Patients with long-term tricuspid regurgitation (TR) are mostly accompanied by hepatic, cardiac, and venous remodeling. Transcatheter tricuspid valve replacement (TTVR) device has emerged as a promising alternative to open-heart surgery for TR patients. No study has assessed the impact of TTVR on hepatic, cardiac, and venous remodeling.Twenty-two patients with TR enrolled in this study underwent TTVR between October 2020 and January 2021. Liver, heart, and veins were reconstructed by three-dimensional computed tomography reconstruction software at baseline and 6 months follow-up.Twenty-two patients were enrolled in this study. The mean age was 64.8 ± 8.2 years, and all patients had severe or greater TR with multiple comorbidities. The left hepatic lobe volume decreased from 518.8 ± 171.9 ml to 470.4 ± 179.6 ml at 6 months during follow-up (p = 0.049). Evidence of a decrease in three hepatic veins parameters and splenic vein parameters was noted from baseline to 6 months. And a significant decrease in right atrial volume (317.5 ml [interquartile range: 216.1 to 497.3 ml] vs. 266.7 ml [interquartile range: 178.7 to 480.7 ml]; p = 0.003) were observed in the study.Six-month outcomes show that TR elimination by LuX-Valve is associated with the reverse remodeling of liver, heart, and veins. Accordingly, LuX-Valve is a promising alternative for patients presenting with severe TR.

Owing to the abundance of calcium and low redox potential and high safety, calcium-ion batteries (CIBs) are considered a promising candidate for post lithium-ion batteries. Cu3(OH)2V2O7·2H2O@rGO with bimetallic redox activity is proposed as a novel cathode material for CIBs.

PURPOSE: To compare scoring systems and their ability to capture short and long-term recovery of paediatric patients with acute encephalitis. To identify clinical predictors of short-term outcomes by correlating functional outcome measures at 1 month post diagnosis of acute encephalitis. METHODS: Patients with encephalitis diagnosed between July 2011 and 2016 based on Granerod’s criteria were studied in this retrospective cohort study. Functional outcome scores on WeeFIM, LOS, GOS-E, mRS and ICF at initial presentation and 1, 3, 6 and 12 months later were compared. RESULTS: WeeFIM and LOS scores both showed maximum change in the first 3 months, reflecting highest recovery in this period. With WeeFIM, the greatest change occurred within the first month following diagnosis. On univariate analysis, seizure frequency in the first month, presence of movement disorder, presence of autonomic dysfunction and lower baseline functional score was associated with poorer WeeFIM scores at 1 month. The latter three variables remained statistically significant on multivariate analysis. CONCLUSION: WeeFIM is a potentially preferred functional outcome assessment tool as it demonstrated greatest recovery within the first month due to a trend of high responsiveness and relatively low ceiling effect. Presence of autonomic dysfunction and movement disorders at diagnosis correlated with poorer outcome at 1 month post diagnosis.

Transcatheter aortic valve implantation (TAVI) has become a popular treatment for surgical high-risk patients with severe aortic stenosis (AS). Recently, we have applied TAVI to the treatment of aortic regurgitation (AR). Compared with conventional surgical procedures, TAVI is less invasive and considered a useful option for these high-risk patients. In this study, we reported a patient who underwent transapical TAVI. The patient was a 52-year-old female with Takayasu arteritis (TA) for 25 years, as well as with severe aortic regurgitation, porcelain aortas, and heart failure. Transapical TAVI successfully was accomplished without neurological complications, and heart failure immediately improved postoperatively.

We present a hybrid quantum system composed of a magnetic vortex and a nanomechanical resonator. We show that the gyrotropic mode of the vortex can coherently couple to the quantized mechanical motion of the resonator through magnetic interaction. Benefiting from the topologically protected properties and the low damping of vortices, as well as the excellent coherent features of nanomechanical resonators, the proposed system can achieve strong coupling and even the ultrastrong coupling regime by choosing appropriate parameters. In combination with other quantum systems, such as a nitrogen-vacancy (NV) center, coherent state transfer between the vortex excitation and the spin can be realized. The purpose of this study is to determine whether preoperative 6-minute walk test (6MWT) is associated with prolonged postoperative hospitalization in high risk patients with severe tricuspid regurgitation (TR) after transcatheter tricuspid valve replacement (TTVR). Forty-one patients with severe TR who underwent TTVR and discharged between September 2018 and April 2021 were enrolled in this study. Patients were divided into 2 groups according to whether the postoperative hospital stay was10 days and patients' data were retrospectively collected. 6MWT was performed before operation. Twenty-one patients were in the control group (≤10 days) and 20 patients were in the prolonged postoperative stay (PPS) group (10 days). 6MWT distance was significantly decreased in PPS group (192.70 ± 62.34 vs 274.57 ± 52.09 m, P .05). PPS group had more patients with severe liver disease (50.00% vs 19.05%, P .05), higher systolic pulmonary artery pressure (45.05 ± 9.28 vs 35.57 ± 8.91 mm Hg, P .05) and longer procedure time (159.85 ± 56.61 vs 124.43 ± 31.67 min, P .05). Multivariable logistic regression analysis found 6MWT267 m was an independent risk factor with the odds ratio of 10.95 (1.66-72.39, P .05) for prolonged postoperative hospitalization in patients who received TTVR. In the present study, we identified that preoperative decreased 6MWT distance was an independent risk factor for prolonged hospitalization in high risk TR patients after TTVR.

Objective. Metformin is a common antidiabetic drug that has been reported to serve as an anticancer agent in combination with other therapies. But the effect of the addition of metformin on the survival of non-small cell lung cancer (NSCLC) patients undergoing chemotherapy is still controversial. We conducted this systematic review to evaluate the survival effect of metformin added to chemotherapy in NSCLC patients. Methods. Electronic literature search was performed in the PubMed, Embase, and Web of Science databases from their inception up to April 2023. The study region, study design, histological subtype of the NSCLC, tumor stage, treatment strategy, sample size, follow-up duration, diabetes status, and HR of OS or PFS of the included studies were extracted. The quality was assessed through Cochrane collaboration’s tool for RCT and the Newcastle–Ottawa scale (NOS) for observational studies, respectively. Results and conclusions. Eleven studies with a total of 4606 patients were finally included. Five RCTs showed a high risk of bias due to the open-label nature while six retrospective studies were of high quality. Two studies of NSCLC patients with diabetes reported significant benefits in overall survival from metformin addition, while one study of patients without diabetes reported a negative effect on the survival of metformin addition. The survival impact of metformin added to chemotherapy on unresectable NSCLC patients remains inconclusive. The survival benefit might be more prominent in patients with diabetes, awaiting further evidence.

Free-form surfaces have been widely used in aerospace, automotive and other fields. Due to its complex geometry, free-form surface inspection is generally conducted by touch-trigger or measuring probe-based Coordinate Measurement Machines or On-machine Measurement. Sampling strategy plays a decisive role in improving both measurement accuracy and efficiency, which is determined by sample size and distribution of sample points. However, it is difficult to simultaneously take the surface curvature, sampling density and approximation error into account, considering the complexity of surface geometry. In this paper, triangle mesh simplification is innovatively adopted in sampling planning to achieve multi-geometric constraints. As triangle mesh has outstanding advantages in representing the surface features, strong stability and is easy to modify its structure, free-form surface is converted to a dense triangle mesh. Triangle mesh simplification is implemented by iteratively contracting triangle edges. An improved quadric error metric is established to decide contraction order and optimal target vertices under discrete curvature constraint. Sampling density is controlled by limiting the triangle edge length. Detailed adaptive sampling algorithm under multi-geometric constraints is then developed. Both simulation and experiment are conducted to validate feasibility and robustness of the proposed method. The results are compared with uniform sampling and existing adaptive sampling strategy to show that the proposed method can prominently reduce sampling error when sample size is small.

Aluminum alloy is applied in aerospace widely and prominently due to its advantages in physical and mechanical properties. The clamp method for thin-walled aluminum alloy workpiece remains an important challenge in processing field. This paper presents an optimized clamp method for thin-walled aluminum alloy rotational conical with large diameter during processing to ensure the wall thickness of the parts in various regions. Based on ANSYS, the vibration modal analysis model and the simplified cutting model are established respectively, for studying the modality and machining distortion. The influences of three clamping conditions on the most sensitive area of parts are compared. Base on the simulation analysis, the verified machining experiments of three different clamp methods are conducted. As the study subjects, the dimensional precision of wall thickness and the weight of parts are analyzed. The results show that the optimized contour support clamping condition can effectively guarantee the quality and precision of parts’ processing.

ObjectiveTricuspid regurgitation (TR) is a common valvular heart disease with unsatisfactory medical therapeutics and high surgical mortality. The present study aims to evaluate the safety and effectiveness of transcatheter tricuspid valve replacement (TTVR) in high-risk patients with severe TR.MethodsThis was a compassionate multicentre study. Between September 2018 and November 2019, 46 patients with TR who were not suitable for surgery received compassionate TTVR under general anaesthesia and the guidance of trans-oesophageal echocardiography and fluoroscopy in four institutions. Access to the tricuspid valve was obtained via a minimally invasive thoracotomy and transatrial approach. Patients’ data at baseline, before discharge, 30 days and 6 months after the procedure were collected.ResultsAll patients had severe TR with vena contracta width of 12.6 (11.0, 14.5) mm. Procedural success (97.8%) was achieved in all but one case with right ventricle perforation. The procedural time was 150.0 (118.8, 180.0) min. Intensive care unit time was 2.0 (1.0, 4.0) days. 6-month mortality was 17.4%. Device migration occurred in one patient (2.4%) during follow-up. Transthoracic echocardiography at 6 months after operation showed TR was significantly reduced (none/trivial in 33, mild in 4 and moderate in 1) and the primary safety end point was achieved in 38 cases (82.6%). Patients suffered from peripheral oedema and ascites decreased from 100.0% and 47.8% at baseline to 2.6% and 0.0% at 6 months.ConclusionsThe present study showed TTVR was feasible, safe and with low complication rates in patients with severe TR.

By direct electrospinning poly(vinylidene fluoride) (PVDF)/Si3N4 blend solution on the Na2Ni[Fe(CN)6] electrode, a flexible separator@electrode combination is obtained. This combination is closely cross-linked by the -CH2 CF2- chain of β-PVDF in the separator and electrode, which delivers unique advantages in flexibility, thermal stability, tensile strength and interfacial resistance. Meanwhile, the PVDF/Si3N4 separator has a higher porosity (84.2%) and ionic conductivity (4.1 × 10−3 S cm−1), making the combination display an excellent electrochemical performance with a high initial coulombic efficiency of 96.8%, capacity retention rate of 92.2% after 503 cycles and rate capability of 78 mAh g − 1 at 5 C.
