Asthma, with several phenotypes and endotypes, is considered particularly suited for precision medicine. The identification of different non-invasive biomarkers may facilitate diagnosis and treatment. Recently, Staphylococcus aureus and its enterotoxins (SE) have been found to have a role in inducing persistent type 2 airway inflammation in severe asthma, but also in such comorbidities as chronic rhinosinusitis with nasal polyposis (CRSwNP).The aim of this retrospective study was to evaluate the prevalence of SE-IgE sensitization in a multicentric Italian cohort of severe asthmatic patients and correlate it with demographic and clinical characteristics.A total of 249 patients were included in the analysis, out of which 25.3% were staphylococcal enterotoxin B (SEB)-IgE positive. We found a meaningful association between SEB-IgE and female gender, a positive association was also measured between CRS and CRSwNP. No significant association was found between SEB-IgE sensitization and atopy, the occurrence of exacerbations and corticosteroid dosages. In the SEB-IgE-positive patient, blood eosinophil count does not appear to be correlated with the severity of the disease. Patients with SEB-IgE sensitization are, on average, younger and with an earlier disease onset, thus confirming the possibility to consider SEB-IgE sensitization as an independent risk factor for developing asthma.Our data confirm that the search for SE in the initial screening phase of these patients is helpful to better phenotype them, may predict the evolution of comorbidities and lead to a targeted therapeutic choice; in this point of view this represents a goal of precision medicine.

Introduction: Type 2 targeting biologics have reached the market first for asthma and since 2019 also for CRSwNP. As clear guidelines and predictors for optimal biological choice are missing, patients are sometimes required to switch biologic therapy in order to find the optimal treatment result. In this paper, we evaluate reasons for switching biologics and the treatment effects after each sequential switch. Materials and methods: Ninety-four patients who switched from one biologic to another for their treatment of CRSwNP and asthma were evaluated. Results: Twenty patients experienced satisfactory control of CRSwNP, but insufficient control of severe asthma. Fifty-one patients experienced satisfactory control of severe asthma, but insufficient control of CRSwNP/EOM. Twenty-eight patients experienced insufficient control of both upper and lower airways. Thirteen patients had to switch because of side effects. Furthermore, two cases are described to clarify clinical decision-making. Discussion: For abovementioned patients, a multidisciplinary approach is mandatory to find the best suitable biologic. It seems ineffective to switch to a second anti-IL5 treatment if the first one is not successful. Most patients that failed omalizumab and/or an anti-IL-5 treatment are well controlled on dupilumab. Therefore, we suggest to use dupilumab as first choice when switching biologic agents.

The complex pathophysiology of chronic rhinosinusitis with nasal polyps (CRSwNP) generates a spectrum of phenotypes with a wide variety of inflammatory states. We enrolled 44 very-likely-to-be type 2 CRSwNP patients in order to evaluate the load of inflammation and to analyze human interleukins in nasal secretion. Clinical data were collected to evaluate the severity of the disease. High levels of IL-5, IL-4, IL-6, and IL-33 were detected in all type 2 CRSwNP patients. By analyzing type 2 cytokine profiles and local eosinophil count, we identified two coherent clusters: the first was characterized by high levels of IL-4, IL-5, IL-6, and a high-grade eosinophil count (type 2-high); the second had lower levels of cytokines and poor or absent eosinophilic inflammation (type-2 low). IL-5 levels were significantly higher within the type 2 cytokine and it was the most reliable biomarker for differentiating the two clusters. In type 2-high inflammatory profile clinical scores, the mean number of previous surgeries and need for systemic corticosteroids were significantly higher compared to type 2-low. Our research demonstrated the potential role of type 2 biomarkers, and in particular, of IL-5 in identifying patients with a more severe phenotype based on a high inflammatory load.

Objective: The purpose of this article is to describe BPPV in COVID-19 patients by discussing the possible mechanisms underlying the onset of this vertigo. Methods: We studied eight patients (4 F, 4 M, aged between 44 and 69 years) with COVID-19 infections complaining of vertigo. Patients were evaluated at the end of infection with an accurate clinical history, and the investigation of spontaneous, positional and positioning nystagmus. Results: The vestibular findings showed benign paroxysmal positional vertigo (BPPV) in all the patients. Three patients had a mild phenotype of the COVID infection, whereas five subjects were hospitalized for the COVID infection and in three cases intensive care was required. Vestibular evaluation showed an involvement of posterior semicircular canals in five patients and horizontal in three. Three patients were treated with the Epley maneuver, two with Semont, one with Lempert and two with Gufoni maneuvers. Conclusions: We hypothesize that BPPV in COVID-19 infections can be relate to drugs, prolonged bed rest and to direct damage by viral infection on the peripheral vestibular system and in particular on the otolitic membrane due to the cytopathic effect of the virus and to the inflammatory response. Studies on large series of patients are needed to confirm our preliminary observation and to better evaluate the pathophysiological mechanisms underlying BPPV in these patients.

Approccio multidisciplinare ai tumori maligni del vestibolo del naso: verso la definizione di nuovi standard.I tumori maligni del vestibolo del naso, principalmente SCC, sono considerati neoplasie rare. Nel presente lavoro, rivediamo lo stato attuale dell’arte per quanto riguarda la classificazione e il trattamento, e descriviamo le evidenze attuali a supporto di un cambiamento di paradigma. Nell’attuale classificazione AJCC il vestibolo del naso è considerato parte della cavità nasale/etmoide. Nella pratica clinica quotidiana, le lesioni del vestibolo del naso sono spesso erroneamente classificate come primarie della pelle. Questo porta a una sottovalutazione della reale incidenza e a una cattiva gestione. Quando i tumori primitivi del vestibolo del naso sono correttamente classificati come a origine dal naso/seni paranasali, l’attuale AJCC TNM appare inadeguato per la stratificazione prognostica e un vecchio sistema di stadiazione descritto 4 decenni fa da Wang ha dimostrato di essere più affidabile ed è preferito nei centri con il maggior volume di casi trattati. I principi della classificazione di Wang dovrebbero essere applicati e il vestibolo del naso dovrebbe essere riconosciuto dall’AJCC/UICC come una nuova sottosede distinta di naso e seni paranasali. La chirurgia, la radioterapia a fasci esterni (EBRT) e la radioterapia interventistica (IRT, BrachyTherapy BT) sono le attuali opzioni terapeutiche per il carcinoma del vestibolo del naso (NVSCC). Sempre più evidenze dimostrano che la IRT, da effettuarsi in un contesto rigorosamente multidisciplinare, è almeno equivalente alla chirurgia e all’EBRT per il trattamento delle lesioni primarie in termini di esiti oncologici, ma nettamente superiore in termini di risultati estetici e funzionali, supportando l’HDR (high dose rate) IRT come nuovo standard per il trattamento della lesione primaria in questi tumori maligni. Per ottimizzare i vantaggi della IRT come modalità terapeutica primaria abbiamo impostato un nuovo approccio alla fase di impianto della IRT sfruttando i piani anatomici della chirurgia estetica e funzionale del naso e le potenzialità della brachiterapia a intensità modulata e guidata dalle immagini, per evitare la perforazione del setto e dell’ala (impianto anatomico).Nose vestibule malignancies, mainly SCCs, are considered rare neoplasms. In the present paper, we review the current state of the art concerning classification and treatment, and describe current evidence supporting a paradigm shift. In the current AJCC classification nose vestibule is considered part of nasal cavity/ethmoid. In daily clinical practice, nose vestibule lesions are often misclassified as skin primaries. This leads to an underestimation of the real incidence and to a mis-management. When nose vestibule primaries are correctly classified as nose primaries, the current AJCC TNM appears inadequate for prognostic stratification and an old staging system described 4 decades ago by Wang has been demonstrated to be more reliable in the literature and is preferred in centers with the largest volume of cases treated. The principles of Wang classification should be applied and nose vestibule acknowledged as a new distinct subsite of nose and paranasal sinuses by the AJCC/UICC. Surgery, External Beam RadioTherapy (EBRT) and Interventional RadioTherapy (IRT, BrachyTherapy BT) are the current therapeutic options for nose vestibule (NV) SCC.Increasing evidence demonstrates that IRT, with a proper multidisciplinary approach, is at least equivalent to surgery and EBRT for treatment of the primary lesions in terms of oncological outcomes, but markedly superior in terms of cosmetic and functional results, supporting HDR (high dose rate) IRT as the new standard for the treatment of the primary lesion in these malignancies. To optimize the advantages of IRT as primary therapeutic modality we set up a new approach to the implantation phase of IRT exploiting the anatomic planes of esthetic and functional nose surgery and the potential of intensity modulated and image guided brachytherapy to avoid septal and alar perforation (anatomic implantation).

STUDY OBJECTIVES: Different therapeutic strategies have been investigated for the treatment of positional obstructive sleep apnea, but more evidence is needed about efficacy and compliance. The objective of this study was to describe the efficacy of vibrotactile neck-based treatment in patients with positional obstructive sleep apnea with different degrees of obstructive sleep apnea severity who were followed for 6 months. METHODS: This is a retrospective study including 162 patients with positional obstructive sleep apnea undergoing vibrotactile neck-based positional therapy. We compared polysomnographic data obtained at baseline and during positional therapy after 1 month. We performed a subgroup analysis based on obstructive sleep apnea severity. Furthermore, we analyzed follow-up data in 84/162 (51.8%) patients with particular focus on discontinuation and complications related to the device. RESULTS: We observed a significant difference between mean baseline obstructive apnea-hypopnea index (OAHI; 21.9 ± 9.9 events/h) and during positional therapy (12 ± 9.2 events/h; P < .01). Moreover, 87/162 (54.9%) patients showed a reduced baseline OAHI of at least 50% and 38/162 (23.4%) achieved complete disease control (OAHI < 5 events/h). At subgroup analysis, at least 50% reduction from baseline OAHI was observed in 56.8% of patients with mild, 55% with moderate, and 47.4% with severe OAHI, whereas complete control of disease was achieved in 50% of patients with mild, 22.5% with moderate, and 7.9% with severe OAHI. At a 6-month follow-up, only 35/84 patients (41.6%) were regularly using the device, with a mean of 5.9 ± 1.2 days per week. CONCLUSIONS: Our results on the efficacy and long-term adherence to vibrotactile neck-based positional therapy showed that positional therapy can be an efficient first-line treatment option for mild positional obstructive sleep apnea and in selected cases of moderate disease. Purpose of Review: Non-allergic rhinitis (NAR) includes different subtypes, among which NAR with eosinophilia syndrome (NARES) is the most important because of severity of symptoms and the high risk of comorbidities. Its pathophysiology is still object of debate, but a crucial role of chronic eosinophilic inflammation has been recognized. The aim of this review is to critically analyze the current evidence regarding the hypothesis that NARES may be considered a type 2 inflammatory disorder. Recent Findings: The definition and diagnostic criteria for NARES are not universally shared and adopted, thus generating difficulties in reproducing the results. At present, there is extreme heterogeneity in sampling methods and disagreement in the cut-off of local eosinophilic count to determine a diagnosis of NARES. The PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) standard was applied to identify English-language experimental and clinical articles regarding NARES. The search was performed in April 2021. Twenty-six articles were included. Summary: Our data suggest a particular heterogeneity regarding sampling and specific cut-offs adopted for diagnosis of NARES and consensus should be reached. We suggest that eosinophil count should be reported as an absolute value for at least 10 observed rich fields in order to increase the level of standardization. Consensus among authors on this topic should be reached with particular attention to the cut-off for diagnosis. In the future, this limitation may be overcome by the identification of repeatable biomarkers to refine diagnosis and prognosis of NARES. Furthermore, our data strongly suggest that NARES have numerous similarities with clinical features of the most common type 2 diseases such as eosinophilic asthma and chronic rhinosinusitis with nasal polyps (CRSwNP): late onset, association with type 2 comorbidities, selective eosinophilic tissue infiltration, remarkable response to oral and intranasal corticosteroids, and progression in a type 2 CRSwNP.

OBJECTIVES The primary end point of this study was to evaluate the impact of bile acids on severity of laryngo-pharyngeal reflux (LPR) and the possible correlation with esophagitis and upper airway malignancies. The second end point was to evaluate if salivary bile acids and molecules other than pepsin might serve as diagnostic biomarkers of LPR. DESIGN Observational prospective comparative study. SETTING Otorhinolaryngology unit of a tertiary hospital. PARTICIPANTS Sixty-two consecutive adult outpatients suspected of LPR. MAIN OUTCOME MEASURES Bile acids, bilirubin and pepsinogen I-II were measured in saliva. Patients underwent pH metry and based on the results of bile acids were subdivided as acid, mixed and alkaline LPR. RESULTS Significantly higher Reflux Findings Score (RFS) and Reflux Symptoms Index (RSI) were seen in patients with alkaline and mixed LPR compared to acid LPR. Salivary bile acids >1 µmol/L seem to be a reliable indicator of the severity of LPR. Compared to those without, patients with esophagitis or a history of upper airway malignancy have high concentrations of bile acids in saliva. Among the molecules studied, bile acids were the most suitable for diagnosis of LPR, with a sensitivity of 86% and a positive predictive value of 80.7%. CONCLUSIONS Our data suggest that high concentrations of bile acids are associated with higher values of RSI and RFS in LPR as well as a higher risk of esophagitis and history of upper airway malignancies. We finally observed that bile acids provided the best biometric parameters for diagnosis of LPR among the molecules tested.

COVID-19 e olfatto: revisione critica della letteratura sulla valutazione olfattiva.Da dicembre 2019, in seguito alla comparsa in Cina di una nuova infezione da Coronavirus (COVID-19), si è assistito alla diffusione di una pandemia. In un numero significativo di pazienti è stata riscontrata la comparsa di alterazioni dell’olfatto anche in assenza di altri sintomi tipici dell’infezione. In questa revisione della letteratura sono stati inclusi gli articoli pubblicati da gennaio 2020 sull’alterazione olfattiva nei pazienti COVID-19 e in particolare i metodi utilizzati per la diagnosi. L’analisi della letteratura ha mostrato che la diagnosi dell’alterazione olfattiva in questi pazienti è stata eseguita principalmente mediante questionari. Pochi studi si sono basati sulla valutazione olfattiva mediante test psicofisici validati. Sono emerse discrepanze tra l’alterazione olfattiva riportata nei questionari e la valutazione olfattiva eseguita mediante test psicofisici. Ad oggi lo studio dell’olfatto nei pazienti COVID-19 è stato eseguito in maniera eterogenea e principalmente basata sui sintomi riferiti dai pazienti portando a risultati spesso contrastanti. Gli autori suggeriscono di utilizzare test olfattivi psicofisici, semplici, validati e somministrabili anche in assenza di un operatore al fine di confermare l’alterazione olfattiva in questi pazienti che spesso sono in quarantena, isolati o ospedalizzati.Since December 2019, an outbreak of a newly isolated coronavirus (SARS-CoV-2) appeared in Wuhan, China, and then spread worldwide. Recently, it has emerged that a number of patients may present with sudden hyposmia, sometimes without other symptoms of the disease. We performed a critical review on the methods used to date to investigate the olfactory function in COVID-19 patients in order to establish which should be considered the most appropriate to use during this pandemic. Literature analysis showed that the diagnosis of hyposmia in COVID-19 patients was mainly made through subjective symptomatology collected by questionnaires and/or interview. Psychophysical tests were carried out in a few studies showing significant discrepancies between the self-reported sense of smell and test results. To date the methods used by authors to investigate smell impairment in COVID-19 patients have been very heterogeneous and predominantly based on self-reported questionnaires leading to confusing and inconclusive results. We suggest that simple validated self-administered psychophysical olfactory tests could be a valuable instrument to investigate isolated/quarantined or hospitalised COVID-19 patients referring smell impairment in order to confirm olfactory dysfunction.

Benign paroxysmal positional vertigo (BPPV) is the most common cause of peripheral vertigo and its recurrence is fairly common. Several studies correlated the pathophysiological role of different comorbidities—such as diabetes, osteoporosis, vascular, psychiatric and autoimmune diseases—in the development and recurrence of BPPV. The aim of this study is to analyse the pharmacological history of patients with idiopathic BPPV in relation to the risk of developing recurrence. Data regarding 715 patients aged 12 to 87 years (62.7 ± 14) with non-traumatic BPPV were retrospectively evaluated. Il trattamento endoscopico dell’atresia coanale e utilizzo delle dilatazioni mediante balloon: nostra esperienza.Scopo di questo studio è quello di descrivere i risultati della nostra esperienza nel trattamento chirurgico endoscopico della atresia coanale e di illustrare in particolare il nostro protocollo che per mantenere pervio lo spazio coanale non prevede l’applicazione di uno stent ma dilatazioni endoscopiche mediante balloon ripetute nel post-operatorio al fine di stabilizzare il risultato chirurgico. Nel periodo compreso fra dicembre 2014 e dicembre 2018, abbiamo arruolato 46 pazienti affetti da atresia coanale e sottoposti a trattamento chirurgico presso l’Unità di Otorinolaringoiatria dell’Ospedale Pediatrico Bambino Gesù (17 con atresia coanale bilaterale e 29 con atresia coanale monolaterale), Tutti i pazienti sono stati sottoposti a chirurgia endoscopica transnasale. L’incisione della mucosa è stata realizzata con strumentazione a freddo e a caldo (laser a diodi). L’eliminazione della componente ossea è stata possibile mediante l’utilizzo di frese; la calibrazione delle neo-coane è stata eseguita con la tecnica di dilatazione mediante balloon. Nessuno stent è stato utilizzato e nessun paziente ha ricevuto trattamento topico con mitomicina. Il primo controllo endoscopico post-operatorio con eventuale dilatazione è stato eseguito 7 giorni dopo la procedura chirurgica. Una seconda valutazione endoscopica con medicazione e dilatazione è stata effettuata 15 giorni al fine di stabilizzare il risultato chirurgico. Ulteriori controlli sono stati pianificati ad un mese dall’intervento e i successivi sulla base dei progressi del singolo paziente. In nessuno dei 46 pazienti trattati abbiamo osservato complicanze dopo i trattamenti endoscopici. L’età media dei 17 pazienti con atresia coanale bilaterale è stata di 10,4 giorni al momento del primo trattamento; nel 64% dei casi l’atresia era associata ad altre anomalie. Il protocollo terapeutico ha garantito il raggiungimento di outcome soddisfacenti nell’82% dei casi, con una media di 4.6 procedure realizzate per ogni paziente (range: 3-11). I 29 pazienti affetti da atresia monolaterale presentavano un’età media di 36,6 mesi al primo trattamento, una prevalenza di atresia monolaterale destra (11:6), un’associazione con ulteriori anomalie nel 27% dei casi. Abbiamo ottenuto una percentuale di successo dell’85,7% con una media di 3,5 procedure per paziente (range: 3-7). La chirurgia endoscopica transnasale è attualmente considerata il gold standard terapeutico, sia per l’atresia monolaterale che bilaterale. A nostro parere, andrebbe evitato l’utilizzo di stent post-intervento, anche se ancora oggi vengono utilizzati; la recidiva è ridotta dall’utilizzo della dilatazione mediante balloon che, nella nostra esperienza, è un valido aiuto sia nel trattamento dell’atresia primaria che delle recidive.The aim of this study is to report our experience in surgical treatment of unilateral and bilateral choanal atresia. We describe the endoscopic surgical technique for creation of neo-choanes and following repeating balloon dilation after surgery to stabilise the results. The study was carried out from December 2014 to December 2018, enrolling 46 patients who underwent surgery for choanal atresia at the Otolaryngology Unit of the Paediatric hospital Bambino Gesù: 17 with bilateral choanal atresia (Group A) and 29 with unilateral choanal atresia (Group B). All patients underwent transnasal endoscopic surgery. The incision of the mucosa was made with a cold and hot system (laser diode). The elimination of atresic plaque was possible thanks to the use of a drill; the calibration of the neo-choanes was carried out with the help of balloon dilation. No stent was used, and no patient underwent treatment with topical mitomycin. The first endoscopic follow-up was made 7 days after surgery with surgical curettage and a second balloon dilation 15 days after surgery to stabilise the new choanes. Further check-ups were suggested at 1 month after surgery and later based on individual progress. In group A, the average age for the first treatment was 10.4 days, and atresia was associated with other anomalies in 64% of cases. The therapeutic protocol led to a successful outcome in 82% of cases, with an average of 4.6 procedures per patient (range: 3-11). In group B, the average age for the first treatment was 36.6 months, association with other anomalies was 27% of cases and the prevalent side was the right (11:6). We obtained surgical success in 85.7% of cases and the average number of endoscopic dilations was 3.5 (range: 3-7). Transnasal endoscopic surgery is now considered the therapeutic gold-standard for both unilateral and bilateral atresia. Even if still used, post-surgery stenting can be avoided in our opinion; relapse is reduced with the use of balloon dilation, which, in our experience, is a valid aid in both primary atresia treatment and in cases of relapse.

The objective of this study is to analyse the relationship between the endoscopic signs of laryngo-pharyngeal reflux, the lingual tonsil hypertrophy (LTH) and its inflammation, adopting Narrow Band Imaging (NBI) technology. Background Management of vestibular schwannoma (VS) is a complex process aimed at identifying a clinical indication for fractionated stereotactic radiotherapy (sRT) or radiosurgery, microsurgical resection, or wait and scan (WS). We describe the experience of our VS multidisciplinary team (MDT) at a tertiary university referral center created for diagnosis, treatment, and follow-up of VS patients. Methods We conducted a retrospective study on 132 consecutive patients referred to the MDT and managed by observation (WS), microsurgery, or fractionated sRT. The analysis included patient age, tumor size, hearing level, facial nerve function, tumor control, complications, and quality of life questionnaires. Results Among the patients, 21% were subjected to microsurgery, 10% to sRT, and 69% to WS. The median follow-up time was 30 months. Outcomes based on different management modalities are described. Statistically significant differences among groups were detected in terms of quality of life (physical domain). Conclusions MDT may provide the best individualized therapy for VS patients compared with a single gold-standard strategy.

Irreversible bilateral sensorineural hearing loss is a common side effect of platinum compounds. Because of the extended overall survival, a prolonged hearing surveillance and management of hearing impairments are emerging concerns for pediatric oncology.In this retrospective observational study, we enrolled 38 children out of 116 treated at our institution by chemotherapy (cisplatin and/or carboplatin) with or without irradiation between 2007 and 2014, submitted to hearing monitoring before every cycle of chemotherapy, and who completed a 5-year long-term audiological follow-up. Chemotherapy regimens, demographic findings, cumulative doses, and cranial irradiation were compared.At the end of 5-year follow-up, ototoxicity was significantly increased compared to that observed at the end of chemotherapy (52.5% vs 39.5%, p 0.001). A late onset of hearing loss was experienced in 13.1% of children, while in 26.3% progressive hearing loss was measured. Deafness at the end of chemotherapy and irradiation were significant prognostic factors for late ototoxicity outcomes (Odds Ratio 7.2-CI 1.67-31.1-p 0.01 and 5.25-CI 1.26-21.86-p 0.01 respectively). No significant differences were found between cisplatin and combined treatment (i.e., cisplatin shifted to carboplatin during monitoring for the onset of ototoxicity) and ototoxicity was not associated with platinum compounds cumulative dose (p 0.05). 13.1% of children needed hearing aids at the end of follow-up.Long-term monitoring of at least 5 years prevents the harmful effects of hearing deprivation identifying late onset/progressive hearing loss after platinum compound chemotherapy in children thanks to early hearing rehabilitation, especially in those who underwent multimodal therapy or subjected to irradiation.

Objectives Tinnitus is a common symptom largely impactful on quality of life, especially in the elderly. Our aim was to evaluate the efficacy of self‐administered screening tests to correlate the severity of subjective perception of tinnitus with emotional disorders and the overall cognitive status. Methods Patients aged ≥ 55 years with chronic tinnitus were recruited and submitted to a complete audiological evaluation; Tinnitus Handicap inventory (THI); Hospital Anxiety and Depression Scale (HADS‐A and HADS‐D) and Mini‐Mental State Examination (MMSE). Demographic and audiological features of patients with and without cognitive impairment (MMSE score cut‐off of 24/30) were analyzed in order to reveal the relationship among tinnitus, emotional disorders, and cognitive dysfunction. Results 102 patients were recruited (mean age: 70.4 ± 9.6). THI score was directly related to HADS‐A score (r = .63) HADS‐D score (r = .66), whereas there was no relationship between tinnitus severity and MMSE (r = .13). CI and n‐CI groups did not differ in the characteristics of tinnitus (p > .05), however, hearing threshold (p = .049) and anxious depressive traits measured with HADS‐A (p = .044) and HADS‐D (p = .016) were significantly higher in the group with cognitive impairment. Furthermore, age ≥ 75 years (p = .002, OR = 13.8), female sex (p = .032; OR = 6.5), severe hearing loss (p = .036; OR = 2.3), and anxiety (p = .029; OR = 9.2) resulted risk factors for CI. Therefore, in CI group MMSE score was inversely related to age (r = −.84). Conclusions Cognitive impairment and psychiatric discomfort should be considered in tinnitus patients, related to increasing age, female sex, and severe hearing loss. Thus, self‐administered questionnaires can be useful in addressing clinical approach., Anxious/depressive traits are higher prevalent in geriatric tinnitus patients. Risk of cognitive dysfunction increases in relation to increasing age, female sex and severe hearing loss. Aging is an independent factor for the development of cognitive dysfunction in tinnitus‐suffers as well as severe hearing loss; Self‐administered screening tests for global cognitive and emotional status are recommended in managing of older patients with tinnitus.

BACKGROUND Salvage total laryngectomy (TL) and laryngopharyngectomy (LP) after chemoradiotherapy may produce disfiguring defects with severe complications that require complex reconstructions. METHODS Between January 2012 and December 2018, we enrolled 25 patients who underwent internal mammary artery perforator (IMAP) flap reconstruction after salvage TL or LP. We performed retrospective review of clinical charts to collect information such as history, timing of reconstruction, type of defect, follow-up, donor and recipient site complications, and overall flap survival (OFS) rate. Three years overall survival (OS), relapse-free survival (RFS), and disease-specific survival (DSS) were calculated. RESULTS The OFS rate was 95%. One partial flap necrosis was recorded. No donor-site complications were found. The mean follow-up was 18 months. Three years OS was 44%, RFS was 47%, and DSS was 54%. CONCLUSIONS IMAP flap appears to be safe, versatile, and easy to harvest, with minimal donor site morbidity. It is a reliable option in Head&Neck reconstruction, in salvage surgery as well.

Objective: The aim of our study was to analyze the montelukast effectiveness in improving oculonasal symptoms, patient-reported outcomes (PROs), and eosinophilic biomarkers in patients with nonallergic rhinitis eosinophilic syndrome (NARES).Methods: We enrolled prospectively 80 symptomatic patients treated with 10 mg once a day of montelukast in mono-therapy for 2 months. All patients were investigated before and after treatment. Nasal symptoms (nasal obstruction, rhinor-rhoea, sneezing, nasal itching), ocular symptoms (redness/puffiness, watery eyes), and other PROs (olfactory dysfunction,difficulty going to sleep, nighttime awakenings, and nasal congestion on awakening) were scored by visual analogic scale. Thefollowing clinical scores were assessed: Total Nasal Symptom Score (T4NSS), Total Ocular Symptom Score (T2OSS), TotalSymptom Score of Patient-Reported Outcomes (TSS-PROs), and a Composite Symptoms Score (CSS). Patients were classified asresponders when a reduction of at least 50% of the CSS was observed. Before and after treatment, the eosinophilic biomarkersin nasal lavage were analyzed: nasal eosinophilia (number of eosinophils per high power field), eotaxin-1 and eotaxin-2.Results: Aft er tre atment, s ig nificant reductions were observed for all the symptom scores. Forty-two of 78 patients were con-sidered responders. A significant reduction of eosinophils in nasal mucosa and of levels of eotaxin-1 and eotaxin-2 in nasal lavagewere observed after treatment in responder patients. Patients with asthma had an increased probability to be responders.Conclusion: NARES patients may benefit from treatment with montelukast. In particular, the presence of concomitantasthma may be predictive of a greater efficacy.

Chronic suppurative otitis media (CSOM) may result in a serious impairment of the quality of life (QoL). The most relevant characteristics of CSOM are well represented in the "Chronic Otitis Media Outcome Test 15" questionnaire (COMOT-15). The aim of the study was to translate and adapt the COMOT-15 questionnaire into Italian.This is a prospective study conducted on 52 Caucasian Italian-speaking patients with CSOM and 52 controls. The linguistic adaptation of the COMOT-15 consisted of the translation of the English version into Italian and reliability assessment of the translation. Psychometric measurements included internal consistency and test-retest reliability.Cronbach's α was 0.95. The test-retest reliability showed a strong positive correlation for total score and all subscales. Age and gender had no influence on the scores. The correlation between the COMOT-15 and the pure tone average showed a significant association for the scale "Hearing Function".The Italian COMOT-15 questionnaire provides good internal consistency and is suitable for QoL evaluation in Italian-speaking patients suffering from CSOM. In addition, it is able to evaluate the subjective symptoms perceived by patients.Adattamento culturale e validazione in italiano del questionario COMOT-15 nella valutazione degli outcomes del trattamento dell’otite media cronica.L’otite media cronica suppurativa (OMCS) può determinare una grave compromissione della qualità della vita (QoL) dei pazienti. Gli aspetti clinici più rilevanti dell’OMCS sono rappresentati nel questionario “Chronic Otitis Media Outcome Test 15” (COMOT-15). Scopo dello studio è tradurre e adattare alla lingua italiana il questionario COMOT-15.Abbiamo effettuato uno studio prospettico di validazione arruolando 52 pazienti con diagnosi di OMCS e 52 controlli. Il processo di adattamento linguistico del questionario è consistito nella traduzione della versione originale inglese in italiano e nella misura dell’affidabilità della stessa. Le misure psicometriche che abbiamo preso in considerazione includevano coerenza interna e affidabilità test-retest.Il Cronbach α era pari a 0,95. Abbiamo riscontrato una forte correlazione positiva sia per il punteggio totale che per tutte le sottoscale. Età e sesso non hanno influenzato i punteggi. L’analisi delle correlazioni tra COMOT-15 e soglia uditiva per la via aerea ha mostrato un’associazione significativa per la sottoscala “Funzione uditiva”.Sulla base dei nostri risultati è stato possibile dimostrare che la versione italiana del questionario COMOT-15 da noi proposta ha una buona coerenza interna ed è particolarmente adatta per la valutazione della QoL nei pazienti di lingua italiana affetti da otite media cronica suppurativa. Inoltre, esso è in grado di valutare i sintomi soggettivi percepiti dai pazienti.

Introduction: Stapes surgery is a safe procedure, with favourable hearing outcome. The objective of the study is to assess the long-term hearing results, addressing the bone conduction (BC) decay and the need for hearing aids in otosclerosis patients. Methods: We enrolled patients who underwent stapes surgery by means of stapedectomy or stapedotomy between 1991 and 2001. All enrolled patients underwent pure-tone audiometry (PTA) between September 2017 and June 2018. A set of questions was administered to record the prevalence of subjective symptoms and the need for hearing aids. Results: Seventy patients were enrolled for a long-term evaluation; 37 patients underwent bilateral surgery; therefore, 107 ears were included in the analysis. The average follow-up period was 22 years. No statistically significant difference was found between early and late post-operative air conduction (AC) PTA (41 vs. 49 dB; p > 0.05) nor between early and late post-operative BC-PTA (29 vs. 37 dB; p > 0.05). A significant difference was observed for AC at 8 kHz (65 vs. 78 dB; p < 0.05) and BC at 2 and 4 kHz (28 vs. 40 dB and 45 vs. 58 dB, respectively; p < 0.05). Conclusions: This is, to our knowledge, the longest mean follow-up time in the literature. A mild decrease in both AC and BC threshold can be expected and the sensorineural decay is more pronounced on the high frequencies. The subjective hearing symptoms and overall sound perception are satisfactory.

Parastomal recurrence of squamous cell carcinoma (SCC) with tracheal involvement following salvage total laryngectomy after prior concurrent chemoradiotherapy is one of the most insidious challenges in head and neck surgery because a complex reconstruction is often required for covering a large area of skin loss, filling the dead space beneath, tracheal reconstruction and suspension, and tracheostome resurfacing. The aim is to describe our experience with the internal mammary artery perforator (IMAP) propeller flap for tracheal and tracheostome reconstruction and neck resurfacing after parastomal and cervical trachea resection, especially for suspension and anchoring the stump of the residual distal trachea to the island flap itself. We describe IMAP flap reconstruction after resection of parastomal recurrence of SCC requiring cervical trachea resection in five patients between January 1, 2005 and August 30, 2019. IMAP propeller flap was successfully used for reconstruction after complex resection of parastomal recurrence of SCC with cervical trachea involvement in all cases. The mean length and width were, respectively, 16.8 cm (range 13–23) and 6.9 cm (range 5.5–8). We did not report complications of both the donor and the recipient site. Pharyngo-cutaneous or tracheoesophageal fistulas and wound dehiscence were not observed. to the best of our knowledge, this is the first report about the use of the IMAP propeller flap in this more complex clinical setting and we provide the message that this surgical procedure is worthy of consideration.

Introduction: Tinnitus is a common and disabling symptom often associated with hearing loss. While clinical practice frequently shows that a certain degree of psychological discomfort often characterizes tinnitus suffers, it has been recently suggested in adults as a determining factor for cognitive decline affecting attention and memory domains. The aim of our systematic review was to provide evidence for a link between tinnitus, psychological distress, and cognitive dysfunction in older patients and to focus on putative mechanisms of this relationship. Methods: We performed a systematic review, finally including 192 articles that were screened. This resulted in 12 manuscripts of which the full texts were included in a qualitative analysis. Results: The association between tinnitus and psychological distress, mainly depression, has been demonstrated in older patients, although only few studies addressed the aged population. Limited studies on cognitive dysfunction in aged patients affected by chronic tinnitus are hardly comparable, as they use different methods to validate cognitive impairment. Actual evidence does not allow us with certainty to establish if tinnitus matters as an independent risk factor for cognitive impairment or evolution to dementia. Conclusion: Tinnitus, which is usually associated with age-related hearing loss, might negatively affect emotional wellbeing and cognitive capacities in older people, but further studies are required to improve the evidence.

Polidocanol sclerotherapy of head and neck venous malformations (VMs) and lymphatic malformations (LMs) has been reported only in limited series. In this manuscript we evaluated the efficacy and safety of polidocanol sclerotherapy in a series of head and neck venous and lymphatic malformations.This retrospective observational study analysed data on 20 head and neck VMs and LMs that underwent to percutaneous or endoscopic intra-lesional 3% polidocanol microfoam sclerotherapy at our institution. Clinical response was ranked as excellent, moderate and poor based on volume reduction by MRI and resolution of symptoms.The median volume decreased from 19.3 mL to 5.8 mL after sclerotherapy (mean volume reduction: 72.98 ± 16.1%). An excellent-moderate response was observed in 94.4% of cases. We observed a mean volume reduction of 79.5 ± 16.1 in macrocystic LMs, of 76.1 ± 13.0% in VMs, of 60.5 ± 10.9% in mixed lymphatic ones and 42.5% in microcystic lymphatic ones.Polidocanol sclerotherapy appears to be an effective and safe treatment for venous and lymphatic head and neck malformations. We observed the best responses in macrocystic LMs and VMs, whereas mixed lymphatic ones showed a moderate response and microcystic lymphatic ones a poor response.La scleroterapia con polidocanolo nella gestione delle malformazioni venose e linfatiche del distretto testa-collo.La scleroterapia con polidocanolo (POL) nelle malformazioni venose (VM) e nelle malformazioni linfatiche (LM) della testa e del collo è stata descritta solo in casisitiche limitate. In questo lavoro abbiamo valutato l’efficacia e la sicurezza del POL in pazienti affetti da VM e LM della testa e del collo.Sono stati analizzati i dati di 20 pazienti affetti da VM e LM del testa-collo, sottoposti a scleroterapia con POL al 3%, iniettato nella lesione per via percutanea o endoscopica. La risposta è stata classificata come eccellente, moderata o scarsa in base alla riduzione del volume valutata mediante risonanza magnetica e alla risoluzione dei sintomi.Il volume mediano si è ridotto da 19,3 ml a 5,8 ml dopo il trattamento (riduzione volumetrica media: 72,98 ± 16,1%). Il 94,4% dei casi ha ottenuto una riposta eccellente-moderata. In particolare è stata osservata una riduzione volumetrica media del 79,5 ± 16,1% nelle LM macrocistiche, del 76,1 ± 13,0% nelle VM, del 60,5 ± 10,9% LM miste e del 42,5% nell’unica LM microcistica.La scleroterapia con POL sembra essere un trattamento efficace e sicuro: i risultati migliori sono stati ottenuti nelle LM macrocistiche e nelle VM, mentre le LM miste hanno mostrato una moderata risposta e le LM microcistiche una scarsa risposta.

Quando è indicato un approccio chirurgico multidisciplinare nel management dei tumori primitivi cutanei non melanocitari della testa e del collo e dei tumori localmente avanzati del distretto testa-collo con interessamento della cute?I tumori della pelle non melanocitari, che comprendono soprattutto il carcinoma squamocellulare e il carcinoma basocellulare, sono i tumori maligni più frequenti nella popolazione caucasica, e la cute della testa e del collo rappresenta la sede più coinvolta. Tali tumori non dovrebbero essere sottovalutati, in particolare le cosiddette lesioni ad alto rischio e i tumori cutanei avanzati richiedono un iter diagnostico più accurato e un trattamento chirurgico più aggressivo, che dovrebbe essere gestito coinvolgendo il chirurgo testa-collo, il dermatologo e il chirurgo plastico. Le neoplasie primitive cutanee del distretto testa-collo sono spesso trascurate e non sono routinariamente discusse in seno a teams chirurgici multidisciplinari. Allo stesso modo, per i tumori primari della testa e del collo con infiltrazione cutanea, il coinvolgimento del dermatologo e del chirurgo plastico può più efficacemente definire la diagnosi e pianificare il trattamento. La gestione di questi pazienti pone problemi sia terapeutici che etici, perché la prognosi infausta è gravata da importanti inestetismi facciali, ferite aperte maleodoranti e dolore intrattabile. Pertanto, anche nei pazienti con malattia avanzata non suscettibile di chirurgia radicale, la chirurgia potrebbe ugualmente trovare spazio, ma con finalità palliativa, al fine di migliorare la qualità di vita.Non-melanoma Skin cancer, including cutaneous squamous cell carcinoma and basal cell carcinoma, is the most common form of malignancy in the Caucasian population, and the skin of the head and neck is the site most involved. They should not be underestimated; in particular, high-risk lesions and advanced skin cancers require accurate diagnostic work up, aggressive surgical treatment and should be managed by the head and neck surgeon, the dermatologist and the plastic surgeon. Cutaneous head-neck malignancies are often overlooked or not routinely treated with a multidisciplinary surgical approach. Similarly, for primary head and neck cancers with involvement of surrounding skin, the involvement of the dermatologist and the plastic surgeon could better define an adequate diagnosis and treatment planning. The management of these patients presents both therapeutic and ethical problems, because the poor prognosis is burdened by facial disfigurement, open malodorous wounds and intractable pain. Therefore, in patients with advanced disease that is not candidate to radical surgery, palliative surgery with flap reconstruction could take place and could be proposed to improve quality of life.

Introduction allergy may be an important risk factor for adenotonsillar disease in children, although conflicting results have been reported in the literature. In previous articles, authors often failed in distinguishing between adeno-tonsillar hypertrophy and recurrent tonsillitis and in not discriminating between isolated or combined adenoid and tonsillar hypertrophy. Aim to evaluate clinical evidence and biomarkers linking allergy to different phenotypes of adeno-tonsillar disease. Furthermore, we questioned whether anti-allergy treatment might prevent occurrence of adeno-tonsillar disease or improve its specific management. Methods our systematic review, in accordance with the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) process, yielded 1010 articles finally screened. This resulted in 21 full texts that were included in a qualitative analysis. Results literature data support the association between allergy and combined adeno-tonsillar hypertrophy and isolated adenoid hypertrophy, whereas describe a mainly negative correlation between allergy and isolated tonsillar hypertrophy. The results of this review suggest that local allergic inflammation may play a role in adeno-tonsillar hypertrophy. Data correlating bacterial recurrent tonsillitis and allergy are few, although evidence from the lab revealed that allergy might suppress innate immunity in tonsillar tissue by reducing levels of anti-microbial proteins. Conclusion basing on our qualitative analyses allergy should not be misdiagnosed in children with combined adenotonsillar hypertrophy or isolated adenoid hypertrophy, whereas evidence do not support a link between allergy and isolated tonsil hypertrophy. Finally, some data support a link between allergy and recurrent adeno-tonsillar infection although future studies are required to confirm this data. We summarized our conclusions in a practical algorithm.

Surgery, external beam radiotherapy (EBRT), and interventional radiotherapy (IRT, BrachyTherapy BT) are the current therapeutic options for nose vestibule (NV) squamous cell carcinoma (SCC). In this article, we evaluate the nose functional parameters of patients affected by SCCs of the NV, primarily treated by interstitial IRT comparing them with healthy controls and with patients treated with intensity-modulated EBRT.Ten patients treated by using IRT (group 1), 10 healthy controls and eight patients treated by EBRT (group 2) on the region of the nose were submitted to clinical evaluation (with the NOSE scale score), rhinomanometry, olfactory testing, nasal citology, and evaluation of mucociliary clearance through saccharine test.No long-term skin or cartilaginous toxicity are recorded. The olfactometry threshold discrimination identification TDI is lower in EB group. The mean NOSE scale score was significantly higher in group 2 than in group 1 and healthy controls (p0.05). The distribution of cytologic patterns resulted significantly different as well. Patients treated by EB have a significantly impaired mucociliary clearance, with a mean time for the transport of the stained marker, which is more than double in the patients treated by EB than in those treated with IRT (p0.001).Nasal function and cytological findings are significantly better, substantially preserved, in patients treated by IRT than in those treated by EBRT, bringing new relevant evidence for the establishment of interstitial IRT as the new standard for the treatment of the primary lesion in cT1 and cT2 -Wang staging NV SCCs.

Background allergic rhinitis is a common childhood disease responsible for a major impact on quality of life and health care resources. Many hypotheses have been proposed to explain the link between allergy and otitis media, although a definitive mechanism has not been identified yet. One of the major critical points is that authors failed in distinguishing among different phenotypes of middle ear inflammation. This review pointed out literature evidence from the laboratory and clinical experience linking allergy to different phenotypes of otitis media in children. Methods we performed a systematic review in accordance with the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) process. Our search yielded 3010 articles that were finally screened. This resulted in 20 manuscripts of which the full texts were included in a qualitative analysis. We paid particular attention in distinguishing among phenotypes of otitis media. Results clinical evidence and analyses of biomarkers suggested that allergy may be linked to some phenotypes of otitis media and, in particular, to otitis media with effusion (OME) and acute re-exacerbations in children with middle ear effusion. It was not possible to perform the analysis for allergy and acute and chronic otitis media because of paucity and heterogeneity of data. Conclusion allergy should be considered in the diagnostic work up of children with OME as well as OME should be excluded in children with persistent moderate to severe AR. In these cases clinicians should evaluate prompt and accurate treatment of allergy in improving outcomes, although futures studies are required to increase evidence supporting that anti-allergy treatment may be effective in the recovery and outcome of otitis media with effusion.

Chronic rhinosinusitis (CRS) is a multifactorial disease due to chronic inflammation of sino-nasal mucosa that persists for more than 12 weeks. Since the first edition of EPOS guidelines, CRS has been divided into two main phenotypes: CRS with nasal polyposis (CRSwNP) and CRS without nasal polyposis (CRSsNP). In recent years, several authors agreed that CRS is not a uniform disease process but rather includes different phenotypes and endotypes. Differences may be observed in terms of risk factors, co-morbid conditions associated with CRS and disease control by medical treatment and surgery. The role of allergy in chronic rhinosinusitis (CRS) has been largely discussed in the literature, but nevertheless it remains controversial. Allergy and CRS are very prevalent diseases and frequently co-occur; however, direct causality has never been clearly demonstrated. Because the association between the two diseases has not been clear for years, making an evidence-based decision of whether to evaluate or treat allergies in CRS patients was difficult. Recently, it is clear that the prevalence of allergy is variable between the different subtypes of CRSwNP and the association seems to be very strong particularly with allergic fungal rhinosinusitis (AFRS) and central compartment atopic disease (CCAD).

Background. Irreversible bilateral sensorineural hearing loss is a common side effect of platinum compounds. Because of extended overall survival period of children, a prolonged hearing surveillance and management of hearing impairments are emerging concerns for pediatric oncology. Methods. In this retrospective observational study we enrolled 38 children out of 116 treated at our institution by chemotherapy (cisplatin and/or carboplatin) with or without irradiation between 2007–2014, and submitted to hearing monitoring before every cycle of chemotherapy and that completed a 5 years long-term follow-up. Chemotherapy regimens, demographic findings, cumulative doses and cranial irradiation were compared. Results. At the end of 5-years follow-up ototoxicity was significantly increased compared to that at observed the end of chemotherapy (52.5% vs 39.5%, p 0.05). 13.1% of children needed hearing aids at the end of follow-up. Conclusions. Our study confirms the effectiveness of long-term follow-up in identifying late onset/progressive hearing loss after platinum compound chemotherapy and allows us to avoid the harmful effects of hearing deprivation with hearing aid intervention.

Cooperazione tra otorinolaringoiatra e odontostomatologo in oncologia cervico-cefalica.I pazienti affetti da tumori della testa e del collo (HNC) richiedono cure specialistiche lungo tutto l’iter diagnostico-terapeutico. La collaborazione tra otorinolaringoiatra e odontostomatologo risulta fondamentale, poiché le diverse sedi anatomiche coinvolte dalla patologia oncologica hanno caratteristiche peculiari, e i migliori risultati in termini di aspettativa e qualità di vita possono essere raggiunti solo attraverso un approccio multidisciplinare. La collaborazione sinergica di tutti gli operatori sanitari nella diagnosi precoce di tali tumori consente una prognosi migliore per questi pazienti; in quest’ottica, subito dopo la diagnosi di patologie odontostomatologiche potenzialmente maligne (PMD), i pazienti dovrebbero sottoporsi a uno screening ORL completo per l’individuazione di lesioni del tratto aero-digestivo superiore. È fondamentale che, dopo la diagnosi di carcinoma e prima di qualsiasi trattamento oncologico, questi pazienti ricevano una valutazione dentale e parodontale accurata e completa, per ottimizzare il trattamento e ridurre al minimo le complicanze o gli effetti collaterali. La pianificazione multidisciplinare ablativa, ricostruttiva e protesica nella chirurgia oncologica testa-collo è diventata indispensabile per offrire ai pazienti il miglior risultato funzionale ed estetico; un miglioramento della funzione orale e della relativa qualità di vita è infatti strettamente correlato a una corretta riabilitazione protesica. Infine, prima e dopo l’intervento chirurgico e/o la radioterapia, gli obiettivi principali del trattamento odontoiatrico in questi pazienti sono rappresentati dalla prevenzione e dal trattamento delle malattie dentali e degli effetti collaterali dei vari trattamenti oncologici a livello del cavo orale.Head and neck cancer (HNC) patients require specialized care throughout the continuum of cancer diagnosis, treatment, and survival. Cooperation between ENT and dentists is crucial, since all of the different anatomic areas affected by cancer have distinctive characteristics and the best results in terms of life expectancy and patients’ quality of life can only be achieved through a multidisciplinary approach. The synergic cooperation of all health care workers in the early diagnosis of HNC allows a better prognosis for such patients. As soon as dentists diagnose a Potentially Malignant Disorder (PMD), patients should undergo complete ENT screening for detection of lesions in any portion of the upper aerodigestive tract. Before and after cancer diagnosis and before any oncologic treatment, it is critical that these patients undergo an accurate and complete dental and periodontal assessment to optimize treatment and minimize complications or side effects. Multidisciplinary ablative, reconstructive and prosthetic programing in HeadNeck oncologic surgery has become imperative to offer patients the best functional and esthetic outcome. An improvement in oral function and associated quality of life is strictly related to correct prosthetic rehabilitation. Finally, after surgery and/or radiotherapy (RT), the main objectives of dental treatment in these patients are the prevention and therapy of dental diseases and the side effects of oncological therapies involving the oral cavity.

Purpose of the Review We provide a systematic review of experimental and clinical evidences linking allergy to acute, including common cold, and chronic rhinosinusitis in children. Furthermore, we questioned if anti-allergy treatment may prevent the occurrence of rhinosinusitis or improve outcomes of its specific management. Recent Findings Allergic rhinitis is a common childhood disease in industrialized countries that is responsible for a major impact on quality of life and healthcare resources. Over the years many authors tried to correlate allergy with comorbidities and in particular to the onset of rhinosinusitis including common cold, even though conflicting results are frequently reached. We performed a systematic review in accordance with the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) process. Our search yielded 7103 that were finally screened. This resulted in 25 publications of which the full texts were assessed and included in a qualitative analysis per different phenotypes of rhinosinusitis. Summary The evidence suggests that allergy may lead to overall impairment of mechanical and immunological defense function of the nasal mucosa against viruses and that anti-allergy treatment may significantly decrease the number and severity of upper respiratory tract infections including common colds in children. It was not possible to perform the analysis for allergy and post-viral acute rhinosinusitis, bacterial acute rhinosinusitis, and recurrent acute rhinosinusitis because of paucity and heterogeneity of data. Although there is no definitive proof of causation linking allergy to chronic rhinosinusitis, studies lead to suppose that anti-allergy treatment may improve outcomes of specific CRS treatments.

Backgroundin the era of new biological agents it is important to identify patients who may benefit from conventional therapies such as endoscopic sinus surgery (ESS) plus long-term local corticosteroids from those with patterns of inflammation that are more difficult to control post-operatively and who may benefit from other therapies.Objectivedetermine if preoperative assessment of type and grade of inflammation and clinical factors can predict disease control with ESS plus long-term local corticosteroids in chronic rhinosinusitis with nasal polyps (CRSwNP).MethodsEighty patients treated with ESS plus mometasone-furoate 200 μg BID for CRSwNP and followed for at least 1 year were enrolled (November 2017–December 2018) in this prospective observational study. Type and grade of inflammation were evaluated preoperatively by nasal cytology. Based on cellular pattern, patients were grouped as neutrophilic (n = 20), eosinophilic (n = 38), or mixed eosinophil-neutrophilic (n = 22). SNOT-22 and Lund-Kennedy Endoscopic Score were evaluated at baseline and at 3, 6, 9, and 12 months after surgery and used to define disease control.ResultsThe cumulative probability of remaining free of significant modification of endoscopic score (Lund-Kennedy Endoscopic Score >2) at 3, 6, 9, and 12 months was 0.84, 0.76, 0.71, and 0.68, respectively. At 12-month postoperative evaluation good disease control was observed in 54 of 80 patients (67.5%). Objective. The aim of this study was to obtain data from a review of cases of brown tumors (BT), which are benign lesions of bone characterized by giant cells that arise during hyperparathyroidism (HPTH). BTs may affect the maxillofacial area and manifest as a brownish, slow-growing swelling causing difficulty in the differential diagnosis. Study Design. We present data from 4 new cases of oral BTs based on a systematic literature review conducted by searching EMBASE, Medline, and CENTRAL databases, according to the PRISMA guidelines. Only articles in English were considered. Individual patient data were analyzed to identify risk factors for multiple or extraoral maxillofacial BTs. Results. In total, 167 cases (163 from 136 articles and 4 new cases; mean age 36.6 years; male-to-female ratio 1:2) were retrieved. Chronic rhinosinusitis (CRS) is defined the following way in the 2012 EPOS guidelines: nasal and paranasal sinus inflammation lasting a minimum of 12 weeks, plus at least two of the following features: blocked nose, rhinorrhoea, pain or pressure sensation over the face, hyposmia or anosmia. According to this guideline, at a minimum, the nose should be obstructed or there must be nasal drip anteriorly or posteriorly to qualify for the diagnosis. This clinical diagnosis can then be confirmed objectively through CT scanning of the sinuses or nasal endoscopy, which also permits characterisation as chronic rhinosinusitis with or without nasal polyp formation (CRSsNP and CRSwNP, respectively). Polyps characteristically contain elevated levels of IL-5 and IL-13, produced by T-helper 2 cells, as well as histamine. Through the endoscope, polyps take on a translucent, glistening appearance, with a colour between yellowish-grey and white, and are seen to contain abundant inflammatory debris of a gelatinous consistency. They arise from the mucosal linings of the nasal sinuses or the nose itself. They are particularly frequent at the ostia of the sinuses. Due to being poorly vascularised, polyps have a greyish-white appearance. It is fairly uncommon to find polyp formation confined unilaterally, despite the occurrence of a sole polyp on occasion at the middle meatus or the posterior recess of the sphenoethmoid. Polyps which form unilaterally may exhibit a difference in appearance and are a suspicious feature for inverting papilloma, fungal infection or a neoplasm. Combining sinusal CT with MRI is often beneficial diagnostically. A confirmation of unilaterality through imaging should make the clinician evaluate other possible diagnoses. Pathophysiology of chronic rhinosinusitis with nasal polyps is presented.

Background We evaluated the prognostic value of nasal cytology and clinical factors in predicting nasal polyp (NP) development in patients with history of nonallergic chronic sinonasal inflammation. Methods This was a retrospective case-control study of 295 patients followed at our institution for a mean of 85.70 ± 19.41 months. According to the inclusion criteria we enrolled 84 cases with persistent eosinophilic nonallergic sinonasal inflammation (group A) and 106 cases with neutrophilic inflammation (group B), both without evidence of NPs at the baseline. We considered as controls 105 patients affected by nonallergic noninfectious vasomotor rhinitis without evidence of inflammation at nasal cytology (group C). Patients were checked every 6 months for NPs. Temporal analyses was performed by Kaplan-Mayer curves and odds ratios were evaluated by logistic regression analyses. Results The percentage of patients that developed NPs was higher in group A (29/84 [34.52%]) than in group B (17/106 [16.03%]) and group C (5/104 [4.7%]) (p < 0.05). Logistic regression analyses showed that eosinophilic patients had a higher risk of NP development over the years than neutrophilic patients compared to controls (odds ratio [OR], 10.55 vs 3.2). We also demonstrated that hypereosinophilia, asthma, and aspirin intolerance may increase the OR differently in eosinophilic patients. Conclusion Our data suggest that early identification of inflammatory patterns and associated clinical factors in patients affected by chronic nonallergic sinonasal inflammation have a prognostic value that can help to identify patients with different risks of NP development. Our data confirm that detection of nasal eosinophilic inflammation represents an early marker for identification of a more aggressive inflammatory phenotype.

Background The aim of the present study was to measure levels of granulocyte-macrophage colony-stimulating factor (GM-CSF) in nasal lavage of patients affected by chronic eosinophilic sinonasal inflammation to clarify the relationship with eosinophilic tissue infiltration and clinical features. Methods Between November 2012 and June 2013, we selected 70 patients with chronic eosinophilic inflammation (average age 41.8 years) who were classified into the following groups: persistent allergic rhinitis (group 1), noninfectious non-allergic rhinitis with eosinophilia syndrome (group 2) and chronic rhinosinusitis with polyps (group 3). Finally, we enrolled 20 healthy subjects as controls (group 4). All patients underwent symptoms score questionnaire based on a visual analogue scale, nasal endoscopy and/or computed tomography (CT) scan, and allergy testing. Nasal cytology by scraping of the mucosa and GM-CSF assays in nasal lavage were performed in all subjects. Results Detectable levels of GM-CSF were found in 34 of 70 (48.57%) patients, with an average concentration of 2.67 ± 0.8 pg/mL, whereas in controls only 1 of 20 individuals showed detectable GM-CSF levels. Eosinophil infiltration was significantly higher in patients with detectable GM-CSF compared to those with undetectable levels (49.4% vs 39.2%, respectively; p < 0.05). Furthermore, significant weakly-moderate correlation was found between GM-CSF levels and percentage of eosinophil infiltration in tissue (p < 0.05). Correlation between symptom scores and GM-CSF levels was significant only in group 2, which showed higher average concentrations of GM-CSF compared to groups 1 and 3 (2.9 pg/mL vs 1.6 pg/mL and 1.8 pg/mL, respectively; p < 0.05). Conclusion Our data confirm that GM-CSF is more frequently detectable in nasal lavages of patients affected by chronic sinonasal eosinophilic inflammation than in controls. Statistical analyses revealed a significant weakly-moderate correlation between GM-CSF levels in nasal lavage of all patients and percentage of eosinophil infiltration of nasal mucosa.

MRI is a widespread and greatly helpful diagnostic tool, yet its use on cochlear implant patients is restricted by the presence of an inner magnet. We report on a case of magnet dislodgment after 1.5T MRI in a 31-year-old female with a Hi-Res 90K cochlear implant. In this case, it was possible to implement an alternative, totally noninvasive approach based on an external manual repositioning rather than a surgical procedure of the displaced magnet. The use of "one-shot" CO2 laser technique for a primary small-fenestra stapedotomy is well established, but few papers report the long-term functional results. We retrospectively reviewed medical records of 198 patients, treated for otosclerosis from January 2008 to December 2011, at the Department of Head and Neck Surgery, Catholic University of Rome. Statistical comparison between audiological thresholds obtained 24 h preoperatively, at early (4 weeks) and late postoperative examinations (mean time 45 months), was performed. Comparison of preoperative vs both early and late postoperative ACPTA showed a statistically significant difference (respectively 55 vs 33 and 31 dB; p 0.05). Both early and late postoperative ABG improved significantly compared to the preoperative one (respectively 10 and 9 vs 32 dB; p 0.05), early vs late postoperative ABG (respectively 10 vs 9 dB; p > 0.05) and early vs late ABG gain (respectively 22 vs 23 dB; p > 0.05). No subjects developed postoperative complications requiring revision surgery or late deterioration of hearing threshold. The analysis of our data suggests that "one-shot" CO2 laser stapedotomy is an effective and safe procedure: it allows a rapid stapedotomy without damages for the inner ear and optimal functional results that remain stable during the years.

Background Nasal polyposis is probably a multifactorial disease, but so far, no genetic susceptibility factor has been identified. The observed associations between the ADRB2 arg16gly polymorphism and asthma-related phenotypes as well as those between nasal polyposis and asthma have prompted us to evaluate the potential involvement of this polymorphism in sinonasal polyposis. Methods We enrolled in our study, 56 patients and 47 sex- and age-matched controls. Genomic DNA from cases and controls was extracted and genotype was assessed by a polymerase chain reaction amplification/Nco I digestion assay. Statistical analysis was performed using JMP software (version 5.1). Results The “number of arg alleles” is significantly higher in cases than in controls (p = 0.0386 at t-test; substantially confirmed by nonparametric tests, p = 0.0396 by Wilcoxon/Kruskal-Wallis tests). Conclusion Although results of this study are preliminary because of the small size of the sample, the arg16 allele seems to be associated with an increased risk of sinonasal polyposis suggesting ADRB2 as a susceptibility gene. This finding, if confirmed, would have a clinical value in helping to assess the genetic risk for sinonasal polyposis thus opening new perspectives for the study of molecular factors underlying the development of nasal polyps.
