Recently particular interest has been shown in the relationship of the aging phenomenon to lipid peroxidation by free radicals. We studied changes in lipid peroxidation in the human placenta by measuring lipoperoxides from early pregnancy to term. In addition, we studied changes in the activities of superoxide dismutase, catalase, glutathione peroxidase and the concentration of alpha-tocopherol in the placental tissues. The degree of lipid peroxidation in placental tissue in early pregnancy was significantly higher than that in late pregnancy. Lipid peroxidation in the case of spontaneous abortion was slightly higher than that in normal pregnancy. The activities of SOD and catalase increased significantly, but glutathione peroxidase activity remained almost the same throughout development. On the other hand, the alpha-tocopherol concentration decreased gradually as gestation progressed. These results suggest that early placental tissue is functionally immature, then gradually suppresses lipoperoxide formation as pregnancy advances. The placental tissue protects the fetus from many kinds of radicals in the feto-maternal circulation.

The present study examined pregnancy-related changes in the level of lipoperoxides and antioxidative substances such as superoxide dismutase (SOD), catalase, glutathione peroxidase (GSH-Px) and vitamin E in the maternal and fetal serum, liver, lungs and placenta using Wistar rats. Pregnant rats fed a standard diet (control animals) showed an increase of lipoperoxides in the blood to a level 3 times greater than the non-pregnant level. When the rats were fed a vitamin E-deficient diet, lipoperoxides were produced in much greater amounts. Fetal blood also contained greater concentrations of lipoperoxides by the use of a vitamin E-deficient diet. However, liver and lung tissues contained lipoperoxides in essentially constant concentrations throughout non-pregnancy and pregnancy. Fetal liver and lung tissues showed higher concentrations than the maternal concentrations. Fetal blood and tissue concentrations of vitamin E reflected the maternal concentrations, and the values in vitamin E-deficient animals were as small as 0.1–0.2 of the values in normally fed animals. As a protective factor against lipid peroxidation, SOD was slightly increased in the liver tissues of pregnant control animals, but catalase and GSH-Px were significantly decreased in the organ. A similar tendency was observed in vitamin E-deficient animals.

A method for evaluating ultrashort light pulses is proposed. The temporal shape and the time-dependent phase change of the pulses is determined using self-consistent iterative calculations based on experimental data of the second-harmonic intensity autocorrelation and the spectra of the pulses. The self-phase modulation originating from the optical Kerr effect (OKE) and/or the saturable effect in the absorber dye solution is taken into account, and two parameters, the amount and the recovery time of the OKE, are introduced into the calculation of the temporal shape and the phase change of the pulses. These two parameters are chosen to give the best agreement of experimental and calculated second-harmonic intensity autocorrelation and spectra. The calculated results determined the lower limits of the recovery time of the OKE. >

The present paper describes a lightning surge calculation including the wave deformation due to corona. The corona wave deformation is represented by a linear corona model consisting of R-C parallel circuits with diodes, which is easily achieved by the electromagnetic transients program, EMTP. This simple corona model gives satisfactory accuracy compared with field test results. The lightning surge calculation was carried out using a two-conductor model consisting of a phase wire from which a back-flash occurs, and an equivalent ground wire. The two-conductor model gives almost the same accuracy as that using a multiconductor full representation model, for example an eight-conductor model for a twin-circuit line with two ground wires, with a computation time and memory less than a half those of the full representation model. It is observed from calculated results that the corona, in general, reduces voltages at towers and substations by more than 10%. The wave deformation due to the frequency-dependent effect of a transmission line generally has no significant effect on a lightning surge, and thus may be neglected in a lightning surge calculation.

The concentration of lipoperoxides in liver tissue was extensively high in the fetus and early newborn, but it decreased sharply thereafter. However, the concentrations of lipoperoxides in the blood and lung tissue were low in the fetal period, but increased after birth reaching a peak after about 10 days of life. Then they gradually decreased with development. The activities of enzymes such as superoxide dismutase, catalase and glutathione peroxidase together with the concentration of alpha-tocopherol (vitamin E) were extremely low in the fetal and early newborn periods, but increased gradually with development.

The hyperlipemia of pregnancy consists primarily of an increase in triglycerides. In this study a comparison was made of the changes in the level of serum lipid concentration and the major lipoprotein subfraction in pregnant and non-pregnant SHR and Wistar rat. The results obtained were as follows. The serum triglyceride concentration is significantly high in the last period of pregnancy, i.e. 8.9 fold in Wistar rat and 13.0 fold in SHR. The atherogenic Index in the last period of pregnancy was 0.92 in Wistar rat and 2.54 in SHR. These figures show a 1.2 fold and a 3.4 fold increase over the non-pregnant level. Lipoprotein metabolism in pregnancy is characterized by an increase in cyromycrone and VLDL which is a major carrier of triglycerides. The amount of triglycerides in cyromicrone fraction was increased 9 times in Wistar rat and 64 times in SHR, and Triglycerides in VLDL fraction was not significantly different between Wistar rat and SHR during pregnancy. From the above results, SHR is a strain which easily manifests hyperlipemia and hyperlipoproteinemia during pregnancy when compared to the Wistar rat.

Pregnant rats were fed a vitamin E-supplemented diet, and blood and liver tissue concentrations of lipoperoxides were determined in fetuses and newborn rats. For comparison, a standard diet and a vitamin E-deficient diet were given to different animal groups. Fetal rats of all three groups showed very low blood and liver tissue levels of lipoperoxides, and there were no apparent intergroup differences. However, the levels increased sharply immediately after birth in the vitamin E-deficient rats, with the blood level being 4 times greater than in the other groups 10 days after birth, and the liver level 6 times greater than in the other groups 3 days after birth. Thus, vitamin E was revealed to be an important protective factor against abnormal synthesis and accumulation of lipoperoxides in liver tissues, particularly during the early stages after birth.

This paper describes observation of lightning currents at Tokyo Skytree, which is a 634-m high freestanding broadcasting tower stands in the eastern area of central Tokyo on Kanto Plain. Direct observation of lightning current by using a pair of Rogowski coils started in the end of February 2012, and high-speed cameras were set 6 months later. Both 19 downward and 23 upward lightning flashes were observed at Tokyo Skytree from 2012 to 2014. This paper reports on updated lightning parameters, such as current and charge transfer of return strokes of downward and upward lightning. This report also describes the relationship between occurrence of downward/upward lightning and meteorological conditions. The authors investigated their meteorological conditions by data assimilation with Numerical Weather Forecasting and Analysis System (NuWFAS) developed by CRIEPI, and obtained altitudes of -10°C temperature level, where the lower charge centre frequently exists in thunderclouds. During 2012 to 2014, downward flashes to the tower occurred when the altitude of -10°C temperature level was higher than 5.5 km except at 2 cases. In contrast, except at 3 case, upward flashes occurred when the -10°C temperature level was lower than 5.5 km.

In this paper, the authors propose a concept of the voltage dip hazard map. It visualizes the magnitude and occurrence probability of instantaneous voltage dips caused by lightning in consideration of the frequency distribution of lightning and all inter-connected networks on a system. This map clarifies the characteristics of voltage dips on a system from several aspects such as magnitude and occurrence probability of voltage dips at each consumer, the distribution of them on the system, and the relationship between regional lightning frequency and voltage dips. We show an example of the map using a ten generators model and verify the availability of the proposed concept.

cAREL is a cAMP-responsive endothelial cell line carrying a luciferase reporter gene introduced by stable transfection of a luciferase enhancer trap into rabbit aortic endothelial cells. Luciferase gene expression in cAREL was stimulated 233-fold by 8-BrcAMP. Treatment with the β-adrenoceptor agonist isoproterenol induced a 7.0-fold increase in luciferase expression, which was partially blocked by either β1- or β2-adrenoceptor antagonists and totally blocked by propranolol and by a combination of β1- plus β2-adrenoceptor antagonists. Receptor stimulation was mimicked by cholera toxin, forskolin, 8-BrcAMP, and isobutylmethylxanthine but not by 8BrcGMP, dexamethasone, or phorbol 12-myristate 13-acetate. Stimulation by isoproterenol was completely blocked by H-89, a protein kinase A inhibitor. cAREL was also stimulated by A-23187, and this effect was abrogated by EGTA and H-89. cAREL is the first cAMP-sensitive endothelial cell line described, and it can be useful as a positive control, as a model for cAMP regulation, as a background to genetic introduction of receptors, as an indicator of intracellular pathway activation, and as a tool to investigate cAMP effects on other signaling pathways.

Background: Immune checkpoint inhibitors (ICIs) improved the long-term survival outcomes in patients with advanced non-small cell lung cancer (NSCLC), whereas the role of salvage surgery after ICIs was unknown. The object of this study was to investigate characteristics and outcomes of patients who underwent salvage surgery after ICIs., Methods: Retrospective chart review was performed on the basis of our multi-institutional database in search of consecutive patients who underwent salvage surgery after ICIs for initially unresectable NSCLC between 2016 and 2022. Patient characteristics, intraoperative findings, perioperative outcomes, histopathological findings, progression-free survival (PFS), and overall survival (OS) were investigated., Results: Fifteen patients with a median age of 71 years were included in the study. The surgical approach was open thoracotomy in 5 and robotic or thoracoscopic surgery in 10 patients. Resection was performed for primary lesions in 8 and metastatic lesions in 7 patients. Postoperative complication was noted in 1 patient with grade 1 phrenic nerve palsy. The median PFS was 47.9 months, and the median OS was not reached. Three-year PFS was 0% in those with metastatic lesions and 87.5% in those with primary lesions (P=0.12)., Conclusions: Salvage surgery after ICIs may be associated with low perioperative morbidity and acceptable long-term outcomes in selected patients. Background: Despite advancements in ultrasonography, locating peripheral veins for catheter placement remains a challenge in patients with altered anatomy owing to multiple surgeries. Herein, we highlight the potential of using the radial vein as an alternative site for ultrasound-guided peripheral venous catheterization., Case Presentation: We present two cases of patients with extensive surgical histories, including multiple abdominal surgeries, leading to difficult peripheral venous access. Traditional sites for peripheral venous catheterization were unsuitable due to vein narrowing or lack of visibility. In both cases, ultrasonography helped identify the radial vein as the only viable site for catheter placement. The patients underwent successful ultrasonography-guided catheterization of the radial vein without complications, facilitating medical management, including anesthesia induction and intraoperative monitoring., Conclusions: The radial vein is a feasible and safe alternative for ultrasound-guided peripheral venous access in patients where traditional venous access sites are compromised., (© 2024. Recent studies have enthusiastically examined the developmental origin of moral self-licensing, which is a tendency to act immorally after acting morally. However, it has not been considered enough how children evaluate personality traits of individuals who show moral licensing behavior and whether there is any developmental change in this evaluation. This study examined the developmental change in moral evaluation, social preference, and prediction of moral behaviors for moral licensing characters as well as moral or immoral characters. In total, 36 5- and 6-year-old children, 36 7- and 8-year-old children, and 58 university students participated in the study. The results revealed that 7- and 8-year-olds and adults evaluated moral licensing characters as more moral and likable than those who behave immorally, unlike 5- and 6-year-olds, who did not distinguish between the immoral and moral licensing characters. Importantly, 7- and 8-year-olds judged the moral licensing character as neutral in both moral evaluation and judgment of social preference, suggesting that they thought the immoral behavior was canceled out owing to prior moral behavior in the moral licensing character. However, adults still judged the moral licensing character as immoral and dislikable. Moreover, children's prediction of moral behavior for all characters showed the same tendency as moral evaluation, whereas adults' prediction was slightly different from their moral evaluation. Taken together, our findings revealed that the evaluation of individuals who show moral licensing behavior changed developmentally, and a moral licensing effect was found when evaluating others' moral traits from around 7 or 8 years of age., (Copyright © 2024 Elsevier Inc. Aim: A recombinant monoclonal antibody against the hepatitis B surface antigen glycan isomer (HBsAgGi) was newly developed using the O-glycosylated PreS2 peptide in M-HBsAg of hepatitis B virus (HBV) genotype C. However, the association between HBsAgGi and the development of hepatocellular carcinoma (HCC) during nucleoside/nucleotide analog (NA) therapy remains unknown., Methods: A total of 112 HBV genotype C-infected patients who were treated with NA were included in this study. We assessed the association between HBV markers, including HBsAgGi and other conventional markers, and the development of HCC during NA therapy., Results: Ten patients developed HCC during the follow-up period. Of the HBV markers, HBsAg (≤3.53 log IU/mL; p = 0.047), HBsAgGi/HBsAg ratio (≥1.10; p = 0.035), and HBV DNA (≤6.3 log copies/mL; p = 0.012) at baseline and HBsAg (≤3.19 log IU/mL; p = 0.033) and HBsAgGi/HBsAg ratio (≥1.09; p = 0.003) at 48 weeks after NA therapy were significantly associated with the development of HCC according to the log rank test. In contrast, no significant association was observed between HBsAgGi and the development of HCC. Multivariate analysis revealed that a platelet count at baseline ≤88 × 10
3 /mm3 (p = 0.026; hazard ratio [HR], 10.577) and an HBsAgGi/HBsAg ratio at 48 weeks after NA therapy ≥1.09 (p = 0.040; HR, 10.099) were independently and significantly associated with the development of HCC., Conclusions: Our findings suggest that a combination of on-treatment HBsAgGi and HBsAg predicts the development of HCC during NA therapy., (© 2024 Japan Society of Hepatology.)- Published

89. The Neural Basis of a Cognitive Function That Suppresses the Generation of Mental Imagery: Evidence from a Functional Magnetic Resonance Imaging Study.

- Abstract
This study elucidated the brain regions associated with the perception-driven suppression of mental imagery generation by comparing brain activation in a picture observation condition with that in a positive imagery generation condition. The assumption was that mental imagery generation would be suppressed in the former condition but not in the latter. The results show significant activation of the left posterior cingulate gyrus (PCgG) in the former condition compared to in the latter condition. This finding is generally consistent with a previous study showing that the left PCgG suppresses mental imagery generation. Furthermore, correlational analyses showed a significant correlation between the activation of the left PCgG and participants' subjective richness ratings, which are a measure of the clarity of a presented picture. Increased activity in the PCgG makes it more difficult to generate mental imagery. As visual perceptual processing and visual imagery generation are in competition, the suppression of mental imagery generation leads to enhanced visual perceptual processing. In other words, the greater the suppression of mental imagery, the clearer the presented pictures are perceived. The significant correlation found is consistent with this idea. The current results and previous studies suggest that the left PCgG plays a role in suppressing the generation of mental imagery.

90. Pulmonary cyst newly formed after lobectomy in various underlying lung conditions.

- Abstract
Background: It has been recently recognized that pulmonary cyst may develop after pulmonary resection, causing various symptoms. Most previously reported cases were after upper lobectomy in patients with chronic obstructive lung disease (COPD)., Case Presentation: Case 1 was a man in his 70 s with interstitial pneumonia (IP). Right lower lobectomy was performed for metastatic lung tumor using video-assisted thoracoscopic surgery (VATS). On postoperative day (POD) 19, computed tomography (CT) revealed a large cyst at the upper interlobular surface of the middle lobe, with pneumoderma and pneumomediastinum. The cyst was incised, polyglycolic acid (PGA) sheet and fibrin glue were applied, and the cyst was sutured. The sutured line was covered again with PGA sheet and fibrin glue. Case 2 was a man in his 70 s with COPD. Right upper lobectomy for primary lung cancer was performed using VATS. On POD 17, CT revealed a large pulmonary cyst at the apex of S6 and massive air leakage was observed. The same surgical procedure as that used in case 1 was performed. Cases 3 and 4 were healthy donors for living-donor lung transplantation. Two months after the right lower lobectomy in Case 3 and 3 months after the left lower lobectomy in Case 4, the patients had respiratory symptoms such as dyspnea and hemosputum. CT revealed a large cyst on the diaphragmatic surface of the right middle lobe in Case 3 and on the posterior mediastinal surface of the left upper lobe in Case 4. Cyst incision, soft coagulation, and application of PGA sheet with fibrin glue were performed in both cases. CT performed 1 year after surgery showed no development of a pulmonary cyst or air space in these four cases., Conclusions: Pulmonary cysts newly formed after lobectomy can develop not only in COPD or IP but also in healthy lungs. Our findings suggest that incision of the cyst and application of fibrin glue and PGA sheet with or without suturing the cyst wall is effective for management., (© 2024. The Author(s).)

Damage thresholds and structures on a metal aluminum and an aluminum oxide crystal induced by the soft x-ray free electron laser irradiations were evaluated. Distinctive differences in damage thresholds and structures were observed for these materials. On the aluminum oxide crystal surface, in particular, a novel, to the best of our knowledge, surface processing, which we suggest defining as "peeling," was recognized. Surface structures formed by peeling had extremely shallow patterning of sub-nanometer depth. For the newly observed peeling process, we proposed a scission of chemical bond, i.e., binding energy model, in the crystal.

- Abstract
Atezolizumab plus bevacizumab (Atezo + Bev) is the first immunotherapy for hepatocellular carcinoma (HCC), and in the current guidelines, it is positioned as the first-line chemotherapy for unresectable cases. Herein, we report a case of HCC with pseudoprogression followed by a complete response to Atezo + Bev. A 56 year-old man was diagnosed with intermediate-stage HCC, as defined by the Barcelona Clinic Liver Cancer system stage B. Computed tomography (CT) revealed multiple lesions in the liver without any extrahepatic lesions. First, he was treated with transcatheter arterial chemoembolization (TACE); however, multiple residual lesions were observed on CT scan 2 months after TACE. Therefore, treatment with Atezo + Bev was initiated 4 months after TACE. After the third administration of Atezo + Bev, a CT scan showed progressive disease in intrahepatic lesions, along with increased serum levels of tumor markers. Although TACE was planned again, Atezo + Bev was continued while the patient was waiting for hospitalization. After the fifth administration of Atezo + Bev, serum levels of tumor markers decreased to the normal range. Magnetic resonance imaging showed prominently reduced tumor size. Therefore, Atezo + Bev was continued, and after the eighth administration, the CT scan showed the disappearance of all the liver lesions, indicating a complete response. In immunotherapy, the therapeutic response can sometimes be obtained in an atypical pattern due to either an increase in tumor burden or the appearance of new lesions, called "pseudoprogression," which is rare in HCC., (© 2023. In this study, figure correction of a master mandrel of a Wolter mirror by organic abrasive machining (OAM) was demonstrated. In OAM, a flow of slurry, dispersed with organic particles, locally removes the surface of a workpiece in contact with a rotating machining tool. A computer-controlled machining system was used to perform the selective removal of a fused silica surface at a spatial resolution of 200 µm. A master mandrel of a Wolter mirror for soft x-ray microscopes was fabricated with a figure accuracy of <1 nm root mean square, which is sufficient for diffraction-limited imaging at a wavelength of 10 nm., (© 2023 Author(s). - Abstract
A direct-acting antiviral (DAA)-induced sustained virological response (SVR) reduces the risk of mortality. However, the risk factors associated with liver-related and non-liver-related mortality following a SVR after DAA treatment are unclear. We assessed the incidence and risk factors of liver-related and non-liver-related mortality in 1180 patients who achieved a SVR after DAA treatment. During the follow-up period after DAA treatment (median duration, 1099 [range: 84-2345] days), 53 (4.5%) patients died: 15 due to liver-related mortality, 25 due to non-liver-related mortality and 13 due to unknown causes. The all-cause, liver-related and non-liver-related mortality rates were 14.9, 4.2 and 7.0/1000 person-years, respectively. In a multivariate analysis, the development of hepatocellular carcinoma (HCC) after DAA treatment (p = .009; hazard ratio [HR], 31.484), an estimated glomerular filtration rate (eGFR) at baseline ≤61.68 ml/min/1.73 m
2 (p = .015; HR, 6.607), and an α-fetoprotein level post-treatment ≥7.6 ng/ml (p = .041; HR, 18.490) were significantly associated with liver-related mortality. Furthermore, eGFR ≤67.94 ml/min/1.73 m2 at baseline (p = .012; HR, 3.407) and albumin-bilirubin (ALBI) grade ≥ 2 at SVR (p = .024; HR, 3.449) were significantly associated with non-liver-related mortality. Early diagnosis and therapeutic interventions for HCC development after DAA treatment are important to reduce liver-related mortality. The ALBI grade, which reflects the hepatic functional reserve, is a useful predictor of non-liver-related mortality after a SVR induced by DAA treatment. 95. Short-term hepatocyte function and portal hypertension outcomes of sofosbuvir/velpatasvir for decompensated hepatitis C-related cirrhosis.

- Abstract
Background: It is unclear whether hepatocyte function and/or portal hypertension improves if a sustained virologic response (SVR) is achieved with direct-acting antivirals in patients with decompensated hepatitis C-related cirrhosis., Methods: We examined the safety and efficacy of a 12-week course of sofosbuvir/velpatasvir (SOF/VEL) in 20 patients with decompensated hepatitis C-related cirrhosis. We also investigated changes in the hepatocyte receptor index (LHL15) and blood clearance index (HH15) by Tc-99 m-galactosyl human serum albumin scintigraphy, liver stiffness measurement (LSM) by transient elastography, and hepatic venous pressure gradient (HVPG) in patients who achieved an SVR at 24 weeks after treatment (SVR24)., Results: One patient discontinued treatment because of rectal variceal hemorrhage, and 19 patients completed treatment. SVR24 was achieved in 17 patients (89%). Median LHL15 increased from 0.72 pre-treatment to 0.82 after SVR24 (p = 0.012), and median HH15 decreased from 0.82 pre-treatment to 0.76 after SVR24 (p = 0.010). The percentage of patients with LSM ≥ 20 kPa was 90% before treatment and remained at 90% after SVR24. However, the percentage with severe portal hypertension (defined as HVPG ≥ 12 mmHg) decreased from 92% pre-treatment to 58% after SVR24 (p = 0.046). Patients with a decreased HVPG from pre-treatment to after SVR24 had a smaller pre-treatment spleen volume than those with an increased HVPG (median, 252 vs. 537 mL, p = 0.028)., Conclusion: Achieving SVR24 with SOF/VEL treatment in patients with decompensated hepatitis C-related cirrhosis can be expected to improve hepatocyte function and portal hypertension on short-term follow-up., (© 2023. 96. Severity of Liver Fibrosis Is Associated with the Japanese Diet Pattern and Skeletal Muscle Mass in Patients with Nonalcoholic Fatty Liver Disease.

- Abstract
It is not fully clear as to which dietary patterns are associated with the pathogenesis of nonalcoholic fatty liver disease (NAFLD) in Asia. We conducted a cross-sectional study of 136 consecutively recruited patients with NAFLD (49% female, median age 60 years). Severity of liver fibrosis was assessed using the Agile 3+ score, a recently proposed system based on vibration-controlled transient elastography. Dietary status was assessed using the 12-component modified Japanese diet pattern index (mJDI12). Skeletal muscle mass was assessed by bioelectrical impedance. Factors associated with intermediate-high-risk Agile 3+ scores and skeletal muscle mass (75th percentile or higher) were analyzed by multivariable logistic regression. After adjustment for confounders, such as age and sex, the mJDI12 (OR: 0.77; 95% CI: 0.61, 0.99) and skeletal muscle mass (75th percentile or higher) (OR: 0.23; 95% CI: 0.07, 0.77) were significantly associated with intermediate-high-risk Agile 3+ scores. Soybeans and soybean foods were significantly associated with skeletal muscle mass (75th percentile or higher) (OR: 1.02; 95% CI: 1.00, 1.04). In conclusion, the Japanese diet pattern was associated with the severity of liver fibrosis in Japanese patients with NAFLD. Skeletal muscle mass was also associated with the severity of liver fibrosis, and intake of soybeans and soybean foods.

Background: Although the associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) induces more rapid liver regeneration than portal vein embolization, the mechanism remains unclear., Aim: To assess the influence of inflammatory cytokines and endothelial nitric oxide synthase (eNOS) activation on liver regeneration in ALPPS., Methods: The future liver remnant/body weight (FLR/BW) ratio, hepatocyte proliferation, inflammatory cytokine expression, and activation of the Akt-eNOS pathway were evaluated in rat ALPPS and portal vein ligation (PVL) models. Hepatocyte proliferation was assessed based on Ki-67 expression, which was confirmed using immunohistochemistry. The serum concentrations of inflammatory cytokines were measured using enzyme linked immune-solvent assays. The Akt-eNOS pathway was assessed using western blotting. To explore the role of inflammatory cytokines and NO, Kupffer cell inhibitor gadolinium chloride (GdCl
3 ), NOS inhibitor N-nitro-arginine methyl ester (L-NAME), and NO enhancer molsidomine were administered intraperitoneally., Results: The ALPPS group showed significant FLR regeneration (FLR/BW: 1.60% ± 0.08%, P < 0.05) compared with that observed in the PVL group (1.33% ± 0.11%) 48 h after surgery. In the ALPPS group, serum interleukin-6 expression was suppressed using GdCl3 to the same extent as that in the PVL group. However, the FLR/BW ratio and Ki-67 labeling index were significantly higher in the ALPPS group administered GdCl3 (1.72% ± 0.19%, P < 0.05; 22.25% ± 1.30%, P < 0.05) than in the PVL group (1.33% ± 0.11% and 12.78% ± 1.55%, respectively). Phospho-Akt Ser473 and phospho-eNOS Ser1177 levels were enhanced in the ALPPS group compared with those in the PVL group. There was no difference between the ALPPS group treated with L-NAME and the PVL group in the FLR/BW ratio and Ki-67 labeling index. - Abstract
Background: Primary spontaneous pneumothorax occasionally relapses, even after bullectomy; therefore, coverage of the bullectomy staple line for pleural reinforcement is common in Japan. However, the appropriate covering materials have not yet been determined., Methods: This was a longitudinal prospective cohort study. Data were available for patients aged < 40 years with primary spontaneous pneumothorax who underwent their first thoracoscopic bullectomy between July 2015 and June 2021. We used oxidized regenerated cellulose (ORC) sheets from July 2015 to June 2018, and polyglycolic acid (PGA) sheets from July 2018 to June 2021. The postoperative recurrence-free survival rate was evaluated. The characteristics of the recurrent cases (radiographic, intraoperative, and pathological findings) were also evaluated. The extent of pleural adhesions was classified into the following three groups: none, medium, or extensive., Results: A total of 187 patients were included in the study. There were 92 and 95 participants in the ORC and PGA sheet groups, respectively. The postoperative recurrence-free survival rates were significantly higher in the PGA sheet group than in the ORC sheet group (ORC group vs. PGA group, 82.9% vs. 95.4%, p = 0.031). In recurrent cases, there was a significant difference in terms of pleural adhesion (0.0% [12 of 12, none] vs. 100.0% [four of four, extensive], p < 0.001)., Conclusions: Compared with ORC sheets, PGA sheets are an effective material for preventing early recurrence of primary spontaneous pneumothorax. - Abstract
Background: The number of octogenarians requiring surgery for hepatocellular carcinoma (HCC) is increasing. However, the safety of hepatectomy in octogenarians remains controversial. The aim of this retrospective study was to determine the effect of age on the short- and long-term outcomes of hepatectomy for HCC to evaluate the safety of hepatectomy for octogenarians., Methods: Data from a total of 845 patients who underwent initial hepatectomy for HCC between April 1990 and March 2021 were retrospectively reviewed. Patients were categorized based on the age at the time of surgery (<80 years, n = 790; ≥80 years, n = 55), and the short- and long-term postoperative outcomes of the two groups were compared to evaluate whether hepatectomy is appropriate for octogenarian patients., Results: The proportion of octogenarian patients undergoing hepatectomy increased from 2.6% in 1990-1995 to 16.8% in 2016-2020 (P < 0.001), and the overall incidences of anatomical and major hepatectomy have increased. There was no significant difference in the morbidity rate between the octogenarians and the group of patients <80 years old (60.0% vs. 54.4%, P = 0.420), and the 90-day mortality rate was 0% in the octogenarian group. - Abstract
Cholangiocarcinoma (CC) has a poor prognosis and different driver genes depending on the site of onset. Intrahepatic CC is the second-most common liver cancer after hepatocellular carcinoma, and novel therapeutic targets are urgently needed. The present study was conducted to identify novel therapeutic targets by exploring differentially regulated genes in human CC. MicroRNA (miRNA) and mRNA microarrays were performed using tissue and serum samples obtained from 24 surgically resected hepatobiliary tumor cases, including 10 CC cases. We conducted principal component analysis to identify differentially expressed miRNA, leading to the identification of miRNA-3648 as a differentially expressed miRNA. We used an in silico screening approach to identify its target mRNA, the tumor suppressor Sloan Kettering Institute (SKI). SKI protein expression was decreased in human CC cells overexpressing miRNA-3648, endogenous SKI protein expression was decreased in human CC tumor tissues, and endogenous SKI mRNA expression was suppressed in human CC cells characterized by rapid growth. SKI-overexpressing OZ cells (human intrahepatic CC cells) showed upregulation of cyclin-dependent kinase inhibitor p21 mRNA and protein expression and suppressed cell proliferation. Nuclear expression of CDT1 (chromatin licensing and DNA replication factor 1), which is required for the G1/S transition, was suppressed in SKI-overexpressing OZ cells. SKI knockdown resulted in the opposite effects. Transgenic p21-luciferase was activated in SKI-overexpressing OZ cells. These data indicate SKI involvement in p21 transcription and that SKI-p21 signaling causes cell cycle arrest in G1, suppressing intrahepatic CC cell growth. Therefore, SKI may be a potential therapeutic target for intrahepatic CC., (© 2022 The Authors.
