52. Degenerative diseases of the lumbar spine. Comparison of the multiecho data image combination sequence with magnetization transfer saturation pulse versus lumbar myelography/postmyelographic computed tomography

Abstract

PURPOSE: To compare a T2* weighted 2D spoiled gradient echo multiecho sequence (MEDIC) with magnetization transfer saturation pulse versus lumbar myelography and postmyelographic computed tomography (pCT). MATERIAL AND METHODS: 84 disk levels in 27 patients were examined. The vertebral bodies, intervertebral disks, neural foramina, cauda equina, ligamentum flavum, and narrowing of the spinal canal were investigated using an evaluation scale. In addition, the pCT and the MEDIC sequence were evaluated for assessing the narrowing of the neural foramina in a phantom study using a cadaver preparation of the lumbar spine. RESULTS: A total of 28 disk herniations, 11 cases of osteophytes narrowing the spinal canal or the neural foramina, and 7 spinal canal stenoses were detected. The pCT was significantly better in visualizing the cauda equina. The MEDIC sequence was significantly superior in visualizing the extension of the ligamentum flavum. No statistical differences between either image modality were found concerning assessment of the narrowing of the neural foramina and the spinal canal, nor regarding evaluation of the vertebral disk and the vertebral body. Measurements of the phantom study showed that the MEDIC sequence did not overestimate narrowing of the neural foramina. CONCLUSIONS: The T2* MEDIC sequence has proved to be as accurate as pCT in evaluating osteophytes and narrowing of the neural foramina. In doubtful standard magnetic resonance imaging findings, this means that an additional axial T2* MEDIC sequence may be of value in reaching the same accuracy as pCT.

Abstract

Eine Indikation zum Einsatz der Computertomographie (CT) und der Magnetresonanztomographie (MRT) bei diffusen Lebererkrankungen besteht nicht. Das Erscheinungsbild der chronischen Hepatitis ist uncharakteristisch, das Bild der Leberzirrhose ist vom Stadium abhängig. Erst das mit der Hepatitis, der Leberzirrhose und der Hämochromatose gehäufte Auftreten von fokalen Leberläsionen, insbesondere des Hepatozellulären Karzinoms bedingt den zunehmenden Einsatz der beiden Verfahren. Hier zeigen sich CT und MRT je nach technischem Entwicklungsstand und Einsatz von Kontrastmitteln von wechselnder Sensitivität und Spezifität. Der MRT gelingt eine bessere Charakterisierung der Läsionen durch unterschiedliches Erscheinungsbild in einzelnen Sequenzen, die CT erfasst in einem Untersuchungsgang Abdomen und Becken und ist so, auch wegen der besseren Verfügbarkeit, die geeignete Stagingmodalität. Komplikationen der Zirrhose wie Aszites, Ösophagusvarizen und andere Folgen der portalvenösen Hypertonie werden erfasst. Eine Schnittbilddiagnostik ist notwendig vor Therapien wie Lebertransplantation, transjugulärem portosystematischen Shunt (TIPSS), vor lokal abladierenden Verfahren oder transarterieller Chemotherapie.

71. Frequency and nature of incidental extra-enteric lesions found on magnetic resonance enterography (MR-E) in patients with inflammatory bowel diseases (IBD)

Abstract

BACKGROUND: The aim of this study was to determine the occurrence of extra-enteric findings in a large cohort of patients undergoing magnetic resonance enterography (MR-E) and to classify the clinical significance of these findings. MATERIALS AND METHODS: We retrospectively analyzed 1154 MR-E performed in 1006 patients referred to our radiological department between 1999-2005. The reasons for referral were suspected or proven inflammatory bowel diseases (IBD) (n = 710), further diagnostic work-up for small bowel disease because of non-specific abdominal symptoms (SBD; n = 182) or suspected small bowel malignancies (SBM; n = 114). All extra-enteric findings were reviewed by a radiologist and a gastroenterologist and were classified as having high, moderate, or low significance for further diagnostic or therapeutic procedures. RESULTS: The average age of all patients was 40+/-16 (Mean+/-SD) years (y) (IBD 35+/-13 y; SBD 49+/-16 y; SBM 57+/-15 y). A total of 1113 extra-enteric findings were detected in 600 of 1006 patients (59.6%). Of these findings 180 (16.2%) were judged as having a high, 212 (19.0%) a moderate and 721 (64.8%) a low significance. On a per group basis in patients with IBD 12.0% of the findings were of major clinical significance compared to 13.7% and 33.3% in patients with SBD and SBM, respectively. The most common major findings were abscesses (69.9%) in the IBD group and extraintestinal tumors, metastases or masses in the SBD and SBM groups (41.9% and 74.2%, respectively). CONCLUSIONS: MR-E reveals a substantial number of extra-enteric findings, supporting the role of a cross-sectional imaging method for the evaluation of the small bowel.

Abstract

Bisher sind in der Schnittbildgebung zwei Projektionsverfahren üblich: Die Average-Projektion, die das Bildrauschen gut unterdrückt, jedoch die Kantenschärfe verringert und die Maximum-Intensitäts-Projektion (MIP), deren Bilder sich durch eine gute Kantenschärfe auszeichnen, gleichzeitig aber ein höheres Bildrauschen aufweisen. Daher wurde in der vorliegenden Arbeit eine Synthese beider Verfahren, softMip, mit dem Ziel die jeweiligen Vorteile zu vereinen, entwickelt. In einer Phantom-Studie mit sieben verschiedenen CT-Geräten wurden das Bildrauschen und die Kantenschärfe der drei Projektionsverfahren (MIP, Average und softMip) ermittelt. Anschließend wurden die drei Verfahren in einer klinischen Studie bei der Suche nach Harnwegskonkrementen in 31 ULD-CT-Abdomenaufnahmen eingesetzt. Für jedes Projektionsverfahren wurden sowohl Anzahl, Größe und Lokalisation der Harnwegskonkremente als auch die subjektiven Kriterien Bildrauschen, Kontrast, Konturschärfe, Artefakte und Bildeindruck erhoben. softMip weist im Phantomversuch ein geringeres Bildrauschen als MIP auf (p, Two projection algorithms exist in the cross sectional imaging: Average projection, which features good suppression of image noise, however the edge sharpness diminishes. In maximum-intensity-projection (MIP), images show good edge sharpness, but also amplified image noise. We developed softMip, a synthesis of both algorithms which tries to unite the respective advantages. In a phantom trial on seven different CT-devices, image noise and edge sharpness of the three projection algorithms (MIP, Average and softMip) were aquired. Furthermore image quality of the transition from Average to MIP by means of softMip was compared to the image quality of alpha blending average projection and MIP. In a clinical trial we investigated softMip compared to MIP and average projection in ULDCT examinations of 31 patients with suspected calcifications in the urinary tract.
