Les personnes vivant avec des déficiences physiques et des incapacités fonctionnelles associées, qui utilisent un fauteuil roulant comme principal mode de locomotion (UFR), sont confrontées à des difficultés dans leur vie quotidienne. En conséquence, leur participation à des activités récréatives et de loisirs s’avère souvent entravée. Une telle situation accélère le développement ou l'exacerbation de problèmes secondaires liés à la santé physique et au bien-être psychologique, créant ainsi un cercle vicieux d'inactivité. De plus en plus de preuves scientifiques démontrent que l'activité physique adaptée (APA) permet aux UFR d'atténuer, voire de surmonter, ces problèmes. Bien qu’un large éventail de sports adaptés soit disponible, les activités de pagaie adaptées n’ont reçu que très peu d’attention à ce jour. L’identification et la modélisation des facteurs influençant leur pratique sont cruciaux dans l’objectif de promouvoir la participation à des activités de pagaie adaptées. Cette étude visait à développer un nouveau modèle conceptuel des facteurs influençant la pratique des activités de pagaie adaptées en utilisant un processus articulé autour de quatre phases : 1) une recherche systématisée de la littérature et une synthèse des principaux modèles conceptuels disponibles en APA, 2) la conceptualisation d'un modèle préliminaire pour la participation à des activités de pagaie adaptées, 3) la validation de ses construits et de leurs interactions par trois experts de l'APA et deux experts scientifiques indépendants afin de cocréer le modèle final (c.-à-d. Le modèle montréalais d'activités de pagaie adaptées - M2APA) et 4) le test de l'applicabilité du M2APA auprès de trois UFR ayant déjà participé à des activités de pagaie adaptées. Sur la base de cette démarche, le M2APA offre un modèle complet cartographiant les principaux facteurs personnels et environnementaux à prendre en compte dans la pratique d'activités de pagaie adaptées. En outre, le M2APA fournit des indications pour promouvoir la participation à des activités de pagaie adaptées et ainsi, renforcer l'inclusion sociale des UFR., Individuals with physical impairments and related disabilities who use a wheelchair as their primary mode of locomotion (WCUsers) face challenges in their daily lives that interfere with meaningful recreational and leisure participation. This accelerates the development or exacerbation of physical health and psychological well-being difficulties, thus creating a vicious cycle of inactivity. Emerging evidence suggests that adapted physical activity (APA) allows WCUsers to mitigate, even overcome, these difficulties. Nowadays, though broad ranges of adapted sports are available, adapted paddling activities have received limited attention. Identifying and modeling the factors influencing their practice remains crucial to further promote adapted paddling activities. This study aimed to develop a novel conceptual model of factors influencing the practice of adapted paddling activities using a four-phase process: 1) the systematized search of the literature and a synthesis of currently-available conceptual models in APA practice, 2) the conceptualization of a preliminary model for the practice of adapted paddling activities, 3) the validation of its construct and content by three APA experts and two independent scientific experts to co-create the final model (i.e., Montreal Model of Adapted Paddling Activities – M2APA) and 4) the testing of the applicability of the M2APA with three WCUsers who previously practiced adapted paddle boarding. Based on these findings, the M2APA offers a comprehensive model allowing one to map key personal and environmental factors needing consideration for the practice of adapted paddling activities. Moreover, the M2APA provides insights to promote participation into adapted paddling activities and, ultimately, strengthen social inclusion of WCusers.

L’éducation par les pairs est de plus en plus utilisée en promotion de la santé. Elle consiste à faire appel à des personnes pour offrir des interventions de prévention et de promotion de la santé (PPS) à leurs pairs. Une formation est généralement offerte aux pairs avant qu’ils débutent leur implication en PPS. Peu d’études ont examiné l’implantation de programmes de promotion de la santé (PPS) offerts par des pairs, ses déterminants et leurs mécanismes d’action. Cette thèse s’inscrit dans le cadre d’une étude quasi-expérimentale visant à évaluer les effets du programme Vivre en Équilibre (VEE), un PPS offert par des pairs ciblant la peur de tomber et ses conséquences sur la participation sociale des aînés. La thèse vise à développer et appliquer un modèle conceptuel des facteurs influençant l’implantation de programmes offerts par des pairs. Les objectifs spécifiques sont: 1) de développer un modèle conceptuel de l’implantation des PPS offerts par des pairs; 2) de documenter l’implantation de VEE; et 3) d’identifier les facteurs associés à son implantation. À la suite d’une recension des écrits, un modèle conceptuel de l’implantation des PPS offerts par des pairs a été élaboré en se basant sur des travaux conceptuels existants et en adoptant une conceptualisation des PPS en tant que systèmes complexes. Ce modèle postule que des facteurs liés aux individus, au programme et au contexte influencent l’implantation et suggère trois mécanismes (interactions, auto-organisation, processus d’adaptation) par lesquels ces facteurs agissent. Une étude de cas multiples a permis d’analyser l’implantation de VEE dans six résidences pour aînés du Québec. La population rejointe, la fidélité d’implantation, les adaptations, la réponse au programme et les facteurs associés à l’implantation du programme ont été documentés à l’aide de grilles d’observation, de journaux de bord, de fiches de présences, de questionnaires téléphoniques et d’entrevues réalisées auprès des pairs, des répondants de résidences et d’un sous-groupe de participants au programme. Une analyse intra- et inter-cas a permis d’identifier les facteurs associés à l’implantation et de les comparer au modèle élaboré. Au total, 71 aînés ont participé au programme. Les participants recrutés correspondaient majoritairement à la population ciblée par le programme. Les pairs ont offert toutes les activités du programme, mais ont rapporté avoir fait certaines adaptations. Le taux de participation (91%) et le niveau de satisfaction des participants, des pairs et des répondants des résidences étaient élevés. L’analyse révèle que des facteurs liés aux individus (ex. : attitudes des participants, expérience des pairs), au programme (ex. : qualité du contenu et du matériel, formation des pairs) et au contexte (ex. : climat d’implantation) ont influencé l’implantation du programme par le biais des mécanismes identifiés dans le modèle conceptuel. La thèse montre que VEE peut être implanté avec succès par des pairs aînés dans des résidences. Les résultats soutiennent aussi le modèle conceptuel élaboré dans le cadre de cette thèse. Ils apportent un éclairage sur les facteurs associés à l’implantation de VEE et guideront les chercheurs et les intervenants intéressés par l’implantation de PPS offerts par des pairs., Peer education is increasingly being used in health promotion. In this strategy, peer leaders deliver health promotion programs (HPPs) to their peers. Peer leaders usually attend training before delivering HPPs. Few studies have examined the implementation of peer-led HPPs, along with their determinants and mechanisms of action. This thesis was undertaken as part of a quasi-experimental study aiming to evaluate the effectiveness of Vivre en Équilibre (VEE), a peer-led HPP targeting fear of falling and its consequences on older adults’ social participation. The purpose of this thesis is to develop and apply the implementation framework of peer-led HPPs. More specifically, the objectives are to: 1) develop a conceptual framework of peer-led HPPs; 2) document the implementation of VEE; and 3) identify the factors associated with its implementation. Following a literature review, a conceptual framework of peer-led HPPs was developed based on existing conceptual work and on complex systems theory concepts. This framework postulates that factors related to individuals, to the program and to the context influence implementation. It further suggests three mechanisms (interactions, self-organization, adaptation process) through which these factors may act. A multiple case study analyzed the implementation of VEE among six independent-living residences for older adults in Quebec. Program reach, fidelity of implementation, adaptations, responsiveness, and factors associated with implementation of the program were documented. Observation grids, peer leaders’ logbooks, attendance sheets, phone questionnaires and interviews (with peer leaders, activity coordinators of the residences, and a subgroup of program participants) were used to collect the data. Content analysis and case comparisons helped identify factors associated with implementation of VEE and compare them with the conceptual framework developed. Overall, 71 older adults participated in the program. Most participants corresponded to the program’s target population (i.e. older adults with fear of falling). Peers delivered all program content but adapted some elements. The participation rate (91%) and the satisfaction level of participants, peers and activity coordinators were high. The analysis revealed that some factors related to individuals (e.g., participants’ attitudes, peer leaders’ experience), to the program (e.g., materials and content quality, training) and to the organizational context (e.g., implementation climate) facilitated VEE implementation through mechanisms identified in the framework. This thesis shows that VEE can be successfully delivered by peer leaders in independent-living residences. The results also support the conceptual framework and shed light on factors associated with the implementation of VEE. These results can guide researchers and practitioners interested in implementing peer-led HPPs.

Les troubles musculo-squelettiques du membre supérieur (TMS-MS) peuvent avoir un impact négatif sur l'autonomie d’une personne. À ce jour, il n’existe pas d’outils en français évaluant les limitations d'activités et les restrictions de participation, disposant de bonnes propriétés métrologiques et d'applicabilité dans un contexte clinique marqué par des contraintes de temps. L’Upper Limb Functional Index (ULFI), qui présente de bonnes qualités métrologiques et applicabilité clinique adéquate, s'avère un outil intéressant pour les ergothérapeutes de par son approche centrée sur la personne. Toutefois, il n'était disponible qu'en anglais et en espagnol. La présente recherche, constituée de deux études, visait à adapter l’ULFI à la population canadienne française et à évaluer ses propriétés métrologiques et son applicabilité clinique. La première étude a examiné sa cohérence interne, sa validité convergente et son applicabilité clinique auprès de 50 patients bilingues atteints de TMS-MS. La seconde étude visait à analyser sa fidélité test-retest et sa sensibilité au changement auprès de 60 patients francophones. Les résultats ont révélé que l'ULFI-CF possède des propriétés métrologiques solides : une cohérence interne élevée (α de Cronbach = 0,93), une excellente fidélité test-retest (CCI = 0,87-0,95), une excellente validité convergente (r = 0,70-0,85) et une sensibilité au changement de bonne à excellente (tailles d'effet = 0,49-0,88 et r = 0,64 pour la section 1). L'ULFI-CF démontre également une bonne applicabilité clinique. En conclusion, l’ULFI-CF s’avère un outil pertinent pour les cliniciens œuvrant auprès d’une clientèle canadienne française souffrant d'un TMS-MS dans un contexte clinique marqué par des contraintes de temps., Upper limb musculoskeletal disorders (UL-MSD) can negatively impact on patients' autonomy. There are no existing outcome measures available in French to assess activity limitations and participation restrictions caused by UL-MSD that have sound metrological properties and are applicable in a clinical context where clinicians have limited assessment time for each patient. The Upper Limb Functional Index (ULFI), having good metrological qualities and clinical applicability, is a relevant tool for clinicians since it promotes a patient-centered approach. However, the tool was only available in English and Spanish until now. This research project included two studies and aimed at adapting the ULFI to the French Canadian population and to assess the metrological properties of the adapted version (ULFI-FC) and its clinical applicability. The first study examined its internal consistency, convergent validity and clinical applicability among 50 bilingual patients with UL-MSD. The second study aimed to analyse its test-retest reliability and responsiveness among 60 French-speaking patients. The findings revealed that the ULFI-FC possesses sound metrological properties: a high internal consistency (Cronbach α = 0.93 for Part 1), an excellent test-retest reliability (ICC = 0.87-0.95), an excellent convergent validity (r = 0.70-0.85), and good to excellent responsiveness (effect size = 0.49-0.88; and r = 0.64 for Part 1), as well as good clinical applicability in a context where assessment time devoted to each patient is limited. In conclusion, the ULFI-CF is a relevant and attractive tool assessing activity limitations and participation restrictions for clinicians working with French Canadian UL-MSD patients in a busy clinical setting.
