Abstract

International audience; OBJECTIVE: To assess the relationship between hypertension and perceived material insecurity in a disadvantaged Caribbean population. PATIENTS AND METHODS: We used data from a cross-sectional study involving 2420 disadvantaged subjects aged 18-69 years, included consecutively at three Guadeloupian health centers. Hypertension was diagnosed over two consultations with a total of six blood pressure measurements. Perceived material insecurity was assessed using a closed-ended question. RESULTS: The prevalence of hypertension was estimated at 17.7% among subjects who believed their material situation would improve in the future, at 28.2% among those who believed it would remain the same, and at 43.3% among those who believed it would deteriorate. A multivariate logistic regression analysis showed that hypertension risk more than doubled (OR: 2.35 - P: 0.002) among subjects who believed that their material situation would deteriorate in the future compared to those who believed that their situation would improve, with no significant sex-related differences. 87. Achieved anticoagulation vs prosthesis selection for mitral mechanical valve replacement: a population-based outcome study.

Abstract

Background: Thromboembolic events (TEs) are frequent after mechanical mitral valve replacement (MVR), but their association to anticoagulation quality is unclear and has never been studied in a population-based setting with patients who have a complete anticoagulation record.Methods: We compiled a complete record of all residents of Olmsted County, MN, who underwent mechanical MVR between 1981 and 2004, for all TE, bleeding episodes, and international normalized ratios (INRs) measured from prosthesis implantation.Results: In the 112 residents (mean [+/- SD] age, 57 +/- 16 years; 60% female residents) who underwent mechanical MVR, 19,647 INR samples were obtained. While INR averaged 3.02 +/- 0.57, almost 40% of INRs were < 2 or > 4.5. Thirty-four TEs and 28 bleeding episodes occurred during a mean duration of 8.2 +/- 6.1 years of follow-up. There was no trend of association of INR (average, SD, growth variance rate, or intensity-specific incidence of events) with TE. Previous cardiac surgery (p = 0.014) and ball prosthesis (hazard ratio [HR], 2.92; 95% CI, 1.43 to 5.94; p = 0.003) independently determined TE. With MVR using a ball prosthesis, despite higher anticoagulation intensity (p = 0.002), the 8-year rate of freedom from TE was considerably lower (50 +/- 9% vs 81 +/- 5%, respectively; p < 0.0001). Compared with expected stroke rates in the population, stroke risk was elevated with non-ball prosthesis MVR (HR 2.6; 95% CI, 1.3 to 5.2; p = 0.007) but was considerable with ball prosthesis MVR (HR 11.7; 95% CI, 7.5 to 18.4; p < 0.0001). INR variability (SD) was higher with a higher mean INR value (p < 0.0001). INR variability (HR 2.485; 95% CI, 1.11 to 5.55; p = 0.027) and cancer history (p < 0.0001) independently determined bleeding rates.Conclusion: This population-based comprehensive study of anticoagulation and TE post-MVR shows that, in these closely anticoagulated patients, anticoagulation intensity was highly variable and not associated with TE incidence post-MVR. Higher anticoagulation intensity is linked to higher variability and, thus, to bleeding. The MVR-ball prosthesis design is associated with higher TE rates notwithstanding higher anticoagulation intensity, and its use should be retired worldwide. [ABSTRACT FROM AUTHOR]

90. Echocardiographic evaluation of patients with sickle cell disease. A study based on Etendard cohort.

Abstract

Echocardiography is the main non-invasive screening tool for pulmonary hypertension (PH) in sickle cell disease (SCD), but relies mostly on the tricuspid regurgitation velocity (TRV) assessment, without distinction between pre- and post-capillary patterns. Using Etendard Cohort data, the aim of this study was to refine echocardiographic assessment of PH in SCD patients. The French Etendard Study is a prospective cohort initially designed to assess the prevalence of PH among 398 SCD participants. We analysed echocardiography data of the 96 Etendard patients who underwent Right Heart Catheterisation (RCH), because of a TRV ≥ 2.5 m/s, and compared them to invasive haemodynamic measurements. PH was found in 24/96 patients with 11 precapillary and 13 post-capillary. In addition to TRV, multivariate analysis identified indexed left atrial volume (LAV ind) and lateral E' wave velocity (E'lat) as independent echocardiographic predictors of PH (OR = 1.06 and 0.6, respectively, P < 0.01). They both had good accuracy to predict PH with an optimal cut-off of 48 mL/m2 for LAV ind (AUC = 0.78, Se = 81% and Sp = 75%) and 12 cm/s for E'lat (AUC = 0.8, Se = 72% and Sp = 79). Using the association of a TRV ≥ 3 m/s or a TRV [2.5; 2.9] m/s with LAV ind > 48 mL/m2 and E'lat < 12 cm/s, we could predict PH with a PPV of 68% and NPV of 90%. Moreover, comparing echocardiography data of pre- and post-capillary PH patients, we observed that pulmonary acceleration time (PAcT) was different in the two groups (104 ± 22 ms vs. 160 ± 21 ms, P < 0.001) with an excellent accuracy for the differentiation of both phenotypes (AUC = 0.95, optimal cut-off = 115ms, Se = 100% and Sp = 78%). Echocardiography can accurately estimates the probability of PH in SCD patients with an integrated approach using TRV, LAV ind and E'lat. In addition, among patients with suspected PH, measuring PAcT as a surrogate of PVR allows a good differentiation between pre and post-capillary phenotypes. [ABSTRACT FROM AUTHOR]

92. Surgical treatment of subaortic stenosis after biventricular repair of double-outlet right ventricle

Abstract

Out of 180 patients who underwent biventricular repair of double-outlet right ventricle between 1980 and 1995, 9 (5%) required reoperation because of subaortic stenosis. Two other patients who initially underwent operation elsewhere underwent reoperation at our institution because of subaortic stenosis. The median age at biventricular repair was 4 months. Repair consisted of tunnel construction from the left ventricle to the aorta in nine patients; the remaining two patients received an arterial switch operation with ventricular septal defect closure. Subaortic stenosis developed with time: the mean postoperative left ventricle-to-aorta gradient after repair was 10 +/- 19 mm Hg (range, 0 to 50 mm Hg) and became 84 +/- 27 mm Hg (range, 40 to 124 mm Hg) in a mean delay of 45 +/- 66 months (range, 1 to 213 months). At reoperation, the obstruction was caused by the protrusion of the inferior rim of the ventricular septal defect into the left ventricular outflow tract associated with subaortic hypertrophied muscle and membrane. The 11 patients underwent 15 reoperations. Surgical technique consisted of an extended septoplasty in 6 reoperations. In this technique an incision was made in the septal patch and was extended into the muscle toward the apex until a large opening of the left ventricular outflow pathway was obtained. A new patch was then secured to streamline the left ventricular outflow tract. None of the patients who underwent extended septoplasty had to undergo reoperation. There were no early or late deaths. At 115 +/- 85 months after biventricular repair, all patients were in New York Heart Association functional class I or II and the mean postoperative left ventricle-to-aorta gradient was 20 +/- 24 mm Hg (range, 0 to 60 mm Hg). We conclude that after biventricular repair of double-outlet right ventricle, the subaortic region is at risk for the development of stenosis. Surgical treatment adapted to the anatomy of the obstruction can offer good early and midterm results. It seems that an aggressive approach by an extended septoplasty avoids multiple reoperations. Abstract

The object of this enquiry was to estimate the number of self monitoring devices for blood pressure measurement in Guadeloupe in 2005 and to determine how they were used. The enquiry included 509 patients aged over 35 years and living in Guadeloupe meeting the inclusion criteria of PRETRAHGUAD: PREvalence et TRAitement de Hypertension en GUADeloupe. The enquiry was made by Qualistat, a survey institute. Designed as a survey, it was based on the two degree quota method. It was carried out by telephone by CATI (Computer Assisted Telephone Inquiry) networked computer programming by ten operators trained by experts of the HTA-GWAD organisation. In addition to questions on socio-demographic background and biometric parameters, the operators also asked about observance of antihypertensive treatment, the ownership of a self monitoring blood pressure device and its usage. A subject who said he took a treatment validated as antihypertensive was considered to be a "treated hypertensive". The results indicate that in 2005, in Guadeloupe, 32% of the population over 35 years of age were treated hypertensives, making a total of 51,230 individuals. Forty three percent of treated hypertensives and 19% of other subjects admitted to having used a self monitoring blood pressure device. Twenty six percent of the study population owned a device. Of the estimated total of 43,220 devices in circulation, nearly half of them were wrist devices. Regular usage (at least once per month) was declared by 73% of hypertensive and 46% of other subjects. Out of the total population, the medical practitioner had spoken of self monitoring of blood pressure to his patients and had incited only 18% to carry it out. The authors conclude that, in 2005, one third of adults over 35 years of age were treated for hypertension and 43% of treated hypertensives owned a self monitoring blood pressure device. The incitation by medical practitioners to use this method of self monitoring remained modest. This technique would seem to be little used by medical practitioners although the patients have started to equip themselves with the device.

Abstract

Purpose: Cardiac deconditioning due to immobilization is a risk factor for cardiovascular disease. The physiology of cardiac adaptation to deconditioning has not been fully elucidated. The purpose of the present study was to assess the effects of 21-days of strict head-down (-6 degrees) bed-rest (BR) deconditioning on left ventricular (LV) dimensions and mass measured by MRI. Methods: Ten healthy men (mean age 32±6) were enrolled; the experiment was conducted at DLR (Koln, Germany) as part of the European Space Agency BR studies. Steady-state free precession MRI images (7mm thickness, no gap, no overlap) were obtained (Symphony 1.5T, Siemens) in a stack of short-axis views from LV base to LV apex, before (PRE), at the end of BR (HDT20), and four days after the BR conclusion (POST). Endocardial and epicardial semi-automated contouring was performed using freely available software (Segment). Results: At HDT20, significant reductions in LV mass (16%), end-diastolic (26%) and end-systolic (27%) volumes and stroke volume (27%) were observed, while ejection fraction did not change. These changes were accompanied by a measured decrease (14%) in plasma and blood volume (by gas-rebreathing technique), as well as by a significant reduction (14%) in VO2max aerobic power, measured using a graded cycle ergometer test protocol to volitional fatigue, at one day after the BR conclusion, while expiratory exchange ratio did not change. At POST, LV volumes were restored, while LV mass was still trending towards control values. Conclusions: Cardiac adaptation to deconditioning affected LV mass and dimensions, as a combined result of LV remodeling and fluids loss, accompanied by worsening in aerobic power. Abstract

Background The European Atherosclerosis Society Familial Hypercholesterolaemia Studies Collaboration (FHSC) global registry provides a platform for the global surveillance of familial hypercholesterolaemia through harmonisation and pooling of multinational data. In this study, we aimed to characterise the adult population with heterozygous familial hypercholesterolaemia and described how it is detected and managed globally. Methods Using FHSC global registry data, we did a cross-sectional assessment of adults (aged 18 years or older) with a clinical or genetic diagnosis of probable or definite heterozygous familial hypercholesterolaemia at the time they were entered into the registries. Data were assessed overall and by WHO regions, sex, and index versus non-index cases. Findings Of the 61 612 individuals in the registry, 42 167 adults (21 999 [53.6%] women) from 56 countries were included in the study. Of these, 31 798 (75.4%) were diagnosed with the Dutch Lipid Clinic Network criteria, and 35 490 (84.2%) were from the WHO region of Europe. Median age of participants at entry in the registry was 46.2 years (IQR 34.3-58.0); median age at diagnosis of familial hypercholesterolaemia was 44.4 years (32.5-56.5), with 40.2% of participants younger than 40 years when diagnosed. Prevalence of cardiovascular risk factors increased progressively with age and varied by WHO region. Prevalence of coronary disease was 17.4% (2.1% for stroke and 5.2% for peripheral artery disease), increasing with concentrations of untreated LDL cholesterol, and was about two times lower in women than in men. Among patients receiving lipid-lowering medications, 16 803 (81.1%) were receiving statins and 3691 (21.2%) were on combination therapy, with greater use of more potent lipid-lowering medication in men than in women. Median LDL cholesterol was 5.43 mmol/L (IQR 4.32-6.72) among patients not taking lipid-lowering medications and 4.23 mmol/L (3.20-5.66) among those taking them. Among patients taking lipid-lowering medications, 2.7% had LDL cholesterol lower than 1.8 mmol/L; the use of combination therapy, particularly with three drugs and with proprotein convertase subtilisin-kexin type 9 inhibitors, was associated with a higher proportion and greater odds of having LDL cholesterol lower than 1.8 mmol/L. Abstract

In previous studies, we have reported that angiotensin II (Ang II) induced occurrence of spontaneous calcium releases in rat cardiac myocytes, with an increased sensitivity of hypertrophied myocytes. In this study, valsartan, a non-peptide antagonist of Ang II ATl receptor subtype was used to assess the involvement of the ATl receptors in Ca2+ release phenomena Ang II dependant (10-7M). Freshly isolated left ventricular myocytes from normal and hypertensive adult rats (RHR) were used. The variation of the ratio of Indo-1 emission (405nm/480nm) was taken as an index of intracellular Ca2+ variations on unstimulated or stimulated myocytes. Five parameters were measured from each Ca2+ transient: amplitude, duration with a rise time and a fall time, and frequency of spontaneous calcium releases. From each cell, results are expressed as a percent change in baseline values.

Abstract

Transthyretin amyloidosis (ATTR) is a severe and rare disease characterized by the progressive deposition of misfolded transthyretin proteins, causing irreversible organ damage. Transthyretin amyloidosis can present as a hereditary ATTR or acquired wild-type ATTR form. Genetic testing is critical for determining a hereditary predisposition and subsequently initiating appropriate family screening. In France, strict regulations govern genetic testing that aim to protect patients and their families affected by hereditary diseases such as ATTR. However, challenges persist in establishing an effective genetic testing pathway. A multidisciplinary group of French experts convened to discuss the challenges associated with an ATTR genetic diagnosis and to propose improvement strategies. Key challenges include the lack of pathway standardization, communication gaps between healthcare professionals (HCPs) and patients, and difficulties in complying with regulatory requirements. Concerns about patient data safety and outsourced testing quality further complicate matters. Proposed strategies included the development of stakeholder mapping tools for HCPs and patients, educational programs to improve literacy on genetic testing regulations, increase disease awareness among medical geneticists and genetic counselors, and strengthening HCP-patient communication through educational materials. These initiatives aim to streamline the genetic testing pathway, enhance compliance with regulations, and ultimately provide optimal support for patients and families with ATTR., (© 2024. Abstract

Type I interferon (IFN-I) plays an important role in the innate immune response through inducing IFN-I-stimulated genes (ISGs). However, how alternative splicing (AS) events, especially over time, affect their function remains poorly understood. We generated an annotation (113,843 transcripts) for IFN-I-stimulated human B cells called isoISG using high-accuracy long-read sequencing data from PacBio Sequel II/IIe. Transcript isoform profiling using isoISG revealed that isoform switching occurred in the early response to IFN-I so that ISGs would gain functional domains (e.g., C4B) or higher protein production (e.g., IRF3). Conversely, isoforms lacking functional domains increased during the late phase of IFN-I response, mainly due to intron retention events. This suggests that isoform switching both triggers and terminates IFN-I responses at the translation and protein levels. Furthermore, genetic variants influencing the isoform ratio of ISGs were associated with immunological and infectious diseases. 100. Clinical outcomes for 2788 patients with transthyretin amyloidosis: Tafamidis meglumine early access program in France.

Abstract

Background: Early access experience in France with tafamidis meglumine, a selective transthyretin stabilizer for transthyretin-related amyloidosis cardiomyopathy (ATTR-CM), following transthyretin-related amyloidosis (ATTR) polyneuropathy approval and positive ATTR-ACT study results., Aim: To describe the characteristics and clinical outcomes for patients in the French ATTR-CM tafamidis meglumine early access programme (28 Nov 2018 to 01 Jun 2021)., Methods: Patients with confirmed ATTR-CM received tafamidis meglumine 20mg/day or 80mg/day. Demographic and clinical data were collected prospectively until patients discontinued treatment or died, or the programme ended., Results: Overall, 222 physicians from 126 centres enrolled 2788 patients. The median age was 82years, 81.6% were male and New York Heart Association severity was class I for 12.8%, class II for 60.1% and class III for 27.0%. Overall, 1943 (74.6%) had genetic testing, and the results were available at tafamidis start for 1208 (62.2%) patients: 995 (82.4%) had wild-type ATTR and 213 (17.6%) had hereditary ATTR. Most patients started treatment≤12months after diagnosis (88.3%): 2268 (81.3%) at 20mg/day, with 401 (17.7%) increasing to 80mg/day. Median follow-up duration was 11.8months. New York Heart Association class improved or remained stable for 1299 (77.6%), whereas 376 (22.4%) worsened between inclusion and last follow-up. Among patients initiated at 80mg, 297 (81.1%) improved or remained stable and 69 (18.9%) worsened. New York Heart Association class progression did not vary with age. The 18-month survival rates were 89.8% (95% confidence interval: 87.0-92.0) among patients aged<80years, and 86.5% (95% confidence interval: 83.9-88.7) among those aged≥80years., Conclusions: Early tafamidis meglumine access was given to 2788 patients with ATTR-CM.
