5G millimeter wave (mmWave) cellular networks have been reported to deliver 1-2 Gbps downlink throughput, via speed-tests. However, these speed-tests capture only a few seconds of throughput and are not representative of sustained throughput over several minutes. We report the first measurements of sustained throughput in three cities, Miami, Chicago, and San Francisco, where we observe throughput throttling due to rising skin temperature of the phone when it is connected to a deployed 5G mmWave base-station (BS). Radio Resource Control (RRC) messaging between the phone and the BS indicates the reduction in the number of aggregated mmWave channels from 4 to 1 followed by a switch to 4G. We corroborate these measurements with infra-red images as the phone heats up. Thus, mmWave throughput will be limited not by network characteristics but by device thermal management.

Coronavirus disease 2019 (COVID-19)-associated invasive fungal infections are an important complication in a substantial number of critically ill, hospitalized patients with COVID-19. Three groups of fungal pathogens cause co-infections in COVID-19: Aspergillus, Mucorales and Candida species, including Candida auris. Here we review the incidence of COVID-19-associated invasive fungal infections caused by these fungi in low-, middle- and high-income countries. By evaluating the epidemiology, clinical risk factors, predisposing features of the host environment and immunological mechanisms that underlie the pathogenesis of these co-infections, we set the scene for future research and development of clinical guidance.

Invasive pulmonary aspergillosis (IPA), invasive mucormycosis (IM), and invasive fusariosis (IF) are associated with high mortality and morbidity. Fosmanogepix (FMGX) is a first-in-class antifungal in clinical development with demonstrated broad-spectrum activity in animal models of infections. We sought to evaluate the benefit of combination therapy of FMGX plus liposomal amphotericin B (L-AMB) in severe delayed-treatment models of murine IPA, IM, and IF. While FMGX was equally as effective as L-AMB in prolonging the survival of mice infected with IPA, IM, or IF, combination therapy was superior to monotherapy in all three models. These findings were validated by greater reductions in the tissue fungal burdens (determined by quantitative PCR) of target organs in all three models versus the burdens in infected vehicle-treated (placebo) or monotherapy-treated mice. In general, histopathological examination of target organs corroborated the findings for fungal tissue burdens among all treatment arms. Our results show that treatment with the combination of FMGX plus L-AMB demonstrated high survival rates and fungal burden reductions in severe animal models of invasive mold infections, at drug exposures in mice similar to those achieved clinically. These encouraging results warrant further investigation of the FMGX-plus-L-AMB combination treatment for severely ill patients with IPA, IM, and IF.

BackgroundImmune checkpoint inhibitors (ICI) have improved overall survival in patients with different cancer types. However, treatment efficacy varies between patients depending on several factors. Recent research suggested that antibiotic-induced dysbiosis can impair ICI efficacy. Here we review the impact of antibiotic use in clinical outcome of patients with gastrointestinal cancer treated with ICI.MethodsThis is a systematic review and utilized a thorough search of MEDLINE, Cochrane, Scopus, EB-SCO, Web of Science of studies published till September 2023. The aim of the study is to determine the association between antibiotic use and ICI treatment efficacy in patients with gastrointestinal cancers (GI). We utilized a meta-analysis of the association between the use of antibiotics and overall survival and progression-free survival.ResultsNine studies met the inclusion criteria with a total of 2,214 patients. The most common type of cancers was hepatocellular carcinoma (HCC). The majority of the studies were retrospective, and one was collective of clinical trials. The use of antibiotics was associated with decreased both overall survival [haz-ard ratio (HR) 1.92, 95% confidence interval (CI) 1.41, 2.63] and progression-free survival [HR 1.81, 95% CI 1.29, 2.54].ConclusionThe use of antibiotics may affect clinical outcomes in patients with GI cancers treated with ICI. Cellular operators have begun deploying 5G New Radio (NR) in all available bands: low (< 1 GHz), mid (1 - 6 GHz), and high (> 24 GHz) to exploit the different capabilities of each. At the same time, traditional 4G Long Term Evolution (LTE) deployments are being enhanced with the addition of bands in the unlicensed 5 GHz (using License Assisted Access, or LAA) and the 3.5 GHz Citizens Broadband Radio Service (CBRS) resulting in throughput performance comparable to 5G in mid-band. We present a detailed study comparing 4G and 5G deployments, in all bands in Chicago, and focused mmWave measurements and analysis in Miami. Our methodology, based on commercial and custom apps, is scalable for crowdsourcing measurements on a large scale and provides detailed data (throughput, latency, signal strength, etc.) on actual deployments. Our main conclusions based on the measurements are (i) optimized 4G networks in mid-band are comparable in both throughput and latency to current deployments of 5G (both standalone (SA) and non-standalone (NSA)) and (ii) mmWave 5G, even in NSA mode, can deliver multi-Gbps throughput reliably if the installation is dense enough, but performance is still brittle due to the propagation limitations imposed by distance and body loss. Learning the covariance matrices of spatially-correlated wireless channels, in millimeter-wave (mmWave) vehicular communication, can be utilized in designing environmen-taware beamforming codebooks. Such channel covariance matrices can be represented on non-Euclidean Riemannian manifolds, thanks to their symmetric positive definite (SPD) characteristics. Consequently in this paper, we propose a Riemannian-Geometric machine learning (G-ML) approach for estimating the channel covariance matrices based on unsupervised K-Means model. The proposed K-means algorithm utilizes Log-Euclidean metric (LEM) as the distance measure among channel covariance matrices over the Riemannian manifolds. Molds are ubiquitous in the environment, and immunocompromised patients are at substantial risk of morbidity and mortality due to their underlying disease and the resistance of pathogenic molds to currently recommended antifungal therapies. This combination of weakened-host defense, with limited antifungal treatment options, and the opportunism of environmental molds renders patients at risk and especially vulnerable to invasive mold infections such as Aspergillus and members of the Order Mucorales. Currently, available antifungal drugs such as azoles and echinocandins, as well as combinations of the same, offer some degree of efficacy in the prevention and treatment of invasive mold infections, but their use is often limited by drug resistance mechanisms, toxicity, drug-drug interactions, and the relative paucity of oral treatment options. Clearly, there is a need for agents that are of a new class that provides adequate tissue penetration, can be administered orally, and have broad-spectrum efficacy against fungal infections, including those caused by invasive mold organisms. Ibrexafungerp, an orally bioavailable glucan synthase inhibitor, is the first in a new class of triterpenoid antifungals and shares a similar target to the well-established echinocandins. Ibrexafungerp has a very favorable pharmacokinetic profile for the treatment of fungal infections with excellent tissue penetration in organs targeted by molds, such as the lungs, liver, and skin. Ibrexafungerp has demonstrated in vitro activity against Aspergillus spp. as well as efficacy in animal models of invasive aspergillosis and mucormycosis. Furthermore, ibrexafungerp is approved for use in the USA for the treatment of women with vulvovaginal candidiasis. Ibrexafungerp is currently being evaluated in clinical trials as monotherapy or in combination with other antifungals for treating invasive fungal infections caused by yeasts and molds. Thus, ibrexafungerp offers promise as a new addition to the clinician's armamentarium against these difficult-to-treat infections.

Candida albicans biofilms are a complex multilayer community of cells that are resistant to almost all classes of antifungal drugs. The bottommost layers of biofilms experience nutrient limitation where C. albicans cells are required to respire. We previously reported that a protein Ndu1 is essential for Candida mitochondrial respiration; loss of NDU1 causes inability of C. albicans to grow on alternative carbon sources and triggers early biofilm detachment. Here, we screened a repurposed library of FDA-approved small molecule inhibitors to identify those that prevent NDU1-associated functions. We identified an antihelminthic drug, Niclosamide (NCL), which not only prevented growth on acetate, C. albicans hyphenation and early biofilm growth, but also completely disengaged fully grown biofilms of drug-resistant C. albicans and Candida auris from their growth surface. To overcome the suboptimal solubility and permeability of NCL that is well known to affect its in vivo efficacy, we developed NCL-encapsulated Eudragit EPO (an FDA-approved polymer) nanoparticles (NCL-EPO-NPs) with high niclosamide loading, which also provided long-term stability. The developed NCL-EPO-NPs completely penetrated mature biofilms and attained anti-biofilm activity at low microgram concentrations. NCL-EPO-NPs induced ROS activity in C. albicans and drastically reduced oxygen consumption rate in the fungus, similar to that seen in an NDU1 mutant. NCL-EPO-NPs also significantly abrogated mucocutaneous candidiasis by fluconazole-resistant strains of C. albicans, in mice models of oropharyngeal and vulvovaginal candidiasis. To our knowledge, this is the first study that targets biofilm detachment as a target to get rid of drug-resistant Candida biofilms and uses NPs of an FDA-approved nontoxic drug to improve biofilm penetrability and microbial killing.

During hematogenously disseminated candidiasis, blood borne fungi must invade the endothelial cells that line the blood vessels to infect the deep tissues. Although Candida albicans, which forms hyphae, readily invades endothelial cells, other medically important species of Candida are poorly invasive in standard in vitro assays and have low virulence in immunocompetent mouse models of disseminated infection. Here, we show that Candida glabrata, Candida tropicalis, Candida parapsilosis, and Candida krusei can bind to vitronectin and high molecular weight kininogen present in human serum. Acting as bridging molecules, vitronectin and kininogen bind to αv integrins and the globular C1q receptor (gC1qR), inducing human endothelial cells to endocytose the fungus. This mechanism of endothelial cell invasion is poorly supported by mouse endothelial cells but can be restored when mouse endothelial cells are engineered to express human gC1qR or αv integrin. Overall, these data indicate that bridging molecule-mediated endocytosis is a common pathogenic strategy used by many medically important Candida spp. to invade human vascular endothelial cells.

In this paper, we propose a novel graph kernel method for the wireless link scheduling problem in device-to-device (D2D) networks on Riemannian manifold. The link scheduling problem can be considered as a binary classification problem since each D2D pair can only hold the state active or inactive. Our goal is to learn a novel metric that facilitates the design of an efficient but less computationally demanding machine learning (ML) solution for the binary classification task of link scheduling problem that requires no channel state information (CSI) and a fewer number of training samples as opposed to other benchmark ML algorithms. To this aim, we first represent the wireless D2D network as a graph and model the features of each D2D pair, including its communication and interference links, as regularized (i.e., positively-shifted) Laplacian matrices which are symmetric positive definite (SPD) one. By doing so, we represent the feature information of each D2D pair as a point on the SPD manifold, and we analyze the topology through Riemannian geometry. We compute the Riemannian metric, e.g., Log-Euclidean metric (LEM), which are suitable distance measures between the regularized Laplacian matrices. The LEM is then utilized to define a positive definite graph kernel for the binary classification of the link scheduling decisions. Simulation results demonstrate that the proposed graph Kernel-based method is computationally less demanding and achieves a sum rate of more than 95% of benchmark algorithm FPLinQ [1] for 10 D2D pairs without using CSI and less than a hundred training network layouts.

In this paper, we investigate the fundamental problem of wireless link scheduling in device-to-device (D2D) networks, through the lens of Riemannian geometry. Our goal is to find a novel metric to characterize interference among D2D pairs, which can pave the way towards efficient and fast scheduling algorithms. Towards achieving this goal, we first model the connectivity pattern of each D2D pair, including its interference links, as a positively-shifted Laplacian matrix, which is a symmetric positive definite (SPD) one. Noting that SPD matrices constitute a non-Euclidean manifold, we represent each of the D2D pairs as a point on the SPD (i.e., conic) manifold, which is analyzed via Riemannian geometry. Accordingly we employ Riemannian metrics (e.g., Log-Euclidean metric "LEM"), which are suitable measures of distances on manifolds, to characterize the interference among D2D points on the SPD manifold. To validate the effectiveness of the proposed LEM-based interference measure, we propose a sequential link selection algorithm that schedules D2D pairs in a descending order of their signal-to-noise ratio (SNR), while keeping their LEM distances towards the already-scheduled pairs on the Riemannian manifold to be greater than a certain LEM threshold. Such LEM-based condition is equivalent to limiting the interference from potential D2D pairs to be below certain threshold.
