Grose Knochendefekte im Bereich des Acetabulums stellen bei der Huftprothesenrevision ein groses Problem dar. Es mus oft eine grose Menge von allogenen Knochentransplantaten zur Defektsanierung eingesetzt werden (Dartee et al. 1988). Da die Transplantation von allogenem Knochen stets das Risiko einer Infektionsubertragung (z.B. HIV, HBV, CMV) mit sich bringt (Leads 1988), werden in letzter Zeit vermehrt Sterilisationsmasnahmen zur Erhohung der Transplantatsicherheit angewendet. Im Kreiskrankenhaus in Garmisch-Partenkirchen wird zu diesem Zweck seit 1988 die Autoklavierung durchgefuhrt. In dieser Studie sollte die Heilungstendenz derartig behandelter Transplantate untersucht werden.

Die klassischen Indikationen fur den Einsatz von Knochentransplantaten sind die Defektuber-bruckung nach Tumorresektion, die Prothesenrevision und grose posttraumatische Knochendefekte (Schweiberer et al. 1989). Zur Reduktion des Ubertragungsrisikos von Infektionskrankheiten (HIV, HBV, Lues u.a.) bei der Transplantation allogenen Knochens (Leads 1988; Buck et al. 1990) ware die Sterilisation ossaren Gewebes vorteilhaft. Inwiefern sich zu diesem Zweck eine Autoklavierung eignet sowie welche Zeitdauer und Temperatur hierbei notig sind, ohne einen allzu hohen Qualitatsverlust zu verursachen, sollte in einer tierexperimentellen Studie untersucht werden.

Der positive Einflus elektromagnetischer Felder (EMF) auf die Heilung von Pseudarthrosen, Prothesenlockerungen und Huftnekrosen ist mittlerweile unbestritten. Nicht geklart sind jedoch die Auswirkungen von EMF auf Tumorwachstum und Zellproliferation. Um diesem Problem naherzukommen, wurden in vitro zwei Tumorzellinien, die humanen Mammakarzinomzellinien MCF7 und MDA, auf Anderungen nach intermittierendem EMF (6 h an/6 h aus) untersucht. Hierzu wurden die mitochondriale Aktivitat als Parameter fur die Proliferation einer Zelle herangezogen. Gleichzeitig wurde der Einflus verschiedener Zytostatika in Verbindung mit EMF auf die mitochondriale Aktivitat untersucht.

Bereits 1943 vertrat Albee [1] die Meinung, das nur die knocherne Einheilung der transplantierten Knochenschuppen bei Patellarsehnentransplantaten die Rekonstruk-tion des vorhandenen Kreuzbandes definitiv sichern kann: “… the new ligament must maintain its tension and only bony anchorage will assure this when weight- bearing and motion are allowed…”. Gerade im Rahmen des allgemeinen Trends zur Fruhmobilisation sind Qualitat und Quantitat dieser knochernen Inkorporation von Be-deutung. Unseres Wissens existiert zu diesem Problem keine zielgerichtete tierexpe-rimentelle Untersuchung. Die Fragestellung bei diesem Versuchsvorhaben beschaftigt sich mit dem Schicksal der transplantierten Knochenschuppen bei freien Patellar-sehnentransplantaten: Mit dem Einheilungs- und Resorptionsverhalten, sowie den Veranderungen der Ubergangszone vom Band auf den Knochen.

The goals of mechanical ventilatory support in patients with acute respiratory insufficiency (ARI) are to supply adequate ventilation and oxygenation with minimal cardiocirculatory embarrassment. The most common mode for providing this support is continuous positive pressure ventilation (CPPV). Recently, inspiratory pressure support ventilation (PSV) has been made available in several ventilators. This kind of ventilation, which augments spontaneous inspiration with an adjustable amount of inspiratory pressure, is of particular interest as a weaning method. Compared to Synchronized intermittent mandatory ventilation (SIMV) a reduced oxygen breathing cost has been reported [1]. Another alternative to CPPV for treatment of ARI is high frequency ventilation (HFV), [2], which has been used with varying results. A new method of HFV was introduced in 1983, in which a combination of conventional ventilation and HFV was used; this was referred to as combined HFV (CHFV) [3].

Abriebstaube werden regelmasig im umgebenden Gewebe von Endoprothesen beobachtet. Auch bei sogenannten gut vertraglichen Biomaterialien sind histologisch Veranderungen, wie Infiltration von Lymphocyten und Makrophagen, nachzuweisen. Da uber den Einflus von Abriebstauben auf Viabilitat und Funktion genannter Zellpopulationen nur wenig bekannt ist, wurden in der vorliegenden Arbeit drei Biomaterialien in ihrer Wirkung auf immunkompetente Zellen in vitro vergleichend untersucht.

Seit dem Altertum werden verschiedene Naturstoffe zur Blutstillung angewendet. Dies betrifft Baumschwamme, die unter der Bezeichnung Fungur chirurgor bekannt waren, wie Aufschriften von alten Apothekenstandgefasen belegen, und Ameisennester, die Alexander von Humboldt auf seiner sudamerikanischen Reise kennengelernt hat und die er Ameisenzunder nannte (14). Aber auch tierische Membranen sind hier zu nennen, wie von Jeger und Wohlgemuth beschrieben (16). Diese Naturstoffe, die im Korper resorbiert werden, sind aufgrund von Uberlieferung bekannt und wurden teilweise empirisch durch chemische Veranderungen verbessert. Es handelt sich hier um Makromolekule, die man heute den Kohlenhydraten oder Proteinen zuordnet. Baumschwamme und Algenpraparate bestehen aus komplexen Strukturen, die sich von Uronsauren ableiten. Tierische Membranen sind aus Eiweis wie dem Kollagen aufgebaut: Grundstoffe, die heute zur Herstellung von Hamostyptika, insbesondere von Schwammen, verwendet werden.

In a double-blind study the dose-dependent effect of 10, 20, 40, and 70 mg of SSHA was tested in volunteers. Whereas only a minor prolongation of the aPTT was noted, a clear anti-factor Xa effect was proven (method of Yin), as well as the release of lipoprotein lipase. In a subsequent study, 12 volunteers were treated daily with a dose of 40 mg for 11 days. On the first day of treatment a dosage of 40 mg SC was administered three times every 90 min. There was no cumulative effect with this dose and a clear plateau effect was recorded daily after each application. Repeated administration therefore results in an identical reproducible effect. Thus, no tachyphylaxis occurred under long-term treatment with 40 mg SSHA.

In a prospective study of 10 patients (7 men, 3 women, mean age 26.2 years), who underwent thoracotomy because of recurrent spontaneous pneumothorax, we performed light and scanning electron microscopic investigations of the resected pleura. The purpose of our work was to explain pleural changes which allowed the penetration of air through the intact wall of the blebs. In 8 of 10 cases we found small bullae at the top of the upper lobe, the remaining 2 showed up with massive pleural scar. The morphological and ultrastructural findings indicated that the presence of air induced a reactive process at the visceral pleura which led to degeneration and scarification. The changes consisted of simple membrane formation up to complete destruction of the pleural layer. The development of blebs is due to an air fistula coming out of the lung parenchyma. Penetration of air through the bleb's wall is explained with the increasing pressure inside and the dehiscence of degenerated pleural structure. In our opinion spontaneous pneumothorax in young people is caused by a localized disease of the upper lobe. The pleural changes are the visible effects of a degenerative process induced by air fistula from lung parenchyma. Therefore wedge-resection of that region is a rational therapy in patients with spontaneous pneumothorax.

Die Transplantation von allogenen Bankknochen stellt heute bereits einen klinischen Routineeingriff dar. Gerade diese scheinbar problemlose Verwendbarkeit von Fremdknochen fuhrt jedoch leider noch immer zu einer volligen Negierung immunologischer Aspekte dieses Verfahrens, wie die soeben publizierten„Richtlinien zum Fuhren einer Knochenbank“ [9] nachhaltig beweisen. Ein Grund hierfur liegt nicht zuletzt im Fehlen geeigneter Nachweismethoden der immunologischen Reaktionen. So wurde es bislang versaumt, einen Test zu entwickeln, der den spezifischen Problemen des Knochens angepast ist [1]. Ein solches, auch klinisch anwendbares Testverfahren zum Nachweis immunologischer Reaktionen mus dabei folgende Kriterien erfullen: klinische Relevanz der Ergebnisse gute Reproduzierbarkeit zuverlassige Anwendbarkeit auch bei vorbehandelten Transplantaten geringe Belastung fur den Patienten (relativ) einfache Handhabung (relativ) schnelle Testauswertung

In the treatment of bone and soft tissue infections only minor active agent concentration can be attained at the site of infection with the use of conventional therapy procedures, after oral or parenteral administration of antibiotics. The possibility of administering drugs locally exists during the course of surgery, after the infection focus has been removed. This is also true for the targeted application of drugs by modern surgical techniques such as endoscopy. Beginning with a gentamicin collagen compound, developed for treatment of bone infection osteomyelitis, a sustained release system was contrived. Through shell-like coating of the collagen fibrils of this carrier system with resorbable poly(esters), a sustained liberation system of the incorporated aminoglycoside could be achieved. The liberation kinetics were dependent on the nature and concentration of the aliphatic poly-(esters) used as links to the poly(glycolic), poly(lactic) or poly(butyric acid). During the course of this research it could further be shown that drugs can also be placed on medical devices such as artificial joints through use of these carriers in the form of thin films.

Der Einflus immunologischer Faktoren auf Vorgange bei der Knochenheilung ist bislang weitgehend ungeklart: Einerseits wird Immunreaktionen nach ossaren Allo-Transplantaten (Tx) eine Verschlechterung der Tx-Inkorporation zugeschrieben [1,3], andererseits existieren aber auch Hinweise auf eine Beteiligung des Immunsystems bei Knochenbildung und -umbau (siehe Tabelle 1). Gerade im Hinblick auf die Transplantation ossaren Gewebes stellt sich nun die Frage, inwieweit Immunreaktionen im Sinne einer Abstosungsreaktion zu bewerten und demzufolge unerwunscht sind oder als Faktoren des physiologischen Heilungsvorganges auftreten.

The concept of combined anaesthesia, the centrally-acting muscle relaxant guaifenesin (My 301) in a 5% solution with ketamine (and/or fentanyl) in addition to the inhalation of nitrous oxide and halothane is based upon the principle of "balanced anaesthesia". Guaifenesin amplifies the effect of several anaesthetics, which complement one another, allowing the dosage to be decreased and thereby reducing the cardiovascular stress. To induce anaesthesia, the combination of a cataleptic anaesthetic (ketamine = Ketanest, Ketolar) and a centrally acting skeletal muscle relaxant (guaifenesin = Myolaxin, My 301) is used. Because of the risk of aspiration the animal should be intubated as soon as possible. Anaesthesia will be prolonged by the carrier gas nitrous oxide (65%, weakly analgesic)/oxygen (65%), low concentrations of halothane (1.0 to 0.6 to 0.4%, weakly hypnotic and analgesic) and by a continuous drip infusion of 5% guaifenesin (relaxing, mild analgesic and sedative). The effect of all the other anaesthetics is increased by guaifenesin. To increase the analgesia and to control the cardiovascular parameters the additional injection of ketamine or fentanyl is recommended. The recovery period is short and the general condition is good both after a lengthy anaesthesia of 9 hours (n = 32) and after anaesthesia of 2 hours. No significant adverse effects on the cardiovascular system were detected.

Cell mediated immune reactions, demonstrated by leukocyte migration assay and histological analysis of spleen and lymph nodes, could be observed after fresh autografting of cortical segments in rats. The detected immune answer showed a bone-specific reaction pattern. In consideration of the temporal correlation of the observed immune reactions and the histological signs of graft incorporation, a connection of bone healing and immune system may be possible.

Ein adaquates Debridement mit Entfernung von avitalen, minderperfundierten und kontaminierten Gewebeanteilen ist die wichtigste Voraussetzung in der Behandlung von traumatischen Wunden [7]. Ebenso stellt in der septischen Traumatologie das radikale Debridement den ersten therapeutischen Schritt dar [8].

Messung der intraperitonealen fibrinolytischen Aktivitat an 50 abdominalchirurgischen Patienten (davon 4 Kontrollen). Instillation von Streptokinase/Streptodornase (Varidase®) in die Bauchhohle. Bestimmung von Gesamtprotein, Plasminogen, Antiplasmin, α2 Mx-Plasmin-Komplex, spontaner Fibrinolyse und Fibrinolyse + Aktivator zu verschiedenen Zeitpunkten postoperativ. Ergebnisse: Die intraperitoneale Fibrinolyse kann durch lokale Substitution eines Plasminogenaktivators (Varidase®) stimuliert werden. Systemische Wirkungen treten erst nach hoher Dosierung auf. Wundheilungsstorungen oder verstarkte Blutungsneigung wurden nicht beobachtet.

Preservation of an adequate circulating blood volume (ACBV) is a critical parameter in the managment of intensive care patients. Central venous pressure (CVP) and especially pulmonary capillary wedge pressure (PCWP) are commonly used to judge on the volume status and to achieve ACBV.

Measurement of intraperitoneal fibrinolytic activity on 50 patients having undergone abdominal surgery (four of them controls). Instillation of streptokinase/streptodornase (Varidase) into the abdominal cavity. Determination of total protein, plasminogen, antiplasmin, alpha 2 Mx-plasmin-complex, spontaneous fibrinolysis and fibrinolysis + activator at different points in time p.op.Intraperitoneal fibrinolysis may be stimulated by local substitution of a plasminogen activator (Varidase). Systemic effects are confined to a high dose. No impaired wound healing nor increased propensity to hemorrhaging occurred.

Schon geringfugige Verwachsungen im Eileiter oder im Eierstockbereich konnen zu einer Sterilitat fuhren. In einer kontrollierten Untersuchung wurde der Verwachsungsstatus von 103 Patientinnen mit mikrochirurgischen Sterilitats-Operationen erfast. Zur Adhasionsprophylaxe wurden vor Operationsende intraperitoneal verabreicht a) Streptokinase/Streptodornase oder b) Hydrocortisonacetat oder c) nur Ringer-Laktat. Bei geplanten Kontroll-Laparoskopien ca. 8 Tage nach dem Primareingriff wurden bei ca. 10% der Frauen neugebildete Verwachsungen meist mittlerer Auspragung gesehen. Ein signifikanter Unterschied zwischen den Prophylaxe-Masnahmen war unter den gewahlten Bedingungen nicht erkennbar.

Even minor adhesions in the oviduct or in the ovarial region may lead to infertility. In a controlled trial the adhesion status was documented in 103 patients who underwent microsurgical operations for infertility. Solutions of (a) streptokinase/streptodornase, (b) hydrocortisone acetate or (c) Ringer's solution only were administered intraperitoneally as adhesion prophylaxis. During a control laparoscopy 1 week after the primary operation, newly formed adhesions of mainly intermediate extent were observed in about 10% of the patients. There was no significant difference in efficacy between the prophylactic treatment.
