The present study investigated the involvement of human papillomavirus (HPV)16 and HPV18 in oropharyngeal malignancies in order to understand the oncogenic mechanisms, and to identify biomarkers for early detection and treatment targets. Given the rising incidence of HPV-associated cancer, particularly in India, this holds significance in elucidating the molecular basis of these diseases. Structural validation of HPV16 and 18 oncoproteins E6 and E7 was conducted using computational tools, while gene expression profiles related to oral squamous cell carcinoma (OSCC) were analyzed to assess differential expression. The presence of HPV in patient tissue sections was examined using reverse transcription-PCR. The present study revealed the interactions of HPV16 and 18 E6/E7 oncoproteins, highlighting their role in cancer progression by targeting key tumor suppressors, such as p53 and retinoblastoma protein. Further analysis demonstrated the involvement of HPV16 and 18 E6/E7 oncoproteins in cancer pathways, signaling and telomere regulation, which supports the development of future targeted therapies. HPV16 and 18 E6/E7 represent promising therapeutic targets in OSCC, and provide further insights into potential diagnostic and treatment avenues. Abstract Background The dual-bolus protocol enables accurate quantification of myocardial blood flow (MBF) by first-pass perfusion cardiovascular magnetic resonance (CMR). However, despite the advantages and increasing demand for the dual-bolus method for accurate quantification of MBF, thus far, it has not been widely used in the field of quantitative perfusion CMR. The main reasons for this are that the setup for the dual-bolus method is complex and requires a state-of-the-art injector and there is also a lack of post processing software. As a solution to one of these problems, we have devised a universal dual-bolus injection scheme for use in a clinical setting. The purpose of this study is to show the setup and feasibility of the universal dual-bolus injection scheme. Methods The universal dual-bolus injection scheme was tested using multiple combinations of different contrast agents, contrast agent dose, power injectors, perfusion sequences, and CMR scanners. This included 3 different contrast agents (Gd-DO3A-butrol, Gd-DTPA and Gd-DOTA), 4 different doses (0.025 mmol/kg, 0.05 mmol/kg, 0.075 mmol/kg and 0.1 mmol/kg), 2 different types of injectors (with and without "pause" function), 5 different sequences (turbo field echo (TFE), balanced TFE, k-space and time (k-t) accelerated TFE, k-t accelerated balanced TFE, turbo fast low-angle shot) and 3 different CMR scanners from 2 different manufacturers. The relation between the time width of dilute contrast agent bolus curve and cardiac output was obtained to determine the optimal predefined pause duration between dilute and neat contrast agent injection. Results 161 dual-bolus perfusion scans were performed. Three non-injector-related technical errors were observed (1.9%). No injector-related errors were observed. The dual-bolus scheme worked well in all the combinations of parameters if the optimal predefined pause was used. Linear regression analysis showed that the optimal duration for the predefined pause is 25s to separate the dilute and neat contrast agent bolus curves if 0.1 mmol/kg dose of Gd-DO3A-butrol is used. Conclusion The universal dual-bolus injection scheme does not require sophisticated double-head power injector function and is a feasible technique to obtain reasonable arterial input function curves for absolute MBF quantification.

Background: The misuse of and addiction to opioids is a national crisis that affects public health as well as social and economic welfare. There is an urgent need for strategies to improve opioid use disorder treatment quality (e.g., 6-month retention). Substance use disorder treatment programs are challenged by limited resources and a workforce that does not have the requisite experience or education in quality improvement methods. The purpose of this study is to test a multicomponent clinic-level intervention designed to aid substance use disorder treatment clinics in implementing quality improvement processes to improve high-priority indicators of treatment quality for opioid use disorder.Methods: A stepped-wedge randomized controlled trial with 30 outpatient treatment clinics serving approximately 2000 clients with opioid use disorder each year will test whether a clinic-level measurement-driven, quality improvement intervention, called Coaching for Addiction Recovery Enhancement (CARE), improves (a) treatment process quality measures (use of medications for opioid use disorder, in-treatment symptom and therapeutic progress, treatment retention) and (b) recovery outcomes (substance use, health, and healthcare utilization). The CARE intervention will have the following components: (1) staff clinical training and tools, (2) quality improvement and change management training, (3) external facilitation to support implementation and sustainability of quality improvement processes, and (4) an electronic client-reported treatment progress tool to support data-driven decision making and clinic-level quality measurement. The study will utilize multiple sources of data to test study aims, including state administrative data, client-reported survey and treatment progress data, and staff interview and survey data.Discussion: This study will provide the field with a strong test of a multicomponent intervention to improve providers' capacity to make systematic changes tied to quality metrics. The synthesized thioureas, 1-(3-fluorobenzoyl))-3-(4-(diethyl aminophenyl) thiourea (I1), 1-benzoyl-3-(2-chlorophenyl) thiourea (I2) and 1-(2-fluorobenzoyl)-3-(2-chlorophenyl) thiourea (I3) along with triphenylphosphine were reacted with Cu(I) chloride in mole ratios 1:2:1 by using dry acetone as solvent under nitrogen to get 1–3. The synthesized thioureas and metal derivatives were characterized by spectroscopic techniques such as IR and multinuclear (1H, 13C) NMR. Compound 3 is analyzed by single crystal X-ray analysis and data reveal that the Cu is four coordinate having tetrahedral molecular geometry. The interaction of 1–3 with DNA is ascertained by cyclic voltammetry, determining binding constant, binding energy and diffusion coefficient. The findings suggest that the complexes interact with DNA in an electrostatic mode. The antioxidant activity data show that 3 has the highest free radical scavenging ability having Ic50 value of 10 µg/mL. The synthesized compounds were also screened against various bacterial strains and found some encouraging results. The binding interactions of the metal complexes with a specific protein were further validated by molecular docking studies and the results obtained show their strong interaction with amino acid residue in the binding pocket of the target protein. Introduction: Orthostatic hypotension (OH) and atrial fibrillation (AF) are both regarded as independent risk factors for transient ischemic attack (TIA). However, the clinical implication of OH in the presence of AF is unclear. This study investigates, for the first time, the association between blood pressure (BP), OH and mortality in a cohort of patients with AF and TIA symptoms. Aim: To investigate the incidence of the association between OH, AF and TIA. Methods: This retrospective observational study utilised the Leicester one-stop transient TIA clinic patient database to consider the initial systolic and diastolic BP of 688 patients with a diagnosis of AF. The primary outcome was time until death. Covariant measures included status of AF diagnosis (known or new AF), cardiovascular risk factors, and primary clinic diagnosis [cerebrovascular (CV) versus non-cerebrovascular (non-CV)]. Statistical models adjusted for sex, age, previous AF diagnosis. Results: Mortality rate was higher in the over 85 age group [191.5 deaths per 1000 person years (py) (95% CI 154.0–238.1)] and lower in the aged 75 and younger age group [40.0 deaths per 1000 py (95% CI 27.0–59.2)] compared to intermediate groups. A 10 mmHg increase in supine diastolic BP was associated with a significant reduction in the hazard of mortality for patients suspected of TIA with AF [adjusted HR 0.79 (95% CI 0.68–0.92), p < 0.001]. The mortality rate for patients with OH was 119.0 deaths per 1000 py compared with a rate of 98.0 for patients without OH (rate ratio 1.2, p = 0.275). Conclusion: Higher diastolic BP may be a marker for reduced mortality risk in patients with a previous AF diagnosis and non-CV diagnosis. Lower diastolic BP and the presence of AF pertain to a higher mortality risk. This study raises the importance of opportunistic screening for both OH and AF in patients presenting to TIA clinic. New instruments are needed to assist substance use disorder (SUD) clinics in monitoring client treatment progress. This paper describes the development of an 8-item monitoring tool and results from a pilot to understand the tool's structure and reliability. 393 clients completed the tool upon treatment entry. A factor analysis resulted in a 2-factor solution: SUD symptoms and treatment progress. Multiple regression analyses showed that clients in inpatient detoxification had lower scores than those in less intensive levels of care. Acceptable internal consistency reliability was found. This study shows promise for the tool and sets the stage for future validation work. [ABSTRACT FROM AUTHOR]
