52. Impacts of the socio-political instability in Hong Kong on university students’ learning experience

Abstract

[EN] Since the Anti-Extradition Law Social Movement in 2019, Hong Kong has entered an era of socio-political instability. The conflict between student protestants and the government has become increasingly intense, whereas several universities even became the battlefield of the protest and were abruptly shut down in November 2019. To add fuel to the fire, the outbreak of COVID-19 in early 2020 has prompted all universities in the territory to suspend all face-to-face classes and activities for two semesters. All these have impacted university students in Hong Kong socially, academically and psychologically.In light of this, this study aims to explore the ways to which the learning experience of university students in Hong Kong have been impacted by the socio-political challenges. Through conducting a mixed-methods study at a public university in Hong Kong, it was found that university have a high perceived level of stress, high political involvements, unsatisfactory learning experience and poor learning motivation under this socio-political instability. It is hoped that this study can provide informed insights for teachers to understand students’ burdens, stresses, and emotional instability associated with socio-political unrest.

Abstract

There have been few studies performed to examine the pathophysiological differences between different types of psychosis, such as between delusional disorder (DD) and schizophrenia (SZ). Notably, despite the different clinical characteristics of DD and schizophrenia (SZ), antipsychotics are deemed equally effective pharmaceutical treatments for both conditions. In this context, dopamine dysregulation may be transdiagnostic of the pathophysiology of psychotic disorders such as DD and SZ. In this study, an examination is made of the dopamine synthesis capacity (DSC) of patients with SZ, DD, other psychotic disorders, and the DSC of healthy subjects. Fifty-four subjects were recruited to the study, comprising 35 subjects with first-episode psychosis (11 DD, 12 SZ, 12 other psychotic disorders) and 19 healthy controls. All received an 18F-DOPA positron emission tomography (PET)/magnetic resonance (MR) scan to measure DSC (Kocc;30–60 value) within 1 month of starting antipsychotic treatment. Clinical assessments were also made, which included Positive and Negative Syndrome Scale (PANSS) measurements. The mean Kocc;30–60 was significantly greater in the caudate region of subjects in the DD group (ES = 0.83, corrected p = 0.048), the SZ group (ES = 1.40, corrected p = 0.003) and the other psychotic disorder group (ES = 1.34, corrected p = 0.0045), compared to that of the control group. These data indicate that DD, SZ, and other psychotic disorders have similar dysregulated mechanisms of dopamine synthesis, which supports the utility of abnormal dopamine synthesis in transdiagnoses of these psychotic conditions.

83. Mammary Analogue Secretory Carcinoma of the Palate: Case Report and Review of the Literature

Mammary analogue secretory carcinoma (MASC) is a recently described salivary gland tumor, with a limited number of published reports. Less than three hundred cases have been reported in the literature and only 18 of these cases have been reported in minor palatal salivary glands, though publication bias is likely a factor. We present a case of a 57-year-old male who was diagnosed with MASC tumor presenting in a minor salivary gland and briefly review the current literature. MASC has a variety of histological features and different range of clinical behaviors. The histopathological diagnosis of MASC can be difficult, and the immunohistochemical profile of MASC is still being updated. The gold standard for MASC diagnosis is cytogenetics, with the majority having a translocation t(12;15)(p133;q25). Presently, there is no conclusive evidence that MASC should be treated differently than any other low-grade malignant salivary gland tumors, though high-grade transformation has been described.

84. Reconstruction for Complex Oromandibular Facial Defects: The Fibula Free Flap and Pectoralis Major Flap Combination

Background. Extensive through-and-through oromandibular defects after advanced oral carcinoma excision pose a reconstructive challenge for the head and neck surgeon. These complex oromandibular wounds often involve the mandible, oral and/or aerodigestive mucosa, and the external skin. As a result, these defects are often not amenable to reconstruction with a single flap due to the volume of soft tissue needed and the three-dimensional reconstructive requirement. The use of two free flaps has often been suggested to overcome this reconstructive challenge. A simpler and less technically demanding way to deal with this may involve the use of a free flap in combination with a pedicled regional flap. We present our experience of the use of a simultaneous microvascular fibula free flap (FFF) with a pectoralis major myocutaneous flap (PMMC) for addressing these defects. Methods. A retrospective chart review was performed of patients treated with a FFF and PMMC combination for the reconstruction of oromandibular defects at the University of Mississippi Medical Center (Jackson, MS) between October 2013 and February 2016. A minimum follow-up of 12 months was required. Data collected included the extent and location of tumor involvement, size of the postablative defect, tumor histology, clinical and pathological staging, length of follow-up, functional outcomes, and associated complications. Results. A total of three patients were identified to have been treated with the above technique. Defects repaired involved through-and-through mandibular defects with associated oral mucosa and external skin defects. In all cases, the FFF was used for restoring bony continuity with the skin paddle used to reconstruct the intraoral lining. The PMMC was used for reconstruction of the external skin defect and for providing soft tissue bulk. The average size of the fibula skin paddle used for intraoral reconstruction was 7.7 cm×11.7 cm. The average size of the PMMC paddle was 7.3×9 cm. The mean follow-up was 21.7 months. Both the FFF and PMMC survived in all cases, although postoperative wound healing complications occurred in two of the three patients. There was one partial flap loss. Two patients regained good oral intake while one patient tolerated oral intake but was PEG tube-dependent. Conclusions. The combination of pectoralis major myocutaneous flap and a vascularized free fibular flap is a viable option for the reconstruction of complex through-and-through oromandibular defects. This technique may be useful when a single microvascular free flap is not sufficient for reconstruction of such defects.

Abstract

This study aims to develop a super slice interpolation (SSI) method that generates thin-slice images from multichannel multislice images by exploiting the intra-slice coil sensitivity variations. SSI first calculates the thin-slice sensitivity maps by through-plane interpolation of the sensitivity maps computed from the acquired multislice images. It then reconstructs multiple thin-slice images from each acquired image using a through-plane regularized sensitivity encoding (SENSE) like procedure that consists of an initial SENSE reconstruction and denoising to set the prior information image, and subsequent regularized SENSE reconstruction. We evaluated SSI using multislice brain and abdominal images with typical slice thickness. SSI successfully separated each acquired image into two thinner ones without magnitude bias. Compared with the original thick-slice images, SSI revealed more anatomical details that were consistent with those in the separately acquired thin-slice images. SSI presents a novel slice interpolation approach to obtain thin-slice images from the multichannel thick-slice images.
