72. Clinical outcome when left atrial posterior wall box isolation is included as a catheter ablation strategy in patients with persistent atrial fibrillation.

Abstract

Purpose: Isolation of the pulmonary veins alone (PVI) is associated with a 50 to 70 % success rate in paroxysmal atrial fibrillation (AF) but is significantly lower for persistent AF. We sought to evaluate patient outcomes in terms of safety and efficacy when posterior left atrial box isolation is included as a catheter ablation strategy in patients with mainly persistent AF. Methods: We performed an audit of 100 patients undergoing left atrial (LA) box isolation. Recurrence of arrhythmia was detected by evaluating symptoms and continuous 24 h ECG monitoring at 2, 6 and 12 months post procedure. Results: Seventy-two patients had persistent AF prior to procedure. Average duration of AF was 5.4 ± 5.2 years. All patients underwent circumferential PVI plus linear posterior LA lines to complete box isolation. At a mean follow-up of 12.5 ± 4.2 months, 75 patients were free from atrial fibrillation, 50.6 % of these were taking no antiarrhythmic medication. Twenty-five patients had recurrence of AF, 84 % of whom had previous persistent AF. The average time to recurrence post procedure was 5.9 ± 4.4 months. Thirteen patients underwent repeat procedures for recurrent AF. There were no adverse events relating to the procedure. Conclusion: These results suggest that the strategy of left atrial box isolation is safe and effective, worthy of further evaluation in a multicentre registry. [ABSTRACT FROM AUTHOR]

Abstract

Background:Currently there is no consensus regarding indications and timing of permanent pacemaker implantation (PPI) following transcatheter aortic valve replacement (TAVR). Despite all patients receiving a balloon-expandable valve platform with the same inclusion criteria and no baseline electrocardiographic exclusion criteria, the rates of new PPI ranged from 0% to 33.3% at 6 low (<100 TAVR/year), 4 medium (100 to 200 TAVR/year), and 3 high-volume (>200 TAVR/year) centers. We sought to understand this variability by comparing peri-procedural ECG characteristics and PPI rates at centres with different procedural volumes.Methods:As per protocol, patients could be discharged home with a new intraventricular conduction delay in the absence of a new high-grade AV block if the length of the QRS was stable or decreasing after 24 hrs. PPI indication and timing was at the discretion of the individual site. Of the total cohort, 61 patients had pre-existing pacemakers and were excluded, resulting in a final cohort of 350 patients.Results:The overall PPI rate was 6.6% (23) in pacemaker-na?ve patients. The rate of new PPI was similar at high (4.6%), medium (9.9%) and low (7.1%) volume centres (p=0.32). The baseline ECG characteristics of the total cohort were: 209 sinus rhythm (59.7%), 61 atrial fibrillation (17.4%), 35 LBBB (10%), and 45 RBBB (12.9%). In patients with no pre-existing BBB, the rate of new LBBB following TAVR was 28.1%, of which 48.7% had resolution of their LBBB prior to discharge. The rate of BBB was similar by site volume at both baseline (High: 25%, Medium: 22.5%, Low: 20.5%, P=0.67) and immediately post TAVR (High: 36.2%, Medium: 45%, Low: 47.2%, P=0.15). Of the total cohort, 16 (4.6%) had PPI during index admission, and 7 (2%) had PPI within 15 days post procedure with no associated harm. In patients that received PPI, the proportion of patients who achieved next day discharge was 39.1% versus 80.1% in the overall cohort (p<0.001).Conclusion:Although there was a heterogenous approach to new conduction disorders, the peri-procedural electrocardiographic characteristics and rates of PPI were similar when assessed by hospital volume. A consistent approach with adherence to the Vancouver 3M Clinical Pathway may further improve outcomes and reduce LOS.

Abstract

Objective: To implement detailed EU cardiac computed tomography angiography (CCTA) quality criteria in the multicentre DISCHARGE trial (FP72007-2013, EC-GA 603266), we reviewed image quality and adherence to CCTA protocol and to the recommendations of invasive coronary angiography (ICA) in a pilot study., Materials and Methods: From every clinical centre, imaging datasets of three patients per arm were assessed for adherence to the inclusion/exclusion criteria of the pilot study, predefined standards for the CCTA protocol and ICA recommendations, image quality and non-diagnostic (NDX) rate. These parameters were compared via multinomial regression and ANOVA. If a site did not reach the minimum quality level, additional datasets had to be sent before entering into the final accepted database (FADB)., Results: We analysed 226 cases (150 CCTA/76 ICA). The inclusion/exclusion criteria were not met by 6 of the 226 (2.7%) datasets. The predefined standard was not met by 13 of 76 ICA datasets (17.1%). This percentage decreased between the initial CCTA database and the FADB (multinomial regression, 53 of 70 vs 17 of 75 [76%] vs [23%]). The signal-to-noise ratio and contrast-to-noise ratio of the FADB did not improve significantly (ANOVA, p = 0.20; p = 0.09). The CTA NDX rate was reduced, but not significantly (initial CCTA database 15 of 70 [21.4%]) and FADB 9 of 75 [12%]; p = 0.13)., Conclusion: We were able to increase conformity to the inclusion/exclusion criteria and CCTA protocol, improve image quality and decrease the CCTA NDX rate by implementing EU CCTA quality criteria and ICA recommendations., Key Points: • Failure to meet protocol adherence in cardiac CTA was high in the pilot study (77.6%). • Image quality varies between sites and can be improved by feedback given by the core lab. • Conformance with new EU cardiac CT quality criteria might render cardiac CTA findings more consistent and comparable.

77. Is There a Role for Bare-Metal Stents in Current STEMI Care?

Abstract

Early attempts at percutaneous coronary intervention (PCI) using balloon angioplasty were largely hampered by technical limitations. Although balloon angioplasty was moderately successful at relieving an obstruction, the procedure frequently resulted in dissections which, if uncontrolled, often led to abrupt vessel closure. Furthermore, while acute luminal gain could be impressive, a combination of elastic recoil and smooth muscle hyper-proliferation often negated the benefits of an acceptable immediate angiographic result. Elastic recoil could occur minutes to hours post-procedure resulting in acute myocardial infarction (AMI) and need for emergent coronary artery bypass grafting (CABG). This led to a pivotal development in the history of PCI with introduction of the metallic stent, which drove rapid improvements in short- and long-term procedural safety and efficacy. Stent design has been a remarkable area of technological advances with pivotal milestones including evolution of metallic architecture and introduction of the drug-eluting stent (DES). Although DES is now considered the default option for most PCI, bare-metal stents (BMS) still represent a sizeable proportion of stent procedures in some countries and in some settings may have arguable advantage.
