordance with active aortitis. Systemic lupus erythematosus was suggested as possibly associated with nondissecting thoracic aortic aneurysm, especially in patients undergoing chronic steroid therapy in which inflammation continued.

A series of bimetallic complexes of general formula [(phen)Re(CO)(3)LOs(trpy)(bpy)](3+) (bpy = 2,2'-bipyridine, phen = 1,10-phenanthroline, trpy = 2,2':6',2' '-terpyridine, and L = 4,4'-bipyridine (4,4'-bpy), trans-1,2-bis(4-pyridyl)ethylene (t-bpe), or 1,2-bis(4-pyridyl)ethane (bpa)) and the model mononuclear species [(phen)Re(CO)(3)L](+) and [Os(trpy)(bpy)L](2+) have been synthesized and their photophysical and photochemical properties studied. In the binuclear species an efficient Re(I)-Os(II) energy transfer is observed, which is analyzed in terms of Förster theory. In the binuclear [(phen)Re(CO)(3)(t-bpe)Os(trpy)(bpy)](3+) complex, the trans to cis isomerization of the coordinated t-bpe ligand, characteristic of the [(phen)Re(CO)(3)(t-bpe)](+) subunit, is inhibited by competitive intramolecular energy transfer.

Ultrasonographically, the femoral component and the tibial plate of total knee prostheses are strongly echogenic, while the high-density polyethylene insert is hypoechoic. This study evaluated the influence of mediolateral and anteroposterior stability after total knee arthroplasty (TKA) on range of motion using real-time monitoring with ultrasound. Mediolateral stress translation, which is increased by horizontal resection of more bone at the ends of the femur or tibia for easy prosthesis implantation, was examined on coronal scans at the level of the collateral ligaments. Anteroposterior drawer was examined on sagittal scans at the level of the patellar tendon. Mediolateral translation (0-10 mm; mean 2.24 mm) did not correlate with range of motion, while anteroposterior drawer (2-10 mm, mean 5.05 mm) correlated well with range of motion. These ultrasonographic findings suggest that horizontal over-resection of the ends of the femur and tibia contributed to joint laxity, which would not result in better ROM. Rollback and sliding of the femoral component on the tibia, which is believed to be correlated with anteroposterior drawer, may be important in achieving better range of motion and obtaining excellent results in TKA.

An experimental study was conducted on a drug delivery system (DDS), using porous apatite ceramics (PAC): hydroxyapatite block (HAb) [Ca10(PO4)6(OH)2] having a porosity of 35-48% and pore size range of 50-300 microm, and beta-tricalcium phosphate block (TCP) [Ca3(PO4)2] having a porosity of 75-80% and pore size range of 100-400 microm, for sustained release of a chemotherapeutic agent. Methotrexate (MTX) was loaded in the pores of PAC blocks by centrifuging the blocks in MTX solution. Impregnation of MTX in PAC blocks (1 cm3) was confirmed by a magnetic resonance imaging (MRI) study using Gadolinium-DTPA enhancement. The MRI showed high signal intensity in the PAC, which was confirmed by dye loading into the pores. To estimate the MTX-releasing capability of the PAC, the blocks were stored in 3 mL of phosphate-buffered saline (PBS) at 37 degrees C and the PBS was replaced every 48 h. The amount of MTX released was assayed by high-performance liquid chromatography. This study showed that MTX-impregnated PAC (0.63-2.25 mg/block) released the drug in a steady manner and maintained its concentration (0.1-1.0 microg/mL) up to 12 days. This concentration is high enough to be effective against tumor cells. Chemotherapeutic agent-impregnated PAC, prepared by simple centrifugation, could be a valuable form of local chemotherapy when used as a strut graft to repair bone defects. This new DDS material could also be used as an adjuvant to extended curettage and provide a means to reduce the recurrence of tumors without risk of systemic toxicity.

A new exposure for the direct lateral approach to the hip through the gluteus medius by anterior half osteotomy of the greater trochanter is described. The skin incision is made with the greater trochanter longitudinally at its midpoint and extends parallel to the shaft of the femur along its anterior border. The anterior two thirds of the tendinous gluteus medius muscle is divided. The incision continues to the middle of the greater trochanter and through the distal half of the vastus lateralis muscle. An anterior thin slice osteotomy of the greater trochanter is performed. The gluteus minimus is exposed and split in the line of its fibers or detached from the greater trochanter and the acetabular roof is exposed. In order to provide a wide exposure, a Charnley transverse retractor is used and a Calipers Dual Pin Retractor is inserted over the edge of acetabulum by elevating the soft tissues from the ilium. The Calipers Dual Pin Retractor is used to precisely position the prosthesis in an anatomic and functional position and eliminate leg length discrepancy.

The relationship between the range of motion following total knee arthroplasty (TKA) and the height of chairs when rising from a seated position was analyzed. Forty-six TKA subjects were evaluated; 16 had osteoarthritis, 30 had rheumatoid arthritis. Subjects were divided into two groups based on their degree of knee flexion (average: 96.0 degrees; range: 75 degrees to 135 degrees; group 1 contained 24 subjects with < 100 degrees of flexion and group 2 contained 22 subjects with > 100 degrees of flexion) in order to evaluate the acceptable knee flexion angle required for comfortably rising from a chair. To evaluate the flexion-extension angle of the knee and hip joints, three goniometers, a large reaction force plate, and a switch sensor on the chair surface to detect the initiation of rising were used. One chair had a height equal to the subjects' lower leg length, while the height of the second chair was 120% of the subjects' lower leg length. Analysis showed that TKA patients with less knee flexion (< 100 degrees) required a high angular velocity of the hip and excessive swing velocity to lift the trunk forward than did those with a larger degree of knee flexion (> 100 degrees). We concluded that a minimum of 100 degrees of postoperative flexion is desired and that a higher chair is more suitable for TKA patients.

To increase the range of motion in the knee, posterior cruciate ligament-retaining total knee arthroplasty (TKA), which has a roll back (RB) mechanism, is used. The roll back mechanism is the complex movement of the femoral condyles relative to the tibial plateau, which results in translation of the tibiofemoral contact point posteriorly as the knee is flexed. This study was undertaken to assess the function of the posterior cruciate ligament (PCL) and posterior tibial tilt following PCL-retaining TKA. Eleven fresh-frozen cadaveric normal lower extremity specimens (average age: 75.4 years; range: 69 to 88 years) were obtained for this study. The test was performed on a loading frame after specimen preparation. The calcaneal bone was fixed on the rigid frame. A load of 9.5 kg was applied perpendicularly by the moving frame to femoral head through four casters. PCL strain and RB were measured before and following TKA by changing the posterior tilt of tibial component and the external rotation of femoral component. PCL strain at 105 degrees flexion in the 0 degree posterior tilt group (3.3% +/- 2.4%) was decreased in the 10 degrees posterior tilt group (0.6% +/- 0.5%), and this decrease was statistically significant (p < 0.05). RB was measured as anterior-posterior translation by using an arthroscopic probe. After TKA, RB was decreased in the medial compartment but was increased in the lateral component. These different movements of the tibiofemoral contact points consequently caused rotation in the flexion position. Posterior tilt of the tibial component decreased the efficiency of the PCL's function. Retaining the PCL physically is not the same as preserving the function of the PCL because of the lack of the ACL, the change of the joint line, the differences in articular geometry, and the balance of the ligaments.

Hydroxyapatite blocks (HAB) can be used to administer antibiotics or anticancer drugs because its porous structure allows the gradual administration of the pharmacologic agents. A novel drug delivery system using hydroxyapatite blocks was developed for osteomyelitis and postoperative infections occurring after joint replacement. To load the antibiotics, hydroxyapatite blocks were mixed with an antibiotic solution and centrifuged at 1500 rpm for 15 minutes or decompressed in vacuum container at 5 to 10 in. Hg for 20 minutes. Fifteen patients with osteomyelitis including one with tuberculosis and four with infections subsequent to joint replacement were treated with antibiotic-loaded hydroxyapatite blocks in combination with intravenous injection. Except in one case, all of the foci had completely healed at follow-up (range: 13 to 71 months; average: 39.7 months). These new methods are simple and can safely treat osteomyelitis in a one-stage operation.

The equalization of leg-length discrepancy during total hip arthroplasty can be achieved through the use of a simple measuring device, the Calipers Dual Pin Retractor. During total hip arthroplasty a stable reference point must be established to determine leg-length changes. The Calipers Dual Pin Retractor was developed to establish a fixed point on the pelvis, which would remain constant throughout the procedure, from which the distance to the trochanteric area could be measured prior to the dislocation of the hip. This measurement is then used to aid in the proper placement of the femoral components.

This anatomical study concerns the arterial supply of the human acetabulum. The purpose is to contribute to prevention of postoperative necrosis of the acetabular bone fragment after rotational acetabular osteotomy (RAO). The arterial supply of the acetabulum was determined using a corrosion casting method. The acetabular branch of the obturator artery supplies the acetabulum through the acetabular notch. The pubic branches supply the pelvic surface of the acetabulum. Deep branches of the superior gluteal artery supply the superior region and the inferior gluteal artery supplies the postero-inferior region. From these results, we recognized that the important artery for achieving successful RAO is the acetabular branch of the obturator artery that supplies the acetabulum through the acetabular notch.

Although tuberosity calcaneal fracture is rarely found, we report here a neglected avulsion (so-called "beak") fracture of the tuberosity. The etiology of this fracture is presumed to be, in this old female, diabetic (diabetes mellitus) with accompanying osteoporosis. As it was neglected and long-standing, the Achilles tendon lengthening was required on the surgical intervention.

A novel drug delivery system for osteomyelitis was developed using porous hydroxyapatite blocks (HA-b) loaded with antibiotics by centrifugation. In the study, 10 mm3 HA-b was placed in a container and mixed with antibiotic solution; the antibiotic was then loaded into the pores of the HA-b by centrifuging at 1500 rpm for 15 min. Slow release of HA-b in both moist form and dried form (by heating at 160 degrees C) was tested after loading with the antibiotic arbekacin sulfate (ABK), 1-N-(S)-4 amino-2-hydroxybutyryl dibekacin. To estimate the concentration of antibiotic, both forms of HA-b were placed in 3 mL of phosphate buffered saline (PBS), which was replaced every 48 h. In both groups, which were loaded with 70 mg ABK per one block of HA (concn 0.5 microgram/mL) which is sufficiently high to control most pathogens, was maintained for 21 exchanges of PBS (after 42 days). Minimum inhibitory concentration for methicillin-resistant Staphylococcus aureus (MRSA), 3.13 micrograms/mL, was maintained until nine exchanges took place (after 18 days). The centrifugation method is simple, and dried ABK produced by heating loaded HA-b is particularly useful in clinical applications for osteomyelitis.

The clinical efficacy and tolerability of sodium hyaluronate, a high-molecular-weight hyaluronic acid preparation, in the treatment of patients with periarthritis of the shoulder was investigated in a multicenter, cooperative study. Of 70 patients seen at 12 participating centers between April and September 1992, follow-up was possible in 62 (28 men and 34 women; mean age, 65.4 years). Patients received sodium hyaluronate 25 mg (1% solution in an ampule) once weekly for 5 weeks by injection into the glenoid cavity or the subacromial bursa. If periarthritis resolved during this period, treatment was stopped; if periarthritis had not fully resolved by the end of the 5 weeks, weekly or biweekly treatment was continued as appropriate. The mean treatment period was 8.16 +/- 0.88 weeks, and the mean total administered dose was 6.05 +/- 0.61 ampules. Rates of improvement over pretreatment values were 75.0% for pain at rest, 73.7% for pain on motion, and 78.8% for pain on pressure. Significant increases in joint angles (measured as components of joint range of motion after administration of sodium hyaluronate) were observed; activities of daily living also improved significantly (P < 0.05 to P < 0.001). No adverse reactions were observed. These results strongly suggest that sodium hyaluronate is an effective agent for the treatment of patients with periarthritis of the shoulder.

We carried out the ankle arthroplasty by excision of the talar body (subtotal talectomy) on 10 ankles of nine patients with talar body tumor, paralytic talipes equinovarus, talipes varus due to spinal injury, or comminuted fracture of the talar body. Talar body excision was initiated by lateral incision and subsequent osteotomy of the fibula. The fibula was everted, leaving the lateral ligament (calcaneofibular ligament) intact. After the talus was exposed, the talar body was excised, leaving about 1.5 cm of head unresected. Subsequently, the fibula was shortened slightly, and the tibia was pulled down to the level of the calcaneus to form a joint, instead of arthrodesis being performed. After surgery, the joint formed by the tibia and calcaneus was mobile in seven of the 10 feet and immobile (arthrodesis) in the remaining three feet. The average follow-up period was 6 years. Although postoperative x-ray revealed slight osteoarthritic changes of Chopart's joint and the tibiocalcaneal joint, none of the patients showed ankle pain that impaired activities of daily living. Subtotal talectomy allows correction of talipes equinus without Achilles tendon lengthening.

A novel drug delivery system was developed for osteomyelitis using porous hydroxyapatite blocks (HA-b) that were impregnated with antibiotics by a centrifuge method. For the experimental study, a 10 mm3 HA-b was placed in a container, mixed with an antibiotic solution and centrifuged at 1500 rpm for 15 min for the purpose of impregnating antibiotics into the pores. The slow release activity of antibiotic (Arbekacin sulfate [1-N-(S)-4 amino-2-hydroxybutyryl dibekacin]) from the HA-b was tested. An evaluation was made of the slow-releasing capabilities of the ABK from HA-b which was still maintained at 0.5 microgram/ml within 21 exchanges of PBS after 42 days. Consequently, seven patients with osteomyelitis, including one with tuberculosis and two with infected hip arthroplasty, have been treated. On a follow-up study, all of the foci had completely healed by the end of the follow-up period without complications. This new method is simple and can be performed safety as a one-stage operation.

Depressed tibial plateau fractures in 13 patients were treated under arthroscopic control. In 3 patients, bone autograft was used; in the remaining 10, hydroxyapatite (HAP) grafts were used. The depression of the articular surface was managed by elevation of the subchondral plate through a small window. Assisted by fluoroscopy, we corrected and performed the grafting under arthroscopic control. Follow-up periods averaged 5 years 6 months for the autograft and 18 months for the HAP grafts. At the conclusion of the follow-up period, x-ray radiographs and computed tomography (CT) scan showed no evidence of arthrosis and knee motion was normal. There were no significant differences between the two groups. The interporous spaces showed new bone formation on closed biopsy 6 months postoperatively.

Newborn rat calvaria bone cells obtained by digestion were fractionated on columns of wheat-germ agglutinin (WGA) sepharose 6MB for osteoclast isolation. The initial nonspecific binding cells which were passed through the WGA sepharose column by a buffer acquired a high enzyme activity of alkaline phosphatase, but not that of acid phosphatase. However, elution of cells using a buffer with the addition of N-acetyl-D-glucosamine resulted in a high acid phosphatase activity but no alkaline phosphatase activity. The former WGA binding negative fraction enriched osteoblasts averaging 30 microns in size. The latter WGA binding positive fraction enriched osteoclasts ranging from 20 microns to 60 microns in size. The electron-microscope clearly demonstrated the cellular details of osteoclasts. Isolated cell counts showed a ratio of six to four. These results indicate that our method of osteoclast isolation is simple and useful in lectin affinity chromatography because all cells have sugar moieties on their surface and the binding of osteoclasts can be reversed by the addition of specific lectin-binding sugars to the eluting buffer.

A unique chondrosarcoma developed in the distal right middle phalangeal bone of the foot in a 38-year-old woman. In a review of statistics on the localization of 264 chondrosarcomas in the Bone Tumor Registry of Japan since 1964, 4 were in the foot and 11 were in the hand.

FITC-conjugated lectins, Con-A, DBA, GS-I, GS-II, PNA, MPA, RCA-I, SBA, UEA-I, WGA were used for demonstration of lectin bindings of human synovial lining cells, obtained from the patients with rheumatoid arthritis (RA), osteoarthritis (OA), aseptic necrosis (AN), and traumatic injury (TI). In the RA samples, GS-I binding to the cytoplasmic sites was predominantly noted and moderate SBA and MPA bindings were observed. However, PNA was not significant. In the OA samples, predominant binding was found in GS-I and SBA lectins, moderate binding in MPA and PNA. In the AN samples, binding was predominant in MPA, moderate in GS-I, SBA and PNA. After neuraminidase treatment the intensity of fluorescence increased significantly with PNA and moderately with SBA in the RA samples. These results suggested that the inflammatory lining cells produce galactose group and the content of neuraminic acids in the synovial membranes of the RA appears to be greater than in those of other diseases.
