- Abstract
Objective: To determine the levels of 2 endogenous inhibitors of angiogenesis, thrombospondin 1 (TSP-1) and pigment epithelium-derived factor (PEDF), in the vitreous fluid from patients with and without diabetes. Methods: The levels of TSP-1 and PEDF in vitreous samples from diabetic and age-matched nondiabetic patients were determined by Western blot analysis. Results: We observed significant amounts of TSP-1 and PEDF in the vitreous samples of control eyes. The TSP-1 levels varied in samples from patients with diabetes. In contrast, PEDF levels showed little or no change in vitreous samples from patients with or without diabetes. However, the PEDF protein exhibited variation in its molecular weight among the samples. We consistently observed lower levels of TSP-1 in diabetic patients who expressed the higher-molecular-weight PEDF isoform. Conclusions: In diabetes, changes in the TSP-1 level may play a role in shifting the angiogenic balance and contributing to the pathogenesis of diabetic retinopathy. Although the PEDF level did not change, the diabetic samples with the higher-molecular-weight PEDF isoform consistently showed lower levels of TSP-1. Clinical Relevance: The presence of the higher-molecular-weight PEDF isoform may be associated with greater risk of severe diabetic retinopathy. [ABSTRACT FROM AUTHOR]

The article discusses the U.S. Ski and Skateboard Association (USSA) Central Division qualifying junior ski competitions at Marquette Mountain, Michigan from February 24-26, 2011 to determine the Midwest representatives for the 2011 Rocky/Central Junior Championships in Steamboat Springs, Colorado. Competitors were J3 and J4 boy and girl alpine skiers from Michigan, Ohio and Illinois among others. J3 skiers competed in downhill, slalom and giant slalom races while J4 skiers tried for the super G, giant slalom and slalom races.

The article focuses on the results of a study conducted by the researchers at the University of Wisconsin Retina Service in Madison regarding the efficacy of bevacizumab monotherapy for treatment of exudative age-related macular degeneration (AMD). Dr. Justin L. Gottlieb states that bevacizumab was used primarily in about 85% of anti-vascular endothelial growth factor (VEGF) treatments. Significant improvements in visual acuity and reductions in retinal thickening were also confirmed.

Purpose: Historically, supplemental dextrose to infusion fluid has been used to reduce the need for intraoperative lensectomies to maintain visualization during diabetic vitrectomy. Valved, small-gauge vitrectomy has reduced surgical time and decreased intraoperative fluid flow. Assessment of supplemental dextrose in modern vitrectomy is presented in this study., Methods: A retrospective cohort study of diabetic patients undergoing vitrectomy was conducted. The dextrose group received supplemental dextrose in the infusion fluid, while the nondextrose group used a standard balanced salt solution (BSS Plus). Group assignment was per surgeons' typical practice patterns. Eyes with tractional retinal detachments were also evaluated as a subgroup., Results: Three hundred thirty phakic eyes were included. Supplemental dextrose was used in 199 eyes (60.3%). One unplanned lensectomy was performed in this series, in the nondextrose group, not statistically different from the dextrose group, with zero lensectomies (P = 0.4). Cataract survival curves overlapped for all eyes and for the tractional retinal detachment subgroup., Conclusion: In modern vitrectomy, unplanned lensectomy is rare. No difference was observed in the rate of intraoperative lensectomies or overall postoperative cataract course with or without dextrose supplementation to the infusion fluid. Standard solutions appear to be adequate for infusion, even for diabetics.

Introduction: West Nile virus disease, which is endemic to the United States, is a rarely reported systemic infection that can be difficult to diagnose. Chorioretinitis is an uncommon manifestation of West Nile virus but has pathognomonic ocular findings that can aid in diagnosis., Case Presentation: A 66-year-old man presented with acute onset fever, chills, and dyspnea. He underwent an extensive but nondiagnostic workup during hospitalization. New visual complaints prompted ophthalmology consultation. Funduscopic examination showed macular hemorrhages and midperipheral chorioretinal lesions. Fluorescein angiography revealed target-like lesions in a radial distribution, which is pathognomonic for West Nile virus chorioretinitis. Serology confirmed the diagnosis of West Nile virus disease. Systemic and ocular symptoms improved with supportive care., Discussion: West Nile virus disease has many nonspecific manifestations. History of recent mosquito exposure is not always readily elicited. In patients with visual symptoms, eye examination can help in its diagnosis., Conclusions: West Nile virus should be considered in patients with acute febrile or neurological illness during mosquito season., (Copyright© Board of Regents of the University of Wisconsin System and The Medical College of Wisconsin, Inc.)

- Abstract
Objective: To compare optical coherence tomography (OCT)-measured retinal thickness and visual acuity in eyes with diabetic macular edema (DME) both before and after macular laser photocoagulation., Design: Cross-sectional and longitudinal study., Participants: Two hundred ten patients (251 eyes) with DME enrolled in a randomized clinical trial of laser techniques., Methods: Retinal thickness was measured with OCT and visual acuity was measured with the electronic Early Treatment of Diabetic Retinopathy procedure., Main Outcome Measures: Optical coherence tomography-measured center point thickness and visual acuity., Results: The correlation coefficients for visual acuity versus OCT center point thickness were 0.52 at baseline and 0.49, 0.36, and 0.38 at 3.5, 8, and 12 months after laser photocoagulation. The slope of the best fit line to the baseline data was approximately 4.4 letters (95% confidence interval, 3.5-5.3) of better of visual acuity for every 100-mum decrease in center point thickness at baseline with no important difference at follow-up visits. Approximately one third of the variation in visual acuity could be predicted by a linear regression model that incorporated OCT center point thickness, age, hemoglobin A1C, and severity of fluorescein leakage. The correlation between change in visual acuity and change in OCT center point thickening 3.5 months after laser treatment was 0.44, with no important difference at the other follow-up times. A subset of eyes showed paradoxical improvements in visual acuity with increased center point thickening (7%-17% at the 3 time points) or paradoxical worsening of visual acuity with a decrease in center point thickening (18%-26% at the 3 time points)., Conclusions: There is modest correlation between OCT-measured center point thickness and visual acuity, and modest correlation of changes in retinal thickening and visual acuity after focal laser treatment for DME. However, a wide range of visual acuity may be observed for a given degree of retinal edema. Thus, although OCT measurements of retinal thickness represent an important tool in clinical evaluation, they cannot substitute reliably as a surrogate for visual acuity at a given point in time. This study does not address whether short-term changes on OCT are predictive of long-term effects on visual acuity.
