BackgroundPharmacokinetic data for bedaquiline in children are limited. We described the pharmacokinetics and safety of bedaquiline in South African children and adolescents receiving treatment for multidrug/rifampicin-resistant tuberculosis (MDR/RR-TB) in routine care.MethodsIn this observational cohort study, children aged 6-17 years receiving bedaquiline at recommended doses as part of MDR/RR-TB treatment underwent semi-intensive pharmacokinetic sampling. Bedaquiline and the M2 metabolite plasma concentrations were quantified, and nonlinear mixed-effects modeling performed. Pediatric data were described using a pre-established model of bedaquiline pharmacokinetics in adults. The exposure reference was 187 µg ⋅ h/mL, the median weekly area under the curve (AUC) of adults at week 24 of treatment with bedaquiline. Safety was assessed through monthly clinical, blood and electrocardiogram monitoring, and treatment outcomes described.ResultsFifteen children (3 human immunodeficiency virus [HIV]-positive) with median age 13.3 years (range 6.5-16.3) were included. A bedaquiline pharmacokinetic model was adapted to be allometrically scaled in clearance and volume, centered in the median child population weight. Bedaquiline bioavailability was 57% of that in adults. Overall bedaquiline exposures were below target, and AUC reference attainment was achieved in only 3 (20%) children. Ten children experienced 27 adverse events at least possibly related to bedaquiline; no adverse events led to bedaquiline withdrawal. Two adverse events (arthritis and arthralgia) were considered severe, and 2 children had mild QT interval corrected for heart rate using Fridericia's formula (QT) prolongation.ConclusionsThe evaluated doses of bedaquiline in children ≥ 6 years of age were safe but achieved slightly lower plasma concentrations compared to adults receiving the recommended dose, possibly due to delayed food intake relative to bedaquiline administration.

Background. Peer-reviewed literature demonstrates increasing support for the use of focused abdominal sonography in trauma (FAST) in the setting of blunt trauma, one study demonstrating the sensitivity and specificity of FAST for the detection of free fluid to be 0.64 - 0.98 and 0.86 - 1.00, respectively, compared with abdominal CT. Utilising ultrasound in trauma triage increases efficiency and cost-effectiveness and reduces reliance on CT, compared with using CT alone. There is little evidence to support relying solely on a negative FAST and physical examination for patient management. Method. A retrospective descriptive study of 172 adult patients who received FAST for the evaluation of blunt abdominal trauma between 22 July 2007 and 21 January 2008 at Tygerberg Hospital was performed. Ultrasound findings were correlated with CT scan findings, operative findings if managed surgically, clinical outcomes whether managed surgically or conservatively, as well as postmortem findings in deceased patients. Results. FAST was negative in 147 (85.5%) patients. Twenty-four (16.3%) of these patients died from all-cause mortality, none of which was due to intra-abdominal injury. Seven patients with negative FAST underwent CT scan owing to change in clinical course, and 3 patients with negative FAST underwent laparotomy owing to change in clinical course, with positive findings in 2 patients – a bowel injury requiring resection (not seen on CT) and a diaphragmatic rupture seen on CXR. A negative FAST was shown to be an excellent predictor for the absence of significant intra-abdominal trauma. The mortality rate among 25 FAST positive patients was 24% (N=6). Only one of these patients (with a splenic rupture) was suspected to have died from abdominal pathology.

Astrophysics - Astrophysics of Galaxies - Abstract
The inner $\sim$200 pc region of the Galaxy contains a 4 million M$_{\odot}$ supermassive black hole (SMBH), significant quantities of molecular gas, and star formation and cosmic ray energy densities that are roughly two orders of magnitude higher than the corresponding levels in the Galactic disk. At a distance of only 8.2 kpc, the region presents astronomers with a unique opportunity to study a diverse range of energetic astrophysical phenomena, from stellar objects in extreme environments, to the SMBH and star-formation driven feedback processes that are known to influence the evolution of galaxies as a whole. We present a new survey of the Galactic center conducted with the South African MeerKAT radio telescope. Radio imaging offers a view that is unaffected by the large quantities of dust that obscure the region at other wavelengths, and a scene of striking complexity is revealed. We produce total intensity and spectral index mosaics of the region from 20 pointings (144 hours on-target in total), covering 6.5 square degrees with an angular resolution of 4$"$,at a central frequency of 1.28 GHz. Many new features are revealed for the first time due to a combination of MeerKAT's high sensitivity, exceptional $u,v$-plane coverage, and geographical vantage point. We highlight some initial survey results, including new supernova remnant candidates, many new non-thermal filament complexes, and enhanced views of the Radio Arc Bubble, Sgr A and Sgr B regions. Water repellent (or “hydrophobic” or “non-wetting”) soils have been studied by soil scientists for well over a century. These soils are typified by poor water infiltration, which leads to increased soil erosion and poor crop growth. However, the importance of water repellence on determining soil properties is now becoming recognised by geotechnical engineers. Water repellent soils may, for example, offer novel solutions for the design of cover systems overlying municipal or mine waste storage facilities. However, investigations into factors affecting their mechanical properties have only recently been initiated. This purpose of this paper is to introduce geotechnical engineers to the concept of water repellent soils and to discuss how their properties can be evaluated under an unsaturated soils framework. Scenarios in which water repellent properties might be relevant in geotechnical applications are presented and methods to quantify these properties in the laboratory and in the field examined.

In a modern and fast-evolving technological world, precarity has become more notable. Digital transformation has ushered in an era of ‘datafication’, profoundly impacting societies and individuals in such a way that there are emerging complexities and potential vulnerabilities in our interactions with technology. Thus, it is crucial that the Humanities subjects focus on human beings, their culture and values. This book focuses on the challenges and opportunities experienced in the Digital Humanities. The main thesis of this book is on Digital Humanities in precarious times, while also reporting on topics and research methods in a variety of Humanities subject fields. Digital Humanities is a dynamic multidisciplinary, interdisciplinary and transdisciplinary field that encompasses a wide array of disciplines, methodologies and approaches. It represents a fusion of computational methods with humanistic inquiry, leveraging technology to explore and analyse various facets of human culture, society and history. At its core, this field’s nature allows scholars from diverse backgrounds – including literature, history, linguistics, cultural studies and more – to collaborate and engage in innovative research projects that transcend traditional disciplinary boundaries. All the chapters in this book represent a scholarly discourse and provide original research, they are based on different methodologies ranging from an interdisciplinary approach, a philosophical desk study, case studies, qualitative studies and a semi-structured survey.

MeerKAT's large number of antennas, spanning 8 km with a densely packed 1 km core, create a powerful instrument for wide-area surveys, with high sensitivity over a wide range of angular scales. The MeerKAT Galaxy Cluster Legacy Survey (MGCLS) is a programme of long-track MeerKAT L-band (900-1670 MHz) observations of 115 galaxy clusters, observed for $\sim$6-10 hours each in full polarisation. The first legacy product data release (DR1), made available with this paper, includes the MeerKAT visibilities, basic image cubes at $\sim$8" resolution, and enhanced spectral and polarisation image cubes at $\sim$8" and 15" resolutions. Typical sensitivities for the full-resolution MGCLS image products are $\sim$3-5 {\mu}Jy/beam. The basic cubes are full-field and span 4 deg^2. The enhanced products consist of the inner 1.44 deg^2 field of view, corrected for the primary beam. The survey is fully sensitive to structures up to $\sim$10' scales and the wide bandwidth allows spectral and Faraday rotation mapping. HI mapping at 209 kHz resolution can be done at $0

This paper presents normalizing flows for incremental smoothing and mapping (NF-iSAM), a novel algorithm for inferring the full posterior distribution in SLAM problems with nonlinear measurement models and non-Gaussian factors. NF-iSAM exploits the expressive power of neural networks, and trains normalizing flows to model and sample the full posterior. By leveraging the Bayes tree, NF-iSAM enables efficient incremental updates similar to iSAM2, albeit in the more challenging non-Gaussian setting. We demonstrate the advantages of NF-iSAM over state-of-the-art point and distribution estimation algorithms using range-only SLAM problems with data association ambiguity. - Abstract
Drawing insight and inspiration from our comrade Aziz Choudry, in this paper, we explore the role of political education and knowledge making in social movement struggles to contest the state and capital around access to water and other public services. Drawing on a decade of organising work in the city of Cape Town and at wider scales, we highlight the importance of learning and knowledge generation in social movements and the everyday strategies of organising movements use to overcome divisions based on 'race', gender, class and nationality, and to confront those obstacles presented or exacerbated by exclusionary governance arrangements, commodified service delivery and the coercive apparatus of the state. Rejecting elite led organising in favour of working-class self-organisation, these strategies seek to build individual and organisational solidarity across the diverse and fragmented experiences of those living on the margins of capitalist society.
